9. Surgical treatment of displaced radial neck fractures in children with metaizeau technique: Late-term results Metaizeau technique : Late term results

Aim: in this study, we aimed to analyze the functional and radiological outcomes of displaced radial neck fractures in children treated with the Metaizeau technique . Material and Methods: This was a retrospective study, and data were collected prospectively. Thirty-eight patients with grade III and grade IV radial neck fractures according to the classification of Judet et al were managed surgically by Metaizeau technique. All patients were surgically treated with percutaneous k-wire leverage reduction and retrograde transphyseal k-wire fixation. The functional results were evaluated using the Metaizeau functional scoring system. Clinical evaluation and results were performed using the Mayo elbow performance score (MEPS), ROM of the elbow (flexion, extension) and forearm (supination, pronation) was measured with a goniometer. Complications were also evaluated. Results: The average follow-up time was 64.9 months (28 -120 months), there were no patients with nonunion, avascular necrosis, infection, posterior interosseous nerve (PIN) injury, heterotopic ossification or radioulnar synostosis. There were 31 (88.5 %) excellent or good results and 4 (11.5 %) fair results according to the Metaizeau classification. The final X-rays showed fracture healing in all patients. No patient developed complications. Discussion: Intramedullary pinning, as described by Metaizeau, is a reliable and reproducible surgical method to treat radial neck fractures in children, which provides excellent or good results and has a low risk of complications.

Between 1982 and 1989 in Department of Orthopaedic Surgery and Traumatology Clinic in Ege University, fourteen consecutive patients underwent hip arthrodesis with fıxation by a cobra head compression plate. The mean age was 24 years, the youngest patient was 19 years old and the oldest patient was 62 years old. In this series 7 hip arthrodesis (%50) were performed for tuberculosis of the hip. The results were examined 5 months-7 years (average 3.5 years) following the last operation in 3 female and 9 male patients. All the hips treated in this study had successful bone fusion at the postoperative 4th month. All of the patients were out of bed within the first postoperative week and ambulation with crutches was begun after the second week. We came to conclusion that hip arthrodesis still has a justifiable place in the operative treatment of hip dislocation., 1982 ile 1989 yılları arasında E.Ü.T.F Ortopedi ve Ttavmatoloji Ana Bilim Dalında uygun endikasyon gösteren ondört olguda Kobra kompresyon plağı ile kalça artrodezi uygulandı. Ortalama yaşı 24 (Min: 19, Mak: 62) olan olguların 3'ü kadın 11'i erkekti. Artrodez nedeni 7 olguda (%50) tüberküloz idi. Ortalama 3.5 (Min: 5 ay, Mak: 7 yıl) yıl aradan sonra tekrar değerlendirilen olgularda ortalama 4. ayda kaynama gerçekleştiği görüldü. Hastaların tümü birinci postoperatif haftada ayağa kaldırılarak ikinci postoperatif haftada koltuk değneği ile yürütüldü. Genelde çok tartışmalı bir girişim olan kalça artrodezinin ortopedik cerrahide hala kullanılması gerektiği durumlarda Kobra kompresyon plağı ile alınan sonuçlar yeterli olmaktadır.

