The study cohort comprises adolescents and young adults who underwent prenatal care at a reference maternity hospital between January and December 2021. Individual procedure costs were extracted from hospital records and converted into monetary values using the Brazilian Federal Fee. The sample size for each group was determined based on preliminary data collected during a pilot study. Descriptive analyses presented costs stratified by age groups. To estimate budgetary impacts and sensitivity we used national live birth incidence rates.The mean cost per case of adolescent pregnancy was US$ 704.92 (SD: 1,707.74) and for adult pregnancy was US$ 592.40 (SD: 941.43). The cost of hospital admission accounted for 61% of the total cost of adolescent pregnancies. The annual budget impact of pregnancies among women under 30 years was estimated at US$ 1 billion, with 239 million coming from women under 20 years old.Pregnancy in adolescence may lead to a significant annual budget impact, representing 24% of costs attributed to pregnancies among women under 30 years. Costs related to hospital admissions and complications represent a substantial share of the total costs of adolescent pregnancy.Rates of early pregnancy have significant social and economic consequences that extend beyond the individual sphere, impacting public/governmental budgets. Fetal anomalies among young women and adolescents pose major public health concerns. This systematic review aims to elucidate the relationship between young maternal age and the incidence of fetal abnormalities. According to the systematic review and meta-analysis PRISMA protocol, cohort, cross-sectional and case–control studies were scrutinized to include 80,393,450 participants across diverse regions. The meta-analysis utilized Odds Ratios (OR) as the effect measure, adopting a random-effects model. The screening process involved 157 selected and verified manuscripts, which ultimately resulted in the final inclusion of 20 studied in the meta-analysis. The criterion for young maternal age was the age of ≤ 20 years. The meta-analysis revealed a pooled OR of 0.93 (95% CI: 0.82–1.05, p = 0.252), indicating no statistically significant association between young maternal age (≤ 20 years) and fetal anomalies. However, considerable heterogeneity (I² = 96.21%) was noted, prompting the use of a random-effects model to derive the reported results. The meta-analysis did not find statistically significant differences in the occurrence of congenital anomalies in fetuses of younger women than in overall population. Although due to significant heterogeneity of the analyzed studies, and a risk of bias, caution should be exercised when interpreting the results, further investigation may be warranted to understand the relationship between maternal age and risk of fetal anomalies. Nevertheless, the study shows significant differences, which diminish in collective analysis, suggests that factors beyond age may be influential. Abstract Background Integration of maternal mental health into primary health care is considered a strategy to improve access to mental health support in low- and middle-income countries (LMICs). Health care workers’ (HCWs) and traditional practitioners’ (traditional healers, herbalists, traditional birth attendants, spiritual healers, prophets, and community health volunteers) perceptions of the availability and management of adolescent mothers’ maternal mental health care were explored in rural Malawi. Recognizing and identifying the barriers associated with access to maternal mental health support is essential to improving the mental health of adolescent mothers. Methods A descriptive qualitative design (DQ) was used to explore HCWs’ and traditional practitioners’ perceptions of maternal mental health services for adolescent mothers. In-depth interviews were conducted with HCWs (n = 6), and three focus group discussions were conducted with 30 community-based traditional practitioners. Participants were purposefully recruited because they provide formal or informal health services to adolescent mothers during the postnatal period at Mitundu Rural Hospital and its catchment area in Lilongwe District, in Malawi. Interviews were analyzed using reflective thematic analysis and inductive thinking. Results Thematic analysis found four themes to describe access to maternal mental health care for adolescent mothers. Participant perceptions were themed around health system challenges and how cultural background and beliefs influence access to mental health care and support. The themes were: (1) Inadequate staff development; (2) Limited resources (medication and infrastructure); (3) Limited policy and guidelines implementation; and (4) Cultural background and belief influence on help-seeking. HCWs suggested improving pre-service and in-service training to improve mental health assessment of mothers, while traditional practitioners wanted to increase their awareness of mental health issues. Conclusion Participants emphasized that multifaceted factors influenced access to mental health support. These factors affect the assessment, treatment, and support of adolescent mothers and help-seeking by mothers. Therefore, strengthening the healthcare system and empowering providers with the knowledge and skills to recognize at-risk mothers and provide timely support is essential.

Abstract Background Worldwide, a significant number of girls become mothers during adolescence. In Bangladesh, adolescent childbirth is highly prevalent and has adverse effects on children’s health and undernutrition. We aimed to identify the relationship between the undernutrition of children and adolescent motherhood, the factors associated with adolescent mothers’ age at first birth, and to examine the programmatic factors and gaps influencing children’s undernutrition in Bangladesh. Methods We analysed the ‘Bangladesh Demographic and Health Survey’ BDHS-17-18 data and desk review. To examine the factors associated with adolescent motherhood and its impact on child undernutrition, data from 7,643 mother-child pairs were selected. Child stunting, wasting, and underweight were measured according to the World Health Organisation (WHO) median growth guidelines based on z-scores − 2. Univariate, bivariate, simple, and multiple logistic regressions were used for analyse. We followed the systematic procedures for the literature review. Results Approximately, 89% of adolescents aged ≤ 19 years were married and 71% of them gave their first childbirth. Children of adolescent mothers (≤ 19 years) were significantly 1.68 times more wasted (aOR: 1.68; 95% CI: 1.08 to 2.64), 1.37 times more underweight (aOR: 1.37; 95% CI: 1.01 to 1.86) and either form 1.32 times more stunting, wasting or underweight (aOR:1.32; 95% Cl: 1.05 to 1.66) compared to the children of adult mothers (> 19 years) after adjusting potential confounders. The factors associated with mothers’ first childbirth during adolescence were the age gap between husband and wife 5–10 years (aOR: 1.81; 95% Cl: 1.57–2.10) and age gap > 10 years (aOR: 2.41; 95% Cl: 1.96–2.97) compared with the age group

