El síndrome de embolia grasa (SEG) es una entidad poco frecuente de difícil diagnóstico clínico y una causa importante de morbimortalidad en pacientes con politraumatismos. Es la manifestación clínica de la presencia de glóbulos grasos en la circulación sistémica y se caracteriza, fundamentalmente, por el desarrollo de insuficiencia respiratoria, síntomas neurológicos y exantema petequial. Se asocia principalmente con complicación de fracturas de huesos largos, aunque puede ser causada por otros tipos de patologías. El diagnóstico se ha basado clásicamente en los hallazgos clínicos. Por este motivo, presentamos cuatro casos donde la Resonancia Magnética (RM) de encéfalo tuvo un rol fundamental en el diagnóstico.Fat Embolism Syndrome (FES) is an uncommon entity characterized by a difficult clinical diagnosis and high morbi-mortality in polytrauma patients. It is the clinical manifestation of the presence of fatty cells in the systemic circulation and it is mainly characterized by the development of respiratory failure, neurologic symptoms and petechial rash. Although it has been largely associated with long-bone fractures, it may be caused by other types of pathologies. Clinical findings have been classically used for the diagnosis. Therefore, we report on four cases where brain Magnetic Resonance Imaging (MRI) had a key role in the diagnosis.

Objetivos. Determinar retrospectivamente el índice de falsos negativos y el grado de subestimación de la punción con aguja gruesa eco-guiada de nódulos mamarios. Establecer la incidencia de las diferentes lesiones que se manifiestan como nódulos mamarios. Materiales y Métodos. Se estudiaron las punciones de nódulos mamarios desde enero del 2000 a septiembre del 2010. Las mismas fueron realizadas por un radiólogo con 10 años de experiencia y se utilizaron agujas de corte de 14-gauge y 10 cm de longitud. Para la guía ecográfica se usaron dos ecógrafos con transductores lineales de 5-12 MHz. Se punzaron lesiones de moderada y alta sospecha de malignidad (BI-RADS 4 y 5) y algunas lesiones probablemente benignas (BI-RADS 3). Se correlacionaron los hallazgos histológicos de la punción y de la cirugía para cada lesión. A los casos con resultado benigno que no fueron operados se les realizó un seguimiento clínico de entre uno y diez años. Resultados. Se realizaron 190 punciones. El índice de falsos negativos fue del 3%, la sensibilidad de 95% y la especificidad de 100% (VPP de 100% y VPN de 95%). Discusión. Hubo coincidencia entre los resultados de las punciones y la cirugía en un 94%. El índice de falsos negativos es aceptable en comparación con otros reportes donde varía entre 0 y 8%. Conclusiones. Este estudio indica que la punción con aguja de corte eco-guiada es un método válido, con alto grado de precisión y bajo índice de subestimación y falsos negativos para el diagnóstico de nódulos mamarios malignos y benignos. Esto fue comprobado mediante la escisión quirúrgica y/o el seguimiento clínico- imagenológico de las diferentes lesiones.Objectives. To retrospectively determine the false negatives rate and the extent of underestimation of ultrasound-guided core biopsy of breast nodules, and to determine the incidence of the various lesions presenting as breast nodules. Materials and Methods. We studied core biopsies of breast nodules from January 2000 to September 2010, performed by a radiologist with 10 years experience in breast imaging, using 14-gauge and 10-cm needles. For ultrasound guidance, two ultrasound machines with a 5-12 MHz linear transducer were used. Biopsied lesions had a moderate or high suspicion of malignancy (BI-RADS 4 and 5) and some were probably benign lesions (BI-RADS 3). Histological findings of core-biopsy were correlated with those of surgery for each lesion. Cases of benign lesions which had not been operated on underwent a 1- to 10-year clinical and radiological follow-up. Results. A total of 190 biopsies were performed. The false negative rate was 3%, sensitivity was 95% and specificity was 100% (PPV 100% and NPV 95%). Discussion. There was 94% consistency between core biopsy results and surgery. The false negative rate is acceptable as compared to other reports, where it ranges from 0 to 8%. Conclusions. This study indicates that ultrasound-guided core biopsy is a valid method, with a high degree of accuracy and a low rate of false negatives and underestimation, in the diagnosis malignant and benign of breast nodules. This has been confirmed by surgical excision and / or clinical and imaging follow-up of different lesions.

