We present the case of a newborn infant who developed pneumothorax. The pneumothorax was managed by providing 100% oxygen (O
2 ) with the "nitrogen washout" concept. Post- hyperoxia treatment, the infant developed signs of pulmonary hypertension, evidenced by an echocardiogram. He was successfully treated with inhaled nitric oxide. The primary purpose of this report is to highlight the point that caution should be exercised in treating stable pneumothorax with 100% O2 .