In this experimental study, the role of citric acid which was marked with Tc 99m has been evaluated by scintigraphic techniques, in fracture healing of experimental model which was formed in the rabbit tibia. Citric acid has an important effect on the mineral metabolism of bone cell. However its role is unknown. No scientific study has been found about this subject until today Fractures were formed in 32 mafe rabbits with transverse osteotomy in the Ievel of right tibio-fibular junction. Two different types of groups were formed by two different types of intramedullary nailing for the reason of forming the primary and secondary healing. The stages of fracture healing were followed scintigraphically. At the first group on the 14th day average (L/K) ratio was 26, then it increased to 4.0 on 29th day. Then, by 62nd day,a slight decrease to 30 from 62nd day, to 105th day, it decreased sharply to 7.5. Within 6 months, a clear straight Iine was seen in the second group, on the 10th day, the average (L/K) ratio that was postoperatively made a peak 15 and reaching to 26th; by an evident decrea, it reached to 3.5. Then its decrease continued and by the 43th day, it reached 2.0 but 59th, Iittle increase was seen Iike a value of 2.5. This increase came down under the vaule of 2.0 on the 97st day and showed a continuity by a straight Iine up to 6th month. In the general evaluation of the two groups, the citric acid in fracture healing was hold on the high values. According to our observations, the remodelation period Iasts Ionger than radiologic appearances at the molecular Ievel. In this Iong time period, holding of citric acid continues even if it gets Iess. These observations showed that, citric acid had an important role in fracture healing and it also showed that it could be used as an agent in scintigraphic evaluations as a specific substance to the bone., Bu deneysel çalışmada, tavşan tibiasında oluşturulan deneysel modeldeki kırık iyileşmesinde, Tc99m ile işaretlenen sitrik asidin davranışı sintigrafik olarak değerlendirilmiştir. Sitrik asidin kemik hücre metabolizmasında önemli bir etkisi vardır. Ancak nasıl bir rol oynadığı bilinmemektedir. Bu konuda bugüne kadar yapılmış bir çalışma bulunmamaktadır. Otuziki tavşanda sağ tibio-fibular bileşke düzeyinde transvers osteotomi ile kırık oluşturuldu. Primer ve sekonder kallus oluşumu açısından iki ayn tipte intramedüller çivileme ile iki ayrı grup oluşturuldu. Kırık iyileşmesi evreleri sintigrafik olarak izlendi. Birinci grupta, 14. günde ortalama (L/K) oram 26 iken, 29 günde pik yaparak 4.0'a ulaştı. Daha sonra 62 güne doğru, 3.0'a hafif bir eğri şeklinde düşüş gözIendi. 62 günden 105 güne doğru keskin bir düşüş ile 1.5 değerine inildi. Altı aylık izlemede belirgin bir düz çizgi izlendi ikinci grupta ortalama (L/K) oranı, postoperatif 10. günde 7.5 değerinde pik yaparak, 26 günlere doğru belirgin bir düşüş ile 3.5'e ulaşılmıştır. Daha sonra devam eden düşüş 43 güne doğru 20 değerine ulaşmış ancak 59. günlerde 2.5'Iik değerle hafif bir artış gözlenmiştir. Bu artış 91. günde 20 değerinin biraz altına düşerek 6. aya kadar uzun bir süreçte düz bir çizgi olarak devamlılık göstermiştir. Her iki grubun ortak değerlendirilmesinde, kırık iyileşmesinde sitrik asit, yüksek değerlerde tutuluş göstermiştir. Bulgularımıza göre remodelasyon dönemi radyolojik görünümden daha uzun süre moleküler düzeyde sürmektedir. Bu uzun süreli dönemde sitrik asit tutuluşu azalarak da olsa sürmektedir. Bu bulgular sitrik asidin kemik iyileşmesinde önemli bir rol oynadığı ve kemiğe spesifik bir madde olarak sintigrafik değerlendirmelerde kullanılabilecek bir ajan olabileceğini göstermiştir.

20. Travmatik torako lomber vertebra kırıkları

- Abstract
From 1986 to 1988, 171 patients with thoraco-lumbar vertebrae fructures were admitted to the Department of Orthopaedic Surgery and Traumatology clinic of Ege University. The stable fractures of 108 patients were treated with conservative methods, with a postural reduction, body cast and total contact plastic thoracolumbosacral orthosis. Unstable fractures of 63 patients underwent early surgical reduction and external fixation with double Harrington instrumentation.The decrease in angle of kyphosis were more significant in surgical cases than the conservative.The difference in decreasing of scoliosis was not important between surgical and conservative treatmentThe follow-up vertebral index was found similar in both of methodsThere were no significant neuorological recovery between two methods. The incidence of complications were higher in surgical cases. The reason. The reason is mostly the surgical cases were injured with more severe trauma., Travmatik vertebra kırığı olan 171 hasta 1986 ile 1988 yılları arasında kliniğimize başvurmuştur. Olguların 108'inde postural redüksiyon, gövde alçısı, polietilen ortez veya korse ile konservatif, 63 olgu ise açık redüksiyon ve Harrington rodu ile stabilize edilmiştir. Başvuru esnasında yapılan radyolojik tetkik ve klinik bulgulara göre stabilite saptanmıştır. Çoğunlukla stabil ve nörolojik lezyonu olmayanlar konservatif, instabil veya nörolojik lezyonu olguyarda ise cerrahi tedavi uygulanmıştır. Ortalama takip süresi 12 aydır (min.:4, mak.:26). Konservatif tedavide kifoz 0.22 , skolyoz 1.8, cerrahi tedavide ise kifoz 9.84, skolyoz 2 düzelmiştir. En sık gelişen komplikasyon üriner enfeksiyon olup 57 (yüzde 33.3) olguda görülmüştür.