Introducción: La Espectroscopía por Resonancia Magnética (ERM) es una técnica no invasiva que permite un análisis del metabolismo de las lesiones o del tejido normal, aumentando la especificidad del método. De esta manera, a la información morfológica aportada por las imágenes de resonancia magnética (RM) se le suma la información bioquímica que brinda la ERM. Si bien el "gold standard" para establecer el diagnóstico definitivo de una lesión cerebral sigue siendo la biopsia, la ERM es un método no invasivo y libre de complicaciones que posibilita determinar el tipo de lesión y evitar biopsias innecesarias en procesos no tumorales. El objetivo del presente trabajo es comprobar si la ERM monovóxel protón de hidrógeno (H+) de tiempo de eco (TE) largo es capaz de discriminar la naturaleza tumoral o no de las lesiones cerebrales y clasificarlas en grados de malignidad. Material y método: Se trata de un estudio prospectivo en el que se incluyó a pacientes de ambos sexos y de distintas edades a los que se les realizó un estudio estándar de RM completado con ERM monovoxel. Resultados: Se analizaron 47 lesiones y se caracterizaron adecuadamente 43 (92,9%), con una sensibilidad (S) del 96,8% (IC 89-100), una especificidad (E) del 89,6% (IC 76-100), un valor predictivo positivo (VPP) del 91,1% (IC 80-100) y un valor predictivo negativo (VPN) del 96,3% (IC 87-100). Muchas son las variables que influyen en la adquisición de un espectro factible de ser analizado, pudiendo surgir de éstas diferencias inter-observador. Sin embargo, se obtuvieron resultados similares a los de otras publicaciones. Conclusión: La ERM sumada a la RM demostró ser un método confiable para determinar la naturaleza tumoral o no de una lesión cerebral, con valores estadísticos aceptables.The Magnetic Resonance Spectroscopy (MRS) is a non-invasive technique which allows study of the metabolism of lesions or of normal tissue, increasing the method's specificity. In this way, the biochemical information provided by MRS is added to the morphologic information provided by the Magnetic Resonance Imaging (MRI). Even though the gold standards to determine the definite diagnosis of a brain lesion is still the biopsy, the MRS is a non-invasive method, free of complications which would help determine the type of lesion and avoid unnecessarybiopsies in non-tumor processes. The objective of this work is to determine if the monovoxel MRS hydrogen proton (H+) long Eco Time (TE) is capable to differentiating or not the nature of the tumor from the brain lesions and classify them into levels of malignity. Material and Method: This is a retrospective study in which female and male patients of any ages were selected. A standard study of MRI was performed in them and it was completed with monovoxel ERM. Results: 47 lesions were analyzed and 43 (92.9%) were adequately characterized, with a sensibility (S) of 96.8% (IC 89-100), specificity (E) of 89.6% (IC 76-100), positive predictive value (PPV) of 91.1% (IC 80-100) and a negative predictive value (NPV) of 96.3% (IC 87-100). There are many variables that can influence the acquisition of a spectrum capable of being analyzed and from them, inter-observer differences can emerge. However, our results were similar to those in other publications. Conclusion: The MRS together with the MRI proved to be a reliable method to determine whether a brain lesion is a tumor or not, with acceptable statistic values.

El quiste de colédoco tipo IV A es una enfermedad muy poco común caracterizada por una dilatación quística congénita del árbol biliar que se extiende también a los canalículos biliares intrahepáticos. Se comunica el caso de una mujer de 19 años de edad con ictericia y dolor abdominal. La paciente fue sometida a una amplia evaluación clínica, analítica y por imágenes. La tomografía computada y la colangiopancreatografía por resonancia magnética revelaron un quiste coledociano de 13.5 cm con compromiso intrahepático. Se combinó la resección de la parte extrahepática del quiste con una hepaticoyeyunoanastomosis en Y de Roux. Se descubrió una estructura ductal en la parte distal del quiste extrahepático. No hubo complicaciones perioperatorias ni alejadas en el seguimiento.

El divertículo gigante de colon (DGC) es una rara complicación de la enfermedad diverticular. Se considera divertículo gigante cuando su diámetro supera los 4 cm. Descrito por primera vez en la literatura francesa en el año 1946, se han publicado hasta la fecha 155 casos. Nuestro paciente, de 59 años, refirió clínica de dolor abdominal leve de 4 meses de evolución. Se le solicitó un estudio de colon por enema, el que reveló una formación sacular de 15 cm en colon sigmoideo, de contenido aéreo y cuello corto, a través del cual se opacificó la cavidad con la sustancia baritada, confirmando el diagnóstico de DGC. Al paciente se le realizó, además, tomografía computada y resonancia magnética. El diagnóstico se confirmó con cirugía y estudio histopatológico. Se practicó una resección sigmoidea con DGC y anastomosis colorrectal lateroterminal. El estudio anatomopatológico informó DGC tipo II.Giant colonic diverticulum is a rare diverticular disease complication. It is considered giant when its diameter is longer than 4 cms. It was reported the first time by french authors in 1946 and there is 155 cases reported in world literature. Our patient aged 59 years old, referred four month of abdominal pain. A barium enema was made showing a sigmoid cavity with air and fill of barium contrast with short neck continuing with the colon lumen. It was also studied with CT and MRI. The diagnosis was confirmed by surgery and histopathologic exam. The pathologic report informed giant colonic diverticulum type II.

