The aim of this work was to validate the measurements of three physiological parameters, namely, body temperature, heart rate, and peripheral oxygen saturation, captured with an out-of-the-lab device using measurements taken with clinically proven devices. The out-of-the-lab specialized device was integrated into a customized mHealth application, e-CoVig, developed within the AIM Health project. To perform the analysis, single consecutive measurements of the three vital parameters obtained with e-CoVig and with the standard devices from patients in an intensive care unit were collected, preprocessed, and then analyzed through classical agreement analysis, where we used Lin's concordance coefficient to assess the agreement correlation and Bland–Altman plots with exact confidence intervals for the limits of agreement to analyze the paired data readings. The existence of possible systematic errors was also addressed, where we found the presence of additive errors, which were corrected, and weak proportional biases. We obtained the mean overall agreement between the measurements taken with the novel e-CoVig device and the reference devices for the measured quantities. Abstract Background Significant hemodynamic changes occur during liver transplantation, emphasizing the importance of precious and continuous monitoring of cardiac output, cardiac index, and other parameters. Although the monitoring of cardiac output by pulse indicator continuous cardiac output (PiCCO) was statistically homogeneous compared to the clinical gold standard pulmonary artery catheterization (PAC) in previous studies of liver transplantation, there are fewer statistical methods for the assessment of its conclusions, and a lack of comparisons of other hemodynamic parameters (e.g., SVRI, systemic vascular resistance index). Some studies have also concluded that the agreement between PiCCO and PAC is not good enough. Overall, there are no uniform conclusions regarding the agreement between PiCCO and PAC in previous studies. This study evaluates the agreement and trending ability of relevant hemodynamic parameters obtained with PiCCO compared to the clinical gold standard PAC from multiple perspectives, employing various statistical methods. Methods Fifty-two liver transplantation patients were included. While Passing-Bablok plots showed favorable agreement for SVRI overall and during various phases, the agreement for other parameters was less satisfactory. The ICC results confirmed good overall agreement between the two devices across most parameters, except for SVRI during the new liver phase, which showed poor agreement. Additionally, four-quadrant and polar plot analyses indicated that all agreement rate values fell below the clinically acceptable threshold of over 90%, and all angular deviation values exceeded ± 5°, demonstrating that PiCCO is unable to meet the acceptable trends. Using the TIM, the interchangeability rates were found to be quite low: 20% for CO and CI, 16% for SVRI, and 13% for SVI. Conclusions Our study revealed notable disparities in absolute values of CO, CI, SVRI and SVI between PiCCO and PAC in intraoperative liver transplant settings, notably during the neohepatic phase where errors were particularly pronounced. Consequently, these findings highlight the need for careful consideration of PiCCO’s advantages and disadvantages in liver transplantation scenarios, including its multiple parameters (such as the encompassing extravascular lung water index), against its limited correlation with PAC.

Purpose: Evaluating the implementation of environmental management plans (EMPs) in highway construction projects is essential to avoid climate change. Public evaluations can help ensure that the EMP is implemented correctly and efficiently. To allow public evaluation of EMP implementations, this study aims to investigate performance indicators (PIs) for assessing EMP implementation in highway construction projects. To that end, the study objectives are to compare the critical PIs between environment auditors (EAs) and environment officers (EOs) and among the main project stakeholders (i.e. clients, contractors and consultants), create components for the critical PIs and assess the efficiency of the components. Design/methodology/approach: The paper identified 39 PIs from interviews with environmental professionals and a systematic literature review. Then a questionnaire survey was developed based on the PIs and sent to EAs and EOs. The data were analyzed via mean score ranking, normalization, agreement analysis, factor analysis and fuzzy synthetic evaluation (FSE). Findings: The analyses revealed 21 critical PIs for assessing EMP implementation in highway construction projects. Also, the critical PIs can be grouped into four components: ecological, pollution, public