Abstract

BackgroundMeasuring the incidence of HIV and hepatitis C virus (HCV) infection among people who inject drugs (PWID) is key to track progress towards elimination. We aimed to summarise global data on HIV and primary HCV incidence among PWID and associations with age and sex or gender.MethodsIn this systematic review and meta-analysis, we updated an existing database of HIV and HCV incidence studies among PWID by searching MEDLINE, Embase, and PsycINFO, capturing studies published between Jan 1, 2000, and Dec 12, 2022, with no language or study design restrictions. We contacted authors of identified studies for unpublished or updated data. We included studies that estimated incidence by longitudinally re-testing people at risk of infection or by using assays for recent infection. We pooled incidence and relative risk (RR; young [generally defined as ≤25 years] vs older PWID; women vs men) estimates using random-effects meta-analysis and assessed risk of bias with a modified Newcastle-Ottawa scale. This study is registered with PROSPERO, CRD42020220884.FindingsOur updated search identified 9493 publications, of which 211 were eligible for full-text review. An additional 377 full-text records from our existing database and five records identified through cross-referencing were assessed. Including 28 unpublished records, 125 records met the inclusion criteria. We identified 64 estimates of HIV incidence (30 from high-income countries [HICs] and 34 from low-income or middle-income countries [LMICs]) and 66 estimates of HCV incidence (52 from HICs and 14 from LMICs). 41 (64%) of 64 HIV and 42 (64%) of 66 HCV estimates were from single cities rather than being multi-city or nationwide. Estimates were measured over 1987-2021 for HIV and 1992-2021 for HCV. Pooled HIV incidence was 1·7 per 100 person-years (95% CI 1·3-2·3; I2=98·4%) and pooled HCV incidence was 12·1 per 100 person-years (10·0-14·6; I2=97·2%). Young PWID had a greater risk of HIV (RR 1·5, 95% CI 1·2-1·8; I2=66·9%) and HCV (1·5, 1·3-1·8; I2=70·6%) acquisition than older PWID. Women had a greater risk of HIV (RR 1·4, 95% CI 1·1-1·6; I2=55·3%) and HCV (1·2, 1·1-1·3; I2=43·3%) acquisition than men. For both HIV and HCV, the median risk-of-bias score was 6 (IQR 6-7), indicating moderate risk.InterpretationAlthough sparse, available HIV and HCV incidence estimates offer insights into global levels of HIV and HCV transmission among PWID. Abstract

In Africa, camel herding is often viewed as a subsistence or ancient activity restricted to marginal drylands and arid or desert zones. However, camel products are increasingly sold in many cities, and camels are more and more used for tourist purposes. We can also see various public or private interventions in the arid zones aiming to develop processing units for milk or meat based on a business model. This present research aimed to evaluate the multiple economic and non-economic contributions of camel activities in North African drylands through analysis at the household and territorial levels using a multifunctional approach. Based on a study in five provinces of East and South Morocco, this article proposes a framework based on rural livelihoods and ecosystemic services for assessing the contribution of camel rearing to the rural economy and territorial development. It highlighted the multiple economic and non-economic functions of this activity, including domestic, trade, and services, as well as cultural attachment and local knowledge. We conclude by discussing current changes in camel herding, between supporting specialization for value chain participation and maintaining the overall sustainability of the camel systems in dryland areas.

Abstract

Livestock plays a significant role in food production, income generation, soil quality, transportation and the overall welfare of communities, especially in developing nations. Sheep and goats are vital sources of food, livelihood and urgent financial needs. Researching livestock practices can help optimize production, enhance productivity, and improve market access, ultimately boosting the economic well-being of developing communities. Research is needed to identify climate-resilient livestock practices, develop drought-resistant breeds, and optimize feed management strategies. By enhancing the resilience of livestock systems, research can help safeguard food security and livelihoods in the face of climate change. Climate change, evidenced by temperature changes and increased extreme weather events, negatively impacts traditional farming systems and livestock production by affecting pastures, feeds, water quality and livestock diseases. Breeds of the Coastal Zone of Western Desert (CZWD) such as the Barki sheep and goat as well as the Saidi breed in Upper Egypt (UE), have adapted to these harsh conditions and are crucial for smallholders. However, there's an information gap regarding these breeds' productivity in smallholder systems. This study utilizes the Twelve Month Monitoring (12MO) survey to estimate the productivity and demographic parameters of these breeds from 2018 to 2020.

