Abstract

2-s2.0-85074084911, Neuralgic amyotrophy is characterized by recurrent, painful, unilateral neuropathy involving mainly the upper brachial plexus followed by muscle weakness and muscle wasting. There are two forms: Idiopathic and hereditary. Hereditary neuralgic amyotrophy is an autosomal dominant disease that is often linked to a mutation of SEPT9, a gene of the Septin family. The phenotypic spectrum of the disease may include hypotelorism, cleft palate, and other minor dysmorphisms. The age of onset is from infancy to adulthood. Hereditary neuralgic amyotrophy can be triggered by external stimuli such as infections, vaccinations, cold, stress, surgery, and strenuous exercise. Here, we report a six-year-old girl who was found to have mutation in the SEPT9 gene when she presented with recurrent attacks of painful brachial plexopathy following vaccinations, and was diagnosed as having hereditary neuralgic amyotrophy. © Telif Hakkı 2019 Türk Pediatri Kurumu Dernegi Makale metnine.

Abstract

Objective: Idiopathic intracranial hypertension is a syndrome characterized by symptoms and signs of increased intracranial pressure. The aim of this study is to retrospectively evaluate the etiological and clinical features of the cases with the diagnosis of intracranial hypertension. Material and Methods: 14 patients with at least one year of follow up at Ege University Medical School Pediatric Neurology Department with Idiopathic intracranial hypertension diagnosis were included. The etiologic, clinical, treatment features and prognoses were evaluated. Results: Among 14 patients 9 were girls and 5 were boys. Their ages ranged from 3-17 years and the mean age was 10.42 (± 4.65 years). The most common complaint was headache. Seven patients (57.14%) were diagnosed as idiopathic intracranial hypertension. The etiologic factors detected in patients with secondary intracranial hypertension were sinusitis, hypervitaminosis A, obesity, sagittal sinus thrombosis, transverse sinus thrombosis and neuro-Behcet. All of the cases received medical treatment. Optic nerve fenestration was applied to a patient who did not benefit from medical treatment. One patient had recurrence. Conclusion: Idiopathic intracranial hypertension is one of the preventable causes of visual loss and rapid diagnosis and treatment are important because of the rare occurrence of visual field loss and decrease in the accuracy of visual acuity despite adequate treatment. © 2018 by Türkiye Klinikleri.

Abstract

The aim of this study is to analyze the epidemiology of the clinical and genetic features of childhood-onset limb-girdle muscular dystrophies (LGMD) in the Aegean part of Turkey. In total fifty-six pediatric cases with LGMD followed in four different pediatric neurology departments in the Aegean region of Turkey were evaluated. Among them, LGMD2C was the most common followed by LGMD2A, LGMD2D, and LGMD2F with equal frequencies. In twenty-eight patients (50%) the diagnosis could be confirmed by genetic analysis, where SGCG proved to be disease-causing in most of the cases. About half of the patients were diagnosed with whole exome or targeted gene sequencing. A positive correlation between muscle biopsy and genetic findings were observed in 11% of the patients. We report one novel frameshifting mutation in TTN. Knowledge on frequencies of childhood-onset limb-girdle muscular dystrophies and related genes in Turkey will lead to a prompt diagnosis of these neuromuscular disorders. © 2018 Pacini Editore S.p.A. Abstract

Aim To present the impact of treatments on health-related quality of life (HRQoL) from the double-blind, randomised phase III COMBI-d study that investigated the combination of dabrafenib and trametinib versus dabrafenib monotherapy in patients with BRAF V600E/K-mutant metastatic melanoma. COMBI-d showed significantly prolonged progression-free survival for the combination. Methods HRQoL was evaluated using the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire-C30, a generic cancer questionnaire (completed at baseline, during study treatment, at progression and post progression) assessing various dimensions (global health/QoL, functional status, and symptom impact). A mixed-model, repeated-measures analyses of covariance evaluated differences between arms. Results Questionnaire completion rates were >95% at baseline, >85% to week 40 and >70% at disease progression. Baseline scores across both arms were comparable for all dimensions. Global health dimension scores were significantly better at weeks 8, 16 and 24 for patients receiving the combination during treatment and at progression. The majority of functional dimension scores (physical, social, role, emotional and cognitive functioning) trended in favour of the combination. Pain scores were significantly improved and clinically meaningful (6-13 point difference) for patients receiving the combination for all follow-up assessments versus those receiving dabrafenib monotherapy. For other symptom dimensions (nausea and vomiting, diarrhoea, dyspnoea, and constipation), scores trended in favour of dabrafenib monotherapy. Conclusion This analysis demonstrates that the combination of dabrafenib and trametinib provides better preservation of HRQoL and pain improvements versus dabrafenib monotherapy while also delaying progression. Abstract

PubMed: 14748262 Lyme disease, an infection caused by Borrelia burgdorferi, has been reported in many countries from America and Europe, however, knowledge about its epidemiology in Turkey is incomplete. In this study, the clinical characteristics of 12 cases with Lyme disease who were diagnosed with the positivity of B. burgdorferi antibodies by western blot method in the laboratory of Ankara Bayindir Medical Center, have been reviewed. Physicians' recognition of early symptoms such as erythema migrans and later findings pertaining to the nervous system, joints, eye, and skin, and general awareness of the role of tick bites may increase the rate of diagnosis and allow earlier treatment of Lyme disease.
