1. Campylobacter fetus foodborne illness outbreak in the elderly

Abstract: In June 2021, a cluster of seven cases of Campylobacter fetus infections occurred in a rehabilitation center and caused significant morbidity in elderly patients including five with bacteremia and two with osteoarticular medical device infections. The genetic identity identified by whole genome sequencing of the different Campylobacter fetus strains confirms a common source. This foodborne illness outbreak may have resulted from the consumption of unpasteurized dairy products, such as a cow's raw milk cheese resulting from a farm-to-fork strategy.

Abstract: ABSTRACT The aim of this study was to evaluate diagnostic means, host factors, delay of occurrence, and outcome of patients with COVID-19 pneumonia and fungal coinfections in the intensive care unit (ICU). From 1 February to 31 May 2020, we anonymously recorded COVID-19-associated pulmonary aspergillosis (CAPA), fungemia (CA-fungemia), and pneumocystosis (CA-PCP) from 36 centers, including results on fungal biomarkers in respiratory specimens and serum. We collected data from 154 episodes of CAPA, 81 of CA-fungemia, 17 of CA-PCP, and 5 of other mold infections from 244 patients (male/female [M/F] ratio = 3.5; mean age, 64.7 ± 10.8 years). CA-PCP occurred first after ICU admission (median, 1 day; interquartile range [IQR], 0 to 3 days), followed by CAPA (9 days; IQR, 5 to 13 days), and then CA-fungemia (16 days; IQR, 12 to 23 days) (P < 10−4). For CAPA, the presence of several mycological criteria was associated with death (P < 10−4). Serum galactomannan was rarely positive (

Abstract: We report 2 cases of pulmonary Bordetella hinzii infection in immunodeficient patients. One of these rare cases demonstrated the potential transmission of the bacteria from an avian reservoir through occupational exposure and its persistence in humans. We establish bacteriologic management of these infections and suggest therapeutic options if needed.

Abstract: We report the repeated isolation of Bordetella petrii in the sputum of a 79-year-old female patient with diffuse bronchiectasis and persistence of the bacterium for >1 year. The patient was first hospitalized due to dyspnea, which developed into severe cough with purulent sputum that yielded B. petrii on culture. After this first episode, the patient was hospitalized an additional 4 times with bronchorrhea symptoms. The isolates collected were analyzed by using biochemical, genotypic, and proteomic tools. Expression of specific proteins was analyzed by using serum samples from the patient. The B. petrii isolates were compared with other B. petrii isolates collected from humans or the environment and with isolates of B. pertussis, B. parapertussis, B. bronchiseptica, and B. holmesii, obtained from human respiratory tract infections. Our observations indicate that B. petrii can persist in persons with chronic pulmonary obstructive disease as has been previously demonstrated for B. bronchiseptica.

Abstract: From 2008 to 2013, 39 Helcococcus kunzii strains were collected from human clinical specimens (79% from foot ulcers), and 85% of the 39 patients were infected. Matrix-assisted laser desorption ionization–time of flight (MALDI-TOF) mass spectrometry and molecular methods accurately identified all isolates. This large study of clinical observations confirms the potential pathogenic role of H. kunzii .