21. Rijid ve stabil elastik intramedüller çivilemenin kallus oluşumuna etkisi

In this experimental study, for the aim to the treatment of fresh fractures, the effect of rigid or stable elastic intramedullary nailing models were investigated on the callus formation. Open transverse osteotomy was perlormed at the right tibiae of 42 male albino rabbits. In the 1. Groups rabbits (n:21 ), rigid (Küntscher like), in the 2. Group rabbits (n:21) elastic stable intramedullary nailing (Ender Iike) were perlormed During the period of fracture healing in the stages of chondrogenesis, enchondrogenesis, and remodelling; scintigraphic and radiographic evaluations were made. Quantitative technetium scintigraphy was used to measure regional blood flow within the tibiae during consolidation, up to 16 weeks after operation. The ROI values of the area of bone fracture and the same area at the opposite tibiae were compared In the 1. group, in 29-35 days; in the 2 groups in 21-29 days, L/C rates showed a significant increase. In this in vivo study, a relation couldn't be found between scintigraphic and radiological findings. This point has a clinical importance. At the results of general evaluation of experimental values, secondary callus which was Iormed in the example of elastic stable inlramedullary nailing shows Iaster and short period of remodelation in the scintigraphic observation. Contrary to the radiologic findings, remodeiation happens faster in the secondary fracture healing. Prolonging of the activity in the primary fracture healing shows a Iate reorganization of the medullary callus., Bu deneysel çalışmada, taze kırıkların tedavisi amacıyla uygulanmakta olan rijid ve stabil elastik intramedüller çivileme modellerinin kallus oluşumuna etkisi araştırıldı. Kırkiki adet beyaz erkek tavşanın sağ tibialarına subperiostael transvers osteotomi uygulandı. Birlncl grup (n:21) tavşanlarda rijid (Küntscher gibi), 2 grup (n:21) tavşanlarda stabil elastik (Ender gibi) çivileme gerçekleştirildi. Kırık iyileşme sürecinde; kondrojenezis, enkondrojenezis ile remodelasyon dönemlerinde klinik, sintigralik ve radyolojik değerlendirme yapıldı. Deneye postoperatil 4 ayda son verildi. Yedi ve on günlük aralar ile Tc99m MDP kemik sintigrafisi uygulandı. Kırık odak çevresi (L) ile karşı taraf tibiasında aynı bölgeden (K) çizilen ilgi alanlarından elde edilen değerler kıyasIandı. L/K oranları 1 grupta 29-35 günlerde, 2. grupta 21-29. günlerde belirgin bir artış gösterdi. Deney verilerinin genel incelemesi sonucunda stabil elastik intramedüller çivileme örneğinde oluşan sekonder kallus sintigrafik izlemede daha hızlı ve kısa süreli bir remodelasyon göstermektedir. Radyolojik bulguların tersine remodelasyon sekonder kırık iyileşmesinde daha hızlı olmaktadır. Primer kırık iyileşmesinde aktivitenin daha uzun süreli olması medüller kallusun geç reorganizasyonunu göstermektedir.

22. Olekranon kırıklarının cerrahi tedavisi

From 1980 to 1987, forty two olecranon fractutes were treated at the Ege University Orthopaedic Surgery and Traumatology Clinic. Thirty eight patients underwent surgical treatment with various technique. Vasey Weber tension-band wiring technique was performed in thirty five cases. The average age was 28 years. The average hospitalization time was eight days and operation time in third days following the administration. The average follow up of sixteen patients were 3 years. The results were evaluated according to Wolfgang score: 11 excellent, 3 god and 2 fair. Symptomatic metal prominence were seen in three patients. One of them had painful bursitis. All fractures healed and Vasey-Weber tension-band wiring technique was found easy to use, effective and economic., 1980 ile 1987 yılları arasında E.Ü. Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalına başvuran 42 olekranon kırığının 38’i cerrahi olarak tedavi edilmiştir. 35 olguda cerrahi tedavi Weber-Vasey tansiyon band tekniği ile olmuştur. Hastaların ortalama yaşı 28’dir. Cerrahi olarak tedavi görenlerin hastahanede kalış süresi 8 gün olup ortalama 3. günde ameliyatları yapılmıştır. Son kontrole gelen 16 hastanın ortalama takip süresi 3 yıldır. Wolfgang ve ark larının değerlendirme skoruna göre, 11 çok iyi, 3 iyi, 2 orta sonuç alınmıştır. 3 hastada komplikasyon olarak Kirschner teli migrasyonu görülmüştür. Bütün hastaların kırıkları sorunsuz olarak kaynamış olup tansiyon band tekniği, kullanışı kolay, etkili ve ekonomik bulunmuştur.