Introducción: Los marcadores tumorales son importantes al momento de decidir la realización de biopsias para el diagnóstico de cáncer de próstata. El objetivo de este trabajo es evaluar la exactitud diagnóstica del APE libre como marcador tumoral en casos de pacientes con sospecha de cáncer prostático en su relación con el APE total (APEL/T) y determinar un valor de corte ajustado. Material y Método: En total, fueron evaluados 248 pacientes de entre 38 y 88 años de edad, en un período comprendido entre noviembre de 2001 y junio de 2007, con un análisis del APE libre, identificación de la presencia de un área hipoecogénica como ayuda diagnóstica y un análisis anatomopatológico para confirmación pospunción prostática endorrectal, realizado en forma prospectiva. Resultados: La identificación de pacientes con cáncer de próstata a través de la sospecha por el APE libre es relativa pero puede ser potenciada con otros hallazgos. Un 23,3% de pacientes con APEL/T por debajo de 0,20 presentaron biopsias positivas, pero, con un valor de 0,14, el porcentaje fue del 60%. Discusión: La identificación de pacientes para ser estudiados mediante Biopsia Prostática Endorrectal (BPE) es un problema de la práctica diaria en los consultorios de Urología. Debido a que la BPE es un procedimiento diagnóstico medianamente invasivo y no exento de morbilidad, es que se debería ajustar su indicación. La relación APEL/T debe ser considerada en este contexto con mayor importancia. Conclusión: La identificación de valores alterados de la relación APE L/T con un valor de corte ajustado a 0,14 puede ayudar en el diagnóstico presuntivo de cáncer de próstata y a la elección de pacientes para biopsia a fin de ahorrar biopsias innecesarias.Introduction: The tumor markers are important to decide to perform biopsies in the prostate cancer diagnosis. The goal of this article is to evaluate the diagnosis precision of FPSA (Free Prostatic Specific Antigen) as a tumor marker in patients with Prostatic Cancer suspicion and the FPSA ratio. Material and Method: A total of 248 patients aged between 38 and 88 years old were evaluated from November 2001 to June 2007. It was a prospective study analyzing the FPSA, the presence of echographic hypoecogenic lesion and the anatomopathologic final diagnosis to confirm either malignancy or benignity. Results: The selection of patients to perform Prostatic Endorectal Biopsy (PEB) is a diary dilemma in Urology cabinets. Since PEB is an invasive procedure it is necessary to adjust its indications. Until now the parameters that Urologists are using to indicate the biopsy are: Serum PSA, Clinics findings, and the Endorectal Ultrasound findings (EU). The ratio FPSA/Serum PSA must be considered with more emphasis. Conclusion: The identification of FPSA ratio alteration with a more confidential cut level adjusted to 0.14 can be more sensitive in the prostate cancer diagnosis and for the selection of patients to perform biopsies or avoid unnecessary invasive procedures.

La neumatosis intestinal es una entidad muy infrecuente asociada a varias patologías, como el infarto intestino-mesentérico, la enterocolitis necrotizante y la enfermedad pulmonar obstructiva crónica. Se caracteriza por la presencia de gas en la subserosa o submucosa a través del tracto gastrointestinal. Presentamos el caso de un paciente de sexo masculino de 63 años de edad que consultó por dolor en abdomen superior, vómitos y fiebre elevada (39º) nueve días después de una gastrectomía total por cáncer. La radiografía directa de abdomen constató distensión intestinal y la tomografía computada (TC) demostró distensión intestinal, edema mesentérico, neumatosis intestinal a través del intestino delgado y neumatosis portal, preferentemente en el lóbulo hepático izquierdo. Se realizó una laparotomía de urgencia que reveló únicamente distensión intestinal por adherencias, sin evidenciar necrosis intestinal. El paciente evolucionó desfavorablemente, falleciendo posteriormente. Reportamos un nuevo caso y revisamos la literatura de la neumatosis intestinal asociada con neumatosis portal.The pneumatosis intestinalis is a very infrequent condition associated with a number of diseases, such as mesenteric infarction, necrotizing enterocolitis, and obstructive pulmonary disease characterized by the presence of subserosal or submucosal gas cyst throughout the gastrointestinal tract. A 63- year- old man complained of upper abdominal pain, vomiting and high fever (39º C) on the nine day after total gastrectomy for cancer. Abdominal X-ray revealed intestinal distension. The abdominal Computed Tomography (CT) showed intestinal dilatation, mesenteric oedema, diffuse pneumatosis throughout the small intestine and gas in the portal venous system predominantly in the left hepatic lobe. It was performed emergency activity that revealed intestinal distension secondary to adhesion without intestinal necrosis. The patient had a downhill course and died thereafter. We report a new case and reviewed the literature of pneumatosis intestinalis associated with hepatic portal venous gas.

