Objective: Perform the validation and psychometric evaluation of the Brazilian-Portuguese translation of the Functional Outcome of Sleep Questionnaire 10 (FOSQ10). Materials and methods: 182 patients (65 females 48.3 ± 14.4 years and 117 males 46.9 ± 12.4 years), were evaluated by sleep physicians suspected of having Obstructive Sleep Apnea, underwent polysomnography and completed the FOSQ-10 and the Epworth Sleepiness Scale. APA & NCME, 2014 was used to validate the data as the American Educational Research Association recommended. Results: Quality indicators such as Bartlett's test of sphericity (χ2 = 1108.2; gL = 45; p = 0.000010) and KMO (0.83), and adherence measures, attest to the quality of the model. The indicators TLI (0.97), CFI (0.98), and RMSEA (0.04) fall within the expected values. Using the Eigenvalue > 1 technique, two factors explain 53% and 13.3% of the variances. In the Parallel Analysis technique, a single factor explained 59.4653% of the random variance, and the Unidimensionality indicators UniCo = 0.921, ECV = 0.822, and MIREAL = 0.253, were supported. Construct Validity: reliability coefficients Cronbach’s α = 0.87, McDonald’s ordinal Omega index 0.9, and the Composite Reliability 0.891 were satisfactory. Convergent validity: There was a significant Spearman correlation between FOSQ-10 and the Epworth Sleepiness Scale (r = 0.364 [−0.487; −0.226]). Criterion validity: Was not possible to differentiate the groups based on the severity of AHI using FOSQ-10P. Conclusions: The Brazilian translation of FOSQ-10 is valid and reliable for identifying significant effects of excessive daytime sleepiness in patients with Obstructive Sleep Apnea. Transoral laryngeal procedures are considered minimally invasive but may trigger important complications such as severe bradycardia and even asystole mediated by vagal reflex. The literature on this subject is rare. This article aims to review the literature, explain associated mechanisms, establish prophylactic strategies, and highlight the importance of intraoperative safety protocols.

Introduction: Current Brazilian legislation requires that all workers exposed to noise are to be given an audiogram upon hiring, after 6 months of employment (first periodic test), and annually after the first periodic test. In other countries, the regulations of hearing conservation programs do not include the requirement for audiometric testing at 6 months of employment, but only annually. There is no evidence that the periodicity adopted by Brazilian legislation is the most appropriate. Objective: The present study aimed to evaluate the first 3 occupational audiometric tests of workers exposed to noise. Methods: Historical cohort study with cross-sectional analysis. Participants were all male metallurgy workers aged up to 40 years. The first 3 audiograms of each worker were analyzed: pre-employment audiometric test, periodic audiometric test 1, and periodic audiometric test 2. For each worker, mean frequency thresholds were calculated at 3, 4, and 6 kHz in the left and right ears for each test. Statistical analysis was performed using the nonparametric Wilcoxon test. Results: A total of 988 workers were included. There was a significant difference in auditory thresholds between the pre-employment test and the 2 subsequent periodic tests for the right and left ears. There was no significant difference between Test1 and Test2 in either ear. Conclusion: Given the lack of difference between the first 2 periodic tests, we believe that they could be merged into a single test, i.e., first periodic audiometric testing could be performed at 12 months of employment without compromising workers’ health.

Introdução: O fumo pode interferir negativamente na qualidade vocal e outros fatores associados, como o uso profissional da voz, hábitos, etilismo e refluxo gástrico-esofágico, podem potencializar essa interferência. O objetivo do estudo foi analisar a associação do tabagismo e dos demais fatores aos parâmetros acústicos vocais. Forma de estudo: Estudo de coorte contemporâneo com corte transversal. Materiais e métodos: Participaram do estudo 80 adultos, entre 35 e 60 anos, classificados nos grupos fumante (GF) e controle (GC). Foi realizada aplicação de questionário e gravação das vozes. A avaliação acústica foi realizada com o software Praat. Foram utilizados os testes Mann-Whitney, qui-quadrado e regressão linear simples. Resultados: O GF apresentou maior prevalência de etilismo, tosse, hábito de pigarrear e uso profissional da voz, assim como apresentou valores aumentados para relação ruído-harmônico (NHR), jitter e shimmer. Foi estabelecida correlação entre o gênero feminino e todos os parâmetros acústicos, assim como entre o RGE, o aumento do jitter e o hábito de fumar, o agravamento da frequência fundamental, o aumento do jitter, o shimmer e a NHR. Conclusão: O fumo interfere nos parâmetros acústicos de modo isolado e/ou associado ao etilismo, tosse, hábito de pigarrear, RGE e uso profissional da voz.