Abstract Background The postnatal period is an important time for adolescent mothers to regain their health as they adjust to life with their infants. However, it is also a time when mothers are vulnerable to mental health problems. Generalized anxiety disorders (GADs) are among the common mental disorders that can impact mothers. Anxiety disorders can have adverse effects on a child’s cognitive development. However, there is a scarcity of studies pertaining to anxiety disorders among adolescent mothers in Malawi. Methods A cross-sectional survey was conducted among adolescent postnatal mothers aged ≤ 19 years to establish the prevalence of probable GAD and identify cultural and social factors that influence anxiety in this population. Adolescent mothers were recruited from the Mitundu Rural Hospital catchment area in Lilongwe district, Malawi. A two-stage random sampling method was employed: clinics were randomly selected, and participants were recruited via systematic random sampling. The Generalized Anxiety Disorder (GAD-7) scale was used to assess anxiety. Data were analyzed using SPSS version 27. Results Of the 395 adolescent mothers who participated, 34% were aged 14–17. Abstract Introduction In Rwanda, maternal community health workers play a critical role to improving maternal, newborn and child health, but little is known about their specific experiences with adolescent mothers, who face unique challenges, including trauma, ongoing violence, stigma, ostracism, mental health issues, barriers within the healthcare system, and lack of access to the social determinants of health. This study explored the experiences of maternal community health workers when caring for adolescent mothers in Rwanda to inform the delivery of trauma- and violence-informed care in community maternal services. Methods Interpretive Description methodology was used to understand the experiences of 12 community health workers purposively recruited for interviews due to their management roles. To gain additional insights about the context, seven key informants were also interviewed. Findings Maternal community health workers provided personalized support to adolescent mothers through the provision of continuity of care, acting as a liaison, engaging relationally and tailoring home visits. They reported feeling passionate about their work, supporting each other, and receiving support from their leaders as facilitators in caring for adolescent mothers. Challenges in their work included handling disclosures of violence, dealing with adolescent mothers’ financial constraints, difficulties accessing these young mothers, and transportation issues. Adolescent mothers’ circumstances are generally difficult, leading to self-reports of vicarious trauma among this sample of workers. Conclusion Maternal community health workers play a key role in addressing the complex needs of adolescent mothers in Rwanda. However, they face individual and structural challenges highlighting the complexities of their work. To sustain and enhance their roles, it is imperative for government and other stakeholders to invest in resources, mentorship, and support. Additionally, training in equity-oriented approaches, particularly trauma- and violence-informed care, is essential to ensure safe and effective care for adolescent mothers and to mitigate vicarious trauma among maternal community health workers.

Experiencing intimate partner violence (IPV) increased the likelihood of probable GAD (aOR 4.80, 95% CI: 1.23–18.82, p = 0.02), while those who had contact with a health worker postnatally (aOR 0.38, 95% CI: 0.17–0.83, p = 0.02) and those who were "not prayerful" (aOR, 0.43, 95% CI: 0.21–0.87, p = 0.02) were less likely to report probable GAD. Conclusions: Given that the prevalence of probable GAD among adolescent mothers in Malawi is higher compared to the global estimates of their peers, policies and guidelines that prioritize the maternal mental health of adolescent mothers in Malawi are required. The findings also highlight the importance of enhancing social support among family and community. Adolescent mothers face numerous challenges while breastfeeding. This study aimed to assess the breastfeeding needs of adolescent mothers. For this systematic review, Web of Science, PubMed, Scopus, Cochrane Library, SID and Magiran databases were searched. The initial search yielded 2,290 studies, of which 41 were included in this review. Adolescent mothers' breastfeeding requirements were grouped into 8 categories: (1) social support from healthcare providers, partners and families; (2) school support for breastfeeding mothers; (3) breastfeeding counselling based on cultural sensitivities; (4) educational assistance from health providers for adolescent mothers' families; (5) changing harmful cultural values and judgments about adolescent mothers' breastfeeding; (6) additional home or outpatient visits in the days following hospital discharge; (7) peer support and counselling; and (8) economic needs. To promote breastfeeding, policymakers and healthcare providers should devise specifically tailored programmes and interventions to cater to the specific requirements of adolescent mothers. Adolescent pregnancy can impact the educational attainment of adolescent mothers and language development of their children. However, support services and interventions can mitigate these risk factors. Adolescent mothers have shown success in implementing various language facilitation strategies (LFS) with their children when coached. We developed a triadic language intervention in a facilitated playgroup context to support adolescent mothers' use of language strategies when interacting with their children. The current pilot study utilized a pre-test, post-test descriptive approach to analyze differences in adolescent mothers' language use and parenting behaviors and their children's language use after participation in facilitated playgroups. All adolescent mothers demonstrated enhanced parenting practices. Changes in mothers' and children's language from pre- to post-test were variable across participants. While this pilot study shows promising evidence of the utility of a triadic intervention situated in playgroups with adolescent families, further research is required to address the limitations of this study. Background/Aims: Treatment and interventions for adolescent mothers during birth are often informed by research conducted with adult women. This approach does not consider the fact that adolescents are still in a developmental phase with age-specific needs and the consequent effect on their birth experiences. This review's aim was to explore the available literature in relation to the need for age-specific care for adolescents during birth. Methods: An aggregated qualitative review was conducted using narrative synthesis with comprehensive searches of EBSCOhost, Scopus, JSTOR, Pubmed, Science Direct and Cochrane. Thematic analysis and a narrative synthesis were used to present the results based on the research objectives. Results: A total of seven studies were included, published between 2002 and 2022. Eight elements of age-specific care for adolescent mothers were identified: be sensitive to her needs, manage her pain, treat her with respect, provide information, include her in decision making, guide her, have empathy for her needs and offer non-judgmental care. Conclusions: There