Abstract

Introduction Prevention of cardiovascular disease (CVD) is of key importance in reducing morbidity, disability and mortality worldwide. Observational studies suggest that digital health interventions can be an effective strategy to reduce cardiovascular (CV) risk. However, evidence from large randomised clinical trials is lacking. Methods and analysis The CV-PREVITAL study is a multicentre, prospective, randomised, controlled, open-label interventional trial designed to compare the effectiveness of an educational and motivational mobile health (mHealth) intervention versus usual care in reducing CV risk. The intervention aims at improving diet, physical activity, sleep quality, psycho-behavioural aspects, as well as promoting smoking cessation and adherence to pharmacological treatment for CV risk factors. The trial aims to enrol approximately 80 000 subjects without overt CVDs referring to general practitioners' offices, community pharmacies or clinics of Scientific Institute for Research, Hospitalization and Health Care (Italian acronym IRCCS) affiliated with the Italian Cardiology Network. All participants are evaluated at baseline and after 12 months to assess the effectiveness of the intervention on short-term endpoints, namely improvement in CV risk score and reduction of major CV risk factors. Beyond the funded life of the study, a long-term (7 years) follow-up is also planned to assess the effectiveness of the intervention on the incidence of major adverse CV events. A series of ancillary studies designed to evaluate the effect of the mHealth intervention on additional risk biomarkers are also performed. Ethics and dissemination This study received ethics approval from the ethics committee of the coordinating centre (Monzino Cardiology Center; R1256/20-CCM 1319) and from all other relevant IRBs and ethics committees. Abstract

Transcatheter mitral valve replacement (TMVR) is an emerging alternative treatment for those patients not qualified for surgery. However, TMVR can determine an obstruction of the left ventricular outflow tract (namely, neo-LVOT) induced by the transcatheter heart valve (THV) displacing the native mitral valve leaflet towards the myocardial wall. This condition can lead to haemodynamic impairment and ultimately patient death. We sought to predict the neo-LVOT obstruction by first developing patient-specific simulations of the THV deployment and then comparing predictions with post-TMVR diagnostic images and 3D printed human models. Using pre-TMVR computed-tomography (CT) imaging, patient-specific anatomies of two patients were reconstructed and then meshed with ABAQUS/Explicit solver. The latter was used to simulate the crimping and deployment of SAPIEN 3 THV (Edwards Lifesciences, Irvine, CA) and then simulate the cardiac beating. The neo-LVOT cross sectional area was then computed. For both patients, rigid heart models were printed with SLS rapid prototyping technology while the SAPIEN 3 device was printed with flexible resin to manually positioning the device in the human host. Numerical predictions and 3D printed model measurements of neo-LVOT area agreed well with those obtained by post-TMVR CT imaging. Both computational modelling and 3D printing revealed a comprehensive assessment of the TMVR feasibility, which is not readily conferred by conventional CT imaging. Particularly, the realistic printed models could become an efficient and indispensable tool to help the heart team visualizing the LVOT obstruction to anatomical scale, in a life-size replica of patient-specific cardiac anatomy.

Abstract

Background: Identifying candidates for left ventricular assist device surgery at risk of right ventricular failure remains difficult. The aim was to identify the most accurate predictors of right ventricular failure among clinical, biological, and imaging markers, assessed by agreement of different supervised machine learning algorithms. Methods: Seventy-four patients, referred to HeartWare left ventricular assist device since 2010 in two Italian centers, were recruited. Biomarkers, right ventricular standard, and strain echocardiography, as well as cath-lab measures, were compared among patients who did not develop right ventricular failure (N = 56), those with acute–right ventricular failure (N = 8, 11%) or chronic–right ventricular failure (N = 10, 14%). Logistic regression, penalized logistic regression, linear support vector machines, and naïve Bayes algorithms with leave-one-out validation were used to evaluate the efficiency of any combination of three collected variables in an “all-subsets” approach. Results: Michigan risk score combined with central venous pressure assessed invasively and apical longitudinal systolic strain of the right ventricular–free wall were the most significant predictors of acute–right ventricular failure (maximum receiver operating characteristic–area under the curve = 0.95, 95% confidence interval = 0.91–1.00, by the naïve Bayes), while the right ventricular–free wall systolic strain of the middle segment, right atrial strain (QRS-synced), and tricuspid annular plane systolic excursion were the most significant predictors of Chronic-RVF (receiver operating characteristic–area under the curve = 0.97, 95% confidence interval = 0.91–1.00, according to naïve Bayes). Conclusion: Apical right ventricular strain as well as right atrial strain provides complementary information, both critical to predict acute–right ventricular failure and chronic–right ventricular failure, respectively.

