Relevance. The prevalence of endometriosis, which occurs against the background of oxidative stress, varies from 10% to 80%, which requires close attention to increase the reproductive potential of Russia. Identification of predictors of pathology is the most important tool in the preventive and therapeutic model, in which it is important to use, among other things, non-drug methods of treatment. Aim. To obtain a medical, socio-obstetric and oxidant assessment (LPO) in patients with endometriosis treated with non-drug therapy (magnetophoresis of 5% iodine ions and 1% solution of caripazim). Materials and methods. A randomized trial with cross-sectional analysis of case histories was conducted in 89 patients with a verified diagnosis of endometriosis. Research tools: gynecological examination, enzyme-linked immunosorbent assay of LPO indicators (superoxide dismutase, catalase, malondialdehyde), questionnaire for medical, social and obstetric assessment, according to the questionnaire (WDEQ-A, USA). Magnetophoresis of 5% iodine ions and 1% solution of caripazim was carried out with the activation of one pole (Polyus-3 apparatus). Multivariate regression analysis and nonparametric methods were used to assess the parameters of pain syndrome, lipid peroxidation and values of medical, socio-obstetric status before and after treatment. In the control group I (n=29), treatment was carried out in accordance with clinical recommendations for endometriosis. In group II (n=30), treatment was carried out, including magnetophoresis of 5% iodine ions. In group III (n=30), treatment was carried out in the form of magnetophoresis with a 1% solution of caripazim. The effectiveness of treatment was evaluated according to the monitoring of clinical indicators, antioxidant and medical-socio-obstetric status. Results. The article presents an experimental study of the effect of the ultrasonic diamond smoothing mode on the surface roughness of the roller bearing roller end face in the presence of graphite nanopowder on the treated surface. In the presence of graphite nanopowder on the surface, the diamond smoothing process is accompanied by the formation of a solid antifriction coating on the treated surface, which can affect the formation of the microrelief of the treated surface. The design of an ultrasonic installation is described for diamond smoothing of the rollers end surface with the possibility of simultaneous application of a solid antifriction coating to this surface. The installation is assembled on the basis of a 16K20 screw-cutting machine. It consists of a device for installing rollers in the chuck of the lathe and an ultrasonic transducer mounted on the caliper of the machine. A diamond tip was used as a working tool in this device which was attached to the end of the oscillation concentrator of the ultrasonic transducer. Axle box bearing rollers of 36-42726E2M series (GOST 18855-94) were used as experimental samples. The subject of research was regression dependences of the influence of ultrasonic processing mode on the treated surface roughness. Full factorial experiment method was used to conduct the research. The tool tip rounding radius, tool transverse feed, specimen rotation speed, and tool pressure force on the machined surface of the specimen were used as varying factors. The power-law dependence of treatment indices on the adopted varying factors was taken as a mathematical model. It is shown that the greatest influence on the surface roughness is exerted by the tool transverse feed and the diamond tip rounding radius.

Whooping cough is a highly contagious respiratory disease. despite ongoing vaccination, the disease is endemic in developing and developed countries. newborns and children who are not vaccinated or partially immunized become most severely ill. The severity of symptoms depends on factors such as the patient's age, the strength of the immune response, and the degree of systemic bacterial dissemination. The most severe form of pertussis is malignant pertussis, which is characterized by large leukocytosis, refractory hypoxemia, pulmonary hypertension and cardiopulmonary impairment. This article is devoted to the clinical observation of a patient with a malignant course of whooping cough.

