Purpose: This study examines online speech processing in typically developing and late-talking 2-year-old children, comparing both groups' word recognition, word prediction, and word learning. Method: English-acquiring U.S. children, from the “When to Worry” study of language and social–emotional development, were identified as typical talkers ( n = 67, M age = 27.0 months, SD = 1.4; Study 1) or late talkers ( n = 30, M age = 27.0 months, SD = 2.0; Study 2). Children completed an eye-tracking task assessing their ability to recognize both nouns and verbs, to use verbs to predict an upcoming noun's referent, and to use verbs to infer the meaning of novel nouns. Results: Both typical and late talkers recognized nouns and verbs and used familiar verbs to predict the referents of upcoming nouns, whether the noun was familiar (“You can eat the apple”) or novel (“You can eat the dax”). Late talkers were slower in using familiar nouns to orient to the target and were both slower and less accurate in using familiar verbs to identify the upcoming noun's referent. Notably, however, both groups learned and retained novel word meanings with similar success. Conclusions: Late talkers demonstrated slower lexical processing, especially for verbs. Yet, their success in using familiar verbs to learn novel nouns suggests that, as a group, their slower processing did not impair word learning in this task. This sets the foundation for future work investigating whether these measures predict later language outcomes and can differentiate late talkers with transient delays from those with language disorders.

Late talkers are a heterogeneous group of children who experience delayed language development in the absence of other known causes (Fisher, 2017). Late talkers show delays in expressive phonological development, but less is known about their receptive phonological development. In the current study, a group of U.S. monolingual English-speaking typical talkers (n = 23, mean age = 26.27 months, 57 % male; 78.3 % White) and late talkers (n = 22, mean age = 24.57 months, 59% male, 72.7% White) completed a Looking-While-Listening task to assess their sensitivity to mispronunciations. Results revealed that late talkers and typical talkers looked less to the referent of a word when it was mispronounced than when it was correctly pronounced, suggesting they were sensitive to mispronunciations. Moreover, there were no significant differences between the two groups in their sensitivity to mispronunciations.

Recent advances in recording technology provide unique opportunities to observe children’s everyday speech environments using daylong audio recordings. A number of studies have proposed that speech may support learning differently when speech is directed to a child (target child directed speech, tCDS) than when that speech is directed to others (ODS) (Shneidman & Goldin-Meadow, 2012; Weisleder & Fernald, 2013). To identify periods of tCDS and ODS, researchers typically rely on the time-consuming and laborious work of human listeners who consider numerous features when making judgments. Human listeners are also used to identify periods when children are sleeping or awake. In this paper, we detail our efforts to automate these processes. We analyzed over 1,000 hours of audio from daylong recordings of 153 English- and Spanish-speaking families in the U.S. with 17- to 28-month-old children that had been previously coded for periods of sleep, tCDS, and ODS. We first explored patterns of features that characterized periods of tCDS and ODS. Then, we evaluated two classifiers that were trained using automated measures generated from LENATM, including frequency (AWC, CTC, CVC) and duration (meaningful speech, distant speech, TV, noise, silence) measures. Results revealed high sensitivity and specificity in classifying periods of sleep, and moderate sensitivity and specificity in classifying periods of tCDS and ODS. Model-derived predictions from our tCDS/ODS classifier yielded similar patterns of correlations as previously-published findings, with variation in tCDS, but not ODS, positively linked to children’s later vocabularies (Weisleder & Fernald, 2013). This work offers promising tools for streamlining work with daylong recordings, thereby facilitating research that aims to better understand how children learn from their everyday speech environments.

Purpose: Late talkers are a group of children who exhibit delays in language development without a known cause (Fisher, 2017). While a hallmark of late talkers is a reduced expressive vocabulary, little is known about late talkers’ processing of semantic relations among words in their emerging vocabularies. This study uses an eye-tracking task to compare two-year-old late talkers’ and typical talkers’ sensitivity to semantic relationships among early acquired words.Method: U.S. monolingual English-speaking late talkers (n = 21) and typical talkers (n = 24) completed a looking-while-listening task in which they viewed two images on a screen (e.g., a shirt and a pizza) while they heard words that referred to one of the images (e.g., Look! Shirt!; target-present condition) or a semantically related item (e.g., Look! Hat!; target-absent condition). Children’s eye movements (i.e., looks to the target) were monitored to assess their sensitivity to these semantic relationships. Results: Both late talkers and typical talkers looked longer at the semantically related image than the unrelated image on target-absent trials, demonstrating sensitivity to the taxonomic relationships used in the experiment. There was no significant group difference between late talkers and typical talkers. Both groups also looked more to the target in the target-present condition than in the target-absent condition.Conclusions: These results reveal that, despite possessing smaller expressive vocabularies, late talkers have encoded semantic relationships in their receptive lexica and activate these during real-time language comprehension. This study furthers our understanding of late talkers’ emerging linguistic systems and language processing skills.

