Mukopolisakkaridozis (MPS) Glikozaminoglikan (GAG) metabolizmasında rol alan enzimlerin eksikliği sonucu oluşan yaklaşık insidansı 3.4-4.5/100.000 olan kalıtsal lizozomal depo hastalığıdır. Farklı enzimlerin eksikliğine göre 7 tip içerisinde sınıflandırılmakla birlikte hastalığın seyri nedeniyle erken yaşlarda ölümler görülebilmektedir. Kulak Burun Boğaz Bulguları bu hastalarda özellikle üst havayolu obstrüksiyonuna neden olan adenoid ve tonsil hipertrofisi, ağız açıklığında kısıtlılık, dil hipertrofisi, efüzyonlu otitis media, işitme, denge, ses ve konuşma problemleridir. Farklı enzim eksikliğinde farklı bulgular ön plana çıkabilmektedir.Çalışmamızda 57 mukopolisakkaridozisli hastanın kulak burun boğaz muayenesi ve odyolojik bulguları, 14 hastanın denge organizasyon testi, 17 hastanın ses verileri incelenmiştir.Çalışmamızda mukopolisakkaridozis hastalarında yapılan odyolojik değerlendirmede %43.8 hastada anormal timpanometrik bulgular tesbit edilmiştir. Ayrıca %14 hastanın saf ses odyometrisinde sensörinöral işitme kaybı, %33.3 hastanın saf ses odyometrisinde iletim tipi işitme kaybı bulguları gözlemlenmiştir. Bu bulguların GAG birikiminin hem iç kulakta birikimiyle sensörinöral işitme kaybı, ayrıca efüzyonlu otitis media ile birlikte iletim tipi işitme kaybına bağlı olabileceği düşünülmüştür. Çalışmamızda 14 mukopolisakkaridozis hastasının bilgisayarlı dinamik postürografi verileri değerlendirilmiştir. Denge organizasyon testinde bileşik denge skorlarına bakıldığı zaman hastaların %69.2'si normal, %30.8'i düşük değerler sergilemiştir. Kulak Burun Boğaz hekimine başvuran Mukopolisakkaridozis hastalarının şikayetleri sağlıklı genel topluma kıyaslandığında çok farklılık göstermemekle beraber (efüzyonlu otitis media, işitme kaybı, üst solunum yolu obstrüksiyonu) bu hastaların çoğunda semptomlar daha erken yaşlarda başlar, daha agresif tedavi, daha sık ve uzun süreli takipler gerektirir.Anahtar kelimeler: Mukopolisakkaridozis, bilgisayarlı dinamik postürografi, işitme kaybı,akustik ses analizi Mucopolysaccharidosis (MPS) is a hereditary lysosomal storage disease, which develops due to a deficiency in enzymes that play a role in glycosaminoglycan (GAG) metabolism with the incidence 3.4-4.5/100.000 of living births.There are 7 subtypes of MPS in which different enzyme types are deficient. In all cases even though progression of clinical findings is not prominent, the disease advances and causes death at early ages.Otorhinolaryngological findings in MPS patients may include tonsil and adenoid hypertrophy, as well as tongue hypertrophy causing upper airway obstruction and sleep apnea, otitis media with effusion, chronic otitis media, hearing and ballance problems and voice disorders.In our study 57 patients with mucopolysaccharidosis were evaluated with their otorhynolaryngologic and audiometric examinations. We also evaluated computerized dynamic posturography findings of 14, perceptual and acoustic voice analyse findings of 17 patients with MPS disease.In our study, abnormal tympanometric findings were found in %43.8 of the patients. In addition, sensorineural hearing loss (SNHL) was observed in %14 and conductive hearing loss (CHL) was observed in %33.3 on pure tone audiogram of the patients. These finding suggest that accumulation of GAGs in inner ear may cause SNHL, as well as in middle ear with otitis media with effusion causing CHL.While complaints of MPS patients who apply to the Ear Nose Throat physician do not differ much when compared to a healthy general population (effusion otitis media, hearing loss, upper respiratory tract obstruction), most of these patients develop symptoms at early ages, require more aggressive treatment, more frequent and longer follow-up and healthcare attention.Keywords: Mucopolysaccharidosis, Computerized Dynamic Posturography, SensoryOrganization Test, hearing loss, acoustic voice analysis 57

Ailevi Akdeniz ateşi (AAA) dünya çapında en yaygın görülen monogenik otoinflamatuvar ve herediter periyodik ateş sendromudur. Genellikle çocuk yaşta başlar ve düzensiz aralıklarla tekrarlayan ve ortalama 0,5-3 gün sürüp kendiliğinden iyileşen ataklarla karakterizedir. Literatürde AAA hastalarında işitmeyi araştırmak için birkaç çalışma yapılmış ve farklı sonuçlar rapor edilmiştir. Bu çalışmada, literatürde ilk defa, AAA hastalarının atak sırasında ve ataksız dönemde işitmelerini değerlendirmeyi amaçladık. 