COVID-19 has seen a rise in cardiovascular diseases (CVD). Treatment (rehabilitation) of COVID-19 survivors is one of the most difficulttasks, and non-drug correction of lipid metabolism in COVID-19 survivors is complicated by the low level of research on this issue.Aim. To conduct a comparative assessment of lipid metabolism and to estimate the efficacy of COVID-19 patients at the stages ofrehabilitation in a sanatorium.Material and methods. The study enrolled 117 patients with CVD and after endovascular treatment (EV) who underwent COVID-19 forthe last 6 months. Three groups for non-drug treatment with intranasal B1 electrophoresis and low-intensity infrared laser irradiation(LILI) were formed in patients with CVD who underwent COVID-19.Results and discussion. A significant increase in total cholesterol (TC) and triglycerides (TG) was found in 75.5% of patients afterCOVID-19, which was reflected in the electrocardiogram (ECG) in the form of right ventricular (RV) overstress. In addition to drugtherapy, a combination of intranasal drug electrophoresis B1 and low-intensity infrared laser radiation (LILI) in pulsed mode (l=904nm, power (PM) – 8-10 W/cm2, pulse duration 100-150 ns, frequency 80- 10000 Hz) on projection zones of the liver (angle of the 9thintercostal space in front/right and behind/right 5 minutes each) with special physical exercises can become as a method of additionallipid-correcting action. Lipidogram values in patients with IBI, HD and MI differ significantly, with the prevalence of total cholesterollevels, especially in patients with CAD and MI after a history of covid-infection, demonstrating the negative impact of viral infection onmetabolic processes. Physiotherapeutic treatment in sanatoriums helped to eliminate lipid-vascular shifts – potential risks of thromboticcomplications.Conclusion. Lipid metabolism correction in patients with CVD after COVID-19 allows reducing the risk of complications of theunderlying disease and increasing the efficiency at the stage of recovery (rehabilitation) in a sanatorium.

The aim of research study is a revelation of the multi-layered image of the tiger in the spiritual and religious life of Vietnam, from local totem beliefs to world religions through explanations and illustrations of thematic wood prints of several centers. Results. An analysis of the symbolic connotations associated with one of the key figures of the country’s zoomorphic pantheon makes it possible to evidentiate the ambivalence of its essence, a vivid embodiment of the contradictory features of which is interpreted in different ways in culture and art. Scope of the results. The results of this study can heighten the interest in the culture of Vietnam as a whole, as well as to contribute to the representation expansion about popular folk prints as one of the most traditional and original types of Vietnamese fine art and can be applied in its in-depth analysis. The relevance of the article is determined by the lack of research on this topic in the Russian-language literature.

The fifth article in a series dedicated to the restoration and museification of the Darovoe-Monogarovo-Cheremoshnya memorial space. The renewed F.M. Dostoevsky’s Estate Museum “Darovoe” opened in November 2021. The results of the restoration and the perspectives of its development were discussed at a round table on August 27, 2022, during the conference “9th Summer Readings in Darovoe.” Here is an abridged transcript of the discussion. The conflict between scholars and restorers described in the previous article led to a dialogue between the academic and museum communities; creative entrepreneurs have also joined the discussion. Both painful points and “points of growth” in the new life of the Museum-Estate “Darovoе” are highlighted.

Information security tools are an integral part of system users. The concept of information security implies the development and expansion of the scope of innovative technologies in information processing. To keep the information security system up to date, it is necessary to periodically update and supplement the structure of information protection, information security threat model and hardware and software complex. This paper analyzes the existing methods of information protection and proposes the implementation software system modules for personality recognition by photo and video, voice recognition, deep neural network and the creation of configuration for its weights file. Based on the generated data set, a method for synthesizing the parameters of a mathematical model of a convolutional neural network, presented as an array of real numbers, which are unique identifiers of a personal computer user, has been developed and proposed. This study uses the features of simulation modeling of user authorization systems, as well as the error function when compiling a neural network model. The training model of multi-factor biometric authentication is trained using categorical cross-entropy. The training sample is generated by adding distorted images from the database by changing the receptive fields of the convolutional neural network. The objective of this study is the application of new methods and means of protecting information of workstations from information threats. The result of the study is the developed information security system designed to ensure the information security of users of personal computers and workstations of enterprises.

This study is aimed at solving such a fundamental task as the use of modern information technologies in the development of electronic or digital democracy in Russia, as the main direction of the strategy for supporting democratic institutions and democratic processes and spreading democratic values. E-democracy is the basic foundation of the information society, accumulating a number of traditional and innovative tools that are successfully used in the democratic institutions of the modern state in the regulation of socially significant relations. The development of modern democratic processes is based not so much on public authorities, but on civil society, which is gradually transforming into a digital society. For this reason, the issue of searching for and actively implementing mechanisms that contribute to the development of information technology infrastructures, communication systems, and increasing the information literacy of the population seems to be relevant.

