Through a two-part, mixed method approach, a survey based upon the comprehensive sleep quality assessment—The Pittsburgh Sleep Quality Index—along with a voluntary interview, was provided to participants from two public high schools in the northeastern region of the United States. Purpose: During the third-year emergency medicine (EM) clerkship, medical students are immersed in traumatic incidents with their patients and clinical teams. Trauma-informed medical education (TIME) applies trauma-informed care (TIC) principles to help students manage trauma. We aimed to qualitatively describe the extent to which students perceived the six TIME domains as they navigated critical incidents during their EM clerkship.Methods: We employed a constructivist, modified grounded theory approach to explore medical students’ experiences. We used the critical incident technique to elicit narratives to better understand the six TIME domains as they naturally appear in the clerkship. Participants were asked to describe a traumatic incident they experienced during the clerkship, followed by the clerkship’s role in helping them manage the incident. Using the framework method, transcripts were analyzed 1) deductively by matching transcript excerpts to relevant TIME domains and 2) inductively by generating de novo themes to capture factors that affected students’ handling of trauma during critical incidents.Results: Twelve participants were enrolled and interviewed in July 2022. “Safety” was the most frequently described TIME domain, whereas “Gender, Cultural, and Historical issues” and “Peer Support” were discussed least. Inductive analysis revealed themes that hindered or supported their ability to manage traumatic experiences, which were grouped into three categories: 1) student interactions with the learning environment: complex social determinants of health, inequalities in care, and overt discrimination; 2) student interactions with patients: ethically ambiguous care, witnessing acute patient presentations, and reactivation of past trauma; and 3) student interactions with supervisors: power dynamics, invalidation of contributions, role-modeling, and student empowerment.Conclusion: The six TIME domains are represented in students’ perceptions of immediate, stressful critical incidents during their EM clerkship, with “Safety” being the most commonly described; however, the degree to which these domains are supported in students’ experiences of the EM clerkship differ, and instances of inadequately experienced domains may contribute to student distress. Understanding the EM clerkship through the specific lens of students’ experiences of trauma may be an effective strategy to guide curricular changes that promote a supportive learning environment for students in the emergency department.

Abstract: Background: The involvement of Wnt-1-induced secreted protein-1 (WISP1/CCN4) in several inflammatory reaction has recently been proposed. Nevertheless, this protein's involvement in rheumatoid arthritis (RA) remains debated. Associations between poorly diagnosed RA and several classical markers derived from demography and biochemistry have been reported. Aim: We sought to investigate the reliability and effectiveness of serum concentrations of CCN4, vascular cell adhesion molecule-1 (VCAM-1), matrix melloprotenase-3 (MMP-3), and granulocyte-macrophage colony-stimulating factor (GM-CSF) in monitoring and predicting RA and bone damage, and their correlation with RA disease course. Methods: The study analyzed 128 patients with RA, comprising 68 newly diagnosed and 60 previously diagnosed patients, as well as 60 controls. Biomarker levels were measured with enzyme linked immuno-sorbent assays. Routine laboratory parameters such as serological, clinical, biochemical, and hematological parameters were additionally measured. Demography, anthropometry, and clinical symptom data were collected through interviews and a questionnaire. The joint disease activity score 28 (DAS28) was used to determine disease activity. Results: Concentrations of four biomarkers were significantly higher in the RA group than the healthy controls. Elevated biomarker concentrations were also observed in patients with high, rather than moderate or low, DAS28-ESR activity status, except for monocyte count, hematocrit (%), and urea level. Furthermore, CCN4 level positively correlated with VCAM-1, MMP-3, and GM-CSF levels, DA-S28-CRP and DAS28-ESR. The levels of three predictive markers, CCN4, VCAM-1, and MMP-3, were elevated in non-treated patients, whereas GM-CSF level showed no difference. The highest area under the curve was 73.3% for CCN4, with 93.3% sensitivity and 64.7% specificity. Conclusion: Our data suggest that CCN4 can be reliably used to indicate activity and therapeutic response associated with RA, thus facilitating earlier RA diagnosis.

