1. Effect of ambient environment and magnetic field on laser-induced cobalt plasma

Abstract

Plasma parameters of magnetically confined Cobalt (Co) plasma have been evaluated by using laser-induced breakdown spectroscopy at various laser irradiances, under different ambient pressures of two environments such as Ar and Ne and at different time delays. The effect of laser irradiance on Co plasma is exposed in the presence and absence of a Transverse Magnetic field (TMF) while keeping environmental gas pressure constant, i.e., 10 Torr. For this purpose, Co pellets were exposed to Nd: YAG laser (1064 nm, 10 ns) at various laser irradiances ranging from 1 to 2.9 GW cm−2. To investigate the impact of background gas pressures, Co pellets were exposed to various pressure varying from 5 to 760 Torr of Ar and Ne. In the case of time delay variation, the Co plasma parameters Texc and ne decrease exponentially. A significantly pronounced effect of the presence of an external TMF of strength 0.9 T on time-integrated Co plasma parameters has been revealed. Plasma parameters Texc and ne are considerably increased in the presence of TMF in both ambient environments because of being constrained to a very small region due to which collisions will be enhanced. Implementation of the 0.9 T TMF on a laser-induced plasma of Co is responsible for the confinement of plasma. The surface morphology of laser-irradiated Co samples was also discussed to confirm the effect of TMF. Fine and uniform structures are observed in samples treated in the presence of TMF by using the SEM technique.

Abstract

BACKGROUND: One key element of lung-protective ventilation is the use of a low tidal volume (V(T)). A sex difference in use of low tidal volume ventilation (LTVV) has been described in critically ill ICU patients. OBJECTIVES: The aim of this study was to determine whether a sex difference in use of LTVV also exists in operating room patients, and if present what factors drive this difference. DESIGN, PATIENTS AND SETTING: This is a posthoc analysis of LAS VEGAS, a 1-week worldwide observational study in adults requiring intra-operative ventilation during general anaesthesia for surgery in 146 hospitals in 29 countries. MAIN OUTCOME MEASURES: Women and men were compared with respect to use of LTVV, defined as V(T) of 8 ml kg(−1) or less predicted bodyweight (PBW). A V(T) was deemed ‘default’ if the set V(T) was a round number. A mediation analysis assessed which factors may explain the sex difference in use of LTVV during intra-operative ventilation. RESULTS: This analysis includes 9864 patients, of whom 5425 (55%) were women. A default V(T) was often set, both in women and men; mode V(T) was 500 ml. Median [IQR] V(T) was higher in women than in men (8.6 [7.7 to 9.6] vs. 7.6 [6.8 to 8.4] ml kg(−1) PBW, P*** < 0.001). Compared with men, women were twice as likely not to receive LTVV [68.8 vs. 36.0%; relative risk ratio 2.1 (95% CI 1.9 to 2.1), P*** < 0.001]. In the mediation analysis, patients’ height and actual body weight (ABW) explained 81 and 18% of the sex difference in use of LTVV, respectively; it was not explained by the use of a default V(T). CONCLUSION: In this worldwide cohort of patients receiving intra-operative ventilation during general anaesthesia for surgery, women received a higher V(T) than men during intra-operative ventilation. The risk for a female not to receive LTVV during surgery was double that of males. Height and ABW were the two mediators of the sex difference in use of LTVV. Abstract

Patient monitoring is now an accepted standard in medical care to ensure that a patient's condition can be evaluated over time and any deterioration identified quickly, enabling appropriate care to be instituted. This article reviews the basic components of any monitoring device, and its importance in relation to improving patient safety. Issues related to increasing complexity of monitoring devices are also discussed. Basic monitoring modalities are described including pulse oximetry, non-invasive blood pressure, electrocardiography, temperature and capnography. Advanced cardiovascular monitoring is also discussed including invasive pressure monitoring, and invasive and non-invasive cardiac output monitoring devices. Neurological monitoring systems which evaluate intracranial pressure, cerebral blood flow and brain electrical activity in the critical care and perioperative environment are also described in relation to acute brain injury. Point-of-care testing (PoCT) modalities are reviewed as all monitoring devices can be regarded as forms of PoCT devices. Common features of PoCT devices together with advantages and disadvantages of PoCT are defined. A range of PoCT devices are discussed.

