Objective: This study aims to retrospectively examine the demographic characteristics, main causes of pain, diagnoses and applied treatment methods of patients who applied to a pain clinic. It is expected that these data will contribute to the evaluation of current clinical practices and the development of more effective pain management strategies.Methods: The study was conducted with a retrospective design among patients who presented to our clinic between January 2023 and December 2023. During this period, the medical records of the patients who applied to the pain clinic were examined. The patients' (age, gender), reasons for application (pain localization) and treatment approaches (drug therapies, interventional procedures) were recorded.Results: When demographic characteristics were examined, 56.8% of the 1210 patients included in the study were female, 43.2% were male, and the mean age was determined as 47.42±20.56. When the complaints of the patients were examined, the reasons for application were back pain, leg pain and widespread body pain in order of frequency. In terms of treatment methods, the most frequently performed procedure was the plan block, followed by epidural analgesia. Conclusion: In our study, it was observed that the majority of patients applying to the pain clinic applied due to chronic pain and needed multidisciplinary treatment approaches. These data are important for the development of new strategies for pain management in clinical practices and for increasing patient satisfaction. It is recommended that more studies be conducted, especially on the effectiveness of interventional methods and the role of psychological support.

Objective of the Study: Fentanyl and atropine are drugs used for analgesia during induction and for premedication, respectively. The aim of this study was to observe the effect of atropine used for premedication on cough developing in association with fentanyl use during anesthesia induction. Methods: This was a single-blind, prospective, multi-arm, parallel, randomized clinical trial involving 120 patients between ages 3 and 15 years undergoing general anesthesia. Patients were randomized into three groups: a control group (Group C), an atropine group (Group A), and a lidocaine group (Group L). Results: Incidence of cough was 45% (n = 16) in Group C, 16% (n = 6) in Group A, and 16% in Group L (n = 6) (p = 0.009). Cough was moderate in two cases in Group A and in two in Group L, and mild in all others in these groups. In Group C, cough was mild in seven cases, moderate in six, and severe in two. Conclusion: We determined that premedication with atropine suppressed the incidence and severity of fentanyl-induced cough during induction of general anesthesia. We think that, due to its antimuscarinic effect, atropine is as effective as lidocaine as a premedication.

This study aimed to compare the effectiveness of dextrose prolotherapy, ozone therapy, and home exercise programs in patients with knee osteoarthritis. Seventy-five patients with knee osteoarthritis were divided into three groups, with 25 in each group. At week 0 (baseline), week 3, and week 6, 12.5% dextrose (intraarticular and periarticular) was administered to the dextrose prolotherapy group. At week 0 (baseline), week 1, and week 2 15 μg/mL ozone (intraarticular and periarticular) was administered to the ozone therapy group. Both groups were also given a home exercise program. The third group was given a home-based exercise therapy program for 12 weeks. All groups were evaluated for VAS, WOMAC, TUG, ROM-active, and ROM-passive values at weeks 0 (baseline), 6, and 12. Ozone therapy more effectively improved VAS scores than dextrose prolotherapy and VAS and WOMAC scores than home-based exercise therapy in the 6th week. Ozone therapy also more effectively improved VAS and WOMAC-stiffness scores than dextrose prolotherapy and VAS, WOMAC, and ROM-active scores than home-based exercise therapy in the 12th week. Both dextrose prolotherapy and ozone therapy are effective in knee osteoarthritis treatment. Ozone therapy should be used in suitable cases rather than dextrose prolotherapy.

Brain death is characterized by irreversible loss of all brain functions and it is defined as a clinical condition where intracranial circulation stops. Apnea test is one of the tests which is used in the clinical diagnosis of brain death, absence of brain stem reflexes, and as a complement to absence of motor response to painful stimulants. Apnea test is a test which has a high complication rate. Complications such as hypoxia, hypotension, acidosis, cardiac arrhythmia, asystole, pneumothorax, pneumomediastinum and pneumoperitoneum may develop during an apnea test. In this case report, a patient with biletarel pneumothorax due to air trapping because of an oxygen cannula blocking the airway.

