Abstract Background Subclinical venous thromboembolism is a hidden pathology which may present with catastrophic consequences if not diagnosed at an early stage. This study was undertaken to estimate the occurrence and associated risk factors of subclinical deep vein thrombosis after lung resection for lung cancer patients. A prospective observational cohort study was performed in a tertiary cardiothoracic surgery center. One hundred fifty patients who underwent different types of lung resection for lung cancer were enrolled. Caprini’s risk score was assessed in all patients. All patients received prophylactic stockings and anticoagulants. On the 5th postoperative day, a duplex venous ultrasound of bilateral lower limbs was performed on every asymptomatic patient. Results Out of 150 patients enrolled in the study, 147 patients completed the study. Four patients (2.72%) developed subclinical deep vein thrombosis. The patients were divided into 2 groups: group 1 (n = 143) post-lung resection and no DVT and group 2 (n = 4) with post-lung resection subclinical DVT. No patient developed postoperative clinical DVT. The incidence was found to be highest in the group of individuals who had a longer stay in the ICU (odds ratio 37.9) (p = 0.04). Among the various pathologies, the incidence was higher in patients who received preoperative chemotherapy (odds ratio 21.9) (p = 0.001). One patient in the subclinical DVT group (25%) died, while no mortality was observed in the no DVT group. Conclusions The incidence of subclinical deep vein thrombosis is low in the postoperative period among patients undergoing lung resection for lung cancer if appropriate prophylactic measures are applied. However, patients receiving preoperative chemotherapy and those with longer periods of immobilization are at a higher risk of developing postoperative DVT despite anticoagulant prophylaxis. Due to the sample size and design limitations, the mentioned risk factors could be associated with DVT not a cause of DVT. It might be justified to screen these high-risk groups to detect subclinical DVT to allow for post-discharge prophylaxis.

The plurality of nation in this title foregrounds the challenge of teaching a geopolitical entity whose survival depends on building emotional ties of belonging. These ties can be problematic in diverse societies in which collective identities compete for recognition. In Canada, nationhood tied to language and culture is claimed by French-speaking Quebecers; it is also invoked by many Western-Canadian politicians to express a growing alienation from Eastern Canada's perceived socio-economic dominance. In Canada's constitution, the term First Nations represents the indigenous peoples who are the country's original inhabitants. In this context, teaching the nation(s) is indeed challenging. In response, the authors adopt duoethnography as both research methodology and pedagogy in their content-based English for Academic Purposes (EAP) courses. They first explore their experiences and emotional attachments to nationhood, reflecting on their influences on teaching around language and citizenship. They then provide two EAP assignments as examples: The first is a course assignment in which students critically examine hyphenated national identities through duoethnographic inquiry. The second is called the Get Involved project, which examines service learning and citizenship. Both examples demonstrate the importance of critical affective literacies to expand the pedagogical repertoires of EAP teachers and students in a time of resurgent nationalism.

Hydraulic steel gates are commonly used for their superior characteristics in carrying both tensile and compressive stresses. Corrosion, operation, and maintenance costs make it essential to search for suitable alternatives. In this research, Glass Fiber Reinforced Polymers (GFRP) were investigated as a promising choice that can be used to resist flexure stresses as in case of hydraulic gates. Firstly, typical mechanical tests were conducted on nine specimens made of pure steel beams, four ribbed steel beams, twenty-four specimens of pure GFRP beams, along with four combined specimens from both materials. GFRP beams were also used in cooperation with polyester cemented sand to form a sandwich like beam. It was concluded that beams from GFRP achieved similar flexural capacity of steel beams of slimmer thickness. Combinations of steel and GFRP show satisfactory results while maintaining member ductility. The load capacity of the strengthened steel beams increased by 80% to 97% compared to specimens made from steel only. Failure modes for all specimens were introduced and compared. Moreover, numerical investigation was conducted using ABAQUS nonlinear module. Finite element models for three small gates of dimensions (1.0 x 1.0 m). A steel gate of thickness 14mm, Pure GFRP gates of thicknesses 29mm and 45mm were simulated. The 29mm GFRP gate was able to bear almost the same loads of the steel gate with equivalent deflection. Finally, it can be concluded that for small-scale hydraulic gates, GFRP can effectively be used as a suitable alternative for steel gates after setting limitations for failure strains.

