We designed this study to determine diagnostic value of computerized tomography (CT) that was proven nodules in explant liver. We compared between CT images of liver nodules before liver transplantation(LT) and histological findings nodules of explant liver. 75 (%73.5) patients were male and 27 ( %26.5) patients were female. Average age was 47.3±11,8. LT was performed to cause 33 (%32.3) patients viral hepatitis, 7(%6.8) patients fulminant hepatitis, 27(%26.5) patients hepatocellular carcinoma (HCC) and 35 (%34.4) patients other etiologies. Serum alpha-fetoprotein median level was 4,3 ng/ml (0,5–608).63 of 102 (%61.8) patients were not determined nodules in explant liver. Histological findings in 19 (%18.7) patients HCC, 11(%10,7) displastic nodules and HCC, 7 (%6.9) patients displastic nodules, 2(%1.9) patients regenerative nodules in explant liver. We have reached CT images 33 of 102 patients. It was compared between nodules that similar locations explant liver and total 55 nodules that determine by CT. 17(%31) are regenerative nodules, 30 (%54.5) nodules HCC and 8(%14.5) nodules displastic in CT. 6 ( %10.9) nodules regenerative nodules, 37 (%67.3) nodules HCC, 8(%14.5) nodules displastic, 3(%5) nodules displastic nodules and HCC, 1(%1.8) nodule was regenerative nodule and HCC focus in explant liver. Despite advances in surgical techniques and products for stoma care, stoma-related complications are still common. A retrospective analysis was performed of the medical records of 462 consecutive patients (295 [63.9%] female, 167 [36.1 %] male, mean age 55.5 +/- 15.1 years, mean body mass index [BMI] 25.1 +/- 5.2) who had undergone stoma creation at the Gastroenterological Surgery Clinic of Turkiye Yuksek Ihtisas Teaching and Research Hospital between January 2008 and December 2012 to examine the incidence of early (ie, within 30 days after surgery) stoma complications and identify potential risk factors. Variables abstracted included gender, age, and BMI; existence of malignant disease; comorbidities (diabetes mellitus, hypertension, coronary artery disease, chronic respiratory disease); use of neoadjuvant chemoradiotherapy; permanent or temporary stoma; type of stoma (loop/end stoma); stoma localization; and the use of preoperative marking of the stoma site. Data were entered and analyzed using statistical software. Descriptive statistics, chi-squared, and Mann-Whitney U tests were used to describe and analyze all variables, and logistic regression analysis was used to determine independent risk factors for stoma complications. Introduction: Spontaneous rupture of hepatocellular carcinoma (HCC)is a rare but fatal complication that can be occurred during follow up inadvanced stages of chronic liver failure.Case Report: A 66-year-old male was admitted with tachycardia,fade and abdominal distentionto the emergency clinic. The patient’s hemoglobinlevel was 5 mg/dl and the hematocrit value was 19 %.Abdominal contrast-enhancedcomputed tomography (CT) detected an extensive hematoma in the perihepatic areawith a laceration of 37.7x35 mm in the dome of the liver extending from segmentV to segment VIII. An emergent explorative laparotomy was performed inhemorrhagic shock status. The tumor mass was resected with negative surgicalmargins. Patient’s postoperative course was uneventful.Discussion: Spontaneous HCC rupture is a catastrophic situationwhich generally occurs suddenly and leads to coagulopathy, hemodynamicinstability and liver failure. Hematokritte ani düşüş tanıkoydurucu olabilmektedir. Ultrasongrafi ve BT rüptüre karaciğer kitlesi,intraperitoneal sıvı veya hematomu göstermede faydalıdır. Rüptürü HSK’ nunrezeksiyonu mümkünse önerilen tedavi yöntemidir. Seçilmiş ve cerrahiyapılamayacak hastalarda transarteriyel kemoterapi (TAKE) yöntemi alternatifyöntemdir. Sonuç:SpontanHCC rupture hayatı tehtid eden ciddi bir durumdur. Acil durumlarda rüptüretümörün rezeksiyonu hem hayat kurtarıcı hem de primer hastalığın tedaviseçeneğidir.