Rationale: Non-small cell lung cancer (NSCLC) is an aggressive disease with median survival of 1214 months in inoperable patients in the pre-immunotherapy era. Nowadays, under treatment with checkpoint inhibitors median survival is 1922 months. However, only a proportion of patients are sensitive to immune therapy. In this regard, inclusion into clinical trials remains a priority option for patients from medical perspective. Aim: To compare the results in NSCLC patients treated in accordance with the current clinical guidelines and in international clinical trials using the propensity score matching. Materials and methods: The study included data from 344 patients with histologically verified unresectable advanced NSCLC without activating mutations, who received the 1st line systemic medical therapy at various combinations (single agent chemotherapy, platinum doublet-based therapy, chemoimmunotherapy, single agent immunotherapy) within the compulsory health insurance (CHI), and from 90 patients, who received therapy in clinical trials. A direct comparison of long-term treatment results was carried out with the log-rank method. To exclude any influence of individual factors on survival rates, an univariate regression analysis and pseudorandomization accounting for these factors were carried out. Results: The direct comparison of the treatment results showed a higher progression-free survival rate in the patients treated according to clinical trial protocols, than in those treated under CHI (13.3 [95% confidence interval (CI) 8.118.5] months vs 6.4 [95% CI 5.96.9] months). Pseudorandomization of patients based on a combination of statistically significant parameters from the CHI and clinical trial groups showed a significantly longer time to progression in the trial group (13.3 [95% CI 8.318.3] vs 6.3 [95% CI 4.87.7] months). Conclusion: Participation in clinical trials is per se a factor that can significantly impact the longer duration of the treatment effect. This indicates the necessity of the most active use of this tool in clinical practice.

Background: Iron has dual properties: it may promote both tumor growth and cell apoptosis. Compared to healthy cells, cancer cells are more dependent on the iron levels. Ferroptosis can be triggered directly in cancer cells, which would result in their self-destruction. Identification of iron balance abnormalities and their correction could impact the effects of specific treatments in cancer patients. Aim: To assess the prevalence of iron deficiency in patients with metastatic colorectal cancer (mCRC) and to identify an association of low serum iron levels with clinical and morphological characteristics of the disease. Materials and methods: The study included 69 treatment-nave patients with mCRC. Iron deficiency was defined as low serum iron levels before the initiation of any specific therapy: serum iron concentration 10.7 mcmol/L in men and 9.0 mcmol/L in women. Results: The mean age of the mCRC patients was 61.1 years (range, 28 to 83 years), 35/69 (50.7%) were men. The bigger proportion of the tumors was left-sided (62.3%). In 48.3% of the patients, the disease was diagnosed at the metastatic stage. The most frequent locations of metastasis were liver (41.3%) and lungs (32.1%). 55.1% (38/69) of the patients had undergone a non-radical resection or primary curative surgery. KRAS mutations were found in 37.7% of the patients. Low serum iron levels were found in 53.6% (37/69) of the total sample of the mCRC patients and in 72.4% (19/38) of the patients with a non-resected primary tumor (p = 0.05). Conclusion: Irrespective on the clinical and morphological characteristics, the majority of patients with metastatic colorectal cancer have iron deficiency anemia before the initiation of specific anti-tumor therapy.

The fourth of a series of articles dedicated to the restoration and museification of the memorial space of Darovoe — Monogarovo — Cheremoshnya. Dostoevsky’s Darovoe Museum-Estate opened anew on November 20th, 2021, while the restoration of the Church of the Descent of the Holy Spirit is still running. The authors of the article concentrate on the pressing issues of the ongoing processes and suggest reflecting on the basic principles ruling contemporary Russian restorers. The directives of the “Venice Charter” (1964), when absolutized, lead as in this case to the destruction of authenticity. Instead of Dostoevsky’s estate, we end up with Ivanov’s, and instead of the Church Dostoevsky looked at, we canonize the architectural preferences of the archpriest of the Church at the beginning of the 20th century. Of particular concern is the fate of the Linden Grove, now on the edge of disappearance. The causes of the losses we are witnessing are academic as well as social and moral.

