2. Activation of NLRP3 inflammasome in patients with renal transplantation: relation to allograft dysfunction, inflammation, and renal fibrosis

Background Chronic allograft dysfunction (CAD) represents a complex process of inflammatory response, fibrosis, and tissue restoration, mediated by a myriad of proinflammatory cytokines, enzymes, and growth factors. NOD-like receptor protein3 (NLRP3) inflammasome is one of the NLR protein families. It drives proteolysis of proinflammatory caspases which induces maturation of pro-interleukin (IL)-1β and pro-IL-18. The aim of this work is to study NLRP3 inflammasome activation in patients with renal transplantation in relation to allograft function, inflammation, and renal fibrosis.Methods Thirty renal transplant recipients (RTR): 15 patients with stable renal function (Group I), 15 patients with CAD (Group II), and 15 healthy subjects (Group III) were included. Serum and urinary NLRP3, caspase-1, and interleukin-1β were measured using an enzyme-linked immunosorbent assay (ELISA) kit. Serum high sensitivity C-reactive protein (hs-CRP) level was measured, and neutrophil-lymphocyte ratio (NLR) was calculated. Renal function was evaluated by serum creatinine (S.Cr) and estimated glomerular filtration rate (eGFR). Resistive index (RI) was calculated with Doppler ultrasonography. In patients with CAD, renal interstitial fibrosis (RIF) was graded in renal biopsies.Results Serum NLRP3, caspase-1, serum and urinary IL-1β, and hs-CRP showed a statistically significant increase in RTR, particularly in those with CAD compared to the controls with a positive correlation with each other, their urinary level, S. 5. The Utility of [18]F-Fluorocholine Positron Emission Computed Tomography and [18]F-Fluorodeoxyglucose Positron Emission Tomography–Computed Tomography in Evaluating Breast Cancer Phenotypes: A Pilot Study

Background: The utility of the [18]F fluorodeoxyglucose positron emission tomography–computed tomography ([18]F FDG PET-CT) marker for breast cancer is well established. Given its limitations in localizing FDG-negative malignant tumors, the expression of [18]F-fluorocholine ([18]-FCH) may potentially be helpful to improve the overall accuracy in evaluating breast cancer. This study determined the potential of [18]- FCH PET CT as a potential marker in assessing breast cancer phenotypes. Methods: We recruited consecutive patients with biopsy-proven breast carcinoma who underwent [18] F-FCH PET-CT following the [18]F-FDG PET-CT imaging. The subjects were dichotomized into human epidermal growth factor receptor 2 (HER2)-negative and HER2-positive genotypes. The maximum standardized uptake value (SUVmax; g/dL) was used to predict the two groups of variables. Global health status (GHS) score based on the EORTC quality of life questionnaire (QLQ) was used to evaluate the outcome of the cohort subjects at 6, 12, and 24 months. Results: There were 21 females with a mean age of 54.48 ± 12.17 years. Eighteen patients had invasive ductal carcinoma (18/21;85.8%) on histology, with 11 (52.4%) were HER2-negative genotype. There was higher sensitivity and specificity of [18]-FCH-PET/CT in breast lesions at 40% and 68.8% compared to [18]FDGPET/CT with 33.3% and 66.7%, respectively. There were significant differences between [18]F-FCH SUVmax (g/dL) of the HER-negative as compared to the HER2- positive group (1.99 g/dL vs. 0.2 g/dL; P < .05). High SUVmax (g/dL) of [18]F-FCH had predicted the HER-negative genotype at the cutoff value of 0.75 (P < .05). High [18]F-FCH showed significantly poor scoring of GHS parameters compared to low FCH at 6 months (mean SUVmax 8.06 vs. 5.40 respectively; P < .05). Conclusion: [18]F-FCH PET-CT is a potential marker in localizing and predicting aggressive breast carcinoma phenotypes.

