This paper deals with the language performance of Iraqi patients suffering from stroke. It defines what is meant by stroke, exhibits stroke types, and discusses aphasia disorder that is resulted from stroke in the left hemisphere of the brain. The researcher takes a case of three stroke patients who live in Dhi Qar province for this study and communicates with them to find which language areas are affected and what difficulties the patients face due to their brain injury. Since language is located on the left hemisphere of the brain, the selection of the case is limited to those who suffer from stroke in their left hemisphere where language areas are located.

3. Osteochondrolipoma of the foot treated by surgical excision: a case report and literature review

Abstract Background Osteochondromas, classified as a new benign subtype of lipomas and characterised by chondroid and osseous differentiation, are rare lesions that have been infrequently reported in previous literature. The maxillofacial region was reported as the most frequent localization, with infrequent occurrence in the lower limb. This paper represents the first documented case report of osteochondrolipoma in the foot. Case presentation A 51-year-old male patient presented with a chief complaint of right foot pain at the plantar aspect, accompanied by the observation of swelling between the first and the second metatarsal shafts. His complaint of pain and swelling started 10 and 4 years prior, respectively. Since their onset, both symptoms have progressed in nature. Imaging revealved a large mass exhibiting a nonhomogenous composition of fibrous tissue and bony structures. Surgical intervention through total excision was indicated. Conclusion Osteochodrolipoma is a benign lesion that can affect the foot leading to decreased functionality of the foot due to the pain and swelling. Surgical excision is the recommended approach for this lesion, providing both symptomatic relief and confirmation of the diagnosis through histopathological examination.

Abstract Background Several studies emerging from developed countries have highlighted a significant number of potentially avoidable emergency department (ED) visits by cancer patients during the end-of-life period. However, there is a paucity of information from developing nations regarding palliative care practices and the utilization of the ED by palliative care patients. Herein, we aim to characterize ED admissions among patients receiving palliative care at our tertiary center in Saudi Arabia. Methods This is a retrospective, cross-sectional study evaluating ED visits amongst adult patients with advanced cancer who were receiving treatment under the palliative care department. This study took place over a period of 12 months from July 2021 through to July 2022. Three palliative care specialist physicians independently and blindly reviewed each patient’s ED visits and determined whether the visit was avoidable or unavoidable. Results A total of 243 patients were included in the final analysis, of which 189 (78.1%) patients had unavoidable visits and 53 (21.9%) patient visits were classified as avoidable. A significantly higher proportion of breast cancer patients presented with unavoidable admissions (14.3% vs. 3.8%, P = 0.037) compared to other cancer types. Background: Pre-eclampsia (PE) is associated with a high rate of maternal and perinatal morbidity and mortality, so its prediction has a tremendous impact on its management. Objectives: To evaluate the role of uterine artery Doppler indices in the prediction of pre-eclampsia during the first trimester of pregnancy. Patients and methods: During the early stages of their pregnancies, 60 expectant mothers received early pregnancy care at South Valley University Hospital between weeks 9 and 11. Assessments included clinical exams, medical history, lab tests, abdominal ultrasound, and uterine artery Doppler scans at 9, 11, and 18 weeks. Results: The mean age of patients was 24.8 ± 3.4 years, with a mean body mass index (BMI) of 24 ± 2.3 kg/m2. There were 13 patients (23.6%) with PE; out of them, 9 patients had a severe form of PE, and 42 patients (76.4%) had no PE. Background:Gestational diabetes mellitus (GDM) is defined as the development of carbohydrate intolerance during pregnancy, excluding overt diabetes. GDM poses a risk of short-term adverse perinatal outcomes and long-term metabolic morbidity to women and their newborns. Objectives:The aim of this study was to determine the role of first trimester glycosylated haemoglobin as a predictor of gestational diabetes mellitus. Patients and methods: The study was carried out at Qena University Hospital, Department of Obstetrics and Gynecology. The glcosylated hemoglobin investigation included 200 high risk pregnant ladies from the start trimester taking care of our outpatient facility and in tolerant from May 2019 to May 2020. Results: Patients who developed gestational diabetes mellitus with HA1c < 5.7% were 12.4% while with HA1c ≥ 5.7% were 47.3%. Also, there is significant statistically difference between both groups (p value= 0.015).ROC curve for HbA1c in 1st trimester as a predictor of gestational diabetes mellitus showed that the cut-off point was ≥6.2%, sensitivity was 93.3% and specificity was 90.0%. Conclusion: The detection of glycosylated hemoglobin in the first trimester is a promising predictor biomarker for diagnosis of GDM and in the future may replace other basic investigations.

Background: Hysteroscopy is one of the most significant examinations in females with recurrent implantation failure (RIF). It permits dependable visual evaluation of the cervical canal and uterine space and it taken into consideration as a gold standard for diagnosing intra-uterine pathology and has slight intra-operative and postoperative morbidities. Objectives: This work aimed to clarify the roles of Hysteroscopy and endometrial biopsy in evaluation of endometrium after ICSI failure in females with un-explained Infertility. Patients and methods: This was a prospective cohort investigation carried out at outpatient infertility clinic of the Assisted Reproductive Unit, Obstetrics and Gynecology department, Qena University Hospital. Women attend to Assisted Reproductive unit planned for intracytoplasmic sperm injection (ICSI). Results: Based on histology as a reference standard, hysteroscopy identified the chronic endometritis pathology in 26 patients (true positives) that also found by histology. Our study revealed that Hysteroscopy had overall sensitivity, specificity, and diagnostic accuracy of 49.06%, 53.19% and 51% respectively in detecting the chronic endometritis in our patients with positive predictive value (PPV) was 54.17% while the negative predictive value (NPV) was 48.08%. Conclusion: Hysteroscopy is of a great value in the assessment and diagnosis of the endometrial pathology in patients with unexplained RIF. There is no consensus on the everyday use of hysteroscopy before ICSI; hence, the importance of hysteroscopy as a routine in the managing of non-fertile females still remains a subject of investigation.

