Artificial intelligence (AI) is revolutionizing various fields of medicine, and anesthesia is no exception. AI in anesthesia is not only streamlining the work of anesthesiologists but also improving patient outcomes. The integration of biotechnology and AI has created opportunities for personalized care in anesthesia, with the aim of optimizing patient outcomes. The use of biotechnology in conjunction with AI has further expanded the possibilities in anesthesia care. Biotechnology tools such as genomics and proteomics can provide insights into individual patient responses to anesthesia drugs, enabling personalized drug dosing and reducing the risk of adverse events. Similarly, the use of biosensors and wearable devices can provide continuous monitoring of vital signs and other physiological parameters, allowing for real-time adjustments to anesthesia care. In future the integration of AI and biotechnology in anesthesia care has the potential to transform the field of anesthesia and improve patient outcomes. The development of personalized care plans and the early identification of at-risk patients can lead to a safer and more efficient anesthesia experience for patients.

Surgeons and anesthesiologists play a critical role in emergency medicine, where timely intervention can mean the difference between life and death. The success of emergency medical interventions often relies heavily on the coordinated efforts of these two professions, who work in close collaboration to provide safe and effective care. Surgeons perform the necessary surgical procedures, while anesthesiologists administer anesthesia and monitor the patient’s vital signs throughout the procedure. In this paper, we explore the vital role of surgeons and anesthesiologists in emergency medicine. We review the various situations where their expertise is needed, such as trauma cases, organ failure, and life-threatening infections. We also examine the challenges and opportunities for collaboration that exist in emergency medicine, including the need for effective communication, teamwork, and coordination. Moreover, we highlight the importance of advanced training and experience for both professions, as they face unique challenges and stressors in emergency situations. We discuss the need for ongoing education and professional development to ensure that they are equipped with the knowledge and skills

Endotracheal tube (ETT) is commonly used to provide positive pressure ventilation. ETT cuff pressure is maintained within a range of 20 to 30 cmH2O. High pressure affects micro-circulation and integrity of the tracheal mucosa, resulting in complications. The cuff pressure is measured by connecting a pressure gauge device to pilot balloon. Two variety devices are available; analog or digital manometers. Present study is used to study the efficacy of AG CUFFILLL (Digital) compared with standard PORTEX Cuff inflator/pressure gauge (Analog) for the measurement of ETT cuff pressure.This study was conducted after obtaining institutional ethical clearance and written informed consent.100 patients between the age of 18-70yrs who fulfilled inclusion criteria. We measured ETT cuff pressure using AG CUFFILL (Digital) and standard PORTEX Cuff inflator/pressure gauge and compared the pressure recording of the two equipment and we studied ease of use for both.Out of 100 patients between the age of 18-70yrs, 61 patients (61%) were male and 39 patients (39%) were female. Mean cuff pressure of ETT using AG CUFFILL was 36.29±6.36 cmH2O and in PORTEX was 33.97±6.16cm H2O. Mean difference in the values between the cuff pressure measured by AG CUFFILL and PORTEX was ± 2.67. Comparing the two devices, it was noticed that in 05 (5%) patients measuring ETT cuff pressure using AG CUFFILL was difficult as the values were fluctuating. Based on results, authors recommend AG CUFFILL for ETT cuff pressure measurement owing to its accuracy, non-bulky, and ease to carry.

Background: The Corona virus disease 2019 (COVID-19) is a contagious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). From October, 2021 onwards many countries were experiencing a further occurrence of SARS-CoV-2 infections despite of successful vaccination campaigns. This situation has been suggested to be caused by the greater infectiousness of the delta variant of SARS-CoV-2, and by progressively decreasing immunity as time passes from earlier vaccination. In view of the current occurrence, several countries are planning to administer a third precautionary dose of COVID-19 vaccine. Since the critical care nurses are more vulnerable for COVID, we studied the precautionary COVID vaccination related side effects among them. Subjects and Methods: This A cross-sectional study was conducted among 75 critical care nurses who had received covishield vaccine. The survey included a total of 13 questions and was divided into 2 parts. The first part dealt with demographic data and second part with questions related to study. Results: 75 critical care nurses participated in the study voluntarily.

Majority of neurosurgical procedures are performed under general anaesthesia. It is still possible to perform some of the procedures under regional anaesthesia like scalp block. It involves blocking the nerves that innervate the scalp. The technique is easy to perform, but the practitioners should be aware of possible complications because of closer relation of blood vessels with nerves. Despite various advantages over general anaesthesia scalp block is not practiced routinely. This article reviews the relevant anatomy and technique of scalp block.

Jejunostomy is a surgical procedure where in a tube is placed in the lumen of proximal part of jejunum. This technique is mainly used to administer nutrition in case of patients with obstructive or operative area related to upper gastrointestinal system. This is one of the type to feed the nutrition and or medications by enteral means.

Covid-19 raged for more than two years in almost all parts of the world including India and the long term effects are yet to be studied. Presently with the availability of vaccine has reduced the suffering even after covid-19 infection. This study is aimed to understand the patient with past history of covid-19 infection and serum albumin levels and the outcome. A total of 52 patients admitted to intensive care unit with past history of COVID-19 infection, between 01.11.2021 and 31.01.2022, were enrolled in this study. The required data were collected from the records. Of the total 52 patients included in the study, 41 (78.85%) were males and 11 (21.15%) were females. Amongst these 52 patients, 18 (34.6%) were detected with hypoalbuminemia. Higher association of co-morbidities (22.22% patients) was seen in patients with hypoalbuminemia. Death rate in hypoalbuminemia group was 11.11% while that in those with normal albumin levels was only 2.94%.Patients with hypoalbuminemia and previous covid 19 infection were having higher mortality rate as compared to normoalbuminemia.