is a dearth of research on the experiences and needs of adolescent mothers during birth. Birth impacts developmental tasks for adolescents and age-specific care is vital for more positive birth experiences. This study investigates trends and prevalence of Intimate Partner Violence (IPV) among out-of-wedlock adolescent mothers and their likelihood of being IPV victims later in a marriage. We address possible selection bias using a propensity score matching technique. The study uses the Kenyan DHS and finds that the prevalence of IPV (a composite measure of emotional, physical, and sexual violence) was 43%, but 28%, 12%, 34%, and 13% for emotional, severe physical, less-severe physical, and sexual IPV, respectively. Overall, out-of-wedlock adolescent mothers are associated with approximately 14% higher likelihood of IPV later in marriage than non-adolescent mothers. Policy and intervention plans for out-of-wedlock adolescent motherhood are clear strategies for abating IPV. This could be addressed by advocating for improved human capital among girls, laws to combat domestic violence and rape, and managing cultural practices like acceptance of "wife-beating." BackgroundMethodsResultsConclusionBreastfeeding has a positive impact on child and maternal health outcomes. Black and Latina women and adolescent mothers have lower rates of breastfeeding initiation and continuance in the U.S. Maternal depression and psychosocial stressors may contribute to reduced rates of breastfeeding. The current study aims to better understand behaviours and associated factors related to breastfeeding in a diverse group of adolescent mothers attending a teen-tot clinic for postpartum and infant well care.Participants were 191 mother-infant dyads. Mother’s age ranged from 13 to 25, and 54% of mothers identified as Latina, 22% Black, 11% more than one race and 5% white. Demographic information and breastfeeding behaviour were abstracted from the medical record. Rates of postpartum mood/anxiety symptoms and psychosocial stressors were obtained from screening measures completed at medical visits.Analyses revealed that 87% of adolescent mothers in the sample initiated breastfeeding at birth and the racial/ethnic breakdown of those mothers closely mirrored the overall population (58% Hispanic or Latina, 17% Black, 10% more than one race, 5% white). At 2 months postpartum, only 41% of the population was still breastfeeding. Mothers with significant mood/anxiety symptoms at the newborn visit were more likely to be breastfeeding at the 1- and 2-month visits. Mothers with psychosocial stressors at the newborn visit were less likely to be breastfeeding at the 1- and 2-month visits.Efforts to promote health equity through breastfeeding for at-risk mothers must occur within the first few weeks postpartum and must consider associated factors including postpartum mood/anxiety symptoms and psychosocial stressors. BACKGROUND AND OBJECTIVES: Although mortality rates are highest for infants of teens aged 15 to 19, no studies have examined the long-term trends by race and ethnicity, urbanicity, or maternal age. The objectives of this study were to examine trends and differences in mortality for infants of teens by race and ethnicity and urbanicity from 1996 to 2019 and estimate the contribution of changes in the maternal age distribution and maternal age-specific (infant) mortality rates (ASMRs) to differences in infant deaths in 1996 and 2019. METHODS: We used 1996 to 2019 period-linked birth and infant death data from the United States to assess biennial mortality rates per 1000 live births. Pairwise comparisons of rates were conducted using z test statistics and Joinpoint Regression was used to examine trends. Kitagawa decomposition analysis was used to estimate the proportion of change in infant deaths because of changes in the maternal age distribution and ASMRs. RESULTS: From 1996 to 2019, the mortality rate for infants of teens declined 16.7%, from 10.30 deaths per 1000 live births to 8.58. The decline was significant across racial and ethnic and urbanization subgroups; however, within rural counties, mortality rates did not change significantly for infants of Black or Hispanic teens. Changes in ASMRs accounted for 93.3% of the difference between 1996 and 2019 infant mortality rates, whereas changes in the maternal age distribution accounted for 6.7%. CONCLUSIONS: Additional research into the contextual factors in rural counties that are driving the lack of progress for infants of Black and Hispanic teens may help inform efforts to advance health equity.

Background: The prevalence of breastfeeding is less common among adolescent mothers than adultmothers. These mothers experience various issues during breastfeeding. The present study aimed toexplore the normative needs of adolescent mothers during breastfeeding from health care providers’perspective.Methods: This qualitative content analysis study was conducted from October 2022 until June 2023.14 health care providers who had worked in the field of breast milk were purposefully selected withmaximum variation. Face-to-face semi-structured interviews were conducted and sampling continueduntil data saturation. Data analysis was performed using Graneheim and Lundman’s method withMAXQDA software version 10.Results: The main concepts obtained from the data were classified into one theme entitled,“comprehensive support”, and seven categories including “need to correct wrong traditional beliefs”,“educational and counseling needs”, “providing quality services”, “need for psychological support”,“need for protective laws”, “financial needs”, and “the need for social network support”.Conclusion: Adolescent mothers in Iran have various needs during breastfeeding, and they requirethe assistance of their families, healthcare providers, and the government to fulfill them. Therefore, itis also recommended that policymakers in the health system should design policies to accommodatethe requirements of this group of mothers. In addition to policy development in the health system, theinfrastructure required for policy and law to be executed should be considered.

The mother's behavior in giving breast milk, which can affect breastfeeding satisfaction, is one of the achievements of the mother's role. Breastfeeding satisfaction is a key factor in the success of exclusive breastfeeding programs, where family support is an important factor that can influence breastfeeding satisfaction. The purpose of this study was to determine the relationship between family support and breastfeeding satisfaction in adolescent mothers. In this study, the method used was a descriptive correlation with a cross-sectional approach. The sample set was 61 people with inclusion criteria. A questionnaire based on the Maternal Breastfeeding Evaluation Scale (MBFES) and a questionnaire based on Family Support were used to collect data. The bivariate analysis technique with chi-square. The results of this study showed a p-value 0,000 that there is a relationship between family support and breastfeeding satisfaction in adolescent mothers. Breastfeeding satisfaction is influenced by family support because most adolescent mothers consult with their parents and husbands before deciding to breastfeed their children.

