The follicular variant of Becker's nevus is an under‐reported entity. We present the rare occurrence of follicular Becker's nevus in 7 patients, confirmed through dermoscopy and histopathological examination. Dermoscopy shows perifollicular hypopigmentation surrounded by a well‐defined net‐like pigmentation corresponding clinically to the presence of folliculocentric macules. Histology shows prominent basal and suprabasal melanization surrounding the follicle, corresponding to well‐defined net‐like pigmentation seen on dermoscopy. However, the melanization does not extend along the entire length of the follicular epithelium leading to perifollicular hypopigmentation on dermoscopy. This study conducts the first meta‐analysis to assess the aggregated risk of malignancy associated with each category of the International System for Reporting Serous Fluid Cytopathology (ISRSFC) for reporting serous effusion cytology, while also evaluating diagnostic accuracy. PubMed/MEDLINE and Embase were systematically searched using the keywords "(pleural, peritoneal, and pericardial effusions) AND (serous effusion cytology) OR (International System for Reporting Serous Fluid Cytopathology)". Articles underwent risk of bias assessment using the QUADAS‐2 tool. After excluding inadequate samples, a meta‐analysis determined sensitivity and specificity for different cutoff points, including "atypical considered positive," "suspicious of malignancy considered positive," and "malignant considered positive." Summary receiver operating characteristic curves assessed diagnostic accuracy, and the diagnostic odds ratio was pooled. Sixteen retrospective cross‐sectional studies, totaling 19,128 cases, were included. Sensitivity and specificity for the "atypical and higher risk categories" considered positive were 77% (95% confidence interval [CI], 68%–84%) and 95% (95% CI, 93%–97%) respectively. For the "suspicious for malignancy and higher risk categories" considered positive, sensitivity and specificity were 57% (95% CI, 49%–65%) and 100% (95% CI, 99%–100%) respectively. Sensitivity and specificity for the "malignant" category considered positive for malignancy were 70% (95% CI, 60%–77%) and 99% (95% CI, 98%–99%), respectively. The pooled area under the curve ranged from 85% to 89.5% for each cutoff. This meta‐analysis underscores the ISRSFC's accuracy in reporting serous fluid cytology. It emphasizes the diagnostic importance of the "suspicious" and "malignant" categories in identifying malignancy, and the role of the "benign" category in ruling out malignancy. A 45‐year‐old male presented with a 2‐year history of erythematous indurated plaques on the right palate and adjacent palatobuccal mucosa, accompanied by dull pain and loosening of the third molar, necessitating extraction. Radiological imaging revealed soft tissue density with cortical bony erosions in the bilateral upper alveolar ridge. A single sub‐centimetric right cervical lymph node was palpable. Biopsy showed an upper dermal infiltrate composed of cells with nuclear grooving and abundant eosinophilic cytoplasm. Through this case, we highlight a rare manifestation of a systemic disease. An 18-year-old male presented with a non-healing penile ulcer, initially diagnosed as a syphilitic chancre but later confirmed to be extranodal NK/T-cell lymphoma. ABSTRACT Background Methods Results Conclusions Xeroderma pigmentosum (XP) is a genodermatosis, characterized both by premature aging and significantly increased risk of non‐melanoma and melanoma skin cancers at an early age. However, limited literature is available on the common histopathological subtypes of basal cell carcinoma (BCC) in these patients.In this ambispective case series, we recruited patients of XP who had either a currently present skin tumor or previously excised one, provided their histopathological sections were available. Cytogenetic analysis was performed in all the patients. The histopathological findings were recorded by two trained dermatopathologists.Of 22 recruited patients with XP, 12 patients had 41 BCCs, and 23 (56.09%) of them showed a high‐risk histopathological subtype. Basosquamous carcinoma (10, 24.39%) was the most frequent high‐risk tumor followed by infiltrative variant (7, 17.07%). Mixed‐histopathological subtypes were noted in six (14.63%) cases, and all of them had a high‐risk component. The low‐risk subtypes included nodular (n = 17, 41.46%) and superficial BCC (1, 2.43%). Though a deep dermal involvement was seen in 27 (65.85%) lesions, none showed a perineural invasion or necrosis. Three (7.31%) lesions showed ulceration, while 10 (24.39%) showed presence of pigmentation.A frequent occurrence of high‐risk basal cell carcinoma was noted in patients with XP‐mandating creation of appropriate treatment and follow‐up strategies in these patients. Though she was rotating the injection sites and using a fresh needle each time, she reported that the injection site used to develop some perilesional erythema followed by healing with hyperpigmentation, unlike before. This recent onset metabolic derangement along with the unusual pattern of hyperpigmentation made us question her insulin injection technique.