Abstract Investigation of a unique and fast method for the determination and separation of a mixture of three drugs viz., ciprofloxacin (CIP), Ibuprofen (IBU), and diclofenac sodium (DIC) in actual samples of human plasma. Also, the technique was used to look at their pharmacokinetics study. Hydrocortisone was chosen as the internal standard (IS). The drugs were chromatographically separated using an Acquity ultra-performance liquid chromatography UPLC ® BEH C18 1.7 µm (2.1 × 150 mm) column with a mobile phase composed of acetonitrile: water (65:35, v/v) adjusted to pH 3 with diluted acetic acid. Plasma proteins were precipitated with acetonitrile. The separated drugs ranged from 0.3 to 10, 0.2–11, and 1–25 µg/mL for CIP, IBU, and DIC, respectively. Calibration curves were discovered to achieve linearity with acceptable correlation coefficients (0.99%). Examination of quality assurance samples showed exceptional precision and accuracy. Following the successful application of this improved technique to plasma samples, the pharmacokinetic characteristics of each selected drug were evaluated using (UPLC) with UV detection at 210 nm. Two green metrics were applied, the Analytical Eco-scale and the Analytical GREEnness Calculator (AGREE). Separation was achieved in only 4-min analysis time. The method's validation agreed with the requirements of the FDA, and the results were sufficient.

The Ca2+_Mg2+ ATPase are high attraction calcium pump, that contributes in maintaining plasma membrane of cytoplasm Ca2+, Mg2+ homeostasis by source to the outside of cell. The aim of the study is to evaluate the effect of the ultraviolet radiation on the activity of the Ca2+_Mg2+ ATPase in the membranous vesicles of ileum in rats. Thirty adult Sprague–Dawley rats (age, 3-4 months, weight range, 180 – 200 g) were used in this experiment, which divided in to 5 groups (n = 6 / group). The membrane vesicles isolated from smooth muscles of rats showed high activity Ca2+_Mg2+ ATPase. All isolated membranous vesicles are irradiated with Ultraviolet radiation of 250 nanometers except control group. The irradiation period for each group was (5, 10, 30 and 45) minutes, respectively. The activity of Ca2+_Mg2+ ATPase was decreased with increased time of irradiation. In conclusion, the increased time of irradiation inhibited Ca2+_Mg2+ ATPase activity isolated from ileum smooth muscles of rats. The recommendations are to expose other organs like liver and kidney to UV radiation to explain its effect or using other range of UV radiation to reflect its effect.