Background: The alarming prevalence of teen mothers’ exposure to perinatal intimate partner violence (IPV, 8.3-67%) and attachment disorders (ADs) among their children is a global concern, especially in sub-Saharan Africa with high teenage pregnancy rates. This study, therefore, aimed to examine the link between teen IPV and AD in their offspring. We sought also to explore the mediating roles of postpartum depression symptoms, maternal sensitivity, parenting stress, and perceived social support in the relationship between perinatal IPV and children’s ADs.Method: This cross-sectional study selected a random sample of 309 teen mothers from Nyanza district. This sample size was determined using Yamane's formula, with random sampling. Various instruments were used for data collection, including questionnaires on intimate partner violence, social support, maternal sensitivity, postpartum depression symptoms and parenting stress and early trauma-related disorders. The data was analysed using SPSS, with mediation analyses performed using the PROCESS macro (version 4.1).Results: IPV was found to be significantly associated with attachment disorders. Simple mediation models showed that parenting stress completely mediated these relationships, while postpartum depression, perceived social support, and maternal sensitivity partially mediated the relationship between IPV and children’s ADs. In parallel mediation model, the combined roles of all mediators fully mediated the associations between IPV and ADs.Conclusion: These findings offer valuable insights in designing or strengthening the appropriate interventions to prevent and mitigate the perinatal intimate partner violence and its detrimental impact on children’s attachment disorders. Combating intimate partner violence in post-conflict situations is challenging in teen mothers, however, our results suggest that efforts to address maternal mental health and parenting practices may protect children from attachment disorders.

Objectives: Social isolation and stigma contribute to poor mental health outcomes. Adolescent mothers in Zimbabwe often experience isolation and stigma, lacking social support and resources to navigate motherhood. The study tested the effectiveness of a community-based peer support intervention to mitigate social isolation and stigma of adolescent motherhood in Harare, Zimbabwe. Methods: Community health workers (n = 12) and peer educators (n = 12) in the intervention arm were recruited and trained on co-facilitating peer support groups. Adolescent mothers aged 15–18 years from two low-income high-density communities in Harare were recruited, using a quasi-experimental design. The intervention arm (n = 104) participated in the peer support groups and both arms completed sociodemographic, base-, mid-, and end-line surveys (control arm n = 79). Peer support groups (12 groups with 6–12 participants in each) met in-person twice a month and completed 12 sessions from May to August 2019 addressing participant-identified topics such as income generation and depression. WhatsApp Messenger was used for training and implementation support. Key community stakeholders discussed project progress and recommendations to improve adolescent mothers' health. Data were analyzed using Stata 15. Results: The intervention arm reported lower depressive symptoms and common mental disorders and higher overall, family, friends, and significant-other support, compared to control. The intervention arm felt more engaged with peers, knew who and where to turn to for help, and had coping, parenting and communication strategies to manage life challenges. Conclusions for Practice: The intervention mitigated social isolation and stigma and thereby improved mental health and social support among adolescent mothers in Harare. This paper draws attention to how markers of adulthood linked to education and employment form an influential social narrative and argues that these help to construct teenage motherhood as problematic. Social policies, informed by this narrative, reinforce the idea of a "correct" path through education and into employment from which young mothers deviate and must be realigned to. This paper draws on a sample of former young mothers who had largely progressed into higher education and sheds light on how their pathways were possible and what challenges they encountered. It therefore joins others in challenging common conceptualisations of teenage motherhood as inevitably leading to educational failure. This paper seeks to open a further avenue to this debate however, in that it questions the wisdom of utilising predetermined markers of success to measure the achievement of young mothers. As the accounts discussed here show, a later data collection point enables us to see how "outcomes", but also priorities, change. Furthermore, by highlighting stories of pride and joy outside of markers of adulthood, it also encourages us to reflect on the effects of a normative social narrative which depicts divergence as failure. Study objectives: The aim of this article is to describe the problem of pregnancy in girls under 15 years of age in the Dominican Republic in the period 2000–2021, to develop a specific indicator for this age group and describing the related factors. Methods: This is an exploratory ecological study, based on secondary data sources, such as birth records from the National Statistics Office (NSO) and the Ministry of Public Health (MPH). We calculated the rates of fertility and pregnancy in early adolescence, as well as analyzed their main determining factors and consequences. Results: Fertility Rate in Early Adolescence (FREA) decreases from 6.27 to 1.04 per thousand in the period 2001–2021. The average FREA for 2015–2021 was 1.78. The average Estimated Rate of Pregnancy in Early Adolescence (ERPEA) for the same period was 3.39. Disability-Adjusted Life Years (DALYs) were 11,620 years. Years of Life Lost (YLL) were 9,665.9 years. The prevalence of Low Birth Weight (LBW) in the under 15-year-old age group was 14.2 %. Conclusions: Pregnancy in childhood implies risks for both the mother and the child, including low birth weight. The official fertility rate is substantially underreported (2.84 vs. 1.79).The fertility rate indicator traditionally used does not accurately measure the number of pregnancies in women, particularly in specific age groups or populations where pregnancies may be interrupted by various factors. Therefore, the use of ERPEA is recommended.We emphasize the need for implementing the proposed indicator for the target group, as well as monitoring Sustainable Development Goal indicator 3.7.2.