Abstract

This article follows earlier studies on the physical origin of magnetic anisotropy and the means of controlling it in polynuclear transition metal complexes. The difficulties encountered when focusing a magnetic field on a molecular object have led to consider the electric field as a more appropriate control tool. It is therefore fundamental to understand what governs the sensitivity of magnetic properties to the application of an electric field. We have already studied the impact of the electric field on the isotropic exchange coupling and on the Dzyaloshinskii–Moriya interaction (DMI). Here, we focus on the symmetric exchange anisotropy tensor. In order to obtain significant values of anisotropic interactions, we have carried out this study on a model complex that exhibits first-order spin–orbit coupling. We will show that (i) large values of the axial parameter of symmetric exchange can be reached when close to the first-order spin–orbit coupling regime, (ii) both correlated energies and wave functions must be used to achieve accurate values of the symmetric tensor components when the DMI is non-zero, and (iii) finally, an interferential effect between the DMI and the axial parameter of symmetric exchange occurs for a certain orientation of the electric field, i.e., the latter decreases in magnitude as the former increases. While DMI is often invoked as being involved in magneto-electric coupling, isotropic exchange and the symmetrical anisotropic tensor also contribute. Finally, we provide a recipe for generating significant anisotropic interactions and a significant change in magnetic properties under an electric field. [ABSTRACT FROM AUTHOR]

6. A Method for Capturing and Reproducing Directional Reverberation in Six Degrees of Freedom

Abstract

The reproduction of acoustics is an important aspect of the preservation of cultural heritage. A common approach is to capture an impulse response in a hall and auralize it by convolving an input signal with the measured reverberant response. For immersive applications, it is typical to acquire spatial impulse responses using a spherical microphone array to capture the reverberant sound field. While this allows a listener to freely rotate their head from the captured location during reproduction, delicate considerations must be made to allow a full six degrees of freedom auralization. Furthermore, the computational cost of convolution with a high-order Ambisonics impulse response remains prohibitively expensive for current real-time applications, where most of the resources are dedicated towards rendering graphics. For this reason, simplifications are often made in the reproduction of reverberation, such as using a uniform decay around the listener. However, recent work has highlighted the importance of directional characteristics in the late reverberant sound field and more efficient reproduction methods have been developed. In this article, we propose a framework that extracts directional decay properties from a set of captured spatial impulse responses to characterize a directional feedback delay network. For this purpose, a data set was acquired in the main auditorium of the Finnish National Opera and Ballet in Helsinki from multiple source-listener positions, in order to analyze the anisotropic characteristics of this auditorium and illustrate the proposed reproduction framework., Comment: This work has been accepted for the I3DA 2021 International Conference and will be submitted to IEEE Xplore Digital Library for possible publication

Abstract

Summary: Background: Although immunomodulators have established benefit against the new coronavirus disease (COVID-19) in general, it is uncertain whether such agents improve outcomes without increasing the risk of secondary infections in the specific subgroup of previously immunocompromised patients. We assessed the effect of immunomodulators on outcomes of immunocompromised patients hospitalized for COVID-19. Methods: The protocol was prospectively registered with PROSPERO (CRD42022335397). MEDLINE, Cochrane Central Register of Controlled Trials and references of relevant articles were searched up to 01-06-2022. Authors of potentially eligible randomized controlled trials were contacted to provide data on immunocompromised patients randomized to immunomodulators vs control (i.e., placebo or standard-of-care). Findings: Eleven randomized controlled trials involving 397 immunocompromised patients hospitalized for COVID-19 were included. Ten trials had low risk of bias. There was no difference between immunocompromised patients randomized to immunomodulators vs control regarding mortality [30/182 (16.5%) vs 41/215 (19.1%); RR 0.93, 95% CI 0.61–1.41; p = 0.74], secondary infections (RR 1.00, 95% CI 0.64–1.58; p = 0.99) and change in World Health Organization ordinal scale from baseline to day 15 (weighed mean difference 0.27, 95% CI -0.09–0.63; p = 0.15). In subgroup analyses including only patients with hematologic malignancy, only trials with low risk of bias, only trials administering IL-6 inhibitors, or only trials administering immunosuppressants, there was no difference between comparators regarding mortality. Interpretation: Immunomodulators, compared to control, were not associated with harmful or beneficial outcomes, including mortality, secondary infections, and change in ordinal scale, when administered to immunocompromised patients hospitalized for COVID-19. Abstract