23. Sorunlu femur kırıklarının İlizarov eksternal fiksatorü ile tedavisi

Femur fractures below endoprostheses or upper borderline of the hip of the angular plate after treatment of a supracondylar fracture of femur can be catastrophic occurence. These type of rare fracture were treated by Ilizarov's method of percutaneous transosseous osteosynthesis, between May 1993 and September 1993, at the Department of Orthopaedic Surgery and Traumatology Clinic in Ege University. The object of our treatment with the Ilizarov fixator in addition to the anatomical reconstruction is the mobility of the adjacent joints and an immediate ambulatory function with partial and even complete weight bearing. The stategy of our management is lead by the basic ideas reduction and fixation. Our average operating time is 1 hour 30 min. For one or two days, the period of pain and edema, the patient will have mobilising exercises in bed. Afterwards he will begin walking with weight bearing of 15 kg and resume quickly the full weight bearing. The first check X-ray will be done the third postoperative day. The average time of the treatment with Ilizarov apparatus lasted 17 weeks. The results proved that this type of treatment can be an alternative method for these type of fractures., Biri total kalça protezi, diğeri suprakondiler femur kırığı sonrası uygulanan plaklamadan sonra implantlar ile sağlam kemik sınırında femur kırığı oluşan iki olguda skopi kontrolunda kapalı redüksiyon ve İlizarov eksternal fiksatörü ile transossöz osteosentez uygulandı. İzleme süreleri sırası ile 24, 25 hafta olan olgularda 16 ile 18 haftalarda kemik iyieşmesi ve kemik öncesi fonksiyonel duruma dönüş elde edildi. Alınan sonuçlar kemik iyileşmesi ve fonksiyonel açılardan yeterli oldu. Bu tip yaralanmalarda kemik odağı açılmadan kapalı redüksiyon ile uygulanan İlizarov eksternal fiksatorunun bir tedavi seçeneği oluşturduğu saptandı.

Subtrochanteric fractures especially comminuted ones have posed a challenge to orthopaedic surgeons. From February 1990 to September 1993, in Department of Orthopaedic Surgery and Traumatology Clinic of Ege University, 9 (3 female, 6 male) cases with subtrochanteric femur fractures were treated with a Gamma nail. The incidence of the types of fractures, 3 (33%} 2A, 2 (22%} 2B, 4 (44%) 3A, based on the Seinsheimer's classification. The average age was 36 (min. 20, max. 58). The average follow up of cases were 1 0 (min. 5, max. 34} months. AII of the fractures healed and there was a good range of hip movements, there was one (11%) malunion. Using the Postel-Merle d'Aubigne criteria, 4 (44%) excellent, 4 (44%) good and 1 (11%} fair results were found. AII patients who were ambulatory before their fracture were able to bear weight. The Gamma nail allowed immediate weight-bearing and provided a safe fracture healing in a good position., Şubat 1990-Eylül 1993 arasında, Ege Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalı'nda 9 (3 kadın, 6 erkek) subtrokanterik femur kırıklı olgu intrameduller Gamma çivisi ile tedavi edildi. Sheinsheim sımflandırmasına göre kırık dağılımı: 3 (%33} 2A, 2 (%22) 2B, 4 (%44) 3A oldu. Olguların 6 (%67}'sında açık redüksiyon ile intrameduller çivileme yapıldı. Yaş ortalaması 36 ( min. 20, mak. 58) olan olgular ortalama 10 (min. 5, mak. 34) ay izlendi. Kırıkların tümü iyileşti. Kalça hareket genişliği ve Postel-Merle d’aubigne kriterlerine göre olguların 4 (%044}'ünde çok iyi, 4 (%044)'ünde iyi, 1 (%11 )'inde orta sonuç alındı. Kırık yerindeki parçalanmaya bakılmaksızın elde edilen stabil osteosentez ile erken dönemde yüklenmeye başIanabilmesi; Gamma çivisi ile osteosentezin bu tip kırıkların tedavisinde etkin bir yöntem olduğunu göstermektedir.

Between 1981 and 1988, 314 tibials shaft fractures were treated in Orthopaedic and Trumatology Department of Ege University, Medical Faculty. 46 of them underwent closed intramedullary nailing with reaming. Infection was observed in one patient with decubitis problem, when early operation was performed mandatory. The period of healing according to our evaluation in between the date of the operation and the time when the patients becomes ambulatory. The average time to heaIing was 20 weeks. Rotation and angulation deformity, malunion and nonunion were not observed in the 21 patients who were evaluated in for the last control. There is no universally accepted method of management of tibial shaft fractures. Closed intramedullary tibial nailing with reaming incorporated many advantages of nonoperative therapy with increased stability of intemal fıxation. Değerlendirmemizdeki kırık iyileşmesi, ameliyat tarihi ile tam desteksiz yüklenme arasındaki süredir. Bu ortalama 20 hafta olarak bulunmuştur. Son kontrol çağrısına gelen 21 olguda klinik ve radyolojik değerlendirmede rotasyon kusuru ve kaynamama görülmedi. Tibia cisim kırıklarında, halen değişik tedavi metodları kullanılmaktadır. Uygun endikasyon konuları olgularda, kapalı intramedüller Küntscher tekniği ile tedavi seçkin bir yöntemdir.

