The surge of the COVID-19 pandemic (December / 2019 - May/2023) and its catastrophic effect worldwide have necessitated emergent intervention to reduce its influence on people's health and life. To eliminate and reduce the impact of COVID-19 infection, COVID-19 vaccination was emergently authorized in December 2020 which has established good safety and efficacy. Having said that, some adverse effects merged in a few individuals. We are reporting an adolescent patient a 17-year-old female who has been diagnosed with Graves’ disease after post-COVID-19 vaccinations. In addition, she was a confirmed case of COVID-19 infection three months earlier. The patient presented with typical features of hyperthyroidism 30 days post receiving the first dose of the vaccination. Based on the patient's presentation relative to the administration of the vaccine and prior infection of the virus. We proposed the synergistic effect of both factors to induce Graves’ disease in this young healthy female with no family history of autoimmune disease. We are reporting this case for pediatric endocrinologists to be aware of the interaction and possible impact of the COVID-19 vaccine on thyroid function.

Purpose: In lung cancer patients, the distinction between synchronous primary lung cancer and intrapulmonary metastasis can be challenging. The intensity of FDG uptake in pulmonary lesions has been shown to be potentially useful in classifying synchronous lung cancer. The aim of this retrospective study is to investigate the effectiveness of FDG uptake in differentiating metastases from synchronous primary lesions in the setting of lung cancer. Methods: Consecutive patients with primary lung cancer with two or more malignant lung lesions referred for (18F)-FDG PET-CT imaging between 2010 and 2019 were reviewed and classified into synchronous and metastasis groups. Lesional maximum standardized uptake values (SUVmax), relative differences in SUVmax and SUVmax ratios were calculated and compared using receiver operating characteristic (ROC) curve analysis. Intra-group correlation in SUVmax between lesion pairs was examined using Pearson's and Spearman's correlation analysis. Results: 94 patients were included for analysis, divided into synchronous (n = 62; 68 lesion pairs) and metastasis (n = 32; 33 lesion pairs) groups. The correlation of FDG uptake between lesions in the metastasis group was strong (r = 0.81). A significant difference in mean relative difference in SUVmax (synchronous: 0.50±0.23 metastasis: 0.34±0.17, p = 0.001) and mean SUVmax ratio (synchronous: 2.6 ± 1.7 metastasis: 1.7 ± 0.6, p < 0.001) was observed. ROC analysis revealed a fair AUC (0.71–0.72) for these parameters, with an associated sensitivity of 59 % and specificity of 82 % at optimal cut-off values. Conclusion: Differences in FDG uptake intensity among multiple synchronously presenting malignant nodules may be helpful to distinguish second primary lung tumours from metastatic spread.

Midkine (MK) and pleiotrophin (PTN) belong to the same family of cytokines. They have similar sequences and functions. Both have important roles in cellular proliferation, tumors, and diseases. They regulate and are expressed by some immune cells. We have recently demonstrated MK production by some human innate antigen-presenting cells (iAPCs), i.e., monocyte-derived dendritic cells (MDDCs) and macrophages stimulated through Toll-like receptor (TLR)-4, and plasmacytoid dendritic cells (pDCs) stimulated through TLR 7. While PTN production was only documented in tissue macrophages. TLRs 3, 7, 8, and 9 are nucleic acid sensing (NAS) TLRs that detect nucleic acids from cell damage and infection and induce iAPC responses. Objectives: To describe the demographic distribution of cystic fibrosis (CF) in Omani children, estimate the national prevalence, and provide updated mutational panels of the cystic fibrosis transmembrane conductance regulator (CFTR) gene. Methods: We conducted a retrospective cross-sectional study of all CF patients who had been diagnosed and followed-up at Sultan Qaboos University Hospital and Royal Hospital in Oman between 2006 and 2020. Data were collected from electronic hospital records and telephone interviews. Results: A total of 227 patients with CF were included in the study. Geographical clusters of the disease were identified in the governorates of Al-Batinah, A’Dhahirah, and A’Dakhiliyah. Parental consanguinity and family history of CF were present in 68.3% and 69.6% of the patients , respectively. The most common CFTR mutation was p.Ser549Arg (52.0%), followed by p.Phe508del (12.3%), and c.2988+1G>A (4.4%). Three novel CFTR mutations were identified, viz., Leu88TyrFs*, p.Asp192Val, and c.4242+1G>C. Conclusions: The estimated prevalence of CF in Oman is 10.3 per 100 000 individuals. Premarital genetic counseling and preimplantation genetic testing are recommended in CF-prevalent regions.

