The aims of the present study were to evaluate the efficacy and tolerability of ledipasvir/sofosbuvir (LDV/SOF) with or without ribavirin in the treatment of chronic hepatitis C (CHC) in patients with advanced liver disease and to analyse whether the use of LDV/SOF treatment is associated with a new occurrence of hepatocellular carcinoma (HCC) during and after LDV/SOF treatment. The Turkish Early Access Program provided LDV/SOF treatment to a total of 200 eligible CHC patients with advanced liver disease. The median follow-up period was 22 months. All patients were Caucasian, 84% were infected with genotype 1b, and 24% had a liver transplantation before treatment. The sustained virological response (SVR12) was 86.0% with ITT analysis. SVR12 was similar among patients with Child-Pugh classes A, B and C disease and transplant recipients. From baseline to SVR12, serum ALT level and MELD score were significantly improved (P < 0.001). LDV/SOF treatment was generally well tolerated. Only one patient developed a new diagnosed HCC. Seventeen of the 35 patients, who had a history of previous HCC, developed HCC recurrence during the LDV/SOF treatment or by a median follow-up of 6 months after treatment. HCC recurrence was less commonly observed in patients who received curative treatment for HCC compared with those patients who received noncurative treatment (P = 0.007). In conclusion, LDV/SOF with or without ribavirin is an effective and tolerable treatment in CHC patients with advanced liver disease. Eradication is associated with improvements in liver function and a reduced risk of developing a new occurrence of HCC.

The combination of hepatitis B immunoglobulin and potent nucleos(t)ide analogs after liver transplantation is considered as the standard of care for prophylaxis against hepatitis B virus recurrence. However, the recommended doses, route of administration, and duration of HBIG administration remain unclear. Moreover, hepatitis B immunoglobulin-free prophylaxis with potent nucleos(t)ide analogs has shown promising disease outcomes in preventing hepatitis B virus recurrence. The current recommendations, produced by the Turkish Association for the Study of the Liver, Acute Liver Failure and Liver Transplantation Special Interest Group, suggest a reduced need for hepatitis B immunoglobulin administration with effective long-term suppression of hepatitis B virus replication using potent nucleos(t)ide analogs after liver transplantation.

Background and aims: Type 1 hepatorenal syndrome (HRS) is a severe complication of cirrhosis. Administration of terlipressin improves renal function, but its effect on survival is unknown. The aim of this study was to assess the efficacy of terlipressin, an analog of vasopressin with a low profile of side effects, plus albumin in this condition. Material and methods: Six patients with type 1 HRS were treated with terlipressin plus albumin. Terlipressin (4 mg/day) was administered for 3 days and albumin (20 mg/d) for 5 days. Results: Renal function improved in 5 patients (serum creatinine decreased to 1.6 mg/dl from 3.4 mg/dl, and 24 hour urine volume increased to 890 ml from 81 ml). Survival rate was 83% at 1 month. Conclusions: This study shows that in patients with type 1 HRS, terlipressin-induced improved renal function is associated with an increase in survival., Giriş ve amaç: Tip 1 hepatorenal sendrom (HRS) sirozun önemli komplikasyonudur. Terlipressin kullanımı böbrek fonksiyonlarını düzeltmektedir. Bu çalışmada yan etkiler az olan vazopressin anoloğu terlipressin ile albumin kullanımının HRS'da etkileri değerlendirilmiştir. Gereç ve yöntem: Tip 1 HRS tespit edilen 6 olgu terlipressin ve albumin ile tedavi edildi. Terlipressin (4 mg/gün) 3 gün süreyle albumin (20 mg/gün) 5 gün süreyle 6 olguda uygulandı. Bulgular: Böbrek fonksiyonlarında düzelme 5 olguda izlenirken (serum kreatinin 3.4 den 1,6 mg/dl'e azaldı, 24 saatlik idrar miktarı 81 ml'den 890 ml'ye çıktı), 1 aylık yaşam süresi % 83 olarak bulundu. Sonuç: Bu çalışmanın sonuçları terlipressin uygulanması ile böbrek fonksiyonlarını düzelttiğini ve yaşam sürelerini uzattı ğını göstermektedir.