During the period from August 1990 to August 1992, 22 (12 female, 10 male) patients with comminuted, displaced, intra-articular fractures of the distal radius were admitted to the Department of Orthopaedic Surgery and Traumatolog Clinical of Ege University. They were treated by external fixator. This study compares these consecutive cases with Melone type 1 and 2 fractures of the distal radius treatedby Dynafix with 1 0 (% 45) (Group 1 ) patients who sustained similar fractures and 12 (55%) who were treated by the small AO external fixator. Five (23%) patients had open reduction and internal fixation wiht T plate in addition to external fixation. The average follow-up was 18 (min. 12, max. 27) months. Demerit points system used to evaluate end results. it was found that 14 (64%) patients had excellent and good results. Eight (36%) patients had fair and poor results. One (5%) patient had pin-tract infection that was resolved after removal of the pins. Six (27%) patients developed reflex sympathetic dystrophy and needed physiotheraphy for a Iong period of time. The overall good results suggest that maintenance of good anatomical reduction is more important than the type of external fixator in deciding the final outcome., Ağustos 1990 ile Ağustos 1992 arasında Ege Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalı'na başvuran 22 (12 bayan, 10 erkek) radius distal uç parçalı eklem içi kırıklı olgunun 10 (% 45)' unda (Grup 1 ) Dynafix, 12 ( % 55) , sinde (Grup 2) mini AO eksternal fiksatör uygulandı. Bu çalışmanın amacı, seçilen iki ayn tip eksternal fiksatör ile alınan sonuçları karşılaştırmaktır. Değerlendirmede Demerit sistemi kullanıldı Ondört (% 64) olguda çok iyi ve iyi, 8 (% 36) olguda orta ve yetersiz soruç alındı. Bir (% 5) olguda çivi yolu enfeksiyonu, altı (% 28) olguda Sudeck atrofisi gelişti. Alınan iyi sonuçlarda, eklem yüzeyinin iyi anatomik redüksiyonunun belirleyici olduğu görüldü.

27. Çocuk femur cisim kırıklarında elastik intramedüller çivileme

Between August 1990 and December 1991, at the Department of Orthopaedic Surgery and Traumatology Clinic in Ege University, 16 children with 17 femoral shaft fractures were treated with closed elastic stable intramedullary nailing. The ages ranged from 7 to 13 years, with a median age of 10 years. The average postoperative hospitalization was 3 (min 2, max 4) days We reviewed all the cases after an average follow-up 10 (min 6, max. 17) months All patients with an isolated simple fracture became independent using crutches or walkers within 2 weeks of initial treatmeni Complications were minimal, the most common being nail protrusion caused by the distal end of the nail. There were 5 (29%) cases of local tenderness due to nail protrusion. All patients regained full range of motion within 3-4 weeks after the nails was removed. Conpared with conservative treatmeni this method has a marked advantage in allowing increased patients function and independence during the healing phase. This technique should be considered as an effective alternative treatment for femoral shaft fractures in children., Ağustos 1990-Aralık 1991 arasında Ege Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalı'na başvuran 17 femur cisim kırıklı 16 çocuk, kapalı redüksiyon ve elastik intramedüller çivileme ile tedavi edildi. Yaş ortalaması 10 (min. 7, mak. 13) olan olgulara ortalama 10 ay (min. 6, mak. 17) izlendi. Cerrahi girişim sonrası hastanede kalış ortalama 3 gün (min 2, mak 4) oldu. Olgular postoperatif 2. haftada bir çift koltuk değneği ile yürütüldü. Ortalama 10. haftada tam yüklenmeye başlandı. Distal çivi uçlarının cilt altında oluşturduğu lokal duyarlılık (%29) dışında ciddi bir komplikasyon görülmedi. Ortalama 4. ayda (min. 3, mak. 7) çiviler çıkartıldı. Çiviler çıkarıldıktan 3-4 hafta sonra diz hareketleri tamamen düzeldi. Konservatif tedavi yöntemleri ile kıyaslandığında özellikle okul çağı çocuklarda kısa süreli bir immobilizasyon sonrası erken yürümeye izin vererek sorunsuz kırık iyileşmesini sağlaması yöntemin üstünlüğü olmaktadır.