A remoção dos níveis II, III e IV assumiram importância no tratamento do pescoço no carcinoma espinocelular (CEC) da laringe. Este estudo propõe a remoção dos níveis II e III, devido à baixa probabilidade de metástases linfáticas para nível IV. OBJETIVO: Estudar a prevalência de metástases linfáticas no nível IV em CEC da laringe. MÉTODO:Estudo prospectivo de pacientes consecutivos com CEC de laringe submetidos a esvaziamentos cervicais. Níveis cervicais foram identificados e marcados para posterior avaliação histopatológica. RESULTADOS: 6% (3/54) dos espécimes cirúrgicos apresentaram metástases linfáticas para nível IV. Todos os espécimes cirúrgicos cN0 (42) foram isentos de metástases para nível IV. Nos espécimes de pescoços cN (+) (12), 25% foram histopatologicamente positivos no nível IV. Esta diferença entre pescoços cN0 e cN (+) foi estatisticamente significativa (p = 0,009). Não houve metástases isoladas para o nível IV, sempre relacionado a nível II ou III comprometido (p = 0,002). CONCLUSÃO: A prevalência de metástases linfáticas para o nível IV em CEC da laringe no pescoço cN0 foi 0%. Metástase linfática no nível IV foi correlacionada com pescoço de cN (+). Metástases para nível IV foram relacionadas com metástases para o nível II ou III.The removal of level II, III, and IV metastases has gained importance in the treatment of squamous cell carcinomas (SCC) of the neck and larynx. This study assessed the possibility of removing level II and level III metastases only, given the low likelihood of occurrence of metastatic lymph nodes on level IV in SCCs of the larynx. OBJECTIVE: This study aimed to analyze the prevalence rates of metastatic lymph nodes on level IV in laryngeal SCC patients. METHODS: This prospective study enrolled consecutive patients with laryngeal SCC submitted to neck lymph node dissection. Neck levels were identified and marked for future histopathology testing. RESULTS: Six percent (3/54) of the necks had level IV metastatic lymph nodes. All cN0 necks (42) were free from level IV metastasis. Histopathology testing done in the cN (+) necks (12) revealed that 25% of the level IV specimens were positive for SCC. The difference between cN0 and cN (+) necks was statistically significant (p = 0.009). Level IV metastases never occurred in isolation, and were always associated with level II or level III involvement (p = 0.002). CONCLUSION: The prevalence rate for lymph node metastasis in cN0 necks was 0%. Level IV metastatic lymph nodes were correlated to cN (+) necks. Level IV metastasis was associated with the presence of metastatic lymph nodes in levels II or III.

OBJECTIVE: To assess the location of hard gelatin capsules in the pharyngeal phase triggering among asymptomatic adults. INTRODUCTION: The location of the bolus during the pharyngeal phase triggering provides information about the sensorimotor model of the beginning of deglutition onset. PURPOSE: To evaluate the location of hard gelatin capsules in the pharyngeal phase triggering among asymptomatic adults. METHODS: A videofluoroscopy swallowing study was carried out in 60 subjects (14 male and 46 female participants) aged between 27 and 55 years, who were evaluated with hard gelatin capsules #00 and #3 containing barium sulfate, swallowed with liquid food and pudding, in free volume. The first laryngeal elevation movement was the criterion to locate the pharyngeal phase triggering. Statistical analysis was based on the McNemar test. RESULTS: Capsule #3 presented higher percentage of location in the tongue dorsum compared to capsule #00, and capsule #00 presented higher percentage of location in the tongue base and vallecula compared to capsule #3. There was a difference between different capsules swallowed with liquid (p=0.016) and pudding (p=0.037). CONCLUSION: The capsule size influenced the location of the pharyngeal phase triggering. Smaller capsules started pharyngeal phase in the most anterior region (tongue dorsum) compared to larger capsules.

