- Abstract
As the most readily adopted molecular screening test, low-pass WGS of maternal plasma cell-free DNA for aneuploidy detection generates a vast amount of genomic data. This large-scale method also allows for high-throughput virome screening. NIPT sequencing data, yielding 6.57 terabases of data from 187.8 billion reads, from 12,951 pregnant Turkish women was used to investigate the prevalence and abundance of viral DNA in plasma. Among the 22 virus sequences identified in 12% of participants were human papillomavirus, herpesvirus, betaherpesvirus and anellovirus. We observed a unique pattern of circulating viral DNA with a high prevalence of papillomaviruses. The prevalence of herpesviruses/anellovirus was similar among Turkish, European and Dutch populations. Hepatitis B prevalence was remarkably low in Dutch, European and Turkish populations, but higher in China. WGS data revealed that herpesvirus/anelloviruses are naturally found in European populations. This represents the first comprehensive research on the plasma virome of pregnant Turkish women., This study was funded by Scientific Research Projects Coordination Unit of Istanbul University . Primary testicular leiomyosarcoma. Can J Urol 2013; 20(2):6730-6733. Primary testicular leiomyosarcoma is an extremely rare tumor, and, to the best of our knowledge, only 20 cases in adults have been reported in the literature to date. Herein, we present a case of a 68-year-old man who complained of left scrotal swelling for 2 months. Radiological examination revealed a left testicular tumor with no metastases to other organs. A left inguinal orchiectomy was carried out and histopathologic examination revealed an intratesticular leiomyosarcoma. The patient was treated successfully by orchiectomy and received no adjuvant therapy. During follow up until 12 months after surgery, there has been no recurrence or metastases of the disease.

Schwannomas originate from the neural sheaths of peripheral nerves, and are frequent in the head, neck, extremities, mediastinum, and retroperitoneum. In spite of the rich innervations of the external genitalia, schwannomas are rare in the penis. We reported a case of penile schwannoma with clinical presentation and management. A sexually active 28-year-old man presented with a painless, slowly growing, nodular mass on the dorsum of penile shaft of at least 2 years' duration. A nodular fixed mass about 4.0 cm in length was palpated on the dorsal aspect of the proximal penile shaft. Preoperative diagnosis was made with ultrasonography and magnetic resonance imaging. The mass was completely removed with preserving dorsal penile neurovasculature. Pathologic examination of the mass showed a benign schwannoma. No erectile dysfunction and penile curvature were observed over the six months after the surgery. Schwannoma of the penis is a rare benign tumor that can be diagnosed with the help of ultrasonography and magnetic resonance imaging. It should be treated by complete resection. Erectile function can be preserved by preserving dorsal penile neurovasculature.

Objective: The chronicity of the problem in conductive pathologies, the lack of improvement with treatment and the progression of hearing loss are seen as an important criteria for using hearing aids. The aim of this study was to investigate the effect of conductive pathologies on temporal resolution and the use of hearing aids. Material and methods: Of the 70 patients with mild to moderate conductive hearing loss, 24 patients with bilateral, 22 patients with unilateral hearing aids users and 24 patients with no hearing aids were included in the study. 30 healthy subjects were included as a control group. After the complete audiological test battery, gaps in noise, duration pattern and frequency pattern tests were performed for temporal processing. Results: There was no significant difference in temporal processing test performance between bilateral and unilateral hearing aids users when compared with normal hearing. However, the results of patients with long-term conductive hearing loss who had never used hearing aids was significantly lower when compared with the hearing aid users group and the control group. Conclusions: As a result of this study, although the cochlea and auditory nerve are intact in long-term conduction pathologies, the existing hearing loss may lead to auditory deprivation and temporal processing may be affected. Early intervention with appropriate amplification in conductive pathologies may contribute to the temporal processing. [ABSTRACT FROM AUTHOR]

