Background: Total hip replacement (THR) is considered a low-risk and efficient procedure, to improve pain and disability not exposing patients to significant risk burden. Minimally invasive (MIS) approaches of THR are not only desired to add more benefits but also shouldn`t compromise the already accomplished long-term results. Objectives: The purpose of our study was to evaluate the surgical outcome of minimally invasive versus conventional approach total hip arthroplasty. Patients and methods: This study was performed through a prospective; study comparing two patient groups containing 30 patients who had1ry hip replacement (THA) during the period from March 2018 to May 2020 at Qena University Hospital, fifteen of them undergone 1ry THA utilizing conventional approach and the other fifteen patients undergone 1ry THA utilizing mini-invasive approaches. Results: In Conventional THA group (group-A) nine males and six females were included with a mean age (± SD) of 57.2 ± 4.3 years; ranging (50-65 years). Preoperative HHS in-group-A patients ranged 20.3 – 37.1 (mean 26.8 ± 4.7). In Minimally invasive THA group (group-B) eight males and seven females were included with a mean age (± SD) of 57.7 ± 5.1 years; ranging (49-66 years).Preoperative HHS in group-B patients ranged 20.5 – 48.8 (mean 31.1 ± 8.0).The post-operative HHS were 89.56 ±6.11 and 91.94 ±1.91 in the group-A and group-B respectively, no statistically significant variation regarding the mean values was discovered between the two groups. The mean time to resume work in the group-A was 6.87 ±2.23 weeks compared to 5.67 ±1.39 weeks in the group-B with a significant statistical increase regarding group-A. Conclusion: MIS approaches are advantageous in the terms of hospitalization and time to return to work.

This research explores the influence of nepotism perception on hotel employees' intention to leave their jobs, focusing on ethical leadership as a moderator. The data collection process involved 383 employees from five-star hotels in Egypt. PLS-SEM as a significant analytical tool in hospitality research was used. The statistical data analysis was conducted using WarpPLS software version 7.0. Findings revealed that nepotism perception increases employees' intention to leave. Furthermore, a negative effect of ethical leadership exists on nepotism perception and employees' intention to leave. In addition, ethical leadership has been found to moderate the relationship between nepotism perception and employees' intention to leave. This study contributes to the hospitality literature and leadership theory by examining the interplay between nepotism perception and employees' intention to leave, with ethical leadership playing a moderating role. The findings offer insights that may prove valuable for hoteliers seeking to enhance their management practices and foster a positive work environment.

The aim of this study is to explore the effect of digital leadership (DL) on creative performance (CP) by investigating the mediating roles of strategic agility (SA) and organizational learning culture (OLC) in tourism and hospitality industry. The study analyzed 320 responses from middle-level management at five-star hotels and category-A travel agencies in Egypt using the PLS-SEM technique and WarpPLS statistical software 7.0. Findings reveal that digital leadership positively affects creative performance, strategic agility, and organizational learning culture. In addition, CP is positively affected by SA and OLC. Moreover, findings confirm the mediation effects of strategic agility and organizational learning culture in the DL→CP relationship. The study provides practical insights for tourism and hospitality enterprises using Dynamic Capabilities Theory to enhance performance and agility in the digital age.

Objective: This study aimed to investigate the relationship between serum Chlamydia trachomatis antibody titer and tubal block in infertile women. Patients and methods: The study included 80 infertile women scheduled for diagnostic laparoscopy. Using ELISA, the C. trachomatis IgG antibody titer (CAT) was determined in a venous blood sample (5 cc) obtained during laparoscopy. Results: The CAT was positive in 30 patients (23 had tubal block and 7 had patent tubes) and negative in 48 patients (44 had patent tubes and 4 had tubal block). The mean CAT was significantly high (41.7 ± 9.0 U/L) in the infertile patients with tubal block and peritubal adhesion as compared to patients with tubal block only (13.3 ± 3.9 U/L). The latter group had a significantly higher titer than did the women with patent tubes (6.2 ± 3.6 U/L). The CAT values of >8.8 U/L had a sensitivity and specificity of 85.7% and 84.6%, respectively, for tubal occlusion prediction. Conclusion: ELISA can be used as a simple, noninvasive screening test for C. trachomatis IgG antibodies, with a high predictive value for tubal occlusion in infertile Egyptian women, however larger studies are needed to confirm our results.

IFN orchestrates the expression of various genes to halt hepatitis C virus (HCV) replication with the possibility of either reduced or increased liver fibrosis; due to controlled viral replication or overproduction of inflammatory mediators, repectively. In this study, we examined the transcriptional profiling of type I IFN related genes in HCV-chronically infected patients with varying degrees of liver fibrosis. PCR array was used to examine the expression of 84 type I IFN related genes in peripheral blood mononuclear cells (PBMCs) RNA from 12 treatment-naïve chronic HCV patients (5 F0-F1 and 7 F2-F4) and 5 healthy subjects. We further validated our results by quantitative real time PCR (qRT-PCR) in 103 treatment-naïve chronic HCV patients (43 F0-F1 and 60 F2-F4) and 15 controls. PCR array data revealed dysregulation in TLR7 pathway. The expression of TLR7 was decreased by 4 folds and MyD88 was increased by 3 folds in PBMCs of F2-F4 patients when compared to the healthy volunteers (p = 0.03 and 0.002, respectively). In addition, IRF7 and TLR7 showed dramatic downregulation (6 and 8 folds, respectively) in F2-F4 patients when compared to F0-F1 ones. qRT-PCR confirmed the altered expression patterns of TLR7 and MyD88 in F2-F4 patients when compared to either controls or F0-F1 patients. However, by qRT-PCR, IRF7 and NF-κB1 (TLR7 pathway transcription factors) exhibited similar mRNA abundance among F2-F4 and F0-F1 patients. These results suggest that TLR7 and MyD88 are possible candidates as biomarkers for the progression of HCV-induced liver fibrosis and/ or targets for therapeutic intervention.

