Background: Thunderstorm asthma is a term used to describe surges in acute respiratory illnesses following a thunderstorm and is often attributed to an intense exposure to aeroallergens. Several episodes of thunderstorm asthma have been observed worldwide; however, no such cases have been described in Sweden. In Sweden, the most prominent exposure to air-borne pollen occurs during the blooming of the birch. We aimed to explore the associations between respiratory health and the combined exposure to thunderstorms and birch pollen. Methods: We investigated the association between the daily numbers of outpatient visits due to respiratory cases and the combined exposure to thunderstorms and birch pollen during the period of 1 May–31 September in 2001–2017, in Stockholm County, Sweden, by using time series analysis with log linear models. Results: We detected noticeable increases in the number of outpatient visits on both the same day (max 26%; 95% CI 1.16–1.37) and the day after (max 50%; 95% CI 1.32–1.70) the occurrence of a thunderstorm, when the concentrations of birch pollen and the number of lightning discharges were within the highest categories. Conclusions: It is possible that co-exposure to heavy thunderstorms and high concentrations of birch pollen affects the respiratory health of the Stockholm population. To the best of our knowledge, this is the first study addressing the thunderstorm-related respiratory illnesses in Sweden and the effects of birch pollen. Our study may be important for future public health advice related to thunderstorm asthma.

The paper suggests a methodology for predicting next-year seasonal pollen index (SPI, a sum of daily-mean pollen concentrations) over large regions and demonstrates its performance for birch in Northern and North-Eastern Europe. A statistical model is constructed using meteorological, geophysical and biological characteristics of the previous year). A cluster analysis of multi-annual data of European Aeroallergen Network (EAN) revealed several large regions in Europe, where the observed SPI exhibits similar patterns of the multi-annual variability. We built the model for the northern cluster of stations, which covers Finland, Sweden, Baltic States, part of Belarus, and, probably, Russia and Norway, where the lack of data did not allow for conclusive analysis. The constructed model was capable of predicting the SPI with correlation coefficient reaching up to 0.9 for some stations, odds ratio is infinitely high for 50% of sites inside the region and the fraction of prediction falling within factor of 2 from observations, stays within 40-70%. In particular, model successfully reproduced both the bi-annual cycle of the SPI and years when this cycle breaks down.

In the present study, the phenological and quantitative changes in the pollen seasons between 1973 and 2013 in the Stockholm region of Sweden were studied for nine types of pollen (hazel, alder, elm, birch, oak, grass, mugwort, willow and pine). Linear regression models were used to estimate the long term trends in duration, start- and end-dates, peak-values and the yearly accumulated pollen sums of the pollen seasons. The pollen seasons of several arboreal plant species (e.g. birch, oak and pine) were found to start significantly earlier today compared to 41 years earlier, and have an earlier peak-date, while the season of other species seemed largely unaffected. However, the long term trends in the end-dates of pollen seasons differed between arboreal and herbaceous species. For herbaceous species (grass and mugwort), a significant change towards later end-dates was observed and the duration of season was found to have increased. A significant trend towards an earlier end-date was found in the majority of the arboreal plant species (i.e. elm, oak, pine and birch), but the length of the season seemed unaffected. A trend towards an increase in yearly concentrations of pollen was observed for several species; however the reasons for this phenomenon cannot be explained unambiguously by the present study design. The trend of increasing yearly mean air temperatures in the Stockholm area may be the reason to changed phenological patterns of pollen seasons.

In the present study, the airborne concentrations of birch and oak pollen grains and birch pollen allergens have been recorded in samples from a common sampling station in Stockholm. The sampling period was between April 22nd and May 31st 2003. The objectives were to evaluate if analysis of allergen particles in parallel with pollen grains would be relevant to aid subjects suffering from pollinosis. Days with low birch pollen counts had relatively high nominal allergen concentrations in the beginning of the sampling period. The birch pollen grain concentration peaks, during the dry pollination culmination interval in the middle of the period, were associated with correspondingly lower nominal concentrations of allergens than grains. At the end of the sampling period very high nominal amounts of allergen appeared, as reflected by high concentrations of oak pollen grains. Birch pollen and antigens were collected on sampling tape of the Burkard Seven-day-recording Pollen and Spore Trap before, during and after the pollen season. The antigens were then transferred to a polyvinylidene difluoride (PVDF) membrane. For detection and quantification of the antigens a double antibody system and chemiluminescence was used. The chemiluminescence-chemicals emit light at positions in the membrane where antigen is present. The light produced was documented by contact of the blotting membrane with x-ray film, and also measured with a newly developed luminometer. Birch pollen antigenic activity was found on the tapes a few weeks before the local incipient birch pollen season although no birch pollen grains were monitored on a control tape for allergy information service. The concentration of small antigenic particles present in the air after rainfall is discussed.

Airborne concentrations of pollen from Betula (birch), Poaceae (grasses) and Artemisia (mugwort) are compared during a seven year period (90–96) with respect to both quantitative and seasonal aspects, at three different sampling sites, one in Estonia (Tartu) and two in Sweden (Stockholm and Roma on the island of Gotland). All three taxa occur in the region and are well‐known causes of allergic sensitisation. The annual total and peak values of birch, grass and mugwort pollen were found to be much higher in Tartu than in Stockholm and Roma. Both the birch and the grass pollen seasons ended later in Stockholm than in Roma and Tartu. The mugwort flowering season always began earlier in Stockholm than at the other sites, and more days elapsed between start day and peak day in Stockholm than in Tartu.
