1. Managing Fire Risk for Construction of Watercare's HDPE-Lined Central Interceptor Tunnels.

Abstract

Fire life safety (FLS) risk during construction of Watercare's Central Interceptor wastewater project in Auckland, New Zealand, is increased due to the combustible high-density polyethylene (HDPE) plastic used to fully line the main tunnel and link sewers. With the single-bore main tunnel having a length of 14.7 km, fire risk is compounded, as opportunities for egress and smoke management are limited, and so there is increased risk to Tunnel Boring Machine (TBM) crews and to workers in the smaller link sewers. It is the extensive use of polymer linings that make the Central Interceptor project an interesting FLS case study, and a unique construction project in Australasia. The project invested in fire testing to understand the HDPE combustion hazard during construction, and to assess the benefits of water mist suppression as a potential risk mitigation measure. It was found that the material was difficult to ignite and did not promote fire spread. Misting was demonstrated to be effective at suppressing a fire involving HDPE liner. Outcomes of the testing informed a wider risk assessment. A suite of engineering and operational controls was developed. To complement "business as usual" measures, other novel controls were implemented, including a misting system behind the TBM trailing gear and innovative misting "fire breaks" distributed along the length of the tunnel at 1500 m intervals. With the efforts made to minimise the hazard of fire and the potential for localised flashover and spread, and to facilitate self-evacuation, it was concluded that the plastic liner materials do not unacceptably increase risk to life or asset, relative to other tunnels without a combustible liner. Abstract

Co-innovation can be effective for addressing complex challenges involving multiple stakeholders who hold diverse viewpoints and perceptions. Addressing these challenges requires changes at various scales. However, there is limited systematic research on how co-innovation can work in different projects and what impacts it can have on the outcome. Co-innovation principles were tested for their potential to enhance the impact delivered by a fundamental research project investigating options for managing the tomato potato psyllid/Candidatus Liberibacter solanacearum complex in potato crops. A series of interviews and workshops were undertaken to increase interactions within the programme and with industry stakeholders. Additionally, a member of the research team, trained in co-innovation theory and practice, took a leadership role in planning and delivering technology transfer activities. Abstract

Two-person non-zero-sum games of the Prisoner's Dilemma or mixed-motive type have attracted considerable interest in recent years. These games have been discussed extensively in the literature. The games also bear a compelling similarity to many real-life conflicts including the Cold War. In these games a competitive strategy results in a lower payoff to both players than does a cooperative strategy. Adoption of a cooperative strategy leaves the player vulnerable to larger losses, however, unless this strategy is adopted bilaterally. The present study was directed at assessing the effect, on incidence of cooperation, of having two people act as one player in a Prisoner's Dilemma game. In real-life conflict a side is often represented by more than one person, yet it is not known whether one person might not be more cooperative. Experiments in problem-solving have generally shown that groups are superior to individuals. In order to ensure that the subjects were faced with a relatively cooperative opponent and in order to decrease the variability of results arising from variability of both sides, the subjects played against simulated opponents. The results of an analysis of variance of all 12 possible scores resulting from combinations of played, expected, and opponent moves are shown in the article.

Abstract

Objectives: Neonates with respiratory failure are ideally supported with veno-venous rather than veno-arterial extracorporeal membrane oxygenation due to the reduced rate of neurologic complications. However, the proportion of neonates supported with veno-venous extracorporeal membrane oxygenation is declining. We report multisite veno-venous extracorporeal membrane oxygenation, accessing the neck, returning to the inferior vena cava via the common femoral vein in neonates and children less than 10 kg., Design: Retrospective case series with 1 year minimum follow-up., Patients: Patients less than 10 kg supported with veno-venous extracorporeal membrane oxygenation accessing the jugular and returning to the femoral vein., Setting: A 30-bed pediatric intensive care delivering extracorporeal membrane oxygenation to approximately 20 children annually., Interventions: Veno-venous extracorporeal membrane oxygenation accessing the jugular and returning to the femoral vein was delivered using two single lumen cannulae., Measurements and Main Results: January 2015 to August 2019, 11 patients underwent veno-venous extracorporeal membrane oxygenation accessing the jugular and returning to the femoral vein with median weight of 3.6 kg (interquartile range 2.8-6.1 kg), and median corrected gestational age of 13 days (interquartile range, 2-175 d). The smallest patient weighed 2.1 kg. Seven patients had comorbidities. Extracorporeal membrane oxygenation was technically successful in all patients with median flows of 126 mL/kg/min (interquartile range, 120-138 mL/kg/min) and median arterial oxygenation saturation of 94% (interquartile range, 91-98%) at 24 hours. Nine survived to home discharge, and two were palliated. Common femoral vein occlusion was observed in all patients on ultrasound post decannulation. There was no clinical or functional deficit in the cannulated limb at follow-up, a minimum of 1 year post extracorporeal membrane oxygenation., Conclusions: Veno-venous extracorporeal membrane oxygenation accessing the jugular and returning to the femoral vein was performed safely in patients under 10 kg with the smallest patient weighing 2.1 kg. Although occlusion of the common femoral vein was observed in patients post decannulation, subsequent follow-up demonstrated no clinical implications. Abstract

Extracorporeal life support (ECLS) for severe respiratory failure has seen an exponential growth in recent years. Extracorporeal membrane oxygenation (ECMO) and extracorporeal CO 2 removal (ECCO 2 R) represent two modalities that can provide full or partial support of the native lung function, when mechanical ventilation is either unable to achieve sufficient gas exchange to meet metabolic demands, or when its intensity is considered injurious. While the use of ECMO has defined indications in clinical practice, ECCO 2 R remains a promising technique, whose safety and efficacy are still being investigated. Understanding the physiological principles of gas exchange during respiratory ECLS and the interactions with native gas exchange and haemodynamics are essential for the safe applications of these techniques in clinical practice. In this review, we will present the physiological basis of gas exchange in ECMO and ECCO 2 R, and the implications of their interaction with native lung function. We will also discuss the rationale for their use in clinical practice, their current advances, and future directions.