9. Elevating Fake News Detection Through Deep Neural Networks, Encoding Fused Multi-Modal Features

Textual content was initially the main focus of traditional methods for detecting fake news, and these methods have yielded appointed results. However, with the exponential growth of social media platforms, there has been a significant shift towards visual content. Consequently, traditional detection methods have become inadequate for completely detecting fake news. This paper proposes a model for detecting fake news using multi-modal features. The model involves feature extraction, feature fusion, dimension reduction, and classification as its main processes. To extract various textual features, a pre-trained BERT, gated recurrent unit (GRU), and convolutional neural network (CNN) are utilized. For extracting image features, ResNet-CBAM is used, followed by the fusion of multi-type features. The dimensionality of fused features is reduced using an auto-encoder, and the FLN classifier is then applied to the encoded features to detect instances of fake news. Experimental findings on two multi-modal datasets, Weibo and Fakeddit, demonstrate that the proposed model effectively detects fake news from multi-modal data, achieving 88% accuracy with Weibo and 98% accuracy with Fakeddit. This shows that the proposed model is preferable to previous works and more effective with the large dataset.

Abstract Background Monkeypox is a zoonotic viral infection first reported in May 2022. Monkeypox cases present with prodromal symptoms, rash, and/or systemic complications. This study systematically reviews the monkeypox cases presented with any cardiac complications. Methods A systematic literature search was done to locate papers that discuss any cardiac complications associated with monkeypox; then, data were analyzed qualitatively. Results Nine articles, including the 13 cases that reported cardiac complications of the disease, were included in the review. Five cases previously had sex with men, and two cases had unprotected intercourse, which reveals the importance of the sexual route in disease transmission. All cases have a wide spectrum of cardiac complications, such as acute myocarditis, pericarditis, pericardial effusion, and myopericarditis. Conclusion This study clarifies the potential for cardiac complications in monkeypox cases and provides avenues for future research to determine the underlying mechanism. Also, we found that the cases with pericarditis were treated with colchicine, and those with myocarditis were treated with supportive care or cardioprotective treatment (Bisoprolol and Ramipril). Furthermore, Tecovirimat is used as an antiviral drug for 14 days.

11. A Randomized Clinical Trial on the Clinical and Immunological Efficacy of a Mouth Rinse Containing Hydrogen Peroxide and Hyaluronic Acid in Patients with Gingivitis.

OBJECTIVE: The present study aimed to investigate the impact of HA/H2O2 (Hyaluronic Acid/Hydrogen Peroxide) mouthwash on pro-inflammatory cytokines, specifically IL-1β and IL-6, in individuals diagnosed with gingivitis. Furthermore, a comparison was made between the effects of HA/H2O2 and those of CHX (Chlorhexidine) and placebo. MATERIALS AND METHODS: This clinical trial was randomized, double-blinded, and parallel. Fifty-four students with biofilm-induced gingivitis (18 to 23 years old) were randomly assigned to three mouthwash groups: HA/H2O2, CHX, and placebo. Unidentified bottles were delivered to participants. For 14 days, 10 millilitres of HA/H2O2 mouthwash formulation, CHX (0.12%), and placebo were used twice a day. All subjects were assessed twice, once at baseline and once after 14 days. Before and after each participant's mouthwash usage, bleeding on probing was measured as a clinical parameter for inflammation, Salivary IL-1β and IL-6 were also assessed by ELISA. Changes in salivary interleukins and bleeding between baseline and after treatment were determined and compared by the t test with an alpha of 0.05. RESULTS: There was a statistically significant decrease in salivary interleukin-1β and IL-6 in all three groups. However, intergroup comparisons at second visits showed no significant difference between HA/H2O2 and CHX groups (P>0.05) for the interleukins tested. Furthermore, all treatments considerably reduced bleeding on probing, although mouthwash with hyaluronic acid (HA) and hydrogen peroxide (H2O2) had a greater impact than the other two treatments. CONCLUSION: Mouthwash containing HA/H2O2 demonstrated an anti-inflammatory effect, suggesting they are immunomodulatory agents. These findings may be beneficial and encouraging for the use of HA/H2O2 in the treatment of gingivitis; thus, HA/H2O2 rinse has the potential to serve as an appropriate replacement for chlorhexidine (CHX).

In this paper, films of (PMMA-PC/Sb2O3-GO) quaternary nanostructures were prepared by casting method with different concentrations of Sb2O3/GO NPs are (0, 1.4 %, 2.8 %, 4.2 %,and 5.6 %). The structural and dielectric characteristics of nanostructures system (PMMA-PC/Sb2O3-GO) have been explored to use in different solid state electronics nanodevices applications. The morphology of (PMMA-PC/Sb2O3-GO) nanostructures films was studied using a scanning electron microscope (SEM). SEM images indicate a large number of uniform and coherent aggregates or chunks. The Fourier transform infrared spectroscopy(FTIR) analysis were studied to show the interactions between the Sb2O3/GO NPs and PMMA/PC blend. The dielectric properties of nanostructures films were investigated in the frequency range (100HZ-5MHZ). The dielectric constant, dielectric loss, and A.C electrical conductivity increase with the concentration of (Sb2O3-GO) NPs. The dielectric constant and dielectric loss were reduced, whereas electrical conductivity increased with frequency. Finally, results showed the PMMA-PC/Sb2O3-GO nanostructures may be considered as promising materials for solid state electronics nanodevices.

