Background: The torque of maxillary incisors and molars forms an important component of smile esthetics. The inclination of these teeth may be affected by maxillary and mandibular dimensions. The study aimed to evaluate the correlation between the faciolingual inclinations of maxillary incisors and first molars with palatal width (PW), palatal depth (PD), maxillomandibular angle (MMA) and mandibular width (MW) using cone beam computed tomography scans. Methods: A retrospective analysis of cone-beam computed tomography (CBCT) images of 66 adult subjects (males-37; females-29) was performed. The Planmeca Romexis viewer 6.2.1 software was utilized to determine the faciolingual inclination of the maxillary central incisor (UIPP) and first molars (right-MMR; left-MML). The correlation of UIPP, MMR and MML with age, PW, PD, MW and MMA was determined using Pearson's correlation. Results: The mean age of the sample was 32.8±11.4 years. The UIPP showed a mild negative correlation with age (r=-0.430; p<0.001). Only the PW showed a mild significant correlation with MMR (r=-0.287; p=0.019) and MML (r=-0.343; p=0.005). All the other maxillomandibular parameters had insignificant (p>0.05) correlations with the inclinations of maxillary incisors and molars. Conclusion: The current study concludes that the PW had a significant inverse correlation with bilateral maxillary molar inclinations. Other parameters such as MMA, MW and PD had no statistically significant correlation with incisors and molars inclinations. [ABSTRACT FROM AUTHOR]

Objective: To compare the maxillary sinus (MS) and alveolar bone (AB) heights in adult subjects with normal occlusion and those with skeletal anterior open bite (AOB). Study Design: Cross-sectional. Setting: Aga Khan University Hospital, Karachi. Period: December 2020 to June 2021. Material & Methods: The study was conducted on the pretreatment orthodontic records of sixty adult subjects (30 males, 30 females). Skeletal high angle cases with AOB >1 mm were included in skeletal AOB while a matched sample with normal dental and skeletal relationships was included in normal occlusion group. The MS and AB heights were measured on lateral cephalograms and orthopantomograms at interdental regions from maxillary 1st premolar to 2nd molar. The MS and AB heights were compared between males and females and between skeletal AOB group and normal occlusion groups using independent sample t-test. Results: The mean age of the subjects with skeletal AOB (22.68±4.04 years) and those with normal occlusion (23.19±4.68 years) were comparable (p=0.653). In general, MS height and AB heights were greater in males than females. There was no significant difference in the MS height between the skeletal AOB and normal occlusion groups for both genders (p>0.05). However, both males and females with skeletal AOB had greater AB heights between 2nd premolar and 1st molar and between 1st molar and 2nd molar than those with normal occlusion (p<0.05). Conclusion: The MS height in skeletal AOB and normal occlusion groups were comparable. Subjects with skeletal AOB had maximum AB height mesially and distally to maxillary 1st molar that was significantly greater than those with the normal occlusion. [ABSTRACT FROM AUTHOR]

