1. First Impressions of the New da Vinci 5 Robotic Platform and Experience in Performing Robot-assisted Radical Prostatectomy

Abstract: Over the past two decades, robotic surgery has seen substantial advances, with significant growth in novel platforms, particularly since 2019. As a high-volume center experienced with various robotic systems, we share our initial impressions of the new da Vinci 5 platform for robot-assisted radical prostatectomy. Key improvements include enhanced console ergonomics, more precise operative imaging, and the integration of smart commands for streamlined surgical control. Notably, force feedback instruments offer the potential for reduced tissue trauma, although further studies are needed to evaluate long-term outcomes and cost effectiveness. Our early experiences suggest that surgeons familiar with previous da Vinci models, particularly the Xi platform, will find the transition to the da Vinci 5 seamless, with minimal learning curve adjustments. Using propensity score matching, we compared perioperative outcomes for 50 da Vinci 5 RARP procedures (performed after the learning curve) with 150 da Vinci Xi cases. In our experience, optimal performance and perioperative outcomes were obtained with both models. Further studies are needed to identify any clinically significant advantages of one platform over the other. Patient summary: We compared outcomes for patients undergoing removal of the prostate using two different surgical robots (da Vinci Xi and da Vinci 5). Optimal operative outcomes were obtained with both robots, but further studies are needed to evaluate whether one robot is clinically superior to the other.

Abstract: This work aims to provide a new Galerkin algorithm for solving the fractional Rayleigh-Stokes equation (FRSE). We select the basis functions for the Galerkin technique to be appropriate orthogonal combinations of the second kind of Chebyshev polynomials (CPs). By implementing the Galerkin approach, the FRSE, with its governing conditions, is converted into a matrix system whose entries can be obtained explicitly. This system can be obtained by expressing the derivatives of the basis functions in terms of the second-kind CPs and after computing some definite integrals based on some properties of CPs of the second kind. A thorough investigation is carried out for the convergence analysis. We demonstrate that the approach is applicable and accurate by providing some numerical examples.

Abstract: Abstract Background Historically, epidemics have been accompanied by the concurrent emergence of stigma, prejudice, and xenophobia. This scoping review aimed to describe and map published research targeting ethical values concerning monkeypox (mpox). In addition, it aimed to understand the research gaps related to mpox associated stigma. Methods We comprehensively searched databases (PubMed Central, PubMed Medline, Scopus, Web of Science, Ovid, and Google Scholar) to identify published literature concerning mpox ethical issues and stigma from May 6, 2022, to February 15, 2023. The key search terms used were “monkeypox”, “ethics”, “morals”, “social stigma”, “privacy”, “confidentiality”, “secrecy”, “privilege”, “egoism”, and “metaethics”. This scoping review followed the framework proposed by Arksey and O’Malley in 2005 and was further improved by the recommendations of Levac et al. in 2010. Results The search strategies employed in the scoping review yielded a total of 454 articles. We analyzed the sources, types, and topics of the retrieved articles/studies. The authors were able to identify 32 studies that met inclusion criteria. Six of the 32 included studies were primary research. The study revealed that the ongoing mpox outbreak is contending with a notable surge in misinformation and societal stigma. It highlights the adverse impacts of stigma and ethical concerns associated with mpox, which can negatively affect people with the disease. Conclusion The study’s findings underscore the imperative need to enhance public awareness; involve civil society; and promote collaboration among policymakers, medical communities, and social media platforms. These collective endeavors are crucial for mitigating stigma, averting human-to-human transmission, tackling racism, and dispelling misconceptions associated with the outbreak.

