Lymphoma, a common malignancy of liver called as primary lymphoma of liver or primary hepatic lymphoma (PHL). It’s an unusual form of non-Hodgkin’s lymphoma having symptoms such as hepatomegaly and signs of cholestatic jaundice. With appropriate treatment this hepatic lymphoma have relatively good prognosis. We report a case of 40 years old male with abdominal lump in epigastric region, having constant weight loss from one month and past fever history. Blood estimation leucocytosis were found with mild elevation in hepatic enzymes, on (fine needle aspiration cytology) FNAC lesions were seen in liver. Immunohistochemical examination showed high grade non-Hodgkin’s B cell lymphoma. The radiographic and sonographic examinations were done. On diagnosis secondary metabolites of liver, systemic lymphoma with secondary involvement of liver and hepato-cellular carcinoma were seen. The patient responded for the chemotherapy and is still on follow-up.

The cervicofacial flap was first described in 1969. For the past several years, it has been the flap of choice for the reconstruction of facial defects especially cheek defects. In recent years, with the advent of microvascular free flap tissue transfer, the use of a cervicofacial flap has been sparse. This article highlights the importance and application of locoregional flaps such as a cervicofacial flap in the reconstruction of soft tissue defects on a face. This case was unique from the reconstruction point of view. An 81-year-old patient presented to us with a long standing ulcerated growth in his left cheek which was neglected due to lack of symptoms and his poor financial condition. Clinically, the extent was defined from the suborbital skin region till the mid cheek region and medially from the lateral nasal crease up to the cheek prominence area. An initial biopsy suggested basal cell carcinoma. The lesion was excised, and a 4.5×5.5 cm defect was reconstructed using a modified “cervicofacial” flap under local anesthesia. Our experience shows that this technique is a reliable, easy to harvest flap with good outcomes. Moreover, it can be utilized in those candidates not suitable for a free flap procedure under general anesthesia.

Introduction: The potential risk of prolongation of treatment time in cervical cancer has been reported for many low-dose rate (LDR) studies, with an estimated loss of local control ranging from 0.3 to 1.6% per day of treatment prolongation. Since the treatment schedule for fractionated high-dose rate intracavitary brachytherapy (HDRICBT) is not directly comparable with that for LDR studies. Many studies are also present with different results. Aims: To evaluate the adverse effect of treatment prolongation for cervical cancer treated with HDRICBT. Methods and Materials: Hundred patients with biopsy proven squamous cell carcinoma of the cervix with stage IIB to IVA (according to FIGO classification) were entered into protocol using concurrent paclitaxel and radiation. Radiotherapy was conventionally administered: 50.4 Gy/28 fractions by external beam (whole pelvis) followed by HDR-ICBT, 4 fractions of 7 Gy each. Paclitaxel was administered on weekly basis at dose of 40 mg∕m 2 during entire course of external beam radiotherapy. Results: Treatment response was evaluated three months after the end of radiotherapy by means of clinical examination and ultrasonography. Complete Regression (CR) in 83%, partial response (PR) 14% and progressive disease 3%. At 26 months of median follow up 73 patients alive out of 58 patients are diseases free. Conclusion: The results of this study suggest that to achieve better treatment outcome, avoid treatment prolongation and overall treatment time should be less than 50 days.

Squamous cell carcinoma of the head and neck (SCCHN) is a common malignancy. It is the 7th most common malignancy. Approximately 45,000 new cases are diagnosed each year in the USA. In present study 10 patients were selected of age group between 40 - 60 years from Gwalior-Chambal region, among them 3 were females and 7 were males. 3 patients were suffering from tonsilitis, 4 from Laryngeal carcinoma, 2 from carcinoma in maxillary antrum and 1 patient was suffering from carcinoma of whole tongue. The aim of present study is to evaluate the safety of nanoparticle paclitaxel (Nanoxel, a plant alkaloid) in combination with radiation in the treatment of locally advanced head and neck carcinoma. Further the patients were divided in 2 groups. The 1st group received nanoxel (plant alkaloid) administered concomitantly with radiation therapy. 2nd group received gemcitabine administered in combination with radiation therapy. 9 patients showed response for the therapy. The adverse effects in arm A, 2 patients showed anemia less than 8 gm of Hemoglobin, 2 patient showed leucopenia and 1 patient showed diarrhoea, Nausea/vomiting were shown by 4 patients, in arm B, 1 patient showed leucopenia, 1 patient showed anemia less than 9 gm hemoglobin and 5 patient showed sensory neuropathy. Nanoxel (plant alkaloid) showed best results in the case of head and neck carcinoma. The dose of 30 mg/kg b.w showed best safety with best clinical effect in term of less time of patient admission (hardly 30 mins) with less side effects with greater clinical effects in our study.

Background: Breast cancer is one of the most common malignancies found among women in India. Haematological parameters have been shown to predict severity, mortality and treatment follow-up in breast cancer patients. The aim of the study was to compare haematological parameters along with effect of chemotherapy and radiotherapy in different stages of breast cancer patients.Methods: Total 235 human subjects were taken in the study. Out of which 100 normal ages matched healthy subjects were considered as controls and 135 breast cancer patients subjects as cases which were further divided into their respective stages according to TNM classification.Results: Estimation of haematological parameters was done by Sysmex Automated Hematology Analyzer Kx-21. Primary breast lymphoma is a rare tumor that presents commonly as a large mass with no specific mammographic appearance and can be confused with poorly differentiated carcinoma on frozen section. With appropriate treatment, this breast malignancy has a relatively good prognosis. We report a case of primary mammary non-Hodgkin lymphoma (NHL) in a 30-year-old young female presented with the complaint of painless lump in the right breast. Fine needle aspiration cytology (FNAC) suggested malignancy. Investigations ruled out metastasis, hence mastectomy was done. Immunochemistry of specimen revealed NHL of breast. Investigations to rule out any other site of lymphoma proved it to be primary NHL of breast with no metastasis. Anticancer drugs are main treatment rather than surgery so it is very important to accurately diagnose primary lymphoma of breast.

Papillary thyroid carcinoma with metastasis to the skull is extremely rare. We report a case of unsuspected papillary thyroid carcinoma with skull metastasis. A 48-year-old female patient presenting with painless, pulsatile, progressively increasing swelling in the occipitoparietal region of the scalp approached for an X-ray of the skull. Ultrasound of palpable swelling in the neck revealed a heteroechoic lesion with increased vascularity. Foci of calcification were seen involving both lobes of the thyroid. Ultrasound of scalp showed a destructive mass in the skull with increased vascularity. Biopsy of thyroid lesions revealed branching papillae having a dense fibrovascular core covered by cuboidal epithelial cells with nuclei having a clear ground glass appearance. This case illustrates how isolated extensive skull metastasis can be found in papillary carcinoma patients without causing significant morbidity. Therefore, in the clinical course of thyroid papillary carcinoma, skull metastasis should be considered, and the patients should be meticulously investigated and followed up.
