Purpose The present study aimed to determine the serum level of asprosin, a novel glucogenic and orexigenic hormone, in NAFLD and hypothyroidism patients.Methods This is a case–control study that included three groups of age and sex-matched subjects: hypothyroid patients with NAFLD (n = 26), hypothyroid patients without NAFLD (n = 25) and normal control subjects (n = 27). A detailed clinical history was obtained, and an examination was performed. Laboratory tests included fasting plasma glucose and insulin, lipid profile, TSH, free T4, AST, ALT, albumin, and asprosin. Recent developments in large language models (LLMs) have unlocked opportunities for healthcare, from information synthesis to clinical decision support. These LLMs are not just capable of modeling language, but can also act as intelligent “agents” that interact with stakeholders in open-ended conversations and even influence clinical decision-making. Rather than relying on benchmarks that measure a model’s ability to process clinical data or answer standardized test questions, LLM agents can be modeled in high-fidelity simulations of clinical settings and should be assessed for their impact on clinical workflows. These evaluation frameworks, which we refer to as “Artificial Intelligence Structured Clinical Examinations” (“AI-SCE”), can draw from comparable technologies where machines operate with varying degrees of self-governance, such as self-driving cars, in dynamic environments with multiple stakeholders. Developing these robust, real-world clinical evaluations will be crucial towards deploying LLM agents in medical settings.

Abstract Human editors and proofreaders now face a new, and possibly serious, challenge: the emergence of artificial intelligence (AI) tools that some consider to be as efficient and precise as experts in editing/proofreading. This research aims to investigate editors’ and proofreaders’ perceptions of current AI tools. It examines whether editors/proofreaders view AI as an opportunity or a threat and considers their insights into the future of AI tools for them. The study collected qualitative data through email questionnaires from 17 professional editors and proofreaders purposively appointed from a society of professional editors and proofreaders in Egypt. The results revealed that the responses regarding AI for editors and proofreaders are generally mixed, with a range of both positive and negative perspectives. Some responses highlight the opportunities and benefits that AI tools can bring, such as increased efficiency, time-saving, and improved productivity. Others express concerns about potential threats, such as the possibility of AI replacing humans, ethical considerations, and the need for continued human involvement in the editing/proofreading process. Overall, the attitudes toward AI tools for editing and proofreading reflect a paradoxical view of the technology’s impact on the field. The active engagement and participation of editors and proofreaders are essential for the successful implementation of AI technologies in editorial contexts.

Introduction: Irritable bowel syndrome (IBS) is the most common functional gastrointestinal tract disorder. Bacillus coagulans (BC) is a potent spore-forming probiotic that has been proposed as a potent supplement that can improve gastrointestinal tract dysbiosis and relieve IBS symptoms. We aim to assess the effect of BC on different IBS symptoms severity among other IBS-related symptoms. Methods: We searched seven databases for RCTs that compare the use of BC versus placebo in the management of IBS symptoms. Results: Our search finally retrieved seven RCTs after extensive screening. Abstract Background We compared the rate, type and severity of vulvovaginitis in women who are practicing internal vaginal douching (IVD) or not while using an intrauterine contraceptive device (IUCD). It is a cross-sectional study on 604 consecutive attendees of the outpatient clinics using IUCD for at least 6 months prior to recruitment. Eligible participants were divided into two groups: group 1 (302 women) performing IVD and group 2 (302 women) not practicing this habit (NIVD). The frequency of vaginal infections in IUCD users who perform IVD compared to women who were abstaining from this habit was our primary outcome. Quadrotors have been utilized in a variety of applications where the payload needs to be transferred from one location to another, such as in mine detection, load delivery through hazardous environments, or locations that are inaccessible to ground vehicles. In these applications, the payload will be suspended underneath the vehicle and is subjected to large oscillations due to external disturbances or the acceleration of the vehicle itself. In this article, the most recent advancements in controlling the quadrotors vehicles with suspended loads are presented including control algorithms and techniques to implement these controllers. These techniques include the hardware and sensors besides the testing techniques for the developed control algorithms.

Mixed convective heat transfer and pressure drop penalty of nanofluids flow in an isothermal horizontal tube are numerically examined in developed flow region. The study examines three types of nanofluids, simple nanofluids ([Water]/ Al2O3, TiO2, and Cu), Hybrid nanofluids ([Water]/ Al2O3 + Cu), and Ionic nanofluids ([C4mim] [NTf2]/ Al2O3). Richardson number is varied from 0.016 to 2, and Reynolds number is varied from 500 to 2000. The governing equations are solved numerically via the finite volume method by using the SIMPLER algorithm computer code. The computer code is validated by comparing the average Nusselt number with the experimental published data, a good agreement was observed. Performance evaluation criterion (λ) is introduced to evaluate the heat transfer enhancement gain of nanofluid usage to pressure drop penalty at different concentrations of nanoparticles. Results for nanofluids show that the maximum enhancement of the average Nusselt number is 15.5 % for Al2O3 with a concentration of 2% at Richardson number of 0.016. However, for hybrid nanofluids, no enhancement is noticed. Ionic nanofluid results are promising, as the Nusselt number increases significantly (by 37%) with a concentration of 2.5%. Finally, findings of various types of nanofluids investigated in the same numerical conditions are reported and compared.