Abstract An ever-growing number of students are being diagnosed with Autism Spectrum Disorder (ASD) in Saudi Arabian schools, creating a need for highly effective, research-based strategies for teaching mathematics. However, current research on such strategies is limited. Nevertheless, several studies, including those by Wisniewski and Smith (2002) and Scott (1993), have found the touch-point strategy effective in developing and retaining single-digit addition and mathematical problem-solving skills in students with disabilities, as well as in retaining addition skills among students with ASD. The researchers conducted single-subject research using a reversal design (A-B-A-B) to determine the intervention’s effect on three students with ASD as the main data source. Additionally, an interview with the math teacher of these participants was conducted to gather more in-depth data, which helped explore the effectiveness and challenges of using the touch-point strategy in teaching math addition facts to Saudi students with ASD. Significant improvements in computation and acquisition rate were noted among all participants when the strategy was used to solve single-digit addition problems. Two major themes emerged from the analysis of the interview data: the need for resources to teach the strategy and challenges in implementing the touch-point strategy across the whole class, given the individual needs of each student.

Counting planthoppers manually is laborious and yields inconsistent results, particularly when dealing with species with similar features, such as the brown planthopper (Nilaparvata lugens; BPH), whitebacked planthopper (Sogatella furcifera; WBPH), zigzag leafhopper (Maiestas dorsalis; ZIGZAG), and green leafhopper (Nephotettix malayanus and Nephotettix virescens; GLH). Most of the available automated counting methods are limited to populations of a small density and often do not consider those with a high density, which require more complex solutions due to overlapping objects. Therefore, this research presents a comprehensive assessment of an object detection algorithm specifically developed to precisely detect and quantify planthoppers. It utilises annotated datasets obtained from sticky light traps, comprising 1654 images across four distinct classes of planthoppers and one class of benign insects. The datasets were subjected to data augmentation and utilised to train four convolutional object detection models based on transfer learning. The results indicated that Faster R-CNN VGG 16 outperformed other models, achieving a mean average precision (mAP) score of 97.69% and exhibiting exceptional accuracy in classifying all planthopper categories. The correctness of the model was verified by entomologists, who confirmed a classification and counting accuracy rate of 98.84%. Nevertheless, the model fails to recognise certain samples because of the high density of the population and the significant overlap among them. This research effectively resolved the issue of low- to medium-density samples by achieving very precise and rapid detection and counting.

The yearning to win sports competitions has led some athletes to dope. Doping in sports is a real threat to the ‘Spirit of Sport’ and fairness. The pharmacokinetics of performance-enhancing drugs differ, as do their effects and purposes of use. As one of the most effective and decisive solutions, the idea to issue a prohibited list came to raise the legal awareness level among athletes about the types of prohibited substances and methods they have to avoid and in which time specifically. In addition, for the sake of broader and more comprehensive cooperation between the law, medicine, and pharmacology, to confront the phenomenon, and limit it to the narrowest possible scope on the other hand. The idea to issue the prohibited list came. Historical, descriptive, and legal approaches are employed in conducting this review. Additionally, the method of conceptual analysis is used to discover the exact normative terminology. The most significant finding for this review is that the issuance of the Prohibited List brought greater stability to sporting events. Its annual issuance is legal proof in front of everyone (countries, international sports organisations, and athletes).

Being one of the highest water-risk ranked countries in the world, Kuwait is in an urgent need of new advanced techniques and methodologies to conserve and optimize its water infrastructure. Water consumption, water quality, and network design optimization are among the challenges that face Kuwait water distribution system. This study is a step forward towards the development of an EPANET-based model, which is widely used in various water systems applications worldwide, for different parts of Kuwait water networks. An EPANET model was developed and analyzed for a selected area in Kuwait. The study evaluated the water quality in the chosen area, in particular, the water age. Several realistic scenarios based on different population occupations were considered. The dependence of the water age and the water quality in water distribution networks on the occupation stages for the given area was highlighted. The analysis of the water network showed that partial occupation can hinder the water quality, hence different scenarios shall be included in the design phase of water networks.