Abstract As a network of researchers we release an open-access database (EUSEDcollab) of water discharge and suspended sediment yield time series records collected in small to medium sized catchments in Europe. EUSEDcollab is compiled to overcome the scarcity of open-access data at relevant spatial scales for studies on runoff, soil loss by water erosion and sediment delivery. Multi-source measurement data from numerous researchers and institutions were harmonised into a common time series and metadata structure. Data reuse is facilitated through accompanying metadata descriptors providing background technical information for each monitoring station setup. Across ten European countries, EUSEDcollab covers over 1600 catchment years of data from 245 catchments at event (11 catchments), daily (22 catchments) and monthly (212 catchments) temporal resolution, and is unique in its focus on small to medium catchment drainage areas (median = 43 km2, min = 0.04 km2, max = 817 km2) with applicability for soil erosion research. We release this database with the aim of uniting people, knowledge and data through the European Union Soil Observatory (EUSO).

Abstract

BackgroundPeople who inject drugs (PWID) are at increased risk for HIV and hepatitis C virus (HCV) infection and also have high levels of homelessness and unstable housing. We assessed whether homelessness or unstable housing is associated with an increased risk of HIV or HCV acquisition among PWID compared with PWID who are not homeless or are stably housed.MethodsIn this systematic review and meta-analysis, we updated an existing database of HIV and HCV incidence studies published between Jan 1, 2000, and June 13, 2017. Using the same strategy as for this existing database, we searched MEDLINE, Embase, and PsycINFO for studies, including conference abstracts, published between June 13, 2017, and Sept 14, 2020, that estimated HIV or HCV incidence, or both, among community-recruited PWID. We only included studies reporting original results without restrictions to study design or language. We contacted authors of studies that reported HIV or HCV incidence, or both, but did not report on an association with homelessness or unstable housing, to request crude data and, where possible, adjusted effect estimates. We extracted effect estimates and pooled data using random-effects meta-analyses to quantify the associations between recent (current or within the past year) homelessness or unstable housing compared with not recent homelessness or unstable housing, and risk of HIV or HCV acquisition. We assessed risk of bias using the Newcastle-Ottawa Scale and between-study heterogeneity using the I2 statistic and p value for heterogeneity.FindingsWe identified 14 351 references in our database search, of which 392 were subjected to full-text review alongside 277 studies from our existing database. Of these studies, 55 studies met inclusion criteria. We contacted the authors of 227 studies that reported HIV or HCV incidence in PWID but did not report association with the exposure of interest and obtained 48 unpublished estimates from 21 studies. After removal of duplicate data, we included 37 studies with 70 estimates (26 for HIV; 44 for HCV). Studies originated from 16 countries including in North America, Europe, Australia, east Africa, and Asia. Pooling unadjusted estimates, recent homelessness or unstable housing was associated with an increased risk of acquiring HIV (crude relative risk [cRR] 1·55 [95% CI 1·23-1·95; p=0·0002]; I2= 62·7%; n=17) and HCV (1·65 [1·44-1·90; p

Abstract

The present study sought to identify the communication needs of persons with aphasia (PWA) and of their spouses, that could ultimately be addressed with current communication aids or applications (CA/A). Among users of CA/A we solicited their opinions and experiences with current CA/A. In contrast, among those not using CA/A, we explored why they did not use CA/A. A qualitative experiential research design was used through two in-person focus groups, one in English and one in French, at a large rehabilitation hospital in the Greater Montréal region (Canada). Participants' responses were recorded, transcribed and analyzed. The thematic analysis that ensued allowed the identification of four main themes. The first theme reflects participants' observations and experiences when communicating with aphasia. The second theme regrouped responses related to successful communication with the help of CA/A. The participants' challenges and dissatisfaction with CA/A were grouped into the third theme. Lastly, a fourth theme included participants' general views about CA/A and their wishes for further development. Participants' experience with communication difficulties following aphasia and the barriers and potential facilitators to adoption of CA/A were brought to the forefront. They also highlighted those features necessary to ensuring functional uptake of CA/A by those who would benefit from it. Finally, the importance of providing training to the communication partner and sensitizing the public to the impact of aphasia on people's daily lives and on the potential benefits of using CA/A were raised as necessary future actions.