At the 21 cases of below knee amputations performed by various reasons at the department of Orthopaedic Surgery and Traumatology of Ege University between August 1988 and December 1989, the effects of immediate temporary prosthesis and elastic bandage, plus the effects of both methods in reducing the edema was studied. Volume measurements were taken of 12 cases on 4th or 7th days where postoperative immediate temporery fıtting (prosthesis) could not be performed. Because of infection suspect and again volume measurement were taken in the remaıned 9 cases on the 14th and 15th days. Later within a week different 3 more measurement were taken. The difference between the 1st and 4th measurement was taken. It was seen that in the immediate temporary prosthesis a 5% to 8%, and in the elastic bandage application a 10% to 15% reduction was present. İt was also noted the reduction of edema stopped in 2 weeks at the cases where immediate temporary prothesis was used and in 3 weeks in cases where elastic bandage was used., Diz altı amputasyon güdüğünün rehabilitasyonunda amaç erken protez uygulanması ve diz ekleminde fleksiyon kontraktürünün önlenmesidir. Protez rehabilitasyonuna başlayabilmek için güdük yarası iyileşmeli ve güdük kitlesinin hacmi stabilleşmelidir. Bu nedenle amputasyon sonrası ortaya çıkan ödem önemli bir sorundur. Çalışmamızda postoperatif erken geçici protez ve elastik bandaj uygulanan olguların güdük ödemlerindeki değişiklikler yüzde hacim olarak ölçülmüş ve zaman içindeki azalma karşılaştırılmıştır, Erken geçici protez uygulananlarda %5-8'lik, elastik bandaj uygulananlarda %10-15'lik azalma istatiksel olarak % 95 anlamlı bulunmuştur. Uygun koşullarda erken geçici protez ideal tedavi şekli olmakla birlikte rezidüal güdük ödemini azaltmakta her iki yöntemde etkili olmaktadır.

Between January 1990 and September 1993, 15 patients with 8 fractures of the humeral shaft and 7 dispIaced fractures of the proximal humerus were treated with Seidel's Iocking nail system. The system advocated by Neer and AO were used to classify the fractures. The humeral shaft fractures consisted of 2 spiral and multifragmentary fractures, 2 nonunions and one distal fractures. The displaced proximal humeral fractures could be classified in 4 two-part and 3 three or four part fractures. The average age at operation was 49 (min. 20, max. 65) years. The follow-up ranged up to 18 (min. 4, max. 30) months. The clinical and radioraphic results of patients treated by Seidel nail were reviewed. The clinical results were assessed using Neer's protocol. At the follow-up examination 5 patients reported persisting shoulder disablity. The overall results were good with no cases of pseudarthrosis, infection or radial nerve palsy. AlI the patients regained full shoulder movements with no evidence of rotator cuff Iesions. According to a clinical rating, 6 (40%) patients had an excellent, 5 (33%) good, 4 (27%) fair, while no poor results was seen. A very reliable and succesfull technique in the treatment of diaphyseal and displaced fractures of the proximal humerus., Ocak 1990 ile Eylül1993 tarihleri arasında Ege Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji AnabiIim Dalı'na başvuran yaş ortalaması 49 (min. 20, mak. 65) olan 7 humerus üst uç (1. Grup) ve 8 cisim kırıklı (2. Grup), 15 (9 bayan, 6 erkek) olgu kilitli intramedüller Seidel çivisi ile tedavi edildi. Humerus üst uç kırıklı olgularda Neer sınıflandırmasına göre kırık dağıltmı 1 (% 14) Tip 2a, 4 (%57) Tip 3a, 2 (0/028) Tip 2b; cisim kınklt olgularda AO sınıflandırmasına göre kırık dağılımı 2 (%25) AI, 2 (%25) A3, 1 (%12.5) BI, 2 (%25) B2, 1 (% 12.5) CI olmuştur. Ortalama 16 (min. 5, mak. 48) ay izlenen olgularda radyolojik kırık iyileşmesi, fonksiyonel olarak omuz ve dirsek hareket genişliği değerlendirildi. Humerus üst uç kırıklı olguların tümünde, cisim kırıklı olguların 6 (%86)'sında tam kırık iyileşmesi elde edildi. Neer skoruna göre 1. Grupta; 3 (%43) çokiyi, 2 (%29) iyi, 2. Grupta 3 (%38) çokiyi, 3 (%38) iyi sonuç alındı.