Accessory spleens are present in 10% of population and are formed during embryonic development. When an accessory spleen is located in the pancreas, it may mimic a hypervascular pancreatic tumor and can make a diagnostic difficulty. Most of the cases were identified only after surgical resection. Herein, we present the case of a 52-year old man whom laparoscopic spleenpreserving distal pancreatectomy was performed with suspicion of neuroendocrine tumor. Postoperative histopathological examination revealed the intrapancreatic accessory spleen., Özet Aksesuar dalak toplumun yaklaşık %10’unda görülen ve embriyolojik gelişim sırasında oluşan bir anomalidir. Pankreasta lokalize olduğunda hipervasküler pankreatik tümörü taklit edebilir ve tanısal güçlüğe neden olabilir. Vakaların çoğu cerrahi rezeksiyondan sonra tanımlanır. Bu yazımızda nöroendokrin tümör şüphesiyle laparoskopik dalak koruyucu distal pankreatektomi yapılan 52 yaşındaki bir erkek olguyu sunuyoruz. GİRİŞ ve AMAÇ: AMAÇ: Trokar yeri hernisi, laparoskopik cerrahi sonrası trokar yerlerinde gelişen bir tür insizyonel hernidir. Gelişmesi halinde laparoskopik cerrahinin sağladığı tüm yararları ortadan kaldırıp tekrar operasyon gereksinimine ve iyileşme süresinin uzamasına yol açabilir. Biz bu çalışma ile laparoskopik cerrahinin önlenebilir bir komplikasyonuna dikkat çekmek istedik.YÖNTEM ve GEREÇLER: YÖNTEMLER: Kliniğimizde 42 haftalık bir süre içerisinde yapılmış olan tüm laparoskopik prosedürler retrospektif olarak hasta kayıtlarından incelenmiştir. Trokar yeri hernisi nedeniyle opere ettiğimiz hastalar belirlenmiştir. Hastaların demografik özellikleri, yapılan laparoskopik prosedürler, komorbiditeleri kaydedilmiştir. BULGULAR: Belirtilen süre içinde 3477 hastaya laparoskopik girişim yapılmış olup 4 hastaya trokar yeri hernisi tanısı konmuştur. Hastaların ikisi kliniğimizde, bir tanesi hastanemiz üroloji kliniğinde, diğeri ise dış merkezde opere olmuştur. Hastaların hepsi semptomatik olup acil cerrahi müdahale yapılmıştır.TARTIŞMA ve SONUÇ: SONUÇ: Trokar yeri hernisi gelişimini etkileyen birçok faktör tariflenmiş, gelişimini önlemeye yönelik de birçok yöntem önerilmiştir. Hastaların birçoğu asemptomatik olduğu için gerçek insidansı tam olarak bilinmemektedir. Boyutu ≥ 10mm olan trokarların defektlerinin kapatılması en önemli önleyici yöntem olarak görünmektedir., INTRODUCTION: OBJECTIVE: Trocar site hernia is a type of incisional hernia occuring at the trocar sites after laparoscopic surgery. After the devolopment of trocar site hernia, the benefits of laparoscopic surgery may be disappeared, reoperation may be needed and recovery period may be elongated. In this study, we want to take attention to a preventable complication of laparoscopic surgery.METHODS: We investigated the all laparoscopic procedures performed in a 42 weeks period in our surgery clinic from the patient records, retrospectively. We recorded the information about patient demografics, laparoscopic procedures, patient comorbidities.RESULTS: A total of 3477 patients underwent laparoscopic procedures over the signified period and four hernias were identified as occuring at the trocar entry site. Two patient were operated in our clinic, one of them was operated in urology department of our hospital and the last patient was operated in an other center. All were symptomatic and all were operated emergently.DISCUSSION AND CONCLUSION: CONCLUSION: Many factors affected the devolopment of trocar site hernia were described and many methods were suggested for its development. The true incidence is unknown because most patients are asymptomatic. The closure of fasia defect of ≥10 mm trocar seems the best preventive method.