safety and ecological. Finally, the overall importance of the critical PIs from the FSE is between important and very important. Originality/value: To the best of the authors' knowledge, this paper is the first-of-its-kind study on the critical PIs for assessing EMP implementation in highway construction projects. Capillary refill time (CRT) is an internationally accepted indicator of peripheral circulation. The CRT is measured by applying compression to the fingernail for a few seconds, releasing it, and observing the process of refilling of blood at the fingertip. The international guidelines for the management of sepsis and septic shock 2021 contains an additional new recommendation for CRT measurement to determine the peripheral circulation statuses of adult patients with septic shock. However, the current CRT measurement method lacks objectivity. Previous studies have reported the development of measurement devices and video analysis systems, which involve complex measurement environment construction and assume only in-hospital measurements. In addition, since medical workers are limited in the number of carrying medical devices, devices that can be used to obtain multiple biometric indicators in a single measurement are needed. Hence, a prototype wearable CRT measurement device was developed in this study, and its feasibility was evaluated by comparing the agreement, intra-class correlation coefficient, and coefficient of variation with those of a CRT measurement device developed in the past. The results indicated that mean CRTs between measurement methods were agreement, with moderate or better intra-rater reliability and no difference in coefficient of variation. Balance control can be quantified with measurements from force plates or wearable accelerometers. The purposes of our study were (1) to assess the explanatory power of accelerometer measures to predict center-ofpressure (CoP) sway-area rate across four standing balance tests, (2) to provide expanded Romberg quotient (RQ) reference ranges for older adults, and (3) to perform an agreement analysis between RQs derived from accelerometer measures with those from CoP sway-area rate. Twenty-six healthy older adults completed four 30-s balance tests standing on rigid and foam surfaces with eyes opened and closed on two occasions. Tests were performed on a force plate (1 variable) while wearing accelerometers (5 variables). The outcomes were assessed with linear mixed-effects models, RQs, and agreement analyses. CoP sway-area rate could be significantly predicted by different combinations of accelerometer variables for each balance condition (R²≥0.45). Despite some accelerometer RQs not being significantly different (p≥0.12) from corresponding CoP sway-area rate RQs, there was low agreement between these RQs as determined by intraclass correlation coefficients, Bland-Altman plots, and mountain plots. Clinicians with access to wearable accelerometers but not force plates can use our models to estimate CoP sway-area rate for each standardized test of standing balance. Development of patient-reported outcomes (PROs) to include traditionally clinic-reported data has the potential to decrease the data-collection burden for patients and clinicians and increase follow-up rates. However, replacing clinic report by patient report requires that the data reasonably agree. To assess agreement between PROs and clinical registry data at 1 year after bariatric surgery. Not-for-profit organization, bariatric surgery data registry, PROs platform. Patient- and clinic-reported 1-year postoperative weight and co-morbidities were compared for matched PROs and registry records. The co-morbidities evaluated were diabetes, sleep apnea, hypertension, gastroesophageal reflux disease, and hyperlipidemia. Weight difference in pounds and nominal groupings (binary, 4-level) for co-morbidities were assessed for agreement between data sources using descriptive statistics, Bland–Altman plots, multiple regression, and kappa coefficients. Sensitivity analyses and follow-up by response method were examined. Among 1130 patients with both 1-year PROs and registry weights, 95% of patient-reported weights were within 13 lb of the registry-recorded weight, and patients underreported their weight by ∼2 lb, on average. Percent agreement and kappa coefficients were highest for diabetes (n = 999; binary: 94%, κ =.72; 4-level: 86%, κ =.71) and lowest for gastroesophageal reflux disease (n = 1032; binary: 75%, κ =.40; 4-level: 57%, κ =.35). Of patients eligible for both PROs and registry 1-year follow-up, 21% had PROs only. One-year patient- and clinic-reported weights and disease status for patients with diabetes and hypertension showed high agreement. The degree of bias from patient report was low. Background and