Abstract: International audience; Evidence before this study We searched PubMed on June 30, 2016, for English-language cohort studies published since Jan 1, 1980, of patients with invasive listeriosis worldwide with the keywords " listeria " , " listeriosis " , " maternal " , and " neurolisteriosis ". Studies had to include epidemiological or clinical data on listeriosis. All clinical forms of infection were included (bacteraemia, neurolisteriosis, and maternal–neonatal infection). Host risk factors for listeriosis have been well identified, but the clinical features and prognostic factors of the disease are based on retrospective studies compiling heterogeneous data or random collected cases. Furthermore, no clinical trial has ever been done and medical management is not evidence based. Added value of the study Our study is the first prospective clinical study focusing on all forms of invasive listeriosis. The study is based on a national mandatory system that allowed the nearly complete capture of microbiologically proven cases. The study shows a higher burden of listeriosis than reported before: more than 80% of infected mothers experienced major fetal or neonatal complications (fetal loss, very high prematurity, early or late onset disease); only 39% of patients with neurolisteriosis survived and fully recovered. Abstract: B. petrii infection can persist in persons with chronic obstructive pulmonary disease., We report the repeated isolation of Bordetella petrii in the sputum of a 79-year-old female patient with diffuse bronchiectasis and persistence of the bacterium for >1 year. The patient was first hospitalized due to dyspnea, which developed into severe cough with purulent sputum that yielded B. petrii on culture. After this first episode, the patient was hospitalized an additional 4 times with bronchorrhea symptoms. The isolates collected were analyzed by using biochemical, genotypic, and proteomic tools. Expression of specific proteins was analyzed by using serum samples from the patient. The B. petrii isolates were compared with other B. petrii isolates collected from humans or the environment and with isolates of B. pertussis, B. parapertussis, B. bronchiseptica, and B. holmesii, obtained from human respiratory tract infections. Our observations indicate that B. petrii can persist in persons with chronic pulmonary obstructive disease as has been previously demonstrated for B. bronchiseptica.

Abstract: The excision of the staphylococcal chromosomal cassette mec (SCC mec ) from methicillin-resistant Staphylococcus aureus (MRSA) strains results in methicillin-susceptible S. aureus (MSSA) strains. In order to determine the proportion and diversity of multidrug-resistant MSSA (MR-MSSA) strains derived from MRSA strains, 247 mecA -negative isolates recovered in 60 French hospitals between 2002 and 2004 were characterized. The spa types of all strains were determined, and a subset of the strains ( n = 30) was further genotyped by multilocus sequence typing. The IDI-MRSA assay was used to test the isolates for the presence of the SCC mec element, which was detected in 68% of all isolates analyzed. Molecular analysis of the samples suggested that 92% of the MR-MSSA isolates were derived from MRSA clones of diverse genetic backgrounds, of which the clone of sequence type 8 and SCC mec type IV A accounted for most of the samples. High variations in incidence data and differences in the molecular characteristics of the isolates from one hospital to another indicate that the emergence of MR-MSSA resulted from independent SCC mec excisions from epidemic MRSA isolates, as well as the diffusion of methicillin-susceptible strains after the loss of SCC mec . MR-MSSA could constitute a useful model for the study of the respective genetic and environmental factors involved in the dissemination of S. aureus in hospitals.

Abstract: Tularemia : situation in France, issues and public health risk This is a retrospective study on natural foci of tularemia in animals as well as human cases reported in France between 1999 and 2004. Since 1999, approximately 20 to 60 animals foci of infection in hares are detected every year in 19 to 34 French departments. Human cases often occur in areas where animal foci have erupted, but not always. There are several reasons explaining the presence of this disease on French territory, its underestimation in animals, and probably in man, and its durability. The disease is generally poorly recognised, except amongst shooters who are aware of its existence. Several clinical forms are known in man, sometimes with severe symptoms. The diagnosis of tularemia can be difficult due to its non-specific clinical features. Tularemia is mostly described in hares but other species can be also affected, serving as reservoirs for the infection and sometimes acting as vectors as well. The animal population tested for tularemia should be widened to include other wild species, as well as some of our domestic animals. In man, this diagnosis should be considered in people presenting certain clinical signs. A background on epidemiological data is given. In addition, as Francisella tularensis is one of the bioterrorism agents, it is essential that clinicians and biologists recognise this bacteria and are aware of its dangers. Tularemia was removed from the list of Legally Contagious Disease in veterinary medicine in 1996. It has become a notifiable disease in human medicine in 2002., Il s’agit d’une étude rétrospective de foyers animaux de la tularémie et des cas de contamination humaine recensés en France entre 1999 et 2004. Depuis 1999, environ 20 à 60 foyers sont identifiés chaque année chez des lièvres, dans 19 à 34 départements français. Les cas humains se déclarent souvent dans les régions où sont détectés les cas animaux, mais pas toujours. Plusieurs raisons sont à l’origine de la présence de la maladie sur le territoire, de sa sous-estimation chez les animaux, et vraisemblablement aussi chez l’homme, et de sa pérennité. La maladie est généralement mal connue, sauf chez les chasseurs qui en sont informés. Elle se présente sous de multiples formes chez l’homme et peut occasionner des troubles assez graves. Le diagnostic clinique peut être difficile, à cause des symptômes relativement peu spécifiques. Le lièvre n’est pas la seule espèce animale en cause, il n’est qu’un révélateur d’un foyer de la maladie. Plusieurs autres espèces y sont sensibles et constituent des réservoirs d’infection, certaines jouant également un rôle de vecteur. Il faudrait élargir la recherche de la tularémie à d’autres espèces de la faune sauvage, mais aussi à certaines espèces domestiques. Chez l’homme, cette maladie potentiellement grave devrait faire l’objet d’une recherche systématique face à certains symptômes cliniques. Plusieurs facteurs épidémiologiques sont rappelés. Par ailleurs, Francisella tularensis faisant partie des agents du bioterrorisme, il est essentiel que les cliniciens et les biologistes apprennent à reconnaître cette bactérie et exercent une certaine vigilance. Abstract: We previously reported the case of a human chronic Bordetella bronchiseptica respiratory infection, due to contact with infected rabbits. Lipopolysaccharides of the human isolates, of one rabbit isolate and of isolates from other origins were analyzed with sera from infected mice, rabbit and human. Antigenicity and length of the lipopolysaccharide molecules varied between isolates. We showed a progressive loss of O-chain during infection, associated with an enhanced susceptibility of the isolates to the bactericidal effect of normal serum. This observation suggests the existence of an intracellular niche which selects for strains with distinct lipopolysaccharide types.