10. Enhancement of porous asphalt mixtures modified with various fibers and ethylene–vinyl acetate polymer

Abstract: Abstract Porous asphalt mixture is conventional hot mix asphalt (HMA) with substantially decreased fines, which produces an open-graded mixture that enables the water to flow through an interconnected void space. Porous asphalt is a permeable system that has a lot of benefits. However, because of its open structure, the durability of this mixture decreases, and both its stability and resilient modulus are much lower compared to the dense conventional asphalt mixtures. Also, the high void percentage may lead to an increase in the draindown proportion. Fibers (cellulose or mineral) and polymer-modified binders are recommended for porous asphalt mixtures, especially in hot and moderate climates. The objective of this study is to improve the porous asphalt mixture's performance by using ethylene–vinyl acetate (EVA) polymer-modified bitumen. Two types of fibers (cellulose fibers and glass wool fibers) were used, separately to determine the control mixture. Four different proportions of EVA polymer were added to the bitumen (1%, 2%, 3%, and 4%) and Scanning Electron Microscopy (SEM) was used for better investigating of the bitumen microstructure, then The Marshall mix design was used to determine the optimum EVA content (OEC) for the porous asphalt mixture. Several performance tests were conducted to investigate the characteristics of the porous asphalt mixture, such as the infiltration rate, binder draindown, the wheel track and the cantabro abrasion tests. The findings of the study conclude that the addition of EVA polymer to the porous asphalt mixtures enhances the performance as it increases stability by 20.8% and the infiltration rate by 20.6%. It decreases binder draindown proportion by 33.3%, cantabro abrasion loss by 25.1% and the rut depth at 5,000 cycles and 10,000 cycles by 29.8% and 19.7%, respectively.

13. The relationship between ego identity status and acculturation strategies (comparative study)

Abstract: The present study aims to investigate the acculturation strategies employed by Yemeni students in Turkey and China, and their relationship with Ego identity status and perceived cultural distance. The study involved 506 Yemeni students enrolled in Turkish and Chinese universities, with 290 participants studying in Turkey (57.3 %) and 216 studying in China (42.7 %). The age of the participants ranged from 18 to 42 years, with a standard deviation of 5.45 and a mean of 27.34 years. The results indicated that the strategies adopted by Yemeni students were similar in both countries. Ego-identity states were found to be predictors of acculturation strategies. Identity achievement was found to predict integration strategy, while foreclosure identity predicted separation strategy. Additionally, identity moratorium and identity diffusion predicted the marginalization strategy. Knowledge of the host country's language was a predictor of willingness to integrate into the host society. The immigrant's lack of knowledge of the host country's language led them to adopt a separation strategy. Cultural distance was also found to be highly predictive of the assimilation and separation strategy. Furthermore, the presence of family members with the immigrants led to the adoption of tendencies to preserve the native culture, while immigrants living without their families in the host country tended to adopt the assimilation strategy.

Abstract: ABSTRACT Objective: In the following years after the United States Preventive Service Task Force (USPSTF) recommendation against prostate cancer screening with PSA in 2012, several authors worldwide described an increase in higher grades and aggressive prostate tumors. In this scenario, we aim to evaluate the potential impacts of USPSTF recommendations on the functional and oncological outcomes in patients undergoing robotic-assisted radical prostatectomy (RARP) in a referral center. Material and Methods: We included 11396 patients who underwent RARP between 2008 and 2021. Each patient had at least a 12-month follow-up. The cohort was divided into two groups based on an inflection point in the outcomes at the end of 2012 and the beginning of 2013. The inflection point period was detected by Bayesian regression with multiple change points and regression with unknown breakpoints. We reported continuous variables as median and interquartile range (IQR) and categorical variables as absolute and relative percent frequencies. Results: Group 1 had 4760 patients, and Group 2 had 6636 patients, with a median follow-up of 109 and 38 months, respectively. In the final pathology, Group 2 had 9.5% increase in tumor volume, 24% increase on Gleason ≥ 4+3 (ISUP 3), and 18% increase on ≥ pT3. This translated to a 6% increase in positive surgical margins and 24% reduction in full nerve sparing in response to the worsening pathology. There was a significant decline in post-operative outcomes in Group 2, including a 12-month continence reduction of 9%, reduction in potency by 27%, and reduction of trifecta by 22%. Conclusions: The increasing number of high-risk patients has led to worse functional and oncologic outcomes. The initial rapid rise in PSM was leveled by the move towards more partial nerve sparing. Among some historical changes in prostate cancer diagnosis and management in the period of our study, the USPSTF recommendation coincided with worse outcomes of prostate cancer treatment in a population who could benefit from PSA screening at the appropriate time.

