Background and Aims: Published literature shows mixed reports of the benefits of nonsteroidal anti-inflammatory drugs (NSAIDs) on reducing colorectal polyps in patients with familial adenomatous polyposis (FAP). We conducted a systematic review and performed a meta-analysis to assess the impact of NSAIDs on colorectal polyp burden in patients with FAP. Methods: We searched PubMed, EMBASE, and Cochrane for randomized controlled trials (RCTs) comparing the effect of NSAIDs vs placebo on the percent change in polyp number and polyp size in patients with FAP. Mean differences between the 2 study arms were pooled using RevMan. The risk of bias (RoB) was assessed using the Cochrane Risk of Bias tool for RCTs, and certainty in the evidence was assessed using the Grading of Recommendations Assessment, Development, and Evaluation methodology. Results: The search strategy identified 1021 studies, out of which we included 8 RCTs with a total of 279 patients. Treatment for 6.4 ± 2.2 months with NSAIDs reduced polyp numbers by −17.4% (95% confidence interval −26.41%, −8.29%) (low certainty [I2 89%] due to imprecision and issues with RoB) and polyp size by −15.9% (95% confidence interval −24.98%, −6.73%) (very low certainty (I2 84%) due to imprecision, inconsistency, and issues with RoB). The most common gastrointestinal adverse events reported were stomatitis, diarrhea, and abdominal pain. Side effects leading to drug discontinuation were gastroenteritis and drug allergy. Conclusion: Short-term use of NSAIDs reduced polyp number and polyp size but with low to very low certainty of evidence. Further large multicenter studies are needed to further explore NSAIDs as a chemopreventive measure in patients with FAP.

Colorectal cancer (CRC) is one of the most common types of cancer with a high mortality rate. Colonoscopy is the preferred procedure for CRC screening and has proven to be effective in reducing CRC mortality. Thus, a reliable computer-aided polyp detection and classification system can significantly increase the effectiveness of colonoscopy. In this paper, we create an endoscopic dataset collected from various sources and annotate the ground truth of polyp location and classification results with the help of experienced gastroenterologists. The dataset can serve as a benchmark platform to train and evaluate the machine learning models for polyp classification. We have also compared the performance of eight state-of-the-art deep learning-based object detection models. The results demonstrate that deep CNN models are promising in CRC screening. This work can serve as a baseline for future research in polyp detection and classification.

Colorectal cancer is the third most common cancer diagnosed in both men and women in the United States. Most colorectal cancers start as a growth on the inner lining of the colon or rectum, called 'polyp'. Not all polyps are cancerous, but some can develop into cancer. Early detection and recognition of the type of polyps is critical to prevent cancer and change outcomes. However, visual classification of polyps is challenging due to varying illumination conditions of endoscopy, variant texture, appearance, and overlapping morphology between polyps. More importantly, evaluation of polyp patterns by gastroenterologists is subjective leading to a poor agreement among observers. Deep convolutional neural networks have proven very successful in object classification across various object categories. In this work, we compare the performance of the state-of-the-art general object classification models for polyp classification. We trained a total of six CNN models end-to-end using a dataset of 157 video sequences composed of two types of polyps: hyperplastic and adenomatous. Our results demonstrate that the state-of-the-art CNN models can successfully classify polyps with an accuracy comparable or better than reported among gastroenterologists. The results of this study can guide future research in polyp classification.

Introduction Smartphone-based applications (apps) have been used to improve the quality of bowel preparation (prep) but the success rates have been variable. We have performed a systematic review and meta-analysis to evaluate the impact of smartphone apps on bowel preparation. Methods Electronic databases (MEDLINE, Embase, CINAHL and Cochrane) were reviewed for eligible studies of smartphone apps versus standard education before colonoscopy. The following outcomes were analyzed: pooled rate of adequacy of bowel prep among both arms and Boston bowel preparation score (BBPS) when reported. Pooled analysis was reported as odds ratio (OR) or mean difference in random effect model with Review Manager 5.3 (P ≤ 0.05 for statistical significance). Results Six studies were eligible with smartphone app (810 patients) vs. standard education (855 patients, control group) for bowel prep. The smartphone app group had a higher proportion of adequate bowel prep compared to the control group: 87.5 % vs 77.5 % (five studies), pooled OR 2.67; 95 %CI 1.00 – 7.13 with P = 0.05. There was substantial heterogeneity in studies with I 2 = 78 %. When analysis was limited to randomized controlled trials (RCTs), smartphone app users had a numerically higher rate of bowel cleansing: 87.1 % vs 76.9 %; however, pooled OR was not statistically significant (OR 2.66, 95 %CI 0.92 – 7.69, P = 0.07). When studies using BBPS were evaluated (n = 3), smartphone app users had higher mean scores (better bowel prep) with a mean difference of 0.9 (95 %CI 0.5 – 1.3), which was statistically significant (P