Electro-kinetic remediation technology is one of the developing technologies that offer great promise for the cleanup of soils contaminated with heavy metals. A numerical model was formulated to simulate copper (Cu) transport under an electric field using one-dimensional diffusion-advection equations describing the contaminant transport driven by chemical and electrical gradients in soil during the electro-kinetic remediation as a function of time and space. This model included complex physicochemical factors affecting the transport phenomena, such as soil pH value, aqueous phase reaction, adsorption, and precipitation. One-dimensional finitedifference computer program successfully predicted meaningful values for soil pH profiles and Cu concentration profiles. The model considers that: (1) electrical potential in the soil is constant with the time; (2) the effect of temperature is negligible; and (3) dissolution of soil constituents is negligible. The predicted pH profiles and transport of copper in sandy loam soil during electrokinetic remediation were found to reasonably agree with the bench-scale electro-kinetic experimental results. The predicted contaminant speciation and distribution (aqueous, adsorbed, and precipitated) allow for an understanding of the transport processes and chemical reactions that control electro-kinetic remediation.

The current study was designed to evaluate the changes in vaginal cytology and serum progesterone and estrogen concentration in anestrum bitches treated with a combination of FSH and LH. Seven healthy Mongrel anestrum bitches were treated with daily administration of 75 I.U. FSH plus 75 I.U LH for consecutive nine days and another three bitches were kept as a control without any treatment. Treated and control bitches were examined for the percentages of different vaginal-cytology cells, serum concentration of estrogen and progesterone, and signs of proestrus and estrus in a scheduled timeline. Five of the seven treated bitches (71.4%) showed a gradual decrease in the percentage of parabasal cells, a gradual increase in the percentage of cornified and RBCs, a rapid decrease in the estrogen concentration, and an increase in the progesterone concentration. These five bitches showed signs of proestrus and four of them showed male acceptance and were conceived. These five bitches had high estrogen concentrations before initiation of the treatment. The other two bitches had a low estrogen concentration before the initiation of the treatment and after treatment, they showed a gradual increase in the estrogen concentration, but they did not show characteristic changes in vaginal cytology or progesterone concentration. Control bitches had high estrogen concentrations before treatment, but they did not show characteristic changes in vaginal cytology, estrogen and progesterone concentrations, or signs of proestrus up to one month after the end of the treatment. In conclusion, treating anestrum bitches with a mixture of FSH and LH induced characteristic changes in vaginal cytology and serum estrogen and progesterone concentrations in the majority of bitches. These changes were associated with the induction of fertile estrus in more than half of the treated bitches, and this percentage increased to about 80% in bitches having initial high estrogen concentration. Precise and timely referral for lung transplantation is critical for the survival of cystic fibrosis patients with terminal illness. While machine learning (ML) models have been shown to achieve significant improvement in prognostic accuracy over current referral guidelines, the external validity of these models and their resulting referral policies has not been fully investigated. Here, we studied the external validity of machine learning-based prognostic models using annual follow-up data from the UK and Canadian Cystic Fibrosis Registries. Using a state-of-the-art automated ML framework, we derived a model for predicting poor clinical outcomes in patients enrolled in the UK registry, and conducted external validation of the derived model using the Canadian Cystic Fibrosis Registry. In particular, we studied the effect of (1) natural variations in patient characteristics across populations and (2) differences in clinical practice on the external validity of ML-based prognostic scores. Overall, decrease in prognostic accuracy on the external validation set (AUCROC: 0.88, 95% CI 0.88-0.88) was observed compared to the internal validation accuracy (AUCROC: 0.91, 95% CI 0.90-0.92). Based on our ML model, analysis on feature contributions and risk strata revealed that, while external validation of ML models exhibited high precision on average, both factors (1) and (2) can undermine the external validity of ML models in patient subgroups with moderate risk for poor outcomes. A significant boost in prognostic power (F1 score) from 0.33 (95% CI 0.31-0.35) to 0.45 (95% CI 0.45-0.45) was observed in external validation when variations in these subgroups were accounted in our model. Our study highlighted the significance of external validation of ML models for cystic fibrosis prognostication. The uncovered insights on key risk factors and patient subgroups can be used to guide the cross-population adaptation of ML-based models and inspire new research on applying transfer learning methods for fine-tuning ML models to cope with regional variations in clinical care.