Abstract

The effect of magnetic field on the plasma parameters and surface modification of Cu-alloy has been investigated. For this purpose, we have employed Nd: YAG laser at various irradiances ranging from 1.9 to 5 GW/cm2 to irradiate Cu-alloy under 5 torr pressure of argon, neon, and helium. The evaluated values of excitation temperature (Texc) and electron number density (ne) of Cu-alloy plasma explored by laser-induced breakdown spectroscopy technique are higher in the presence of 1.1 Tesla magnetic field as compared with field-free case. It is true at all irradiances as well as under all environmental conditions. It is also found that trends of both Texc and ne are increasing with increasing laser irradiance from 1.9 to 4.4 GW/cm2. For the highest used irradiance 5 GW/cm2, the decrease in both parameters is observed. The analytically calculated values of thermal beta, directional beta, confinement radius, and diffusion time for laser-irradiated Cu-alloy plasma confirm the validity of magnetic confinement. Scanning electron microscope analysis is utilized to study the surface modifications of laser-irradiated Cu samples and reveals the formation of islands, craters, cones, and droplets. The finer-scale surface structures are grown in case of magnetic. It is also revealed Texc and ne play a substantial part in the growth of surface structures on Cu-alloy.

Abstract

Ion-induced modifications of brass in terms of surface morphology, elemental composition, phase changes, field emission properties and electrical conductivity have been investigated. Brass targets were irradiated by proton beam at constant energy of 3 MeV for various doses ranges from 1 × 1012 ions/cm2 to 1.5 × 1014 ions/cm2 using Pelletron Linear Accelerator. Field Emission Scanning Electron Microscope (FESEM) analysis reveals the formation of randomly distributed clusters, particulates, droplets and agglomers for lower ion doses which are explainable on the basis of cascade collisional process and thermal spike model. Whereas, at moderate ion doses, fiber like structures are formed due to incomplete melting. The formation of cellular like structure is observed at the maximum ion dose and is attributed to intense heating, melting and re-solidification. SRIM software analysis reveals that the penetration depth of 3 MeV protons in brass comes out to be 38 µm, whereas electronic and nuclear energy losses come out to be 5 × 10−1 and 3.1 × 10−4 eV/A respectively. The evaluated values of energy deposited per atom vary from 0.01 to 1.5 eV with the variation of ion doses from 1 × 1012 ions/cm2 to 1.5 × 1014 ions/cm2. Both elemental analysis i.e. Energy Dispersive X-ray spectroscopy (EDX) and X-ray Diffraction (XRD) supports each other and no new element or phase is identified. However, slight change in peak intensity and angle shifting is observed. Field emission properties of ion-structured brass are explored by measuring I-V characteristics of targets under UHV condition in diode-configuration using self designed and fabricated setup. Improvement in field enhancement factor (β) is estimated from the slope of Fowler-Nordheim (F-N) plots and it shows significant increase from 5 to 1911, whereas a reduction in turn on field (Eo) from 65 V/µm to 30 V/µm and increment in maximum current density (Jmax) from 12 µA/cm2 to 3821 µA/cm2 is observed. These enhancements in field emission characteristics are correlated with the growth of surface structures, specifically agglomers which are responsible for electric field convergence. Electrical by four probe method has been correlated with maximum current density and decreasing trend is observed with increasing ion doses.

Prevention of significant morbidity and mortality during sedation requires individuals to be appropriately trained in its use. This article aims to provide information into the techniques used, it discusses the generic principles and risks of sedation and should not be seen as a substitute for formal training. Attention is also drawn to the currently existing guidelines for various procedures.

