Background: This article describes the methods of a door-to-door screening survey exploring the distribution of disability and its major determinants in northeastern Spain. This study will set the basis for the development of disability-related services for the rural elderly in northeastern Spain. Methods: The probabilistic sample was composed of 1,354 de facto residents from a population of 12,784 Social Security card holders (age: ≧50 years). Cognitive and disability screenings were conducted (period: June 2008–June 2009). Screening instruments were the MMSE and the World Health Organization Disability Assessment Schedule. Participants screened positive for disability underwent an assessment protocol focusing on primary care diagnoses, disability, lifestyle, and social and health service usage. Participants screened positive for cognitive functioning went through in-depth neurological evaluation. Results: The study sample is described. Usable data were available for 1,216 participants. A total of 625 individuals (51.4%) scored within the positive range in the disability screening, while 135 (11.1%) scored within the positive range of the cognitive screening. The proportion of positively screened individuals was higher for women and increased with age. Conclusions: Screening surveys represent a feasible design for examining the distribution of disability and its determinants among the elderly. Data quality may benefit from methodological developments tailored to rural populations with a low education level.

[ES] Fundamento: La aparición de un número elevado de casos de brucelosis entre los trabajadores de un matadero en Zaragoza, en el que se sacrificaba ganado ovino procedente de las campañas de saneamiento ganadero, hizo iniciar una investigación para esclarecer el origen del brote. Métodos: El estudio se estructuró en tres partes: 1) Descripción del brote; 2) Estudio de la estructura y actividad del matadero en cuanto al número de animales sacrificados, carga de trabajo del personal y riesgo por zona de trabajo y 3) Estudio de casos y controles no apareado. Resultados: No hubo diferencias significativas entre las tasas de ataque por secciones de trabajo. El análisis del sacrificio reveló una variación concomitante entre las curvas de sacrificio de ovino de saneamiento y la epidémica. El estudio de casos y controles no reveló diferencias significativas para los siguientes factores de riesgo: trabajo en zona de riesgo, uso de medidas de protección y realización de cortes y heridas. Conclusiones: El brote está relacionado con el sacrificio de ovino de saneamiento. El que no se observaran diferencias significativas por secciones o por trabajar en la zona de riesgo es compatible con una exposición generalizada. Tampoco se observaron diferencias entre los trabajadores respecto a la realización de cortes y heridas o el uso de medidas de protección, lo que da importancia a la transmisión aérea. [EN]Background: The onset of a large number of cases of brucellosis among employees at a slaughterhouse in Zaragoza where sheep from livestock culling procedures were being slaughtered, led to an investigation being made for the purpose of shedding light on the cause of this outbreak. Methods: This study was organized into three parts: 1) Description of the outbreak 2) Study of the slaughterhouse layout and activity as regards the number of animals slaughtered, the employee workload and the degree of risk involved in each working area and 3) Unmatched case-control study. Results: No significant differences were found among the attack rates by the sections of the slaughterhouse where the employees in question worked. The slaughtering analysis revealed a concomitant fluctuation between the slaughter of culled sheep and the epidemic. The case-control study revealed no significant differences for the risk factors involving working in a risk area, the use of protective measures and cuts/wounds. Conclusions: This outbreak is related to the slaughter of culled sheep. The fact that no significant difference were found by sections or due to working in a risk area is compatible with an overall explanation. Nor were any differences among the employees found to exist with regard to cuts/wounds or the use of protective measures, which focuses importance on airborne spread.

Individuals residing in institutionalized living facilities are currently a high-risk group as regards scabies outbreaks. This study is aimed at analyzing the characteristics of one of these outbreaks, which occurred at a senior citizen living facility located in the municipality of Barbastro (Huesca) through the Huesca Provincial Epidemiological Monitoring Division.Following the initial notification of cases, a specific scabies prevention protocol was set out. An epidemiological investigation was conducted, proceeding to an active search for cases among all of the individuals residing or working at the facility in question. The information gathered on the cases and contacts by means of a validated survey was included in a database using Epiinfo 6.0. The attack rate and the relative risk of those residing in each area of the center were estimated, taking as a reference that having the lowest attack rate.A total of nineteen (18 and 1 respectively) cases of scabies were detected among the 104 residents and 20 workers employed at the facility. The source of the outbreak could not be specifically determined, although at least two of the first cases had begun to show some symptoms six months prior to their detection. The average delay in diagnosis was 53 days. This outbreak had a greater effect on the males in the assisted living quarters (TA = 77%; RR = 18.5, C195% = 2.7-128.7). The only employee affected was the person in charge of this area, who had not employed the universal personal protection measures during his caregiving tasks.This is a scabies outbreak at a senior citizen living facility, probably due to the delay in diagnosing the first cases and the failure on the part of one caregiver to have employed the personal protection measures. A specific scabies-prevention protocol having been implemented was fundamental for the control of this outbreak.

[ES] Fundamentos: Los géneros Salmonella y Campylobacter son los principales agentes bacterianos causantes de enteritis en el hombre en los países desarrollados. El objetivo del presente trabajo es estudiar las características descriptivas de los casos de infección por Salmonella y por Campylobacter y describir su tendencia y variación estacional en la provincia de Huesca para el período 1996-1999. Método: Las fuentes de información han sido los Laboratorios Microbiológicos Hospitalarios (LMH) que realizan analíticas de coprocultivo tanto de enfermos ambulatorios como hospitalarios. Se toma el municipio como unidad de referencia para representar la distribución espacial. Para el estudio de la tendencia se utiliza un modelo determinista multiplicativo, agrupando los datos en períodos cuatrisemanales. Resultados: Se han recogido un total de 781 casos de infección por Salmonella y 654 casos de infección por Campylobacter, con unas tasas medias anuales de 95 y 79,5 105 habitantes respectivamente. La enteritis por Campylobacter se presenta mayoritariamente en menores de 5 años (73,4 %), contra el 35,5 % para la salmonelosis. La situación se invierte en el caso de porcentaje de ingreso hospitalario; así, el ingreso por salmonelosis es casi 5 veces superior. Ambos géneros presentan una clara estacionalidad, con un pico pronunciado en el mes de agosto. Conclusiones: Se observa una tendencia ascendente en el número de aislamientos positivos para ambos microorganismos y una clara estacionalidad en la época estival. [EN] Background: The Salmonella and Campylobacter genera are the main bacterial agents causing enteritis in humans in the developed countries. The purpose of this paper is to study the descriptive characteristics of the cases of infection by Salmonella and by Campylobacter and to describe their trend and seasonal variance in the province of Huesca for the 1996-1999 period. Method: The sources of information were the Hospital Microbiological Laboratories (HML's) which conduct coproculture analyses of both outpatients and inpatients. The municipality is taken as the reference unit for plotting the space distribution. For the study of the trend, a multiplicative determinist model is used for grouping the data into four-week periods. Results: A total of 781 cases of infection by Salmonella and 654 cases of infection by Campylobacter have been included, respectively showing mean annual rates of 95 and 79.5 x 10(5) inhabitants. The enteritis by Campylobacter occurs mainly in children under five years of age (73.4%), as compared to the 35.5% of the cases of salmonellosis. The situation is the reverse in the case of the hospital admission percentages, hospitalization due to salmonellosis hence being nearly five times greater. Both of these genera are of a clear-cut seasonal nature, showing a marked peak in the month of August. Conclusions: A growing trend in the number of positive isolations for both of these organisms and a clear seasonality in the summertime have been found.