Due to the radiation released by commonly used isotopes, many nuclear, medical, and industrial facilities require proper radiation shielding. In this work, distinct copper borate glasses intercalated with varied aluminum and vanadium oxide (Al2O3 and V2O5) content have been synthesized and used against radiation (gamma rays and fast/thermal neutrons). The different percents were as follows: [60% B2O3 + 35% CuO + (5-x)% Al2O3 + xV2O5], where x = 0, 1, and 2.5 wt.%, which was coded as BCu(5-x)Al:xV. The synthesized glass samples were characterized using Fourier transforms, infrared, and X-Raydiffraction analysis. Experimentally, the radiation shielding possessions of the samples were established using an HPGe detector at the gamma energy lines 0.356 MeV, 0.661 MeV, 1.173 MeV, and 1.332 MeV. Also, the prepared glasses were investigated theoretically using the Monte Carlo code (MCNP5) at photon energies of 0.015–15 MeV. Also, the fast and thermal neutron macroscopic effective removal cross-sections were calculated using MRCsC and JANIS-4.1 software, respectively. The prepared sample BCu2.5Al:2.5V, which has a vanadium and aluminum content of 2.5%, has the highest linear attenuation coefficient as well as the highest removal cross-section for fast, and thermal neutrons.

In recent years, CFRP is the most popular material in strengthening RC members (columns, beams, slabs…etc.) as well as the textile-reinforced mortar (TRM) was also used as an alternative method for strengthening RC columns. The present study includes fifteen circular RC columns: One control column, ten columns strengthened by one or two layers of CFRP using EBR or EBROG methods, two columns was strengthened by textile-reinforced epoxy on grooves and last two columns were strengthened by textile reinforced mortar without grooves. The parameters of this investigation are type of strengthening technique (EBR, EBROG and TRM), number of CFRP layers, and the grooves configuration. The results showed that the columns strengthened longitudinally with one or two layers of CFRP strips using EBR technique enhanced the load-carrying capacity by 2 and 7.13%, respectively, when compared with control column. However, the load carrying capacity of RC columns strengthened by EBROG increased by the range of 11.65% to 71.4%. in comparison with control column. Finally, TRM strengthening method enhanced the load carrying capacity by about 21% to 58.5%. Also, the results showed that the EBROG and TRM approaches were improved the stiffness and ductility of the columns in comparison with EBR method.

Abstract This study aims to explore the flexural behavior of crushed clay brick (CCB) lightweight concrete (LWC) beams strengthened with rubberized strain-hardening cementitious composite reinforced with glass fiber textile mesh layers (GFTM-RSHCC) at the tension side. For this purpose, an experimental investigation consisting of seven simply supported beams, including one un-strengthened specimen, was produced and tested using a monotonic 4-point loading scheme. All specimens had a 120 × 250 mm cross-section, a total length of 2400 mm, and a loaded span of 2200 mm. The studied parameters were the number of GFTM inside the RSHCC (1, 2, or 3) and the thickness of GFTM-RSHCC layer (30 or 40 mm). All the following aspects were tracked: crack pattern, ultimate load, mid-span defection, and ductility. The results show that increasing the number of layers of GFTM and the thickness of RSHCC generally leads to an increase in the ultimate loads and ductility, up to 68% and 83%, respectively, compared to the control beam. Finally, a proposed equation considering the contribution of the GFTM-RSHCC layer was developed to predict the flexural capacity of the strengthened beams. The proposed equation showed good agreement with the experimental results.

