Hepatic hemangiomas are the most common type of benign liver tumors. We present a case of an infected hepatic hemangioma, which posed a diagnostic challenge. A 43-year-old female presented with right upper quadrant pain after blunt force trauma and intentional weight loss. CT and MRI were done, showing a posterior right lobe liver mass. Imaging characteristics were thought to be suggestive of metastatic disease, with a differential diagnosis of abscess. Drainage and biopsy of the mass revealed Streptococcus sp. infection, and pathology showed a cavernous hemangioma with inflammation. Patients presenting with systemic symptoms and an indeterminate liver mass on imaging should be evaluated for infected hepatic hemangiomas. Biopsy may be needed to rule out cancer. Management may include surgical resection; however, antibiotics and percutaneous drainage can suffice, as in this case.

Background: The recent pandemic of COVID19 that has struck the world is yet to be battled by a potential cure. Countless lives have been claimed due to the existing pandemic and the societal normalcy has been damaged permanently. As a result, it becomes crucial for academic researchers in the field of bioinformatics to combat the existing pandemic. Materials and Methods: The study involved collecting the virulent strain sequence of SARS-nCoV19 for the country USA against human host through publically available bioinformatics databases. Using in-silica analysis, reverse vaccinology, and 3-D modelling, two leader proteins were identified to be potential vaccine candidates for development of a multi-epitope drug. Results: It was revealed that the two leader proteins ORF1ab MT326102 and MT326715 had the highest extinction coefficient and the lowest score on the GRAVY. Along with the given parameters, these leader proteins were highly stable and were also antigenic in nature. The two selected epitopes were then docked against their respective alleles to obtain the global energy scores, which was the lowest of all possible pairs. Conclusion: The epitopes which displayed the lowest global energy score on docking with the alleles were selected and proposed as successful and potential vaccine candidates for multi-epitope vaccine development. Multidisciplinary care in healthcare is becoming the preferred approach to optimizing outcome. We present a case in which a patient developed a thrombotic occlusion of the mid left anterior descending coronary artery (very late stent thrombosis) leading to acute myocardial infarction while presenting for a dental extraction. Prior to the dental extraction his antiplatelet medication was held to minimize his risk of bleeding. This raises a question regarding the optimal use of antiplatelet medication during dental procedures. In complex cardiac patients we propose a multidisciplinary approach to antiplatelet management during the periprocedural period.

Dodecyl benzene sulphonic acid (DBSA) doped polyaniline (PANI) nanorods have been synthesized by a templateless route via in-situ emulsion polymerization of aniline in aqueous DBSA solution. XRD patterns, UV-Visible Spectroscopy, and FTIR analysis confirm the formation of PANI and PANI-DBSA, whereas FESEM images revealed their elongated rod shaped morphology. The chloroform dispersion of synthesized PANI-DBSA was spin coated over prefabricated interdigitated Pt patterned glass substrate to realize porous gas sensing active layer. The sensor displays good response in 1-1200 ppm ammonia concentration range and can repeatedly detect 300 ppm of ammonia with response time of 6 s, recovery time of 37 s and relative response value of 9.57. Besides, its response was found to be linear for 1-50 ppm ammonia concentration with relative response per ppm value of 0.01675.

To understand the spread of SARS-CoV2, in August and September 2020, the Council of Scientific and Industrial Research (India) conducted a serosurvey across its constituent laboratories and centers across India. Of 10,427 volunteers, 1058 (10.14%) tested positive for SARS-CoV2 anti-nucleocapsid (anti-NC) antibodies, 95% of which had surrogate neutralization activity. Three-fourth of these recalled no symptoms. Repeat serology tests at 3 (n = 607) and 6 (n = 175) months showed stable anti-NC antibodies but declining neutralization activity. Local seropositivity was higher in densely populated cities and was inversely correlated with a 30-day change in regional test positivity rates (TPRs). Regional seropositivity above 10% was associated with declining TPR. Aim: To observe psychological stress in the acoustic samples of daily-wage workers belonging to different states in India. Method: The study was based on the observations obtained from prior work on detecting psychoacoustic detectors of psychological stress. Acoustic samples were obtained from interviews conducted by NDTV throughout March–May 2020. Spectrogram and Fundamental Frequency variations were used to identify psychological stress. Results: Stark differences existed in mean fundamental frequency plots by daily-wage workers and non-stressed individuals. Distinct density variations were also observed in the acoustic samples between the two groups. Conclusion: Psychoacoustic detectors of psychological stress were confirmed in the acoustic samples of daily wage-workers, concluding that daily-wage workers are under dire psychological stress and are susceptible to stress-related disorders.