Species of the genus Psychotria are used in popular medicine for pain, inflammatory symptoms, and mental disorders. Psychotria capillacea (Müll. Arg.) Standl. (Rubiaceae) is commonly known as coffee and some scientific studies have demonstrated its therapeutic potential. The goal of this study was to investigate the anti-inflammatory and neuroprotective effects, and acetylcholinesterase (AChE) inhibitory activity of a methanolic extract obtained from leaves of P. capillacea (MEPC), as well as the micromorphology and histochemistry of the leaves and stems of this plant. In addition, the MEPC was analyzed by UHPLC-MS/MS and the alkaloidal fraction (AF) obtained from the MEPC was tested in a mouse model of inflammation. MEPC contained three indole alkaloids, one sesquiterpene (megastigmane-type) and two terpene lactones. MEPC (3, 30 and 100 mg/kg) and AF (3 and 30 mg/kg) were evaluated in inflammation models and significantly inhibited edema at 2 h and 4 h, mechanical hyperalgesia after 4 h and the response to cold 3 h and 4 h after carrageenan injection. Scopolamine significantly increased the escape latency, and reduced the swimming time and number of crossings in the target quadrant and distance, while MEPC (3, 30 and 100 mg/kg), due to its neuroprotective actions, reversed these effects. AChE activity was significantly decreased in the cerebral cortex (52 ± 3%) and hippocampus (60 ± 3%), after MEPC administration. Moreover, micromorphological and histochemical information was presented, to aid in species identification and quality control of P. capillacea. The results of this study demonstrated that P. capillacea is an anti-inflammatory and antihyperalgesic agent that can treat acute disease and enhance memory functions in mouse models.

La hernia obturatriz (HO) es poco frecuente y representa del 0.05 al 0.4% del total de hernias. Debido a su rareza y a su presentación inespecífica, el diagnóstico es por lo general tardío y las tasas de mortalidad elevadas (12-70%). Se presenta un caso típico de HO diagnosticada en el preoperatorio mediante TC Multi-corte con RMP y tratado mediante laparotomía de urgencia con buen resultado. El empleo de TC Multicorte con RMP de abdomen y pelvis, en cuadros de obstrucción intestinal en mujeres añosas, sin antecedente de cirugías previas ni hernias objetivables, tiene gran valor para el diagnóstico preoperatorio precoz de HO y podría contribuir a reducir las elevadas tasas de morbilidad y mortalidad.

El Síndrome del Asa de la Arteria Pulmonar, también conocido como Lazo de la Arteria Pulmonar y Sling Sign de la literatura de habla inglesa, es una forma rara de malformación congênita del origen y recorrido de la arteria pulmonar izquierda, que se incluye dentro de las alteraciones en "anillo" que modifican la normal anatomía de los vasos mediastínicos y producen compresiones sobre la vía aérea. Se cree que se produce por un fallo en la formación del sexto arco aórtico, a menudo con evolución fatal si no es tratada a tiempo, debido a la severidad de los trastornos respiratorios que provoca o por anomalías vasculares graves asociadas. Este síndrome fue descrito por primera vez por Glaevecke y Doehle en 1897(1). Si bien es poco frecuente, varios autores han publicado casos de esta anomalía(2-5) principalmente diagnosticada en niños y recién nacidos. Efectuar el diagnóstico en adultos, es extremadamente infrecuente. Es por ello que adquiere mayor interés la descripción de los hallazgos como diagnóstico inicial en este paciente de 51 años de edad, prácticamente asintomático y que concurre a la consulta clínica solamente con disnea de grado leve, detectado en un aparato TC Multicorte realizado para el estudio de su disnea.

Schinus terebinthifolia Raddi (Anacardiaceae) is rich in essential oil, distinguished by a predominance of monoterpenes and sesquiterpenes, it being widely used in traditional medicine for the treatment of inflammations.This study's objective was to investigate the chemical composition of the essential oil of S. terebinthifolia (EOST) collected in six states of Brazil, evaluate its anti-inflammatory effects in mice, and analyze the histochemistry and micromorphology of leaves and stems.Aerial parts of S. terebinthifolia were collected in six states of Brazil, and the essential oil was extracted by hydrodistillation and analyzed by gas chromatography-mass spectrometry (GC-MS). The histochemistry and micromorphology of leaves and stems were performed using standard reagents, light and field emission scanning electron microscopy, beyond energy-dispersive X-ray spectroscopy. The EOST were evaluated for anti-inflammatory activity and hyperalgesia using the carrageenan-induced paw edema methodology.The EOST showed variation across the six states in its yield (0.40%-0.86%) and chemical composition: hydrocarbon monoterpenes (28.76%-47.73%), sesquiterpenes, (31.43%-41.76%), oxygenated monoterpenes (14.31%-19.57%), and oxygenated sesquiterpenes (4.87%-14.38%). Both α-pinene and limonene were predominant constituents of essential in five regions, except for one state where α-phellandrene and limonene were the dominant components. A comprehensive description of the leaf and stem micromorphology and histochemistry was performed. In the in vivo testing, all EOST samples exerted antiedematogenic and anti-hyperalgesic effects, when tested in a carrageenan-induced paw inflammation (mechanical and thermal hyperalgesia) model with oral doses of 30 mg/kg.Our results indicate that the EOST samples collected in six Brazilian states differed in their chemical composition but not their anti-inflammatory and antihyperalgesic effects, which was correlated with the synergistic effect of its components, collaborating the etnhopharmacologycal use of this plant due to its an anti-inflammatory effect. Also, micromorphology and histochemistry of leaves and stems presented in this study provide anatomical and microchemical information, which aids species identification.

