Dear Editor, The COVID-19 pandemic has been such a nightmare for about a year. The COVID-19 is a disease that mainly concerns the specialties of infectious and chest diseases. However, there was such a global pandemic that the experts of these specialties could not bear the patient’s burden. Approximately 150 million people have been diagnosed with COVID-19 and more than 2 million patients have died (1). Physicians of all specialties in medicine have taken care of patients in the COVID-19 outpatient and inpatient clinics in many countries across the world including Turkey (2,3).As dermatologists, we took care and followed up 283 COVID-19 patients in our hospital between August 2020 and January 2021. The criteria for hospitalization were mainly severe involvement of lungs in computed tomography and shortness of breath that clinically required oxygen support. Patients were treated according to the guide by Turkey's Health Ministry Coronavirus Science Council. Favipravir and enoxaparine were given to all hospitalized patients while systemic antibiotics, systemic steroids, tocilizumab and other supportive agents were administered according to the patient's condition. Infection or chest diseases clinic was consulted if needed.The total number of patients with COVID-19 we followed up was 283 (141 women, 142 men). The age range of the patients was 21-95, the mean age was 63.33. 63 of the patients (22%) were under 60 years old. 24 patients (8.5%) were taken to intensive care during follow-up. 21 (7%) of the patients whose age range was between 60-95 years died. The average age of the patients who died due to COVID-19 was 74.6 years (3 male, 9 female). According to the most recent data from the Ministry of Health in Turkey, case-fatality rate is 1.5% (4). However, Çag and colleagues reported the death rate in patients who were hospitalized was 12% in their study that was performed in the first months of the outbreak. In that study, the average age of the patients who hospitalized was 61 years while the average age of patients who died was 74.5 years (5). Except death rate, other parameters are similar to each other. It can be suggested that in the later stages of the pandemic, the case-fatality rate decreased due to increased awareness of the diagnosis and administration of the diseases. Hospitalization time of the patients varied between 1-45 days. Average hospital stay was 8 days. A senior dermatologists say that dermatologists are not a doctor of the skin, in fact, they are a physician who deals with the skin (6). Dermatologists, who grew up with the concept that each skin disease may also be a reflection of an internal problem, did not experience difficulties in pandemic management. Further, they enrich their knowledge about infection and chest diseases. Dermatologists, who are the authors of this paper, were infected with the new coronavirus. After the recovery, they continued their duties in COVID-19 inpatient services.

Amaç: Omalizumab, antihistaminlere dirençli kronik ürtiker tedavisinde kullanılan monoklonal anti IgE antikorudur. Her dört haftada bir uygulama önerilmektedir.Yöntem: Kronik ürtiker tanısıyla izlenen 15 hastanın subkutan omalizumab tedavisine yanıtı yaş, cinsiyet, ürtiker süresi ve şiddetinin omalizumab ile elde edilen remisyon süresine etkisi incelendi. Bulgular: Yaş ortalaması 43.9±7.8 olan 8 kadın 7 erkek hastada ortalama 6.26±2.7 hafta remisyon sağlandı. Yaş, cinsiyet, ürtiker süre ve şiddeti remisyon süresini etkilemezken, ürtikerine astım eşlik eden hastalarda remisyon süresinin kısa olduğu gözlendi. Sonuç: Omalizumab dört hafta olarak belirlenen etki süresinden daha uzun remisyon sağlamaktadır., Objective: Omalizumab is a monoclonal anti-IgE antibody used in the treatment of chronic urticaria resistant to antihistamines. Its application every four weeks is recommended. Methods: The effects of age, gender, duration and severity of urticaria on the duration of remission achieved with omalizumab on the response of 15 patients with a diagnosis of chronic urticaria to subcutaneous omalizumab therapy were investiagted. Results: Remission was achieved for an average of 6.26±2.7 weeks in 8 female and 7 male patients with a mean age of 43.9±7.8 years. While age, gender, duration and severity of urticaria did not affect the remission period, it was observed that the remission period was short in patients with urticaria accompanied by asthma. Conclusion: Omalizumab provides a longer remission than the four-week duration of action.