Today, one of the most significant and widely used engineering fields is heat transfer science. Saving energy and increasing efficiency are crucial given the need for energy management. Numerous sectors, including the cooling of machinery in power plants, the car industry, electronic equipment, and heat exchangers, heavily rely on fluid heat transfer. Improved design and functionality of thermal systems are made possible by increased heat transfer rate by fluids. This study presents the production, characteristics, and potential uses of PEG/MgO/SiC-H2O nanofluids as superior heat transfer media. Results indicate that when the quantity of MgO/SiC nanoparticles increased, the melting time reduced. Additionally, when the MgO/SiC NP concentration increased from 3 to 12 weight percent after 15 minutes, the reduction in melting time reached 65.5%. Additionally, when the concentration of MgO/SiC NPs was increased from 3 weight percent to 12 weight percent at photon wavelength (λ=400 nm), the electrical conductivity of PEG/MgO/SiC-H2O nanofluids increased by about 30.6%. At the same time, the absorbance increased by about 66.4% and the transmittance decreased by 58.8%.

14. Preparation and Properties of ZrO2/SiC-H2O Nanofluids to Use for Energy Storage Application

More than half of the energy used in total comes in the form of heat energy. An essential environmental protection technique to increase energy efficiency is learning how to employ thermal energy storage (TES) technology to fully use intermittent and unstable heat, such as solar heat utilization and industrial waste heat. Sensible heat storage, latent heat storage, and thermochemical heat storage are all types of thermal energy storage. This work describes the creation of ZrO2/SiC-H2O nanofluids and their characteristics for use in energy storage applications. Results reveal that increasing the concentration of ZrO2/SiC NPs from 0.3 gm/L to 1.2 gm/L at photon wavelength (=380nm) increased absorbance by roughly 83.7% and reduced transmittance by 81.2%. Additionally, when ZrO2/SiC NP concentrations rise, the absorbance rises as well, indicating improved nanofluid dispersion. Additionally, when the concentration of ZrO2/SiC NPs reached 1.2gm/L, the electrical conductivity of ZrO2/SiC-H2O nanofluids improved by nearly 74%, and the melting time reduced with an increase in the concentration of ZrO2/SiC nanoparticles.

Polymeric nanocomposites have drawn a lot of interest when it comes to innovative materials because of their enhanced optical, electrical, and magnetic properties. These materials have a high rising modulus, are flame resistant, and may also halt oxidation and agglomeration. These improvements in properties are related to interactions between nanoparticles and polymers. The addition of nanoparticles to polymers prolongs their life, changes their surface via passivation defect levels, and provides low cost, simple device manufacture, as well as adjustable electrical and optical properties. This study examines the properties and potential uses of nanofluids made from inorganic nanostructures doped with PEG. The results demonstrate that when the concentration of ZrO2/SiC NPs increased to 12wt%, the electrical conductivity of nanofluids increased by roughly 43.6%. Additionally, when the concentration of ZrO2/SiC nanoparticles increases, the melting time reduces. Additionally, when the concentration of ZrO2/SiC NPs increases from 3 weight percent to 12 weight percent within 15 minutes, the growth of melting time reaches 51.2%, and the absorbance increases by approximately 80.3% while transmittance decreases by about 82.5%.

Heat is transferred to storage medium during the charging phase of thermal energy storage (TES), and it is then released during the discharging phase. It may be used to industrial operations like metallurgical transformations or solar power facilities. Heat is stored in materials that alter temperature, phase, or chemical composition in sensible, latent, and thermochemical media, respectively. Optimal heat storage has a long history. This study describes investigating the optical and electrical properties of MgO/SiC-H2O nanofluids for applications including thermal energy storage. Results indicate that when MgO/SiC NP concentrations were raised to 1.2 gm/L, absorbance rose by approximately 66.9% and transmittance by about 54%. Additionally, the increase in MgO/SiC NP concentration will raise absorbance, which indicates improved nanofluid dispersion. Additionally, when MgO/SiC nanoparticle concentrations approach 1.2 gm/L, the electrical conductivity of nanofluids increases by roughly 49.2%, and the melting time reduces as the concentration of MgO/SiC nanoparticles rises.

Background: Antenatal care from a qualified clinician is essential for monitoring the pregnancy and minimizing the risks to the mother and child throughout pregnancy and delivery. Objectives: To determine the level of satisfaction among women attending the antenatal care clinics in Qena city also to assess ANC services in pregnant women to identify the level of ANC. Patients and methods: The degree of client satisfaction to ANC and factors affecting satisfaction were estimated in this cross-sectional study of 420 women. There were two groups: satisfied women and not-satisfied women. A structured questionnaire was used. Results:The study revealed that , out of the total clients overall satisfied with ANC were 285 ( 67.9%) more than half of them were rural residents, aged ≥ 29 years old, housewives and almost half of them had a medium social class,The significant risk factor related to degree of satisfaction to ANC wereHealth education about dangerous signs of labour, and Number of visits to ANC center, Ambulance availability, Doctors explain result ,Performance of nursing, Number of doctorsand the least factor Residence. By logistic regression analysisHealth education about dangerous signs of labourwas the most contributing factor associated with degree of satisfaction to ANC (p- value 0.008). Conclusion:The study revealed that, out of the total clients overall satisfied with ANC were 285 (67.9%) but also it revealed weaknesses in several aspects of ANC services, indicating that increased resources and health education are needed to improve the quality of ANC services in the governorate.