The femoral neck fractures remain one of the unsolved fractures. it is a fracture with a high incidence of nonunion and avascular necrosis. Between January 1985 and December 1990 in Department of Orthopaedic and Traumatology Clinic in Ege University, forty-six (32 male, 14 female) mostly young adult patients with femoral neck fractures were treated by the Richards hip screw. In twenty-nine cases, the Richards screw were supported by short threat AO 6.5 mm cancellous screws. Thirty-nine of the patients were observed for an average of 37 (Min, 14, Max. 47) months. Twelve of Group 1 and 27 of Group 2 were reevaluated. The results obtained by using only Richards compression screw (Group 1 ) and the ones supported by canceflous screw (Group 2) were compared. The early complications, the beginning of rehabilitation, early weight bearing, duration of healing, the rotation problems of the fracture area were better in Group 2. The incidence of nonunion was %25 in Group 1. and %14 in Group 2. There was no significant difference in avascular necrosis rates between groups. it was observed that when canceflous screws was used in Garde.n 3. and 4. displaced fractures, the stability was increased in the fractured area., Ocak 1985-Aralık 1990 arasında Ege Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalı'nda yaş ortalaması 32 (Min. 20, Mak. 59) olan 46 (32 erkek, 14 bayan) femur boyun kırıklı olguya dinamik Richards kompresyon çivisi uygulandı. 29 olguda (%63) Richards kompresyon çivisi kısa yivli AO 6.5 mm spongioz vida ile desteklendi. Ortalama 37 (Min. 14. Mak. 47) ay izlenen 39 (12 olgu, Grup 1.,27 olgu Grup 2) yeniden değerlendirildi. Tek başına Richards kompresyon çivisi (Grup 1), spongioz vida ile desteklenen şekli (Grup 2) ile alınan sonuçlar kıyaslandı. Erken gelişen komplikasyonlar, rehabilitasyona başlama zamanı, iyileşme süreci, kırık yerinde rotasyon sorunu, varusa kayma ve nonunion gelişimi açısından Grup 2'de alınan sonuçlar daha iyi oldu. Nonunion görülme sıklığı Grup I'de %25, Grup 2'de %14 olup değişik faktörlerden etkilenen avasküler nekroz oluşumu iki grupta da belirgin bir farklılık göstermedi. Özellikle Garden 3. ve 4. tip kırıklarda eklenen spongioz vidanın kırık yerinde stabiliteyi artırdığı gözlendi.

36. Treatment of Displaced Pylon Fractures with Circular External Fixators of Ilizarov

- Abstract
Twenty cases of pylon fractures were treated with Ilizarov circular external fixators using the technique of ligamentotaxis. Fourteen men and six women, ranging in age from 20 to 59 years, with open or closed distal tibial fractures were included in this study. The fractures were classified according to the AO system and distributed as seven C1, eight C2, and five C3 fractures. Independent from the type of fracture, six were grade II open according to the Gustilo-Anderson classification. For all fractures, an external fixator was constructed, after reduction by traction and olive wires. Mean follow-up was 29 months. All fractures healed by 14 weeks postreduction. The results were assessed using Weber's criteria: 5 (25%) had excellent results, 10 (50%) good, and 5 (25%) poor results.This technique lowers the rated complications of open reduction and internal fixation, and allows restoration of joint-surfaces, reconstruction of length, and alignment of the extremity while maintaining a sufficient range of motion at the joint. We concluded that use of the Ilizarov circular external fixator enables good results in selected ankle traumas when applied with good indications, planning, and surgical experience.

Background: It was aimed to compare conventional surgery and three-dimensional (3D) model-assisted surgery used in the treatment of calcaneal fractures.Materials& Methods: A total of 37 patients with unilateral calcaneal fractures were randomly divided into two groups as a conventional surgery group (n: 19) and a 3D model-assisted surgery group (n: 18). The preoperative, postoperative and last follow up angles of the Bohler and Gissane, calcaneal width and facet height were measured. The duration of the operation, blood loss volume, fluoroscopy usage, instrumentation time for both groups were recorded. Finally, the follow-up AOFAS scores were evaluated. A questionnaire was used to determine the perceptions of the resident doctors about the 3D model.Results: The duration of the operation, blood loss volume, fluoroscopy usage, instrumentation time for 3D model-assisted surgery group were 83.3 ± 4.6 minutes, 83.6 ± 4.6 ml, 6.8 ± 1.4 times and 13.0 ± 0.8 weeks, and as for conventional group they were 130.0 ± 5.8 minutes, 105.1 ± 5.6 minutes, 11.7 ± 1.5 ml, 22.2 ± 2.4 times and 13.3 ± 0.8 weeks, respectively (p < 0.0001). The both groups significantly restored Bohler angle, Gissane angle, calcaneal width and calcaneal facet height after operation (p < 0.0001). The 3D model-assisted group was significantly more succesful in restoration and protection of achieved correction of calcanel facet height (p < 0.0001). The difference was determined among the groups at the final follow-up examination with respect to the amount of change according the values achieved post-op. were significant in Bohler angle (p < 0.001), calcaneal facet height (p < 0.0001) and calcaneal widht (p = 0.017). There was no significant difference between AOFAS scores of the two groups at last follow-up. Resident doctors exhibited high scores of overall satisfaction with the use of a 3D printing model.Conclusions: Compared to the conventional group, the 3D model-assisted group provide successful intervention and reduce operation, instrumentation time and the fluoroscopy usage with less blood loss. Performing 3D-assisted surgery helps the quality of reduction during the surgery and stability of internal fixation to protect achieved reduction at follow-up more succesfully. Objective: Cephalomedullary nails (CMN) are implants with a high success rate in the surgical treatment of trochanteric fractures. The aim of this study is to compare the radiological outcomes and mechanical complications of femoral trochanteric fractures treated with three different CMNs., Methods: Intertrochanteric fractures in patients aged 50 years and older treated with CMN between January 2016 and December 2021 were reviewed retrospectively. A total of 158 cases meeting the criteria were included to final analysis. Cases were divided into three groups based on the type of nail used (helical blade: group 1, n = 54; screw: group 2, n = 53; winged screw: group 3, n = 51). Demographic characteristics, mechanical complications, reduction quality, tip-apex distances (TAD) and Cleveland zones were compared between the groups. Femoral neck shortening, varus collapse, lag sliding, changes in abductor length were compared between study groups. Factors affecting mechanical complications were also analyzed., Results: Study groups were homogenic in terms of demographic characteristics, fracture type and reduction quality. Regarding mechanical complications, no statistically significant difference was found between groups. All three implants had similar outcomes on femoral neck shortening, varus collapse and lag sliding. Pooled analysis of 158 cases showed that mechanical complications increase as the quality of reduction decreases ( p = 0.000) same applies when TAD alters from the desired range ( p = 0.025) and with non-optimally implanted blade according to Cleveland zones ( p = 0, 000)., Conclusion: The radiological outcomes and mechanical complications of helical blade, screw type blade and winged screw type blade proximal femoral nails are similar in selected group. Regardless of the device type, it is necessary to obtain high reduction quality, obtain TAD within described range and optimally place the blade according to Cleveland Zones to reduce the failure rate and avoid complications., Competing Interests: The authors declare there are no competing interests., (©2024 Vahabi et al.)