Leaves from Psychotria poeppigiana Müll. Arg. (accepted as Palicourea tomentosa (Aubl.) Borhidi), Rubiaceae, has traditionally been used in medicine for treatments of inflammation and pain; Synonymously, Cephaelis elata for the treatment of dementia; However, few scientific studies have been evidence demonstrating this activity.The aim of this study was to investigate the chemical composition of P. poeppigiana essential oil obtained from leaves (EOPP) and its antioxidant, anti-inflammatory and acetylcholinesterase (AChE) activities. Molecular docking simulations were carried out with the main constituents.EOPP (hydrodistillation) was analysed by gas chromatography-mass spectrometry (GC-MS). The fractionation of EOPP afforded germacrene D and bicyclogermacrene. The antioxidant activity of EOPP was determined by MDA assay. The inflammatory parameters were evaluated using CFA model (with paw edema, mechanical, thermal hyperalgesia, MPO and NAG) in EOPP (30, 100 and 300 mg/kg), germacrene D and bicyclogermacrene (30 mg/kg). The AChE inhibition was evaluated in rat brain structures and molecular docking simulations were carried out using Autodock v.4.3.2.GC-MS analysis identified 19 compounds, and the major compounds were germacrene D (29.38%) and bicyclogermacrene (25.21%). EOPP exhibited high antioxidant capacity (ICThe chemical compounds of the essential oil from the leaves of P. poeppigiana is based mainly on terpenes, the sesquiterpenes germacrene D (29.38%) and bicyclogermacrene (25.21%) being the major compounds. EOPP presented antioxidant, anti-inflammatory and anti-acetylcholinesterase (AChE) activities. Besides, enzyme-ligand molecular modelling showed the EOPP may act as an anti-hyperalgesic and AChE inhibitory agent. Taken together, these results might be in accordance with if folk use for pain- and inflammation-related symptoms.

Ethnopharmacological relevance Psidium guineense, popularly known as “araca-do-campo”, is used in popular medicine for the treatment of inflammatory diseases. Our research group studied an essential oil obtained from its leaves and reported anti-inflammatory and analgesic properties. However, to date, the anti-inflammatory actions of the leaf extract have not been evaluated although the traditional folk use of this plant has these indications. Aim of study The current study was designed to evaluate the antioxidant and anti-inflammatory effects and toxicity of the hydromethanolic extract of the leaves from P. guineense (HME-PG), as well as to investigate the chemical composition. Materials and methods HME-PG was chemically investigated by Ultra-high performance liquid chromatography tandem mass spectrometry (UHPLC-MS/MS). The antioxidant activity was evaluated with 2, 2-diphenyl-1-picrylhydrazyl (DPPH) and malondialdehyde (MDA). Swiss mice were orally (p.o.) pretreated with HME-PG (30, 100 and 300 mg/kg), and after 1 h received carrageenan via paw injection (edema, cold sensitivity and mechanical hyperalgesia were analyzed) or pleural injection (leukocyte migration was analyzed after 4 h) and for nociception using the formalin model. Acute (14 days) and subacute (28 days) toxicity was assessed with female Wistar rats orally treated with 500 and 2000 mg/kg HME-PG. Results HME-PG showed high levels of phenolic and flavonoid compounds. Six compounds were identified based on UHPLC-MS/MS analysis, including gallic acid, quercetin, 3′-formyl-2′,4′,6′-trihydroxy-5′-methyldihydrochalcone, vanillic acid, ursolic acid and corilagin. HME-PG exhibited an IC50 of 48.14 μg/mL in the MDA assay and an IC50 of 45.15 μg/mL in the DPPH test. The treatment with HME-PG (100 and 300 mg/kg) significantly inhibited edema at all time points evaluated, mechanical hyperalgesia after 4 h and the response to cold 3 and 4 h after carrageenan injection and anti-nociceptive effects in both phases of formalin nociception. All oral HME-PG treatments significantly inhibited leukocyte migration and plasma extravasation in the pleurisy model. Toxicity tests did not cause signs of toxicity in the treated animals. Conclusions The present study showed that HME-PG has antioxidant and anti-inflammatory properties, and no toxicity was detected after acute or subacute treatment with HME-PG, showing the possibility for the safe traditional use of P. guineense.