Amaç: Verruka vulgaris, human papilloma virüslerinin neden olduğu benign epiteliyal proliferatif bir hastalıktır. Siğil tedavisinde topikal ve intralezyonel Dvitamini uygulamaları etkili yöntemlerdir. Bununla birlikte, hastalık ile serum D vitamini düzeyleri arasındaki ilişki belirsizdir. Hastalığın şiddeti ile serum 25-hidroksi D vitamini (25-OH vit D) düzeyleri arasındaki ilişki daha önce araştırılmamıştır. Bu çalışmada verruka vulgarisli hastalarda serum 25-OH vit D düzeyleri ile hastalık şiddeti arasındaki ilişkinin belirlenmesi amaçlanmıştır.Gereç ve Yöntemler: Verruka vulgarisli 40 hasta ile cinsiyet ve yaş açısından eşleştirilmiş 43 sağlıklı gönüllünün serum 25-OH vit D düzeyleri ölçüldü. Hastalar verruka vulgaris hastalık şiddeti ve süresine göre gruplandırılarak serum 25-OH vit D düzeyleri kaşılaştırıldı.Bulgular: Verruka vulgaris hastaları ile kontrol grubu arasında serum 25-OH vit D düzeylerinde anlamlı fark yoktu (p=0.760). Hastalar verruka vulgaris hastalık şiddeti ve süresine göre gruplandırıldığında hafif hastalığı olanlar ile şiddetli hastalığı olanlar arasında serum 25-OH vit D düzeyleri farklı değildi.Sonuç: Serum 25-OH vit D düzeyi verruka vulgaris ile ilişkili değildir. Serum 25-OH vit D düzeyleri hastalık şiddeti ve süresi ile de ilişkili değildir. Verilerimiz verruka vulgarisin önlenmesi veya tedavisi için D vitamini takviyesini desteklememektedir., Objective: Verruca vulgaris is a benign epithelial proliferative disease caused by human papilloma viruses. Topical and intralesional vitamin D applications are effective methods in the treatment of warts. However, the relationship between the disease and serum vitamin D levels is unclear. The correlation between the severity of the disease and serum 25-hydroxy vitamin D (25-OH vit D) levels have not been previously investigated. This study aimed to determine the relationship between serum 25-OH vit D levels and disease severity in patients with verruca vulgaris.Material and Methods: Serum 25-OH vit D levels were evaluated in 40 patients with verruca vulgaris and 43 sex and age-matched healthy volunteers. Serum 25-OH vit D levels were compared by grouping the patients according to the severity and duration of verruca vulgaris disease Results: There was no significant difference in serum 25-OH vit D levels between verruca vulgaris patients and the control group (p=0.760). There was no significant difference between serum 25-OH vit D level, disease severity and disease duration in the verruca vulgaris group (p =0.102, p=0.257, respectively).Conclusion: Serum 25-OH vit D level was not associated with verruca vulgaris. Serum 25-OH vit D levels are not related to disease severity and duration. Our data do not support vitamin D supplementation for the prevention or treatment of verruca vulgaris.

Hydroxychloroquine (HCQ) is a drug that is used for both chemoprophylaxis and treatment of COVID‐19. At the moment, HCQ maintain its popularity and tens of thousands of people across the world are still receiving the current drug.1After the COVID‐19 pandemic, several cutaneous side effects of HCQ are reported.2‐7 Herein, we reported generalized exanthema after a single dose of using HCQ. A 58‐year‐old male patient presented with resistant rashes on his upper extremities and neck region. Rashes appeared all over the body along with severe pruritus just a day after receiving a single 200 mg dose of oral hydroxychloroquine for COVID‐19 prophylaxis.