Objectives This study aims to assess the prevalence and factors associated with anal high-risk human papilloma virus (HR-HPV).Design A cross-sectional study conducted from 24 August 2020 to 24 November 2020.Setting Primary care, Cotonou, Benin.Participants 204 HIV-negative men who have sex with men initiating oral pre-exposure prophylaxis.Primary outcome measure Anal HR-HPV genotypes using GeneXpert HPV assay. Fourteen HR-HPV were evaluated: HPV-16 and HPV-18/45 in 2 distinct channels and the 11 other genotypes as a pooled result (31, 33, 35, 39, 51, 52, 56, 58, 59, 66 and 68). The potential independent variables analysed included anal gonorrhoea and chlamydia infections, and sociodemographic and sexual behaviour factors. To assess the determinants of HR-HPV, univariate and multivariate Poisson regression models were performed by using SAS V.9.4.Results Mean age±SD was 25.9±4.8 years. 131/204 men claimed insertive sex procured more pleasure. Thirty-two participants, accounting for 15.7% of the study sample, had gonorrhoea and/or chlamydia. The prevalence of any HR-HPV genotype was 36.3% (95% CI 30.0% to 43.0%). In total, 7.8% of men had HPV-16 and 7.4% had HPV-18/45. The prevalence for the pooled genotypes (31, 33, 35, 39, 51, 52, 56, 58, 59, 66 and 68) was 29.9%. Receptive anal sex during the last 6 months was strongly associated with prevalent HR-HPV infections. The adjusted proportion ratio (aPR) was 1.93 (95% CI 1.31 to 2.83). Abstract

The objective of this study was to assess the clinical impact and financial costs of next-generation sequencing (NGS) in 5 categories of pediatric and adult hematological cancers. NGS prescriptions were prospectively collected from 26 laboratories, with varied technical and reporting practice (all or only significant targets). Impact was defined by the identification of (1) an actionable mutation, (2) a mutation with prognostic and/or theranostic value, and/or (3) a mutation allowing nosological refinement, reported by local investigators. A microcosting study was undertaken in 4 laboratories, identifying the types and volumes of resources required for each procedural step. Individual index prescriptions for 3961 patients were available for impact analysis on the management of myeloid disorders (two thirds) and, mainly mature B, lymphoid disorders (one third). NGS results were considered to impact the management for 73.4% of prescriptions: useful for evaluation of prognostic risk in 34.9% and necessary for treatment adaptation (actionable) in 19.6%, but having no immediate individual therapeutic impact in 18.9%. The average overall cost per sample was 191 € for the restricted mature lymphoid amplicon panel. Capture panel costs varied from 369 € to 513 €. Unit costs varied from 0.5 € to 5.7 € per kb sequenced, from 3.6 € to 11.3 € per target gene/hot-spot sequenced and from 4.3 € to 73.8 € per target gene/hot-spot reported. Comparable costs for the Amplicon panels were 5–8 € per kb and 10.5–14.7 € per target gene/hot-spot sequenced and reported, demonstrating comparable costs with greater informativity/flexibility for capture strategies. Sustainable funding of precision medicine requires a transparent discussion of its impact on care pathways and its financial aspects.

Abstract

Plain Language Summary The intention of becoming pregnant has an evident impact on the prenatal and postnatal period. For female sex workers (FSWs) in West Africa, among whom pregnancies are frequent, a better understanding of their pregnancy intention and its influence on pregnancy occurrence could help prevent unwanted pregnancies and adverse effects of wanted pregnancies. We recruited 330 FSWs in Benin and 322 in Mali and we followed them for 12 months. We evaluated their pregnancy intention at recruitment and at 6-month follow-up. We assessed pregnancy occurrence with a pregnancy urine test and with a retrospective questionnaire asking on pregnancy occurrence at 6 and 12 months. With these data we estimated the association between intention and pregnancy. 407 FSWs were included in the first 6-month analysis and 284 at 12 months. Sixteen percent of the FSWs had a positive intention of having a pregnancy in the next 6 months in both the first 6-month and 12 months. One out of four were ambivalent and almost 60% had a negative intention. For 38.2% of the FSWs the intention changed between the two periods. Women with an ambivalent intention compared to those with positive intention, had 30% less pregnancies whereas for negative intention, women had 54% less pregnancies during the study follow-up. As for any woman, the level of pregnancy intention influences its occurrence. Moreover, nearly one out of six FSWs wants a baby despite working in the sex trade. Programmatically, early identification of these women could facilitate provision of quality antenatal and postnatal care.