Objectives In this study, we examined (1) whether a reading aloud intervention, Universidade do Bebe (UBB), had impacts on self-regulation; (2) whether effects on child outcomes were mediated by self-regulation; and (3) whether effects of UBB were explained through a sequential pathway of impact, including cognitive stimulation in the home, parent-child interactive reading, and self-regulation. Methods We performed a cluster randomized controlled trial of UBB in child care centers serving low-income children (mean age 37.4 months; SD = 6.5) in Northern Brazil. The child care centers were randomized to receive UBB or standard care (control). Families in UBB could borrow children's books weekly and participate in monthly workshops focused on reading aloud. Parent-child dyads (n = 484, intervention = 232, control = 252) were evaluated at baseline and 9 months later on: child self-regulation, vocabulary, intelligence quotient (IQ), working memory, and phonological memory and measures of cognitive stimulation in the home and parent-child interactive reading. Multilevel analyses accounted for baseline performance, sociodemographics, and clustering within centers and sites. Results The UBB group showed significantly higher self-regulation (Cohen's d = 0.25), compared with the control group, particularly in the subdomains of Attention (d = 0.24) and Impulse Control (d = 0.21). Previously shown impacts of UBB on receptive vocabulary, IQ, and working memory were mediated by self-regulation. Effects of UBB on self-regulation and child outcomes were partially explained through cognitive stimulation in the home and parent-child interactive reading. Conclusion Self-regulation represents an important mechanism by which reading aloud interventions affect language and cognitive outcomes. Investigators should consider the role of self-regulation when refining interventions, seeking to prevent poverty-related disparities.

Young children learn to communicate in the language(s) of their communities, yet the individual trajectories of language development and the particular language varieties and modes of communication children acquire vary depending on the contexts in which they live. This review describes how context shapes language development. Building on the bioecological model of development, we conceptualize context as a set of nested systems surrounding the child, from the national policies and cultural norms that shape the broader environment to the particular communicative interactions in which children experience language being used. In addition, we describe how children's developing sensory-motor, perceptual, and social-cognitive capacities respond to and are tuned by the surrounding environment. Closer integration of research on the mechanisms of language learning with investigation of the contexts in which this learning takes place will provide critical insights into the process of language development.

Children from low-income families are more likely than their higher income peers to show delays in language and literacy skills, both at school entry and across the lifespan. Programs aimed at promoting language and literacy activities in the home, particularly programs that combine distribution of print materials with support and guidance for using them, have been effective in decreasing the word gap, leading to increased school readiness and early literacy. The current study examined the impact of such a program based in pediatric healthcare, Reach Out and Read (ROR), on parents’ use of community resources that also provide access to print—namely, the public library—in the context of a citywide initiative to link literacy resources for low-income families. Effects of both ROR and the library, both individually and combined, on parents’ literacy activities at home were then examined. Significant associations between receiving ROR, using the public library, and parent–child book sharing were found. Implications for intervention and policy are discussed.

Children in low- and middle-income countries (LMICs) are at risk for adverse early development and school readiness outcomes; low parent literacy is common and an important contributing factor. While prevention programs have sought to improve child outcomes by promoting positive parenting practices such as reading aloud, there has been limited evidence of whether such programs have impacts for parents with low literacy in LMICs. This study investigated: (1) whether parent literacy is associated with parent–child reading aloud interactions, the overall cognitive home environment, and child language and cognitive outcomes; and (2) whether impacts of a toddler/preschool reading aloud program differ depending on parent literacy among low-income families in northern Brazil. We performed a secondary analysis of a single-blind cluster randomized controlled trial (RCT) of a parenting intervention (Universidade do Bebe [UBB]) implemented in educational childcare centers in Boa Vista, Brazil. UBB consisted of: (1) a lending library in which children’s books were borrowed weekly; and (2) monthly parent workshops focused on reading aloud. Control families received usual care without the reading aloud program. Five hundred and six mother-infant dyads (279 intervention; 287 control) were enrolled at mean (SD) child age 37.4 (6.5) months. Parents and children were evaluated at baseline/enrollment and approximately 6–9 months later. Measures: (1) Predictor/moderator: parent literacy; (2) Parent outcomes: parent–child interaction during shared storybook reading; cognitive home environment; (3) Child outcomes: expressive and receptive vocabulary; IQ; working memory and short-term phonological memory. Analyses accounted for baseline performance, sociodemographics, and clustering within centers and sites. Parent literacy was positively associated with parent–child verbal/reading interactions and child developmental outcomes, supporting our first hypothesis, that low parent literacy would be associated with lower quantity and quality of parent–child reading and lower scores on assessments of child development. UBB had positive impacts on parent–child interaction, cognitive stimulation, IQ, and receptive vocabulary for both high and low literacy parents, contrary to our second hypothesis, that low parent literacy would be associated with reductions in impacts of the reading aloud intervention. The comparable impacts of a reading aloud program on parent–child verbal/reading interactions and child cognitive outcomes regardless of parent literacy level suggest that preventive interventions in LMICs should consider promotion of shared reading even for low-literacy families.