The aim of this study was to report the early results of laparoscopic colorectal resections for colorectal carcinoma. Materials and methods: Patients who had been hospitalized with diagnosis of colorectal cancer between August 2000 and November 2003 were evaluated and early results reported. Results: Conversion rate was 7% (2 of 28 patients). Postoperative complications were observed in 6 patients, and the most frequent was partial intestinal obstruction. One patient was reoperated for intestinal fistula. Median follow-up was 13.5 (3-40) months. During this period, 1 patient developed distant metastasis. No recurrence was detected in port sites, incisions and pelvis. Two patients died during the study period. Conclusion: Laparoscopic colorectal resections can be performed safely and efficiently in centers experienced in laparoscopic techniques and colorectal surgery. None of our patients developed a deleterious effect due to laparoscopy during the follow-up period., Bu çalışmamızın amacı kolorektal kanserli olgulara uygulanan laparoskopik rezeksiyonların erken dönem sonuçlarını bildirmektir. Gereç ve yöntem: Türkiye Yüksek İhtisas Hastanesi Gastroenteroloji Cerrahisi Kliniği'ne Ağustos 2000 ile Kasım 2003 tarihleri arası nda kolorektal kanser tanısıyla yatırılan ve laparoskopik rezeksiyon yapı lan 28 olgu değerlendirilmiş ve erken dönem sonuçlar bildirilmiştir. Bulgular: Yirmisekiz olgunun 2 (%7)'sinde konversiyona geçildi. Altı olguda (%21) ameliyat sonrası komplikasyon görüldü. En sık görülen komplikasyon kısmi barsak tıkanmasıydı. Bir olgu intestinal fistül nedeniyle yeniden ameliyat edildi. Serimizin ortanca izlem süresi 13.5 (3-40) ay olup, bu süre esnasında bir olguda uzak metastaz görüldü. Port yerinde, insizyon yerinde ve pelviste nüks saptanmadı. İki olgu eksitus oldu. Sonuç: Laparoskopik kolorektal kanser cerrahisi, laparoskopik ve açık kolorektal cerrahi girişimlerde deneyimli merkezlerde güvenle ve yeterli etkinlikle yapılabilir. İzlemde olgularımızın hiçbirinde laparoskopiye ait olumsuz bir etki görülmemiştir.

Hepatocellular carcinoma is a common malignancy worldwide. The most frequent risk factor for hepatocellular carcinoma is liver cirrhosis; however, about 10% to 15% of cases occur in noncirrhotic livers. Hepatocellular carcinoma is also one of the most aggressive tumors, causing frequent intrahepatic metastasis, and recurrence after surgery is common. Extrahepatic metastases are seen in patients with advanced-stage intrahepatic tumors. Adrenal metastases from hepatocellular carcinoma are rarely seen in clinical practice. Bilateral adrenal metastases of hepatocellular carcinoma in a short follow-up period are very rare., Hepatosellüler karsinom, dünya genelinde yaygın bir malignitedir ve karaciğer sirozu hepatosellüler karsinom için en sık rastlanan risk faktörüdür. Olguların yaklaşık %10 ile %15'i non-sirotik karaciğerde gelişir. Hepatosellüler karsinom aynı zamanda, sık intrahepatik metastazlara neden olan agresif bir tümördür ve cerrahi sonrası nüks sıktır. Ekstrahepatik metastazlar ileri evre intrahepatik tümörü olan hastalarda görülür. Hepatosellüler karsinom'un adrenal metastazı klinik pratik içerisinde oldukça nadir görülür. Özellikle kısa dönem takiplerde bilateral adrenal metastazlar oldukça nadirdir.