Soil amplification, vulnerability of buildings, natural period of soils and liquefaction are the main risk factors of a possible earthquake. On the other hand, before definitions of these risk factors, the distribution of the soil classes should be determined. By the way, the determination of the method of the soil classes in 3D is the main purpose of this study. Detailed geological, geotechnical and geophysical analysis have been performed such as standard penetration test (SPT) and index experiments. Eskisehir has 2 different soil groups known as recent alluvium, and Pleistocene aged old alluvium. Because of the difference in fluvial regime, some or all levels of the old alluvium are eroded and the recent alluvium has been sedimented on the area. Sometimes same soil classes with different alluvium formation could have a boundary in the area and this makes it difficult to determine the formation of the alluvium. Although old alluvium, generally, is a much stiffer formation, some levels of old alluvium have low SPT values. There are three ways to determine the vertical profile of the soil. One is the conventional way, the joining of the same soil classes' boundaries with engineering insight, depending on the law of superposition. The second one is 3D kriging of the soil classes depending on the geologic hierarchy and the third one is the indicator kriging. Context. The effects of COVID-19 on the adrenocortical system and its hormones are not well known. Objectives. We studied serum cortisol, serum adrenocorticotropic hormone (ACTH), and their ratio in hospitalized non-critically ill COVID-19 patients. Design. A prospective case-control study. Methods. The study participants were divided into 2 groups. Group 1 consisted of 74 COVID-19 patients. The second group consisted of 33 healthy persons. Early admission above hormones levels was determined and compared between the study groups. Besides that, COVID-19 patients were grouped according to their Glasgow Coma Score (GCS), CURB-65 score, and intensive care unit (ICU) requirement, and further sub-analyses were performed. Results. There were no significant differences in the mean age or gender distribution in both groups. In the patients' group, the serum ACTH concentration was lower than in the healthy group (p<0.05). On the other hand, the serum cortisol levels and cortisol/ACTH ratio of the patients' group were significantly higher than of the healthy controls (p<0.05, all). Further analyses showed that, although serum cortisol and ACTH levels were not high, the cortisol/ACTH ratio was higher in COVID-19 patients with low GCS (<15) than patients with normal GCS (=15) (p<0.05). In COVID-19 in patients with different CURB-65 scores, the cortisol/ACTH ratio was significantly different (p<0.05), while serum cortisol and ACTH were not different in groups (p>0.05). Serum cortisol levels and cortisol/ACTH ratio were higher but ACTH level was lower in the ICU needed COVID-19 patients than in patients who do not need ICU (p<0.05). Conclusion. Our pilot study results showed that the cortisol/ACTH ratio would be more useful than serum cortisol and/or ACTH levels alone in evaluating the adrenocortical system of COVID-19 patients. Identification of wood species with fast, reliable and non-destructive methods is highly important for forestry and wood-related industries. Near-infrared spectra of anatomically similar pine species (Pinus sylvestris L. and Pinus nigra J.F. Arnold) were taken and analysed by partial least squared discriminant analysis (PLS-DA) for comparing the efficiency of preprocessing methods. Raw data were subjected to multiple scatter correction (MSC), standard normal variate (SNV), Savitzky-Golay for derivatives (1st and 2nd Dr) and smoothing (Sm) and combination of these preprocessing methods (1st Dr, 1st Dr + SNV, 1st Dr + MSC, Sm + 1st Dr and Sm + 2nd Dr). The success of the models was determined by the accuracies of test sets that did not participate in the calibration phase. In this study, it was determined that not all the preprocessing methods improve the model performance. Smoothing with 1st derivatives (Sm + 1st Dr) enhanced 14.3% improvement and have the best performance (95%) for classification of pine species. For understanding modelled relationship, mean spectra and selectivity ratio were used. It was found that discrimination was held by the differences at their absorption, and the most important variables for wood classification were noted around 4000-7000 cm(-1).

Aim: To investigate the effectiveness of eTansiyon smartphone application in blood pressure control in patients with hypertension. Background: Global prevalence of hypertension and the burden of chronic illness care, especially in primary care, are increasing in world. We have developed eTansiyon to ensure the continuity of patient-physician relationship, so it may help to improve the lifestyle of patients with hypertension, increase their adherence to treatment and achieve the target blood pressure. Methods: This study was a non-randomized controlled study. The sample was selected by random sampling method among the patients registered in 6 Family Health Units (FHUs). Randomization was performed at the FHU level; the units were randomized to 4 control group (CG) and 2 intervention group (IG), so that randomization in this study was 2:1. Both groups were followed up for at least four months. In addition to CG, IG were provided to use eTansiyon. Obtained data were analysed to evaluate differences between groups at the beginning and end of the study, intra-group changes after follow-up and interaction between groups and follow-up period. Findings: The study was performed with 124 patients in CG and 61 patients in IG. At the end of the study, the average systolic blood pressure/diastolic blood pressure (SBP/DBP) of office and home was significantly lower in IG compared to CG (P < 0.001, MD 9.5 mmHg; P = 0.007, MD 3.8 mmHg; P < 0.001, MD 10.6 mmHg; P < 0.001, MD 4.8 mmHg, respectively), and it was found that the proportion of people with target blood pressure in IG was significantly higher than CG (P < 0.001, 49.2%(n = 30) and 22.6%(n = 28), respectively). Repeated measures ANOVA and generalized estimating equations results showed that follow-up period and interaction between groups were significant in terms of office and home SBP/DBP and target blood pressure level during follow-up period (P < 0.001, P < 0.001, P < 0.001, P < 0.001, P < 0.024, respectively).

Methods: Of the 120 participants studied, 29 were cochlear implant users, 26 were hearing aid users, 33 were non-device users with hearing loss, and 32 had normal hearing. Each individual included in the study WHOQOL–BREF Quality-of-Life Scale, Rosenberg Self-Esteem and Speech, Spatial Perception and Qualities of Hearing Scale was applied. Results: We found that the self-esteem level of the group with hearing loss, but without a device, was significantly lower than that of the group with a cochlear implant/hearing aid. There was a moderately statistically significant relationship between self-esteem, quality of life, speech perception, and hearing quality. Conclusions: Using hearing aid and cochlear implants increases self-esteem among people with hearing impairments. Self-esteem is associated with quality of life, speech perception, and hearing quality.