Purpose: The performance of a novel prototype four‐dimensional (4D) digital subtraction angiography (DSA) for cerebral arteriovenous malformation (AVM) diagnosis was evaluated and compared with that of two‐dimensional (2D) and three‐dimensional (3D) DSA. Methods: In this retrospective study, 37 consecutive cerebral AVM patients were included. The standard diagnostic results were concluded from the 2D and 3D DSA. Two 4D DSA volumes were reconstructed for each patient by a commercial and a prototype software, then evaluated by two independent experienced neurosurgeons, who were blinded to the diagnosis and treatment process. The evaluation results were compared with the diagnostic results on Spetzler‐Martin (SM) Grading Scale, number of feeding arteries, number of draining veins, and intranidal aneurysms. Results: Complete agreement was achieved between 4D DSA and 2D and 3D DSA in SM Grading Scale and intracranial aneurysm identification (agreement coefficient: 1) for both reviewers. The agreement coefficients were.888 and.917 for both reviewers in feeding artery number determination using 4D DSA product and 4D DSA prototype, respectively. The agreement coefficients in draining vein number determination were all larger than.94 for both reviewers using both 4D DSA volumes. Conclusions: The performance of this prototype 4D DSA in cerebral AVMs diagnosis was largely equivalent to that of 2D and 3D DSA combination. Magnetic resonance imaging (MRI) is routinely used for preoperative tumor staging and to assess response to therapy in rectal cancer patients. The aim of our study was to evaluate the accuracy of MRI based restaging after neoadjuvant chemoradiotherapy (CRT) in predicting pathologic response. This multicenter cohort study included adult patients with histologically confirmed locally advanced rectal adenocarcinoma treated with neoadjuvant CRT followed by curative intent elective surgery between January 2014 and December 2019 at four academic high‐volume institutions. Magnetic resonance tumor regression grade (mrTRG) and pathologic tumor regression grade (pTRG) were reviewed and compared for all the patients. The agreement between radiologist and pathologist was assessed with the weighted k test. Risk factors for poor agreement were investigated using logistic regression. A total of 309 patients were included. Modest agreement was found between mrTRG and pTRG when regression was classified according to standard five‐tier systems (k = 0.386). When only two categories were considered for each regression system, (pTRG 0‐3 vs pTRG 4; mrTRG 2‐5 vs mrTRG 1) an accuracy of 78% (95% confidence interval [CI] 0.73‐0.83) was found between radiologic and pathologic assessment with a k value of 0.185. The logistic regression model revealed that "T3 greater than 5 mm extent" was the only variable significantly impacting on disagreement (OR 0.33, 95% CI 0.15‐0.68, P =.0034). Modest agreement exists between mrTRG and pTRG. The chances of appropriate assessment of the regression grade after neoadjuvant CRT appear to be higher in case of a T3 tumor with at least 5 mm extension in the mesorectal fat at the pretreatment MRI. Purpose: To investigate the agreement in sleep pattern recording by self-reported sleep questionnaires and actigraphy in adults. Methods: This is a cross-sectional study. Men and women who met inclusion criteria were recruited for this study. The inclusion criteria were apparently healthy Omani nationals ages 19 to 50 years. Sleep questionnaires were randomly distributed in Muscat either directly or via electronic and paper announcements. Data were collected from the participants using the self-reported questionnaires with four piloted questions for sleep pattern identification and through the actigraphy wristband given to subjects to wear for a week. Cohen's kappa test was performed for agreement analysis. Results: A total of 964 Omani subjects between ages 18 and 59 years of both genders were recruited and completed the questionnaires successfully. Out of these, only 321 subjects wore the actigraphy wristband for 1 week (response rate = 33%). Agreement analysis reported a mild level of agreement for the monophasic (41%), moderate level for biphasic (59%), and good level for polyphasic (70%) sleep patterns. The overall agreement level of sleep patterns between the two methods was 57%. There is a low specificity of self-reported assessment in reporting sleep pattern. Conclusion: The average agreement level of subjective versus objective assessments of sleep patterns was moderate at 57% and self-reported sleep pattern is not specific. The study recommends the use of actigraphy along with sleep questionnaires for accurate assessment of sleep patterns in cohort studies. [ABSTRACT FROM AUTHOR]