Aims: This study assessed comparative effectiveness of glargine 300 U/mL (Gla-300) versus degludec 100 U/mL (Deg-100) in insulin-naïve patients with T2D. Methods: This is a retrospective, multicenter, non-inferiority study based on electronic medical records. All patients initiating Gla-300 or Deg-100 were 1:1 propensity score-matched (PSM). Linear mixed models were used to assess the changes in continuous endpoints. Incidence rates (IR) of hypoglycemia were compared using Poisson's regression models. Results: Nineteen centers provided data on 357 patients in each PSM cohort. HbA1c after 6 months (primary endpoint) decreased by − 1.70% (95%CI − 1.90; − 1.50) in Gla-300 group and − 169% (95%CI − 1.89; − 1.49) in Deg-100 group, confirming non-inferiority of Gla-300 versus Deg-100. Fasting blood glucose (BG) decreased by ~60 mg/dl in both groups; body weight remained unchanged. In both groups, the mean starting dose was 12U (0.15U/kg) and it was slightly titrated to 16U (0.20U/kg). IR (episodes per patient-months) of BG ≤70 mg/dl was 0.13 in Gla-300 group and 0.14 in Deg-100 group (p=0.87). IR of BG <54 mg/dL was 0.02 in both groups (p=0.49). No severe hypoglycemia occurred. Conclusion: Initiating Gla-300 or Deg-100 was associated with similar improvements in glycemic control, no weight gain and low hypoglycemia rates, without severe episodes during 6 months of treatment. - Abstract
This study focuses on six Eucalyptusspecies, namely E. badjensis, E. benthamii, E. dunnii, E. grandis, E. globulus, and E. salignaaiming at a sustainable use of their green biomass. These species were grown at the same location and stage of development. Leaf essential oils were extracted by hydrodistillation using a Clevenger apparatus during the four seasons. Gas chromatography–mass spectrometry (GC–MS) and high-performance thin-layer chromatography (HPTLC) techniques were used to analyze the chemical composition. E. badjensisconsistently exhibited a dominant composition, with 1,8-cineole being the predominant component. Notably, the proportion of 1,8-cineole in E. badjensiswas 77.35% in spring (SP), 69.46% in summer (SU), 95.30% in autumn (AU), and 89.30% in winter (WI). E. globulusalso exhibited 1,8-cineole as its primary constituent, with proportions fluctuating slightly across seasons at 84.87%, 79.94%, 81%, and 85.88%, respectively. The proportions and constituents of various species differed significantly. HPTLC was successfully used as a swift technique to monitor the chemical composition of essential oils (EOs) in various Eucalyptusspecies during the seasonality. GC–MS and HPTLC analysis showed that different Eucalyptusspecies displayed unique chemical compositions, while both the chemical profile and productivity of all analyzed EOs were affected by seasonality. This finding was demonstrated in the principal components cluster. The analysis of six species has revealed that the EO of E. benthamiiis the most potent in thwarting the infestation of Cimex lectularius. Additionally, all six EOs demonstrated antioxidant activity in the ABTS model. EOs of E. benthamii, E. dunnii, and E. grandisshowed significant anti-inflammatory activities in experimental models.

Introduction Based on existing data regarding the durability of liraglutide in type 2 diabetes, this study aimed to assess its long-term effectiveness at 5 years and its overall impact on cardiovascular (CV) risk. Methods This was a multicenter retrospective observational study. Liraglutide was used under routine clinical practice conditions. Changes from baseline to 60 months in HbA1c, fasting plasma glucose (FPG), body weight, blood pressure, and lipid profile were assessed. United Kingdom Prospective Diabetes Study (UKPDS) scores were calculated at baseline and after 60 months to assess changes in the estimated 5- and 10-year risk for fatal and nonfatal coronary heart disease (CHD) and fatal and nonfatal stroke. Results Overall, 103 patients (age 59.0 ± 7.9 years, diabetes duration 10.4 ± 6.8 years) were involved in the study. After 60 months, HbA1c levels were reduced by − 1.0 ± 1.2%, FPG levels by − 24.5 ± 43.4 mg/dl, body weight by − 5.3 ± 6.4 kg, systolic blood pressure by − 6.5 ± 18.5 mmHg, diastolic blood pressure by − 3.6 ± 11.8 mmHg, and total cholesterol by − 16.9 ± 37.4 mg/dl. Research on pharmacology and phytochemistry originating from medicinal plants has resulted in various publications highlighting the Cerrado in central-western Brazil, which has a remarkable diversity of plant species. The reserve area selected was the Cerrado stricto sensu settlement “17 April”, Mato Grosso do Sul (MS), Brazil. However, no ethnopharmacological review focusing on the plants present in the reserve area exists, even though the consumption of medicinal plants is a widespread practice. The aims of this study were to 1) survey and document the medicinal plants present in the reserve area; 2) provide an overview of recent ethnopharmacological, phytochemical and pharmacological studies of these species; and 3) provide insight for future studies. A literature search was conducted, and relevant information was collected from authentic resources using databases such as Science Direct, PubMed, Google Scholar, Web of Science and Scopus, as well as peer reviewed articles, books and theses. Eighty-nine species belonging to 39 different families were found; the most abundant were Fabaceae (n = 13), Myrtaceae (n = 7), Rubiaceae (n = 7) and Bignoniaceae (n = 5). In terms of it empirical use, the most utilized parts were leaves (41%), bark (22%) and roots (15%). The most widespread traditional use, according to the literature review of the following plants involves the treatment of gastro-intestinal system diseases (41 spp). Chemical studies reported a high presence of terpene, phenol, and alkaloid classes. Only three are listed in the RENISUS: Casearia sylvestris, Copaifera langsdorffii and Stryphnodendron adstringens. This study demonstrated a large number of medicinal plants in an area of the Cerrado in the state of Mato Grosso do Sul, Brazil. Noting the importance of biodiversity for the development of new pharmacological approaches, many studies prove the empirical use of medicinal plants.