CONTEXT: Deep neck infections have high potential for severe complications and even death, if not properly managed. The difference between clinical and computed tomography findings may demonstrate that clinical evaluation alone underestimates disease extent, which may lead to conservative treatment with worse prognosis. OBJECTIVE: To compare clinical and computed tomography findings from neck spaces affected by deep neck infections and to determine the main clinical and radiological features associated with these. TYPE OF STUDY: Non-randomized retrospective study. SETTING: Department of Otolaryngology and Head and Neck, Universidade Estadual de Campinas. METHODS: Medical charts of 65 patients with deep neck infections were evaluated. Age, gender, clinical complaints, physical findings, computed tomography scan and x-ray imaging, microbiology, treatment and outcome were analyzed. All clinical signs and symptoms were evaluated and stratified in order of frequency. The frequency of neck space involvement in such infections was also assessed from the clinical and tomographic evaluation. All clinical and computed tomography findings were compared with surgical observation. RESULTS: The most frequent clinical findings were neck swelling, local pain, erythema and locally increased temperature. Physical evaluation showed that the most affected site was the submandibular triangle (49.2% of cases). However, computed tomography showed this to be the lateropharyngeal space (65% of cases) and that more than one deep cervical space was compromised in 90% of cases, as demonstrated by the extent of swelling and increased contrast signs in soft tissue. DISCUSSION: The most frequent clinical symptoms of deep cervical infections were cervical pain, increased cervical volume and fever. The important signs seen via computed tomography were increased contrast in soft neck tissues and swelling. Such examination is the most important method for correct evaluation of cervical spaces involved in infection, and thus for correct surgical drainage. CONCLUSIONS: The most frequent clinical findings were cervical mass, neck pain, local erythema and locally increased temperature. Computed tomography demonstrated that the lateropharyngeal space was the most affected neck space. More than one deep neck space was compromised in 90% of cases. Clinical evaluation underestimated the extent of deep neck infection in 70% of patients.

Abstract Bezold's abscess is a rare complication of acute otitis media, but it should be recognized and aggressively treated. Otolaryngologists must be aware of this diagnosis, and multidisciplinary care should be provided as soon as possible.

Introduction: Squamous cell carcinoma is the most common neoplasm of the larynx, and its evolution depends on tumor staging. Vascular endothelial growth factor is a marker of angiogenesis, and its expression may be related to increased tumor aggressiveness, as evidenced by the presence of cervical lymphatic metastases. Objectives: To evaluate the expression of the vascular endothelial growth factor marker in non-glottic advanced squamous cell carcinoma of the larynx (T3/T4) and correlate it with the presence of cervical lymph node metastases. Methods: Retrospective clinical study and immunohistochemical analysis of vascular endothelial growth factor through the German scale of immunoreactivity in products of non-glottic squamous cell carcinomas. Results: This study analyzed 15 cases of advanced non-glottic laryngeal tumors (T3/T4), four of which exhibited cervical lymphatic metastases. There was no correlation between vascular endothelial growth factor expression and the presence of cervical metastases. Conclusion: Although vascular endothelial growth factor was expressed in a few cases, there was no correlation with the spread of cervical lymph metastases.

INTRODUCTION: Minor structural alterations of the vocal fold cover are frequent causes of voice abnormalities. They may be difficult to diagnose, and are expressed in different manners. Cases of intracordal cysts, sulcus vocalis, mucosal bridge, and laryngeal micro-diaphragm form the group of minor structural alterations of the vocal fold cover investigated in the present study. The etiopathogenesis and epidemiology of these alterations are poorly known. OBJECTIVE: To evaluate the existence and anatomical characterization of minor structural alterations in the vocal folds of newborns. METHODS: 56 larynxes excised from neonates of both genders were studied. They were examined fresh, or defrosted after conservation via freezing, under a microscope at magnifications of 25× and 40×. The vocal folds were inspected and palpated by two examiners, with the aim of finding minor structural alterations similar to those described classically, and other undetermined minor structural alterations. Larynges presenting abnormalities were submitted to histological examination. RESULTS: Six cases of abnormalities were found in different larynges: one (1.79%) compatible with a sulcus vocalis and five (8.93%) compatible with a laryngeal micro-diaphragm. No cases of cysts or mucosal bridges were found. The observed abnormalities had characteristics similar to those described in other age groups. CONCLUSION: Abnormalities similar to sulcus vocalis or micro-diaphragm may be present at birth.