Background: Cardiac arrest remains a critical global health challenge, necessitating effective interventions to improve outcomes. Autopulse Mechanical Chest Compression has emerged as a potential adjunct to conventional manual cardiopulmonary resuscitation (CPR). This study aimed to comprehensively investigate the association between Autopulse use and various outcomes following cardiac arrest. Methods: A thorough literature review was conducted using PubMed, Embase, Scopus, Google Scholar, CINAHL, and the Cochrane Library. Articles were evaluated for eligibility according to predefined criteria. The collected data was synthesized, and the findings were subsequently presented. Results: Sixteen studies were included in the study. A synthesis of diverse studies revealed nuanced perspectives on Autopulse Mechanical Chest Compression. Enhanced CPR success and improved survival rates in out-of-hospital cardiac arrest were demonstrated, while potential drawbacks were suggested, warranting scrutiny of device design and implementation strategies. Meta-analyses on ROSC favored Autopulse, indicating a statistically significant advantage over Manual CPR (relative risk 1.43, 95% CI 1.07-1.92). Complication analyses showed no significant difference (relative risk 1.11, 95% CI 0.93-1.33), emphasizing comparable safety profiles. Patient survival rates exhibited variations, with Autopulse demonstrating higher rates in some studies, but nuances were observed in 30-day survival and hospital discharge rates. Conclusion: Autopulse Mechanical Chest Compression holds promise as a supplementary strategy in cardiac arrest resuscitation. The ability to control the function and structure of some promising nanosystems using an external stimulus is attractive research to develop programmable and reconfigurable intelligent nanomaterials. The focal point of this review is the silicon-based nanoporous materials, and particularly the mesoporous silica materials (MSMs) class (pore size: 2–50 nm), due to their important intrinsic properties, such as high surface area, highly ordered nanostructure, narrow pore size distribution, various dimensions (one-dimensional, two-dimensional, and three-dimensional), and easily functionable. One of the most essential organic components that can be incorporated in MSMs is organic photochromic molecules (OPMs), such as azobenzene, stilbene, dithienylethenes, and spiropyrans. OPMs can be incorporated into MSMs, to form photochromic mesoporous organosilica materials (PMOMs), in two different ways: physical (non-covalent immobilization) or chemical (covalent immobilization) binding. PMOMs are considered smart nanomaterials because they have the ability to undergo reversible changes in the solid state when exposed to an external electromagnetic radiation. PMOMs have been the subject of many research studies during the last decade due to their potential applications, especially as chemosensors. This review discusses the main families of OPMs, their incorporation into MSMs using different methods, and the applications of some PMOMs as chemosensors.

Flexible job shop scheduling problem (FJSSP) is a complex and challenging problem that plays a crucial role in industrial and manufacturing production. FJSSP is an expansion of the standard job shop scheduling problem (JSSP). One of FJSSP’s objectives that the manufacturing system competing for is minimizing the makespan. This paper uses a new nature-inspired metaheuristic optimization algorithm called the Apiary Organizational-Based Optimization algorithm (AOOA) to solve the FJSSP. This Algorithm simulates the organizational behavior of honeybees inside the apiary and translates their activities and vital processes during their lifecycle into phases that can solve such NP-hard problems. Two benchmark datasets, Brandimarte and Hurink, with 10 MK instances and 24 (edata, rdata, and vdata) instances respectively, were used to demonstrate the ability of AOOA to solve FJSSP. Moreover, the results of AOOA were compared with a set of state-of-the-art algorithms and statistically measured using the paired samples t-test and p-value, RPD, and group-based superiority statistical analysis to test its performance. AOOA outperformed Elitism GA, Enhanced GA, Improved GA, and MOGWO in solving all 10 MK instances and HICSA in solving 9 MK instances out of 10. Moreover, AOOA overcame CS, CS-BNG, CS-ILF, CHA, and MCA in solving 24, 12, 12, 23, and 24 instances of edata, rdata, and vdata, respectively. AOOA proved its robustness, showing promising outcomes.

In cyber security, the most crucial subject in information security is user authentication. Robust text-based password methods may offer a certain level of protection. Strong passwords are hard to remember, though, so people who use them frequently write them on paper or store them in file for computer .Numerous of computer systems, networks, and Internet-based environments have experimented with using graphical authentication techniques for user authentication in recent years. The two main characteristics of all graphical passwords are their security and usability. Regretfully, none of these methods could adequately address both of these factors concurrently. The ISO usability standards and associated characteristics for graphical user authentication and possible attacks on nineteen recognition-based authentication systems were discussed. In this study, differentiation table of attack patterns for all recognition-based techniques is shown. Finally, the positive and negative aspects of nineteen methods were explained in the form of a detailed table.