Aberrant aggregation of the transactive response DNA-binding protein (TDP-43) is associated with several lethal neurodegenerative diseases, including amyotrophic lateral sclerosis and frontotemporal dementia. Cytoplasmic neuronal inclusions of TDP-43 are enriched in various fragments of the low-complexity C-terminal domain and are associated with different neurotoxicity. Here we dissect the structural basis of TDP-43 polymorphism using magic-angle spinning solid-state NMR spectroscopy in combination with electron microscopy and Fourier-transform infrared spectroscopy. We demonstrate that various low-complexity C-terminal fragments, namely TDP-13 (TDP-43300–414), TDP-11 (TDP-43300–399), and TDP-10 (TDP-43314–414), adopt distinct polymorphic structures in their amyloid fibrillar state. Our work demonstrates that the removal of less than 10% of the low-complexity sequence at N- and C-termini generates amyloid fibrils with comparable macroscopic features but different local structural arrangement. It highlights that the assembly mechanism of TDP-43, in addition to the aggregation of the hydrophobic region, is also driven by complex interactions involving low-complexity aggregation-prone segments that are a potential source of structural polymorphism.

Anthropogenic activities in the environment affect the ecosystem and can play an important role in selecting and spreading antibiotic‐resistant bacteria (ARB) and genes (ARGs). The dairy farm environment may serve as a hotspot and reservoir for exchanging and spreading ARGs, but studies are scarce. Here, we investigated and characterized the extended‐spectrum β‐lactamase producing Escherichia coli strains recovered from the dairy farm environment co‐harboring blaOXA−1, catB3, and arr-3 genes. The isolates were identified and characterized by PCR, antimicrobial susceptibility testing, conjugation assay, whole genome sequencing (WGS), and multiple bioinformatics tools. Seven E. coli strains co‐harboring blaOXA−1, catB3, and arr-3 genes were identified which belonged to distinct sequence types (STs) and carried diverse plasmid replicon types. The conjugation assay revealed a successful transfer of blaOXA−1, catB3, and arr-3 genes into the recipient E. coli J53 with a co‐conjugation frequency ranging from (2.25 ± 0.3) × 10−4 to (3.85 ± 0.3) × 10−3. Bioinformatics analysis of WGS revealed the diversity of acquired ARGs, conferring resistance to aminoglycosides, beta‐lactams, quinolones, tetracyclines, macrolides, trimethoprim–sulfamethoxazole, phosphonic, phenicol, and rifamycin. The genetic environment analysis showed that aac(6′)‐Ib-cr-blaOXA−1‐catB3-arr-3-qacE1-sul1 was the common genetic backbone among the seven E. coli strains. Among the mobile genetic elements, insertion sequences were the predominant elements as compared to transposons. The phylogenetic analysis demonstrated a close relationship between the E. coli of this study and other strains of human–animal‐environment origin retrieved from the NCBI database. This study presented the whole genome‐based characterization of E. coli strains carrying the blaOXA−1‐catB3-arr-3 genes. It provided evidence that the dairy environment may harbor a variety of ARGs and act as a potential reservoir for their spread in the ecosystem. The results recommend the routine surveillance of ARGs carrying bacteria in dairy environments and the need for additional studies to understand the dissemination mechanism within One Health perspective to prevent their further spread. Abstract Background Liver transplantation (LT) helped to save the life of end stage liver disease (ESLD) patients; however, there is a debate on the persistence of cognitive impairment. The study aimed to evaluate cognitive functions in patients with ESLD before and after liver transplantation and to assess its relation to hepatic encephalopathy (HE). Thirty recipients 47.6 ± 11 years undergone living donor liver transplantation at the transplantation center of both Ain Shams Center for Organ Transplant and Egypt air organ transplant unit were prospectively assessed by Trail Making Test, Wechsler Memory Scale–Revised, Benton Visual Retention—for the evaluation of cognitive functions before and 3 months after transplantation. Results The mean age of the patients was 47.6 ± 11 years, 17 males and 13 females. Eight out of 30 (26.7%) had past history of hepatic encephalopathy. The study reported significant improvement in the post-operative 3 months scores of Trail Making Test part (A), the digit span forward test, digit span backward test and the correct score difference of the Benton Visual Retention, as p value was (0.02), (0.01) (0.02), and (0.01) respectively, compared to the pre-operative scores. However, there was no difference in the scores of part (B), verbal association I, II, information subtest of WMS. Cognitive performance showed no significant difference between patients with or without history of HE. Conclusions Patients with ESLD have significant cognitive impairment that showed improvement after LT; HE did not correlate with cognitive function. Hence, transplantation has a favorable outcome on the cognitive impairment.