Lung cancer has the highest morbidity rate among all malignant tumors in men and the highest mortality rate in men and women in Russia. In total, 49 145 new cases of lung cancer were registered (diagnosed) in Russia in 2019. The majority of cases are related to exogenic carcinogens and mainly tobacco smoke. For several decades surgical resection with preoperative cytotoxic therapy was an optimal approach for maximal cure rate. This year recommendations were updated with new strategies including adjuvant anti-PD-L1 atezolizumab following completion of chemotherapy in PD-L1 positive patients and osimertinib for EGFR mutated cases. For this moment available data suggest the increase in disease free survival. Strategic approach to treatment for inoperable patients varies according to the status of driver mutations. New approach includes pretreatment option of testing for a wide spectrum of alterations with NGS based panels. Significant changes were incorporated into treatment of ALK mutated NSCLC with two new options of brigatinib for TKI naive patients and lorlatinib for those who progress on second generation drugs. Treatment strategy for patients without activating mutations is based on PD-L1 status. Tsis year recommendations included atezolizumab as a new monotherapy option for patients with high depression of PD-L1. Also treatment options for pembrolizumab, nivolumab and atezolizimab were widened with prolonged treatment schedules.

Extensive-stage small cell lung cancer (ES-SCLC) is characterized by an aggressive course, a high recurrence rate and fast progression. For a long time, the survival prognosis for the most patients suffering from this disease remained unfavorable. The situation changed with the appearance of chemoimmunotherapy in clinical practice. Chemotherapy based on durvalumab in comparison with the standard chemotherapy demonstrated the statistically and clinically significant increase in median overall survival in patients with previously untreated ES-SCLC in the CASPIAN international trial. This article deals with the case of the application of standard chemotherapy in combination with durvalumab as a first-line ES-SCLC therapy. The patient started receiving durvalumab therapy in June 2017 as a part of the CASPIAN international trial. In March 2021 the duration of therapy was 45 months, the patient had a complete regression of the disease.

Photobioacoustic methods of complex exposure allow correcting vegetative, vascular-cardiac disorders in children with autonomicnervous system somatoform dysfunction syndrome (ANSSDS) without pharmacological load. Aim. To optimize the treatment of children with somatoform vegetative dysfunction (ANSSDS), often suffering from recurrent infections,using physiotherapy methods.The aim of the study was to optimize the treatment of children with somatoform vegetativedysfunction, often suffering from recurrentinfections, using physiotherapy methods. Material and methods. The study included 140 children with autonomic nervous system somatoform dysfunction syndrome (ANSSDS),often suffering from recurrent infection. For the first time, clinical complaints, functional (ECG and cardiointerval recording KIR)and vegetative indicators (Kerdo, Hildebrandt index, Biend, Baoev-Parin) before/after application of the innovative photobioacousticcomplex, including: BАС-BFB-bioacoustic effects on the head and LLLR on the projection of the cubital region and thymus were analyzed.The duration of the procedure was 30 minutes and the course of treatment was 10 procedures. Before and after the procedures,vegetative indices and indicators of myocardial bioelectrogenesis were examined, comparing them with the norm in healthy children. Results. 35% of children were diagnosed with parasympathicotonia (sympathetic imbalance -SI); 54.1% – sympathetic (SA) and10.9% – mixed (etonia – E) type of vegetative dystonia. There were reveled gender-age differences in subjective complaints of childrenwith ANSSDS, whose index vegetative indicators were correlated with the type of vegetative support. Children with SA were 2.5 timesmore likely to be diagnosed with cardiac syndrome and elevated blood pressure; in children with SI – 3.5 times more often detected:gastric, astheenoeurotic syndrome and allergic diseases; in children with E: asthenonevrotic, neurosis-like disorders and sleep disturbancesthat were differentiated after the use of photobioacoustic complex. Complaints and vegetative indices in children with SI andE were corrected more significantly than in SA. Conclusion. Evaluation of vegetative indicators in children 7-12 years with ANSSDS, often suffering from recurrent infection, allows topersonify treatment programs and reduce the level of acute recurrent morbidity by 2.5 times.