Musa Balbisiana Colla Underground Stem (MBCUS) catalyst was treated thermally mixing with 5:1 w/w of Strontium Oxide (SrO) and the dynamic sites were reformed. The MBCUS-SrO showed sharper crystalline phases as evidence from XRD and TEM analysis. The composition and morphology were characterized from BET, SEM, EDX thermo-gravimetric analysis (TGA) and XRF analysis. The optimization process for biodiesel production from Jatropha curcas L oil (JCO) having high percentage of free fatty acids was carried out using orthogonal arrays adopting the Taguchi method. The linear equation was obtained from the analysis and subsequent biodiesel production (96% FAME) was taken away from the JCO under optimal reaction conditions. The biodiesel so prepared had identical characteristics to that with MBCUS alone, but at a lower temperature (200˚C) and internal vapour pressure. Metal leaching was much lower while reusability of the catalyst was enhanced. It was also confirmed that the particle size has little impact upon the conversion efficacy, but the basic active sites are more important.

The efficient and economic utilization of natural renewable and waste materials of various industries and biomass having non-homogeneous composition is a new dimension of research for biodiesel pro- duction. A combination of these renewable, waste materials and traditional heterogeneous catalyst can also be looked after for the possible solution of heterogeneous catalytic transesterification. This review discusses industrially derived and naturally occurring materials containing calcium, sodium, potassium etc, which were found instrumental for biodiesel production. About 60 research articles and patents have been reviewed and the findings are analysed in this article for developing industrial scale heterogeneous catalytic pilot plant facilities for biodiesel production. © 2015 by Authors, Published by BCREC Group. Ischemic colitis due to medications is common, and a number of cases have been described with pseudoephedrine as the culprit agent. We present here an interesting case of a healthy female with no risk factors who developed pseudoephedrine induced ischemic colitis. This case serves to remind the healthcare providers about the utmost importance of obtaining a comprehensive history to aid with the diagnosis.