Feeding concerns are common in the first 2 years of life and typically reflect maternal perceptions occurring within the larger context of the parent-child relationship. We aimed to determine whether (1) maternal depressive systems predicted perceived picky eating, mediated by maternal negative perceptions; (2) receipt of the Video Interaction Project (VIP) parenting intervention impacted perceived picky eating through this pathway; and (3) perceived picky eating was associated with child growth or subsequent dietary patterns.We performed a partial longitudinal analysis of 187 low-income, predominantly Hispanic mother-child dyads enrolled in a randomized controlled trial of the VIP. Mother-infant dyads were enrolled postpartum in an urban public hospital. Participants randomized to the VIP met with an interventionist on days of well-child visits; sessions were designed to facilitate interactions in play and shared reading through provision of learning materials and review of videotaped parent-child interaction; the curriculum did not contain feeding-specific elements. We used structural equation modeling to determine direct, indirect, and total effects of maternal depressive symptoms, maternal negative perceptions, and the VIP on perceived picky eating. We then tested associations between perceived picky eating and (1) child growth, using multivariable linear regression and multilevel modeling; and (2) subsequent child dietary consumption, using multivariable multinomial logistic regression.Maternal depressive symptoms had significant total effects on negative maternal perceptions (β = 0.32, p0.001) and perceived picky eating (β = 0.21, p0.01) after controlling for potential confounders. This effect was partially mediated by maternal negative perceptions (indirect effect: β = 0.06, p = 0.04). When used in the model as the predictor, the VIP had a significant total effect on perceived picky eating (β = -0.16, p = 0.02), which was partially mediated by maternal depressive symptoms and negative perceptions (indirect effect: β = -0.05, p = 0.02). Perceived picky eating was not associated with child diet at age 2 years or adiposity from 6 months to 3 years.Maternal concerns about picky eating may reflect deeper depressive symptoms and negative perceptions of her child's behavior. Interventions designed to facilitate positive parenting in general may lessen feeding-specific concerns, such as picky eating. Although reassurance about growth and nutritional outcomes for children perceived as picky eaters is appropriate, clinicians should also consider probing for underlying symptoms of depression that could lead to eating concerns.

Purpose The language that children hear early in life is associated with their speech-language outcomes. This line of research relies on naturalistic observations of children's language input, often captured with daylong audio recordings. However, the large quantity of data that daylong recordings generate requires novel analytical tools to feasibly parse thousands of hours of naturalistic speech. This study outlines a new approach to efficiently process and sample from daylong audio recordings made in two bilingual communities, Spanish–English in the United States and Quechua–Spanish in Bolivia, to derive estimates of children's language exposure. Method We employed a general sampling with replacement technique to efficiently estimate two key elements of children's early language environments: (a) proportion of child-directed speech (CDS) and (b) dual language exposure. Proportions estimated from random sampling of 30-s segments were compared to those from annotations over the entire daylong recording (every other segment), as well as parental report of dual language exposure. Results Results showed that approximately 49 min from each recording or just 7% of the overall recording was required to reach a stable proportion of CDS and bilingual exposure. In both speech communities, strong correlations were found between bilingual language estimates made using random sampling and all-day annotation techniques. A strong association was additionally found for CDS estimates in the United States, but this was weaker at the Bolivian site, where CDS was less frequent. Dual language estimates from the audio recordings did not correspond well to estimates derived from parental report collected months apart. Conclusions Daylong recordings offer tremendous insight into children's daily language experiences, but they will not become widely used in developmental research until data processing and annotation time substantially decrease. We show that annotation based on random sampling is a promising approach to efficiently estimate ambient characteristics from daylong recordings that cannot currently be estimated via automated methods.

We sought to determine whether pediatric primary care interventions targeting positive parenting among low socioeconomic status mothers resulted in reduced referrals to the New York City Early Intervention Program (NYC-EIP). Participants in Building Blocks (BB) and the Video Interaction Project (VIP) were linked with the NYC-EIP administrative dataset to determine referrals. In all, 139 of 422 study participants (31.4%) meeting inclusion criteria were referred to the NYC-EIP. Although referrals did not differ overall by group (VIP 29.8%; BB 33.8%; control 35.3%), differences were found for mothers with education/literacy of seventh grade or higher (interaction p = .02). In that subgroup, VIP was associated with reduced referrals by age 3 years (22.4%; adjusted odds ratio 0.53; 95% confidence interval [0.29, 0.97]), compared with BB (35.0%) and controls (34.3%), with survival analysis showing reduced cumulative risk ( p = .04). We conclude that VIP resulted in reduced referrals for early intervention evaluation among children of mothers with seventh-grade education or higher.