Background: Depression is a major public health concern among Indian adolescents. Pre- and post-natal depression can often alter fetal development and have negative consequences on the physical and mental health of the mother. This paper aims to draw attention to the prevalence of depression and its correlates among currently married, ever-pregnant adolescents from two Indian States, i.e. Uttar Pradesh and Bihar. Methods: This study utilizes data from a subsample (n = 3116) of the prospective cohort study Understanding the Lives of Adolescents and Young Adults (UDAYA) among 10 to 19 year-old adolescents. Bivariate analysis was performed to assess the prevalence of depression by sociodemographic and behavioral characteristics. To further access the predictors associated with depression a logistic regression model was applied. Results: Around one-tenth (9%) of pregnant adolescents had depression. Regression analysis indicated that substance use, religion, autonomy, considering attempting suicide, premarital relationship, violence, dowry, adverse pregnancy outcome, menstrual problem, and parental pressure for the child immediately after marriage were significantly associated with depression. Conclusions: This study confirms the pre-existing annotation that teen pregnancy is linked with depression. Findings indicate that Adolescent mothers experiencing violence, and a history of adverse pregnancy outcomes are at increased risk of developing depression. This longitudinal randomized controlled trial examined the impact of a doula home visiting intervention for young, low-income mothers on parenting and toddler social-emotional development and tested whether intervention effects were moderated by maternal emotional and behavioral health characteristics. 156 mothers were offered home visits from a home visitor starting in mid-pregnancy through several years postpartum, with a community doula also working with the mother during pregnancy and after the birth. 156 received case management. Interviews, video recordings of mother-child interactions, and toddler assessments were conducted at 3 weeks, 3 months, 13 months, and 30 months of age. Intent-to-treat analyses conducted with the full sample showed some intervention effects. Moderation analyses, however, showed that most effects were concentrated among mothers engaged in high levels of risk-taking (delinquent behaviors, school suspensions, smoking, alcohol use, sexual risk-taking). Among higher risk-taking mothers, the intervention was related to less intrusiveness during early infancy, less psychological and physical aggression during toddlerhood, more sensitive parenting attitudes, and greater toddler social relatedness. Maternal depressive symptoms were only a moderator for toddler behavior problems. These findings suggest that doula home visiting may be a particularly effective model for enhancing sensitive, non-aggressive parenting among young mothers with a history of risk-taking behavior. Aim: This study aims to investigate associations of formal childcare with maternal and child outcomes in a large sample of adolescent mothers. Background: Forty percent of adolescent girls in Africa are mothers. Increasing evidence shows positive impacts of formal childcare use for adult women, but no known studies in the Global South examine associations for adolescent mothers and their children. Methods: We interviewed 1046 adolescent mothers and completed developmental assessments with their children (n = 1139) in South Africa's Eastern Cape between 2017 and 2019. Questionnaires measured childcare use, maternal and child outcomes and socio‐demographic background variables. Using cross‐sectional data, associations between formal childcare use and outcomes were estimated in multivariate multi‐level analyses that accounted for individual‐level and family‐level clustering. Results: Childcare use was associated with higher odds of being in education or employment (AOR: 4.01, 95% CIs: 2.59–6.21, p <.001), grade promotion (AOR: 2.08, 95% CIs: 1.42–3.05, p <.001) and positive future ideation (AOR: 1.58, 95% CIs: 1.01–2.49, p =.047) but no differences in mental health. Childcare use was also associated with better parenting on all measures: positive parenting (AOR: 1.66, 95% CIs: 1.16–2.38, p =.006), better parental limit‐setting (AOR: 2.00, 95% CIs: 1.37–2.93, p <.001) and better positive discipline (AOR: 1.77, 95% CIs: 1.21–2.59, p =.003). For the children, there were no differences in temperament or illness, but a significant interaction showed stronger associations between childcare use and better cognitive, language and motor scores with increasing child age (AOR: 5.04, 95% CIs: 1.59–15.96, p =.006). Conclusions: Adolescent mothers might benefit substantially from formal childcare, but causal links need to be explored further. Childcare use was also associated with improved parenting and better child development over time, suggesting positive pathways for children. At an average of $9 per month, childcare provisions for adolescent mothers may offer low‐cost opportunities to achieve high returns on health and human capital outcomes in Sub‐Saharan African contexts. Background: Parents are the primary caregivers and influential figures in adolescents' lives. They play a crucial role in shaping their children's attitudes, beliefs, and behaviors regarding sexual reproductive health. A vast difference in knowledge between adolescents and their parents indicates failure in knowledge translation and is associated with sexual and reproductive consequences, such as teenage pregnancies. Methods: In this cross-sectional study between December 2020 and December 2021, 834 teenage mothers and their parents/guardians (n = 861) were enrolled from all five provinces of Rwanda. Knowledge of sexual reproductive health (SRH) rights was assessed, and the differences between the understanding of parents and their children were analyzed using Chi-square tests. Results: There were statistically significant differences for all the assessed aspects of SRH knowledge rights between parents and their teenagers (ie, on the age of consent,   safe abortion, right to education for teenage mothers, understanding of financial responsibilities, and perceptions of legal consequences). Conclusion: There is a significant knowledge difference between the two generations regarding their understanding of SRH rights. It is recommended that future programming should devise innovative ways that bring together parents and their children to discuss sexual reproductive health rights, which will result in empowered children and teenagers in Rwanda. Background/objectives The Family Nurse Partnership is an intensive home visiting programme for adolescent mothers. We aimed to evaluate the effectiveness of the Family Nurse Partnership on outcomes up to age 7 using national administrative data. Design We created a linked cohort of all mothers aged 13–19 using data from health, educational and children’s social care and defined mothers enrolled in the Family Nurse Partnership or not using Family Nurse Partnership system data. Propensity scores were used to create matched groups for analysis. Setting One hundred and thirty-six local authorities in England with active Family Nurse Partnership sites between 2010 and 2017. Participants Mothers aged 13–19 at last menstrual period with live births between April 2010 and March 2019, living in a Family Nurse Partnership catchment area and their firstborn child(ren). Interventions The Family Nurse Partnership includes up to 64 home visits by a family nurse from early pregnancy until the child’s second birthday and is combined with usual health and social care. Controls received usual health and social care. Main outcome measures Indicators of child maltreatment (hospital admissions for injury/maltreatment, referral to social care services); child health and development (hospital utilisation and education) outcomes and maternal hospital utilisation and educational outcomes up to 7 years following birth. Data sources Family Nurse Partnership Information System, Hospital Episode Statistics, National Pupil Database. Results Of 110,520 eligible mothers, 25,680 (23.2%) were enrolled in the Family Nurse Partnership. Enrolment rates varied across 122 sites (range: 11–68%). Areas with more eligible mothers had lower enrolment rates. Enrolment was higher among mothers aged 13–15 (52%), than 18–19 year-olds (21%). Indicators of child maltreatment: we found no evidence of an association between the Family Nurse Partnership and indicators of child maltreatment, except for an increased rate of unplanned admissions for maltreatment/injury-related diagnoses up to age 2 for children born to Family Nurse Partnership mothers (6.6% vs. 5.7%, relative risk 1.15; 95% confidence interval 1.07 to 1.24). Child health and developmental outcomes: there was weak evidence that children born to Family Nurse Partnership mothers were more likely to achieve a Good Level of Development at