Abstract

In a quite short time after its envisioning, Information Centric Networking paradigm seems to be a standing reality: name-driven primitives have been defined to support networking functionalities and a few architectures have been implemented. In fact, a content consumer can now search for a data item by name and completely disregard its IP address. But the question to raise is: is this searching mechanism really information-centric or is it rather name-centric only? This paper defines the requirements enabling the transition to a paradigm for content exchange which is exclusively based on information. A running example, based on the Web of Data, has been illustrated to describe the key functionalities of the proposed approach. The envisioned solution grounds on a mechanism to embed content in names which, notably, is architecture-agnostic and can run on top of all existing ICN flavors designed so far. This mechanism supports a semantic-based retrieval of content, that returns a list of contents potentially satisfying the consumer, incrementing retrieval precision and recall. The enhancement in terms of recall comes at the cost of a communication overhead, which has been analytically modeled and estimated in this paper.

Abstract

Right ventricular failure (RVF) after left ventricular assist device (LVAD) implantation is associated with increased morbidity and mortality, but the identification of LVAD candidates at risk for RVF remains challenging. We undertook a systematic review and meta-analysis of observational studies of risk factors associated with RVF after LVAD implant. Thirty-six studies published between 1 January 1995 and 30 April 2015, comprising 995 RVF patients out of a pooled final population of 4428 patients, were identified. Meta-analysed prevalence of post-LVAD RVF was 35%. A need for mechanical ventilation [odds ratio (OR) 2.99], or continuous renal replacement therapy (CRRT; OR 4.61, area under the curve 0.78, specificity 0.91) were the clinical variables with the highest effect size (ES) in predicting RVF. International normalized ratio [INR; standardized mean difference (SMD) 0.49] and N-terminal pro-brain natriuretic peptide (NT-proBNP) (SMD 0.52) were the biochemical markers that best discriminated between RVF and No-RVF populations, though NT-proBNP was highly heterogeneous. Right ventricular stroke work index (RVSWI) and central venous pressure (CVP) (SMD −0.58 and 0.47, respectively) were the haemodynamic measures with the highest ES in identifying patients at risk of post-LVAD RVF; CVP was particularly useful in risk stratifying patients undergoing continuous-flow LVAD implant (SMD 0.59, P < 0.001, I2 = 20.9%). Finally, pre-implant moderate to severe right ventricular (RV) dysfunction, as assessed qualitatively (OR 2.82), or a greater RV/LV diameter ratio (SMD 0.51) were the standard echocardiographic measurements with the highest ES in comparing RVF with No-RVF patients. Longitudinal systolic strain of the RV free wall had the highest ES (SMD 0.73) but also the greatest heterogeneity (I2 = 74%) and was thus only marginally significant (P = 0.05). Abstract

Information Centric Networking represents a fundamental technology for the Future Internet. Its baseline functionalities can be extended to support publish-subscribe communication schema. But, in case of consumer mobility, its benefits slam against two main drawbacks. On one hand, available handover management solutions temporarily leave wrong forwarding information within network routers and do not take care of data dissemination across stale paths. On the other hand, mobile consumers inevitably lose content updates during the handover and definitively become unaware about the latest version of the content to request. By leveraging the potentials of the Software-Defined Network paradigm, this paper formulates new methodologies willing to solve these problems. Specifically, it proposes protocols for (1) dynamically updating forwarding functionalities through the control plane when the consumer detaches from the network and (2) restoring the synchronization between consumer and producer when the former one attaches to a new network attachment point. It addition, it develops preliminary analytical models for evaluating the average number of control information these protocols require to exchange per unit of time. The resulting analysis illustrates the pros and cons they achieve in different network configurations.