The obturator hernia (OH) is rare and accounts for 0.05 to 0.4% of all hernias. Because of its rarity and its nonspecific presentation, diagnosis is usually late and has high mortality rates (12-70%). A typical case of OH diagnosed preoperatively by Multislice CT with MPR and treated using emergency laparotomy with good results, is presented. The use of Multislice CT with MPR of the abdomen and pelvis, in symptoms of intestinal obstruction in elderly women with no history of previous surgery or objectified hernias, has great value for early preoperative diagnosis of OH and could help reduce the high rates of morbidity and mortality. Keywords: Early diagnosis, Multislice CT, Obturator hernia. Resumen: La hernia obturatriz (HO) es poco frecuente y representa del 0.05 al 0.4% del total de hernias. Debido a su rareza y a su presentacion inespecifica, el diagnostico es por lo general tardio y las tasas de mortalidad elevadas (12-70%). Se presenta un caso tipico de HO diagnosticada en el preoperatorio mediante TC Multi-corte con RMP y tratado mediante laparotomia de urgencia con buen resultado. El empleo de TC Multicorte con RMP de abdomen y pelvis, en cuadros de obstruccion intestinal en mujeres anosas, sin antecedente de cirugias previas ni hernias objetivables, tiene gran valor para el diagnostico preoperatorio precoz de HO y podria contribuir a reducir las elevadas tasas de morbilidad y mortalidad.Palabras clave: Diagnostico temprano, Hernia obturatriz, TC multicorte.

El Síndrome del Asa de la Arteria Pulmonar, también conocido como Lazo de la Arteria Pulmonar y Sling Sign de la literatura de habla inglesa, es una forma rara de malformación congênita del origen y recorrido de la arteria pulmonar izquierda, que se incluye dentro de las alteraciones en "anillo" que modifican la normal anatomía de los vasos mediastínicos y producen compresiones sobre la vía aérea. Se cree que se produce por un fallo en la formación del sexto arco aórtico, a menudo con evolución fatal si no es tratada a tiempo, debido a la severidad de los trastornos respiratorios que provoca o por anomalías vasculares graves asociadas. Este síndrome fue descrito por primera vez por Glaevecke y Doehle en 1897(1). Si bien es poco frecuente, varios autores han publicado casos de esta anomalía(2-5) principalmente diagnosticada en niños y recién nacidos. Efectuar el diagnóstico en adultos, es extremadamente infrecuente. Es por ello que adquiere mayor interés la descripción de los hallazgos como diagnóstico inicial en este paciente de 51 años de edad, prácticamente asintomático y que concurre a la consulta clínica solamente con disnea de grado leve, detectado en un aparato TC Multicorte realizado para el estudio de su disnea.

Treatment with liraglutide in randomized controlled trials is associated with significant reductions in glycated hemoglobin (HbA1c) and weight loss in type 2 diabetes patients. The aim of this retrospective observational study was to investigate correlations of glycemic control and weight outcomes with baseline characteristics of patients starting liraglutide in outpatient clinics in Italy.Type 2 diabetes patients were followed from baseline to 4, 8, and 12 months. Changes in glycemic parameters, weight, blood pressure, and lipids were assessed. Subanalyses were performed according to baseline characteristics. Multivariate linear and logistic regressions were used to assess correlations between glycemic efficacy, weight reduction, and liraglutide discontinuation after 12 months and baseline characteristics.Four hundred and eighty-one patients were included. Mean (SD) age at baseline was 57.3 (9.2) years, diabetes duration was 9.5 (6.8) years, weight was 106.7 (20.8) kg, body mass index (BMI; calculated as kg/m(2)) was 37.1 (6.6), HbA1c was 8.7% (1.3%), fasting plasma glucose was 168.5 (45.3) mg/dL; 38.2% were treated previously with insulin and 52.2% were treated with metformin alone. After 12 months, mean (SD) changes were HbA1c -1.2% (1.4%), fasting plasma glucose -28.3 (41.1) mg/dL, weight -3.5 (5.8) kg, BMI -1.3 (2.1), waist circumference -2.6 (6.7) cm (all, P0.001). Drop in weight and HbA1c did not differ between baseline BMI classes ≤30 or30. Weight loss was unchanged among diabetes duration quartiles, and HbA1c reduction was significantly greater in patients with ≤4 years of diabetes duration (P = 0.01). Non-insulin-treated patients reached HbA1c ≤7% significantly more often than treated patients (44.2% vs 21.2%; odds ratio = 2.94; P0.001) and had significantly greater weight loss (-4.5 [8.2] kg vs -2.6 [5.4] kg; P = 0.03). Patients on metformin reached HbA1c target more frequently than others (43.1% vs 29.7%; odds ratio = 1.80; 95% CI, 1.05-3.07). Significant positive determinants for HbA1c reduction after 12 months were baseline HbA1c, age, and prior metformin monotherapy, and weight loss at 12 months was positively correlated with baseline weight, and negatively correlated with prior insulin treatment. Overall, 5.0% of patients interrupted liraglutide before the 12th month due to lack of glycemic control; they were less frequently treated with metformin only before liraglutide (29.2% vs 50.2%; P = 0.04).Treatment with liraglutide in a real-world setting is associated with low therapy failure, good glycemic response, weight loss, and improvement in systolic blood pressure and lipid profile. The HbA1c drop did not differ among baseline BMI classes, indicating that efficacy is maintained in patients with lower BMI. The probability of reaching HbA1c ≤7% was significantly higher in patients previously treated with metformin alone and without any previous insulin. This could reinforce the hypothesis that better results with liraglutide could be achieved in patients after early metformin failure.