Background and aims: Although duodenal ulcer is easy to diagnose, being characterized by burning epigastric pain, ulcer like dyspepsia also has similar symptoms. In this study, the clinical symptoms of duodenal ulcer and ulcer like dyspepsia were evaluated for differential diagnosis. Materials and methods: Patients were evaluated according to parameters such as demographic features, nutritional status, symptoms and smoking. Upper gastrointestinal endoscopic examination was performed on all patients and then they were divided into duodenal ulcer and ulcer like dyspepsia groups. Results: Duodenal ulcer was present in 86(19%) of the 440 patients There were no significant differences between the two groups according to the parameters of (pain character, location, severity and radiation, nausea and vomiting, early satiety and postprandial bloating, and pyrosis). However, the parameters of gender occupation and smoking were different between groups. Conclusion: Ulcer like symptoms are not helpful in the differentiation of duodenal ulcer and ulcer like dyspepsia and endoscopic evaluation is necessary for a definitive diagnosis., Giriş ve amaç: Duodenal ülserde epigastrik yanıcı ağrı tanı için tipik olmasına rağmen ülser benzeri dispepside de aynı semptom vardır. Her iki hastalığın semptomlarını karşılaştırabilmek için endoskopik sonuçlara göre olgular iki gruba ayrılarak semptomlar karşılaştırıldı. Gereç ve yöntem: Olgular demografik, beslenme, semptomlar ve sigara kullanımı gibi özelliklerine göre değerlendirildiler. Üst gastrointestinal sistem incelemesi sonrasında ülser ve ülser benzeri dispepsi olguları iki gruba ayrılarak daha önceki sorgulamalar değerlendirildi. Bulgular: Duodenal ülser 440 olgudan 86'da (%19) mevcuttu. Gruplar arasında semptomlarda (ağrı karakteri, yeri, şiddeti, yayılması, bulantı, kusma, erken doyma, yeme sonrası şişkinlik, pirozis vb.) anlamlı farklılık izlenmedi. Gruplarda sadece cinsiyet, meslek ve sigara kullanımı ile ilgili farklar bulundu. Sonuç: Ülser benzeri semptomlar ülser ve ülser benzeri dispepsi ayırıcı tanısında yeterli değildir, kesin tanı için endoskopik inceleme yapılmalıdır.

Objective: The aim of this study was to review the complications of endoscopic retrograde cholangiopancreatography (ERCP) in the patients. Method: To evaluate the frequency of complications associated with diagnostic and therapeutic ERCP, all endoscopic procedures from 1993 to 1999 were retrospectively reviewed. Results: A total of 705 procedures were performed on 504 patients. Overall compli­cation was %8 (n: 42) and mortality was 0.5% (n: 3). Pancreatitis and cholangitis were the most common complication after diagnostic ERCP with a rate of 2% (n: 10) and bleeding after endoscopic sphincterotomy with a rate of 2 % (n: 10). Conclusions: These complication rates can be said reasonable for biliary therapeutic interventions. Metot: ERKP komplikasyonlarını araştırmak için 1993-1999 tarihleri arasındaki tüm endoskopik incelemeler değerlendirildi. Bulgular: Toplam 504 hastada 705 kez girişim uygulandı, genel komplikasyon oram %8 (n: 42) ve mortalité %0.5 (n: 3) olarak bulundu. Helicobacter pylori (Hp) infection is an infectious disease that can cause various gastrointestinal disorders such as gastrit, peptic ulcus and MALTOMA. These disorders and related complications can be prevented by the eradication of Hp. In this study we aimed to investigate the effectiveness of Ranitidine bismuth citrate and clarithromycine combination which have been in use against this infection in our country for three years. 