2. Long-term follow-up of patients with Buerger's disease after autologous stem cell therapy.

Objective: We investigated the long-term results of autologous bone marrow mononuclear cells (ABMMNCs) implantation in patients with Buerger's disease (BD). Methods: Twenty-eight patients (25 males and 3 females) who had BD and critical unilateral limb ischemia were investigated between April 2003 and August 2005. The patients were administered multiple injections of CD34+ and CD45+ positive ABMMNCs into the gastrocnemius muscle, the intermetatarsal region, and the dorsum of the foot (n=26) or forearm (n=2) and saline injection into the contralateral limb. Results: The mean follow-up time was 139.6±10.5 months. No complication related to stem cell therapy was observed during the follow-up. The ankle-brachial pressure index evaluated at 6 months and 120 months was compared to the baseline scores (p<0.001 and p=0.021, respectively). Digital subtraction angiography (DSA) was performed for all patients at baseline, 6 months, and 120 months. The angiographic improvement was 78.5% and 57.1% at 6 and 120 months, respectively. Patients demonstrated a significant improvement in the quality of life parameters at 6 months compared to baseline (p=0.008) and 120 months compared to the baseline (p=0.009). The 10-year amputation-free rate was 96% (95% CI=0.71-1) in ABMMNC-implanted limbs and 93% (95% CI=0.33-0.94) in saline-injected limbs (p=1). Conclusion: Autologous stem cell therapy could be an alternative therapeutic method for BD at long-term follow-up. [ABSTRACT FROM AUTHOR]