BackgroundProgressive supranuclear palsy is a neurodegenerative disorder associated with tau protein aggregation. Tilavonemab (ABBV-8E12) is a monoclonal antibody that binds to the N-terminus of human tau. We assessed the safety and efficacy of tilavonemab for the treatment of progressive supranuclear palsy.MethodsWe did a phase 2, multicentre, randomised, placebo-controlled, double-blind study at 66 hospitals and clinics in Australia, Canada, France, Germany, Italy, Japan, Spain, and the USA. Participants (aged ≥40 years) diagnosed with possible or probable progressive supranuclear palsy who were symptomatic for less than 5 years, had a reliable study partner, and were able to walk five steps with minimal assistance, were randomly assigned (1:1:1) by interactive response technology to tilavonemab 2000 mg, tilavonemab 4000 mg, or matching placebo administered intravenously on days 1, 15, and 29, then every 28 days through to the end of the 52-week treatment period. Randomisation was done by the randomisation specialist of the study sponsor, who did not otherwise participate in the study. The sponsor, investigators, and participants were unaware of treatment allocations. The primary endpoint was the change from baseline to week 52 in the Progressive Supranuclear Palsy Rating Scale (PSPRS) total score in the intention-to-treat population. Adverse events were monitored in participants who received at least one dose of study drug. Prespecified interim futility criteria were based on a model-based effect size of 0 or lower when 60 participants had completed the 52-week treatment period and 0·12 or lower when 120 participants had completed the 52-week treatment period. This study is registered at ClinicalTrials.gov, number NCT02985879.FindingsBetween Dec 12, 2016, and Dec 31, 2018, 466 participants were screened, 378 were randomised. The study was terminated on July 3, 2019, after prespecified futility criteria were met at the second interim analysis. A total of 377 participants received at least one dose of study drug and were included in the efficacy and safety analyses (2000 mg, n=126; 4000 mg, n=125; placebo, n=126). Least squares mean change from baseline to week 52 in PSPRS was similar in all groups (between-group difference vs placebo: 2000 mg, 0·0 [95% CI -2·6 to 2·6], effect size 0·000, p>0·99; 4000 mg, 1·0 [-1·6 to 3·6], -0·105, p=0·46). Most participants reported at least one adverse event (2000 mg, 111 [88%]; 4000 mg, 111 [89%]; placebo, 108 [86%]). Fall was the most common adverse event (2000 mg, 42 [33%]; 4000 mg, 54 [43%]; placebo, 49 [39%]). Proportions of patients with serious adverse events were similar among groups (2000 mg, 29 [23%]; 4000 mg, 34 [27%]; placebo, 33 [26%]). Fall was the most common treatment-emergent serious adverse event (2000 mg, five [4%]; 4000 mg, six [5%]; placebo, six [5%]). 26 deaths occurred during the study (2000 mg, nine [7%]; 4000 mg, nine [7%]; placebo, eight [6%]) but none was drug related.InterpretationA similar safety profile was seen in all treatment groups. No beneficial treatment effects were recorded. Although this study did not provide evidence of efficacy in progressive supranuclear palsy, the findings provide potentially useful information for future investigations of passive immunisation using tau antibodies for progressive supranuclear palsy.FundingAbbVie Inc.

In the Framework of identifying defects in structures, Damage Index Method (DIM) is considered one of the trustable tools used intensively. In the present research, DIM – modal displacement based - was applied on experimental data to detect damage for masonry piers. Damage index (DI) was obtained by comparing modal displacement functions for damaged and sound conditions. Firstly, two scaled down piers were prepared experimentally for examination. The piers were of same unit area while the thicknesses were 170, and 260 mm, respectively. The physical models were subjected to different artificial crack patterns. The cracks were created for each pier on three steps representing different intensity of damage. The piers were then examined under free vibration mode at sound condition and after each degradation. The collected records were processed, and DIM was applied. Furthermore, parametric numerical analysis was used to study the effect of crack dimensions, location, shape, and repetition among the simulated piers. The results showed that the selected DI was effective in detecting cracks in masonry structures for all examined cases. Moreover, the investigation showed that as the severity of cracks increased, the identification of damage became easier and was reflected in an increase in DI value. DIM was also able to detect expected route of cracks. On the other hand, DIM can’t be used to obtain degradation level directly but for comparison between severity of cracks in different stages of damage.