BACKGROUND AND OBJECTIVES: Heterogeneity in risk among low-income families suggests the need for tiered interventions to prevent disparities in school readiness. Smart Beginnings (SB) integrates two interventions: Video Interaction Project (VIP) (birth to 3 years), delivered universally to low-income families in pediatric primary care, and Family Check-Up (6 months to 3 years), targeted home visiting for families with additional family risks. Our objective was to assess initial SB impacts on parent-child activities and interactions at 6 months, reflecting early VIP exposure. METHODS: Two-site randomized controlled trial in New York City (84% Latinx) and Pittsburgh (81% Black), with postpartum enrollment and random assignment to treatment (SB) or control. At 6 months, we assessed parent-child interactions through surveys (StimQ, Parenting Your Baby) and observation (video-recorded play, coded by using Parent-Child Interaction Rating Scales – Infant Adaptation). RESULTS: A total of 403 families were enrolled at child’s birth (201 treatment) with 362 (89.8%) assessed at 6 months. Treatment families had increased StimQ, including total score (Cohen’s d = 0.28; P < .001) and domains reflecting reading (d = 0.23; P = .02) and teaching (d = 0.25; P = .01), and Parent-Child Interaction Rating Scales – Infant Adaptation, including a cognitive stimulation factor (d = 0.40; P < .001) and domains reflecting support for cognitive development (d = 0.36; P < .001), and language quantity (0.40; P < .001) and quality (d = 0.37; P < .001). Thus, significant effects emerged across a broad sample by using varied methodologies. CONCLUSIONS: Findings replicate and extend previous VIP findings across samples and assessment methodologies. Examining subsequent assessments will determine impacts and feasibility of the full SB model, including potential additive impacts of Family Check-Up for families at elevated risk.

Background: Although one fifth of US households speak a language other than English, erroneous information and myths about bilingualism abound. Parents often turn to their pediatricians with questions such as: Does learning two languages cause delays? Should I speak to my child in English? Nevertheless, pediatricians get little training on bilingualism and many don’t feel comfortable advising families on this topic. [Figure: see text] OBJECTIVES: Poor early childhood development in low- and middle-income countries is a major public health problem. Efficacy trials have shown the potential of early childhood development interventions but scaling up is costly and challenging. Guidance on effective interventions’ delivery is needed. In an open-label cluster-randomized control trial, we compared the effectiveness of weekly home visits and weekly mother-child group sessions. Both included nutritional education, whose effectiveness was tested separately. METHODS: In Odisha, India, 192 villages were randomly assigned to control, nutritional education, nutritional education and home visiting, or nutritional education and group sessions. Mothers with children aged 7 to 16 months were enrolled (n = 1449). Trained local women ran the two-year interventions, which comprised demonstrations and interactions and targeted improved play and nutrition. Primary outcomes, measured at baseline, midline (12 months), and endline (24 months), were child cognition, language, motor development, growth and morbidity. RESULTS: Home visiting and group sessions had similar positive average (intention-to-treat) impacts on cognition (home visiting: 0.324 SD, 95% confidence interval [CI]: 0.152 to 0.496, P = .001; group sessions: 0.281 SD, 95% CI: 0.100 to 0.463, P = .007) and language (home visiting: 0.239 SD, 95% CI: 0.072 to 0.407, P = .009; group sessions: 0.302 SD, 95% CI: 0.136 to 0.468, P = .001). Most benefits occurred in the first year. Nutrition-education had no benefit. There were no consistent effects on any other primary outcomes. CONCLUSIONS: Group sessions cost $38 per child per year and were as effective on average as home visiting, which cost $135, implying an increase by a factor of 3.5 in the returns to investment with group sessions, offering a more scalable model. Impacts materialize in the first year, having important design implications.

Recent advances in large-scale data storage and processing offer unprecedented opportunities for behavioral scientists to collect and analyze naturalistic data, including from under-represented groups. Audio data, particularly real-world audio recordings, are of particular interest to behavioral scientists because they provide high-fidelity access to subtle aspects of daily life and social interactions. However, these methodological advances pose novel risks to research participants and communities. In this article, we outline the benefits and challenges associated with collecting, analyzing, and sharing multi-hour audio recording data. Guided by the principles of autonomy, privacy, beneficence, and justice, we propose a set of ethical guidelines for the use of longform audio recordings in behavioral research. This article is also accompanied by an Open Science Framework Ethics Repository that includes informed consent resources such as frequent participant concerns and sample consent forms.

Laboratory observations are a mainstay of language development research, but transcription is costly. We test whether speech recognition technology originally designed for day-long contexts can be usefully applied to this use-case. We compared automated adult word and child vocalization counts from Language Environment Analysis (LENATM) to those of transcribers in 20-minute play sessions with Spanish-speaking dyads (n = 104) at 1;7 and 2;2. For adult words, results indicated moderate associations but large absolute differences. Associations for child vocalizations were weaker with larger absolute discrepancies. LENA has moderate potential to ease the burden of transcription in some research and clinical applications.