Background: Earthquakes are natural disasters that threaten human life and cause loss of life and property in a very short time. In our study, we aim to carry out the medical analysis of the earthquake victims who came to our hospital after the Earthquake in the Aegean Sea and to share our clinical experiences., Methods: We retrospectively analyzed patients the medical data records of earthquake victims brought to our hospital or the injured who applied due to the Aegean Sea earthquake. Patients demographic data, their complaints and diagnoses, hour of admission, their clinical courses, hospital arrangements (admission, discharge, and transfer), time spent until the operation, anesthesia methods, surgical intervantions, intensive care needs, crush syndrome, presence of acute renal failure, number of dialysis, mortality, and mor-bidity were reviewed., Results: A total of 152 patients were brought to our hospital due to the earthquake. The most intense period of admission to the emergency department was the 1st 24-36 h. Mortality rate was found to be higher depending on the increase of age. While the most common cause of admission for the mortal earthquake survivors was to be trapped in the wreckage, the survivors applied for other reasons as well such as falling down. The most common type of fracture observed in survivors was the lower extremity fractures., Conclusion: Epidemiological studies can make an important contribution to the management and organization of the future earthquake-related injuries by healthcare institutions.

Intra-articular lesions combined with a tibial tuberosity fracture reported in the literature are; 3 patellar tendon ruptures, 2 anterior cruciate ligament tears, 2 medial collateral ligament tears, 2 medial meniscus tears, one arcuate ligament tear, and one lateral meniscus tear. In our study, both cases sustained an avulsion fracture of the tibial tuberosity. Preoperative MRI in one case revealed posterior cruciate ligament rupture. Under the image intensifier, we treated both patients by closed reduction and percutaneous screwing with 2 cancellous screws. Radiographic assessment showed complete healing of the avulsion fractures in both cases. Both of our patients gained previous levels of daily and sporting activity prior to the injury, and were completely asymptomatic. Our objective in reporting this case study is to point to the fact that there is no previous reporting of the avulsion fracture of the tibial tuberosity accompanied by posterior cruciate ligament rupture in the literature and to evaluate the findings of the minimal invasive treatment method we applied to both cases.

- Abstract
Background: In orthopaedic traumatology; acute compartment syndrome is a common complication associated with tibial fractures. In our study, we investigate differences, if any, between open and closed tibialfractures in terms oftheir risk of compartment syndrome., Methods: Fourty-two tibial fractures of 39 patients, 29 (69%) closed and 13 (31%) open, having the risk of acute compartment syndrome according to the clinical examination, were monitored following traumas during which their DP (DP= Diastolic blood pressure -Absolute compartment pressure) and absolute compartment pressures were measured via intracompartmental pressure measuring equipment., Results: Average absolute compartment pressure was found to be 31.25 mmHg in open and 29,32 mmHg in closed fractures (p=O,484) whereas average DP was measured to be 47,46 mmHg in open and 49,66 mmHg in closedfractures (p=O,415). According to student's test, the difference between the two groups was not significant. We found the incidence of acute compartment syndrome as 7, 7% in open fractures, 6.9% in closed and 7, 1 % in overall tibial fractures andfasciotomy was carried out in those cases., Conclution: Statistically not significant difference between the pressure levels,found in open and closed tibialfractures, showed that open type oftibialfractures holds the risk of compartment syndrome as much as the closed one.