The aim of this study was to obtain new catalysts for the processing of carbon-containing polymer waste based on polyethylene and polypropylene, represented mostly by lids from beverages bottled in plastic containers, which accumulate in huge quantities in landfills, by the method of thermocatalytic hydrogenation into liquid fuels and other products. The process was carried out in the presence of fuel oil as a binder, a source of hydrogen and additional hydrocarbons. Thus, two tasks can be solved simultaneously: recycling the polymer waste and obtaining the alternative raw materials from the polymer waste in order to save resources and improve the environmental situation in general. New catalysts based on activated zeolite modified with Mo(VI) and W(VI) salts of various concentrations for the thermocatalytic hydrogenation processing of waste plastics into motor fuels were synthesized. The composition, structure, morphology and adsorption properties of the catalysts were determined by different physicochemical methods. The suitability of the obtained catalysts for use in the thermocatalytic hydrogenation processing of plastic waste into fuels was determined. The catalysts were tested during the processing of a mixture of polyethylene-polypropylene: a paste-forming agent (fuel oil) at T=450 °C and a pressure of 0.6 MPa. The individual and group composition of gasoline, diesel and gas oil fractions was determined by chromatography coupled with mass spectrometry. The maximum yield of the gasoline fraction (16.9 wt.%) and diesel fraction (39.31 wt.%) was obtained on a 2%W(VI)/diatomite catalyst.

Third paper on the topic. The formation of the restoration project of the Darovoe Museum is reaching a turning point. Bringing the creative laboratory of the museum construction into public space allows the scientific community to influence the process of establishing a new museum of the writer and sets a precedent for transparency in the restoration process. The combination of museum design, natural landscape, and architectural research with the achievements of philologists and historians are forming a new kind of interdisciplinary cooperation between scientists and practitioners. In fact, the process is now facing a conflict, ultimately of axiological nature. The article includes proposals of famous scholars to the General Directorate of the Cultural Heritage for the Moscow Region and the Protocol of the meeting of the section of the Council for the Cultural Heritage of the Ministry of Culture of the Russian Federation; it presents the contradictions causing conflict. The concept of a natural museum of Dostoevsky’s childhood, defended by the academic community, is winning the right to implement with great effort.

Dostoevsky scholars have long discussed the future image that the Darovoe Museum is to have: the following work presents one of the final stages in designing the estate restoration project. Taking the process of creating the Darovoe Museum into the public space allows the academic community to influence the process of re-shaping the new Museum; thus the phenomenon of an open restoration experience emerges.

The article presents an overview of the seventh edition of Darovoe Summer Readings, held in Zaraysk (Moscow region), and in Dostoevsky’s estate in the village of Darovoe. The Readings focused on two topics: the reception of Dostoevsky’s work, and the search for archival documents about the context of the early biography and the creative sources of the author of The Brothers Karamazov. The closing round table was dedicated to the perspective developments of the Museum in the land of the writer’s childhood.

A key advantage of amino-modified SiO2 nanoparticles for delivery of phosphorylated nucleosides is a broad possibility for functionalization. It can be modified with ligands currently investigated in targeted drug delivery. To improve the efficacy for intracellular delivery, SiO2 nanoparticles were functionalized with tumor-targeting ligands folic acid, biotin or 5-fluorouracil. Studies of accumulation of these conjugates in HCT116 colon carcinoma cells revealed that the uptake of modified conjugates was significantly bigger compared to unmodified nanoparticles, with the biotinylated conjugate as the preferred compound. The nanocomposites of biotin modified SiO2 and 2′,3′-dideoxyuridine triphosphate showed a pronounced antiproliferative potency relative to the unmodified nanocomposites. Thus, multi-functionalization of SiO2 nanoparticle based conjugates has a major potential for delivery of nucleoside triphosphate analogues, therefore tentatively enhancing their bioactivity. Background. Chronic endometritis is one of the causes of infertility, miscarriage, failed in vitro fertilization and embryo transfer attempts. Treatment of chronic endometritis is quite complicated due to a multifactorial nature of endometrium morpho-functional disorders. There are lots of approaches to a comprehensive treatment of chronic endometritis which indicates a lack of a single algorithm for a management of such patients to date. One of the modern, perspective and high-tech methods for treatment of chronic endometritis is photodynamic therapy (PDT). Aim. To study chronic endometritis treatment efficacy in patients with infertility when using the method of PDT and low doses of 17b-estradiol. Materials and methods. 85 female patients of reproductive age with chronic endometritis were examined and treated. All patients were divided into 2 groups: group 1 (43 patients) got treated by a method of intravenous PDT with low-dose transdermal 17b-estradiol; group 2 (42 patients) received low-dose transdermal therapy with 17b-estradiol. Clinical examination and laboratory tests, pelvic ultrasound, endometrial aspiration biopsy followed by pathomorphlogical and immunohistochemical examination were carried out to all patient. Results. Ultrasound examination revealed an increase in endometrium thickness at 12th day following PDT session in group 1 and following transdermal therapy with 17b-estradiol in group 2 resulting in endometrium state improvement almost 2 times compared with baseline values before treatment. Conclusions. PDT is a minimally invasive, gentle and safe treatment method. Due to a diffuser design laser radiation is evenly distributed in the uterine cavity. PDT reliably restores receptor function to progesterone in the endometrial glands.