45 patient ( 32 male, 13 female), mean age 38 (20-60) who were diagnosed as having gastric or duodenal ulcus by the oesophagogastroduodenoscopy (OGD) and the detection of Hp in their biopsies were included in the study. Ranitidine bismuth citrate 2x400 mg for one month + Clarithromycin 2x500 mg were given to the patients for one week. OGD was performed in patients on day 15 and day 45 and biopsies were taken again. Hp negatives in repeated biopsies were considered as “eradication”. Eleven of the 45 patients were excluded in the study, since they did not attend the control, their OGD and subsequent biopsies on day 15 could not be conducted. The eradication rates of the cases on day 15 and 45 days were 85.2% (29/34) and 91.1% (31/34) respectively. The ulcus healing rates were 88.2% (30/34), 94.1% (32/34) respectively. While there was no complain to cause the withdrawal of therapy, the most common side-effects seen were tastelessness and mild abdominal pain. As a result ıt was found that one week Ranitidine bismuth citrate therapy in the Hp eradication and ulcus healing was efficacious and comparable with the other triple combination therapies. Hp eradikasyonu ile bahsedilen gastrointestinal problemler ve bunlara bağlı komplikasyonlar önlenebilmektedir. Biz Hp eradikasyonu için son yıllarda ülkemizde de kullanılmaya başlanan Ranitidine-Bizmut Sitrat ve Klaritromisin kombinasyonunun etkinliğini araştırdık. Endoskopilerinde gastrik yada duodenal ülser tesbit edilen ve biyopsilerinde Hp (+) bulunan, yaş ortalaması 38 (20-60) olan 32 erkek ve 13 kadından oluşan 45 kişi çalışmaya alındı. Olgulara 1 ay süre ile Ranitidine- Bizmut Sitrat 2x400 mg + 1 hafta süre ile Klaritromisin 2x500 mg verildi. Olgulara 15, 45. günlerde endoskopi yapılarak biyopsileri alındı. Tekrarlanan biyopsilerinde Hp (-) olanlarda eradikasyon olduğu kabul edildi. Çalışmaya alınan 45 olgudan 11 tanesi kontrole gelemediği için 15. günde endoskopi ve biyopsileri yapılamadı ve bu olgular çalışma dışında tutuldu. Olguların 15, 45 günlerdeki Hp eradikasyon oranları sırası ile %85.2 (29/34), % 91.1 (31/34) idi. Olgulardaki ülser iyileşme oranları ise sırası ile %88.2(30/34), % 94.1 (32/34) idi. Hiç bir olguda tedavinin kesilmesini gerektiren bir yan etki oluşmazken en sık görülen yan etki tat değişikliği ve hafif karın ağrısı idi. Sonuç olarak Hp eradikasyonu ve ülser iyileşmesinde bir haftalık Ranitidin Bizmut Sitrat tedavisi diğer 3'lü kombinasyon tedavileri ile kıyaslanabilecek oranlarda etkin bulunmuştur. Background/aim: Percutaneous endoscopic gastrostomy (PEG) is a preferable way of nutritional support in patients with normal gastrointestinal function that can not be fed via oral way due to several causes. The aim of this study is to analyze the association between the nutritional status of the cases and early PEG complications. Material and methods: One hundred and twenty eight PEG applied cases in Inonu University, Faculty of Medicine, Department of Gastroenterology between August 2003 - December 2007 that could not be fed orally at least for the last four weeks were included to the study. The cases were determined in terms of malnutrition before the procedure using following criteria: body mass index (BMI)5% within the last three months. All the complications within 30 days after the PEG procedure were recorded. Additionally, the age, sex, functional status, anti-biotherapy, and duration of the hospitalization