Abstract: In this paper, we improved recent results on the existence of solutions for nonlinear fractional boundary value problems containing the Atangana-Baleanu fractional derivative of order $ \zeta \in (1, 2) $. We also derived the exact relations between these fractional boundary value problems and the corresponding fractional integral equations in infinite dimensional Banach spaces. We showed that the continuity assumption on the nonlinear term of these equations is insufficient, give the derived expression for the solution, and present two results about the existence and uniqueness of the solution. We examined the case of impulsive impact and provide some sufficiency conditions for the existence and uniqueness of the solution in these cases. We also demonstrated the existence and uniqueness of anti-periodic solution for the studied problems and considered the problem when the right-hand side was a multivalued function. Examples were given to illustrate the obtained results.

17. Step-by-step Peritoneal Bladder Flap Bunching (PBFB) technique: an innovative approach following lymph node dissection in robotic radical prostatectomy

Abstract: ABSTRACT Introduction: Robot-assisted radical prostatectomy (RARP) has become a popular surgical approach for localized prostate cancer due to its favorable oncological and functional outcomes, as well as lower morbidity. In cases of intermediate- and high-risk prostate cancer, bilateral pelvic lymphadenectomy (PLND) is recommended as an adjunct to RARP (1–3). Despite its benefits, PLND can lead to surgical complications, with postoperative lymphocele formation being the most common. Most postoperative lymphoceles are clinically insignificant with variable incidence, reaching up to 60% of cases 4. However, a small percentage of patients 2-8% may experience symptomatic lymphoceles (SL), which can cause significant morbidity (4, 5). Surgical technique: We perform our RARP technique with our standard approach in all patients (6). After vesicourethral anastomosis a modified PF created to prevent symptomatic lymphocele. We start by suturing the peritoneal fold on the right side, medially to the vas deferens, followed by a similar stitch on the left side to approximate the edges in the midline. A running suture bunches the bladder peritoneum from both sides, passing through the pubic bone periosteum to secure it in place (7). This approach keeps the lateral pelvic gutters open for lymphatic drainage, while allowing fluid drainage from the true pelvis into the abdomen. A pelvic ultrasound was done for all patients at 6 weeks post operative, and additional clinical follow-up was carried out at 3 months following surgery. Considerations: We have demonstrated a modified technique of peritoneal flap (PBFB) with an initial decrease in postoperative symptomatic lymphoceles, the technique is feasible, safe, does not add significant morbidity, and does not require a learning curve.

18. Ion-exchange enhancement of borosilicate glass vials for pharmaceutical packaging

Abstract: Pharmaceutical containers for parenteral use, including vials, ampoules, prefilled syringes, and cartridges, are traditionally made of glass. However, the most commonly used type, borosilicate glass, is susceptible to issues such as breakage, corrosion, and delamination, which can jeopardize the safety and efficacy of the enclosed drugs. To address these concerns without compromising the visual or qualitative aspects of borosilicate medical glass vials, this study aimed at the enhancement of their mechanical, chemical, and corrosion resistance. A single ion exchange treatment (IET) in a salt bath of molten KNO3 at temperatures of 400, 450, and 500 °C for 2, 12, and 24 h was applied. The effects of the ion exchange process performed under different conditions were assessed by measuring Vickers hardness, crushing load, and chemical durability. The mechanical load required to crush full-body vials after the ion exchange process at 500 °C for 2, 12, and 24 h showed an increase in the applied force values (1650 ± 80, 2340 ± 80, and 2325 ± 40 N) compared to untreated vials (1157 ± 20 N).No radial cracks were observed on the surface of treated glass vials after indentation, indicating the presence of compressive stresses that prevented the initiation and propagation of cracks. The EDS analysis confirmed an increase in potassium concentration and a decrease in sodium content near the surface of samples modified by ion exchange treatment. The treated samples showed appropriate chemical stability in different acidic, basic, and neutral solutions. Conspicuous changes are noticed in the Raman spectra after IET, specifically in the Qn species region. The results indicate the potential of the ion exchange treatment in enhancing the properties of borosilicate glass vials by relatively simple and easily scalable techniques.