Sustainable practices in construction industry are still facing numerous challenges despite the continuous efforts and campaigns for sustainable construction worldwide. This study aims to investigate the drivers of sustainable construction in the Kuwaiti construction industry with focus on Compressed Earth Blocks (CEBs) as an alternative sustainable construction material. It also aims to explore the level of awareness and acceptance of sustainable construction material among different stakeholders. The research was organized in three phases. The first phase identified the main drivers from relevant literature and similar studies in the past. In the second phase, a questionnaire-based survey was designed and implemented based on the findings from the literature. Two different questionnaires were designed for the survey: the first one was dedicated for stakeholders with limited knowledge about technical aspects of construction industry (owners, clients, real estate companies) and the second one was dedicated for practitioners and specialists (construction companies, engineers, project managers). In the third phase, statistical analyses were performed to compare the influence of the drivers on the stakeholders’ choices. Friedman test and Bonferroni-Dunn test were used to statistically demonstrate the level of importance of the investigated drivers. The results showed that the most influencing drivers in the choice of sustainable materials from the practitioners’ perspective are “Energy Efficiency”, “Availability and Accessibility to Resources”, “Environmental Impact”, “Enhancement of Health and Safety” and “Knowledge and Skill of Personnel” with no significant difference, followed by the “Client Demands” and the “Cost Savings” then finally the “Awareness on Sustainability”. It was also concluded that the clients’ perspective ranked the “Cost Saving” and the “Enhancement of Health and Safety” as the most important drivers followed by the “Availability and Accessibility to Resources” and the “Environmental Impact”. The survey also showed that despite the poor level of awareness on environmental sustainability, the results related to the level of acceptance of sustainable construction materials are optimistic. This observation demonstrates the readiness/willingness of clients and practitioners to shift towards sustainable construction materials, but the lack of knowledge and awareness can be the most challenging barrier to more sustainable practices. Hence, it is believed that strategies for promoting sustainability in construction industry should start by spreading knowledge among all involved stakeholders.

Conventional steel bars are mostly used as the main load transfer mechanism between jointed slabs in rigid pavements; however, they are generally prone to corrosion which reduces the load transfer efficiency at the joints. This study evaluates the performance of steel bars wrapped with Carbon Fiber Reinforced Polymer (CFRP) sheets, introducing a corrosion-free alternative to conventional steel bars while maintaining the required strength. This paper explains the test setup of large-scale shear strength and load transfer efficiency tests that are currently conducted on the slab samples to evaluate the structural performance of the proposed dowel bars and concrete mix designs.

The extreme arid climatic conditions and poor asphalt mix design characteristics have further accelerated the rate of deterioration of in-service asphalt pavements in Kuwait. Pavement distresses, such as raveling, rutting, and fatigue cracks, worsen in severe climatic and loading conditions, such as high temperatures, elevated humidity levels, and high traffic loads on the pavement surfaces. In this study, a life performance evaluation using mechanistic–empirical analysis of a new modified Superpave mix design was undertaken. The performance life of the modified Superpave asphalt mixture was evaluated, and the results showed that the new modifications to the mix improved the rutting and fatigue cracking resistances of the asphalt mixture for the unconditioned state. However, fatigue cracking resistance under the moisture-conditioned state still needs further improvement for the newly modified Superpave asphalt mixture.