before the PEG were examined. Results: Eighty four patients (65.6%) had findings of malnutrition before the PEG application. Early complications seen in patients with malnutrition were significantly higher than the patients without malnutrition (41.7% vs. 18.2%, p = 0.04). Wound infection surrounding the PEG tube site was the most common early complication. Mortality rate within the first month was not different between the groups. Early complications were significantly increased with hospitalization more than ten days before the PEG procedure and malnutrition in the multivariate analysis (OR:2.74; P=0.04 and OR:2.70; p=0.04, respectively). Conclusion: Malnutrition increases the early complications of PEG procedure independent from the underlying diseases and functional status of the patients. Hence, PEG must be performed before the existence of malnutrition in patients who cannot take orally and expected not to be fed orally for a long time and the cases with malnutrition should be followed closely for development of the possible PEG complications. Key words: Percutaneous endoscopic gastrostomy, Complications, Malnutrition., Giriş ve Amaç: Perkütan endoskopik gastrostomi (PEG) çeşitli nedenlerle oral yoldan beslenemeyen ancak gastrointestinal fonksiyonları normal olan hastalarda nütrisyon desteği sağlamak için tercih edilen bir yöntemdir. Bu çalışmada, PEG uyguladığımız olguların irdelenmesi, olguların nütrisyonel durumlarıyla erken dönemde görülen PEG komplikasyonları arasındaki ilişkinin araştırılması amaçlanmıştır. Gereç ve yöntem: Çalışmaya; İnönü Üniversitesi Tıp Fakültesi Gastroenteroloji Kliniği'nde Ağustos 2003 - Aralık 2007 tarihleri arasında en az 4 hafta süreyle oral beslenemeyeceği öngörülerek PEG uygulanan 128 olgu alınmıştır. İşlem öncesinde olgular malnütrisyon yönünden; beden kitle indeksi (BKİ)%5 olması kriterleriyle değerlendirilmiştir. İşlem sonrası 30 gün içinde görülen tüm komplikasyonlar kaydedilmiştir. Ayrıca hastaların yaşları, cinsiyetleri, fonksiyonel durumları, antibiyotik tedavileri ve PEG öncesi hastanede kalış süreleri de incelenmiştir. Bulgular: PEG uygulaması öncesi 84 (%65.6) olguda malnütrisyon bulguları saptandı. Malnütrisyonlu olguların bulunduğu grupta, malnütrisyon bulgusu olmayanlara göre anlamlı ölçüde fazla erken komplikasyon görüldü (%41.7'ye %18.2, p< 0.05). En çok görülen erken komplikasyon yara çevresi enfeksiyonu idi. İşlem sonrası 1. ayda mortalite sıklıkları bakımından her iki grup arasında anlamlı fark saptanmadı. Multivariate analiz ile malnütrisyonun ve PEG öncesi 10 günden uzun hastanede yatış süresinin, hastalarda erken komplikasyon riskini artırdığı anlaşıldı (sırasıyla OR:2.74; P=0.04 ve OR:2.70; p=0.04). Sonuç: Malnütrisyon; PEG uygulanan olgularda erken komplikasyonları, altta yatan hastalıklardan ve olguların fonksiyonel durumundan bağımsız olarak artırmaktadır. Bu nedenle oral yoldan beslenemeyen ve uzun süre oral beslenemeyeceği düşünülen olgulara malnütrisyon gelişmeden PEG uygulanmalı ve malnütrisyonlu olgular gelişebilecek komplikasyonlar bakımından daha sıkı izlenmelidir. Endoscopic procedures have lower complications, morbidity and mortality rates than surgery, in this respect endoscopic removal of common bile duct stones has become a routine procedure. We report on 182 patients treated between 1993 and 1999 with common bile duct stones (82 men, 100 women, median age 57 years). Endoscopic retrograde cholangiopancreatography (ERCP) was successful in 179 of 182 patients (98%). Endoscopic