Mide divertikülü çok nadir görülen bir patolojidir. Yaklaşık olarak 10.000 endoskopinin 4’ünde görülür ve çoğunluk- la tesadüfen saptanır. Mide’de en sık gastroözofageal bileşkeye yakın arka duvarda yerleşir. 1 cm’den 10 cm genişliğine kadar olan olgular bildirilmiştir. Çok küçük olanlar ülser veya kanser ile, özofagogastrik bileşkeye yakın olanlar ise paraözofageal herni ile karıştırılabilir. Melanoma is a neoplastic disorder produced by malignant transformation of normal melanocytes, the cells that produce the pigment melanin. During the first trimester of fetal life, precursor melanocytes arise in the neural crest. As the fetus develops, these cells migrate to areas including the skin, meninges, mucous membranes, upper esophagus, and eyes. In each of these locations, melanocytes have demonstrated a potential for malignant transformation. The eye, central nervous system, gastrointestinal tract, and even the gallbladder have been reported as primary sites of the disease. Melanoma represents 4% to 5% of all skin malignancies. Over 90% are found on the skin; however, 4% of melanomas are discovered as metastases without an identifiable primary site. Although malignant melanoma is only the eighth most common cancer diagnosis in the United States, the large majority (80%) of melanoma patients present in the productive years from age 25 to 65. The mean age for diagnosis of melanoma is 48., Melanoma, normal melanositlerin malign dönüşümünden ortaya çıkan neoplastik bir bozukluktur. Melanositler, melanin pigmentini üreten hücrelerdir. Objectives: Surgical operations have been shown to cause a variety of changes in cytokines and acute phase proteins. These changes can be markedly attenuated or prevented by cyclo-oxygenase inhibition. Therefore the authors assessed the effect of piroxicam treatment, having the contradictory results regarding this subject, on cytokine and acute phase protein changes after surgery. Material and methods: The systemic cytokine and acute phase protein responses to major gastrointestinal surgery were studied in 24 patients undergoing elective gastric or rectal surgery. The patients were divided into two groups. Both groups had 12 patients and consisted of six patients undergoing subtotal distal gastrectomy and six patients undergoing low anterior resection. Piroxicam (10 mg, IM), a cyclo-oxygenase inhibitor, was administered to group II patients, but not to group I patients, 12 hour before surgery and 1 and 12 hours after surgery. Blood samples were collected 8 hour before surgery and one and five days after surgery for interleukin-6, tumor necrosis factor-a, C-reactive protein and fibrinogen determinations. Bir siklooksijenaz inhibitörü olan Piroxicam (10 mg, intramuskuler) Grup I hastalara cerrahiden 12 saat önce ve 1 ile 12 saat sonra olmak üzere toplam 30 mg uygulandı. Grup II hastalara ise Piroxicam uygulanmadı. Human cystic echinococcosis is a zoonosis transmitted by ingesting Ecchinococcus granulosus ova eliminated from infected dogs. A 41-year-old man was admitted to the hospital with symptoms of abdominal tenderness located in the epigastric region, fever and vomiting. .We diagnosed a recurrent hydatic disaese and after admission to our hospital he began to cough with greenyellowish colored sputum with abscess. Posteroanterior chest X-ray and right pleural US revealed no fluid collection but right lower lung atelectasis and diaphragma elevation. With these results we performed an operation for broncho-biliary fistula. He was discharged from the hospital on the ninth postoperative day. Careful assessment and early treatment of septic complications are essential in the successful treatment of this rare condition. PA akciğer grafisinde ve sağ plevral US’de sıvı saptanmadı ve sağ akciğer alt lob atelektazisi ve diyafram elevasyonu tespit edildi. Bu bulgularla bronkobiliyer fistül tanısıyla hasta ameliyata alındı. Postoperatif dokuzuncu günde hasta taburcu edildi. Bu nadir görülen durumun başarılı tedavisi için septik komplikasyonların dikkatli değerlendirme ve erken tedavisi önem arz etmektedir. ÖzetFamiliyal adenomatöz polipozis (FAP) gastrointestinal sistemde özellikle kolon ve rektumda çok sayıda adenomatöz poliplerin varlığı ile karakterize otozomal dominant geçiş gösteren bir sendromdur. Bu sendromda; adrenal adenomlar, osteomalar, desmoid tümörler gibi bening tümörler, tiroid ve pankreatik kanserler, hepaoblastomalar, santral sinir sistemi tümörleri ile diş anomalilerinin görülme olasılığı artmıştır. FAP-related complications, for which medical attention is essential, are not rare and their estimated lifetime risk presumably exceeds 30%. Bu sendromda; adrenal adenomlar, osteomalar, desmoid tümörler gibi bening tümörler, tiroid ve pankreatik kanserler, hepaoblastomalar, santral sinir sistemi tümörleri ile diş anomalilerinin görülme olasılığı artmıştır. Burada 28 yaşında karın duvarında kitle şikayeti ile başvuran ve FAP zemininde gelişen rektum kanseri ve desmoid tümör tespit edilen hasta sunuldu