Abstract: International audience; Misidentification of rare Clostridium species often originated from the environment as clinically relevant species is problematic. A strain isolated from a traumatic leg wound first identified as C. clostridioforme was finally identified as the rare Clostridium celerecrescens. Two similar misidentifications are reported in the literature. In order to help the phenotypic differentiation of C. celerecrescens from the close species of the "C. clostridioforme group", an identification table and differential susceptibilities to 4 selected antibiotics are proposed. Once a clinical isolate is referred to this group, identification should be definitively confirmed by unambiguous methods such as 16s rDNA sequencing.

Abstract: Burkholderia gladioli , primarily known as a plant pathogen, is involved in human infections, especially in patients with cystic fibrosis (CF). In the present study, the first respiratory isolates recovered from 14 French patients with CF and 4 French patients without CF, identified by 16S rRNA gene analysis, were tested for growth on B. cepacia selective media, for identification by commercial systems, and for their antimicrobial susceptibilities, and were compared by pulsed-field gel electrophoresis (PFGE). Patients' data were collected. All 18 isolates grew on oxidation-fermentation-polymyxin B-bacitracin-lactose medium and Pseudomonas cepacia agar, but only 13 grew on Burkholderia cepacia selective agar. API 20NE strips did not differentiate B. gladioli from B. cepacia , whereas Vitek 2 GN cards correctly identified 15 isolates. All isolates were susceptible to piperacillin, imipenem, aminoglycosides, and ciprofloxacin and were far less resistant to ticarcillin than B. cepacia complex organisms. Fifteen PFGE types were observed among the 18 isolates, but shared types were not identified among epidemiologically related patients. The microbiological follow-up of CF patients showed that colonization was persistent in 3 of 13 documented cases; B. gladioli was isolated from posttransplantation cultures of blood from 1 patient. Among the patients without CF, B. gladioli was associated with intubation (three cases) or bronchiectasis (one case). In summary, the inclusion of B. gladioli in the databases of commercial identification systems should improve the diagnostic capabilities of those systems. In CF patients, this organism is more frequently involved in transient infections than in chronic infections, but it may be responsible for complications posttransplantation; patient-to-patient transmission has not been demonstrated to date. Lastly, B. gladioli appears to be naturally susceptible to aminoglycosides and ciprofloxacin, although resistant isolates may emerge in the course of chronic infections.