sphincterotomy, removal of choledochal stones or palliation with stent was successful in 175 of 179 patients (97%). Overall success rate of endoscopic treatment in choledochal stones was 96%. Complication rates were as follows: pancreatitis %1, postsphincterotomy bleeding %1, and perforation 0.5%. One patient with hemorrhage died after surgery because of pancreatic ascites and sepsis. It is concluded that endoscopic removal of bile duct stones is a method with a high rate of success and low rate of complication. Bu çalışmada 1993 ve 1999 yılları arasında tedavi gören 181 koledok taşlı olgu değerlendirilmiştir (82 erkek 100 kadın, ortalama yaşları: 57). Endoskopik retrograd kolanjiopankreatografi (ERKP) 182 hastanın 179'unda (%98) başarılı oldu. Endoskopik sfinkterotomi, basket ile taş çıkarılması ya da stent ile palyasyon sağlanması 179 hastanın 175'de (%97) başarılı oldu. Koledok taşlarında genel endoskopik tedavi başarısı ise % 96 olarak bulundu. Bu hastalarda görülen komplikasyon oranları ise sırayla Pankreatit % 1, sfinkterotomi sonrası kanama % 1, perforasyon % 0.5 olarak bulunurken bir hasta kanama nedeniyle yapılan cerrahi girişim sonrasında sepsis ve pankreatik assit ile kaybedilmiştir. In 41 diabetic patients with clinical and electrophysiological evidences of motor sensory peripheral neuropathy and in 20 normal control subjects, sympathetic skin responses (SSRs) were recorded from both hands and feet by electrical stimuli to the median nerve at the wrist. The SSRs were present in all limbs of normal subjects, but were not be able to record at the feet in 14.6 % and the hands in 4.9 % of the diabetic patients. There were no significant associations between abnormalities of SSRs, age, duration and form of diabetes, signs of peripheral neuropathy and electrophysiological abnormalities. Absence of SSR in the feet was more frequent found in patients with symptoms of autonomic dysfunction as bladder dysfunction (p< 0.05), diarrhea (p< 0.03) and male impotence (p< 0.05). These results suggest that SSR is a valuable test in the assessment of autonomic involvement in diabetic peripheral neuropathy. Kontrol olgularının hepsinde, el ve ayaktan tüm uyarılarda SDC'ler mevcuttu. Diabetik hastaların %14.6'inde ayakta, %4.9'unda elde SDC'ler kaydedilemedi. Diabetik hastalarda SDC bozukluğu ile hastanın yaşı, diabetin tipi, süresi, periferik nöropatinin klinik ve elektrofizyolojik bulguları arasında anlamlı bir ilişki olmadığı görüldü. Ayaklarda SDC yokluğu mesane disfonksiyonu (p< 0.05), ishal (p< 0.03) ve erkeklerde impotans (p< 0.05) gibi otonomik disfonksiyon semptomlu hastalarda daha sık bulundu. Sonuçlar SDC'nin diabetik periferal nöropatili hastalarda otonomik tutulumu göstermekte değerli bir inceleme yöntemi olduğunu telkin etmektedir. Background/aim: Non-variceal upper gastrointestinal system bleedings (NVUGISB) are one of the most frequent emergencies seen in the gastroenterology clinics. In the present study, we aimed to determine the etiologic and risk factors, treatment modalities and mortality rate of NVUGISB cases. Material and methods: 524 cases with NVUGISB referred to İnönü University Medical School, Gastroenterology Department between 2003-2007 years and examined with endoscopy were evaluated retrospectively in terms of demographical characteristics, etiology, risk factors, treatment modalities and mortality. Results: Mean age of 404 male (77%) and 120 female (23%), 524 cases in total was 58.4 (18-92). 