1. Short‐ and long‐term prognostic performance of exercise ECG and myocardial perfusion SPECT.

Abstract

Background: Myocardial perfusion SPECT (MPS) and exercise electrocardiography (Ex‐ECG) results are of prognostic importance for short‐term follow up duration. However, the value of MPS or Ex‐ECG findings for long‐term risk assessment is less evident as underlying risk factors for ischemic heart disease (IHD) gain in importance. Objectives: To assess the short‐ and long‐term prognostic value of MPS and Ex‐ECG in relation to known risk factors. Methods and Materials: An observational study of 908 patients (age 63 years, 49% male, 45% prior IHD) referred for MPS and Ex‐ECG. Follow‐up was divided into two periods (short‐term: <5 years and long‐term: >5 years). Cardiac events were defined as a composite of acute myocardial infarction, unstable angina, unplanned revascularization and cardiovascular death. Results: The composite endpoint occurred in 95 patients (short‐term follow up) and in 94 patients (long‐term follow up). In multivariable models stress testing had a strong predictive value for short‐term follow up (HR for MPS = 2.9, CI = 1.9–4.5, p < 0.001 and HR for Ex‐ECG = 2.1, CI 1.3–3.3, p = 0.002), but no predictive value for long‐term follow up (HR for MPS = 0.9, CI = 0.5–1.5, p = 0.70 and HR for Ex‐ECG = 1.0, CI = 0.6–1.6, p = 0.92). Male sex and prior IHD were significant predictors regardless of follow up duration. Age, diabetes and decreased exercise capacity were risk factors for long‐term follow up. Conclusions: The prognostic value of MPS and Ex‐ECG results are strong for short‐term follow up but diminish over time and do not contribute significantly in multivariable models after 5 years. 7. An Analysis of the Structures at Risk from Percutaneous Pinning of Distal Radius Fractures and a Comparison of Two Pinning Techniques: A Cadaveric Study

Abstract

Background: A stab incision and blunt dissection prior to wire placement are believed to decrease the risk of injury to underlying structures during percutaneous pinning of distal radius fractures (DRF). However, only a few studies have compared stab incision and blunt dissection to direct wire placement. The aim of this cadaveric study is to analyse the structures at risk during percutaneous pinning of DRF and compare the two methods of wire placement. Methods: A total of 10 cadavers (20 upper limbs) were divided into two groups of five each. Five 2.0 mm Kirschner (K)-wires were inserted into the distal radius under fluoroscopic control in a standard fashion to simulate percutaneous pinning of DRF. In group 1, the K-wires were inserted directly, whereas in group 2, the wires were inserted after making a stab incision and blunt dissection to reach the bone. Each cadaveric limb was then dissected carefully to measure the distance of the K-wires from the branches of the superficial radial nerve (SRN), the cephalic vein and the first dorsal compartment and to determine the structures injured (pierced or in close contact) by the K-wires. Results: Out of the 100 K-wires placed, 18 wires were in close contact or pierced an underlying structure. These included 11 wires injuring tendons, six wires injuring branches of the SRN and one wire injuring the cephalic vein. Direct wire placement (group 1) resulted in injury to eight structures (44.4%) while stab incision and blunt dissection prior to wire placement (group 2) resulted in injury to 10 structures (55.5%). This difference was not statistically significant. Conclusions: Percutaneous pinning of DRF is associated with a high risk of injury to the extensor tendons and branches of the SRN. This risk is not reduced by making a stab incision and blunt dissection prior to K-wire placement.

Abstract

One of the key issues for SERS-based trace applications is engineering structurally uniform substrates with ultrasensitivity, stability, and good reproducibility. A label-free, cost-effective, and reproducible fabrication strategy of ultrasensitive SERS sensors was reported in this work. Herein, we present recent progress in self-assembly-based synthesis to elaborate precisely shaped and abundant gold nanoparticles in a large area. We demonstrated that shape control is driven by the selective adsorption of a cation (Na+, K+, and H+) on a single facet of gold nanocrystal seeds during the growth process. We studied SERS features as a function of morphology. Importantly, we found a correlation between the shape and experimental SERS enhancement factors. We observed a detection threshold of 10−20 M of bipyridine ethylene (BPE), which matches the lowest value determined in literature for BPE until now. Such novel sensing finding could be very promising for diseases and pathogen detection and opens up an avenue toward predicting which other morphologies could offer improved sensitivity. © 2021 by the authors. 11. Systemic thrombolysis with newer thrombolytics vs anticoagulation in acute intermediate risk pulmonary embolism: a systematic review and meta-analysis