Abstract: Carbon-based materials have attracted significant attention in various catalytic applications. However, they are rarely reported for high-temperature catalytic reactions, owing to their limited thermal stability compared to other common materials such as silica and alumina, especially in oxidation reactions. CO2 methanation became a pivotal research hotspot due to its ability to contribute to greenhouse gas mitigation. In addition, CO2 methanation reactions can be carried out below 400 °C in a hydrogen atmosphere, which suits the thermal stability of many modified carbon materials. However, the number of reviews on CO2 methanation does not match the huge number of the experimental publications on CO2 methanation particularly reviews on carbon-supported catalysts. Motivated by the paucity of literature, including reviews on carbon-supported catalysts for CO2 methanation, this review is focused on the catalytic performance of the carbon-supported catalysts of CO2 methanation. It offers significant comparisons among all reported carbon-supported catalysts, providing a comprehensive study on the effect of the carbonaceous supports, such as graphene, biochar, and carbon nanotubes on the catalytic activity. In addition, it investigates the impact of promoters on the catalytic performance of the carbon-supported catalysts in CO2 methanation and highlights the preparation methods and their optimized metal compositions that lead to the highest activity and selectivity. We conclude with a brief synopsis on the current challenges and perspectives on the future directions. This study paves the way for broader usage of carbon-supported catalysts for different thermal catalytic applications, not limited to CO2 methanation.

20. A case report of MoCD etiology in a neonate: A novel homozygous MoCS2 variant

Abstract: Key Clinical Message Molybdenum cofactor deficiency is a rare and fatal genetic disorder. Due to recurrence in the family, the etiological diagnosis could have impacted family planning and alertness to future offspring. Abstract Molybdenum cofactor deficiency (MoCD) is a rare and fatal genetic disorder that impairs molybdenum‐dependent enzymes, resulting in conspicuous elevated urine sulfite levels and lowered serum uric acid levels. The disorder may be early‐onset, causing high fatality in neonates due to secondary complications, or late‐onset, manifesting in the first 2 years of life. Severe seizures, progressive neurological degeneration, motor abnormalities, and feeding difficulties are hallmarks of MoCD. Due to the similarity of clinical findings with those of sulfite oxidase deficiency and its neurological findings with hypoxic–ischemic encephalopathy, determining the true etiology remains challenging in MoCD patients. This case report presents a neonate in the first week of life with early onset refractory seizures, motor abnormalities, hypoactivity, and poor feeding behavior. Administering anti‐epileptic drugs did not improve the patient's condition, who started decompensating further. Nevertheless, a thorough screening for metabolic disorders revealed low serum uric acid and high sulfite levels in the urine, indicating potential MoCD. A whole exome sequencing (WES) was thus consulted for confirmatory diagnosis. Unfortunately, the patient's WES results were received after his demise, revealing MoCD caused by a novel variant of the MoCS2 gene that has not yet been reported to the best of our knowledge. This case emphasizes the need to disseminate crucial information regarding MoCD and its etiologies for prompt molecular diagnosis to reduce morbidity and mortality.

The study started with a review of the literature and previous studies related to critical thinking, its components, and e-communities where the study took place. The researcher designed the study instruments, including a pre-post critical thinking test, and a rubric. The researcher designed the proposed program for developing adult EFL learners' critical thinking. A group of voluntary adult Egyptian EFL learners (n=20) participated in the study. They studied eight online-based sessions, including the pre-post tests. Learners' scores on the pre and post-tests were statistically analyzed using the t-test, and the effect size. Besides, a qualitative analysis of the students' performance and satisfaction was conducted. Both quantitative and qualitative analyses revealed that the proposed program based on an e-community context was effective in developing adults' EFL critical thinking.

Abstract: Background: Osteochondral lesions (OCLs) are a collection of lesions that affect both the articular hyaline cartilage and the subchondral bone plate. Objectives: This study was aimed to analyse the use of advanced magnetic resonance imaging (MRI) using T2 mapping sequence in evaluating OCLs of the ankle joint. Patients and Methods: This study was carried out on 30 cases; were divided in to two groups: patients’ group (n=20): clinically presented with ankle pain 13 females and 7 males. Control group (n=10): five males and five females. All patients were subjected to simultaneous measurement by MRI. Results: The number of diagnoses more than number of patients because there is overlap of diagnosis. By using routine MRI & T2 Mapping at the ankle joint, we found that 14 cases have OCLs, sex cases were diagnosed as grade 1a, four cases were diagnosed as grade 2a, and four cases were diagnosed as grade 3a. Multiple rows were placed at regions of interest of the ankle articular cartilage; we found that T2 values were significantly increased in our cases in comparison to the control group. Conclusion: Intact articular cartilage is characterized by a normal signal intensity in various pulse sequences. In T2 maps, normal ankle articular cartilage at different locations often has a value below 60 milliseconds. An assessment was conducted to compare the effectiveness of MRI alone against MRI using a T2 mapping sequence. There is a considerable level of agreement between using merely MRI and using MRI with a T2 mapping sequence.