Poverty related disparities in early child development and school readiness are a major public health crisis, the prevention of which has emerged in recent years as a national priority. Interventions targeting parenting and the quality of the early home language environment are at the forefront of efforts to address these disparities. In this article we discuss the innovative use of the pediatric primary care platform as part of a comprehensive public health strategy to prevent adverse child development outcomes through the promotion of parenting. Models of interventions in the pediatric primary care setting are discussed with evidence of effectiveness reviewed. Taken together, a review of this significant body of work shows the tremendous potential to deliver evidence-based preventive interventions to families at risk for poverty related disparities in child development and school readiness at the time of pediatric primary care visits. We also addresss considerations related to scaling and maximizing the effect of pediatric primary care parenting interventions and provide key policy recommendations.

OBJECTIVES: To determine impacts on social-emotional development at school entry of a pediatric primary care intervention (Video Interaction Project [VIP]) promoting positive parenting through reading aloud and play, delivered in 2 phases: infant through toddler (VIP birth to 3 years [VIP 0–3]) and preschool-age (VIP 3 to 5 years [VIP 3–5]). METHODS: Factorial randomized controlled trial with postpartum enrollment and random assignment to VIP 0-3, control 0 to 3 years, and a third group without school entry follow-up (Building Blocks) and 3-year second random assignment of VIP 0-3 and control 0 to 3 years to VIP 3-5 or control 3 to 5 years. In the VIP, a bilingual facilitator video recorded the parent and child reading and/or playing using provided learning materials and reviewed videos to reinforce positive interactions. Social-emotional development at 4.5 years was assessed by parent-report Behavior Assessment System for Children, Second Edition (Social Skills, Attention Problems, Hyperactivity, Aggression, Externalizing Problems). RESULTS: VIP 0-3 and VIP 3-5 were independently associated with improved 4.5-year Behavior Assessment System for Children, Second Edition T-scores, with effect sizes (Cohen’s d) ∼−0.25 to −0.30. Receipt of combined VIP 0-3 and VIP 3-5 was associated with d = −0.63 reduction in Hyperactivity (P = .001). VIP 0-3 resulted in reduced “Clinically Significant” Hyperactivity (relative risk reduction for overall sample: 69.2%; P = .03; relative risk reduction for increased psychosocial risk: 100%; P = .006). Multilevel models revealed significant VIP 0-3 linear effects and age × VIP 3-5 interactions. CONCLUSIONS: Phase VIP 0-3 resulted in sustained impacts on behavior problems 1.5 years after program completion. VIP 3-5 had additional, independent impacts. With our findings, we support the use of pediatric primary care to promote reading aloud and play from birth to 5 years, and the potential for such programs to enhance social-emotional development.

Parental engagement is critical to children’s educational achievement. Before and during elementary school, it is crucial for parents to be involved in their children’s education in order to foster development and achievement. Hispanic parents’ immigrant status, coupled with a lack of English proficiency, means that they often find themselves of low socioeconomic status (SES). Being low SES also means that parents possess fewer resources for engaging with their children. The current study seeks to understand low-income, primarily Hispanic mothers’ perceptions of their roles in their first grade children’s education. Mothers were interviewed regarding parenting confidence related to teaching their children, and responses were analyzed using qualitative research methods. Mothers in this study associated their roles in their children’s education with two primary areas: helping their children to learn, and raising their children to be well-behaved and respectful. The main barrier to parental confidence in these roles appeared to be mothers’ lack of English proficiency. This is consistent with previous research demonstrating that Hispanic parents maintain the perception of a lack of proficiency in English as a significant barrier to parental involvement in their children’s education in the United States. Future interventions with teachers and parents may benefit from these findings in consideration of the optimal ways to involve parent related to their perceived personal strengths regarding parental engagement.

Fifty years of research have documented a sobering reality: There are substantial differences among parents in how they engage and communicate with their children, and these differences impact the development of a child’s language and cognitive skills. Studies initiated during the War on Poverty first explored how parents’ verbal engagement with young children varied among families differing in education and income, or socioeconomic status (SES) [e.g. Bee, Van Egeren, Pytkowicz Streissguth, Nyman, & Leckie, 1969; Hess & Shipman, 1965; Schachter, 1979]. In their 1995 monograph Meaningful Differences in the Everyday Experience of Young American Children , Betty Hart and Todd Risley were the first to document huge disparities in the sheer amount of language that caregivers in different families directed to young children. Although they found substantial variability in child-directed speech within as well as between SES groups, the differences between children in advantaged and disadvantaged families were surprisingly large. They also found that those children who did not have the benefits of rich verbal engagement early in life were more likely to be behind in cognitive and language skills in kindergarten and elementary school. Hart and Risley’s [1995] discovery of a 30-million-word gap in language to children from higherand lower-SES backgrounds over the first three years of life is now widely cited in the popular press as well as in academic journals. But for more than a decade, this powerful study was essentially ignored. In the 1960s, claims that some learning difficulties in children from disadvantaged families could be linked to inadequate cognitive stimulation at home came to be known as the “cultural deficit” model [Riessman, 1962]. A fierce backlash emerged in the 1970s, rejecting this view as unsubstantiated by scientific evidence and as deeply disrespectful of minority parents in poverty whose use of language with children was grounded in cultural traditions of parenting different from those in more affluent mainstream families [Fernald & Weisleder, 2011]. Consistent with these criticisms, a dominant view in the field of language acquisition through the 1990s was that focusing on SES differences in speech