Abstract

Abstract Background Randomized controlled trials (RCTs) comparing systemic thrombolysis to anticoagulation in intermediate risk pulmonary embolism (PE) have yielded mixed results. A prior meta-analysis on this topic had included studies that used lower than standard dose of thrombolytics and included thrombolytic agents that are no longer available. Hence, interpreting the findings of that paper is not valid in contemporary practice. Objectives We undertook a systematic review and meta-analysis of randomized controlled trials of systemic thrombolysis with newer thrombolytic agents vs anticoagulation in intermediate risk PE. Methods This systematic review and meta-analysis is reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) statement. Results Nine randomized controlled trials were included in the study. We did not find any difference in in-hospital mortality (RR: 0.79; 95% CI: 0.42–1.50; I2: 0) or risk of major bleeding (RR:2.08;95% CI: 0.98–4.42; I2: 23.9%) between systemic thrombolysis and anticoagulation. Systemic thrombolysis was associated with lower risks for vasopressor use (RR: 0.27; 95% CI: 0.11–0.64, I2: 0) and secondary/rescue thrombolysis (RR: 0.25; 95% CI: 0.14–0.45; I2: 0). But systemic thrombolysis was found to have an increased risk of intracranial hemorrhage (RR: 4.55; 95% CI: 1.30–15.91; I2:0). There was no difference in mechanical ventilation between the two groups (RR: 0.61; 95% CI: 0.31–1.19, I2:0). Conclusion In our meta-analysis of randomized controlled trials of systemic thrombolysis vs anticoagulation in intermediate risk PE, we did not find any difference in in-hospital mortality or overall risk of major bleeding. With systemic thrombolysis, we found lower risks for vasopressor use and need for secondary/ rescue thrombolysis and an increased risk of intracranial hemorrhage.

Abstract Aims In this study, we estimated the 30 day all‐cause and heart failure‐specific readmission rates, predictors, mortality, and hospitalization costs in patients with obstructive sleep apnoea admitted with acute decompensated heart failure with reduced ejection fraction. Methods and results This is a retrospective cohort study using the Agency of Healthcare Research and Quality's National Readmission Database for the year 2019. The primary outcome was the 30 day all‐cause hospital readmission rate. The secondary outcomes were (i) in‐hospital mortality rate for index admissions; (ii) 30 day mortality rate for index hospitalizations; (iii) the five most common principal diagnosis for readmission; (iv) readmission in‐hospital mortality rate; (v) length of hospital stay; (vi) independent risk factors for readmission; and (vii) hospitalization costs. We identified 6908 hospitalizations that met our study definition. The mean patient age was 62.8 years, and women comprised only 27.6% of patients. The 30 day all‐cause readmission rate was 23.4%. 48.9% of readmissions were due to decompensated heart failure. Abstract

The optical properties of gold and silver nanoparticles (NPs) dispersed in water and distributed in shape are investigated by introducing a shape distributed effective medium theory (SDEMT). This model takes into account the variation of depolarization parameter induced by a NP shape distribution. Simulations show that the shape distribution induces an inhomogeneous broadening and a decrease of the amplitude of the plasmon band. The number of plasmon bands and their positions depend on both the mean value of depolarization parameter and the NP material. By fitting the measured absorption spectra with the SDEMT, we unambiguously demonstrate that the depolarization parameter distribution, i.e., the shape distribution of nanoparticles can be deduced from absorption spectra. Abstract

Ticks are highly specialized distributed worldwide obligatehematophagous ectoparasites of mammals, birds, and reptiles. Amongst the species of ticks infecting cattle in India, Rhipicephalus microplus is the most widely distributed tick and established as the principal vectors of pathogens like Babesia and Anaplasma species (Ghosh and Nagar, 2014). Changes in the ovipositional and hatching pattern of these parasites are observed depending on the atmospheric conditions. Severalextrinsic factors Such as rainfall, temperature, humidity,floods, winds and physical damage affect the successful reproductive performance and survival of ixodid ticks after detachment from their hosts (Dipeolu, 1984). This study was aimed to evaluate the period of oviposition and hatching of eggs in engorged female ticks R.(B.) microplus collected from cattle in and around Indore.

19. Mortality and bleeding associated with the management of sub-massive pulmonary embolism: a systematic review and Bayesian network meta-analysis

Abstract

Abstract Current guidelines recommend anticoagulation (AC) for low and intermediate-risk pulmonary embolism (PE) and systemic thrombolysis (tPA) for high risk (massive) PE. How these treatment options compare with other modalities of treatment such as catheter directed thrombolysis (CDT), ultrasound assisted catheter thrombolysis (USAT), and administering lower dose of thrombolytics (LDT) is unclear. There is no study that has compared all these treatment options. We conducted a systematic review and Bayesian network meta-analysis of randomized controlled trials in patients with submassive (intermediate risk) PE. Fourteen randomized controlled trials were included, comprising 2132 patients. On Bayesian network meta-analysis, a significant decrease in mortality was noted in tPA versus AC. There was no significant difference between USAT versus CDT. For risk of major bleeding, there was no significant difference in relative risk of major bleeding between tPA versus AC and USAT versus CDT. tPA was found to have a significantly higher risk of minor bleeding and a lower risk of recurrent PE compared to AC. Systemic thrombolysis is associated with a significant reduction in mortality and recurrent PE compared to anticoagulation but an increased risk of minor bleeding. There was no difference in risk of major bleeding. Our study also shows that while the newer modalities of treatment for pulmonary embolism are promising, there is lack of data to comment on the purported advantages.