Medicinalleech application is a common traditional therapy used for various disorders inmany cultures for thousands of years. Although it is usually a safe method,serious side effects can also be seen. As a rare side effect of this method, cutaneouspseudolymphoma is a reactive skin disease caused by foreign antigens, infectiousprocesses and arthropod bites. Idiopathic cases sometimes can be seen.In this article, we present a case of cutaneouspseudolymphoma in an elderly woman with multiple nodules over her lumbar regionemerging after leech application., Tıbbi sülük uygulaması binlerce yıldırbirçok kültürde çeşitli hastalıkların tedavisinde kullanılan yaygın birgeleneksel tedavi yöntemidir. Genellikle güvenli bir yöntem olmasına rağmen,bazen ciddi yan etkiler de görülebilir. Bu metodun nadir bir yan etkisi olarakkutanöz psödolenfoma, yabancı antijenler, enfeksiyöz süreçler, artropodısırıkları ve bazen idiopatik nedenlerle oluşan reaktif bir deri hastalığıdır. Bu yazıda, sülük uygulaması sonrasıçok sayıda nodülü olan ve kutanöz psödolenfoma tanısı konulan bir kadın olguyusunuyor ve literatürü gözden geçiriyoruz.

Captopril inhibits angiotensin-converting enzyme and it is the most used sub-lingual drug in emergency departments to decrease blood pressure. Sublingual captopril is safe, effective and useful in treating hypertensive urgency. However, it is known that captopril has fewer systemic and dermatologic side effects such as headache, weakness and oral ulcers, oral pemphigus, consecutively. No data is available about captopril induced oral mucositis. Herein, we present a 50-year-old hypertensive woman who uses sublingual captopril frequently and suffers from recurrent, reversible and painful oral mucositis that thought to be caused by sublingual captopril.

OBJECTIVE: The aim of this study was to emphasize the importance of history and physical examination in patients presenting with hair loss, and to determine in which patients laboratory tests are necessary.MATERIAL AND METHODS: 622 patients with hair loss complaint, who applied to Karabük Training and Research Hospital Dermatology Clinic between April 2017 and April 2018 were included. The required complete blood count, serum glucose, vitamin B12, vitamin D, TSH, serum iron, iron binding capacity, and ferritin results and accompanying diseases were recorded in the patients diagnosed with any hair loss in the hospital database.RESULTS: Of the 622 patients, 339 (55%) were female and 283 (45%) were male. 321 (51%) of the patients had diffuse hair loss, 210 of them (33%) alopecia areata, 72 of them (12%) were diagnosed with androgenetic alopecia. The number of patients with scarring hair loss complaints was 14, and the number of patients with tinea capitis diagnosed with hair loss complaints was 5. Iron deficiency anemia was detected in 10% of female patients who complained of diffuse hair loss and Vitamin D deficiency was detected in 30% of patients. It was 34 in scarring alopecia patients. There were no abnormal laboratory findings in women with alopecia areata, androgenetic alopecia and scatricial alopecia except iron deficiency seen in 10% of women with diffuse hair loss.No abnormal laboratory findings could be associated with hair loss in all male patients presenting with hair loss.CONCLUSIONS: Our findings showed that laboratory examination of patients who presented with hair loss complaints did not give information about the cause except for selected patients. It will be a time and economic impact of not wanting unnecessary tests with a good anamnesis., AMAÇ: Bu çalışmanın amacı saç dökülmesi şikâyetiyle başvuran hastalarda öykü ve fizik muayenenin önemini vurgulamak, hangi hastalarda laboratuvar tetkilerinin gerekli olduğunu ortaya koymaktır.GEREÇ VE YÖNTEM: Çalışmaya Karabük Eğitim ve Araştırma Hastanesi