Different kids grow up in different environments. Imagine Kid A: she gets to play and talk with her parents a lot, she has a yummy dinner every night with her family, she gets to travel on airplanes, and she has lots of books in her bedroom. Now imagine Kid B: his parents are really busy, he does not play or talk with grownups very much, he watches TV a lot, he does not get to eat much healthy food, and he almost always stays in his house. Kid A gets a lot of chances to have fun and learn, and Kid B gets fewer chances. Kid A gets to read about adventures all over the world, and Kid B does not get as much practice with reading. Did you get a lot of chances to play, talk, and learn when you were little? Have you ever thought about how other kids’ lives were different from yours? Have you ever thought about all the complicated stuff you learned, like language? There are lots of scientists who study how babies and little kids learn, and we want to tell you about some important research findings. After you finish reading, we hope you feel excited about how this science could help give every kid a fair chance to learn.

We studied associations between 2 pediatric primary care interventions promoting parental responsiveness and maternal depressive symptoms among low-income mothers. This randomized controlled trial included 2 interventions (Video Interaction Project [VIP], Building Blocks [BB]) and a control group. VIP is a relationship-based intervention, using video-recordings of mother–child dyads to reinforce interactional strengths. BB communicates with parents via parenting newsletters, learning materials, and questionnaires. At mean (SD) child age 6.9 (1.2) months, depressive symptoms were assessed with the Patient Health Questionnaire-9 (PHQ-9), parental responsiveness was assessed with StimQ-I. A total of 407 dyads were assessed. Rates of mild depressive symptoms were lower for VIP (20.6%) and BB (21.1%) than Controls (32.1%, P = .04). Moderate depressive symptoms were lower for VIP (4.0%) compared to Controls (9.7%, P = .031). Mean PHQ-9 scores differed across 3 groups ( F = 3.8, P = .02): VIP mothers scored lower than controls ( P = .02 by Tukey HSD). Parent–child interactions partially mediated VIP-associated reductions in depressive symptoms (indirect effect −.17, 95% confidence interval −.36, −.03).

Objective To investigate pathways by which interventions that promote shared reading and play help prevent child behavior problems. We examined whether family processes associated with the family investment pathway (eg, parental cognitive stimulation) and the family stress pathway (eg, mothers' psychosocial functioning) mediated impacts of a pediatric-based preventive intervention on child behavior. Study design The sample included 362 low-income mothers and their children who participated in a randomized controlled trial of the Video Interaction Project, a pediatrics-based preventive intervention that promotes parent-child interactions in the context of shared reading and play. Parent-child dyads were randomly assigned to group at birth. Three mediators—parental cognitive stimulation, maternal stress about the parent-child relationship, and maternal depressive symptoms—were assessed at child ages 6 and 36 months. The outcome, child externalizing behaviors, was assessed at 36 months. We used a series of path analytic models to examine how these family processes, separately or together, mediated the impacts of the Video Interaction Project on child behavioral outcomes. Results Intervention impacts on child behavior were mediated by enhancements in cognitive stimulation and by improvements in mothers' psychosocial functioning. A sequential mediation model showed that Video Interaction Project impacts on cognitive stimulation at 6 months were associated with later decreases in mothers' stress about the parent-child relationship and that this pathway mediated intervention impacts on child behavioral outcomes at 3 years of age (P = .023). Conclusions Using an experimental design, this study identifies pathways by which parent-child interactions in shared reading and play can improve child behavioral outcomes. Objective To determine whether educational media as actually used by low-income families promote parent–child cognitive stimulation activities. Methods We performed secondary analysis of the control group of a longitudinal cohort of mother–infant dyads enrolled postpartum in an urban public hospital. Educational media exposure (via a 24-hour recall diary) and parent–child activities that may promote cognitive stimulation in the home (using StimQ) were assessed at 6, 14, 24, and 36 months. Results Data from 149 mother–child dyads, 93.3% Latino, were analyzed. Mean (standard deviation) educational media exposure at 6, 14, 24, and 36 months was, respectively, 25 (40), 42 (58), 39 (49), and 39 (50) minutes per day. In multilevel model analyses, prior educational media exposure had small positive relationship with subsequent total StimQ scores (β = 0.11, P = .03) but was nonsignificant (β = 0.08, P = .09) after adjusting for confounders (child: age, gender, birth order, noneducational media exposure, language; mother: age, ethnicity, marital status, country of origin, language, depressive symptoms). Educational media did predict small increases in verbal interactions and toy provision (adjusted models, respectively: β = 0.13, P = .02; β = 0.11; P = .03). In contrast, more consistent relationships were seen for models of the relationship between prior StimQ (total, verbal interactions and teaching; adjusted models, respectively: β = 0.20, P = .002; β = 0.15, P = .006; β = 0.20, P = .001) and predicted subsequent educational media. Conclusions Educational media as used by this sample of low-income families does not promote cognitive stimulation activities important for early child development or activities such as reading and teaching.