Abstract

The chapter focuses on Internet topology models. First, it investigated topology representations provided at different levels of granularity. Second, it reviewed topology models based on the graph theory and related topology generator tools. Third, it studied analytical models showing average shortest path and distribution of the shortest path length for scale-free networks. In fact, starting from these concepts, the accuracy of reference models for the shortest path are studied and compared through computer simulations. Obtained results demonstrate that available models are able to catch the average value and the distribution of the shortest path distribution over a very broad set of conditions. More specifically, when m = 1, the Gamma distribution shows the lowest distance for all N values. When m = 2 and m = 3, instead, the Weibull distribution provides the lowest error. The lognormal distribution always registers the worst behavior. On the contrary, all the evaluated models require a case-by-case tuning of their parameters, which represents an important limit to be solved in future research activities.

Abstract

Recently, the growth of Internet of Things (IoT) devices combined with edge computing opened many opportunities for several novel applications. Typical examples are Unmanned Aerial Vehicles (UAV) that are deployed for photogrammetry, surveillance, disaster rapid response and environmental monitoring. A common challenge across all these networked applications is the ability to provide a persistent service - a service able to continuously maintain a high level of performance - responding to events that may change the state of the network, e.g., nodes or link failures. To cope with this challenge, in this paper we propose APRON, an edge cloud-assisted architecture for distributed and adaptive task planning management in a network of IoT devices, e.g., drones. APRON uses a novel planning strategy that, leveraging a Jackson's network model, supports monitoring and control operations while the states of the (edge or cloud) network evolve. By using APRON, edge computing application programmers can design and implement a wide range of IoT task management policies leveraging different protection methodologies across several failure models.

Abstract

Children constitute a vulnerable population and special considerations are necessary in order to provide proper care for them during disasters. After disasters such as Hurricane Katrina, the rapid identification and protection of separated children and their reunification with legal guardians is necessary to minimize secondary injuries (i:e, physical and sexual abuse, neglect and abduction). At Camp Gruber, an Oklahoma shelter for Louisianan's displaced by Hurricane Katrina, of the 254 children at the camp, 36 ((i.e, 14.2%) were separated from their legal guardians. It took 6 months to reunify the last children; 70% of the children were with their legal guardian after 2 weeks. Imagine not knowing for 2 weeks (or 6 months) if your children are dead or alive. To exacerbate these natural challenges, during a disaster Internet connectivity is scarse or unreliable.

Abstract

Objectives: The rapid fatality of pancreatic cancer is, in large part, the result of diagnosis at an advanced stage in the majority of patients. Identification of individuals at risk of developing pancreatic adenocarcinoma would be useful to improve the prognosis of this disease. There is presently no biological or genetic indicator allowing the detection of patients at risk. Our main goal was to identify copy number variants (CNVs) common to all patients with sporadic pancreatic cancer. Methods: We analyzed gene CNVs in leukocyte DNA from 31 patients with sporadic pancreatic adenocarcinoma and from 93 matched controls. Genotyping was performed with the use of the GeneChip Human Mapping 500K Array Set (Affymetrix). Results: We identified 431 single nucleotide polymorphism (SNP) probes with abnormal hybridization signal present in the DNA of all 31 patients. Of these SNP probes, 284 corresponded to 3 or more copies and 147 corresponded to 1 or 0 copies. Several cancer-associated genes were amplified in all patients. Conversely, several genes supposed to oppose cancer development were present as single copy. Conclusions: These data suggest that a set of 431 CNVs could be associated with the disease. This set could be useful for early diagnosis.