Abstract Introduction Since many countries in sub‐Saharan Africa are willing to implement HIV oral pre‐exposure prophylaxis (PrEP) for men who have sex with men (MSM), data are needed to assess its feasibility and relevance in real life. The study objectives were to assess drug uptake, adherence, condom use and number of sexual partners, HIV incidence and trends in the prevalence of gonorrhoea and chlamydia. Methods In this oral PrEP demonstration study conducted prospectively in Benin, a combination of tenofovir disoproxil fumarate‐TDF 300 mg and emtricitabine‐FTC 200 mg (TDF‐FTC) was offered daily or on‐demand to MSM. Participants were recruited from 24 August to 24 November 2020 and followed over 12 months. At enrolment, month‐6 and month‐12, participants answered to a face‐to‐face questionnaire, underwent a physical examination and provided blood samples for HIV, gonorrhoea and chlamydia. Results Overall, 204 HIV‐negative men initiated PrEP. The majority of them (80%) started with daily PrEP. Retention rates at month‐3, 6, 9 and 12 were 96%, 88%, 86% and 85%, respectively. At month‐6 and month‐12, respectively, 49% and 51% of the men on daily PrEP achieved perfect adherence (self‐reported), that is seven pills taken during the last week. For event‐driven PrEP, the corresponding proportions for perfect adherence (last seven at‐risk sexual episodes covered) were 81% and 80%, respectively. Abstract

Myelodysplastic syndromes (MDS) with ring sideroblasts are hematopoietic stem cell disorders with erythroid dysplasia and mutations in the SF3B1 splicing factor gene. Patients with MDS with SF3B1 mutations often accumulate excessive tissue iron, even in the absence of transfusions, but the mechanisms that are responsible for their parenchymal iron overload are unknown. Body iron content, tissue distribution, and the supply of iron for erythropoiesis are controlled by the hormone hepcidin, which is regulated by erythroblasts through secretion of the erythroid hormone erythroferrone (ERFE). Here, we identified an alternative ERFE transcript in patients with MDS with the SF3B1 mutation. Induction of this ERFE transcript in primary SF3B1-mutated bone marrow erythroblasts generated a variant protein that maintained the capacity to suppress hepcidin transcription. Plasma concentrations of ERFE were higher in patients with MDS with an SF3B1 gene mutation than in patients with SF3B1 wild-type MDS. Thus, hepcidin suppression by a variant ERFE is likely responsible for the increased iron loading in patients with SF3B1-mutated MDS, suggesting that ERFE could be targeted to prevent iron-mediated toxicity. The expression of the variant ERFE transcript that was restricted to SF3B1-mutated erythroblasts decreased in lenalidomide-responsive anemic patients, identifying variant ERFE as a specific biomarker of clonal erythropoiesis.