Dermatoloji Polikliniğine Nisan 2017-Nisan 2018 tarihleri arasında saç dökülmesi şikayeti ile başvuran 622 hasta dâhil edildi. Hastane veri tabanındaki elektronik dosyalar yardımıyla herhangi bir saç dökülme türüne ait tanı alan hastaların tanı ve tedavi süreci içinde istenen tam kan sayımı, serum glukoz, vitamin B12, vitamin D, TSH, serum demir, demir bağlama kapasitesi sonuçları, eşlik eden hastalıkları kaydedildi. Bu veriler hastaların demografik özellikleriyle ve literatürle karşılaştırıldı.BULGULAR: 622 hastanın 339’u (%55) kadın, 283’ünü (%45) erkek idi. Saç dökülmesi yakınmasıyla başvuran hastaların 321’ini (%51) yaygın saç dökülmesi, 210’unu (%33) alopesi areata, 72’sini (%12) androgenetik alopesi tanısı alan hastalar oluşturdu. Skarla sonuçlanmış saç kaybı yakınması olan hasta sayısı 14, saç dökülmesi yakınması nedeniyle başvurup tinea capitis tanısı alan hasta sayısı da 5 olarak belirlendi. Yaygın saç dökülmesi yakınmasıyla başvuran kadın hastaların %10’unda demir eksikliği anemisi, %30’unda Vitamin D eksikliği saptandı. Yaygın dökülme yakınması olan kadın hastaların %10’unda görülen demir eksikliği dışında, alopesi areata, androgenetik alopesi ve skatrisyel alopesili kadın hastalarda anormal labaratuar bulgusu saptanmadı. Saç dökülmesi ile başvuran tüm erkek hastalarda da saç dökülmesiyle ilişkilendirilebilecek anormal labaratuar bulgusu saptanmadı.SONUÇ: Sonuç olarak saç dökülmesi şikâyeti ile başvuran bazı hastalarda laboratuar tetkikleri, saç dökülmesinin nedeni hakkında fikir vermektedir. İyi bir anamnez ve fizik muayene hangi hastada hangi testlerin istenmesi gerektiği konusunda yol gösterici olabilmektedir.

Amaç: Uyuz, sarkoptes scabies var hominisadlı bir ektoparazitin neden olduğu kaşıntılı bir deri hastalığıdır. Değişen hayattarzlarına bağlı olarak endemiler yapmaktadır. Bu çalışmada, son yıllardaki uyuziçin insidans ve demografik faktörlere dikkat çekmeye çalıştık.Yöntemler: Karabük Eğitim ve Araştırma HastanesiDermatoloji polikliniğinde 2013 Ocak -2018 Temmuz arasında, uyuz tanısı alanhastalar retrospektif olarak incelendi. Hastane kayıtları kullanılarak yapılançalışmada, uyuz tanılı hastaların yaş ve cinsiyetleri alındı. Uyuz tanısınınkonulduğu mevsim not edildi. Bulgular: Çalışmamızda, son beş yıl içinde,hastanemiz dermatoloji polikliniğinde uyuz görülme sıklığının % 0,4 den %1'ekadar yükseldiğini saptadık (r=0,886, p=0,001).Uyuz tanısı alan hastaların yaş ortalamasının, bu 5 yıl içinde 49,7 yıldan 31,4yıla düştüğü gözlemlendi (r=-0,753, p=0,006) . Uyuz tanılı kadın hastalar dahafazlaydı (%56 vs %44) ve uyuzlu kadın hastaların yaş ortalaması, erkekhastaların yaş ortalamasına göre daha yüksek idi (44 yıl ve 37 yıl) (p=0,001).Sonuç: Sonyıllarda, uyuz tanısı konulan olguların arttığı ve uyuz görülme yaşınınazaldığı gözlemlenmiştir., Aim: Scabies is a pruritic skin disease causedby an ectoparasite named sarcoptes scabies var hominis. It sometimes makes endemicsdepending on lifestyles of communities. In this study, we tried to point out incidenceand demographic factors for scabies in recent years.Methods: A retrospective review was carried outin patients with scabies diagnosis between 2013 January and 2018 July at dermatologyoutpatient clinic of Karabuk Research and Training Hospital. Age and gender ofpatients, and season of diagnosis were recorded from hospital database.Results: In this study, we found that incidenceof scabies increased from 0.4 % to 1% between study period (r=0.886, p=0.001 ).Mean age of scabies patients decreased from 49.7 years to 31.4 years within 5years (r=-0.753, p=0.006). There were more female patients than male patients(56% vs 44%), and their mean age was older than males (44 years vs 37 years) (p=0.001).Conclusion: Scabies cases are increasing in number and mean age of patients are decreasing gradually at recent years.