The proliferation of social media platforms has significantly accelerated our access to news, but it has also facilitated the rapid dissemination of fake news. Automatic fake news detection systems can help solve this problem. Although there is much research in this area, getting an accurate detection system is still a challenge. This article proposes a novel model to increase the accuracy of fake news detection. The theory behind the proposed model is to extract and combine global, spatial, and temporal features of text to use in a new fast classifier. The proposed model consists of two phases: first, global features are extracted by TF-IDF, spatial features by a convolutional neural network (CNN), and temporal features by bi-directional long short-term memory (BiLSTM) simultaneously. Then a fast learning network (FLN) is used to efficiently classify the features. Extensive experiments were conducted using two publicly available fake news datasets: ISOT and FA-KES. These two have different sizes; therefore, the proposed architecture (CNN+BiLSTM+FLN) can be evaluated much better. Results demonstrate the proposed model’s superiority in comparison with previous works.

As a result, pharmaceutical research has developed a strong interest in the transdermal route of drug delivery. They are non-invasive that can improve patient compliance, however, The barrier function of the skin's top layer presents the biggest challenge for transdermal delivery systems. Transfersomes are vesicles composed of phospholipids and edge activators, used to transport drug from outer skin layer into the systematic circulation through semipermeable membrane. Ondansetron is an anti-emetic medication that has been given parenterally and orally. Formulating this medication as transdermal transfersomes may provoke a great advantage in medical adherence. Conclusion; Ondansetron has been successfully delivered transdermally by Using a transfersomal gel formulation by vortexing sonication method.

Background Functional neurological disorder (FND) is a complex condition with neurological symptoms but no clear structural or biochemical explanation. Myths and misconceptions about FND can lead to misdiagnosis and inappropriate treatment. This study aimed to assess knowledge and common myths about FND among medical students and practitioners.Methods Data were collected from 324 participants using a structured questionnaire. The questionnaire included demographics, general information about FND and myths about FND. Data were analysed using non-parametric tests and Spearman’s r for correlations.Results The majority of participants were clinical-years medical students (65.1%) and female (59.6%). Overall, knowledge about FND was limited, with a mean score of 42.3% of correct answers. Common myths included the belief that FND is a psychological disorder and that patients feign symptoms. Knowledge scores differed significantly among different grades/experience levels, with postgraduate practitioners having the highest scores. There was a positive correlation between knowledge scores and confidence in managing FND.Conclusion This study highlights the prevalence of myths and misconceptions about FND among medical students and practitioners, emphasising the need for accurate education to improve diagnosis and management. Healthcare professionals should take a biopsychosocial approach to FND, considering the complex interplay between biological, psychological and social factors. Efforts to increase awareness and reduce stigma associated with FND are crucial for promoting better care. Targeted educational interventions may be beneficial to improve the understanding and management of FND among medical professionals.

This work aims to design of PEO/CuO new structures and investigating optical, and electronic characteristics to use in various electronic and optoelectronics devices like diodes, transistors, photovoltaic cell, electronic gates, sensors and other electronics devices. Using the B3LYP-DFT with a suitable 6-31G basis set for pure PEO and SDD basis set for nanocomposite, a good optimization structure for the predicted nanocomposites was obtained. Geometrical values that have been calculated. The results indicated that the studied nanocomposite need small energy to become cationdue to ionization potential is decrease with addition nanoparticle to the pure PEO, but the electronic affinity is an increase with addition nanoparticles to the pure PEO. When compared to other nanocomposite, the total ground state energy of PEO has the highest value of total energy, but ET dropped with the addition of nanoparticles to pure PEO. With the addition of nanoparticles to pure PEO, the hardness decreases, making nanocomposite softer, lowering a species' barrier to losing electrons. The studied nonocomposite direct electronic transition from the valence to conduction band with wave length falls within the solar spectrum range. The results revealed that the (PEO-CuO) nanocomposite has a wide range of applications in the fields of electronics and photo-electronics.

Background: The most crucial problem with in vitro fertilization (IVF) cycles is still oocyte quality. The women age and the condition of their ovarian reserve are the primary determinants of oocyte quality. Objectives: to assess the effects of intracytoplasmic sperm injection (ICSI) on the result of pregnancies and the coenzyme Q10 (CoQ10) value in follicular fluid (FF) in the women who had the procedure. Patients and methods: this cohort investigation was conducted on 81 infertile patients (age between 20-42 years, both normal or poor responders’ patients and patients with unexplained infertility) who underwent ICSI cycles. Results: patients were divided into two groups: the pregnant group (n= 32) and the non-pregnant group (n= 49).There was a statistically insignificant difference in antral follicle count (AFC), number of retrieved oocytes, number of embryos, number of metaphase II (MII) oocytes, and maturation index between pregnant and non-pregnant females. CoQ10 level in FF was substantially greater in pregnant than non-pregnant females. Conclusion: FF CoQ10 levels were positively correlated with eventual embryo quality and rates of conception. Our findings might be in favour of CoQ10 supplementation in women undergoing IVF for enhancement of the ovum and embryo quality.

Films of PVA/PVP/MnO2 nanostructures were fabricated for pressure sensors fields with lightweight, flexible and low cost compare with other sensors. The structure, dielectric and optical characteristics of PVA/PVP/MnO2 nanostructures have been studied. The results showed that the dielectric constant, dielectric loss and A.C electrical conductivity of PVA/PVP blend are enhanced with the increase in MnO2 NPs content. The dielectric constant and dielectric loss decrease while the A.C electrical conductivity increases with the increase in frequency. The optical characteristics of PVA/PVP/MnO2 nanostructures showed that the absorbance was rise with increasing of the MnO2 NPs content. The indirect energy gap of PVA/PVP blend was reduced with increase in MnO2 NPs content. The optical constants of blend are changed with the increase in MnO2 NPs content. The pressure sensor application results of PVA/PVP/MnO2 nanostructures showed that the electrical capacitance (Cp) increases with the rise in applied pressure.