217 of the cases (41%) were smoking, 36 (7%) were drinking alcohol, 143 (27%) were using acetyl salicylic acid (ASA), 21 (23%) non-steroidal anti-inflammatory drug (NSAID), 28 (5%) anticoagulant agent, 5 (1%) corticosteroid, and 269 (51%) had co-morbidity. The first three most common causes of bleeding detected by endoscopy were duodenal ulcer (245 (47%)), gastric ulcer 122 (%23) and erosive bulbitis (37 (7%)). 503 (%96) of the cases responded to medical therapy whereas 11 (2.1%) cases required surgical intervention. Only ten patients (1.9%) died. Conclusion: Duodenal ulcer is the most common cause of NVUGISB detected by endoscopy. ASA or NSAID were found as risk factors in half of the bleeding cases. Bleeding is seen in males and elders more commonly and advanced age and co-morbid diseases increase the mortality. Bu çalışmada; VDÜGİSK tanılı olgularımızın etiyolojik faktörlerinin, risk faktörlerinin, tedavilerinin ve mortalitelerinin değerlendirilmesi amaçlanmıştır. Gereç ve yöntem: İnönü Üniversitesi Tıp Fakültesi Gastroenteroloji Kliniği'nde 2003-2007 yılları arasında VDÜGİSK nedeniyle endoskopi yapılarak izlenmiş olan 524 olgu demografik özellikleri, etiyolojileri, risk faktörleri, tedavileri ve mortaliteleri yönünden retrospektif olarak değerlendirilmiştir. Bulgular: Dört yüz dört erkek (%77), 120 kadın (%23), toplam 524 olgunun yaş ortalaması 58.4 (18-92) idi. Olguların 217'sinde (%41) sigara, 36'sınde (%7) alkol, 143'ünde (%27) aspirin 121'inde (%23) nonsteroid anti-inflamatuvar ilaç (NSAİİ), 28'inde (%5) antikoagulan ilaç, 5'inde (%1) steroid kullanımı, 269'unda (% 51) yandaş hastalık saptandı. Endoskopi ile en sık saptanan ilk üç kanama nedeni; 245 (%47) olguda duodenum ülseri, 122 (%23) olguda mide ülseri, 37 (%7) olguda eroziv gastrit idi. Olguların 503'ü (%96) medikal ve endoskopik tedaviye, 11'i (%2.1) cerrahi tedaviye yanıt verdi. Toplam olarak 10 olgu (%1.9) kaybedildi. Sonuç: Endoskopik olarak, VDÜGİSK'nin en sık nedeninin duodenal ülser olduğu saptanmıştır. Kanamalı olguların yarısında Aspirin veya NSAİİ kullanımı risk faktörü olarak bulunmaktadır. Dieulafoy vascular lesion, a rare cause of massive upper gastrointestinal bleeding, has a high mortality rate when diagnosis and treatment are delayed. A 41-year old woman was admitted to the hospital because of massive upper gastrointestinal bleeding. Upper gastrointestinal endoscopy did not revealed localization of bleeding. Emergency surgery was performed and localization of bleeding was seen within 4 cm of the gastroesophageal junction on lesser curvature on the stomach. Histopathoiogicai study of wedge resection material showed Dieulafoy vascular malformation. Dleulafoy's lesion Is a difficult lesion to recognize. It should be considered when evaluating any unexplained acute and recurrent major gastrointestinal bleeding. Masif üst gastrointestinal kanama ile hastanemize müracaat eden 41 yaşındaki bayan hastanın üst gastrointestinal sistem endoskopisinde kanama yeri tam olarak belirlenemedi. Acil cerrahi girişim uygulandığında, kanama odağının gastroözofagial bileşkeye 4 cm uzaklıkta küçük kurvatur tarafında olduğu görüldü. Dieulafoy vasküler malformasyonu olarak şüphelenilen bölge zedge rezeksiyon He çıkarıldı. Histopatolojik İncelemede İse tanı doğrulandı. Dieulafoy vasküler maiformasyonunun tanınması güç olabilmektedir. Fasciola Hepatica is a trematode that infects cattle and sheep, humans may be infected sporadically. We describe the case of a 43-year-old female who had a history of upper right