Abstract Background Given the high prevalence of neck pain, the neck disability index (NDI) has been used to evaluate patient status and treatment outcomes. Modified versions were proposed as solutions to measurement deficits in the NDI. However, the original 10-item NDI was scored out of 50 and is still the most frequently administered version. Examining the extent of agreement between traditional and Rasch-based versions using Bland-Altman (B&A) plots will inform our understanding of score differences that might rise from using different versions. Therefore, the objective of current study was to describe the extent of agreement between different versions of NDI. Methods The current study was a secondary data analysis. The study data was compiled from two prospectively collected data sources. We performed a comprehensive literature search to identify Rasch approved NDI within four databases including Embase, Medline, PubMed, and Google Scholar. Alternate forms and scorings were compared to each other and to the standard NDI. We graphed B&A plots and calculated the mean difference and the 95% limits of agreement (LoA; ±1.96 times the standard deviation). Results Two Rasch approved alternative versions (8 vs 5 items) were identified from 303 screened publications. We analyzed data from 201 (43 males and 158 females) patients attending community clinics for neck pain. We found that the mean difference was approximately 10% of the total score between the 10-item and 5-item (− 4.6 points), whereas the 10-item versus 8-item and 8-item versus 5-item had smaller mean differences (− 2.3 points). The B&A plots displayed wider 95% LoA for the agreement between 10-item and 8-item (LoA: − 12.0, 7.4) and 5-item (LoA: − 14.9, 5.8) compared with the LoA for the 8-item and 5-item (LoA: − 7.8, 3.3). Conclusion Two Rasch-based NDI solutions (8 vs 5 items) which differ in number of items and conceptual construction are available to provide interval level scoring. They both provide scores that are substantially different from the ordinal NDI, which does not provide interval level scoring. Smaller differences between the two Rasch solutions exist and may relate to the items included. Due to the size and unpredictable nature of the bias between measures, they should not be used interchangeably.

Bland-Altman analysis is a very commonly used method in the biomedical research. This method is used to study the agreement between two measurements that are in continuous scale. Although this method is commonly used in medical research, the statistical software packages do not have the menu-driven operation for the Bland-Altman analysis. Hence this paper intends to provide an R function (BA.plot) for Bland-Altman analysis.

Purpose: Active anterior rhinomanometry (AAR) and computed tomography (CT) are standardized methods for the evaluation of nasal obstruction. Recent attempts to correlate AAR with CT-based computational fluid dynamics (CFD) have been controversial. We aimed to investigate this correlation and agreement based on an in-house developed procedure. Methods: In a pilot study, we retrospectively examined five subjects scheduled for septoplasty, along with preoperative digital volume tomography and AAR. The simulation was performed with Sailfish CFD, a lattice Boltzmann code. We examined the correlation and agreement of pressure derived from AAR (RhinoPress) and simulation (SimPress) and these of resistance during inspiration by 150 Pa pressure drop derived from AAR (RhinoRes150) and simulation (SimRes150). For investigation of correlation between pressures and between resistances, a univariate analysis of variance and a Pearson's correlation were performed, respectively. For investigation of agreement, the Bland–Altman method was used. Results: The correlation coefficient between RhinoPress and SimPress was r = 0.93 (p < 0.001). RhinoPress was similar to SimPress in the less obstructed nasal side and two times greater than SimPress in the more obstructed nasal side. A moderate correlation was found between RhinoRes150 and SimRes150 (r = 0.65; p = 0.041). Conclusion: The simulation of rhinomanometry pressure by CT-based CFD seems more feasible with the lattice Boltzmann code in the less obstructed nasal side. In the more obstructed nasal side, error rates of up to 100% were encountered. Our results imply that the pressure and resistance derived from CT-based CFD and AAR were similar, yet not same. Osteoporosis a major public health problem of the elderly, is associated with substantial morbidity and socio economic burden. The aim of the study was to screen women with low bone mass using the indigenously developed Osteocalcin (OC) ELISA kit and compare it with commercial ELISA kit and evaluate. The diagnostic potential of the assay was assessed in 359 samples from neighboring tertiary care hospitals over a period