The Si3N4/PVA/PEO structures were optimized and effectively simulated with a B3LYP / LanL2DZ primer. The structure stability, optical, thermal and electronic properties of Si3N4/PVA/PEO were studied. The obtained results indicated to the PVA/PEO/Si3N4 structures may be used for various optoelectronics devices with low cost and high flexibly.

Obesity in the pediatric population is considered a growing problem. It is likely that there will be a significant impact related to obesity on the health of future generations. Obesity has increased the incidence of a spectrum of diseases ranging from microvascular complications over the retina and peripheral nerves to an increased incidence of cancer. We have conducted an electronic search in MEDLINE, PubMed, ISI Web of Science, and Scopus scientific databases targeting studies published between 2000 till 2019. Several modalities have shown a wide spectrum of the effectiveness of weight control among adolescents. Despite achieving short-term success among obese adolescents, maintaining such change is challenging. The emergence of metabolic or bariatric surgeries has opened the door for long-term control over weight gain with considerable remission of unfavorable metabolic mediated or modulated effects associated with obesity such as diabetes mellitus and hypertension. The most commonly practiced metabolic surgery among adolescents is sleeve gastrectomy which is associated with comparable weight and metabolic control and a lesser risk of complication. Anesthesia is considered a major challenge among the pediatric population, especially those with significant obesity. Preoperative evaluation is always warranted to exclude and manage different associated comorbidities. The anesthetic challenges associated with pediatric obesity begin with intubation. Maintenance and emergence from anesthesia along with postoperative antiemetics and analgesia can pose additional challenges. Managing the postoperative period is considered a cornerstone in the early detection and management of any postoperative complication. Especially those complications related to the metabolic and nutritional aspects of the bariatric surgery. Finally, despite being a valuable option in managing obesity, bariatric surgery in adolescents comes with significant anesthetic challenges that need to be consistently evaluated and managed.

Background: Missile-penetrating head injury is a challenging problem for treating physicians. In Iraq and during 2003–2007, there was a civil war, which led to multiple causalities after suicide bombings or other clashes with coalition forces. A brain computed tomography (BCT) scan is considered as an initial diagnostic test to evaluate the next management plan. The authors tried to find the relationship between BCT scan findings and patients’ outcomes. Objective: This study aimed to explore the relationship between BCT scan findings of patients with missile-penetrating head injury and their prognosis. Materials and Methods: This was a cohort retrospective clinical study performed in Karbala, Iraq in the period from January 2005 till January 2006. Seventy patients with missile-penetrating head injuries were admitted and evaluated in Al-Hussain General Hospital (affiliated with the University of Al-Ameed). All patients had BCT scans and their outcome was evaluated using a modified Glasgow Outcome Scale. Results: In this study, the mortality rate is 15.71%. BCT scan findings are correlated with patients’ outcomes, which revealed the injury of one lobe carries the best outcome, whereas those with more than two lobes or posterior fossa injury carry the worse prognosis. Moreover, intraventricular hemorrhage (IVH) showed the highest mortality rate, whereas tract hematoma alone seems to carry a good prognosis with the least mortality. Conclusion: BCT scan is essential in both diagnosis and prognosis of patients with missile-penetrating head injury. IVH and posterior fossa injury lead to the highest mortality rate.

This paper aims to investigate the structural, optical and electrical properties of PEO doped with NiO. The DFT calculations have been performed using Gaussian 09 package of programs. The calculated electronic properties included the total energy, HOMO and LUMO energies, energy gap, ionization energy, electron affinity, electronegativity, electrochemical hardness, electronic softness and electrophilic index. The obtained results showed that the doping PEO with NiO improved the structural, optical, electronic and electrical characteristics where the energy band gap decreases about 67.4% with addition of NiO which make the (PEO-NiO) composites are promising materials for flexible optoelectronics fields in the development of electronics applications.

Abstract Background:Abnormal uterine bleeding is a common presenting symptom in the family practice setting. The most common causes are uterine fibroids, dysfunctional uterine bleeding and adenomyosis. All conditions were accompanied by chronic blood loss disabling normal lifestyle with variable degrees of morbidity. Objectives: This study is aiming to evaluate the safety, efficacy, and complications of uterine artery embolization(UAE) in the management of abnormal uterine bleeding (AUB)in different age groups. Patients and Methods: This a prospective randomized clinical trial prospective study included 20 patients referred for UAE. Twelve patients had uterine fibroids, 4 patients had adenomyosis and other 4 patients were diagnosed as dysfunctional uterine bleeding. The embolizing materials were used includingPolyvinyl alcohol, Tris- acryl gelatin microspheresand gelatin sponge. All patients had AUB. Results: Most patients showed good therapeutic responses through the follow up period (6-12 months). Complete control of bleeding was achieved in 90% of patients and recurrence occurred only in 10%. There was complete improvement of the pelvic pain and bulk related symptoms. Reduction of uterine and fibroid size occurred with average uterine and dominant fibroid volumes loss of 39%, and 57% respectively. However, the reduction in the Junctional zone thickness was 5.6 %. Conclusion: UAE is considered minimally invasive, safe, and effective method that plays an important role in treatment of variable types of AUB causes, aiming for uterine preservation and avoiding hysterectomy giving the chance for future fertility. It has high success rate with obvious low morbidity and mortality rates.