The flood of research output and increasing demands for peer reviewers have necessitated the intervention of artificial intelligence (AI) in scholarly publishing. Although human input is seen as essential for writing publications, the contribution of AI slowly and steadily moves ahead. AI may redefine the role of science communication experts in the future and transform the scholarly publishing industry into a technology-driven one. It can prospectively improve the quality of publishable content and identify errors in published content. In this article, we review various AI and other associated tools currently in use or development for a range of publishing obligations and functions that have brought about or can soon leverage much-demanded advances in scholarly communications. Several AI-assisted tools, with diverse scope and scale, have emerged in the scholarly market. AI algorithms develop summaries of scientific publications and convert them into plain-language texts, press statements, and news stories. Retrieval of accurate and sufficient information is prominent in evidence-based science publications. Semantic tools may empower transparent and proficient data extraction tactics. From detecting simple plagiarism errors to predicting the projected citation impact of an unpublished article, AI’s role in scholarly publishing is expected to be multidimensional. AI, natural language processing, and machine learning in scholarly publishing have arrived for writers, editors, authors, and publishers. They should leverage these technologies to enable the fast and accurate dissemination of scientific information to contribute to the betterment of humankind.

Summary: Background: Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods: A multimethods analysis was performed as part of the GlobalSurg 3 study—a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital. Findings: Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. Rice serves as the primary food source for nearly half of the global population, with Asia accounting for approximately 90% of rice production worldwide. However, rice farming faces significant losses due to pest attacks. To prevent pest infestations, it is crucial to apply appropriate pesticides specific to the type of pest in the field. Traditionally, pest identification and counting have been performed manually using sticky light traps, but this process is time-consuming. In this study, a machine vision system was developed using a dataset of 7328 high-density images (1229 pixels per centimetre) of planthoppers collected in the field using sticky light traps. The dataset included four planthopper classes: brown planthopper (BPH), green leafhopper (GLH), white-backed planthopper (WBPH), and zigzag leafhopper (ZIGZAG). Five deep CNN models—ResNet-50, ResNet-101, ResNet-152, VGG-16, and VGG-19—were applied and tuned to classify the planthopper species. The experimental results indicated that the ResNet-50 model performed the best overall, achieving average values of 97.28% for accuracy, 92.05% for precision, 94.47% for recall, and 93.07% for the F1-score. In conclusion, this study successfully classified planthopper classes with excellent performance by utilising deep CNN architectures on a high-density image dataset. This capability has the potential to serve as a tool for classifying and counting planthopper samples collected using light traps.

Heterozygous de novo mutations in the neuronal protein Munc18-1/STXBP1 cause syndromic neurological symptoms, including severe epilepsy, intellectual disability, developmental delay, ataxia and tremor, summarized as STXBP1 encephalopathies. Although haploinsufficiency is the prevailing disease mechanism, it remains unclear how the reduction in Munc18-1 levels causes synaptic dysfunction in disease as well as how haploinsufficiency alone can account for the significant heterogeneity among patients in terms of the presence, onset and severity of different symptoms. Using biochemical and cell biological readouts on mouse brains, cultured mouse neurons and heterologous cells, we found that the synaptic Munc18-1 interactors Doc2A and Doc2B are unstable in the absence of Munc18-1 and aggregate in the presence of disease-causing Munc18-1 mutants. In haploinsufficiency-mimicking heterozygous knockout neurons, we found a reduction in Doc2A/B levels that is further aggravated by the presence of the disease-causing Munc18-1 mutation G544D as well as an impairment in Doc2A/B synaptic targeting in both genotypes. We also demonstrated that overexpression of Doc2A/B partially rescues synaptic dysfunction in heterozygous knockout neurons but not heterozygous knockout neurons expressing G544D Munc18-1. Our data demonstrate that STXBP1 encephalopathies are not only characterized by the dysfunction of Munc18-1 but also by the dysfunction of the Munc18-1 binding partners Doc2A and Doc2B, and that this dysfunction is exacerbated by the presence of a Munc18-1 missense mutant. These findings may offer a novel explanation for the significant heterogeneity in symptoms observed among STXBP1 encephalopathy patients. [ABSTRACT FROM AUTHOR]