of 2 years. OC levels were estimated by the developed indigenous assay in samples, correlated with the Bone Mineral Density (BMD) measurements and compared by a commercial ELISA kit. On the basis of T-scores the women were stratified into Normal and case groups as Osteopenia and Osteoporosis. The serum biochemical parameters calcium and phosphorus were estimated on an auto-analyzer. To compare two different assays Bland–Altman plot and Deming linear regression analysis was performed. The prevalence of Osteopenia was high (56%) and Osteoporosis (13%) in the healthy Indian women aged 21–65 years with significant differences in OC levels in normal and women with low bone mass. Good correlation (p < 0.0001) in the OC levels by the two assays was observed. Cut off limits established earlier with indigenous assay (11.9 ng/mL and 14.9 ng/mL) for Osteopenia and Osteoporosis were similar to those with the commercial kit (13.2 ng/mL and 16.8 ng/mL) respectively. The diagnostic sensitivity, specificity and accuracy of the OC prototype was > 85%. The cost effective OC prototype can be used in screening and management of Indian women with low bone mass. This study aims to determine the agreement between morbidities recorded by the survey staff (TA) through questionnaire and that revealed via examination by medical officer. Methodology: A long term follow up epidemiological study was launched in 1985 covering a cohort of 80021 gas exposed people and 15931 unexposed people. A list of 40 symptoms was provided for recording the morbidities The International Classification of Diseases was followed for coding the morbidity by Medical Offic- ers. Data presented here were collected during January-December, 2016 and total 3393 multi morbid persons, ageing more than 32 years from cohort were taken. For finding the agreement of pattern morbidity between the technical assistant and the medical doctors, Kappa statistics was used. Results: Overall Kappa ‘moderate (0.41-0.60)’ agreement was ob- served in respiratory morbidity. In ophthalmic morbidity ‘Sub- stantial (0.61-0.80)’ was observed. We found overall (Respiratory, Ophthalmic, GIT and Skin) ‘Moderate (0.41-0.60)’ agreement. Conclusions: Morbid agreement analysis established that substan- tial agreement between TA and medical doctor in affected areas where as in control area moderate agreement were observed. It is better to verify all symptomatic morbid persons by medical officer.

Summary: What is known and objectives: Formulae estimating glomerular filtration rate (GFR) are frequently used to guide drug dosing. The objectives of this prospective single‐center study were to evaluate agreement between these equations and measured creatinine clearance (CrCl) in non‐critically ill surgery patients with normal kidney function and augmented renal clearance (ARC, CrCl ≥ 130 mL/min/1.73 m²), to determine predictors for disagreement, define a GFR estimator cut‐off value identifying ARC and determine the ARC prevalence and duration in non‐critically ill surgical patients. Methods: Hospitalized adult non‐critically ill abdominal and trauma surgery patients were eligible for inclusion. Measured CrCl based on an 8‐hour urinary collection (CrCl8h) was used as the primary method for determining kidney function. Agreement between equations and measured CrCl8h was assessed in terms of precision, defined as a bias within ±10 mL/min/1.73 m². Predictors for disagreement were identified for the most precise estimator using an ordinal logistic regression model with negative bias, agreement and positive bias as outcome variables. A receiver operating characteristic (ROC) analysis was performed to identify an estimator cut‐off predicting ARC, which was subsequently applied for the daily proportion of patients displaying ARC and ARC duration. Results and discussion: During the study period (14/11/2013 ‐ 13/05/2014), in 232 adult non‐critically ill abdominal and trauma surgery patients, all estimators tend to underestimate CrCl8h (mean bias ranging from 17 to 22 mL/min/1.73 m²), especially in patients displaying ARC (mean bias ranging from 44 to 56 mL/min/1.73 m²). eGFRCKD‐EPI performed the best. Younger age and low ASA score independently predicted underestimation of CrCl8h. Three different eGFRCKD‐EPI cut‐offs with decreasing sensitivity and increasing specificity (84, 95 and 112 mL/min/1.73 m²) identified, respectively, 65%, 44% and 14% patients displaying ARC. The median ARC duration was 4, 4 and 3 days, respectively. What is new and conclusion: In surgical patients, eGFR frequently underestimates measured CrCl, especially in young patients with low ASA score. eGFR cut‐offs predicting ARC were identified. In this paper, we describe and validate the EquiMoves system, which aims to support equine veterinarians in assessing lameness