Abstract

BackgroundPeople who inject drugs (PWID) experience a high prevalence of incarceration and might be at high risk of HIV and hepatitis C virus (HCV) infection during or after incarceration. We aimed to assess whether incarceration history elevates HIV or HCV acquisition risk among PWID.MethodsIn this systematic review and meta-analysis, we searched MEDLINE, Embase, and PsycINFO databases for studies in any language published from Jan 1, 2000 until June 13, 2017 assessing HIV or HCV incidence among PWID. We included studies that measured HIV or HCV incidence among community-recruited PWID. We included only studies reporting original results and excluded studies that evaluated incident infections by self-report. We contacted authors of cohort studies that met the inclusion or exclusion criteria, but that did not report on the outcomes of interest, to request data. We extracted and pooled data from the included studies using random-effects meta-analyses to quantify the associations between recent (past 3, 6, or 12 months or since last follow-up) or past incarceration and HIV or HCV acquisition (primary infection or reinfection) risk among PWID. We assessed the risk of bias of included studies using the Newcastle-Ottawa Scale. Between-study heterogeneity was evaluated using the I2 statistic and the P-value for heterogeneity.FindingsWe included published results from 20 studies and unpublished results from 21 studies. These studies originated from Australasia, western and eastern Europe, North and Latin America, and east and southeast Asia. Recent incarceration was associated with an 81% (relative risk [RR] 1·81, 95% CI 1·40-2·34) increase in HIV acquisition risk, with moderate heterogeneity between studies (I2=63·5%; p=0·001), and a 62% (RR 1·62, 95% CI 1·28-2·05) increase in HCV acquisition risk, also with moderate heterogeneity between studies (I2=57·3%; p=0·002). Past incarceration was associated with a 25% increase in HIV (RR 1·25, 95% CI 0·94-1·65) and a 21% increase in HCV (1·21, 1·02-1·43) acquisition risk.InterpretationIncarceration is associated with substantial short-term increases in HIV and HCV acquisition risk among PWID and could be a significant driver of HCV and HIV transmission among PWID. Abstract

Abstract Background In Benin, the burden of HIV is disproportionately high among female sex workers (FSWs). HIV testing and knowledge of status are starting points for HIV treatment and prevention interventions. Despite the importance given to testing services in HIV control, its uptake among FSWs remains suboptimal in Benin. HIV self-testing (HIVST) may be useful for increasing testing rates in FSWs. Methods We conducted a pilot study of the distribution of saliva-based HIVST among FSWs in Cotonou and its surroundings, Benin. The HIVST promotion and distribution model included three complementary strategies: community-based, facility-based and secondary distribution. In this qualitative study, we explored the elements influencing HIVST implementation, distribution and use among FSWs. We assessed HIVST acceptability and feasibility in this population. We conducted 29 semi-structured individual interviews with FSWs. Data were interpreted with a thematic analysis method, using the Theoretical Domains Framework. Results Only two FSWs (6.9%) were aware of HIVST before participating in the study. All participants were interested in using HIVST if available in Benin. Many advantages of HIVST were mentioned, including: autonomy, privacy, accessibility, time saving, and the fact that it is a painless test. Barriers to the use of HIVST included: the fear of unreliability, the lack of psychological support and medical follow-up and the possibility of result dissimulation. Participants thought HIVST was easy to use without assistance. HIVST enabled linkage to care for a few FSWs in denial of their HIV-positive status. No case of suicide or violence associated with HIVST was reported. HIVST secondary distribution within FSWs social network was well received. FSWs’ boyfriends and clients showed interest in using the device. Some FSWs reported using HIVST to practice serosorting or to guide their decisions regarding condom use. Conclusions Our study shows a very high level of acceptability for HIVST among FSWs in Cotonou and its surroundings. Results also demonstrate the feasibility of implementing HIVST distribution in Benin. HIVST should be implemented in Benin quickly and free of charge for all individuals at risk of HIV. HIVST offer should be integrated with comprehensive sexual health and prevention services.

Abstract

Manure valorization through on-farm use or market transactions is an ancient and widespread practice in the mixed crop-livestock systems of the semi-arid areas of North Africa. While research has long focused on the manure contribution to soil fertility at the plot level, little has been done concerning livelihood conditions. The present paper aims to assess the contribution of manure use and exchange on the livelihoods of rural communities using an original dataset collected in 2021 among 150 farmers in Central Tunisia. This analysis is carried out within the analytical agroecology framework combined with factor analysis methods. Results showed that manure use and valorization differ along the watershed, from a socioeconomic perspective in small farms operating under rainfed tree-pastoral systems, to an environmental and agronomic perspective in the mixed rainfed-irrigated systems downstream. Manure flow analysis confirmed that on-farm manure balance is positively correlated to economic wealth. However, the manure fluxes questioned the environmental sustainability of the vulnerable zones. Its use and management could significantly impact livelihood discrepancies in the future, with the increasing of demand and use of manure in more favorable zones such as irrigated lands at the detriment of the rainfed zones. Abstract