For decades, chronic pain was managed with an almost conventional approach of using a wide range of analgesic spectrum, surgical approaches and complex interventional pain techniques to modulate or even interrupt pain pathways. These different approaches carry many pharmacological hazards together with the lack of efficacy and safety of many interventional and surgical management techniques for chronic pain have mandated searching for other effective therapies including alternative treatments. Cannabinoids are naturally occurring substances that are derived from Cannabis sativa L. The usage of cannabinoids and their related synthetic chemical compounds has emerged as a choice in the management of different chronic pain conditions is being evaluated, however, the efficacy is still not consistently established. In the present investigation, therefore, we discuss the different aspects related to cannabinoids and their implications in the management of chronic pain conditions. This review will also discuss the safety profile of the cannabinoids together with the legal considerations that hinder their use in different countries.

Background: Novel psychoactive substances (NPSs) are relatively new substances in the illicit drug market, not previously listed in the United Nations Office on Drugs and Crime (UNDOC). Strox and Voodoo are considered some of the most popular blends of NPS in the Egyptian drug market. Objectives: The current study was conducted to assess NPS's use pattern: Voodoo and Strox among acutely intoxicated patients presented to the poison control center of Ain Shams University Hospitals (PCC- ASUH). Methods: A single center based cross-sectional study was carried out in the PCC-ASUH among acutely intoxicated patients presenting to the emergency department (ED) over four months (from January–April 2019. using a previously adopted and validated Fahmy and El-Sherbini socioeconomic scale (SES). Data were presented as mean, median and range as appropriate. Both smoking and crowding indexes were calculated and presented as previously reported. Results: Fifty-one patients were presented to the ED of PCC-ASUH during the study period. A total of 96.1% (n = 49) were males. The mean age was 25 ± 7.5 years. The most common NPS used was Strox: 54.9% (n = 28), followed by Voodoo: 27.4% (n = 14). Neurological and gastrointestinal (GI) symptoms were the most frequent presentations. The most common motive behind NPS use was the desire to give a trial of new psychoactive substances. The mean SES score was 35.1 ± 13.17. Most patients have the preparatory as the highest education 36.0% (n = 18). Conclusions: NPS use is common among young males in preparatory education from different social classes, starting it most commonly as a means to experiencing a new high. Neurological and GI manifestations are the most common presenting symptoms of NPS intoxication.

33. A 16S rRNA Gene and Draft Genome Database for the Murine Oral Bacterial Community

ABSTRACT A curated murine oral microbiome database to be used as a reference for mouse-based studies has been constructed using a combination of bacterial culture, 16S rRNA gene amplicon, and whole-genome sequencing. The database comprises a collection of nearly full-length 16S rRNA gene sequences from cultured isolates and draft genomes from representative taxa collected from a range of sources, including specific-pathogen-free laboratory mice, wild Mus musculus domesticus mice, and formerly wild wood mouse Apodemus sylvaticus. At present, it comprises 103 mouse oral taxa (MOT) spanning four phyla—Firmicutes, Proteobacteria, Actinobacteria, and Bacteroidetes—including 12 novel undescribed species-level taxa. The key observations from this study are (i) the low diversity and predominantly culturable nature of the laboratory mouse oral microbiome and (ii) the identification of three major murine-specific oral bacterial lineages, namely, Streptococcus danieliae (MOT10), Lactobacillus murinus (MOT93), and Gemella species 2 (MOT43), which is one of the novel, still-unnamed taxa. Of these, S. danieliae is of particular interest, since it is a major component of the oral microbiome from all strains of healthy and periodontally diseased laboratory mice, as well as being present in wild mice. It is expected that this well-characterized database should be a useful resource for in vitro experimentation and mouse model studies in the field of oral microbiology. IMPORTANCE Mouse model studies are frequently used in oral microbiome research, particularly to investigate diseases such as periodontitis and caries, as well as other related systemic diseases. We have reported here the details of the development of a curated reference database to characterize the oral microbial community in laboratory and some wild mice. The genomic information and findings reported here can help improve the outcomes and accuracy of host-microbe experimental studies that use murine models to understand health and disease. Work is also under way to make the reference data sets publicly available on a web server to enable easy access and downloading for researchers across the world.

Pyoderma gangrenosum is an uncommon inflammatory disorder characterized by neutrophilic infiltration of the skin. It can present as skin papules or pustules that progress into painful ulcers. 30–40% of the cases are associated with other systemic diseases such as inflammatory bowel diseases, rheumatoid arthritis, and proliferative hematological disorders. Uniquely, this condition has been associated with systemic lupus erythematosus (SLE). The rarity of this disorder poses a diagnostic and therapeutic challenge. We present a case of a 55-year-old female with a history of SLE and chronic right leg ulcer, presented with increased pain from the ulcer associated with a mild flare of her cutaneous lupus; examination revealed circumferential skin ulcer measuring about 25 cm extending around the right leg above the ankle with prominent fibrinous material and surrounding erythema. Blood work showed elevated WBC with neutrophilic predominance. Serology revealed a positive ANA, elevated RNP, smith, and SSA/Ro antibodies with normal anti-CCP level. Skin biopsy was taken, and it showed a diffuse neutrophilic and lymphocytic infiltrate consistent with the diagnosis of pyoderma gangrenosum. The patient was then treated with topical and systemic steroids and sequentially with dapsone, methotrexate, mycophenolate, and cyclosporine for over a two-year period but failed to show any improvement. Therefore, a trial of intravenous immunoglobulin (IVIG) therapy was attempted and produced a dramatic response after two-month infusions characterized by shrinking in the size of the ulcer and resolving pain. We believe that refractory PG poses a therapeutic challenge, and despite a lack of specific guidelines, IVIG can be attempted if initial suppressive treatment fails to show signs of improvement.