This may result in a soft tissue oriented approach for the diagnosis of the underlying sagittal skeletal discrepancy, which may subsequently lead to the development of a treatment plan that may give better soft tissue outcome. Methodology: A sample of 198 subjects (99 males and 99 females; mean age = 23.6 ± 4.6 years) was uniformly divided into Class I, II and III sagittal groups on the basis of soft tissue angle of convexity. Various sagittal parameters including ANB angle, AB plane angle, Downs angle of convexity, Wits appraisal, Beta angle and W angle were assessed on pretreatment lateral cephalograms. Correlation between various skeletal analyses and the soft tissue angle of convexity was determined using Spearman's correlation. Results: The ANB angle showed the highest correlation with the soft tissue angle of convexity (r = 0.907), whereas W angle showed the least correlation (r = 0.744). Amongst the sagittal groups, no significant correlation was present in Class I group. Downs angle of convexity showed a moderate positive correlation in Class II (r = 0.514) and Class III (r = 0.533) sagittal groups with the soft tissue parameter. Conclusion: ANB angle and Downs angle of convexity were found to be reliable skeletal indicators in assessing the facial soft tissue profile. Hence, the number of cephalometric analyses for evaluating the sagittal skeletal jaw discrepancy may be limited to fewer analyses of higher diagnostic performance, which are more reliably related to the overlying facial pattern. BACKGROUND: Angle's paradigm had ruled the orthodontic diagnosis and treatment planning for the past several decades, but the recent introduction of the soft tissue paradigm has significantly changed the dynamics of orthodontic practice. This study was designed to identify skeletal analyses that best correlate with the parameter use to assess facial soft tissue profile. Objective: To assess the level of awareness among orthodontic practitioners about the diagnosis and management of orthodontically induced white spot lesions., Methods: The cross-sectional study was conducted from August 28, 2022, to March 3, 2023, at Bakhtawar Amin Medical and Dental College, Multan, and comprised orthodontic specialists and postgraduate residents. Data was collected using a 14-item questionnaire regarding diagnosis and management of orthodontically induced white spot lesions. The questionnaire was disseminated online, and the responses were compared between the groups. Data was analysed using SPSS 24., Results: Of the 278 subjects, 205(73.7%) were residents; 156(75%) females and 49(24%) males with mean professional experience of 4.24±4.08 years. There were 73(26.3%) specialists; 44(60.3%) females and 29(39.7%) males with mean professional experience 9.07±4.85 years. There were 48(66%) specialists and 131(64%) residents who thought the most commonly affected teeth with WSL were maxillary central incisors, while 30(41%) specialists and 38(33%) residents said the least commonly affected tooth was maxillary canine. Among the specialists, 29(38%) considered halting treatment and regular follow-up as the best approach for managing white spot lesions detected during orthodontic treatment, while 76(37%) residents preferred to use fluorides and casein phosphopeptide-amorphous calcium phosphate. There were significant differences between the specialists and residents for the items related to the incidence of white spot lesions, timing for additional precautions and measures for detection, management during active treatment and modalities of prevention (p<0.05)., Conclusion: Despite being fairly common in orthodontic patients, the awareness regarding white spot lesions and related management protocols was found to be dubious in orthodontic practitioners, depicting lack of a standardised protocol.

The face is the most expressive zone of the human body that communicates our feelings and thoughts. This may also influence the interaction between people. The aesthetic adjunctive procedures are life-changing. In contemporary orthodontic treatment, orthognathic surgeries are performed to correct the functional aspects of dento facial deformities. In cases where the aesthetic outcome is not improved, patient dissatisfaction is often encountered. Many adjunctive surgical procedures can be used to enhance the anaesthetics of orthodontic or orthognathic surgical cases. Dwelling not merely on the ideal occlusion, the results could be enhanced by analysing the whole-face to improve the overall treatment outcome.

The total sample was distributed into three equal groups (i.e., long, normal and short face) on the basis of soft tissue vertical pattern. Pre-treatment lateral cephalograms were used to assess various vertical linear and angular parameters. Various skeletal analyses and soft tissue parameters were correlated using the Pearson's correlation in different vertical groups, separately for males and females., Results: In males, a weak positive correlation (r = 0.485) was found between skeletal anterior facial height ratio (Sk. LAFH/TAFH) and soft tissue anterior facial height ratio (LAFH/TAFH'), whereas in females maxillary-mandibular plane angle (MMA) showed a weak positive correlation (r = 0.300). In the long face group, a positive but a weak correlation (r = 0.349) was present between cranial base angle (SN-GoGn) and LAFH/TAFH'., Conclusions: Skeletal analyses (MMA, Sk. LAFH/TAFH) significantly correlated to soft tissue parameters. Males andlong faced individuals showed a higher correlation between skeletal and soft tissue parameters as compared to that of the females.