INTRODUCTION: Squamous cell carcinoma is the most common neoplasm of the larynx and glottis, and its prognosis depends on the size of the lesion, level of local invasion, cervical lymphatic spread, and presence of distant metastases. Ki-67 (MKI67) is a protein present in the core, whose function is related to cell proliferation. AIM: To evaluate the expression of marker Ki-67 in squamous cell carcinoma of the larynx and glottis and its correlation to pathological findings. METHODS: Experimental study with immunohistochemistry analysis of Ki-67, calculating the percentage of the cell proliferation index in glottic squamous cell carcinomas. RESULTS: Sixteen cases were analyzed, with six well-differentiated and 10 poorly/moderately differentiated tumors. There was a correlation between cell proliferation index and degree of cell differentiation, with higher proliferation in poorly/moderately differentiated tumors. CONCLUSION: The cell proliferation index, as measured by Ki-67, may be useful in the characterization of histological degree in glottic squamous cell tumors.

INTRODUCTION: Obstructive sleep apnea syndrome has a high prevalence among adults. Cephalometric variables can be a valuable method for evaluating patients with this syndrome. OBJECTIVE: To correlate cephalometric data with the apnea-hypopnea sleep index. METHODS: We performed a retrospective and cross-sectional study that analyzed the cephalometric data of patients followed in the Sleep Disorders Outpatient Clinic of the Discipline of Otorhinolaryngology of a university hospital, from June 2007 to May 2012. RESULTS: Ninety-six patients were included, 45 men, and 51 women, with a mean age of 50.3 years. A total of 11 patients had snoring, 20 had mild apnea, 26 had moderate apnea, and 39 had severe apnea. The distance from the hyoid bone to the mandibular plane was the only variable that showed a statistically significant correlation with the apnea-hypopnea index. CONCLUSION: Cephalometric variables are useful tools for the understanding of obstructive sleep apnea syndrome. The distance from the hyoid bone to the mandibular plane showed a statistically significant correlation with the apnea-hypopnea index.

A residência médica é ensino de pós-graduação caracterizado como treinamento em serviço. OBJETIVO: Apresentar o protocolo de avaliação da ABORL-CCF dos cursos de residência ou especialização. MÉTODO: Desenvolvimento de um protocolo de avaliação; Classificação por qualidade; Comparação do desempenho dos alunos egressos no concurso para obtenção de título de especialista, segundo a classificação de seus programas de origem. RESULTADOS: Foram avaliados 82 programas de ensino em 2004 classificados como A+ (12,20%), B+ (7,3%), B (19,5%), C+ (20,7%), C (17,1%), D (20,7%) e E (2,4%). Houve discrepância em qualidade e distribuição geográfica. A classificação dos candidatos para obtenção do título de especialista foi melhor para os egressos dos programas mais qualificados. A curva das notas das instituições tem a mesma tendência decrescente que a obtida com as médias dos seus alunos. Dos programas inicialmente classificados como E, D, C ou C+, 77% melhoraram de classificação quando reavaliados após três anos. CONCLUSÃO: O protocolo de avaliação foi capaz de discriminar por qualidade os programas de ensino. As notas dos aprovados no concurso para título de especialista têm a mesma tendência da classificação de suas instituições. Houve melhora na classificação dos programas de ensino após a implantação do protocolo.