40 pediatrik AAA hastası (80 kulak) ve 21 yaş ve cinsiyet uyumlu sağlıklı çocuk (42 kulak) çalışmaya dahil edildi. Klinik olarak atak semptomları olan hastalardan C-reaktif protein (CRP) ve eritrosit sedimentasyon hızı (ESH) düzeyleri ölçülmek üzere kan örnekleri alındı. Sağlıklı çocuklar ve atak sırasındaki AAA hastalarına saf ses odyometri (250-8000 Hz) ve distorsiyon ürünü otoakustik emisyon (DPOAE) testi (1, 1.4, 2, 2.8, 4 kHz) yapıldı. AAA hastalarına atak arası dönemde testler tekrar yapıldı. Ek olarak tüm AAA hastalarına kontralateral eşikleri ölçmek üzere akustik refleks testi ve on bir hastaya işitsel beyin sapı cevabı (ABR) ölçümü hem atak hem de atak arası dönemde yapıldı. Sonuçlar, yaş, hastalık başlangıç yaşı, kolşisin kullanım süresi, CRP, ESH düzeyleri ve son 6 aydaki atak sayısı ile ilişki açısından karşılaştırıldı. AAA hastalarının, kontrol gurubuyla karşılaştırıldığında, odyogram eşiklerinde solda 250, 500, 1000, 2000, 4000, 8000 Hz'te ve sağda 4000, 8000 Hz'te yükselme saptandı (p< 0,05). Bulgular DPOAE testinde 1, 1.4, 2, 2.8 kHz'teki sinyal-gürültü oranındaki düşüş ile desteklendi (p< 0,05). Atak periyodunda atak arası dönem ile karşılaştırıldığında, odyogramda solda 500, 1000, 8000 Hz ve saf ses ortalaması, sağda 250, 500, 1000 Hz ve saf ses ortalaması eşiklerinde yükselme saptandı (p< 0,05). Fakat DPOAE testinde destekleyici bulgu saptanmadı. Ek olarak, kontralateral akustik refleks testi eşiklerinde ve ABR testlerinde iki grup arasında istatistiksel anlamlı fark saptanmadı. Korelasyon analizinde, kolşisin kullanım süresi, her iki kulakta saf ses ortalaması ve 8000 Hz'teki eşiklerde düşme ile ilişkili bulundu. Tüm paramatrelere dair detaylı istatistiksel analizlere çalışma içerisinde yer verilmiştir. Sonuçlar, odyometre ve objektif bir yöntem olan otoakustik emisyon testleri ile AAA hastalarında koklear tutulum olduğunu ortaya koymaktadır. Bu sonuçlardan yola çıkarak, atak sırasında işitmede objekif akut etkilenim bulunmadığını söyleyebiliriz ve AAA'da işitme etkileniminin hastalık sürecindeki kronik otoinflamasyon durumu nedeniyle olabileceğini düşünmekteyiz. Özellikle işitme kaybını ifade edemeyen okul öncesi yaştaki AAA hastalarının odyolojik takibi, olası işitme kaybını erken dönemde önleyebilir. FMF is the most common monogenic autoinflammatory disease and hereditary periodic fever syndrome worldwide. It usually has a childhood onset and is characterized by recurrent attacks that occur at irregular intervals, which lasts 0.5–3 days on average and resolve spontaneously. Few studies have been made to investigate hearing in FMF patients and there are various results reported in the literature. In this study, first time in the literature, we aimed to evaluate auditory functions in pediatric FMF patients during the attack and attack-free periods. 40 pediatric FMF patients (80 ears) enrolled in the study as study group and 21 (42 ears) sex and age-matched healthy children as a control group. Blood samples were taken from patients that show clinical attack symptoms to evaluate C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) levels. Control group and FMF patients that were in attack period underwent audiometric evaluation including pure tone audiometry (250-8000 Hz) and Distortion Product Otoaoustic Emission (DPOAE)(1, 1.4, 2, 2.8, 4 kHz) test. The tests were repeated in FMF patients in attack-free period. Additionally, acoustic reflex test made for measuring contralateral treshold for all FMF patients and auditory brainstem response (ABR) measured in some, in both attack and attack-free periods. Correlation between hearing results and age, colchicine usage time, age at the onset of disease, CRP, ESR levels and the number of attacks in last 6 months were studied. Hearing thresholds of FMF patients, compared to control group, were found to be increased at fre quencies of 250, 500, 1000, 2000, 4000, 8000 Hz on the left and 4000, 8000 Hz on the right (p< 0,05). This supported by the DPOAE test and signal/noise ratio of children with FMF were lower at frequencies of 1, 1.4, 2, 2.8 kHz (p< 0,05). In the attack period compared to attack-free period, hearing tresholds in the audiogram were found to be increased at frequencies of 500, 1000, 8000 Hz and pure tone average on the left and 250, 500, 1000 Hz and pure tone average on the right (p< 0,05). But this was not supported by the DPOAE test. Additionally, statistically significant difference between attack and attack-free period for contralateral acoustic reflex test tresholds and ABR results were not found. In correlation analysis, duration of colchicin treatment correlated with decreasing of pure tone averages and tresholds in 8000 Hz in both left and right ears. Detailed statistical analysis of all parameters was included in the study. Results demonstrated cochlear involvement in FMF patients with audiometry and objectively with otoacoustic emission test. Regarding to these results, objective acute impairment in hearing in the attack period was not demonstrated, which may suggest that hearing impairment in FMF is likely due to chronic autoinflammation process in the disease. Audiological follow-up of pediatric FMF patients that are not able to express hearing loss, may prevent possible hearing loss in early stages. 121