age 5 (57.5% vs. 55.4%, relative risk 1.05; 95% confidence interval 1.00 to 1.09). Maternal outcomes: There was some evidence that Family Nurse Partnership mothers were less likely to have a subsequent delivery within 18 months of the index birth (8.4% vs. 9.3%, relative risk 0.92; 95% confidence interval 0.88 to 0.97). Younger and more vulnerable mothers received higher numbers of visits and were more likely to achieve fidelity targets. Meeting the fidelity targets was associated with some outcomes. Limitations Bias by indication and variation in the intervention and usual care over time and between areas may have limited our ability to detect effects. Multiple testing may have led to spurious, significant results. Conclusions This study supports findings from evaluations of the Family Nurse Partnership showing no evidence of benefit for maltreatment outcomes measured in administrative data. Amongst all the outcomes measured, we found weak evidence that the Family Nurse Partnership was associated with improvements in child development at school entry, a reduction in rapid repeat pregnancies and evidence of increased healthcare-seeking in the mother and child. Future work Future evaluations should capture better measures of Family Nurse Partnership interventions and usual care, more information on maternal risk factors and additional outcomes relating to maternal well-being. Plain language summary The Family Nurse Partnership is an intensive home visiting service that offers first-time young mothers up to 64 visits with a family nurse from pregnancy to their child’s second birthday. The Family Nurse Partnership aims to improve birth outcomes, child health and development and promote economic self-sufficiency among young mothers. Previous research in England found no differences in birthweight, maternal smoking, repeat pregnancies or accident and emergency attendances between mothers who did or did not take part in the Family Nurse Partnership. However, children in the Family Nurse Partnership group had better measures of development at school age. We aimed to add to the evidence from earlier studies, by using electronic records that are routinely collected as part of health, education and social care services, to compare outcomes for around 26,000 mothers enrolled in the Family Nurse Partnership between 2010 and 2019 with similar mothers who were not enrolled. This study showed that around one in four mothers who were eligible for the programme were enrolled in the Family Nurse Partnership, and family nurses gave priority to mothers who were younger, more deprived or who had other markers of vulnerability (e.g. a history of substance misuse violence, self-harm or mental health conditions). We found no evidence of a difference in indicators of child maltreatment between mothers who were enrolled in the Family Nurse Partnership and those who were not enrolled, but we found weak evidence to suggest that children born to mothers enrolled in the Family Nurse Partnership were more likely to achieve a Good Level of Development at school entry (age 5). We also saw that mothers enrolled in the Family Nurse Partnership were less likely than those who were not enrolled to have their next child within 18 months of their first child. More research is needed to understand which elements of intensive home visiting services work best, for whom and when. This will help inform decisions about whether it is better to offer highly intensive services for a small portion of the target population or to extend and enhance existing universal health visiting services to better support all adolescent mothers. Scientific summary Background The Family Nurse Partnership (FNP) is an intensive home visiting programme supporting young first-time mothers, which has a strong evidence base from several randomised trials in the USA. Mothers enrolled in the FNP receive up to 64 home visits by a dedicated family nurse, from early pregnancy until the child’s second birthday. The FNP aims to improve birth outcomes, child health and development and promote economic self-sufficiency among young mothers. Although a randomised trial of the FNP in England found no evidence of benefit on smoking in pregnancy, birthweight, hospital admissions before age 2 or second pregnancy within 2 years, improved cognitive development outcomes were reported, and there remains strong support for the programme locally. Our population-based study used longitudinal linked observational data between the health, education and social care sectors to evaluate the effects of FNP on outcomes of eligible mothers and their children up to age 7 and generated evidence on the factors that may influence effectiveness and programme engagement (including participant and programme characteristics). We aimed to generate evidence on which groups of mothers and children benefit from the real-world implementation of FNP in England in order to inform the targeting and commissioning of services. Objectives Determine the rate of and characteristics associated with enrolment in FNP among young mothers across local authorities in England. Determine the effect of FNP on maternal and child outcomes, including identifying which families benefit the most from FNP. Identify contextual and programme factors that might influence the effect of FNP. Results Objective 1 Of 110,520 eligible mothers aged 13–19 years who gave birth between April 2010 and March 2017, 25,680 (23.2%) were enrolled in FNP: 14% were aged 13–15 years. Enrolment rates varied across 122 sites (range: 11–68%). Areas with more eligible mothers had lower enrolment rates. Enrolment was higher among mothers aged 13–15 (52%) than 18-19 year-olds (21%). Only 26% of mothers with markers of vulnerability (including living in the most deprived area decile or previous mental health-related hospitalisations) were enrolled. Objective 2 Indicators of child maltreatment: we found no evidence of an association between FNP and indicators of child maltreatment, except for an increased rate of unplanned admissions for maltreatment/injury-related diagnoses up to age 2 for children born to mothers enrolled in FNP [6.6% vs. 5.7%, relative risk (RR) 1.15; 95% confidence interval (CI) 1.07 to 1.24] and weak evidence of a reduction in the percentage of children with a child protection plan for those born to mothers enrolled in FNP (5.1% vs. 6.1%, RR 0.84; 95% CI 0.71 to 1.00). Child health and developmental outcomes: FNP was associated with an increase in the number of children with ≥ 1 unplanned admission for any diagnosis up to age 2 and the number of children with ≥ 1 A&E attendance by age 2 and age 7. There was weak evidence that children born to FNP mothers were more likely to achieve a Good Level of Development (school readiness) at age 5 (57.5% vs. 55.4%, RR 1.05; 95% CI 1.00 to 1.09). There was no evidence of a difference between groups in Special Educational Needs provision or attainment at Key Stage 1, but FNP was associated with an increase in the percentage of children registered for Free School Meals (51.2% vs. 46.7%, RR 1.09; 95% CI 1.04 to 1.14). Maternal outcomes Mothers enrolled in FNP were more likely to have unplanned hospital admissions for any diagnosis (and for adversity and mental health diagnoses) in the 2 years following birth, compared with mothers who were not enrolled. This effect persisted until 7 years following birth, though the size of the effect decreased over time. Mothers enrolled in FNP were less likely to have a subsequent delivery within 18 months of the index birth (8.4% vs. 9.3%, RR 0.92; 95% CI 0.88 to 0.97) compared to those who were not enrolled. Objective 3 Fifty-eight per cent of mothers enrolled in FNP completed the programme (42% left early). Attrition was 8% during pregnancy, 23% in infancy (between birth and 1 year) and 19% in toddlerhood (between 1 and 2 years). Mothers received on average 38 visits and 42 hours of contact time with family nurses during the programme. Younger and more vulnerable mothers received higher numbers of visits and were more likely to achieve fidelity targets for the expected number of visits at each stage of the programme. After adjusting for these characteristics, mothers who met the target for the number of expected visits in pregnancy were less likely to have a subsequent birth within 18 months than those who did not meet the target; children born to mothers who met the target in infancy and toddlerhood were more likely to have an unplanned hospital admission for maltreatment/injury up to age 2 than those who did not meet the targets in these stages. Conclusions Our findings support previous evaluations of FNP in England, which show no evidence of an impact on child maltreatment outcomes but some weak evidence that FNP is associated with improvements in some child development measures. Limitations Mothers enrolled in FNP were more vulnerable than those who were not, but we could only control for maternal characteristics associated with enrolment