Arelação anatômica entre seio maxilar e ducto lacrimonasal adquiriu maior importância com o advento das microcirugias e cirurgias nasossinusais assistidas por endoscopia e pelo crescente uso da endoscopia nasal na realização das meatotomias médias e dacriocistorrinostomiastransnasais. Não foram encontrados relatos de classificação do seio maxilar quanto ao seu recesso lacrimal, tampouco sua frequência. OBJETIVO: Avaliar a frequência do recesso lacrimal do seio maxilar em peças anatômicas dissecadas. MÉTODO: Foram avaliadas 31 partes de hemicabeças de cadáveres. Procedeu-se à dissecção da área correspondente ao terço médio da face, por acesso lateral, para que se pudesse observar a posição da porção mais lateral do ducto lacrimonasal em relação ao seio maxilar. Os seios maxilares foram avaliados, por dois examinadores simultaneamente, chegando ao consenso do tipo de ducto lacrimonasal. RESULTADOS: Foram encontrados 18 seios maxilares do tipo lateral (58,1%) e 13 do tipo anterior (41,9%). A diferença entre a frequência de seios maxilares do tipo anterior nos lados direito (35,7%) e esquerdo (47,1%) não mostrou significância estatística (p = 0,524). CONCLUSÃO: Observou-se frequência de 41,9% de recessos lacrimais nos seios maxilares.The anatomical relation between the maxillary sinus and the nasolacrimal duct has gained greater importance with the advent of microsurgeries and endoscopic-assisted sinonasal procedures, and the growing use of endonasal surgery to perform middle meatus procedures and transnasal dacryocystorhinostomy. We did not find reports on maxillary sinus classification concerning its lacrimal recess, nor how often it is found. OBJECTIVE: To assess how frequent the lacrimal recess can be found in the maxillary sinuses of dissected anatomical specimens. METHOD: We assessed 31 half-heads from cadavers. We dissected the area corresponding to the middle third of the face, by lateral access so as to be able to observe the most lateral portion of the nasolacrimal duct vis-à-vis the maxillary sinus.The maxillary sinuses were assessed by two examiners simultaneously, getting to a consensus in relation to the type of nasolacrimal duct. RESULTS: We assessed 18 maxillary sinuses of the lateral type (58.1%) and 13 anterior sinuses (41.9%). The difference in frequency of the anterior type of maxillary type of the right side (35.7%) and left (47.1%) did not have statistical significance (p = 0.524). CONCLUSION: We found a frequency of 41.9% of lacrimal recesses in the maxillary sinuses.

Vocal alterations after thyroidectomy are generally related to laryngeal nerve injury or laryngotracheal mobility disorders caused by postoperative fibrosis or strap muscle lesion. AIM: this study aims to evaluate the frequency of vocal and rima glottidis disorders after thyroidectomy. MATERIALS AND METHOD: This is a prospective study based on 35 patients submitted to thyroidectomy under local anesthesia and hypnosedation. All patients underwent voice auditory perception evaluation, voice acoustic tests and videolaryngostroboscopy preoperatively, and at one week and at 30 days postoperatively. Bilateral cricothyroid muscle electromyography was performed on the thirtieth day after surgery to confirm the presence of injury in the external branch of the superior laryngeal nerve. RESULTS: 14.3% of the patients presented posterior glottis deviation before surgery and normal electromyography findings. Transient and permanent vocal alteration occurred in 25.7% and 14.2% of the patients respectively. CONCLUSION: voice disorders evaluated after voice auditory perceptive evaluation and voice acoustic tests were more intense in the group with superior laryngeal nerve external branch injury than in the injury-free dysphonic patient group. Oblique glottis can be present in normal patients; however its onset after thyroidectomy is indicative of superior laryngeal nerve external branch lesion.Alterações vocais após tireoidectomia estão geralmente relacionadas a lesão de nervos laríngeos ou alteração na mobilidade laringotraqueal causada pela fibrose pós-operatória ou lesão da musculatura pré tireoidiana. OBJETIVO: Avaliar a frequência de alterações vocais e da rima glótica após tireoidectomia. MATERIAL E MÉTODO: Estudo prospectivo baseado em 35 pacientes submetidos à tireoidectomia com anestesia local e hipnossedação.Todos os pacientes foram submetidos à avaliação perceptiva auditiva da voz, avaliação acústica da voz e videolaringoestroboscopia nos períodos pré-operatório e pós-operatório uma semana e 30 dias após a cirurgia. Eletromiografia do músculo cricotireoideo bilateralmente foi realizada no trigésimo dia do período pós-operatório para confirmação de lesão do ramo externo do nervo laríngeo superior. RESULTADOS: 14,3% dos pacientes apresentaram desvio posterior da glote no período pré-operatório com eletromiografia normal. Alteração vocal temporária e permanente ocorreu em 25,7% e 14,2% dos pacientes, respectivamente. CONCLUSÃO: Alteração vocal avaliada após análise perceptiva auditiva e acústica da voz foi mais intensa no grupo com lesão do ramo externo do nervo laríngeo superior do que no grupo de pacientes disfônicos sem lesão. Glote oblíqua pode estar presente em pacientes normais, porém o seu aparecimento após tireoidectomia é indicativo de lesão do ramo externo do nervo laríngeo superior.