Taking the example of Kazakhstan, this study examines the formation and analysis of the cultural media space of the digital generation. Information and communication technologies are the basis of a communicative media environment that has an internal regulated structure, which affects the socialisation of a person. The content of the cultural media space of the digital generation is formed on the basis of digital technologies and represents people’s visual worldview with images, signs and symbols. The processes of transformation of society affect the young generation and the content of the media space that determines their social and ethnocultural identity. The study presents the results of working with schoolchildren and students aged 14-18 using focus groups to explore the impact of digital culture (media culture) on adolescents in Kazakhstan. Based on the interdisciplinary approach, the findings demonstrate intergenerational contradictions because of the active involvement of Kazakhstan in the global internet community. The cultural media space of Kazakhstan has been shaping the social communications of the digital generation that has grown up in a sovereign state. The theoretical concepts of P. Bourdieu, C. Mannheim and other scientists served as a methodology for this study. Based on the interdisciplinary approach, the findings demonstrate intergenerational contradictions because of the active involvement of Kazakhstan in the global internet community. The findings also unravel that the transformation of the value orientations of adolescents is influenced by the factors of geographical location and ethnicity.

In present article valuable components of social potential are considered. The authors note that special attention should be paid to the study of the educational potential of modern youth as a resource and reserve, which determines the future state of society. In work on the basis of the authors researches the comparative analysis of basic vital values, educational potential of three groups of the youth of the Volga region – studying, working, jobless is carried out. As a result, the conclusion is presented, which consists in the allocation of groups of young people who have a great educational potential (entrepreneurs and students).

The article examines mythopoetic basis of the spatial imagery in the novels by a Chukchi writer Yuri Rytkheu. It reveals the specificity of the worldview of the author and his characters as well as the evolution of the writer’s creativity. The author argues that mythological representation of space in Rytkheu’s novels follows such general principles of the genesis of myths as anthropomorphism, “the law of participation” (communion), and late animism (Levy-Bruhl). Rytkheu projected traditional mythological spatial forms and images onto the poetics of the text with the help of mythologemes and archetypal binaries (top-bottom, friend-stranger, close-distant, life- death). The analysis of his two novels written with a gap of forty years — Aivangu and In the Mirror of Oblivion — reveals two different approaches to spatial substrates. In the first novel (1964), there prevails “real” space as the novel’s setting and mythologism of images veiled. The second novel written in the 1990s, the years of Rytkheu’s temporary emigration in Europe (Denmark, Germany), undergoes influence of postmodernism and neomythologism. In this novel, mysterious, multi-faceted space becomes a meaningful object of representation shown from a variety of perspectives.