quadrant pain and fever. Fasciolasis was diagnosed on the basis of clinical findings, laboratory investigations, and imaging studies with ultrasonography and cholangiography. Kronik viral hepatit, yaygın bir hastalık olması, siroz ve hepatosellüler karsinoma gibi ciddi komplikasyonları ile tüm dünya için önemli bir sağlık sorunudur. Ülkemiz kronik B hepatiti ve kronik D (delta) hepatiti açısından dünya üzerindeki endemik bölgelerdendir. Kronik viral hepatit C açısından ise Batı Avrupa ve Kuzey Amerika ülkelerine benzer epidemiyolojik bulgulara sahiptir. Kronik viral hepalitlerin sağlık üzerine bu olumsuz etkileri ve oluşturduğu komplikasyonlar açısından tedavi edilmeleri büyük önem taşımaktadır. Biz bu prospektif çalışmamızda kronik viral hepatit nedeniyle tedavisini tamamlamış olduğumuz olgulardaki bulguları sunuyoruz. Kliniğimizde 1993-1996 yılları arasında başvuran ve klinik, serolojik, biyoşimik ve histopatolojik olarak kronik viral hepatit tanısı konulan ve şu anda tedavileri bilmiş ve halen izlenmekte olan 18 hasta çalışmaya alındı. Yaş ortalamaları 36.53+6.22 (23-45) ( 6 kadın, 12 erkek) idi. Hastaların 16'sında etiyolojik ajan hepatit B virüsü, 2 olguda ise hepatit C virüsü İdi. Kronik hepatit C (HCV)'li olgulara 1 yıl süre ile haftada 3 kez 3 milyon, kronik hepatit B olan (HBV) olgulara ise haftada 3 kez 5 milyon ünite rekombinant alfa-2a interferon verildi. Olguların klinik, biyoşimik ve serolojik değerlendirmeleri 2 ay ara ile yapıldı. Tedavi sonundaki değerlendirmelerde HBV gurubundaki 16 olgudan 9'unda (%56.25) tam cevap, 2 sinde kısmi cevap (% 12.25), 5 (%3 1.25) olguda ise primer cevapsızlık vardı. HBV gurubunda Tam cevap veren olgulardan 2 (%14.2)'sinde tedavi sonrası 6. ayda relaps görüldü. Hepatit C gurubunda iki olgunun birisinde tam cevap birinde ise primer cevapsızlık görüldü. Tedaviye cevap veren HCV'li olguda ise tedavi sonrası ikinci ayda relaps oldu. Sonuç olarak rekombinant interferon alfa tedavisi kronik viral hepatit tedavisinde etkin ve efektif bir tedavi yöntemi olmakla beraber arzulanan sonuçları elde etmekten oldukça uzaktır bu nedenle kronik viral hepatitlerin tedavisinde interferon yeni ilaçlarla kombine edilmeli veya yeni tedavi alternatifleri aranmalıdır. Çalışmamız halen devam etmekte olup toplu sonuçlar ileride belirtilecektir. 66 84

ÖZET 8 erkek, 32 kadın toplam 40 şişman hastaya ilk 8 haftada plasebo+düşük kalorili diyet, ikinci 8 haftada düşük kalorili diyete ilave olarak plasebo ile aynı görünüme sahip 2x15 mg dexfenfluramin verildi. Hem plasebo, hem de dexfenfluramin tedavisi döneminde istatistiksel anlamı olan kilo kaybı oluştu (sırasıyla p0.05). Hem plasebo hem de aktif ilaç döneminde sistolik ve diastolik kan basınçlarındaki azalmalar istatistiksel olarak anlamlı iken (sırasıyla p0.05). Kalp dakika atım hızında ise plasebo döneminde olan artış ve dexfenfluramin döneminde olan azalma anlamlı idi (sırasıyla p0.05). İnsülin ve C peptid düzeylerinde her iki dönemde görülen azalmalar anlamlı (sırasıyla p0.05), aktif ilaç döneminde ise minimal olmakla birlikte istatstiksel anlamı olan artış izlendi (p0.05). Pulse rate increased with placebo (p0.05). Reductions were occured in insulin and C peptide levels in both treatment periods (p0.05), significant increase was observed with dexfenfluramine treatment (p0.05). Side effects were short term and occured early in either treatment periods. No statistically significant changes observed in hemotological and any of the other biochemical parameters studied. 77