21. DRESS syndrome in the setting of oxacillin therapy—a call for better patient preparedness: a case report

Abstract

Abstract Background Drug reaction with eosinophilia and systemic symptoms syndrome is a rare but severe and potentially life-threatening hypersensitivity reaction, with significant morbidity and mortality. The clinical presentation of drug reaction with eosinophilia and systemic symptoms may include extensive skin rash, fever, lymphadenopathy, internal organ involvement, eosinophilia, and atypical lymphocytosis, most commonly due to drug-induced reaction. Our case is a rare occurrence of drug reaction with eosinophilia and systemic symptoms syndrome in the setting of oxacillin therapy. Case presentation A 55-year-old Caucasian male presented to the emergency department on account of acute onset, 2-day history of generalized pruritic rash with associated fever, occurring 3 weeks after commencing therapy with intravenous oxacillin for methicillin-sensitive Staphylococcus aureus bacteremia. He had no known drug allergies. Two days prior to hospitalization, he had a telehealth visit with the infectious diseases specialist on account of his rash, and was recommended to use oral diphenhydramine. However, with the onset of fever and persistence of his rash, he was advised to discontinue the oxacillin and present to the emergency department. On examination, he was febrile at 101.2 °F and had a generalized blanchable maculopapular and morbilliform rash involving the face, trunk, upper and lower extremities, but sparing the palms, soles, and oral mucosa. He had palpable nontender lymph nodes in the cervical and inguinal regions bilaterally. Laboratory studies revealed atypical lymphocytosis, eosinophilia, neutrophilia, and elevated serum transaminases. He was started on intravenous diphenhydramine and admitted to the in-patient medical service. On the second day of hospitalization, his fever resolved. However, his rash was persistent and generalized, as well as elevated transaminases and an abnormal cell count on the second day of hospitalization. To complete his 6-week course of antibiotics for methicillin-sensitive Staphylococcus aureus bacteremia, he was switched to an alternative therapy with cefazolin, and he was scheduled for weekly follow-up assessments following hospital discharge. Conclusions Healthcare providers should increasingly be aware of the significant morbidity and mortality attributable to drug reaction with eosinophilia and systemic symptoms syndrome and the potential medications which may incite such life-threatening reactions. Early recognition of drug reaction with eosinophilia and systemic symptoms syndrome and prompt institution of management strategies can promote improved clinical outcomes. Enhanced patient–provider communication strategies should be implemented to better prepare patients for the likelihood of such drug reactions, with the goal of improving patient-centered care and adherence with treatment strategies.

Abstract

The prevalence of chronic pain in the adult population ranges from 2 to 40%, with a median point prevalence of 15% (Manchikanti et al. 2009; Lihavainen et al. 2010). Amongst chronic pain the prevalence of chronic musculoskeletal pain is high, affecting one in four adults (Walsh et al. 2008). Musculoskeletal pain can arise from a variety of common conditions including osteoarthritis, rheumatoid arthritis, osteoporosis, surgery, low back pain, and bone fracture. It is estimated that among US adults, nearly 27 million have clinical osteoarthritis, 5.0 million have fibromyalgia, 4–10 million have carpal tunnel syndrome, 59 million have had low back pain in the past 3 months, and 30.1 million have had neck pain in the past 3 months (Lawrence et al. 2008). Chronic persistent low back and neck pain is seen in 25–60% of patients, 1-year or longer after the initial episode. A 1-year prevalence study of musculoskeletal pain in the Quebec working population found that for both men and women, back pain was the most frequent musculoskeletal symptom that had disturbed their activities during the past year (Leroux et al. 2005). Low back pain is the most prevalent of musculoskeletal conditions; it affects nearly everyone at some point in time and about 4–33% of the population at any given point (Woolf and Pfleger 2003).

Abstract

Summary: Background: Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods: A multimethods analysis was performed as part of the GlobalSurg 3 study—a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital. Findings: Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. Abstract

Solar is one of the most easily exploitable energy, it is moreover inexhaustible. His applications are many and are varied. The heating of the domestic water is one of the most immediate, simplest and also of most widespread exploitation of the solar energy. Algeria, from its geographical situation, it deposits one of the largest high sun surface expositions in the world. The exposition duration of the almost territory exceeds 2000 hours annually and can reach the 3900 hours (high plateaus and Sahara). By knowing the daily energy received by 1 m2 of a horizontal surface of the solar thermal panel is nearly around 1700 KWh/m2 a year in the north and 2263 KWh/m2 a year in the south of the country, we release the most important and strategic place of the solar technologies in the present and in the future for Algeria. This work consists to study, conceive and manufacture solar water heating with the available local materials so, this type of the energy will be profitable for all, particularly the poor countries.