CONTEXT: Ewing's sarcoma is a rare neoplasm, which usually arises in long bones of the limbs and in flat bones of the pelvis, with the involvement of head and neck bones being very unusual. CASE REPORT: a case of Ewing's sarcoma occurring in the mandible of a 35-year-old female. Pain and swelling of the tumor were the main complaints. The early hypothesis was an undifferentiated malignant neoplasm, possibly a sarcoma. The CT scan depicted an expansive lesion, encapsulated, with septa and characteristics of soft tissue, involving the left side of the mandible and extending to the surrounding tissues. The patient underwent surgical excision of the lesion, the definitive diagnosis of Ewing's sarcoma was established, and the patient commenced on radiotherapy.

Abstract Introduction: Although the incidence of Deep Cervical Abscess (DCA) has decreased mainly for the availability of antibiotics, this infection still occurs with considerable frequency and can be associated with high morbidity and mortality. Objective: This study aimed to present our clinical-surgical experience with deep neck abscesses. Methods: A retrospective study analyzed 101 patients diagnosed with deep neck abscesses caused by multiple etiologies, assisted at a medical school hospital during 6 years. One hundred one patients were included and 27 (26.7%) were younger than 18 years old (the children group), 74 patients (73.3%) were older than 18 years old (the adults group). The following clinical features were analyzed and compared: age, gender, clinical symptoms, leukocyte count, the affected cervical area, lifestyle habits, antibiotic therapy, comorbidities, etiology, bacterial culture, time of hospitalization, the need of tracheostomy and complications. Results: There was predominance in the male gender (55.5%) and young people (mean age 28.1 years). All of the 51 patients with associated disease comorbidity were adults. The most frequent etiologies were bacterial tonsillitis (31.68%) and odontogenic infections (23.7%). The most common cervical areas affected were the peritonsillar (26.7%), submandibular/mouth floor (22.7%) and parapharyngeal spaces (18.8%). In children group, the site most commonly involved was the peritonsillar space (10 patients, 37%). In adults group, the site most commonly involved was multispace (31 patients, 41.8%). Streptococcus pyogenes (23.3%) was the most common microorganism present. Amoxicillin associated with clavulanate (82.1%) was the more used antibiotic. The main complications of abscesses were septic shock (16.8%), pneumonia (10.8%) and mediastinitis (1.98%). Tracheostomy was necessary in 16.8% of patients. The mortality rate was 1.98%. Conclusion: The clinical features and severity of DCA varied according to different age groups, perhaps due to the location of the infection and a higher incidence of comorbidity in adults. Thus, DCA in adults is more facile to have multispace involvement and lead to complications and seems to be more serious than that in children.

OBJETIVO: O trauma da laringe é pouco freqüente. O objetivo do presente trabalho é avaliar os procedimentos e resultados no tratamento destas lesões. MÉTODO: Este trabalho baseou-se em estudo prospectivo de 35 de pacientes com trauma de laringe atendidos no período de janeiro de 1990 a abril de 2003. RESULTADOS: A média de idade foi de 31,4 anos, sendo 30 pacientes (85,7%) do sexo masculino. O mecanismo predominante foi o trauma penetrante (30 casos - 85,7%), a maioria causada por ferimento por projétil de arma de fogo (17 casos - 48,6%). Dez pacientes (28,6%) necessitaram de intubação traqueal na admissão hospitalar e o valor médio do RTS foi de 7,28. As observações mais freqüentes no exame clínico das vítimas de trauma penetrante foram exposição de cartilagens da laringe (30%) e saída de ar pelo orifício do ferimento cervical (30%). Nos cinco pacientes (14,3%) com trauma contuso o achado mais freqüente foi enfisema subcutâneo (80%).O tratamento foi cirúrgico em 34 pacientes (97,1%), através de cervicotomia em colar na maioria dos casos (91,2%). A cartilagem tireóide foi a mais lesada (20 casos - 57,1%). Em 33 pacientes operados a lesão foi tratada com sutura, associada a traqueostomia em 24 casos (72,7%). Lesões cervicais associadas ocorreram em 20 casos (57,1%), sendo mais comum as de veia jugular (10 casos). A média do ISS e do TRISS foram, respectivamente, 16,3 e 0,93. A morbidade relacionada diretamente à lesão laríngea foi de 34,3% (12 casos), sendo mais freqüente a disfonia (seis casos). Foi necessária a reexploração cervical em dois pacientes, um devido a abscesso cervical e outro, tardiamente, por estenose supra-glótica, este último tratado com molde. Dois pacientes apresentaram complicações tardias, um com disfagia e outro com disfonia. A mortalidade pós-operatória foi de 5,7% (dois casos), decorrente de complicações não relacionadas ao trauma laríngeo. CONCLUSÕES: A utilização de condutas padronizadas na abordagem do paciente com trauma de laringe, tanto no diagnóstico como no tratamento definitivo, resulta em menor taxa de seqüelas definitivas.