Abstract: Objective: The objective of the study y was to evaluate factors which can improve shock wave lithotripsy (SWL) results to keep up with COVID-19 pandemic. Methods: Between June 2020 and June 2021, patients with radio-opaque or faint radio-opaque upper urinary tract stones, stone attenuation value ≤1200 HU, and stones size 11 cm, and body mass index >30 kg/m2 were excluded from the study. Patients were prospectively enrolled in SWL done at a rate of 40–50 SWs/min under combined ultrasound and fluoroscopy-guided, ramped into high power in the 1st 300 shocks. Success rate and complications were recorded. Results: Five hundred and ninety patients completed the study. The success rate after 1st session was 408/590 patients (69.15%) which was augmented by 2nd session to reach 527/590 patients 89.3%. The success rate was 96.2% at 3 months postoperatively. Most complications were mild (Grade 1 or 2). Conclusions: SWL results improved using slow rate high power from the start of the session under combined fluoroscopy and ultrasound guidance. SWL may be a preferred option during a pandemic.

Abstract: Key Clinical Message Contraindications of spinal manipulative therapy (SMT) have been proposed, which mandate rigorous control for its safe practice when manipulating the upper spine. Here, we report a vascular complication of Neck Manipulation in Eagle syndrome (ES) patient. Abstract SMT is used to treat musculoskeletal conditions such as back pain and neck pain by applying force to the spinal joints. Here, we report a rare but devastating complication of SMT, where a young male patient, 22 years old, with ES, had a large pseudoaneurysm from the internal carotid artery (ICA) after SMT from an unlicensed practitioner, treated successfully with endograft. Clinicians administering SMT should consider an elongated styloid process as a potential contraindication to this therapy.

Abstract: Background:. The loss of soft tissue coverage of tendons is a challenging reconstructive problem after acute hand trauma. Subsequent tendon adhesions and the loss of range of movement in addition to the poor aesthetic outcome and donor site scarring should be avoided when deciding the plan of management. Pelnac is one of the commonly used skin substitutes in reconstructive surgery that can be used for coverage of exposed tendons, but the postoperative functional outcome needs to be addressed in detail. Methods:. Twenty-six patients with acute isolated tendon injuries distal to the wrist joint were included. Two-stage reconstructive procedures were performed; the first one was the application of Pelnac. The second stage was carried out after the complete integration of Pelnac via the application of a split-thickness graft. The function outcome assesses the return of the normal range of motion to the affected hand and the QuickDASH score questionnaire. The aesthetic outcome was assessed using the Vancouver scar scale. Results:. The Pelnac was integrated in 100% of cases, with complete grafts taken in 22 of 26 patients. The mean QuickDASH score was 20.5 ± 15.7, and mean Vancouver scar scale was 3.53 ± 3.2. The full range of motion returned in 22 of 26 patients. Conclusions:. Using Pelnac to cover the exposed hand tendons in an acute setting is a convenient and efficient procedure with minimal morbidity. It can offer a good option for their coverage with preservation of hand function and acceptable aesthetic outcome.