Purpose To investigate development of radiation pneumonitis (RP) in relation to pulmonary function, dosimetric factors, and transforming growth factor beta-1 (TGFβ1) expression in irradiated breast cancer patients. Methods A total of 49 breast cancer patients who received post-operative radiotherapy (RT) were evaluated in terms of pulmonary function tests (PFTs), quality of life (QoL), development of RP, dosimetric factors, cytokine levels, and lung highresolution computed tomography (HRCT) before and after RT. ROC analysis was performed for performance of dosimetric factors in predicting RP, while frequencies of single nucleotide polymorphisms (SNPs) genotyped for TGFβ1 (rs11466345 and rs1800470) were also evaluated. Results All cases with RP (10.2%) recovered clinically at the end of third post-RT month. PFT and HRCT parameters were similar before and after RT overall, as well as by RP and the radiation feld subgroups. ROC analysis revealed the signifcant role of the ipsilateral V5 (cutof value of 45.9%, p=0.039), V10 (29.4%, p=0.015), V20 (23%, p=0.017), and MLD (1200 cGy, p=0.030) in predicting RP. Higher post-RT TGFβ1 levels (p=0.037) were noted overall and in patients with RP. Patient and control groups were similar in terms of frequencies of SNPs genotyped for TGFβ1 (rs11466345 and rs1800470). EORTC QLQ-C30 and QLQ-BR-23 scores were similar in patients with vs. without RP. Conclusion Our fndings revealed signifcant role of dosimetric factors including MLD, V20 as well as the low dose-volume metrics in predicting the risk of RP among breast cancer patients who received post-operative RT. Implementation of RT, extent of radiation feld or the presence of RP had no signifcant impact on PFTs

Background & Objective: The aim of this study was to investigate the relationship of neutrophil to lymphocyte ratio (NLR) and other blood cells markers in craniocervical artery dissection patients and the dynamic changes of these biomarkers during the disease course, and effects on prognosis of the patients. Method: Cranial MR imaging, cranial and cervical MR angiography and DSA were examined, and diffusion weighted imaging (DWI) was performed to show the acute lesion(s). Results: Forty-six patients with craniocervical artery dissection were included in this study; they had a mean age of 42 years. Almost 60% of the patients (n=27) were admitted during acute state. Almost 2/3 of cases had extracranial dissection (n=31) and nearly 40% of the patients (n=18) had trauma. Neutrophil to lymphocyte ratio (NLR) was observed to be higher in the acute phase of the disease while the ratio decreased in the chronic phase. Low hemoglobin, high lymphocyte and a high NLR were found to have a negative correlation with the National Institutes of Health Stroke Scale (NIHSS). Conclusion: Elevation of neutrophil was higher in acute craniocervical artery dissection as a marker of acute inflammatory response. High NLR, low hemoglobin and high eosinophil levels were associated with worse prognosis and functional outcomes.

Giriş: Radyoterapinin doku üzerine birçok histopatolojik olumsuz etkisi bulunmaktadır. Temel etkilerinden biri vasküler ve perivasküler yapıları etkileyerek sirkülasyonu ve buna sekonder doku perfüzyonunu bozmasıdır. Özellikle bu vasküler değişiklikler mikrovasküler cerrahi başarısını, cerrahi planlamayı ve cerrahi sonrası klinik durumları etkileyebilmektedir. Bu retrospektif çalışmada preoperatif radyoterapi tedavisinin baş ve boyun serbest doku rekonstrüksiyonu yapılan hastalarda flep sağ kalımı, yara yeri komplikasyonu ve hasta klinik durumu üzerine etkilerini belirlemeyi amaçladık. Materyal ve Metod: 2015-2021 yılları arasında kliniğimizce, 61‘i preoperatif radyoterapi alan olmak üzere toplam 266 serbest doku ile baş ve boyun rekonstrüksiyonu yapılan hasta retrospektif olarak değerlendirildi. Preoperatif radyoterapi tedavisi alan ve almayan hastalar arasında demografik özellikler, flep kaybı oranı, operasyon ve iskemi süresi, pedikül uzunluğu, seçilen onarım tekniği, hastanede ve YBÜ’de kalış süresi, nüks oranı ve postoperatif cerrahi ve medikal komplikasyonlar kıyaslandı. Bulgular: Preoperatif radyoterapi alan hastalarda istatistiksel anlamlı olarak; hastanede kalış süresi(p:0,001), yaş (0,002), nüks ve ölüm oranı (p:0,001, p:0,025), medikal komplikasyon oranı (p:0,002), yara yeri enfeksiyonu (p:0,004) ve operasyon gerektiren dehissans oranının (p:0,015) yüksek olduğu, bu hastalarda ön planda non-osseöz, pedikülü uzun fasyakütan fleplerin kullanıldığı (p:0,001-p:0,001-p:0,001) saptandı. İskemi ve operasyon süresi, flep kaybı, seroma, hematom ve venöz yetmezlik oranı açısından her iki grup arasında anlamlı fark tespit edilmedi. Sonuç: Preoperatif radyoterapi baş ve boyun serbest doku rekonstrüksiyonunun sonuçlarına etki eden temel faktörlerden biridir. Literatürde preoperatif radyoterapi alan hasta grubunda flep kaybı oranın daha yüksek olduğu çalışmalar olduğu gibi flep kaybı oranının benzer olduğu çalışmalar da bulunmaktadır. Bizim çalışmamız flep kaybı oranında iki grup arasında anlamlı farkın olmaması nedeniyle bu ikinci çalışma grubuna destek olmaktadır. Öte yandan yara yeri enfeksiyonu, dehissans gibi cerrahi komplikasyonlar bu hasta grubunda daha sık görülür. Bu hastaların genellikle daha ileri evre tümör hastaları olmaları, yaşlarının daha ileri olması sebebiyle medikal komplikasyon oranı ve hastanede kalış süresi artmaktadır. Yine aynı şekilde nüks oranının bu hasta grubunda fazla olması flep seçimini de etkileyerek anatomik rekonstrüksiyondan çok yumuşak doku örtümünün sağlanmasına yönelik non- osseöz, uzun pediküllü fasyakütan fleplerin tercih edilmesine sebebiyet vermektedir.