Objective To determine the early impacts of pediatric primary care parenting interventions on parent cognitive stimulation in low socioeconomic status families and whether these impacts are sustained up to 1.5 years after program completion. Study design This randomized controlled trial included assignment to 1 of 2 interventions (Video Interaction Project [VIP] or Building Blocks) or to a control group. Mother–newborn dyads were enrolled postpartum in an urban public hospital. In VIP, dyads met with an interventionist on days of well-child visits; the interventionist facilitated interactions in play and shared reading through provision of learning materials and review of videotaped parent–child interactions. In Building Blocks, parents were mailed parenting pamphlets and learning materials. We compare the trajectories of cognitive stimulation for parents in VIP and control from 6 to 54 months. Results There were 546 families that contributed data. VIP was associated with enhanced reading, parent verbal responsivity, and overall stimulation at all assessment points, with analyses demonstrating a 0.38 standard deviation increase in cognitive stimulation overall. Trajectory models indicated long-term persistence of VIP impacts on reading, teaching, and verbal responsivity. Conclusions VIP is associated with sustained enhancements in cognitive stimulation in the home 1.5 years after completion of the program and support expansion of pediatric interventions to enhance developmental trajectories of children of low socioeconomic status. OBJECTIVES: Many children in low- and middle-income countries fail to reach their developmental potential. We sought to determine if a parenting program focused on the promotion of reading aloud enhanced parent-child interactions and child development among low-income families in northern Brazil. METHODS: This was a cluster-randomized study of educational child care centers randomly assigned to receive an additional parenting program (intervention) or standard child care without a parenting component (control). Parent-child dyads were enrolled at the beginning of the school year and were assessed at enrollment and at the end of the school year. Families in intervention centers could borrow children’s books on a weekly basis and could participate in monthly parent workshops focused on reading aloud. We compared parents and children in intervention and control centers 9 months after the start of the intervention on measures of parent-child interaction and child language, cognitive, and social-emotional development. RESULTS: Five hundred and sixty-six parent-child dyads (279 intervention; 287 control) in 12 child care clusters (26–76 children per cluster) were assessed at enrollment; 464 (86%) contributed follow-up data. Parents in the intervention group engaged in significantly greater cognitive stimulation (Cohen’s d = 0.43) and higher quantity and quality of reading interactions (d = 0.52–0.57) than controls; children in the intervention scored significantly higher than controls on receptive vocabulary (d = 0.33), working memory (d = 0.46), and IQ (d = 0.33). CONCLUSIONS: An innovative program focused on the promotion of parent-child reading aloud resulted in benefits to parent-child interactions and to child language and cognitive development that were greater than those provided by educational child care alone. This promising approach merits further evaluation at scale.

Recent analyses have revealed that child-directed speech contains distributional regularities that could, in principle, support young children's discovery of distinct grammatical categories (noun, verb, adjective). In particular, a distributional unit known as the frequent frame appears to be especially informative (Mintz, 2003). However, analyses have focused almost exclusively on the distributional information available in English. Because languages differ considerably in how the grammatical forms are marked within utterances, the scarcity of cross-linguistic evidence represents an unfortunate gap. We therefore advance the developmental evidence by analyzing the distributional information available in frequent frames across two languages (Spanish and English), across sentence positions (phrase medial and phrase final), and across grammatical forms (noun, verb, adjective). We selected six parent–child corpora from the CHILDES database (three English; three Spanish), and analyzed the input when children were aged 2 ; 6 or younger. In each language, frequent frames did indeed offer systematic cues to grammatical category assignment. We also identify differences in the accuracy of these frames across languages, sentences positions and grammatical classes.

OBJECTIVE: The goal of this study was to determine what effects pediatric primary care interventions, focused on promotion of positive parenting through reading aloud and play, have on the socioemotional development of toddlers from low-income, primarily immigrant households. METHODS: This randomized controlled trial included random assignment to 1 of 2 interventions (Video Interaction Project [VIP] or Building Blocks [BB]) or to a control group. Mother–newborn dyads were enrolled postpartum in an urban public hospital. In VIP, dyads met with an interventionist on days of well-child visits; the interventionist facilitated interactions in play and shared reading through provision of learning materials and review of videotaped parent–child interactions. In BB, parents were mailed parenting pamphlets and learning materials. This article analyzes socioemotional outcomes from 14 to 36 months for children in VIP and BB versus control. RESULTS: A total of 463 dyads (69%) contributed data. Children in VIP scored higher than control on imitation/play and attention, and lower on separation distress, hyperactivity, and externalizing problems, with effect sizes ∼0.25 SD for the sample as a whole and ∼0.50 SD for families with additional psychosocial risks . Children in BB made greater gains in imitation/play compared with control. CONCLUSIONS: These findings support the efficacy of VIP, a preventive intervention targeting parent–child interactions, for enhancing socioemotional outcomes in low-income toddlers. Given the low cost and potential for scalability of primary care interventions, findings support expansion of pediatric-based parenting programs such as VIP for the primary prevention of socioemotional problems before school entry.