In Africa, camel herding is often viewed as a subsistence or ancient activity restricted to marginal drylands and arid or desert zones. However, camel products are increasingly sold in many cities, and camels are more and more used for tourist purposes. We can also see various public or private interventions in the arid zones aiming to develop processing units for milk or meat based on a business model. This present research aimed to evaluate the multiple economic and non-economic contributions of camel activities in North African drylands through analysis at the household and territorial levels using a multifunctional approach. Based on a study in five provinces of East and South Morocco, this article proposes a framework based on rural livelihoods and ecosystemic services for assessing the contribution of camel rearing to the rural economy and territorial development. It highlighted the multiple economic and non-economic functions of this activity, including domestic, trade, and services, as well as cultural attachment and local knowledge. We conclude by discussing current changes in camel herding, between supporting specialization for value chain participation and maintaining the overall sustainability of the camel systems in dryland areas. Abstract

: Introduction: Since many countries in sub‐Saharan Africa are willing to implement HIV oral pre‐exposure prophylaxis (PrEP) for men who have sex with men (MSM), data are needed to assess its feasibility and relevance in real life. The study objectives were to assess drug uptake, adherence, condom use and number of sexual partners, HIV incidence and trends in the prevalence of gonorrhoea and chlamydia. Methods: In this oral PrEP demonstration study conducted prospectively in Benin, a combination of tenofovir disoproxil fumarate‐TDF 300 mg and emtricitabine‐FTC 200 mg (TDF‐FTC) was offered daily or on‐demand to MSM. Participants were recruited from 24 August to 24 November 2020 and followed over 12 months. At enrolment, month‐6 and month‐12, participants answered to a face‐to‐face questionnaire, underwent a physical examination and provided blood samples for HIV, gonorrhoea and chlamydia. Results: Overall, 204 HIV‐negative men initiated PrEP. The majority of them (80%) started with daily PrEP. Retention rates at month‐3, 6, 9 and 12 were 96%, 88%, 86% and 85%, respectively. At month‐6 and month‐12, respectively, 49% and 51% of the men on daily PrEP achieved perfect adherence (self‐reported), that is seven pills taken during the last week. For event‐driven PrEP, the corresponding proportions for perfect adherence (last seven at‐risk sexual episodes covered) were 81% and 80%, respectively. The mean number (standard deviation) of male sexual partners over the last 6 months was 2.1 (1.70) at baseline and 1.5 (1.27) at month‐12 (p‐value for trend Conclusions: In West Africa, oral PrEP introduction in routine practice as a component of a holistic HIV prevention package is feasible and may not result in a significant increase in condomless sex among MSM. Abstract

The study was carried out to assess the diversity of the farming system and the associated development needs of Bedouin communities in the rural area of the hot dry Coastal Zone of Western Desert (CZWD), Egypt with the objective of improving surveillance of the Bedouin communities to the harsh desert conditions. The area has low irritated rainfall (60–140 mm/yr.) and frequent incidences of drought. Specially designed questionnaires were adopted on 151 householders and 51 women groups in 4 locations. More than 50% of the householders are fully engaged in agriculture activities, whereas 31% of them are partially engaged in other businesses. The householders are extended families of 12.2 members on average. Gender differences in education are recognizable; 20% of boys ≥15 years of age are illiterate vs. 60% of girls. Dominant agricultural activities of the householders are sheep raising (98.7%), goat raising (89.4%), olive and figs cultivation (84%), and rain-fed barley (78%). Land tenure averages 31.3 acres/ household. Crop cultivation relies on rainfall, 29% of the householders apply supplement irrigation for fruit trees and vegetables. Natural pasture is an important asset for livestock, where small householders do not hold pastureland. Females do not contribute to field cropping, while 53% of them participate in animal husbandry, especially at the house. The typology of the studied stakeholders showed clear diversification in farming systems. They are clustered into four groups; depending mainly on the livestock assets, followed by the land/crop and human assets. Number of species and flock size are the most important variables for the diversity of stakeholders. The cultivated area is the main variable within land/crop assets, family members in livestock and family size are the main variables from human assets. Most of the householders (95%) give their top need for water cisterns, water reservoirs ranked second (78.6%), followed by raising and improving goats. Pasture development got the lowest priority. Goat and poultry rearing, followed by dairy processing got the top needs of women.
