PURPOSE: The purpose of the study was to evaluate the quality of life (QOL) improvement in evaporative dry eye patients after treatment with intense pulsed light (IPL) therapy compared to punctal plug insertion. METHODS: A prospective comparative interventional study included 30 patients with moderate-to-severe evaporative dry eye. Patients' QOL affection before and after treatment was assessed by the Ocular Surface Disease Index (OSDI) questionnaire. Fifteen patients were treated with IPL therapy (Group 1). The other 15 patients were treated with silicone punctal plug insertion (Group 2). RESULTS: Improvement of patients' QOL was noticed in all patients of Group 1, with significant improvement of OSDI score from a mean value of 56.9 to 22.9 (P = 0.001). Improvement was noticed in 80% of Group 2 patients (mean OSDI score pretreatment: 53.8 and posttreatment: 31.7, P = 0.017). The tear breakup time (TBUT) in Group 1 significantly improved from a mean of 3.2 s to 5.9 s (P = 0.001), whereas it showed no significant changes in Group 2 (mean TBUT pretreatment: 3.6 s and posttreatment: 3.9 s, P = 0.654). Complications occurred in 13.3% in Group 2, including punctal granuloma and proximal canalicular obstruction. No adverse effects were recorded in Group 1. CONCLUSION: IPL therapy had better results with more improvement of patients' QOL compared to punctal plugs. It also showed a better safety profile with no reported complications. more...

Background Recurrent retinal detachment is one of the major complications after silicone oil (SO) removal. Therefore, finding ways to decrease its incidence would be of great significance. Purpose The aim of this study was to compare the rate of retinal re-detachment following SO removal in eyes that underwent scleral self-indentation during chandelier-assisted vitreous base shaving compared to the eyes that underwent scleral indentation by an assistant. Patients and methods This is a retrospective study that included 381 eyes of 381 consecutive patients who underwent SO removal between December 2017 and December 2021, after a successful pars plana vitrectomy for primary rhegmatogenous retinal detachment. Scleral self-indentation (group A) was performed in 186 eyes, while scleral indentation by an assistant (group B) was performed in 195 eyes. The primary outcome measure was the rate of retinal re-detachment after SO removal. The secondary outcome measures were the rate of iatrogenic peripheral retinal tears and intraoperative complications. Results Retinal re-detachment occurred in 21 (11.29%) eyes in group A compared to 38 (19.49%) eyes in group B, with a statistically significant difference (P=0.027). Iatrogenic retinal tear during vitreous base shaving occurred in two (1.08%) eyes in group A compared with 17 (8.71%) eyes in group B, with a statistically significant difference (P=0.001). Slipped infusion cannula during indentation occurred in two (1.02%) eyes in group B and resulted in serous choroidal detachment in one eye, a complication that was not reported in group A. Lens touch occurred in three (1.61%) eyes in group A versus eight (4.1%) eyes in group B, a statistically nonsignificant difference (P=0.065). Conclusion Scleral self-indentation during chandelier-assisted vitrectomy facilitated safe and satisfactory vitreous base shaving with a lower risk of retinal re-detachment after SO removal. Purpose The aim of this study was to evaluate the efficacy of intravitreal injection of Ziv-aflibercept before small-gauge pars plana vitrectomy compared to pars plana vitrectomy alone in eyes with advanced diabetic eye disease. Patients and methods This is a prospective, randomized, interventional study that enrolled 60 eyes of 46 patients with advanced diabetic eye disease between February 2019 and December 2020. Intravitreal Ziv-aflibercept injection was given in 30 eyes before vitrectomy, while the other 30 eyes served as the control group (without intravitreal injection). The recorded data included intraoperative bleeding, surgical time, endodiathermy use, iatrogenic retinal breaks, and incidence of postoperative vitreous hemorrhage at 1 week, 1 month, 3, 6, and 12 months postoperatively. Results Intraoperative bleeding was noted in 18 (60%) eyes in the study group compared to 26 (86.66%) eyes in the control group, with no statistically significant difference (P=0.39). However, grade 2 intraoperative bleeding was reported significantly less in the study group (eight eyes, 26.66%) than in the control group (20 eyes, 66.66%) (P=0.004). At least one intraoperative iatrogenic retinal break was detected in 10 (33.33%) eyes in the study group compared to 18 (60%) eyes in the control group (P=0.44). The mean surgical time was significantly shorter (51.67±7.30 min) in the study group than in the control group (63.17±10.16 min) (P0.05). Conclusion Preoperative intravitreal Ziv-aflibercept significantly decreased the surgical time and reduced the rate of severe intraoperative bleeding during diabetic vitrectomy. more...