that were captured in administrative data. Residual confounding could have limited our ability to detect beneficial effects of the programme. The weak evidence for small improvements in school readiness as measured by a Good Level of Development, and reductions in the number of rapid repeat pregnancies may therefore reflect larger positive effects of the programme. As these outcomes were the only ‘positive’ effects amongst the many outcomes that were evaluated, they could be due to chance. There are challenges in interpreting outcomes captured in administrative data: the increased rates of unplanned admissions and A&E attendances in the mother and child associated with FNP may reflect higher rates of health problems or appropriate care seeking when a health problem such as injury occurs as a result of advice and support from family nurses. This finding could therefore be interpreted as demonstrating that family nurses can have a long-term effect on maternal health care-seeking behaviours. This study was not designed to identify effects on a range of other important outcomes, including changes in self-reported maternal mental health, well-being, confidence, behaviour and mother/parent–child engagement and interaction. Implications Despite reductions in teenage pregnancies over recent decades, there remains a significant population of young and vulnerable mothers in England who need intensive support. Currently, the majority of these mothers are not receiving support from FNP, as it is not offered in all areas and is only offered to around one in four mothers in areas in which it is commissioned. Expecting to detect effects of home visiting that starts in pregnancy on birth outcomes and on relatively insensitive child development measures and other child outcomes may be unreasonable in the context of social disadvantage, discrimination and other challenges that adolescent mothers face before, during and after pregnancy. However, there is strong support for FNP locally, and FNP practitioners report that mothers participating in the programme develop more reflective parenting and awareness of their child’s needs. Without better evidence, removing support for young mothers could be harmful, especially in the context of increasing social disadvantage and widespread health visitor shortages. There remains uncertainty about whether it is better to commission highly intensive services like the FNP versus enhanced universal services. Recommendations for research More research is needed to understand which elements of intensive interventions are most effective, for whom and when and to help inform decisions about whether it is better to commission highly intensive services for a small portion of the target population or to extend and enhance universal services to better support all adolescent mothers. More research is needed to understand the effects of the programme on mothers who are not enrolled in FNP: we do not yet know if FNP diverts resources away from the usual care that an adolescent mother should receive or if FNP has a positive effect on mothers not enrolled in the programme through shared learning and practices. Robust evaluation is needed of modifications to FNP, including changes in the Accelerated Design and Programme Testing sites, outcomes for mothers aged 20–24 years, outcomes for fathers and outcomes for subsequent children. Better measures of changes in maternally reported well-being, confidence, mental health, parent–child interaction and child behaviour would improve evidence on whether the programme affects mothers and their children and how. Abstract Introduction Girls’ and women’s health as well as social and economic wellbeing are often negatively impacted by early childbearing. In many parts of Africa, adolescent girls who get pregnant often drop out of school, resulting in widening gender inequalities in schooling and economic participation. Few interventions have focused on education and economic empowerment of adolescent mothers in the region. We aim to conduct a pilot randomized controlled trial in Blantyre (Malawi) and Ouagadougou (Burkina Faso) to examine the acceptability and feasibility of three interventions in improving educational and health outcomes among adolescent mothers and to estimate the effect and cost-effectiveness of the three interventions in facilitating (re)entry into school or vocational training. We will also test the effect of the interventions on their sexual and reproductive health (SRH) and mental health. Interventions The three interventions we will assess are: a cash transfer conditioned on (re)enrolment into school or vocational training, subsidized childcare, and life skills training offered through adolescent mothers’ clubs. The life skills training will cover nurturing childcare, SRH, mental health, and financial literacy. Community health workers will facilitate the clubs. Each intervention will be implemented for 12 months. Methods We will conduct a baseline survey among adolescent mothers aged 10–19 years (N = 270, per site) enrolled following a household listing in select enumeration areas in each site. Adolescent mothers will be interviewed using a structured survey adapted from a previous survey on the lived experiences of pregnant and parenting adolescents in the two sites. Following the baseline survey, adolescent mothers will be individually randomly assigned to one of three study arms: arm one (adolescent mothers’ clubs only); arm two (adolescent mothers’ clubs + subsidized childcare), and arm three (adolescent mothers’ clubs + subsidized childcare + cash transfer). At endline, we will re-administer the structured survey and assess the average treatment effect across the three groups following intent-to-treat (ITT) analysis, comparing school or vocational training attendance during the intervention period. We will also compare baseline and endline measures of SRH and mental health outcomes. Between the baseline and endline survey, we will conduct a process evaluation to examine the acceptability and feasibility of the interventions and to track the implementation of the interventions. Discussion Our research will generate evidence that provides insights on interventions that can enable adolescent mothers to continue their education, as well as improve their SRH and mental health. We aim to maximize the translation of the evidence into policy and action through sustained engagement from inception with key stakeholders and decision makers and strategic communication of research findings. Abstract Background Previous studies have not synthesized existing literature on the lived experiences of pregnant and parenting adolescents (aged 10–19) in Africa. Such evidence synthesis is needed to inform policies, programs, and future research to improve the well-being of the millions of pregnant or parenting adolescents in the region. Our study fills this gap by reviewing the literature on pregnant and parenting adolescents in Africa. We mapped existing research in terms of their substantive focus, and geographical distribution. We synthesized these studies based on thematic focus and identified gaps for future research. Methods We used a three-step search strategy to find articles, theses, and technical reports reporting primary research published in English between January 2000 and June 2021 in PubMed, Jstor, AJOL, EBSCO Host, and Google Scholar. Three researchers screened all articles, including titles, abstracts, and full text, for eligibility. Relevant data were extracted using a template designed for the study. Overall, 116 studies met the inclusion criteria and were included in the study. Data were analyzed using descriptive and thematic analyses. Results Research on pregnant and parenting adolescents is limited in volume and skewed to a few countries, with two-fifths of papers focusing on South Africa (41.4%). Most of the studies were African-led (81.9%), received no funding (60.3%), adopted qualitative designs (58.6%), and were published between 2016 and 2021 (48.3%). The studies highlighted how pregnancy initiates a cycle of social exclusion of girls with grave implications for their physical and mental health and social and economic well-being. Only 4.3% of the studies described an intervention. None of these studies employed a robust research design (e.g., randomized controlled trial) to assess the intervention’s effectiveness. Adolescent mothers' experiences (26.7%) and their education (36.2%) were the most studied topics, while repeat pregnancy received the least research attention. Conclusion Research on issues affecting pregnant and parenting adolescents is still limited in scope and skewed geographically despite the large burden of adolescent childbearing in many African countries. While studies have documented how early pregnancy could result in girls' social and educational exclusion, few interventions to support pregnant and parenting adolescents exist. Further research to address these gaps is warranted.