Operative trauma affects the physiology of fluid and electrolytes within the body. Magnitude and duration of this response is directly proportional to the severity of the trauma. Inorganic electrolytes like sodium, potassium, chloride and calcium are very important constituents of the body. There are preoperative, intraoperative and postoperative changes in both fluid volume and electrolyte compositions. The cases were selected among the patients operated in the Tertiary Care Hospital and then getting admitted to ICU from November 2020 to April 2021. Males and females, more than 18 years of age, got operated in same hospital were included in the study. The latest preoperative serum sodium and potassium levels had been noted. The postoperative serum sodium and potassium levels along with postoperative day had been noted till the patient was in ICU.There was gradual rise in the serum sodium level till post-operative day -6 and then it started decreasing and attained its normal preoperative value on post-operative day-11. The preoperative sodium level was restored in all cases by the 11th postoperative day. There was a gradual fall in serum potassium level from post-operative day-1 to post-operative day-6 and then it started increasing and attained pre-operative value by post-operative day-11. So, the response of serum sodium and serum potassium is contrary to each other. Based on the study results, increased sodium and decreased potassium levels were observed in postoperative cases. Normalization of these two electrolyte are seen postoperative day seven onwards.

Acute pesticide poisoning is a very common in rural India. The pesticides used in the agriculture are major contributors in poisoning either intentiaonal or accidental. Poisoning itself is a major global concern. Hexaconazole is not as common poison in India and very few cases are reports. We report a rare case of hexaconazole poisoning and its successful management in rural tertiary care unit.

There is considerate amount of stress which lies in the application of antibiotics in critically ill patients. A study on pre-hospital antibiotic administration in such patients would provide crucial data and go a long way in determining and understanding trends in antibiotic prescribing, to identify where necessary steps to be taken to improve prescribing are most needed and to measure progress. Materials and Methods: After ethical committee approval, we have conducted an observational crosssectional study for four months. We collected the data from patient or the relatives regarding prehospital antibiotic therapy on admission. Results: A total of 137 patients got admitted in the Medical intensive care unit (MICU) during the study period. Out of them, 91.24% of patients got admitted directly in MICU and 8.75% were referred from different hospitals. Those who received antibiotics prior to the admission were 2.18% and 93.43% of patients had not received antibiotics; the other patients who were not sure of antibiotic consumption were 4.37%. Among those 2.18% of patients who received, it was observed the most commonly utilized first antibiotic in our study was amoxicillin clavulanate potassium 66.66%. In all the patients that received more than one antibiotic, Metronidazole was the second antibiotic that was administered via intravenous route. Conclusion: Amoxicillin clavulanate potassium was the most commonly and metronidazole was the second most antibiotic prescribed in prehospital admission. Proper guidelines should be initiated for the starting of the antibiotic therapies in India for general practitioners. Keywords: Antibiotics, Incidence, Prehospital, Critically ill.

Myocardial infarction is a crucial and commonly seen in the Indian population. The disease is prevalent in both rural and urban area. Timely detection, awareness of the myocardial infarction among the population especially rural Indians should be taken seriously. Here we discuss an unusual case of myocardial infarction. Introduction: In India agriculture is the primary occupation of the people. With most of the population working in the fields, snakebite becomes a chief occupational hazard and a medical emergency. In India, more than 20,00,000 snake bites reported annually, of which 35,000-50,000 people die and Maharashtra alone contributes more than 2000 deaths each year. Thus this study is aimed to know the clinic-epidemiological pattern of snake bite in rural Maharashtra. Aims and Objectives: To study clinical and epidemiological characteristics found in patients presenting with snakebite in rural tertiary care unit. Design: Descriptive Cross Sectional study. Material and Methods: The study was conducted between 25/05/2017 to 10/09/2017 after obtaining institutional ethical clearance. A total of 70 patients were participated in study. Set questions were asked to the patient to know the clinic-epidemiological characteristics of snake bite. Statistical analysis: The data was analyzed and expressed in terms of percentage. Results: Patients with various age groups got admitted during the study period. Maximum number of (38) patients are in 21 to 40 yrs. Majority of patients were females were 36(51.4 %). Majority of patients (31) were farmer. Most of the patients were working in the farm 47(67.1%) while they had bite. Maximum bites are seen in patients right lower limb 22(31.2%). The most common vehicle used was two-wheeler 38(54.2%). Out of 70 patients admitted in hospital, 36(51.4) patients suffered with vasculotoxicity. 14(20%) patients brought snakes after killing to the hospital for identification. 22(31.4%) patients out of 70 took primary treatment in primary health care center. Out of 27 patients 5 patients required ventilator support who suffered with neurotoxic type of snake bite. We observed no mortality among 70 patients admitted with snake bite. Intraoperative temperature management is one of the important aspects in successful outcome of the surgical patient. Both anesthesia and surgery significantly have an impact on internal thermoregulation of the human body. We conducted this study to correlate nasopharyngeal temperature with transcutaneous carotid artery temperature of adult patients under general anesthesia (GA) in our rural institution.