Type IV A choledochal cyst is a very uncommon disease, characterized by congenital cystic dilatation of the biliary tree extending to the intrahepatic biliary channels also. A case of a 19-year-old female with jaundice and abdominal pain is reported. The patient underwent a thorough clinical, laboratory and imaging examination. Computerized tomography and magnetic cholangiopancreatography revealed a 13.5 cm choledochal cyst with intrahepatic involvement. Excision of the extrahepatic part of cyst was combined with a Roux-en-Y hepaticojejunostomy. A ductal stricture was discovered at the distal part of extrahepatic cyst. There were no perioperative and late complications in the follow-up.

Introduccion. La cefalea cronica diaria (CCD) es un cuadro doloroso cronico frecuente en la practica neurologica. El diagnostico es clinico y el abordaje terapeutico resulta complejo. No esta claro todavia cual es su mecanismo de produccion, pero se reconoce un componente genetico como factor predisponente. Son multiples las areas que participan en la generacion de cefaleas primarias, entre las que se encuentra la sustancia gris periacueductal (SGPA), que participa como neuromoduladora tanto en cefaleas como en otros cuadros dolorosos cronicos. Objetivos. Para evaluar posibles cambios bioquimicos en pacientes con CCD se estudiaron los espectros por resonancia magnetica en la SGPA. Sujetos y metodos. Se estudiaron los espectros en la SGPA en 17 pacientes con CCD y se compararon con el espectro promedio de 17 sujetos sanos a traves del analisis espectroscopico diferencial. Resultados. Los sujetos con CCD presentan una reduccion mayor del 70% del metabolito N-acetil aspartato glutamato (NAAG) en la SGPA. El NAAG es un peptido relacionado con actividad antinociceptiva. Conclusion. La reduccion del NAAG en la SPGA sugiere una alteracion de la neuromodulacion de sistemas antinociceptivos en sujetos con CCD. Queda pendiente de que se establezca si la CCD es la causa o la consecuencia

Background: Guettarda viburnoides Cham. & Schltdl., "veludinho do campo", is used in the Brazilian Amazon for its effects on the central nervous system (CNS) as a "brain tonic"; however, scientific evidence is needed to elucidate its ethnobotanical uses., Objective: This study evaluated the neurobehavioural effects of an ethanolic extract of G. viburnoides (EEGV). Molecular docking, microchemical and morphoanatomical features of the species were investigated., Methods: EEGV was investigated by UHPLC‒MS/MS and dereplication and molecular network were investigated using platforms available for natural product chemistry. For the in vivo assay, EEGV was administered to mice orally (3, 30 or 100 mg/kg). The effect of EEGV on spatial memory was measured using the Morris water maze test in mice with scopolamine-induced amnesia. The depression- and anxiety-like effects were assessed by forced swimming, tail suspension, marble burying and elevated plus maze tests. The AChE inhibition was evaluated in the brains of treated mice and molecular docking simulations were carried out with the main constituents. The leaves and stems of G. viburnoides were analysed via optical microscopy, scanning electron microscopy and energy dispersive X-ray spectroscopy., Results: Secoxyloganin, grandifloroside, hyperin/or isoquercitrin, uncaric acid and ursolic acid were identified by UHPLC‒MS/MS. Molecular networking by three flavonoids, three triterpenes, two coumarins, two iridoids, and one phenolic acid. EEGV reversed these scopolamineinduced effects. In the forced swim and tail suspension test, EEGV (30 and 100 mg/kg) significantly reduced the immobility time. EEGV significantly reduced the number of buried marbles, while in the elevated plus maze test, no changes were observed compared to the Sco group. AChE activity was altered in the hippocampus. Studies of the molecular coupling of iridoid glycosides (grandifloroside and secoxyloganin) and flavonoid hyperin with AChE revealed significant interactions, corroborating the activity indicated by the inhibition assay., Conclusions: These results might be in accordance with medicinal use for neuroprotetor effects and important microchemical and micromorphological data that support the identification and quality control of G. viburnoides., (Copyright© Bentham Science Publishers; For any queries, please email at epub@benthamscience.net.)