and gait performance in horses. The system works by capturing horse motion from up to eight synchronized wireless inertial measurement units. It can be used in various equine gait modes, and analyzes both upper-body and limb movements. The validation against an optical motion capture system is based on a Bland-Altman analysis that illustrates the agreement between the two systems. The sagittal kinematic results (protraction, retraction, and sagittal range of motion) show limits of agreement of ± 2.3 degrees and an absolute bias of 0.3 degrees in the worst case. The coronal kinematic results (adduction, abduction, and coronal range of motion) show limits of agreement of - 8.8 and 8.1 degrees, and an absolute bias of 0.4 degrees in the worst case. The worse coronal kinematic results are most likely caused by the optical system setup (depth perception difficulty and suboptimal marker placement). The upper-body symmetry results show no significant bias in the agreement between the two systems; in most cases, the agreement is within ±5 mm. On a trial-level basis, the limits of agreement for withers and sacrum are within ±2 mm, meaning that the system can properly quantify motion asymmetry. The study described here investigated whether angle-independent vector flow imaging (VFI) technique estimates peak velocities in the portal vein comparably to pulsed wave Doppler (PWD). Furthermore, intra- and inter-observer agreement was assessed in a substudy. VFI and PWD peak velocities were estimated with from intercostal and subcostal views for 32 healthy volunteers, and precision analyses were conducted. Blinded to estimates, three physicians rescanned 10 volunteers for intra- and inter-observer agreement analyses. The precision of VFI and PWD was 18% and 28% from an intercostal view and 23% and 77% from a subcostal view, respectively. Bias between VFI and PWD was 0.57 cm/s (p = 0.38) with an intercostal view and 9.89 cm/s (p <0.001) with a subcostal view. Intra- and inter-observer agreement was highest for VFI (inter-observer intra-class correlation coefficient: VFI 0.80, PWD 0.3; intra-observer intra-class correlation coefficient: VFI 0.90, PWD 0.69). Introduction: The purpose of this study was to compare root and root canal width measurements between digital intraoral radiography (IOR) and micro-computed tomography (μCT). The accuracy of IOR measurements of canine mandibular molars was scrutinized to assess feasibility of developing a model to estimate animal age based on dentinal thickness. Materials and Methods: Thirty-nine canine mandibular first molars were imaged using μCT and IOR. For each tooth, the root and root canal width of the mesial and distal roots were measured by a single observer at 3 marked sites on μCT and IOR. Two different software programs were used to measure the radiographs. The radiograph measurements were compared to each other and to the μCT measurements. The μCT images were considered the anatomic reference standard for structural representation. Results: The data collected demonstrated IOR bias and variability throughout all measurement sites, with some sites being more affected than others. Neither IOR system produced unbiased measurements that closely reflected the μCT measurements consistently. The overall lack of agreement between measurements demonstrated the difficulties in developing a standardized protocol for measuring root and root canal width for the first molar teeth in dogs. Conclusion: Developing a protocol to accurately measure and compare μCT and IOR measurements is challenging. Designing a measurement system that would allow for universal application to age dogs would require continued research utilizing a standardized approach to overcome the limitations identified in this article. Immersive virtual reality has recently developed into a readily available system that allows for full-body tracking. Can this affordable system be used for component tracking to advance or replace expensive kinematic systems for motion analysis in the clinic? The aim of this study was to assess the accuracy of position and orientation measures from Vive wireless body trackers when compared to Vicon optoelectronic tracked markers attached to (1) a robot simulating trunk flexion and rotation by repeatedly moving to know locations, and (2) healthy adults playing virtual reality games necessitating significant trunk displacements. The comparison of both systems showed component tracking with Vive trackers is accurate within 0.68 ± 0.32 cm translationally and 1.64 ± 0.18° rotationally when compared with a three-dimensional motion capture system. No significant differences between Vive trackers and Vicon systems were found suggesting the Vive wireless sensors can be used to accurately track joint motion for clinical and research data.