Hydrodynamic studies of trickle bed reactors (TBRs) are essential for the design and prediction of their performance. The hydrodynamic characteristics involving pressure drop and dynamic liquid saturation are greatly affected by the physical properties of the liquids. In the present study experiments have been carried out in a concurrent downflow air - liquid trickle bed reactor to investigate the dynamic liquid saturation and pressure drop for the water (non-foaming) and 3% polyethylene glycol and 4% polyethylene glycol foaming liquids in the gas continuous regime (GCF) and foaming pulsing regime (FP). In the GCF regime the dynamic liquid saturation was found to increase with increase in liquid flow rate for non-foaming and foaming liquids. While for 3% and 4% polyethylene glycol solutions the severe foaming was observed in the high interaction regime and the regime is referred to as foaming pulsing (FP) regime. The decrease in dynamic liquid saturation followed by a sharp rise in the pressure drop was observed during transition from gas GCF to FP regime. However in the FP regime, a dip in the dynamic liquid saturation was observed. The pressure drop for foaming liquids is observed to be manifolds higher compared to non-foaming liquid in the GCF regime. ©2010 BCREC UNDIP. In many textile industries dyes are used as coloring agents. Advanced oxidation processes are used for degrading or removing color from dye baths. Catalysts play a key role in these industries for the treatment of water. Solid catalysts are usually composed of metals that form supports onto the surface and create metal particles with high surface areas. TiO2 composites containing transition metal ions (silver) and/or bentonite clay were prepared. Photocatalytic efficiencies have been investigated for the degradation of Orange G an azo dye. Various analytical techniques were used to characterize the surface properties of nano-sized titania modified using silver and/or bentonite clay. Scanning electron microscopy (SEM), Transmission electron microscopy (TEM), X-ray diffraction (XRD) and FTIR analyses showed that TiO2 (10 ± 2 nm) and Ag (2 to 3 nm) particles were supported on the surface of the bentonite clay and the size was in the range of 100 ± 2 nm. The modified catalysts P-25 TiO2/Bentonite/Ag and P-25 TiO2/Ag were found to be very active for the photocatalytic decomposition of Orange G. The percent decolorization in 60 min was 98% with both P-25 TiO2/Ag and P-25 TiO2/Bentonite/Ag modified catalysts. Whereas mineralization achieved in 9 hr were 68% and 71% with P-25 TiO2/Bentonite/Ag and P-25 TiO2/Ag catalyst respectively. © 2009 by Authors, Published by BCREC Group. The kinetic behavior of the heterogeneous hydrolysis of aqueous ethyl acetate over an acidic cation exchange resin, Amberlyst 15, was investigated. The experiments were carried out in a fixed bed reactor at temperatures from 313.15 to 343.15 K and feed molar ratios ,θBo (water to ethyl acetate) from 62.4 to 265.88. The conversion of ethyl acetate was found to increase with increasing reaction temperature. Gas bubble formation was observed at reaction temperature ≥ 343.15K. Absence of mass transfer resistance was verified by conducting the experiments at different catalyst loadings (20 g, 40 g, 64 g) under varied feed flow rates (1 to 25 cm3/min ). The kinetic data was correlated with pseudo first order model and model parameters were determined using Nelder-Mead algorithm for minimizing the objective function. © 2009 by Authors, Published by BCREC Group. Odontogenic keratocyst is a cyst of dental lamina origin with a high recurrence rate and well defined histologic criteria. Radiographically, odontogenic keratocyst presents as a unilocular or multilocular radiolucency of the jaws. Its capacity for aggressive growth and recurrence even after many years makes it an entity of special interest. Recently, neoplastic transformations have also been reported. They must therefore be distinguished from other cysts and tumors of the jaws. We report a case of fortuitous bilobular radiolucency in the left ascending ramus of a patient, who reported with a clinical presentation of submandibular space cellulitis.

Background: The American Society of Gastrointestinal Endoscopy (ASGE) published updated guidelines in 2009 to help endoscopists manage the treatment of their patients who have been prescribed antiplatelet therapy (APT). Study aim: To assess the use of APT among endoscopists, and to identify factors guiding their use of APT while treating their patients. Method: A survey questionnaire was distributed to endoscopists at two national meetings to assess their usage of APT while treating patients during the peri-endoscopic period. Results: The survey was provided to 400 attendees of whom 239 (60 %) responded. Only 30 % of respondents followed the ASGE guidelines for treating their patients and 26 % percent of respondents withheld all APT before engaging in any patient procedure. Background and study aims: It has been postulated that the endoscopic ablation of Barrett’s esophagus can lead to complete eradication of the disease. This study was undertaken to evaluate the efficacy of endoscopic eradication therapy for Barrett’s esophagus and the rates of recurrence of intestinal metaplasia. Patients and methods: As part of an initial randomized controlled trial, patients with nondysplastic or low grade dysplastic Barrett’s esophagus underwent mucosal ablation. Following ablation, the patients had annual surveillance endoscopies. Recurrence was defined as the presence of intestinal metaplasia after initial complete eradication had been achieved. Results: A total of 28 patients with Barrett’s esophagus were followed for a mean of 6.4 years after ablation therapy. At baseline, the majority of the patients had nondysplastic Barrett’s esophagus (79 %). Initial complete eradication of intestinal metaplasia was achieved at a mean of 4.1 months. During long-term follow-up, initial recurrence of intestinal metaplasia was seen in 14 of the 28 of patients (50 %) at a mean of 40 months, and further maintenance ablation therapy was applied. At the final follow-up, 36 % of the patients had complete eradication of intestinal metaplasia, 18 % of the patients had intestinal metaplasia, and 21 % had died of unrelated causes; invasive esophageal adenocarcinoma had developed in 1 patient. Conclusions: The long-term results of this study demonstrate a recurrence rate of 50 % after complete eradication of Barrett’s esophagus with endoscopic eradication therapy. In addition, re-recurrence (in 36 %), even after further maintenance endoscopic eradication therapy, and deaths unrelated to the disease (21 %) occurred. Complete remission of Barrett’s esophagus appears to be a difficult goal to achieve. These results call into question the role of ablation in patients with low risk Barrett’s esophagus.