Abstract Open surgery for developmental dysplasia of the hip (DDH) may cause severe pain due to tenotomies and pelvic–femoral osteotomies. This study aims to evaluate the analgesic effect of ultrasound-guided Lumbar Erector Spina Plane (L-ESP) Block in pediatric patients undergoing DDH surgery. Sixty children scheduled for DDH surgery were randomly assigned into two groups. Group I (n = 30) received L-ESP with 0.5 mL/kg of 0.25% bupivacaine before surgery. In contrast, Group II (n = 30), as the control group, received the same volume of saline injection. Identical postoperative analgesia protocol was adjusted for both groups. Parental satisfaction, pain levels, ibuprofen, and opioid consumption were recorded. Pain levels were evaluated with the FLACC (Face, Legs, Activity, Crying, Consolability) scale. Background: The lateral sagittal brachial plexus block is the most used method for pediatric upper extremity surgery, whereas the applications of costoclavicular brachial plexus block are limited. This study aimed to compare the lateral sagittal and costoclavicular approaches for the ultrasound-guided infraclavicular block in pediatric patients. Methods: Sixty pediatric patients aged 5...15 years undergoing hand or forearm surgery were randomly assigned to two groups. Group LS (n.ß=.ß30) received ultrasound-guided lateral sagittal block, and Group CC (n.ß=.ß30) received ultrasound-guided costoclavicular block. The block performing time, needling time, imaging time, needle visibility, number of passes, sensorial/motor block time, and postoperative pain scores were evaluated. Results: The needling time (82.90.ß...ß28.17 seconds vs. 64.77.ß...ß28.11 seconds respectively, p.ß=.ß0.004) and total block performance time (109.53 .. 29.75 seconds vs. 89.70 .. 29.98 seconds respectively, p.ß=.ß0.005) were significantly longer in Group LS than in Group CC. However, there was no significant difference between the groups in imaging time, needle visibility, number of passes, sensorial/motor block time, and postoperative pain scores (p > 0.05). Conclusions: Costoclavicular and lateral sagittal brachial plexus blocks resulted in similar anesthetics effects. Moreover, the costoclavicular method can be a better alternative to lateral sagittal as it has a shorter block performance time.

Abstract Background Varicocele occurs as a result of dilatation of the pampiniform plexus in the spermatic veins. In this study, our primary aim was to evaluate the effect of Transversalis Fascia Plane Block (TFPB) on pain scores in the postoperative period in patients undergoing varicocelectomy surgery, and our secondary aim was to evaluate the effect of TFPB on analgesic consumption. Methods The study was initiated following local ethics committee approval, and sixty ASA I-II patients > 18y scheduled to undergo varicocelectomy and who consented to participation were enrolled. Before the procedure, the patients were randomly assigned two groups: Transversalis Fascia Plan block group (Group TFPB) or surgical incision site infiltration group (Group I).All surgeries were carried out under general anesthesia, and microsurgery using the subinguinal approach. After surgical suturing, TFPB and local infiltration blocks were applied prior to termination of anesthesia.For each block, 20 mL of 0.25% bupivacaine was utilized. Patients' demographic information, passive and active VAS ratings after surgery, usage of non steroidal anti-inflammatory medications and rescue analgesia, and the requirement for rescue analgesia, were recorded. Abstract Introduction: The erector spinae plane block is a newly described and effective interfascial plane block for thoracic and abdominal surgery. This case report describes a patient with multiple rib fractures undergoing ultrasound-guided continuous erector spinae plane block for analgesia. Case report: A 37-year-old male patient was taken for surgical fixation of multiple rib fractures. At the end of the surgery, using ultrasound-guided longitudinal parasagittal orientation 3 cm to the lateral aspect of the T5 spinous process and an in-plane technique, 20 mL 0.25% bupivacaine was administered between the erector spinae muscle and the transverse process, and a catheter was then inserted in the same plane. Before the end of surgery, 1 g paracetamol and 50 mg dexketoprofen were administered. Postoperative analgesia was applied with patient controlled analgesia method using 0.25% bupivacaine via the catheter. The patient's Visual Analogue Scale score at rest in the first 24 h was 0. The patient was monitored for 3 days with Visual Analogue Scale < 4, and the catheter was removed on postoperative day 4. No opioid requirement other than paracetamol and dexketoprofen occurred during this time. No postoperative complications were recorded. Discussion: The erector spinae plane block is an alternative to paravertebral, intercostal, epidural or other regional techniques. It may be a suitable technique in anesthesia and algology practice due to providing analgesia in the postoperative period with a catheter in the erector spinae plane.

Introduction: Hip surgery is a major surgery that causes severe postoperative pain. Although pain during rest is usually considerably reduced mobilization is important in terms of thromboembolic complications. The quadratus lumborum block is a regional analgesic technique that blocks T6-L3 nerve branches. This block may provide adequate analgesia and reduce opioid consumption after hip surgery. Case report: We performed continuous quadratus lumborum type 3 block in two patients who underwent hip arthroplasty. Postoperative 24-h pain scores, local anesthetic consumptions on patient-controlled analgesia and additional analgesic requirement were recorded. In two patients, postoperative pain scores were less than 6 during rest and physiotherapy. Patient was mobilized in the early postoperative period without additional opioid analgesic requirement and without muscle weakness. Discussion: Continuous quadratus lumborum block may be used to relieve postoperative acute pain in hip surgery because it provides one-sided anesthesia without muscle weakness. Embora a dor durante o repouso seja consideravelmente reduzida, a mobilização é importante em termos de complicações tromboembólicas. O bloqueio do quadrado lombar é uma técnica analgésica regional que bloqueia os ramos nervosos de T6-L3. Esse bloqueio pode fornecer analgesia adequada e reduzir o consumo de opioides após cirurgias de quadril.