7. ÖZET 1994 -1997 ilk yansında Selçuk Üniversitesi Tıp Fakültesi Dermatoloji Anabilim Dalına müracaat eden toplam 21530 hastadan ürtiker tanısı alanların 286'sında enfeksiyon odağı araştırıldı. Olguların cinsiyet, yaş grubu, süresine göre ürtiker tipi ve mesleklerine göre enfeksiyon odağı varlığı incelendi. 286 olgunun 187 (%65.4)'si kadın, 99 (%34.6)'u erkek idi. Sürelerine göre ise 222 (%77.6)'si kronik, 64 (%22.4)'ü akut ürtiker idi. 286 olgunun 114 ( %39.8)'ü ev hanımı, 62 (%21.5)'si öğrenci, 35 (%12.2)'i memur, 20 (%7.0)'si öğretmen, 16 (%5.6)'sı serbest meslek, diğerleri de güvenlik görevlisi, sağlık personeli, emekli, çiftçi ve çocuklardan oluşuyordu. Toplam 181 (%63.3) olguda en az bir enfeksiyon odağı tesbit edildi. Kadınların %64.7'sinde, erkeklerin %60.6'sında enfeksiyon odağı tesbit edildi. Akut olguların %62.5'inde fokal enfeksiyon saptanırken, aynı oran kronik ürtikerli hastalarda %63.5 idi. Olguların yaş ortalaması 33.6 idi 25 yaş grubundan itibaren fokal enfeksiyon görülme sıklığının arttığı gözlenirken, en çok %73.8 ile 35-44 yaş grubunda fokal enfeksiyon bulundu. Mesleklerine göre yapılan değerlendirmede ev hanımlarının %67'sinde, öğrencilerin %58'inde, memurların %60'ında öğretmenlerin %69'unda ve serbest meslek sahiplerinin de %69'unda enfeksiyon odağına rastlandı. Saptanan fokal enfeksiyonların %41.6'sı ÜSYE, %13.9'u genital enfeksiyon, %10.1'i yüzeyel mantar enfeksiyonu, %9.1'i dental enfeksiyon, %3.8'i barsak parazitleri, diğerleri de alt solunum yolu enfeksiyonu, pyodermiler, üriner enfeksiyon ve HBs antijeni pozitifliği idi. Genital enfeksiyonların tamamı kadınlarda gözlenirken, yüzeyel mantar enfeksiyonlarının %69'u erkeklerde gözlendi. Bu iki enfeksiyon odağına %80'i 35 yaşının üzerindeki olgularda rastlandı. 286 olgunun sadece 20 (%7)'sinin boğaz kültüründe patojen mikroorganizma ürerken, kadın olgulardan alınan vajinal kültürlerde %13.3 oranında patojen mikroorganizma tesbit edildi. 41 8. SUMMARY Focal infections were searched in 286 patients with urticaria from a total of 21530 patients admitting to Dermatology Department of Selçuk University, Faculty of Medicine between 1994 and first half of 1997. The presence of focal infections was evaluated according to age, sex, clinical urticaria type and work of the patients. Patients were 187 females and 99 males. As clinical types, there were 222 chronic and 64 acute urticaria cases. Works of the cases were as follows: 114 (39.8%) house wifes, 62 (21.5%) students, 35 (12.2 %) officers, 20 (7.0 %) teachers, 16 (5.6 %) traders. Others were security staff, health staff, retired people and children. At least one infection focus was found in 181 patients. (%63.3). 64.7 percent of female cases and 60.6 percent of male cases had focal infection. Average age of the cases was 33.6. After 25 years old, frequency of focal infections increased. Among age groups, 35-44 had most frequent focal infections rate with 73.8 %. In the evaluation of accordingly to works, 67 percent of house wifes, 58 percent of students, 60 percent of officers, 5.6 percent of traders were found to have focal infection. Focal infections found were upper respiratory airway infections (41.6%), genital tract infections (13.9%), superficial mycotic infections (10.1%), dental infections ( 9.1%) and intestinal parasites (3.8%) in frequent order. Remainings were lower respiratory airway infections, pyodermias, urinary tract infections and presence of HBs antigen. All genital infections were observed in female cases whereas superficial dermatomycoses were mostly seen in males with a percentage of 69. Eighty percent of these two infections were determined in cases over 35 years old. Only twenty (7%) cases in throat culture of all patients and 25 (13.3%) cases in the vaginal culture of 187 women gave positive results. 42 47