Amaç: Son yıllarda meme rekonstrüksiyonuna yönelik talep gittikçe artmaktadır. Hastaların beklentilerinin de artması sonucu meme cerrahisinde onkoplastik ve rekonstrüktif tekniklerin gelişimi hız kazanmıştır. Mastektomi sonrası meme rekonstrüksiyonunda başarıyı değerlendirmede hasta memnuniyeti ve yaşam kalitesi en önemli iki değişkendir. Bu çalışmanın amacı; meme rekonstrüksiyonu sonrası onarım metodu, tek veya iki taraflı onarım uygulanması ve radyoterapi (RT) gibi etmenlere bağlı yaşam kalitesini ve hasta memnuniyetini karşılaştırmaktır. Gereç-Yöntem: 2013-2020 yılları arasında kliniğimizde mastektomi sonrası eşzamanlı implant bazlı veya otolog rekonstrüksiyonunu tamamlamış hastalar arasından preoperatif ve postoperatif 1.yıllarında Breast-Q (Rekonstrüksiyon Modülü) formu dolduran hastalar çalışmaya dahil edildi. Breast-Q formunu doldurmayan hastalar, takipten çıkmış hastalar, gecikmiş onarımlar ve doku genişletici uygulanan hastalar çalışma dışı bırakıldı. Hastaların yaşları, BMİ, sigara kullanım durumları, komorbiditeleri, mastektomi endikasyonları, mastektomi tipleri, rekonstrüksiyon tipleri ve adjuvan radyoterapi alma durumları hasta dosyalarından ve hastane verilerinden alınarak karşılaştırıldı. Tüm olgularda tümör rezeksiyonu Genel Cerrahi ekibi tarafından yapıldı. Bulgular: Toplam 128 hastanın ortalama yaşı 45.6, ortalama VKİ’si 26.08 olarak tespit edildi. 106(%82.8) hasta sigara içmiyorken, 106 (%82.8) hastanın da eşlik eden başka bir hastalığı bulunmamaktadır. Mastektomi operasyonları 115 (%89.8) hastada terapötik, 13(%10.2) hastada da profilaktik endikasyonlar ile gerçekleştirilmiştir. 103(%80.5) hastaya meme koruyucu mastektomi (MKM), 21(%16,4) hastaya deri koruyucu mastektomi (DKM) ve 1 (%0.8) hastanın tek memesine MKM, diğer memesine DKM uygulandığı ve 3(%2.3) hastaya da segmenter mastektomi operasyonu uygulandığı tespit edildi. 71 hastada tek taraflı (%55,5), 57 hastada iki taraflı (%44,5) onarım uygulandı. Mastektomi sonrası meme rekonstrüksiyonu; 72 (%56.2) hastanın otolog doku ile, 56 (%43.8) hastanın da implant ile yapıldı. 41 (%32) hasta rekonstrüksiyon sonrasında adjuvan RT alırken, rekonstrüksiyon sonrası flep veya implant kaybı da 8(%6.3) hastada tespit edildi. Hastaların preoperatif ve postoperatif BREAST-Q rekonstrüksiyon modülündeki alt başlıklardaki cevaplarına göre karşılaştırılmalar yapıldı. Hastaların, otolog onarımda preoperatif ile postoperatif karın görünümlerinden anlamlı derecede (p= 0.002) daha memnun olduğu; bilateral otolog onarım uygulanan hastalarda, postoperatif RT almayan hastaların RT alan hastalara göre nipple görünümünde (p= 0.117) memnuniyet farkı istatistiksel olarak anlamlı değilken; bilateral implant ile onarım uygulanan hastalarda, postoperatif RT almayan hastaların RT alan hastalara göre nipple görünümünden anlamlı derecede (p = 0.043) daha memnun oldukları görülmüştür. Bu bulgular dışında unilateral/bilateral, otolog onarım/implant ile onarım ve RT alan/RT almayan şeklinde sınıflandırılan hastalar arasında postoperatif memnuniyet açısından istatiksel olarak anlamlı derecede fark saptanmamıştır. Tartışma: Mastektomi sonrası RT alan hastalarda, her iki rekonstrüksiyon yöntemi için de oldukça yüksek komplikasyon oranları olduğu bilinmektedir. İmplant temelli rekonstrüksiyon, otolog rekonstrüksiyon ile karşılaştırıldığında; rekonstrüksiyon başarısızlığı gelişme riskinin daha yüksek ve postoperatif hasta memnuniyetinin de daha düşük olduğu bilinmektedir1.Çalışmamızda otolog onarım ve implant ile onarım arasında, hasta memnuniyeti açısından anlamlı derecede bir farklılık görülmemesi; göreceli olarak kısıtlı hasta sayısına bağlanabilir. Bizim verilerimize göre RT alan hastalar ile RT almayan hastalar, BREAST-Q bulgularından genel meme görünümüyle alakalı memnuniyet hususunda karşılaştırıldığında; anlamlı derecede farklılık tespit edilmemiştir. RT alan hastaların RT sonrası dönemde implant ile onarımlıların otolog doku ile onarımlılara göre nipple pozisyonu ile ilgili memnuniyetinin daha az olduğu gözlemlendi. RT’nin cilt ve cilt altı dokuda olduğu gibi kas ve kapsül dokusu üzerinde de etkilerine bağlı kas dokusunun kontraksiyonu sonucu oluşan kas-cilt yapışıklıları nedenli kliniğimizde subpektoral planda konulan implantların daha fazla nipple pozisyonda değişikliğe yol açtığını düşünüyoruz. Otolog doku ile onarımlı hastalarda pektoral adele veya kapsülün radyoterapiye bağlı değişimleri cilde yansımadığı için nipple pozisyonundaki değişimlerin daha az yaşandığı varsayılabilir. Bu veriler preoperatif planlama sırasında hastaların ayrıntılı bir şekilde bilgilendirilmesinin ve postoperatif dönemde özellikle implant ile onarımlılarda nipple’a yönelik ek prosedürlerin gerekebileceğinin anlatılmasının önemine işaret etmektedir. Sonuç: Sonuçlarımız, otolog doku ve implant ile rekonstrüksiyonun hastaların psikososyal ve cinsel açıdan memnun olma durumları ile meme görünümünden memnun olma durumlarını benzer derecede iyileştirdiğini gösterdi. Postoperatif RT alan hastalarda nipple görünümünden memnuniyetin azaldığı ve bu hastaların özellikle preoperatif dönemde nipple pozisyonu ile alakalı ayrıntılı bilgilendirilmesi gerektiği tespit edildi.