By 2030, the Kingdom of Saudi Arabia (KSA) is seeking to increase homeownership to 70%. This has led to a considerable investment in residential construction, with the Housing Ministry establishing a stimulus program to enhance the use of Modern Methods of Construction (MMC). These technologies offer several benefits, including cost and time reduction, improved quality and productivity, and job creation. This study quantitatively analyzes the economic impacts of MMC adaptation on the Saudi economy using the Global Trade Analysis Project (GTAP) model. It considers incentives to adopt MMC, its technological advancements, and the skills currently available in the Saudi labor market. Moreover, the embodied carbon emissions of MMC are calculated using the Input–Output Life Cycle Assessment Approach (IO-LCA); the detailed environmental emissions from the transportation phase, accounting for material reduction, are assessed using the COPERT model and compared to conventional construction. The results reveal an increase of 1.92% in Saudi GDP, along with improved welfare of $28.5 billion and a notable increase in the sectors related to construction. Job creation is demonstrated by an increase in the employment of unskilled and skilled labor by 2.02 and 1.5%, respectively. The paper concludes that MMC’s total embodied carbon emissions were approximately 77 million metric tonnes CO2, while its transport-related emissions are less than conventional approaches.

Background: Congenital anomalies affecting the central nervous system are among the most prevalent. The most common central nervous system anomalies are neural tube defects, which affect roughly 1-2 of every 1000 newborns. Objectives: We detect the accuracy of ultrasound to diagnose CNS abnormalities. Patients and methods: Descriptive cross-sectional study on 567 patients coming for antenatal care at Sohag Governorate undergoing mid anatomical scan between September 2021 and December 2022. After getting approval from the ethical committee at Sohag University and written consent from each woman. Clinical or post-natal neuroimaging using computed tomography or magnetic resonance imaging validated the prenatal diagnosis in situations where pregnancy progressed and the fetus survived. The total number of miscarriages, stillbirths, and infant deaths was recorded. Results: Diagnostic accuracy of ultrasound in diagnosis of congenital neurological Anomalies: 2D&3D US had 100% sensitivity and100% specificity with positive predictive value 100% and negative predictive value 100% in all parameters. Conclusions: Based on these findings, we can say that ultrasonography is useful for detecting and diagnosing some defects of the fetal central nervous system.

With the advancement of the robotics world, many path-planning algorithms have been proposed. One of the important algorithms is the Rapidly Exploring Random Tree (RRT) but with the drawback of not guaranteeing the optimal path. This paper solves this problem by proposing a Memorized RRT Optimization Algorithm (MRRTO Algorithm) using memory as an optimization step. The algorithm obtains a single path from the start point, and another from the target point to store only the last visited new node. The method for computing the nearest node depends on the position, when a new node is added, the RRT function checks if there is another node closer to the new node rather than that is closer to the goal point. Simulation results with different environments show that the MRRTO outperforms the original RRT Algorithm, graph algorithms, and metaheuristic algorithms in terms of reducing time consumption, path length, and number of nodes used.

Abstract The integration of microgrids into the existing power system framework enhances the reliability and efficiency of the utility grid. This manuscript presents an innovative mathematical paradigm designed for the optimization of both the structural and operational aspects of a grid-connected microgrid, leveraging the principles of Demand-Side Management (DSM). The focus of this work lies in a comprehensive exploration of the implications brought about by the Renewable Generation-Based Dynamic Pricing Demand Response (RGDP-DR) mechanism, particularly in terms of its influence on the optimal microgrid configuration, considering perspectives from end-users and the utility entity. This inquiry is rooted in a holistic assessment that encompasses technical and economic performance benchmarks. The RGDP-induced DR framework adeptly addresses the needs of the consumer base, showcasing notable efficiency and economic feasibility. To address the intricate nonlinear optimization challenge at hand, we employ an evolutionary algorithm named the "Dandelion Algorithm" (DA). A rigorous comparative study is conducted to evaluate the efficacy of four optimization techniques, affirming the supremacy of the proposed DA. Within this discourse, the complexity of microgrid sizing is cast as a dual-objective optimization task. The twin objectives involve minimizing the aggregate annual outlay and reducing emissions. The results of this endeavor unequivocally endorse the superiority of the DA over its counterparts. The DA demonstrates exceptional proficiency in orchestrating the most cost-effective microgrid and consumer invoice, surpassing the performance of alternative optimization methodologies.