This study investigates the impact of green HRM (GHRM) on green competitive advantage (GCA), examining the mediating role of eco-innovation and the moderating role of green psychological climate (GPC). The study surveyed 472 fulltime employees in five-star hotels and travel agencies, developing and testing a five-hypothesis research model using PLS-SEM. Results revealed that GHRM positively impacts GCA and eco-innovation. Eco-innovation positively impacts GCA. In addition, GPC has a positive moderating influence on the relationship between GHRM and eco-innovation. Furthermore, Eco-innovation significantly mediates the relationship between GHRM and GCA.

Employees are crucial resources for organizations, and their job performance is highly important for their success, especially in the hotel industry. The study investigated the impact of social loafing (SL) on hotel employees' job performance (EJP), considering transactional leadership (TL) as a moderator. A total of 390 responses were gathered from full-time employees employed at 5-star hotels in Egypt. The data was analyzed using PLS-SEM software by WarpPLS 7.0. The study found that a negative effect exists between social loafing and employee job performance. In addition, transactional leadership has a negative effect on SL and a positive effect on EJP. Furthermore, TL has a negative moderating effect on the relationship between SL and EJP, the study indicates that TL weakens the negative relationship between SL and EJP. The study contributes to transactional leadership by recognizing and regulating negative job behaviors in modern organizational contexts, such as levels of workplace social loafing. The study highlights the importance of hotel premises in reducing social loafing in the workplace, highlighting the importance of incorporating ethical concepts with transactional leadership in fostering a healthy environment and improved job performance.

Background: Human papillomavirus (HPV)-related multi phenotypic sinonasal carcinoma (HMSC) is a recently described tumor subtype with an unknown prognosis, often misdiagnosed with other sinonasal carcinomas, and associated with high-risk HPV (HR-HPV). The present study aimed to evaluate the expression of vascular endothelial growth factor (VEGF), Bcl-2-associated X protein (BAX), epidermal growth factor receptors (EGFR), ProEx™C, and human telomerase reverse transcriptase (hTERT) and assess their association with survival and clinicopathological characteristics.Methods: Between 2017 and 2022, 40 HMSC patients underwent surgical resection at the School of Medicine, Zagazig University Hospitals (Zagazig, Egypt). Tissue samples were examined for the presence of HR-HPV; absence of myeloblastosis (MYB), MYB proto-oncogene like 1 (MYBL1), and nuclear factor I/B (NFIB) fusions and the presence of myoepithelial proteins (calponin, S100, SMA), squamous differentiation markers (p63, p40, calponin), VEGF, BAX, ProEx™C, and hTERT by immunohistochemistry. All patients were followed up for about 54 months until death or the last known survival data. Background: Letrozole, an aromatase inhibitor, has recently been introduced as the preferred treatment option for ectopic pregnancy. To date, no study has investigated the effect of letrozole alone on placental tissue. The present study aimed to evaluate the effect of different doses of letrozole on the placenta of rats and to clarify the underlying mechanism. Methods: Sixty pregnant female rats were equally divided into three groups, namely the control group (GI), low-dose (0.5 mg/Kg/day) letrozole group (GII), which is equivalent to the human daily dose (HED) of 5 mg, and high-dose (1 mg/Kg/day) letrozole group (GIII), equivalent to the HED of 10 mg. Background: Diffusion-weighted imaging is a fundamental tool integrated in MR protocols useful in differentiating benign from malignant mediastinal masses, assessing mediastinal lymphadenopathy and investigating central bronchogenic carcinoma. This method is an excellent alternative to CT or PET/CT in the investigation of mediastinal masses. Current applications of diffusion MRI in malignancies include monitoring the treatment response and detecting recurrent cancer. Aim of the work: This study aims to assess the value of using MRI diffusion in differentiating benign and malignant mediastinal masses, differentiating central masses from post obstructive collapse and differentiating lymphoma versus sarcoidosis. Patients and methods: This study included 30 patients; 16 males and 14 females in the period from June 2013 to July 2014. The mean age was 49.3 ± 16.85 (range: 22â82 years).Cases were referred for MRI assessment and were approved by the ethical committee in our department.The complaints varied between dyspnea, chest pain, cough, hemoptysis, fatigue and loss of weight. A superconducting 1.5 T MRI machine with a four-channel body phased-array coil was used for the examination. Biopsy and histopathological assessment was done after that. Results: MRI examination with diffusion imaging was able to differentiate between benign and malignant mediastinal and hilar lesion confirmed by the biopsy and histopathology. Conclusion: MRI with diffusion weighted images can detect and stage lung cancer, differentiate benign from malignant mediastinal masses and differentiate lymphoma from sarcoidosis in mediastinal/hilar lymphadenopathy.