The article is devoted to the issues of socio-cultural adaptation of repatriates on the territory of East Kazakhstan and the analysis of the migration attitudes of the Kazakhs of Mongolia. Since Kazakhstan gained independence on 16 December 1991, the state has been implementing a state program on the return of ethnic Kazakhs to the country; this practice has spread in many states. The repatriation of ethnic Kazakhs to their historic homeland allows solving many socio-economic, demographic problems. At the same time, relocation to a historic homeland causes a new set of problems related to the adaptation of repatriates, their employment, interaction with the local population, search and self-awareness. Integration processes and strategic social programs of Kazakhstan have become the motivation for the formation of the migration interests of ethnic Kazakhs living abroad.

The article covers the history of the totalitarian regime in the 40s of the 20thCentury on the territory of the Soviet Union, including Kazakhstan as an integral part of the Soviet empire. The chronological framework of events is connected with the Second World War, namely with the aggression of the Third Reich against the USSR. The regime of everyday military life reinforced the repressive actions of the Soviet government, the point was directed against the peoples accused of loyalty to the German troops. Chechens who were deported to Kazakhstan, including East Kazakhstan on 23 February 1944, became one of such ethnic groups affected by the totalitarian policy. The methodological basis of the study was an interdisciplinary approach, theoretical and methodological concepts of ‘collective memory’ in the projection of collective-individual, ‘cultural memory’, and ‘generational memory’. In the article, based on a wide range of sources and materials, the integration processes of Chechens and the ethnic dialogue with the Kazakh ethnic group and ethnic palette residing in East Kazakhstan in terms of production and outside its sphere are analysed. The research introduces to the scientific circulation archival and documentary sources and records of materials collected as a result of field expeditions, allowing to show the interaction of Chechens with the local population in conditions of deportation, with an attempt to improve social status by participating in socio-economic processes and preserving ethnic identity. The authors believe that the strategy of behaviour and the adaptation of the Chechen ethnos differed by their internal attitude to the status of the deported; their dispersed state and frontier conditions determined the individual strategy and tactics of survival.

Objectives: Upper airway stimulation (UAS) is an effective treatment for patients with obstructive sleep apnea. The device consists of a stimulating electrode, an internal pulse generator, and a respiratory sensing lead. The purpose of this study is to characterize the incidence of sensing lead malfunction necessitating revision surgery in a high‐volume center in conjunction with a review of the FDA Manufacturer and User Facility Device Experience (MAUDE) database reports on adverse events associated with the sensing lead component. Methods: Patients age ≥18 implanted with the device were reviewed between July 2017 and June 2022. Those determined to have sensing lead malfunction were analyzed. The FDA MAUDE database was reviewed for reports associated with "Inspire Model 4323 Respiratory" and "Inspire Model 4340 Respiratory" between January 2014 and September 2022. Results: One hundred ninety patients underwent UAS at our institution during the study period and four (2.1%) patients were found to have sensing lead malfunction. Analysis of the MAUDE database revealed 122 reports of adverse events associated with the sensing lead component, with 72% (88/122) of these requiring revision surgery. The most frequently reported adverse event was sensing lead tip separation from lead body, noted in 46% of these reports (56/122). Conclusion: The functionality of the sensing lead component is vital to the integrity of the UAS device and is measurable through waveform analysis. To provide optimal care after device implantation, it is essential to have a comprehensive understanding of the scenarios in which this component fails, the incidence of such events, and appropriate management. Objective: This study aims to define the incidence of infection with upper airway stimulation (UAS) devices requiring explantation in a single academic center and identify factors that may influence the risk of infection. Methods: A database of patients who underwent UAS at a single tertiary referral academic center from 2017 to 2021 was retrospectively reviewed to identify patients who developed surgical site infections, with and without subsequent explantation. Additional data for cases complicated by infection was extracted from the electronic medical record (EMR) and included: demographic information, medical history, complications and management, and overall outcomes. In March 2021, 2 modifications to infection control protocols were implemented: double skin preparation with Betadine and chlorhexidine, and MRSA decolonization. Statistical analysis was performed to compare infectious risk before and after these protocol changes. Results: In the study period, 215 patients underwent UAS in the specified time period and 3 cases (1.4%) of postoperative infections were identified, all of which required explantation. The infection rate did not significantly change after modifications to the surgical prep protocol (P =.52). Conclusions: While no significant difference in infection risk was noted after modifying infection control protocols, additional longer-term study is warranted to elucidate effective infection reduction strategies.