Background Conjunctival rotational flap is one of the surgical treatment options for pterygium with variable recurrence rates. Taking a conjunctival flap from the superior bulbar area is sometimes unfeasible due to scarring, ocular surface disease, or previous trabeculectomy. Aim This study aimed to compare the superior versus the inferior conjunctival rotational flap results following primary pterygium excision. Patients and methods This is a retrospective, interventional, comparative study that enrolled 120 patients having primary pterygium. The patients were recruited from the Ophthalmology Department, Beni Suef University Hospital, Beni Suef, Egypt during the period from June 2020 to June 2022. The patients were classified into two groups: group I that included 73 (60.8%) patients who underwent the superior conjunctival flap technique, and group II that included 47 (39.2%) patients who underwent the inferior flap technique. Slit-lamp findings were reported during the scheduled appointments, with a focus on pterygium recurrence and other complications. Postoperative follow-up was performed for 6–18 months. Results The recurrence rate among the inferior conjunctival flap group (12.8%) was higher than in the superior conjunctival flap group (9.6%), with no statistically significant difference (P=0.585). No additional postoperative complications, such as flap separation, dullen formation, or granulomas occurred in either group. Conclusion There was no statistically significant difference in the outcome of the superior and inferior conjunctival rotational flaps in primary pterygium excision. We propose that inferior conjunctival flap preference can result in favorable outcomes in circumstances where the superior bulbar conjunctiva is required for protection or cannot be used.

The ongoing global public health challenge posed by the COVID-19 pandemic necessitates continuous research and surveillance efforts. In this study, we comprehensively analyzed over 1000 COVID-19 RT-PCR tests conducted on a cohort of 1200 patients in Saudi Arabia. Our primary goal was to investigate mutations in specific genes RdRp, N, and E different infection and recovery stages in Saudi patients with SARS-CoV-2. We also extended our analysis to include patients of various nationalities residing in Saudi Arabia, with the overarching objective of assessing these genes as markers for COVID-19 presence and progression. To diagnose and investigate potential genetic variations in COVID-19, we engaged RT-PCR. Our study primarily focused on detecting mutations in the RdRp, N, and E genes in Saudi patients with SARS-CoV-2, as well as individuals from various national residing in Saudi Arabia. This molecular technique provided valuable insights into the virus’s genetic makeup during infection and recovery. In our analysis of 671 positive COVID-19 cases, diverse gene involvement patterns were observed. Specifically, 55.91% had mutations in all three genes (RdRp, N, and E), 62.33% in both N and E genes, and 67.16% in RdRp and N genes. Additionally, 30.75% exhibited mutations exclusively in the RdRp gene, and 51.58% had mutations in the N gene. The N gene, in particular, showed high sensitivity as a marker for identifying active viral circulation. Regarding the temporal dynamics of the disease, the median duration between a positive and a subsequent negative COVID-19 RT-PCR test result was approximately 33.86 days for 44% of cases, 14.31 days for 30%, and 22.67 days for 4%. The insights from this study hold significant implications for managing COVID-19 patients during the ongoing pandemic. The N gene shows promise as a marker for detecting active viral circulation, potentially improving patient care and containment strategies. Establishing a defined positive threshold for diagnostic methods and correlating it with a low risk of infection remains a challenge. Further research is needed to address these complexities and enhance our understanding of COVID-19 epidemiology and diagnostics.