38. Relationship Between CHA₂DS₂-VASc Score on Admission and In-Hospital Major Adverse Cardiovascular Events in Patients Diagnosed With ST-Elevation Myocardial Infarction.

The CHA₂DS₂-VASc (congestive heart failure, hypertension, age, diabetes mellitus, stroke, vascular disease, sex) scoring system, which includes conventional risk factors of coronary artery disease, was originally created to quantify the risk of thromboembolism in patients with atrial fibrillation. This study evaluated the usefulness of this score to predict adverse outcomes in STEMI (ST-elevation myocardial infarction) patients without atrial fibrillation. Primary end points were identified as MACE (major adverse cardiovascular events) which included in-hospital death or cerebrovascular accident. MACE rate was 10% (193 patients). The CHA₂DS₂-VASc score was an independent predictor of MACE (95% CI, 2.31 [1.37-3.9]; P = .0016). Other independent predictors of MACE included heart rate (95% CI, 1.56 [0.97-2.50]; P = .0242), admission Killip class (95% CI, 24.19 [10.74-54.46]; P < .0001), admission creatinine level (95% CI, 1.54 [1.10-2.16]; P = .0024), peak CK-MB level (95% CI, 1.63 [0.98-2.70]; P = .0001), and no-reflow (95% CI, 2.45 [1.25-4.80]; P = .0085). A nomogram was developed to estimate the risk of in-hospital adverse outcomes for STEMI patients. The CHA₂DS₂-VASc score was an independent predictor of MACE in STEMI patients. Background: Chronic obstructive pulmonary disease (COPD) is a prevalent and preventable condition. Mesenchymal stem cell (MSC) therapy is being explored to aid in the regeneration of lung cells and airway structure, aiming to restore lung function., Aim: To examine varied responses of MSCs when cultured with peripheral blood mononuclear cells (PBMCs) from different COPD phenotypes, patients were grouped into ACOS, emphysema, and chronic bronchitis categories., Methods: PBMCs from these groups and controls were co-cultured with MSCs derived from dental follicles, revealing differing rates of apoptosis among COPD phenotypes compared to controls., Results: While the chronic bronchitis group exhibited the least lymphocyte viability (p<0.01), introducing MSCs notably enhanced viability across all phenotypes except emphysema, with the chronic bronchitis group showing the most improvement (p<0.05)., Conclusion: Stem cell therapy might reduce peripheral lymphocyte apoptosis in COPD, with varying responses based on phenotype, necessitating further research to understand mechanisms and optimize tailored therapies for each COPD subtype., Competing Interests: Declaration of Competing Interest We declare that we have no conflict of interest., (Copyright © 2024. - Abstract
Objective: Predictive value of systemic immune-inflammation index (SII) has been shown in clinical outcomes and complexity of coronary artery disease, acute coronary syndrome, and heart failure. We sight to evaluate value of SII in patients with lower extremity arterial disease (LEAD)., Methods: A total of 271 patients diagnosed with LEAD were included to our study. Blood samples of the patients were collected and analyzed for biochemical variables and complete blood count parameters. SII value of each patient was calculated. The complexity of atherosclerotic disease was classified according to Trans-Atlantic Inter-Society Consensus (TASC II) classification., Results: Patients with TASC C-D were older than patients in TASC A-B group (63.06 ± 9.24 years and 60.85 ± 8.75 years, respectively). Other co-morbidities were comparable in both groups. Hemoglobin level and lymphocyte count were significantly lower, neutrophil, platelet counts, and SII values were significantly higher in patients with TASC C-D disease compared to that of patients with TASC A-B disease. SII showed significant correlation with the severity of LEAD (r = 0.363, p < .001). SII value of 664.24 predicted TASC C-D disease with a sensitivity and specificity of 60.8% and 73.3%, respectively. Results of multivariate logistic regression analysis showed that SII had higher odds ratio compared to platelet, neutrophil, and lymphocyte counts., Conclusion: Higher SII may indicate probability of more complex LEAD. - Abstract