Abstract Background The current study was designed to investigate the in vitro effect of commercially available fasciolicides; albendazole (40 and 400 µg/ml), triclabendazole, rafoxanide and nitroxynil (50 and 100 µg/ml, each) against Fasciola gigantica adult worms. For all, worms were incubated for 3 h. Worm's motility was macroscopically and microscopically detected. Reduction of egg deposition was estimated. Alterations of worm's cuticle post-treatments were recorded using scanning electron microscopy (SEM). Results Nitroxynil had the most flukicidal effect with mild movement quickly disappeared within 15 min post-treatment. It showed the highest egg reduction (88.3% and 95% at concentrations of 50 and 100 µg/ml, respectively). Findings of SEM showed severe furrowing and destruction of spines. In rafoxanide-treated group, the motility disappeared 75 min post-treatment, and the egg deposition was significantly (P ≤ 0.05) reduced to 70% and 85% at the same concentrations. Teguments showed thickening, moderate furrowing and destruction of the spines. Albendazole showed the lowest effect: the motility of the worms was observed till 160 min post-treatment and the egg reduction was 43% and 75% at the same concentrations. Interestingly, in albendazole-treated flukes, the tegument had severe furrowing and spines were completely sloughed. Similarly, in triclabendazole-treated flukes, worms motility was observed till 160 min post-treatment and the egg reduction was 76.6% and 88.3%. The tegument showed swelling and mild furrowing with moderately damaged spines. Conclusions Nitroxynil was the most potent flukicide inducing evidential cuticular changes. Although albendazole induced the most potent cuticular damage, it showed the lowest flukicidal effect. Further in vivo study to investigate resistance/susceptibility of Fasciola species in cattle and buffaloes will be carried out.

Postmemory, the narrativised intergenerational transfer of often traumatic experiences, is a crucial component of multilingual identity negotiation. In this article, we focus specifically on the curricular interactions and personal and collective aspirations of multilingual students who use English for academic purposes. We situate our discussions in the literature on critical pedagogy, affect/emotion theory, and memory studies. We utilise duoethnography as a methodology for our dialogic inquiry. A duoethnographic approach enables us to be both self-reflexive and socially transformative through our explorations of lived experiences of language loss and gain and of our historical becoming of professional language educators. We highlight how multilingual identities are constructed, challenged, and reconstructed not only by social practices of sign-use, but also by intergenerational spatial mobility and the distributed nature of postmemory. Finally, we provide pedagogical implications for language education that seek to foster critical affective literacies. Turning to affect and emotion is important to move the discussion of multilingual identities beyond physical signifiers of social differentiation (i.e. race, gender, ethnicity, and class). Pedagogical attention to memory, affect and identity may offer us a more nuanced understanding of teachers' and students' agency and investment in multilingual semiotic practices.

Abstract Background Functional mapping of eloquent brain areas is crucial for preoperative planning in patients with brain tumors. Resting state functional MRI (rs-fMRI) allows the localization of functional brain areas without the need for task performance, making it well-suited for the pediatric population. In this study the independent component analysis (ICA) rs-fMRI functional mapping results are reported in a group of 22 pediatric patients with supratentorial brain tumors. Additionally, the functional connectivity (FC) maps of the sensori-motor network (SMN) obtained using ICA and seed-based analysis (SBA) are compared. Results Different resting state networks (RSNs) were extracted using ICA with varying levels of sensitivity, notably, the SMN was identified in 100% of patients, followed by the Default mode network (DMN) (91%) and Language networks (80%). Additionally, FC maps of the SMN extracted by SBA were more extensive (mean volume = 25,288.36 mm3, standard deviation = 13,364.36 mm3) than those found on ICA (mean volume = 13,403.27 mm3, standard deviation = 9755.661 mm3). This study performed a sensitivity analysis to correlate the frictional head loss calculated by the Darcy-Weisbach (D-W) and the Hazen-Williams (H-W) formulas. For a broad variety of fluid temperatures, velocities, and pipeline diameters, this study considered an extensive discussion and analysis to determine friction loss within pressurized pipelines using Microsoft Excel. Regression analysis and statistical tools were applied to improve the relationship between the two equations. A more accurate expression was developed to calculate the friction loss in terms of the H-W equation. The estimated values were compared with previous experimental and numerical studies, and a good agreement was observed. The proposed model was evaluated using WaterGEMS software in an application example of a water supply system against the D-W and H-W equations. Good agreements were recorded between predicted values and previous studies, with an error of less than 1%. These findings can be used to improve the hydraulic design of engineering applications.