Abstract

The effect of magnetic field on laser-induced breakdown spectroscopy of aluminum (Al) plasma has been investigated. Al targets were exposed to Nd:YAG laser pulses at different irradiances ranging from 1 GWcm−2 to 2.7 GWcm−2, under argon (Ar) and neon (Ne) environments at various pressures ranging from 5 torr to 760 torr and at different time delays from 0.42 μs to 9.58 μs. All spectroscopy measurements were performed in the absence and presence of transverse magnetic field of strength 0.9 tesla. When laser irradiance is increased by keeping the pressure (10 torr) and time delay constant (1.25 μs), both excitation temperature (Te) and number density (ne) increase up to certain values. The same trend is observed for Te and ne when the ambient gas pressure of Ar and Ne is increased by keeping the irradiance (1.7 GWcm−2) and time delay constant. At higher irradiances and pressures, saturation is observed, which is attributed to the self-regulating regime of plasma. In the case of time delay, both electron temperature and number density decay exponentially, which is according to the adiabatic expansion model. It is revealed that emission intensity and electron temperature are higher in the presence of magnetic field as compared to the field-free case, which is attributed to magnetic confinement, as well as the joule heating effect. Plasma plume confinement is confirmed by analytical evaluation factor β. β is an analytical factor that is the ratio of plasma pressure to magnetic pressure, i.e., β=Plasma pressureMagnetic pressure. It confirms the validity of magnetic field confinement if β is less than 1. As the evaluated values of β are less than 1 for all cases, they confirm the validity of magnetic confinement.

Advanced cardiovascular monitoring has a role in the assessment and management of both haemodynamically unstable critically ill and high-risk perioperative patients. This article reviews basic cardiovascular physiology, goal directed haemodynamic therapy and advanced cardiovascular monitoring with evidence and recommendations. Specific monitoring modalities will be described including invasive pressure monitoring, arterial waveform analysis and cardiac output estimation by dilutional, Doppler and echocardiography methods. Emerging technologies will also be discussed.

Abstract

Vascular and clinical assessments of arterio-venous fistula (AVF) function and access are important in patients undergoing or preparing to undergo renal dialysis. Objective assessment techniques include colour duplex ultrasound and more recently medical infrared thermography. Ideally, these should help assess problems relating to fistula failure or to vascular steal from the hand which can result from excessive fistula blood flow. The clinical value of thermography, as yet, has not been assessed for this patient group. The aims of this study were therefore to investigate the relationships between thermography skin temperature measurement and (a) quantitative ultrasound measurement of AVF blood flow, and (b) qualitative clinical assessment of vascular steal from the hands. Fifteen adult patients underwent thermal imaging of the upper limbs, colour duplex ultrasound to derive AVF blood flow from brachial artery blood flow measurements, and a clinical evaluation for vascular steal. Temperature measurements were extracted from the thermograms, including bilateral arm and hand (Fistula − Non-Fistula) differences, for comparison with derived AVF blood flow and steal grading. Derived AVF blood flow ranged from 30 to 1950 ml min−1, with a mean rate close to one litre per minute. Thermography detected the warmer superficial veins in proximity to the patent fistulas, with bilateral differences in fistula region skin temperature correlated with derived AVF blood flow (using maximum temperature measurements the correlation was +0.71 [p < 0.01]; and using mean temperature measurements the correlation was +0.56 [p < 0.05]). When thermography measurements were compared with the clinical assessment of steal the mean hand temperature differences separated steal from non-steal patients with an accuracy of greater than 90%. In summary, we have now demonstrated the potential clinical value of medical infrared thermography for assessing AVF function in renal patients.

Abstract

Objectives: To determine the incidence of sleep-related breathing disorders and nocturnal hypoxaemia in patients discharged from ICU following prolonged mechanical ventilation.¶Design: Prospective, consecutive patient observational study.¶Setting: The medical and surgical wards of a University Hospital.¶Patients and participants: Fifteen consecutive, adult patients discharged from the ICU who had received more than 48 h of mechanical ventilation were studied. Ten healthy volunteers acted as controls.¶Measurements and results: Overnight, multi-channel pneumographic studies were performed on all patients and controls. Chest and abdominal wall movement, air flow, oxygen saturation and snoring were continuously recorded. Data was analysed by both visual inspection of the traces and by computer-based algorithms. An apnoea/hypopnoea index was calculated for each patient and volunteer. Volunteers had an apnoea/hypopnoea index of less than 5 and had no episodes of nocturnal oxygen desaturation (SaO2 < 90 %). Despite oxygen therapy 13/15 patients had episodes of desaturation and 9/15 spent more than 2 h with an SaO2 < 90 %. Eleven patients had an abnormal apnoea/hypopnoea index (range 5–34 events/h). Four patients had predominantly obstructive events while 7 primarily had hypopnoeas.¶Conclusions: Significant overnight oxygen desaturation is common in patients discharged from ICU who have received prolonged mechanical ventilation. This group also has a significant incidence of sleep-related breathing disorders and this mechanism is likely to be important in the pathogenesis of the hypoxaemia.