Objective: A novel posteriolateral surgical approach is described that will provide safe access to intradural and extradural lesions located in the anterior part of the spinal cord (SC) at the C1-2 level and to the odontoid in single session., Methods: A total of five cadavers and two dry C1 vertebrae were used in this study. The study involved obtaining computed tomography magnetic resonance imaging scans and magnetic resonance imaging of all cadaver groups before and after the procedures. Group 1: Control; Group 2: Unilateral C1 posterior arch was removed, the inferomedial part of C1 lateral mass was removed, and access the anterior and lateral aspects of the SC. Group 3: In addition, odontoid was removed, Group 4: In addition, unilateral C1- C2 screw was placed. Group 5: In addition, bilateral C1-C2 screw was placed., Results: The median distance from the midpoint of C1 posterior tubercle to vertebral groove which was removed in groups is 21.4 ± 2.88 mm. The average width of C1 lateral mass was 13.4±2.4 mm. After the lateral mass was drilled, its width decreased to 10,2 mm.This area was sufficient to open a surgical corridor and reach the anterior of SC and odontoid. After the procedure, no instability was found in group 2 without instrumentation on computed tomography and magnetic resonance imaging scans., Conclusions: It is possible to access the anterior C1-C2 via a posterolateral paramass approach by drilling 20%-30% of the lateral mass, providing an open pathway for easy intervention in C1-C2 intradural lesions. 42. Optimization of citrus fiber-enriched vegan cream cheese alternative and its influence on chemical, physical, and sensory properties.

- Abstract
Dairy product alternatives have increased in recent years as a result of medical prescriptions or personal preferences. The main purpose of the present study was to optimize vegan-based cream cheese formulation added with citrus fiber considering the textural and physicochemical properties of the samples. The physicochemical (pH value, water activity, and color), texture, microstructure, and sensory properties of manufactured vegan-based cream cheese were characterized and compared to those of a commercial one. Three optimized products were produced, according to the textural properties. The addition of citrus fiber did not affect the pH and water activity values of the cheese samples significantly. Although citrus fiber had an effect on the color values of the samples, a significant difference in the sensory scores was not recorded by the panelists. The sample having 1.21% citrus fiber (A) showed a hardness value similar to that of control sample and it received high sensory appreciation. The sample added with 1.41% citrus fiber (B) was scored high by the panelists, with no significant difference compared to commercial cream cheese, even though it showed high hardness. - Abstract
In this study, the effects of temperature (22, 24, 26, 28, and 30°C) and strain (0.1%, 1%, and 5%) on cocoa butter (CB) crystallization were investigated by oscillatory test, and the four-parameter Gompertz model was used to interpret the effect of parameters on pre-crystallization, nucleation, and crystal growth stages of CB. Lag time and growth rate were calculated using the Gompertz model using time-dependent storage modulus (G') data. According to the results, CB crystallization at 26°C with a 1% strain value had the highest growth rate value, the shortest lag time, and the formation of β
v polymorph type. Followingly, polymorphic types of the CB crystals were determined based on the melting points of polymorphs via the temperature ramp step, and the results obtained were correlated with a polarized light microscope. In conclusion, using a rheometer in both the observation of the pre-crystallization process and the determination of polymorph types is very important for research and development studies in the chocolate industry for process and formulation optimization. PRACTICAL APPLICATION: This study demonstrates the feasibility of a novel approach for investigating crystallization and oscillatory shear of CB using a rheometer, both for observing crystallization kinetics and determining polymorph type, accompanied by the Gompertz equation to model the crystallization kinetics. According to the results, the effect of process parameters (temperature and shear) on the crystallization behavior of CB can be observed by rheometer, which can provide a detailed perspective for chocolate manufacturers and researchers in research and development studies., (© 2024 The Authors. 44. Fruit-based vegan ice cream-type frozen dessert with aquafaba: effect of fruit types on quality parameters.