As part of a large randomized controlled trial, the authors assessed the impact of 2 early primary care parenting interventions-the Video Interaction Project (VIP) and Building Blocks (BB)-on the use of physical punishment among low-income parents of toddlers. They also determined whether the impact was mediated through increases in responsive parenting and decreases in maternal psychosocial risk.Four hundred thirty-eight mother-child dyads (161 VIP, 113 BB, 164 Control) were assessed when the children were 14 and/or 24 months old. Mothers were asked about their use of physical punishment and their responsive parenting behaviors, depressive symptoms, and parenting stress.The VIP was associated with lower physical punishment scores at 24 months, as compared to BB and controls. In addition, fewer VIP parents reported ever using physical punishment as a disciplinary strategy. Significant indirect effects were found for both responsive parenting and maternal psychosocial risk, indicating that the VIP affects these behaviors and risk factors, and that this is an important pathway through which the VIP affects the parents' use of physical punishment.The results support the efficacy of the VIP and the role of pediatric primary care, in reducing the use of physical punishment among low-income families by enhancing parent-child relationships. In this way, the findings support the potential of the VIP to improve developmental outcomes for at-risk children.

We sought to determine impacts of a pediatric primary care intervention, the Video Interaction Project, on 3-year trajectories of parenting stress related to parent-child interactions in low socioeconomic status (SES) families. A randomized controlled trial (RCT) was conducted, with random assignment to one of two interventions (Video Interaction Project [VIP]; Building Blocks [BB]) or control (C). As part of VIP, dyads attended one-on-one sessions with an interventionist who facilitated interactions in play and shared reading through review of videotaped parent-child interactions made on primary care visit days; learning materials and parenting pamphlets were also provided to facilitate parent-child interactions at home. Parenting stress related to parent-child interactions was assessed for VIP and Control groups at 6, 14, 24, and 36 months using the Parent-Child Dysfunctional Interaction subscale of the Parenting Stress Index- Short Form, with 378 dyads (84%) assessed at least once. Group differences emerged at 6 months with VIP associated with lower parenting stress at 3 of 4 ages considered cross-sectionally and an 17.7% reduction in parenting stress overall during the study period based on multi-level modeling. No age by group interaction was observed, indicating persistence of early VIP impacts. Results indicated that VIP, a preventive intervention targeting parent-child interactions, is associated with decreased parenting stress. Results therefore support the expansion of pediatric interventions such as VIP as part of a broad public health strategy to address poverty-related disparities in school-readiness.

At its best, the field of bilingualism is a dynamic discipline informed by diverse perspectives. Carroll's critical review – situated within a linguistics framework – contends that current research on bilingual exposure and language development suffers from a lack of theoretical clarity, and makes little contribution to our understanding of bilingual acquisition. Carroll's push (Carroll) towards greater precision in our thinking about the relation between input and outcomes is an important and welcome challenge. However, it is also critical to keep in mind the social context that motivates much of the current research on bilingual development, and to leave room for studies whose main goal is to provide answers to societally important questions about bilingual children's health and development.

Infants differ substantially in their rates of language growth, and slow growth predicts later academic difficulties. In this study, we explored how the amount of speech directed to infants in Spanish-speaking families low in socioeconomic status influenced the development of children’s skill in real-time language processing and vocabulary learning. All-day recordings of parent-infant interactions at home revealed striking variability among families in how much speech caregivers addressed to their child. Infants who experienced more child-directed speech became more efficient in processing familiar words in real time and had larger expressive vocabularies by the age of 24 months, although speech simply overheard by the child was unrelated to vocabulary outcomes. Mediation analyses showed that the effect of child-directed speech on expressive vocabulary was explained by infants’ language-processing efficiency, which suggests that richer language experience strengthens processing skills that facilitate language growth.

This research revealed both similarities and striking differences in early language proficiency among infants from a broad range of advantaged and disadvantaged families. English-learning infants (n = 48) were followed longitudinally from 18 to 24 months, using real-time measures of spoken language processing. The first goal was to track developmental changes in processing efficiency in relation to vocabulary learning in this diverse sample. The second goal was to examine differences in these crucial aspects of early language development in relation to family socioeconomic status (SES). The most important findings were that significant disparities in vocabulary and language processing efficiency were already evident at 18 months between infants from higher- and lower-SES families, and by 24 months there was a six-month gap between SES groups in processing skills critical to language development.