whiteThis paper presents a whitenovel Nested Identical Control (NIC) whitemethod for load frequency control of multi-area microgrid systems employing two novel variants of the Chameleon Swarm Algorithm (CSA). Load frequency control, a common strategy to counter power system instabilities from load fluctuations, poses a significant challenge. Maintaining system stability amidst substantial load shifts becomes particularly challenging in complex power networks spanning multiple areas with several generators. To address this, we first designed two novel variants of CSA called Quasi-oppositional CSA (QCSA) and Quasi-Levy-oppositional CSA (QLCSA). In QCSA, a quasi-oppositional-based learning operator is used to improve the diversification and intensification of CSA. The QLCSA incorporates the Levy flight operator into the QCSA to avoid stagnation and local minimal entrapment. We evaluated the efficiency of these variants against the original CSA and five other highly performing algorithms on the IEEE Congress on Evolutionary Computation benchmark functions. whiteSubsequently, we employed the new CSA variants to design a NIC for load frequency control of a two-area microgrid system, considering different cases of uncertainties. The NIC strategy is designed such that the controller parameters of each loop in one area are inherited by the controller of the corresponding loop in the second area, with a target to achieve $\mathrm {10 sec}$ of settling time. Simulated results on the benchmark functions prove the superiority of QLCSA and QCSA over CSA and the selected highly-performing algorithms. The dynamic responses of the NIC-controlled microgrid system whitein numerical time-domain simulations show the effectiveness of the proposed control approaches when compared with PID control tuned using MATLAB’s control system tuner.

After the spread of the coronavirus around the world, the Egyptian government imposed blended learning on higher education institutions. Consequently, colleges and universities in Egypt are entering a new era where learning is not confined to the classroom alone but also through learning management systems (LMSs). Thus, this study adopts the Unified Theory of Acceptance and Use of Technology (UTAUT) to study how students accept and use the LMSs. An online survey was conducted through a structured questionnaire to collect quantitative data for analysis. Obtained data from 803 respondents were analyzed using structural equation modelling Partially least squares regression was used for the model and hypothesis testing. The results show that trust is vital in determining the acceptance and use of LMSs. The study results may provide insights into a better approach to promoting LMS acceptance.

In recent years, the Eridu oil field has emerged as a key player in the petroleum industry in southern Iraq as it is the biggest Iraqi oil discovery in 20 years. Extending along a vast area of 5806 km2, the field has commercial oil reserves in formations such as Mishrif, Nahr Umr, Zubair, and Yamama. However, drilling operations in this field have faced significant challenges, including delays and suspensions caused by wellbore instability. One of the main obstacles encountered during drilling operations in the Eridu oil field is the occurrence of partial losses in weak vugs dolomite formations, as well as issues related to sever borehole instabilities such as drilling in tight holes, caving, and breakout due to shear failure in the borehole wall. To address these challenges, a 1 D Geomechanical model (1-D MEM) was constructed using data from vertical wells to better understand the underlying causes of drilling problems. The findings of the 1-D MEM, particularly in relation to mechanical rock properties, rock Elasticity factors, pore pressure, and fracture gradient complex formations like Tanuma and Mishrif, were instrumented in planning drilling operations for inclined and highly deviated wells. By utilizing open hole well logging data and calibrating the model with various resources of data including drilling observations, core mechanical analyses, and pore pressure measurements, a more accurate assessment of wellbore instabilities was achieved. The analysis revealed that many of the wellbore instabilities, such as pack-off, breakout, and stuck pipe, were attributed to the insufficient mud weight that failed to support the rock in the borehole wall. To avoid these issues, it was determined that a safe mud weight range of 11-12.5 ppg is necessary to prevent wellbore instability in shale formations. The study also highlighted the importance of using proper mud weight to prevent shear failure and other drilling complications. The findings of this study provide insights that can be utilized as a cost-effective tool for planning directional and horizontal drilling operations in the Eridu oil field. The accuracy of the failure criteria and geomechanical model is significantly superior and aligns with the analysis of breakouts observed in the caliper and image logs.

The rating of the villages of Anbar province, depending on Its spatial distribution, is one of the most important scientific aspects of geographical research, so this research was not found in finding a classification shows spatial distribution patterns of the villages of Anbar province and analyzing the characteristics affecting spatial distribution. The search for the question of the nature of spatial distribution of villages in Anbar province, which is like a research problem. The proliferation of villages in Anbar province has shown different distribution patterns as a result of the impact of natural and human properties. The search used modern statistical methods in the analysis by using the nearest style and the standard distance and the adoption of spaces and deployment measures. Three research results are three patterns of spatial distribution of villages in Anbar province.