- Abstract
"Aquafaba", defined as legume cooking water, has a feature that can be used in various formulations as an egg and milk alternative in vegan products and improves functional properties such as foaming, emulsifying and gelling. In this study, it was aimed to investigate the usability of aquafaba in ice cream type frozen desserts containing different fruit purees (strawberry, nectarine and banana) by using its foaming capacity. Rheological properties, microstructure, overrun, melting characteristics, color measurement, dry matter, and sensory properties were investigated in different fruit-based frozen dessert samples. The brix value, density, protein content, foaming capacity (FC) and foaming stability (FS), flow behavior index, consistency coefficient, and overrun of aquafaba were determined as 7.1 ± 0°Bx, 1.022 ± 0.011 g/ml, 1.51 ± 0.41%, 85 ± 0% FC and 81 ± 0.23% FS, between 0.28 and 0.64, between 8.68 and 41.30 Pa·sn, between 116.75 and 395.93%, respectively. The dry matter content of the strawberry, nectarine, and banana-based dessert samples ranged between 17 and 48%, 20-49%, 25-50%, and the first dropping times were determined between 348 and 1538 s, 369-1689 s and 435-1985 s, respectively. - Abstract
Objectives: There is a lack of sufficient evidence regarding the use of extended shelf-life (ExSL) Yttrium-90 ( 90 Y) glass radiomicrospheres in metastatic colorectal cancer (mCRC) patients. We aimed to investigate the efficacy of ExSL 90 Y glass radiomicrospheres with a personalized treatment approach by analyzing 18 F-FDG PET/CT quantitative parameters [metabolic tumor volume (MTV) and total lesion glycolysis (TLG)] separately before and after the treatment., Methods: A total of 93 radioembolization sessions involving 77 patients were included. Simplicit 90 Y software was utilized to perform multicompartmental voxel-based dosimetry. Adverse events were recorded using the CTCAE v5.0 criteria. The survival data were recorded in detail., Results: The overall disease control rate was 84.9%, with a median overall survival (OS) of 12.7 months and median progression-free survival (PFS) of 8.3 months. A statistically significant increase in treatment response rate was observed when there was an increase in absorbed tumor dose for pre-treatment unit MTV ( P = 0.005) and TLG ( P = 0.004) values. We didn't observe any additional side effects/vital risks that could be considered clinically significant., Conclusion: Our study has provided evidence on the therapeutic effectiveness and safety in terms of dose-toxicity profile of ExSL 90 Y glass microspheres in a large cohort of mCRC patients. With a personalized treatment approach, the increase in radiation dose absorbed by the tumor has shown a significant contribution to treatment response rate, as indicated by quantitative measurements obtained through 18 F-FDG PET/CT., (Copyright © 2024 Wolters Kluwer Health, Inc. 46. Higher C-Reactive Protein to Albumin Ratio Portends Long-Term Mortality in Patients with Chronic Heart Failure and Reduced Ejection Fraction.

Background and Objectives : In this study, we aimed to investigate the prognostic value of the C-reactive protein to albumin ratio (CAR) for all-cause mortality in patients with chronic heart failure with reduced ejection fraction (HFrEF). Materials and Methods : In total, 404 chronic HFrEF patients were included in this observational and retrospective study. The CAR value of each patient included in this analysis was calculated. We stratified the study population into tertiles (T1, T2, and T3) according to CAR values. The primary outcome of the analysis was to determine all-cause mortality. Results : The median follow-up period in our study was 30 months. In the follow-up, 162 (40%) patients died. The median value of CAR was higher in patients who did not survive during the follow-up [6.7 (IQR = 1.6-20.4) vs. 0.6 (IQR = 0.1-2.6), p < 0.001]. In addition, patients in the T3 tertile (patients with the highest CAR) had a higher rate of all-cause mortality [n = 90 cases (66.2%), p < 0.001]. Multivariate Cox regression analysis revealed that CAR was an independent predictor of mortality in patients with HFrEF (hazard ratio: 1.852, 95% confidence interval: 1.124-2.581, p = 0.005). In a receiver operating characteristic curve analysis, the optimal cut-off value of CAR was >2.78, with a sensitivity of 66.7% and specificity of 76%. Furthermore, older age, elevated N-terminal pro-brain natriuretic peptide levels, and absence of a cardiac device were also independently associated with all-cause death in HFrEF patients after 2.5 years of follow-up. Conclusions : The present study revealed that CAR independently predicts long-term mortality in chronic HFrEF patients. CAR may be used to predict mortality among these patients as a simple and easily obtainable inflammatory marker.

