Abstract

The condition of arteriovenous fistula (AVF) blood flow is typically checked by using auscultation; however, auscultation should require a qualitative judgment dependent on the skills of doctors, and further attention to contact infection is required. For these reasons, this study developed a non-contact and non-invasive medical device to measure the pulse wave of AVFs by applying optical imaging technology. As a first step toward realization of the quantification judgment based on non-contact AVF measurement, we experimentally validated the developed system, whereby the hemodynamics of 168 subjects were visually and quantitatively evaluated based on clinical tests. Based on the evaluation results, the fundamental statistical characteristics of the non-contact measurement, including the average and median values, and distribution of measured signal-to-noise power ratio, were demonstrated. The clinical test results contributed to the future construction of quantified criteria for the AVF condition with the non-contact measurement.

Abstract

When performing cardiopulmonary resuscitation (CPR) on a patient with cardiopulmonary arrest in bed, it has often been recommended that a backboard be placed behind the patient. However, the effectiveness of the use of a backboard has not yet been adequately studied. As a result, there are differences in the criteria for the use of the backboard in the latest global guidelines. This study tested the usefulness of backboards on five different mattresses and the floor. The depth of manual chest compressions (MCC), the recoil of MCC, MCC success rate, and participant's fatigue were evaluated with and without backboards. The MCC depth was maximum on the floor. On the bed, MCC depths were significantly deeper with backboards inserted than without backboards on all five mattress types. Similarly, MCC recoil depth and success rate, were more effective with the backboard inserted. It is recommended that MCC be performed on the floor only when manpower, environment, time and patient condition permit. However, if MCC must be performed on the bed, it is more effective with backboards placed behind a patient's back on various types of mattresses. [ABSTRACT FROM AUTHOR]

Abstract

The purpose of the present study was to histopathologically and immunohistochemically investigate the distribution of proteoglycans in human periodontal ligament (PDL). Specimens from osteotomy and tooth extraction having healthy PDL were studied. Histologically, PDL consisted of fibrous tissues, involving a compact arrangement area and edematous or myxoid area. Immuno-histochemically, versican binding region (12C5), versican link protein (8A4) and biotinylated hyaluronic acid binding protein (B-HABP) were distributed in PDL. In addition, positive immunore-activity for 12C5 and 8A4 was stronger in the compact arrangement area than in the edematous or myxoid area. Reactivity for B-HABP was stronger in the edematous or myxoid area than in the compact fibrous area. These results suggest that versican and link protein are associated with fibrous tissues, whereas hyaluronic acid is related to the formation of edematous and/or myxoid tissue in human PDL. These substances may play a role in periodontal homeostasis by protecting against mechanical stress.

In this study, we examined whether carnitine deficiency was independently associated with increased tissue accumulation levels of AGE in HD patients.One hundred and twenty-nine HD patients underwent determinations of blood chemistries including serum level of carnitine. Tissue AGE levels were evaluated by measuring skin autofluorescence with an AGE-reader.Serum carnitine levels were significantly lower, while skin AGE levels were significantly higher in HD patients compared with healthy controls (P0.001). In univariate analysis, β(2)-microglobulin (β(2)-MG) and carnitine (inversely) were correlated with skin AGE levels. Multiple stepwise regression analysis revealed that carnitine levels were one of the independent determinants of skin AGE levels (P = 0.024). When β(2)-MG-adjusted skin AGE levels were stratified by serum carnitine levels, a statistical significance and dose-response relationship were observed (P = 0.043). Furthermore, skin AGE levels were one of the independent determinants of serum carnitine levels as well (P = 0.012).The present study demonstrated that decreased carnitine levels were independently associated with increased skin AGE levels in HD patients. Since carnitine is reported to inhibit the formation of AGE in vitro, our study suggests that supplementation of carnitine may be a therapeutic target for preventing the accumulation of tissue AGE and subsequently reducing the risk of CVD in HD patients.