The main aim of this research work was to isolate and characterize the antimicrobial compounds that can be extracted from the leaves of Dodonaea viscosa (D. viscosa) and to assess their antimicrobial potency by established in-vitro agar diffusion method. The methanol extract was liquefied and fractioned by using a separatory funnel and organic solvents having different polarities. The agar diffusion technique was used to assess the antimicrobial potency of extracts and pure compounds against 5 g (+and -) microbial strains. Antimicrobial potency results showed that all extracts and isolated pure compounds provided significant antimicrobial potency against the applied microbial strains. The highest activity chloroform extract was analysed by column chromatography (CC) for the isolation of antimicrobial compounds. The structure of the isolated compounds was characterized based on 1D, 2D NMR and MS spectra. In conclusion, purest compounds might be useful as a remedy for infectious diseases.

The cytokine midkine (MK) is a growth factor that is involved in different physiological processes including tissue repair, inflammation, the development of different types of cancer and the proliferation of endothelial cells. The production of MK by primary human macrophages and monocyte-derived dendritic cells (MDDCs) was never described. We investigated whether MK is produced by primary human monocytes, macrophages and MDDCs and the capacity of macrophages and MDDCs to modulate the proliferation of endothelial cells through MK production. The TLR stimulation of human monocytes, macrophages and MDDCs induced an average of ≈200-fold increase in MK mRNA and the production of an average of 78.2, 62, 179 pg/ml MK by monocytes, macrophages and MDDCs respectively (p < 0.05). MK production was supported by its detection in CD11c+ cells, CLEC4C+ cells and CD68+ cells in biopsies of human tonsils showing reactive lymphoid follicular hyperplasia. JSH-23, which selectively inhibits NF-κB activity, decreased the TLR-induced production of MK in PMBCs, macrophages and MDDCs compared to the control (p < 0.05). The inhibition of MK production by macrophages and MDDCs using anti-MK siRNA decreased the capacity of their supernatants to stimulate the proliferation of endothelial cells (p = 0.01 and 0.04 respectively). This is the first study demonstrating that the cytokine MK is produced by primary human macrophages and MDDCs upon TLR triggering, and that these cells can stimulate endothelial cell proliferation through MK production. Our results also suggest that NF-κB plays a potential role in the production of MK in macrophages and MDDCs upon TLR stimulation. The production of MK by macrophages and MDDCs and the fact that these cells can enhance the proliferation of endothelial cells by producing MK are novel immunological phenomena that have potentially important therapeutic implications.

Brain abnormality causes severe human problems, and thorough screening is necessary to identify the disease. In clinics, bio-image-supported brain abnormality screening is employed mainly because of its investigative accuracy compared with bio-signal (EEG)-based practice. This research aims to develop a reliable disease screening framework for the automatic identification of schizophrenia (SCZ) conditions from brain MRI slices. This scheme consists following phases: (i) MRI slices collection and pre-processing, (ii) implementation of VGG16 to extract deep features (DF), (iii) collection of handcrafted features (HF), (iv) mayfly algorithm-supported optimal feature selection, (v) serial feature concatenation, and (vi) binary classifier execution and validation. The performance of the proposed scheme was independently tested with DF, HF, and concatenated features (DF+HF), and the achieved outcome of this study verifies that the schizophrenia screening accuracy with DF+HF is superior compared with other methods. During this work, 40 patients’ brain MRI images (20 controlled and 20 SCZ class) were considered for the investigation, and the following accuracies were achieved: DF provided >91%, HF obtained >85%, and DF+HF achieved >95%. Therefore, this framework is clinically significant, and in the future, it can be used to inspect actual patients’ brain MRI slices.