37. Routine postoperative blood tests in all patients undergoing Total Hip Arthroplasty as part of an enhanced recovery pathway: Are they necessary?

Abstract

Introduction: The increasing demand for Total Hip replacement (THR)/Total Hip Arthroplasty (THA) continues to place significant financial pressure on the National Health Service (NHS). Many institutions undertake post-operative blood tests routinely in this group of patients. The aim of this study was to identify if such routine blood tests (Full Blood Count (FBC) and Urea and Electrolytes(U&Es)) are required in all THR patients post-operatively., Methods: Single institute, Multi-surgeon, retrospective observational study of consecutive patients who underwent primary elective THR done from Jan 2014 to Dec 2018.Post-operative blood tests and medical records were reviewed to identify derangement in haemoglobin (Hb) level and renal function requiring clinical intervention., Results: Over the period of 4 years, 353 patients underwent elective THR with mean age of 70 years (range: 42-90). There were 203 Males and 150 Females. Mean pre-operative Hb was 134.7 g/l. Mean post-operative drop in Hb was 22.3 g/l. None of the patients in ASA grade 1 and 2 with age ≤70 years required blood transfusion post operatively.6.4% of patients (n = 18/280) with an ASA of 1-2 had postoperative blood results requiring intervention of which only four (1.2%) were ≤70 years of age compared to 17.8% of patients (n = 14/73) with ASA 3-4.Overall none of the patients in ASA grade 1 and 2 with age ≤70 years required blood transfusion post operatively nor had electrolyte disturbance. 1.2% had deranged renal function that needed minor clinical intervention., Conclusion: Routine post-operative blood analyses may not required for all patients undergoing THA. 38. Suicide and undetermined death by drowning.

Abstract

Background Incidence of drowning varies among countries and even communities within the same country. The manner of death may remain undetermined even when the diagnosis of drowning is confirmed or excluded. It is expected that drowning suicide rates in some of the available studies may have been over- or under-reported because of misclassification. Objectives This study presents data on the time trends, age/sex mortality rates from death by drowning in England and Wales between 1979 and 2001. All coroners' verdicts in death by drowning; suicide, deaths undetermined whether accidentally or purposely inflicted were examined. Methods Counts of suicide due to drowning and submersion (ICD 9 codes; E954) and undetermined injury deaths (E984) 1 , reported in England and Wales between 1979 and 2001 were obtained from National Statistics (ONS). National population estimates were used to calculate age-specific rates and age-standardised rates of death by drowning. Results Over the last 23 years, there has been a gradual and significant reduction in drowning deaths. The decline in drowning suicide was less evident in the elderly, particularly those over the age of 75. During the study period, 62 and 38% of all death by drowning attracted an open verdict and suicide, respectively. Elderly drowning appear to attract more verdicts of suicide compared to younger age groups (odds ratio 4.3, 95% CI 2.3-8.3). Women, particularly elderly, are more likely to have a suicide verdict returned in drowning compared to men (odds ratio 1.5, 95% CI 1.1-1.6). Conclusion The high rate of open verdicts in death by drowning over the study period and compared to any other method of fatal self harm in England and Wales confirms the difficulties in reaching a firm conclusion in drowning death. Combining suicide and all undetermined deaths in drowning in nationally collected statistics may result in grossly exaggerated rates and misleading trends in suicidal drowning.

Abstract

The amount of information that should be processed to reach a rational solution is so vast that it overwhelms our limited memory and analytic capacity. To grapple at all with real problems, we must shrink them to mind size. As Nobel Laureate Herbert Simon has suggested, we must learn to "satisfice." Using examples and a case history of managerial succession, Agnew and Brown demonstrate the importance of nonrational factors in decision making. "Skyhooks," their first major concept, are composed of an individual's strong beliefs and biases that appear almost as acts of faith without any obvious foundation. They help the exceptional executive operate on a limited set of alternatives. But while skyhooks give direction, they do not provide the means to reach a destination. Simpler conceptual models, or "walking sticks," are necessary to cover the rocky trails. Agnew and Brown offer four walking sticks that can be applied to managerial succession problems. The first walking stick considers executive decision making in three parts--nonrational, semirational, and rational--and covers the conditions in which each component comes into play. The second helps examine human resources as fixed or fluid and as assets or liabilities to develop a schema for manpower accounting in executive succession. Playing vs. talking a good executive game is the topic for walking stick three. The concern here is to distinguish the real players--who can manage systems--from the mere talkers, who can manage only fragments of systems. The fourth walking stick draws on the law of resource gravitation and crystallization, a law with implications for trainers: You can't fashion management training to fit all comers; instead, you must tailor it to what the trainees already are and know.