Abstract: Abstract Background Plasma cell neoplasm is characterized by complex genetic and prognostic heterogeneity. FAM72D, a gene located on chromosome 1, and the association between its expression and tumor progression and prognosis remain elusive. Methods The present study aims to assess FAM72D mRNA expression in 60 PCM patients and correlate its expression level with clinical and laboratory markers involved in diagnosing and prognosis of PCM using real-time PCR. Results Unpaired t-test revealed a significantly higher FAM72D expression level in the patients than in the control group with a median of 0.890 vs. /0.030, respectively, and p value = 0.000. The highest median level was denoted in newly diagnosed or relapsed patients (1.905, p value = 0.000). A significant positive correlation was found between FAM72D expression level and each of BMPCs count, M band, and β2 microglobulin (p = 0.000, p = 0.002, p = 0.024, respectively), and negative correlations with both serum albumin and hemoglobin level (p = 0.000, p = 0.035, respectively). The risk of relapse was 18.3-fold when the FAM72D level was greater than 1.547. Conclusion The higher FAM72D expression level in newly diagnosed and relapsed myeloma patients and its positive correlation with BMPCs confirm the stimulating effect of FAM72D on myeloma cell proliferation and its poor prognosis.

Abstract: In this work, we introduce a new definition for the fractional differential operator that generalizes several well-known fractional differential operators. In fact, we introduce the notion of the p-proportional ω-weighted κ-Hilfer derivative includes an exponential function, Da,λσ,ρ,p,κ,ω, and then we consider a non-instantaneous impulse differential inclusion containing Da,λσ,ρ,p,κ,ω with order σ∈(1,2) and of kind ρ∈[0,1] in Banach spaces. We deduce the relevant relationship between any solution to the studied problem and the integral equation that corresponds to it, and then, by using an appropriate fixed-point theorem for multi-valued functions, we give two results for the existence of these solutions. In the first result, we show the compactness of the solution set. Next, we introduce the concept of the (p,ω,κ)-generalized Ulam-Hyeres stability of solutions, and, using the properties of the multi-valued weakly Picard operator, we present a result regarding the (p,ω,κ)-generalized Ulam-Rassias stability of the objective problem. Since many fractional differential operators are particular cases of the operator Da,λσ,ρ,p,κ,ω, our work generalizes a number of recent findings. In addition, there are no past works on this kind of fractional differential inclusion, so this work is original and enjoyable. In the last section, we present examples to support our findings.

Abstract: This article is dedicated to propose a spectral solution for the non-linear Fitzhugh–Nagumo equation. The proposed solution is expressed as a double sum of basis functions that are chosen to be the convolved Fibonacci polynomials that generalize the well-known Fibonacci polynomials. In order to be able to apply the proposed collocation method, the operational matrices of derivatives of the convolved Fibonacci polynomials are introduced. The convergence and error analysis of the double expansion are carefully investigated in detail. Some new identities and inequalities regarding the convolved Fibonacci polynomials are introduced for such a study. Some numerical results, along with some comparisons, are provided. The presented results show that our proposed algorithm is efficient and accurate.

Abstract: This paper analyzes a novel use of the shifted Fibonacci polynomials (SFPs) to treat the time-fractional Burgers equation (TFBE). We first develop the fundamental formulas of these polynomials, which include their power series representation and the inversion formula. We establish other new formulas for the SFPs, including integer and fractional derivatives, in order to design the collocation approach for treating the TFBE. These derivative formulas serve as tools that aid in constructing the operational metrics for the integer and fractional derivatives of the SFPs. We use these matrices to transform the problem and its underlying conditions into a system of nonlinear equations that can be treated numerically. An error analysis is analyzed in detail. We also present three illustrative numerical examples and comparisons to test our proposed algorithm. These results showed that the proposed algorithm is advantageous since highly accurate approximate solutions can be obtained by choosing a few terms of retained modes of SFPs.

Abstract: In this article, we construct sufficient conditions that secure the non-emptiness and compactness of the set of antiperiodic solutions of an impulsive fractional differential inclusion involving an ω-weighted ϱ–Hilfer fractional derivative, D0,tσ,v,ϱ,ω, of order σ∈(1,2), in infinite-dimensional Banach spaces. First, we deduce the formula of antiperiodic solutions for the observed problem. Then, we give two theorems regarding the existence of these solutions. In the first, by using a fixed-point theorem for condensing multivalued functions, we show the non-emptiness and compactness of the set of antiperiodic solutions; and in the second, by applying a fixed-point theorem for contraction multivalued functions, we prove the non-emptiness of this set. Because many types of famous fractional differential operators are particular cases from the operator D0,tσ,v,ϱ,ω, our results generalize several recent results. Moreover, there are no previous studies on antiperiodic solutions for this type of fractional differential inclusion, so this work is novel and interesting. We provide two examples to illustrate and support our conclusions.

