Introduction: Different approaches and mesh positions are used for minimally invasive ventral hernia repair (MIS-VHR). Our aim was to evaluate the trends and short-term outcomes of intraperitoneal onlay mesh (IPOM), preperitoneal, and retromuscular repairs for small ventral hernias. Methods: We conducted a retrospective cohort study using the Abdominal Core Health Quality Collaborative (ACHQC). We included elective MIS-VHR in adults with hernia defect width < = 6 cm from 2012 to 2021. We compared patient/hernia characteristics, trends, and short-term outcomes between IPOM, preperitoneal, and retromuscular repairs. Inverse probability of treatment weighting (IPTW) was used to balance baseline characteristics. Results: A total of 7261 patients were included (IPOM = 4484, preperitoneal = 1829, retromuscular = 948). Preperitoneal repair was associated with lower rates of incisional (preperitoneal = 37%, IPOM = 63%, retromuscular = 73%) and recurrent hernias (preperitoneal = 11%, IPOM = 21%, retromuscular = 22%) compared to IPOM and retromuscular. Median defect width was 3.0, 2.0, and 4.0 cm for IPOM, preperitoneal, and retromuscular, respectively. There has been a progressive increase in the proportion of preperitoneal and retromuscular repairs over time (10% in 2013–53% in 2021 of all MIS-VHR). Robotic approach was more frequently utilized in preperitoneal and retromuscular (both > 85%) compared to IPOM (47%). Transversus abdominis release was performed in 14% of retromuscular repairs. After IPTW, no clinically significant differences were noted in the short-term outcomes between IPOM versus preperitoneal. Retromuscular repairs were associated with higher risk of 30-day reoperation (OR = 3.54, 95%CI [1.67, 7.5] and OR = 5.29, 95%CI [1.23, 22.74]) compared to IPOM and preperitoneal repairs, respectively, and higher risk of 30-day readmission compared to preperitoneal repairs (OR = 2.6, 95%CI [2.6, 6.4]). Conclusion: Based on ACHQC data, preperitoneal and retromuscular approaches for MIS-VHR of small hernias have increased over time and are primarily performed robotically. Transversus abdominis release was performed in 14% of retromuscular repairs of these small hernias. Background: Venous thromboembolism is a significant cause of morbidity and mortality among patients undergoing ventral hernia repair. Several risk-assessment models have been developed to predict venous thromboembolism risk for various surgical procedures. However, these models do not include hernia-specific risk factors. Therefore, we sought to evaluate the predictors of venous thromboembolism in patients with a ventral hernia repair in a national hernia-specific database., Methods: The Abdominal Core Health Quality Collaborative database was retrospectively queried for ventral hernia repair data. The ventral hernia repair procedures were divided into 2 groups on the basis of whether the patients developed perioperative venous thromboembolism. Baseline and operative characteristics and perioperative outcomes were compared between the 2 groups. The logistic regression was used to assess the association between different risk factors and venous thromboembolism., Results: A total of 43,558 patients were included in the study. The 30-day venous thromboembolism rate among these patients was 0.62% (n = 269). Venous thromboembolism was associated with greater 30-day readmission (odds ratio, 4.29; 95% confidence interval, 3.20-5.76), reoperation (odds ratio, 3.97, 95% confidence interval, 2.63-6.00), and mortality rates (odds ratio, 5.95, 95% confidence interval, 2.62-13.48), all P < .001. Bivariate analysis identified a statistically significant association between several patient characteristics, operation, and hernia- related details; however, multivariate analysis revealed that that only hernia width (each increasing centimeter: odds ratio, 1.07, 95% CI, 1.05-1.09) and operative time compared with 0-59 minutes (60-119 minutes: odds ratio, 4.23; 95% confidence interval, 1.86-9.62; 120-179 minutes: odds ratio, 5.78; 95% confidence interval, 2.57-13.0; 180-239 minutes: odds ratio, 8.01; 95% confidence interval, 3.54-18.10; and 240+ minutes: odds ratio, 17.4; 95% confidence interval, 7.88-38.37) were significantly associated with venous thromboembolism risk., Conclusions: Venous thromboembolism is an uncommon complication of ventral hernia repair. It is associated with increased readmission, reoperation, and mortality rates. Objective: To evaluate the spectrum of mineral abnormalities and bone disease (BD) in hemodialysis patients at Jordan University Hospital (JUH), Amman, Jordan., Methods: A cross-sectional study was conducted among 63 patients (38 males and 25 females), mean age 44.19 years (range 17-76 years), with chronic kidney disease (CKD) on regular hemodialysis at JUH between November 2004 and April 2005. All patients have undergone complete blood count, chemistry profile, alkaline phosphatase, serum albumin, intact parathyroid hormone (iPTH) and plain x-rays., Results: Bone disorders were identified in 45 patients on x-rays (70%). Osteopenia was found in 43 patients (68.3%), subperiosteal resorption in 24 patients (38.3%) and metastatic calcification in 22 patients (35%). Hypocalcemia was found in 28.6% and hypercalcemia in 7.9%. All patients were taking calcium carbonate, and 55.5% of patients were on vitamin D supplements. The calcium levels in 63.5% and the phosphorus levels in 50.8% of patients were within the recommended guidelines of the National Kidney Foundation Kidney Disease Outcomes Quality Initiative (K/DOQI). Serum i-PTH level was above 300 pg/ml high turnover bone disease in 24.6% of patients, 21.3% had iPTH of 150-300 pg/ml target, and 44.3% had i-PTH levels below 100 pg/mL suggesting a dynamic bone disease. Patients with severe bone disease had a statistically significant higher iPTH levels (p<0.005)., Conclusion: Bone disease and mineral abnormalities are common in hemodialysis patients at JUH. Earlier detection of bone disease and better overall management strategy may reduce the frequency and severity of bone disease in CKD patients in Jordan.