- Abstract
Objective: To radiologically examine the pedicle, lamina, and vertebral artery foraminal anatomies at the C2 vertebra for pedicular and laminar screw instrumentation at the axis in a Turkish population., Methods: From 2018 to 2019, we evaluated 100 patients who underwent cervical computed tomography (CT) for various reasons (excluding cervical pathologies) at Marmara University Hospital. The C2 pedicles were measured on CT images using measurement tools. In addition, axial computed tomography was performed at 0.1 mm intervals. Bilateral measurements were performed for each case., Results: The median right and left pedicle axial diameters were 5.01 and 5.09 mm, respectively for the male patients and 4.31 and 4.38 mm for the female patients, showing a statistically significant difference between the sexes (P < 0.01). Of the patients, 15% had narrow pedicles. The pedicle sagittal diameters were smaller than 5 mm in 30% of the computed tomographic series. The internal height was <2 mm in 4% of the cases., Conclusions: Our findings suggest significant individual and sex-related differences. Vertebral artery groove anomalies are commonly observed. Before performing a posterior craniocervical instrumentation surgery, a computed tomography (CT) examination is beneficial because high-riding vertebral arteries must be kept in mind in determining the appropriate screw diameter and screw trajectory., (Copyright © 2024 Elsevier Inc. - Abstract
Stent under-expansion is a predictor of restenosis and stent thrombosis. It remains uncertain whether enhanced stent imaging (ESI) (CLEARstent) guidance can improve stent under-expansion. Our aim was to assess the effect of using ESI on stent under-expansion, after percutaneous coronary intervention (PCI) in a single center, cross-sectional observational study. Participants attending our cardiology clinic with stable angina or acute coronary syndrome, from March to September 2020 were recruited. A total of 164 patients who underwent post-PCI ESI (CLEARstent) were compared with 77 age- and sex-matched control patients. Post-procedural minimal lumen diameter (MLD) was calculated. The patients in the ESI-guided PCI group, had a median age of 61 (54-69 IQR 25-75) years and 76.8% (n = 126) were males. The patients in ESI-guided PCI group had a greater minimal lumen diameter compared with the X-ray guided PCI group (βeta coefficient:2.88 (95% CI:2.58-2.99) vs βeta coefficient 2.55 (95% CI 2.34-2.63), P < .001). 49. Adaptation of the professionalism mini-evaluation exercise instrument into Turkish: a validity and reliability study.

- Abstract
Background: There is an ongoing search for standardized scales appropriate for each culture to evaluate professionalism, which is one of the basic competencies of a physician. The Professionalism Mini-evaluation Exercise (P-MEX) instrument was originally developed in Canada to meet this need. In this study, it was aimed to adapt the P-MEX to Turkish and to evaluate the validity and reliability of the Turkish version., Methods: A total of 58 residents at Bakirkoy Dr. Sadi Konuk Training and Research Hospital were assessed with the Turkish version of P-MEX by 24 raters consisting of faculty members, attending physicians, peer residents, and nurses during patient room visits, outpatient clinic and group practices. For construct validity, the confirmatory factor analysis was performed. For reliability, Cronbach's alpha scores were calculated. Generalizibility and decision studies were undertaken to predict the reliability of the validated tool under different conditions. After the administration of P-MEX was completed, the participants were asked to provide feedback on the acceptability, feasibility, and educational impact of the instrument., Results: A total of 696 forms were obtained from the administration of P-MEX. The content validity of P-MEX was found to be appropriate by the faculty members. In the confirmatory factor analysis of the original structure of the 24-item Turkish scale, the goodness-of-fit parameters were calculated as follows: CFI = 0.675, TLI = 0.604, and RMSEA = 0.089. In the second stage, the factors on which the items loaded were changed without removing any item, and the model was modified. For the modified model, the CFI, TLI, and RMSEA values were calculated as 0.857, 0.834, and 0.057, respectively. The decision study on the results obtained from the use of P-MEX in a Turkish population revealed the necessity to perform this evaluation 18 times to correctly evaluate professionalism with this instrument. Cronbach's alpha score was 0.844. All the faculty members provided positive feedback on the acceptability, feasibility, and educational impact of the adapted P-MEX., Conclusion: The findings of this study showed that the Turkish version of P-MEX had sufficient validity and reliability in assessing professionalism among residents. Purpose: We investigated the relationship between hearing aid or cochlear implant use and self-esteem, quality of life, hearing quality, and speech perception., Methods: Of the 120 participants studied, 29 were cochlear implant users, 26 were hearing aid users, 33 were non-device users with hearing loss, and 32 had normal hearing. Each individual included in the study WHOQOL-BREF Quality-of-Life Scale, Rosenberg Self-Esteem and Speech, Spatial Perception and Qualities of Hearing Scale was applied., Results: We found that the self-esteem level of the group with hearing loss, but without a device, was significantly lower than that of the group with a cochlear implant/hearing aid. There was a moderately statistically significant relationship between self-esteem, quality of life, speech perception, and hearing quality., Conclusions: Using hearing aid and cochlear implants increases self-esteem among people with hearing impairments.
