Several water assessments of some rivers affected by illegal mining (locally known as galamsey) activities in Ghana have recorded high levels of heavy metals and metalloids (such as lead, cadmium, copper, arsenic, mercury, iron and zinc); this poses significant health threats to communities served by affected rivers. To the best of our knowledge, the haematological study by Armah and colleagues is the only one that revealed staggering folds of blood metal concentrations beyond the WHO-required levels in inhabitants of the Tarkwa Nsuaem Municipality and Prestea-Huni Valley District of Ghana. As inhabitants of communities near galamsey sites are constantly exposed to heavy metals since some individuals rely on polluted rivers for domestic purposes, it is necessary to investigate and understand the threat of hypertension and renal damage associated with this unfortunate situation. We share our perspective and a proposed study to elucidate the exact hypertensive and renal damage associated with heavy metal exposure in galamsey-polluted rivers in Ghana. Onsite/human studies should provide important information about blood metal concentrations and current hypertension, renal function and genetic status of individuals exposed to heavy metals in specific galamsey-infested areas. Experimental studies using Sprague-Dawley rat models should determine the proportion of anthropogenic contribution to the levels of heavy metals in the bloodstream. Furthermore, the experimental studies should determine the extent (concentrations and time) and the possible mechanisms/pathways of galamsey-related heavy metals that trigger the pathogenesis and pathophysiology of hypertension and renal damage. The emergence and spread of extended-spectrum β-lactamase (ESBL)-producing Escherichia coli (E. coli) pose significant challenges to the treatment and control of urinary tract infections, particularly among vulnerable populations, such as the elderly living in nursing care homes. In this study, we investigated the occurrence of ESBL genes in commensal E. coli isolated from urine samples of 118 elderly individuals residing in Ghanaian nursing care homes. A total of 195 ESBL genes were detected among 41 E. coli isolated from the study participants. All the isolates harboured at least one ESBL gene, and the majority of them (70.1%) carried at least four ESBL genes. Among the ESBL genes detected, CTXM825 was the predominant (14.1%). In antimicrobial susceptibility testing, 65.9% of the isolates showed resistance to cefepime, a fourth-generation cephalosporin, while 56.1% showed resistance to cefotaxime, a third-generation cephalosporin. Additionally, 46.3% of the isolates were multidrug-resistant, indicating resistance to antibiotics from multiple classes. In summary, we observed relatively high rates of resistance to antibiotics as well as alarming rates of ESBL genes in the isolated pathogens. These findings emphasise the urgent need for antimicrobial stewardship and infection control programmes to mitigate the spread of multidrug-resistant pathogens in nursing care homes. Background. Schistosomiasis is caused by parasitic flatworms and the disease is endemic to most countries in sub-Saharan Africa including Ghana. The current therapeutic agent for managing this disease solely relies on praziquantel. The continual dependence on this single available drug could lead to possible drug resistance. This study seeks to evaluate the antischistosomal activity of the following Ghanaian medicinal plants: Khaya senegalensis, Vernonia amygdalina, Clausena anisata, and Bridelia ferruginea. Methodology. Two concentrations (100 μg/mL and 50 μg/mL) of each extract were tested in a 96-well plate containing 30 newly transformed schistosomula (NTS). Moreover, six worms of both sexes of adult Schistosoma mansoni were exposed to the extracts diluted in the RPMI medium. The assay was performed in a 24-well plate. The parasitic worms were examined using an inverted optical microscope. Results. At 100 μg/mL and 50 μg/mL, all extracts performed better and showed strong activity (p < 0.001) against NTS; thus, 98.08%, 100%, 80.77%, and 100% for Clausena, Vernonia, Bridelia, and Khaya, respectively, when compared to praziquantel. Strong activity was recorded when the extracts underwent testing against Schistosoma mansoni adults at 100 μg/mL; 96.35%, 100%, and 94.55% for Vernonia, Bridelia, and Khaya, respectively, except for Clausena which exhibited weak activity, i.e., 56.02%. There was no significant difference between Vernonia, Bridelia, and Khaya when compared to praziquantel. Conclusion. At 100 μg/mL, Khaya senegalensis, Vernonia amygdalina, and Bridelia ferruginea extracts demonstrated strong activity against both schistosomula and adult Schistosoma mansoni.