Abstract: A novel strain, C-138T, belonging to the genus Corynebacterium was isolated from a severe thigh liposarcoma infection and its differentiation from Corynebacterium xerosis and Corynebacterium freneyi is described. Analysis of 16S rRNA gene sequences, rpoB sequences and the PCR profile of the 16S–23S spacer regions was not conclusive enough to differentiate strain C-138T from C. xerosis and C. freneyi. However, according to DNA–DNA hybridization data, strain C-138T constitutes a member of a distinct novel species. It can be differentiated from strains of C. xerosis and C. freneyi by colony morphology, the absence of α-glucosidase and some biochemical characteristics such as glucose fermentation at 42 °C and carbon assimilation substrates. The name Corynebacterium hansenii sp. nov. is proposed for this novel species; the type strain is C-138T (=CIP 108444T=CCUG 53252T).

Abstract: We report a detailed characterization of methicillin-susceptible Staphylococcus aureus isolates from five French hospitals negative for both the mecA and the ccrAB loci but positive for the IS 431 ::pUB110::IS 431 :: dcs structure, present in some Staphylococcus cassette chromosome mec (SCC mec ) types. The presence of SCC mec -associated elements suggests that this unusual resistant phenotype is due to a partial excision of SCC mec from epidemic methicillin-resistant S. aureus . The hypothesis of a genetic relatedness is strengthened by common sequence and spa types and similar susceptibility patterns.

Abstract: Background Bordetella pertussis is the main agent of whooping cough. Vaccination with acellular pertussis vaccines has been largely implemented in high-income countries. These vaccines contain 1 to 5 antigens: pertussis toxin (PT), filamentous haemagglutinin (FHA), pertactin (PRN) and/or fimbrial proteins (FIM2 and FIM3). Monitoring the emergence of B. pertussis isolates that might partially escape vaccine-induced immunity is an essential component of public health strategies to control whooping cough. Aim We aimed to investigate temporal trends of fimbriae serotypes and vaccine antigen-expression in B. pertussis over a 23-year period in France (1996–2018). Methods Isolates (n = 2,280) were collected through hospital surveillance, capturing one third of hospitalised paediatric pertussis cases. We assayed PT, FHA and PRN production by Western blot (n = 1,428) and fimbriae production by serotyping (n = 1,058). Molecular events underlying antigen deficiency were investigated by genomic sequencing. Results The proportion of PRN-deficient B. pertussis isolates has increased steadily from 0% (0/38) in 2003 to 48.4% (31/64) in 2018 (chi-squared test for trend, p 481 insertion within the prn gene or a 22 kb genomic inversion involving the prn promoter sequence, indicative of convergent evolution. FIM2-expressing isolates have emerged since 2011 at the expense of FIM3. Conclusions B. pertussis is evolving through the rapid increase of PRN-deficient isolates and a recent shift from FIM3 to FIM2 expression. Excluding PRN, the loss of vaccine antigen expression by circulating B. pertussis isolates is epidemiologically insignificant.

Abstract: A group of six clinical isolates previously identified asCorynebacterium diphtheriaebiovar Belfanti, isolated from human cutaneous or peritoneum infections and from one dog, were characterized by genomic sequencing, biochemical analysis and MALDI-TOF mass spectrometry. The six isolates were negative for the diphtheria toxin gene. Phylogenetic analyses showed that the six isolates (including FRC0190T) are clearly demarcated fromC.diphtheriae, Corynebacterium belfantii, Corynebacterium ulceransandCorynebacterium pseudotuberculosis. The average nucleotide identity of FRC0190TwithC.diphtheriaeNCTC11397Twas 92.6%, and was 91.8% withC.belfantiiFRC0043T.C.diphtheriaesubsp.lausannensestrain CHUV2995Tappeared to be a later heterotypic synonym ofC.belfantii(ANI, 99.3%). Phenotyping data revealed an atypical negative or heterogeneous intermediate maltose fermentation reaction for the six isolates. MALDI-TOF mass spectrometry differentiated the new group from the otherCorynebacteriumtaxa by the presence of specific spectral peaks.rpoBsequences showed identity to atypical, maltose-negativeC.diphtheriaebiovar Belfanti isolates previously described from two cats in the USA. We propose the nameCorynebacterium rouxiisp. nov. for the novel group, with FRC0190T(= CIP 111752T=DSM 110354T) as type strain.
