Acute hemorrhagic edema of infancy is a skin limited leukocytoclastic vasculitis occurring in children younger than 2 years old. Although it has an alarming start, it has a good prognosis. The patients may have a story of drug intake, vaccination or infection. Henoch-Schönlein purpura must be considered in differential diagnosis. With well recognition of this rare disease, the family and the physician's concern will be prevented; unnecessary investigations and interventions will be reduced. [ABSTRACT FROM AUTHOR]

3. D-Dimer Düzeyinin Akut İnfantil Hemorajik Ödemde Prognostik Önemi.

- Abstract
Acute infantile hemorrhagic edema is characterized by a triad of purpuric rash, fever and edema. The disease has a fulminant onset, a benign course and spontaneous resolution in a couple of weeks. It differs from Henoch-Schönlein purpura with several features such as being seen at less than 2 years of age, size of cutaneous lesion and mostly being limited to skin, rare recurrence and gastrointestinalrenal involvement. Our patient had typical acute infantile hemorrhagic edema symptoms and undetectably high D-dimer levels at the beginning. During the course of treatment, the patient's D-dimer level diminished dramatically and it went back to normal by the end of the therapy. Since this observation has not been reported before, it was considered appropriate to present. It is known that there is no specific therapy to this disease. The use of antihistaminics and systemic corticosteroids are debatable and they were reported not to change the course of disease. However, our patient responded well to systemic corticosteroid therapy, and this is seen in decreased D-dimer level. We believe that D-dimer level may be a follow-up marker for the severity of the disease and response to the therapy. [ABSTRACT FROM AUTHOR]

Acute infantile hemorrhagic edema is an unusual form of leukocytoclastic vasculitis that usually occurs in children younger than three years of age. The disease is self-limiting and has a benign clinical course. In this report, two cases who were admitted to our hospital with cutaneous purpuric rash and edema in the extremities and subsequently diagnosed as acute infantile hemorrhagic edema are presented. We emphasize that acute infantile hemorrhagic edema should be kept in mind in the differential diagnosis of Henoch-Schonlein purpura, meningococcemia, and purpura fulminans. [ABSTRACT FROM AUTHOR]

Akut infantil hemorajik ödem 2 yaş altı çocuklarda görülen genellikle deride sınırlı bir lökositoklastik vaskülittir. Gürültülü başlangıcına rağmen selim seyirli bir hastalıktır. Daha öncesinde geçirilmiş enfeksiyon, aşılanma, ilaç kullanım öyküsü olabilir. Henoch-Schönlein purpurası ayırıcı tanıda mutlaka göz önünde bulundurulmalıdır. Akut infantil hemorajik ödem purpurik döküntü, ateş ve ödem triadı ile karakterizedir. Hastalık; gürültülü bir başlangıca, selim bir seyre ve birkaç haftada kendiliğinden iyileşebilen bir karaktere sahiptir. İki yaşından küçük çocuklarda görülmesi, kutanöz lezyonların büyüklüğü ve çoğunlukla deriye sınırlı olması, gastrointestinal ve renal tutulumun genellikle olmaması, tekrarlamanın çok seyrek görülmesi Henöch-Schönlein purpuradan ayırt ettirici özellikleridir. Hastamızda tipik akut infantil hemorajik ödem bulgularının görülmesinin yanında başlangıçta D-dimer seviyelerinin ölçülemeyecek kadar yüksek saptanması dikkatimizi çekti. Tedavinin seyri sırasında klinik iyileşmeyle paralel giden D-dimer seviyesinde belirgin düşme ve tedavinin sonunda normal düzeye dönme; şimdiye kadar bildirilmemiş olduğundan sunulması uygun görüldü. Hastalığın bilindiği gibi özgün tedavisi bulunmamaktadır. Antihistaminikler ve sistemik kortikosteroidlerin kullanımı tartışmalı ve hastalığın seyrini değiştirmediği bildirilmiştir. Fakat hastamız özellikle sistemik kortikosteroidlere iyi cevap vermiş olup, bu cevap D-dimer düzeyi ile ilişkili görünmektedir. Akut infantil hemorajik ödem, süt çocukluğu döneminde görülebilen ve klinik olarak ateş, deride palpabl purpura ve ödem ile karakterize akut kütanöz lökositoklastik vaskülittir. Hastalığın başlangıcı oldukça gürültülü bir seyir izlemesine karşın, klinik gidiş benign karakterde olup kısa sürede tamamen iyileşmektedir. Bu çalışmada; deride purpurik döküntü ve ekstremitelerde ödem yakınmaları ile başvuran, klinik ve laboratuar incelemeleri sonucunda akut infantil hemorajik ödem tanısı almış 3 olgu sunulmuştur. Klinik izlem ve tedavi açısından farklı yaklaşımlar gerektiren özellikle Henoch-Schönlen purpurası, meningokoksemi, purpura fulminans gibi hastalıklarla ayırıcı tanıda akut infantil hemorajik ödemin de düşünülmesi gerektiğini vurgulamak istedik. Acute infantile hemorrhagic edema is an acute cutaneus leucocytoclastic vasculitis that can be seen in infancy and characterized by fever, palpable purpura and edema. Although it presents with severe symptoms; clinical course is benign and the disease resolves in a short time. In this report, three cases are presented that were admitted with cutaneous purpuric rash and edema of the extremities and, subsequently diagnosed as acute infantile hemorrhagic edema. We emphasize that acute infantile hemorrhagic edema should be kept in mind in the differential diagnosis of Henoch-Schonlein purpura, meningococcemia, and purpura fulminans.