Recognizing human emotions by machines is a complex task. Deep learning models attempt to automate this process by rendering machines to exhibit learning capabilities. However, identifying human emotions from speech with good performance is still challenging. With the advent of deep learning algorithms, this problem has been addressed recently. However, most research work in the past focused on feature extraction as only one method for training. In this research, we have explored two different methods of extracting features to address effective speech emotion recognition. Initially, two-way feature extraction is proposed by utilizing super convergence to extract two sets of potential features from the speech data. For the first set of features, principal component analysis (PCA) is applied to obtain the first feature set. Thereafter, a deep neural network (DNN) with dense and dropout layers is implemented. In the second approach, mel-spectrogram images are extracted from audio files, and the 2D images are given as input to the pre-trained VGG-16 model. Extensive experiments and an in-depth comparative analysis over both the feature extraction methods with multiple algorithms and over two datasets are performed in this work. The RAVDESS dataset provided significantly better accuracy than using numeric features on a DNN.

For the past century, suicide has been associated with negative connotations to the liberalistic connotations, however, the fabric of logic and reason behind suicidal ideation continues to exist with countless wrinkles and folds. In my manuscript, a new approach to address the age-old question is provided – CogEpiCrim, which extends the boundaries of our understanding by using the suicidal ideation, not the action, as an infectious disease that can spread across closed communities. The paper describes how CogEpiCrim is used to classify the “existing wrinkles in our understanding as flaws in our approaches, not the fabric”.

Abstract We report a case of a 72 year old hypertensive male who developed severe hypertension followed by neurological deterioration in the immediate postoperative period after transurethral resection of prostate. While arterial blood gas and laboratory tests excluded transurethral resection of prostate syndrome or any other metabolic cause, reduction of blood pressure failed to ameliorate the symptoms. A cranial CT done 4 hours after the onset of neurological symptoms revealed bilateral gangliocapsular and right thalamic infarcts. Oral aspirin was advised to prevent early recurrent stroke. Supportive treatment and mechanical ventilation ensured physiological stability and the patient recovered completely over the next few days without any residual neurological deficit.

Jatropha curcas is a flowering, deciduous and poisonous plant. It is commonly found growing throughout the tropical & sub tropical regions of world particularly in India. Accidental poisoning due to its fruits & seeds has been reported in remote and tribal areas, invariably among children who often fail to recognize it. Sporadic cases often remain unreported, misdiagnosed or neglected; usually it is mass calamities which came to attention. Presented here is an unfortunate incident of mass poisoning, where some people (mostly children) consumed jatropha fruits & seeds mistaking them for edible sunflower seeds. Victims were hospitalized, promptly managed and recuperated successfully.

Public transit ridership offers valuable opportunities for modest amounts of daily physical activity (PA). Transit is a more feasible option for most Canadian commuters who live too far from work to walk or cycle, yet public transit usage in midsized Canadian cities has historically remained low due to inefficient transit service. The objectives of this longitudinal study were threefold: to assess whether the introduction of express transit service in the low-density city of Kingston, Ontario, has translated to greater transit use among a targeted employee group; to document the characteristics of those employees that have shifted to transit; and to examine the PA levels of employees using transit compared to other commute modes. An online survey was administered in October 2013 and October 2014 to all non-student employees at Queen's University. 1356 employees completed the survey in 2013, and 1123 in 2014; 656 of these employees completed the survey both years, constituting our longitudinal sample. Paxos is a prominent theory of state-machine replication. Recent data intensive systems that implement state-machine replication generally require high throughput. Earlier versions of Paxos as few of them are classical Paxos, fast Paxos, and generalized Paxos have a major focus on fault tolerance and latency but lacking in terms of throughput and scalability. A major reason for this is the heavyweight leader. Through offloading the leader, we can further increase throughput of the system. Ring Paxos, Multiring Paxos, and S-Paxos are few prominent attempts in this direction for clustered data centers. In this paper, we are proposing HT-Paxos, a variant of Paxos that is the best suitable for any large clustered data center. HT-Paxos further offloads the leader very significantly and hence increases the throughput and scalability of the system, while at the same time, among high throughput state-machine replication protocols, it provides reasonably low latency and response time.