Sarcoidosis is a systemic inflammatory disorder characterized by “noncaseating granulomas.” It primarily affects the lungs, but multiple other organs can be involved. Sarcoidosis has been increasingly reported in association with cancer. It can precede, follow or occur at the same time as the diagnosis of cancer. We report a case of sarcoidosis that was diagnosed concomitantly with colon cancer, highlighting the diagnostic dilemma of sarcoidosis vs. cancer metastasis, the relationship between the two, and the value of PET scan in follow-up and monitoring of disease activity.

Background: Literature on the outcome of acute kidney injury (AKI) in Sjogren’s syndrome (SJS) is quite scanty. Acute kidney injury has emerged as a significant cause of morbidity and mortality in patients with autoimmune diseases such as systemic lupus erythematosus. Objective: To examine the outcome of AKI with and without SJS. To achieve this, we examined the prevalence, mortality, outcomes, length of stay (LOS), and hospital charges in patients with AKI with SJS compared with patients without SJS from a National Inpatient Sample (NIS) database in the period 2010 to 2013. Design: A retrospective cohort study using NIS. Setting: United States. Sample: Cohort of 977 055 weighted patient discharges with AKI from the NIS. Measurements: Not applicable. Methods: Data were retrieved from the NIS for adult patients admitted with a principal diagnosis of AKI between 2010 and 2013, using the respective International Classification of Diseases, Ninth Revision ( ICD-9 ) codes. The study population divided into 2 groups, with and without Sjogren’s disease. Multivariate and linear regression analysis conducted to adjust for covariates. We omitted patients with systemic sclerosis and rheumatoid arthritis from the analysis to avoid any discrepancy as they were not meant to be a primary outcome in our study. Results: The study population represented 977 055 weighted patient discharges with AKI. Analysis revealed AKI patients with Sjogren’s compared with patients without Sjogren’s had statistically significant lower hyperkalemia rates (adjusted odds ratio: 0.65, confidence interval: 0.46-0.92; P = .017. There was no statistically significant difference in mortality, LOS, hospital charges, and other outcomes. Limitations: Study is not up to date as data are from ICD-9 which are testing data from 2010 to 2013, and data were obtained through SJS codes, which have their limitations. Also, limitations included lack of data on metabolic acidosis, hypokalemia, and not including all causes of AKI. Conclusions: At present, our study is unique as it has examined prevalence, mortality, and outcomes of Sjogren’s in patients with AKI. Patients with Sjogren’s had significantly lower hyperkalemia during the hospitalization. Further research is needed to identify the underlying protective mechanisms associated with Sjogren’s that resulted in lower hyperkalemia. Trial registration: Not applicable.

The research aims to study the effect of the most important factors that affect the production and consumption of wheat crop and try to reduce the food gap between the produced and consumed quantities of this crop in Iraq for the period (1990 - 2014) and its effects on the agricultural sector in general. The fluctuation in the production of the wheat crop is offset by an increase in quantities consumed due to several economic factors. Based on this hypothesis, the research relied on the method of linking two directions, the descriptive method which was based on previous studies on the same subject, Lacey and his methods as he used the method of least squares two-stage And then explain the results of the quantitative method to assess the practical side of the study. The study also included the most important conclusions: wheat production was fluctuated during the study period because of the fluctuation in the productivity of the dunums of wheat where most of the cultivated lands are demotic lands represented in northern Iraq and the consumption of wheat is increasing due to the increase in numbers Population and wheat as a necessary commodity for the Iraqi individual so can not be dispensed or replaced, leading to a gap to be processed by importing, and therefore recommends the research: the use of modern agricultural techniques to increase the productivity of acres through the expansion of the use of modern irrigation systems and proved successful to reduce the dependence on rainwater and reduce the work food gap through the adoption of methods and methods in the preparation of modern production plans that work to raise productivity for this crop.

The current research aims to assess the Brief Book of the Modern and Contemporary History of Asia from the perspective of the students in the Department of History, University of Baghdad. The study is carried out in Iraq and the research group has included students of the Department of History in the College of Education--Ibn Rushd and the College of Education for Women- University of Baghdad. The chosen sample of the assigned research consisted of (72) students from third stage in the Department of History, which equals (0.38%) of the total group members amounted (188) female and male students representing (40) students from the College of Education--Ibn Rushd, and 32 students from the College of Education for Women. The researcher has addressed an open question to a sample of 20 reviewers of students in this department representing (0.11) of the total group originally gathered to make their remarks on the Brief Book of the Modern and Contemporary History of Asia, which is intended to be taught in 2014 within the curriculum prepared by the Ministry of Higher Education and Scientific Research. In the light of the responses and reviews of the assigned research group , the method of this research involved the (questionnaire) consisting of 40 assessing items which are classified to six fields such as: Introduction of the book, General aims, Teaching contents, Teaching activities, Technical direction, Preparing and composing the Brief Book of the Modern and Contemporary History of Asia.

The two subjects theory and traditions have been separately taken in general studies which refer to both general and detailed aspects of them, also explain the effect of each on community and multiple cognitive fields including architecture. However, most of these studies did not address clearly the relationship between them or their effect on each other. So this research studies all of the above in details based on the in-depth study of the intellectual aspects of both concepts, and attempts to find the bases of correlation and difference between them and to put forward the recapitulation out of them. This has been shown during the course of this research down to clarify its reflection on one of the architecture models that is the "Islamic Architecture" as an important and clear model which clarifies the correlation between theory and traditions, and its reflection on the production by displaying the urbanism privacy features for Arab-Islamic City and clarify the details of those features. Hence the research problem can be defined as "Lack of clarity of cognitive perception about the impact of theoretical foundations in contemporary architecture on the formation of general features of traditional architecture in general, and particularly on the traditional architecture which is affected by the Islamic architectural features as a model", so the aim the research is to clarify that cognitive perception. While the research hypothesis can be defined as "the effect of the general basis for the theory of contemporary architecture on specific indicators within the general features of traditional architecture, which adopted the characteristics of Islamic architecture as a model, and resulting a developed architectural feature which are in between".