Objectives: Breast cancer (BC) is the leading cancer among women. Almost 20% of patients develop brain metastases (BM) and die shortly afterward. There is a dearth of data on the survival outcome of BC patients with BM from the Arab world. Methods: Consecutive women diagnosed with BC who developed radiologically-confirmed BM during their illness were identified through the hospital’s electronic patient’s records. Clinicopathological features and treatment outcomes were recorded. Survival was calculated using the Kaplan-Meier method, and factors affecting survival were studied using log-rank analysis. Results: Between January 2003 and June 2015, a total of 692 patients were treated for BC at our institute. Forty-eight (6.9%) developed BM. The median age at the diagnosis of BM was 45.2 years. More than half of cohort (54.2%) had HER2 positive disease, while 27.1% had the triple-negative disease. The median time interval between the diagnosis of BC and the development of BM was 21 months, and median survival after development of brain disease was seven months. On univariate analysis, pathological grade, previous systemic treatment, brain as the first site of metastases, brain as the only site of metastases, treatment of BM, systemic treatment after BM, and diagnosis-specific graded prognostic assessment (DS-GPA) score significantly affected survival. On multivariate Cox regression analysis, the brain as the first site of metastases, treatment for brain disease, treatment type, and DS-GPA score significantly affected survival post-BM. Conclusions: Our data indicate that Omani women are diagnosed with BC at a younger age, develop BM earlier, and carry a poor outcome.

The Gulf Cooperation Council, despite being the home for the world's largest oil and gas reserves and the highest per capita GDP, the Greenhouse Gas emissions from transportation remain poorly understood, and a missing piece in the puzzle of reduction policies. This paper uses Oman as a Gulf Cooperation Council Countries representative model, with a transport-fuel policy to reveal carbon growth from the transportation sector and to examine feasible paths for carbon-neutral mobility. This study uses IPCC guidelines and methodology of 2006 for the assessment of the GHG emissions trends of transportations modes from 2000 to 2015. The results of the analysis indicating that the share of transport emissions in Oman's total GHG emissions has doubled from 8.1% in 2000 to 16.3% in 2015. Transportation emissions became the third larger contributor to overall greenhouse gas emissions in Oman, after the oil and natural gas (26%) and energy industries (20%). Road transport is the biggest contributor to transport sector emissions, and their contribution to total transport emissions increased from 66% in 2000 to 92% in 2015. Carbon emission from the transportation sector will continue to grow steadily due to the high demand for transportation by a growing population. The cheap availability of fuel and affordable car costs, given the limited public transport, will persist as driving factors of the ongoing rise in carbon emission from transportation. Therefore, curbing emission from the Omani transportation system should take place in the socio-economic context of the country through phasing out the fuel subsidies system, developing a reliable public transport system, planning for the electrification of road transportation, and using rail for freight transportation.

Staphylococcus epidermidis has been recently recognized as an emerging nosocomial pathogen. There are concerns over the increasing virulence potential of this commensal due to the capabilities of transferring mobile genetic elements to Staphylococcus aureus through staphylococcal chromosomal cassette (SCCmec) and the closely related arginine catabolic mobile element (ACME) and the copper and mercury resistance island (COMER). The potential pathogenicity of S. epidermidis, particularly from blood stream infections, has been poorly investigated. In this study, 24 S. epidermidis isolated from blood stream infections from Oman were investigated using whole genome sequence analysis. Core genome phylogenetic trees revealed one third of the isolates belong to the multidrug resistance ST-2. Genomic analysis unraveled a common occurrence of SCCmec type IV and ACME element predominantly type I arranged in a composite island. The genetic composition of ACME was highly variable among isolates of same or different STs. The COMER-like island was absent in all of our isolates. Reduced copper susceptibility was observed among isolates of ST-2 and ACME type I, followed by ACME type V. In conclusion, in this work, we identify a prevalent occurrence of highly variable ACME elements in different hospital STs of S. epidermidis in Oman, thus strongly suggesting the hypothesis that ACME types evolved from closely related STs.

In the leiognathid fish, Leiognathus equulus the diameter of the eye lens can be used for identifying first-year animals, while the eye lens weight proved unreliable for age determination.

Asymmetry analyses have been carried out for some bilateral characters of Callionymus margaretae Regan, 1906 collected from the Arabian Sea coasts of Oman. The results showed that the level of asymmetry of the character postorbital length was the highest among those asymmetry values obtained for the remaining characters studied. The lowest value was that for the head length. An increasing trend in the asymmetry value with fish length is also obtained for preorbital and postorbital lengths. The possible cause of the asymmetry in this species is herein discussed in relation to different pollutants and their presence in the area.