This paper reports on a novel approach to setting research priorities relevant to the needs of older LGBT people. Research is growing in this area and has recognised the negative impact of contemporary and historical discrimination towards non-normative genders and sexualities. The results of a symposium, survey and agreement analysis are presented to identify the levels of priority placed on 60 different research topics. Discussion focuses on the novelty and/or similarity to existing research patterns on LGBT ageing, as well as prioritising topics such as: how to include unheard voices; exploring trans* people's experiences and preferences around long-term hormone use; and, embedding research findings into policy and practice. Objectives: To determine the impact of consensus conferences on the frequency of discrepant cases in a surgical pathology practice.Methods: The percentage of discrepancies in cases reviewed at a weekly consensus conference was calculated for the first and last months of a 13-month period. Both interrater agreement and agreement with the consensus diagnoses were assessed. A total of 309 diagnoses were performed for the first month and 518 for the last month. Both absolute and chance-corrected agreement were calculated for each period.Results: Absolute agreement rate increased from 91.2% in the first month to 98.2% in the final month. Chance-corrected agreement increased from 0.80 in the first month to 0.97 in the final month.Conclusions: The consensus conference technique appears to be a useful method to reduce intradepartmental diagnostic discrepancies. It is critical to reliably estimate Low Density Lipoprotein Cholesterol (LDL-C) in patients with concomitant hypertriglyceridemia and low LDL-C. We retrospectively compared the performances of the Friedewald (LDL-F), Martin-Hopkins (LDL-MH) and Sampson (LDL-SA) equations against a direct homogeneous LDL-C assay (dLDL-C) on observations presenting mild hypertriglyceridemia (triglycerides between 1.69 and 3.9 mmol/L) and low LDL-C (< 2.58 mmol/L). Observations were stratified according to their LDL-C. Agreement of the equations with dLDL-C was assessed using Intraclass Correlation Coefficients (ICC) with an agreement cut-off of 0.9, and analysis of Bland-Altman plots. Independently of the LDL-C stratum evaluated, the three equations failed to meet the 0.9 ICC cut-off, although their agreement with dLDL-C improves as LDL-C increases. Analysis of Bland-Altman plots shows a downwards discordance of LDL-F with dLDL-C, and an upwards discordance of LDL-MH and LDL-SA with direct LDL-C. LDL-MH resulted in the least observations outside the Bland-Altman limits of agreement. While no equation can be deemed satisfactory enough to replace direct assays in patients with low LDL-C and concomitant hypertriglyceridemia, LDL-MH seems to perform better than the other equations in estimating LDL-C in these patients.

OBJETIVO: Avaliar a concordância da escala CDR com critérios diagnósticos e mini exame do estado mental (MEEM), e correlação com escala de Blessed, numa amostra de pacientes do sul do Brasil. MÉTODO: A escala foi avaliada em 269 pacientes com doença de Alzheimer (DA), demência vascular e demência questionável num desenho transversal. Os critérios do NINCDS-ADRDA para provável DA e NINDS-AIREN para provável demência vascular foram os padrões-ouro. O MEEM, a escala Blessed para gravidade da demência, o escore isquêmico de Hachinski, e uma bateria de testes cognitivos também foram aplicados. RESULTADOS: A concordância com o padrão-ouro foi boa (kappa=0,73), e com o MEEM em categorias foi moderada (kappa= 0,53). Observou-se correlação significativa da escala CDR com Blessed (r=0,96; p=0,001). Não se observou diferença de escolaridade ou de idade entre as categorias da escala CDR. CONCLUSÃO: A concordância da CDR foi boa para os critérios diagnósticos e moderada para o MEEM. A escala mostrou validade de construto para gravidade de demência. Não se observou impacto da escolaridade sobre este instrumento.OBJECTIVE: The objective of the study was the analysis of agreement between the CDR scale with diagnostic criteria and mini mental state examination (MMSE), as well as correlation with Blessed scale, in a sample of Southern Brazilian patients. METHOD: The CDR scale was cross-sectionaly evaluated in 269 dementia patients Alzheimer’s disease (AD) vascular dementia, and questionable. The NINCDS-ADRDA criteria for probable AD and the NINDS-AIREN for probable vascular dementia were the gold standard. The MMSE, the Blessed scale, the Hachinski ischemic score, and a battery of cognitive tests were also applied. RESULTS: The agreement to gold standard was good (kappa=0.73), while to MMSE categorized was moderate (kappa=0.53). A significant correlation with the Blessed scale (r=0.96; p=0.001) was observed. Education and age were similar among CDR categories. CONCLUSION: The global score agreement of the CDR scale with the gold standard was good, and with the MMSE was moderate. We also observed face validity for dementia severity. No impact of education was observed upon CDR global scores.