Purpose: Over the past three decades, the incidence rate of breast cancer (BC) among Arab women has continually increased. However, data on the prevalence of BRCA1/2 mutations are scarce. Although the population in Saudi Arabia is at large homogeneous and consanguinity is common, especially in the central, eastern, and southern regions of the country, the prevalence of BRCA1 and BRCA2 mutations and the characteristics of BC are not well studied in the country. Methods: This prospective observational study intended to determine the prevalence of BRCA1 and BRCA2 mutations and sought to examine the clinicopathologic features of BC associated with these mutations. Results: Of 310 patients, 270 (87%) had no mutation. BRCA mutations were identified in 40 patients; BRCA1 mutations were found in 11% of patients, and BRCA2 mutations were found in 2% of patients. Variants of unknown significance were found in 15% of patients (45 patients). Triple-negative BC (TNBC) accounted for 86% of all patients with BC and mutations. The following three recurrent deleterious founder BRCA1 mutations were observed: c.4136_4137delCT was observed in five unrelated patients, c.5530delC was observed in three unrelated patients, and c.4524G>A mutations were observed in five unrelated patients. One novel mutation was identified in the BRCA1 gene (c.5512 dup [p.Glu1838Glyfs*42]). Conclusion: Among high-risk Saudi patients with BC, BRCA1 mutations are prevalent (11%). TNBC is the most common BC subtype. Furthermore, age alone does not have a significant association with mutation, but a combination of risk factors such as age, familial history, and TNBC has a significant association with BRCA mutation.

The types of web attack continue to appear and add their impacts on web application security, SQL injection, and XSS is one type of these attack, that causes extremely high risk for web application through stolen critical information or broken web authentication. The aim of this paper is to design and implement software as a service for securing web applications from attack which cover two directions, the first direction: propose modern black-box web application vulnerability scanners that diminish the false positive and false negative drawback of the current black-box web application vulnerability scanner, The second proposed subsystem cloud platform as a service web application firewall hosting web application which scan all http requests to deny or accept it according to dummy execution result.The numbers of vulnerable web applications are selected to evaluate the capability of the proposed system in detect attack and protect vulnerably web application also efficiency has evaluated through measure the server performance when WAF (Web Application Firewall) was disabled and enabled on vulnerably web application and comparison made between these two cases.According to analyzing of the experimental result shows that the proposed system can effectively and efficiently protect web application and discover SQL injection, and XSS vulnerabilities.

49. Producing Bone China with local and manufactured materials as substitutes for the traditional materials

This research studies the possibility of producing Bone China with available local and geological substitutes and other manufactured ones since it’s traditionally produced by Bone ash, Cornish stone, and China clay, while the substitutes are Kaolin instead of China clay and Feldspar potash instead of Cornish stone. Because of the unavailability of Feldspar in Iraq, it was substituted with the manufactured alternative Feldspar. Bone ash was prepared from cow bones with heating treatments, grinding and sifting. The alternative Feldspar was prepared by chemical analysis of the natural Feldspar potash with local materials that include Dwaikhla Kaolin, Urdhuma Silica sand, Potassium Carbonate, and Sodium Carbonate. The mixture was burned at (1250c○) before it was grinded. The mixture’s materials were sifted at (63µm). The samples were formed with slip casting by gypsum mold with a cut cone shape at (2×3×6 cm) to build a thickness of (3mm). The study materials consisted of two groups: Group 1 (BC.K.F) including Bone ash, Dwaikhla Kaolin, and the natural Feldspar, while Group 2 (BC.K.FR) had Bone ash, Dwaikhla Kaolin, and the manufactured alternative Feldspar. Every group included 8 recipes with the same percentages. The two groups were burned at (1220-1260c○), then tests were conducted: Chemical analyses, the outer appearance and color then the laboratory tests: the outer appearance, linear shrinkage, apparent porosity, water absorption, bulk density, hardness, scratching resistance, mineral analysis (XRD), color analyses, optical transmittance. After discussing and analyzing the results, it was concluded that the traditional materials substitutes gave matching and correspondent properties and specification of Bone China ceramic

Summary: Background: Although the 2009–2010 influenza A (H1N1) pandemic was of low severity compared with other pandemics of the 20th century, this pandemic was the first opportunity for countries to implement a real-life pandemic response. The aim of the project was to review the extent to which these plans and planning activities proved useful and to identify areas of pandemic planning that require further strengthening. Methods: We randomly selected seven countries within the WHO European Region to participate in a comprehensive, qualitative study to evaluate the pandemic preparedness activities undertaken prior to March 2009 compared with the subsequent pandemic responses mounted from May 2009 onwards. Research teams visited each country and interviewed stakeholders from health and civil response ministries, national public health authorities, regional authorities and family and hospital doctors. Results: The following six consistent themes were identified as essential elements of successful pandemic preparedness activities: communication, coordination, capacity building, adaptability/flexibility, leadership and mutual support. Regarding future pandemic preparedness activities, an emphasis on these areas should be retained and planning for the following activities should be improved: communication (i.e., with the public and health professionals); coordination of vaccine procurement and logistics; flexibility of response and hospital surveillance. Conclusions: Pandemic preparedness activities were successfully undertaken in the WHO European Region prior to the 2009 pandemic. These activities proved to be effective and were generally appropriate for the response provided in 2009. Nevertheless, consistent themes also emerged regarding specific areas of under planning that were common to most of the surveyed countries.