CONTEXT AND OBJECTIVE: Although there is a close relationship between swallowing and breathing, there are no studies evaluating the radiographic anatomy of the airway and its possible correlation with the radiographic position of the hyoid bone. The aim of this study was to evaluate the possible correlation of the radiographic position of the hyoid bone and airway space (PAS) in lateral radiographs on children with atypical deglutition, in comparison with those with normal swallowing. DESIGN AND SETTING: Cross-sectional analytical study with control group in a public university. METHODS: Using cephalometric analysis on lateral teleradiographs, the distance from the hyoid bone to the mandibular plane (MP-H) and the distance from the hyoid bone to the tuber (T-H) were correlated with the PAS measurement (airway) in two groups: 55 teleradiographs in the experimental group (with atypical deglutition) and 55 teleradiographs in the control group (normal deglutition). Both groups included subjects at the mixed dentition stage. RESULTS: The variable T-H presented a statistically significant correlation with PAS (0.0286) and the variable MP-H had a significant correlation with the variable PAS (0.0053). This positive correlation was significant only in the control group and not in the group with atypical swallowing. CONCLUSIONS: There was a positive correlation between the MP-H and PAS measurements and between the T-H and PAS measurements only in the group with normal swallowing. These correlations were not observed in the group with atypical swallowing.

CONTEXT: According to the literature, laryngeal papillary carcinoma is rare and has a benign prognosis. CASE REPORT: In this report we present a surprising case with nodal metastasis at the time of diagnosis. Computed tomography showed infiltration of the lesion and metastatic lymph nodes. The resected specimen was submitted to histopathological study that confirmed the diagnosis of papillary squamous cell carcinoma.

RESUMO Objetivo Traduzir para o português brasileiro e adaptar culturalmente a definição dos componentes e pontuação (Component Scores Definition) pertencentes ao protocolo para videofluoroscopia Modified Barium Swallow Impairment – MBSImPTM. Método Esse estudo foi desenvolvido com base nas recomendações internacionais para elaboração, tradução e adaptação transcultural em consonância com publicações nacionais para validação de testes internacionais em Fonoaudiologia. Foi formado um comitê de especialistas, composto por dois médicos otorrinolaringologistas e três fonoaudiólogas conhecedoras do exame de videofluoroscopia. A tradução para o português brasileiro foi realizada por dois otorrinolaringologistas com retrotradução de forma independente por dois nativos norte-americanos. A versão final em português brasileiro foi elaborada pelas fonoaudiólogas certificadas após a revisão das traduções e das retrotraduções com resolução de discrepâncias semânticas, idiomáticas, conceituais, linguísticas e contextuais. Essa versão foi testada pelas fonoaudiólogas membros do Comitê, que avaliaram indivíduos saudáveis, com Câncer de Cabeça e Pescoço e Comprometimento Cognitivo Leve. Resultados A tradução da definição dos componentes e pontuação do MBSImP foi realizada de forma independente e os tradutores chegaram a um consenso para a versão final. O protocolo original Modified Barium Swallow Impairment, foi traduzido para “Escala Martin-Harris para videofluoroscopia da deglutição”. Na versão final em português brasileiro alguns termos foram adaptados. A aplicação do protocolo traduzido não apresentou discrepâncias em relação ao protocolo original. Conclusão A tradução do excerto do protocolo MBSImP para o português brasileiro foi compatível com a versão original. A certificação de membros do comitê favoreceu a adaptação do material, permitindo a adaptação conceitual.