ÖzetAortoenterik fistüller(AEF) geçirilmiş aorta cerrahisi hikayesi varlığına ya da yokluğuna göre, primer ve sekonder tipler olarak sınıflandırılabilirler. Sekonder aortoenterik fistül(SAEF); abdominal aortik rekonstriktif cerrahinin seyrek görülen fakat çok önemli bir komplikasyonudur. Bu komplikasyon sıklıkla orijinal cerrahiden aylar, yıllar sonra görülür. Aortik rekonstrüktif cerrahi sonrası SAEF gelişimi ve insidansı %0,4 - 4 olarak rapor edilmiştir. Klinik öykü ve muayene bulgularının nonspesifik olmasından dolayı, aortoenterik fistüllerin preoperatif tanıları güçlük arzeder. Aorta ile sindirim kanalı arasında fistül formasyonu gelişmesi gastrointestinal kanama nedenlerinden biridir. Klinikte nerdeyse tamamen gastrointestinal kanamayla ortaya çıkarlar. Burada geçirilmiş aortik rekonstruksiyon cerrahisinin komplikasyonu olarak gelişen aortoduodenal fistüle bağlı şiddetli gastrointestinal kanama nedeniyle ameliyat ettiğimiz 49 yaşındaki erkek hasta sunuldu.Anahtar sözcükler: Aortoenterik fistül, gastrointestinal kanama, aortik rekonstruktif cerrahi.AbstractAortoenteric fistüla(AEF) can be classified into primary and secondary types according to the presence or absence of a prior history of aortic surgery. Secondary aortoenteric fistula(SAEF) is an uncommon but very important complication of abdominal aortic reconstrictive surgery. This complication often occurs months to years after the original surgery. SAEF develops after aortic reconstructive surgery and the the incidence has been reported to be % 0.4-% 4. Because of the nonspesific nature of the clinical history and physical findings, diagnosis of aortoenteric fistula is difficult to make preoperatively. Fistula formation between the aorta and alimentary tract is one of the causes of gastrointestinal bleeding. The most common clinical manifestation of the aortoenteric fistula is gastrointestinal bleeding. In this article, we report a case of a 49 years old man who underwent surgical treatment because of severe gastrointestinal bleeding which was related to an aortoduodenal fistula due to abdominal aortic reconstruction.Key words: Aortoenteric fistula, gastrointestinal bleeding, aortic reconstructive surgery.

ÖzetCaroli hastalığı intrahepatik safra yollarının kistik dilatasyonu ile karakterize, konjenital, nadir görülen bir hastalığıdır. Caroli hastalığının klinik özellikleri hepatolitiazis veya bakteriyel kolanjite bağlı olarak gelişen sarılık, sağ üst kadran ağrısı, ateşi kapsamaktadır. Endoskopik veya perkutan kolanjiografi geleneksel tanı yöntemidir, fakat günümüzde manyetik rezonans kolanjiopankreatografi tercih edilen tanı aracı olarak ortaya çıkmıştır. Caroli hastalığının tedavisi; kolanjit için antibiyoterapi ve destek tedavisi, hepatolitiazis için ursodeoksikolik asit tedavisini kapsamaktadır. Karaciğerin tek lobunu tutan lokalize Caroli hastalığında cerrahi rezeksiyon başarı ile uygulanmaktadır. Diffüz tutulumlarında ise ortotopik karaciğer nakli seçilecek tedavi olmalıdır. Biz burada 58 yaşında, 3 yıldır karın ağrısı şikayeti, kolanjit atakları olan ve sonuçta cerrahi rezeksiyon ile tedavi edilen Caroli olgusunu sunduk.Anahtar sözcükler : Caroli hastalığı, kistik genişleme, kolanjitAbstractCaroli's disease is a rare congenital disease of the liver characterized by cystic dilatation of the intrahepatic bile duct. The clinical features of Caroli's disease include jaundice, right upper abdominal pain and fever due to the associated complications of hepatolithiasis or bacterial cholangitis. Endoscopic or percutaneous cholangiography is the traditional method of diagnosis, but nowadays magnetic resonance cholangiopancreatography is emerging as the diagnostic modality of choice. The treatment for Caroli's disease includes supportive care with antibiotics for cholangitis and ursodeoxycholic acid for hepatolithiasis. Surgical resection has been used successfully in patients with monolobar disease. For patients with diffuse involvement, the treatment of choice is orthotopic liver transplantation. In this article, we report a 58 years – old male patient with Caroli’s disease. The patient had abdominal pain and episodes of cholangitis for 3 years and finally, he was treated with surgical resection.Key words : Caroli’s disease, cystic dilation, cholangitis