Abstract ER stress is associated with a range of pathological conditions, among which, the ischemia/reperfusion injury is also found. The mechanistic array of links among the ER stress and thrombovascular diseases is poorly understood. The XBP1 gene is a transcription factor which modulates the ER stress response; and the XBP1 (−116 C/G) gene polymorphism causes an impairment of its positive feedback system. In the present study we investigated the prevalence of XBP1 gene (−116 C/G) polymorphism, separately among the patients with atherosclerosis, ischemic stroke and hyperhomocysteinemia. The G allele and the (−116 G/G) genotype of the XBP1 (−116 C/G) gene polymorphism were found to be a significant risk factor for the patients with Ischemic Stroke. Yet, this allele was seemingly less significant in case of patients with atherosclerosis and hyperhomocysteinemia. Hence, the XBP1 (−116 C/G) gene polymorphism and especially its involvement in a homozygous state are suggested to take active role in the ER stress related ischemia/reperfusion injury. Aort kapak hastalıkları, eksantrik ve konsantrik sol ventrikül hipertrofisi (SVH) ve sol ventrikül fonksiyonlarında değişikliklerle birliktelik gösterir. Aort darlığı (AD) olan hastalarda, sol ventrikül geometrisi ve sol ventrikül fonksiyonları arasındaki ilişki ve aort kapak replasmanı (AVR) sonrası sol ventrikül yeniden şekillenmesinin sistolik ve diyastolik fonksiyonlara etkileri genel olarak bilinmemektedir.Bu çalışmanın amacı izole AD olan hastalarda, değişik tipte ve ölçekteki kapaklarla yapılan AVR?nın sol ventrikül fonksiyonları ve şekillenmesi üzerine etkilerini araştırmaktır.İzole AD tanısı almış 85 hastaya, üç gruba ayrılarak (Grup 1, mekanik kapak AVR; Grup 2, stentsiz biyolojik kapak AVR; Grup 3, stentli biyolojik kapak AVR) AVR öncesi ve operasyondan 6 ay sonra, sol ventrikül geometrisi ve fonksiyonlarını değerlendirmek üzere ekokardiyografik inceleme yapıldı. Ortalama yaş dışında, demografik özellikler açısından gruplar arasında anlamlı fark yoktu. 31 hastaya mekanik kapak, 34 hastaya stentsiz ve 20 hastaya stentli kapak kullanıldı (ortalama yaş; 60,6±14,2; 70,8±4,9 ve 71,5±6,5 yıl). New York Heart Association (NYHA) fonksiyonel sınıflaması, transvalvüler gradiyent, aort kapak alanı, ejeksiyon fraksiyonu, sol ventrikül end-diyastolik ve end-sistolik çapları, septum kalınlığı ve Modifiye Simpson Yöntemiyle hesaplanan sol ventrikül kütle ve kütle indeksleri gibi klinik ve ekokardiyografik datalar, grupların preoperatif ve postoperatif karşılaştırması yapılarak değerlendirildi.Sonuç olarak; preoperatif ölçümlerle karşılaştırıldığında hastaların postoperatif NYHA fonksiyonel sınıfı ve ekokardiyografik parametreleri üç grupta da düzelme gösterdi. Ekokardiyografik ölçümlerdeki düzelme oranları açısından üç grup arasında anlamlı fark bulunamadı. Bu ölçümler; ejeksiyon fraksiyonu (Grup 1, %10,7±2,6; Grup 2, %13,2±2,8; Grup 3, %10,0±2,7 artış; p=0,52), sol ventrikül end-diyastolik çap (Grup 1, %7,3±1,1; Grup 2, %2,5±2,1; Grup 3,82%5,6±1,9 azalma; p=0,24), sol ventrikül end sistolik çap (Grup 1, %7,5±2,2; Grup 2, %4,1±3,5; Grup 3, %5,6±3,0 azalma; p=0,86), septum kalınlığı (Grup 1, %9,2±1,7; Grup 2, %8,1±2,0; Grup 3, %13,1±2,1 azalma; p=0,25) ve sol ventrikül kütle indeksi (Grup 1, % 18,0±2,7; Grup 2, % 20,7±1,7; Grup 3, % 17,2±2,0 azalma; p=0,27) olarak sıralanabilir. Ayrıca gruplardaki ?dar ölçekli protez? kullanılan hastalar seçilerek kendi aralarında aynı parametrelere göre karşılaştırılmıştır.Aort darlığı olan hastalarda AVR sonrası erken dönemde, fonksiyonel statusta ve ekokardiyografik ölçümlerde düzelme izlenmiştir. Stentsiz kapak kullanılan hastalarda, ejeksiyon fraksiyonu ve sol ventrikül kütle indeksi parametrelerinde daha belirgin bir düzelme olmakla birlikte, gruplar arasında istatistiksel olarak anlamlı fark bulunamamıştır. Bu sonuç, hasta sayısının yetersiz olması ve gruplardaki benzer ölçekli kapak kullanılan hasta sayılarının homojen olmayışı ile açıklanabilir.Anahtar Kelimeler: İzole aort darlığı, aort kapak replasmanı, sol ventrikül kütle indeksi, stentsiz kapak Aortic valve disease is associated with eccentric or concentric left ventricular (LV) hypertrophy and changes in LV function. The relationship between LV geometry and LV function and the effect of LV remodeling after aortic valve replacement (AVR) on diastolic and sistolic functions, in patients with aortic valve stenosis, are largely unknown.The aim of this study was to evaluate the effect of AVR on LV function and LV remodeling, comparing different types and sizes of prosthesis in patients with isolated aortic valve stenosis.Eighty five patients with isolated aortic valve stenosis were studied in 3 groups (Group 1, mechanical valve; Group 2, stentless valve; Group 3, stented valve) using echocardiographic investigation to assess LV geometry and LV function in a period of before and 6 months after AVR. There were no significant difference in demographic characteristics between groups, except mean age. 31 patients received mechanical valves, 34 stentless and 20 stented (mean age 60,6±14,2 vs. 70,8±4,9 and 71,5±6,5 years). We assessed clinical and echocardiographic datas including; New York Heart Association (NYHA) functional class, transvalvular gradients, aortic valve area, ejection fraction, left ventricular end-diastolic and end-systolic diameters, septum thickness and left ventricular mass and mass indexes calculated with Modified Simpson Tecnique; with comparing groups preoperative and postoperatively.As a result; NYHA functional classes and echocardiographic parameters showed an improvement in all groups, compared with the preoperative measurements. There were no significant differences between groups according to improvement ratio in echocardiographic measurements including ejection fraction (Group 1, %10,7±2,6; Group 2, %13,2±2,8; Group 3, %10,0±2,7 increase; p=0,52), LV end-diastolic diameter (Group 1, %7,3±1,1; Group 2,%2,5±2,1; Group 3, %5,6±1,9 decrease; p=0,24), LV end systolic diameter (Group 1, %7,5±2,2; Group 2, %4,1±3,5; Group 3, %5,6±3,0 decrease; p=0,86), septum thickness (Group 1, %9,2±1,7; Group 2, %8,1±2,0; Group 3, %13,1±2,1 decrease; p=0,25) and LV mass index (Group 1, % 18,0±2,7; Group 2, % 20,7±1,7; Group 3, % 17,2±2,0 decrease; p=0,27). Also, groups are divided as ?narrow sized prosthesis? and compared with each other according to the same parameters.From the early periods after AVR, patients with aortic valve stenosis showed an improvement in functional status and echocardiographic measurements. Stentless prosthesis showed a better improvement in ejection fraction and LV mass index but these results were not statistically significant. - Abstract
Objective: The purpose of this study was to describe the clinical/pathological features and the outcome of the surgical treatment of cardiac hydatid disease in our unit and other hospitals of Anatolia over a fifteen-years period., Methods: Between 1984 and 2001, fifty cases of surgically treated cardiac hydatid disease were identified by systematic literature search from Anatolia. Twelve patients were operated at the Department of Cardiovascular Surgery, Ankara University within the same period. Overall thirty-nine patients were female (mean age 29.8+/-14 years). Sixty-three percent of patients were complaining of dyspnea at the time of referral to the hospital and 22% presented with signs of acute coronary syndrome., Results: The most common cardiac location was the left ventricle (46.7%) followed by the interventricular septum, the right ventricle and atria. The most common procedure was controlled puncture and aspiration of the cyst content, its excision and closure of the resulting cavity, which were performed under cardiopulmonary bypass. Overall Anatolian operative mortality was 4.8% (3 patients). No late deaths but recurrence in one patient have occurred., Conclusion: In view of the lack of efficient alternative treatment options, we recommend surgical intervention even in asymptomatic patients in order to prevent the occurrence of lethal complications.

- Abstract
Cardiac transplantation has emerged as the therapeutic procedure of choice for patients with end-stage heart disease but the availability of donor organs remains major limiting factor allowing only 1% of the candidates actually become transplant recipients. New therapeutic strategies are under intensive research for patients who do not meet patient selection criteria for transplantation or who suffer from donor organ shortage. Even though there are significant developments in the medical treatment of cardiac failure, the prognosis of this condition continues to be poor. The 20-40% of the candidates for cardiac transplantation die while they are included in the waiting list. Twenty percent mortality has been reported within the first year of transplantation with additional yearly mortality of 5% following the first year. Financial constraints, complications of immunosuppression and functional failure of the transplanted hearts are other limiting factors. This has brought up the necessity to search for alternative surgical treatment methods besides increasing the waiting periods of transplant candidates. The aim of this report is to summarize other currently available therapeutic alternatives for patients with end-stage cardiac disease.