Abstract: In this research, a compact combination of Chebyshev polynomials is created and used as a spatial basis for the time fractional fourth-order Euler–Bernoulli pinned–pinned beam. The method is based on applying the Petrov–Galerkin procedure to discretize the differential problem into a system of linear algebraic equations with unknown expansion coefficients. Using the efficient Gaussian elimination procedure, we solve the obtained system of equations with matrices of a particular pattern. The L∞{L}_{\infty } and L2{L}_{2} norms estimate the error bound. Three numerical examples were exhibited to verify the theoretical analysis and efficiency of the newly developed algorithm.

Abstract: Abstract Objective Myomectomy is the preferred surgical approach to manage uterine fibroids. However, uterine fibroids are highly vascular tumors and, consequently, extremely susceptible to problems from myomectomy-related hemorrhage. Hence, we aim to compare oxytocin efficacy and safety profile versus tranexamic acid (TA) with ethamsylate for reducing bleeding during myomectomy. Methods This randomized, double-blinded multicenter study was performed between 20th August 2020 and 20th October 2020 at El-Galaa Teaching Hospital, El Hussein University Hospital, Al-Azhar University Hospitals of Assiut, and Al-Azhar University Hospitals of Damietta. One hundred and eighty patients were enrolled and divided into three groups: group (1) received an injection of 30 IU of oxytocin in 500 ml of normal saline; group (2) received injections of 1 g of TA, 250 mg of Ethamsylate, and 110 ml of normal saline IV; and group (3) received an injection of 110 ml of normal saline IV just before surgical incision. Results In 180 premenopausal women, oxytocin and TA with ethamsylate had no significant value in lowering intraoperative blood loss compared with the placebo for abdominal myomectomy (666.25 ± 183.03, 630.72 ± 145.83, and 646.67 ± 168.92, respectively (P = 0.506)). Non-significant trends were observed for a reduction in operation time (P = 0.760), intra/postoperative blood transfusion (P = 0.624), hospital stay (P = 0.986), postoperative fever (P = 0.659), and wound infection (P = 1). Conclusion Oxytocin and TA with ethamsylate had no significant value in lowering intraoperative blood loss compared with the placebo for abdominal myomectomy which opens a new question about the role of the use of the hemostatic drug during myomectomy especially in centers with limited resources and had higher rates. Trial registration The study was registered on Pan African Clinical Trials Registry with the following number: PACTR202008739887429 and was approved on 24/08/2020.

Abstract: Abstract Objective Cesarean Section (CS) is associated with an increased risk of hemorrhage. Many drugs are used to decrease this risk. We aim to compare the combination of ethamsylate and tranexamic acid, oxytocin, and placebo in women undergoing CS. Methods We conducted a double-blinded, randomized, placebo-controlled trial between October and December 2020 in four university hospitals in Egypt. The study included all pregnant women in labor without any complications who accepted to participate in the study between October and December 2020. The participants were divided into three groups. The subjects were randomly allocated to receive either oxytocin (30 IU in 500 ml normal saline during cesarean section), combined one gram of tranexamic acid with 250 mg of ethamsylate once before skin incision, or distilled water. Our main outcome was the amount of blood loss during the operation. The secondary outcomes were the need for blood transfusion, hemoglobin and hematocrit changes, hospital stay, operative complications, and the need for a hysterectomy. The one-way ANCOVA test was used to compare the quantitative variables between the three groups while the Chi-square test was used to compare the qualitative variables. Post hoc analysis then was performed to compare the difference between every two groups regarding the quantitative variables. Results Our study included 300 patients who were divided equally into three groups. Tranexamic acid with ethamsylate showed the least intra-operative blood loss (605.34 ± 158.8 ml) compared to oxytocin (625.26 ± 144.06) and placebo (669.73 ± 170.69), P = 0.015. In post hoc analysis, only tranexamic acid with ethamsylate was effective in decreasing the blood loss compared to placebo (P = 0.013); however, oxytocin did not reduce blood loss compared to saline (P = 0.211) nor to tranexamic acid with ethamsylate (P = 1).