Adolescent mothers frequently encounter pregnancy-related stigma which increases their risk for depression and this risk is comparatively greater than for adult mothers. The current study investigated the relationship between stigma and depression in adolescent mothers including the role that household composition played in the adolescents' experience of stigma and depression. A sample of 85 adolescent mothers, aged 14–18 years, enrolled at multiple sites of a national school program in Jamaica, completed an interviewer-administered questionnaire that assessed different types of perceived stigma, depression risk, and varying forms of household composition for the adolescent mothers. After adapting the Berger's HIV Stigma Scale for use with adolescent pregnancy, correlational analyses revealed that higher scores on the Personalized Stigma and Negative Self-Image subscales and the overall stigma scale were associated with higher depression scores. Adolescent mothers living in a single-parent household were at increased risk for experiencing stigma while those living in multigenerational households were protected from depression risk. Regression analyses identified exposure to sexual violence during pregnancy as an independent predictor of feeling stigmatized, and depression risk was independently predicted by internalized stigma resulting in a negative self-image. We discuss conceptualizing stigma as heterogenous and bi-directional in its relationship with depression and suggest that more research is needed to evaluate the link between stigma, mental health risk and protective factors for adolescent mothers. The lack of economic resources has a negative effect on the maternal role of younger mothers. In Chile, the majority of adolescent pregnancies occur in socially and economically vulnerable contexts. The current study aimed to examine the relationship between demographic variables within the family context and parenting behaviors among Chilean adolescent mothers (including affection, responsiveness, encouragement, and teaching). These factors were correlated with communication, problem-solving abilities, and personal–social development in typically developing infants. The study included a sample of 79 Chilean adolescent mother–child dyads with children aged 10 to 24 months. Communication, problem-solving, and personal–social development were assessed using the Ages and Stages Questionnaire-3, along with a demographic information questionnaire. The parenting behaviors mentioned above were observed using the Spanish version of Parenting Interactions with Children: Checklist of Observations Linked to Outcomes. The findings indicated that mothers in employment and those who had not dropped out of school had children with better problem-solving skills. Additionally, children residing with their fathers and female children performed better in communication, problem-solving, and personal–social development. Maternal responsiveness was associated with communication and problem-solving, while maternal encouragement was linked to improved problem-solving skills. Maternal teaching was connected to communication, problem-solving, and personal–social development. Background: Adolescent pregnancies are a global problem occurring in many countries. Adolescent pregnancy is a risk factor for stunting in children. Aim: This study was designed to develop and evaluation of nursing interventions to prevent stunting in children of adolescent mothers. Methods: A mixed-methods explanatory sequential design with two phases approach will be applied. Phase I is a qualitative phenomenology descriptive study that will be used. Participants will be adolescent pregnant women from several community health centers (Puskesmas) and healthcare staff in a community public center (Puskesmas) will be selected by purposive sampling. The study will be conducted at community health centers (Puskesmas) in Makassar, South Sulawesi, Indonesia. Data will be gathered through in-depth interviews and focus group discussions and analyzed using thematic analysis. Subsequently, an experimental pre–post-test design with a control group will be used to measure the effectiveness of the nursing intervention on preventing stunting among adolescent mothers in the quantitative phase by behavior toward stunting prevention for adolescent pregnancy and the nutritional status of children. Conclusion: This study will provide insight from both adolescent mothers' and healthcare staff's perspectives on stunting prevention including nutrition of adolescent pregnancy and breastfeeding. We will evaluate the effectiveness and acceptability of nursing intervention in preventing stunting. This will contribute to the international literature on the use of healthcare staff at community health services (puskesmas) to achieve linear growth due to prolonged food insecurity and illnesses in childhood. Objectives Young mothers are vulnerable to postpartum depression due to role transition-related stress. Understanding the causes underlying these stressors is essential for developing effective interventions. Methods This study analyzed the 2018 Indonesian Basic Health Research data. The Mini International Neuropsychiatric Interview was used to assess postpartum depression symptoms in mothers aged 15-24 years with infants aged 0-6 months. In 1285 subjects, the risk factors for postpartum depression were evaluated using multivariate logistic regression. Results The overall prevalence of depression in the 6 months postpartum was 4.0%, with a higher prevalence in urban areas (5.7%) than in rural areas (2.9%). Urban and rural young mothers showed distinct postpartum depression risk factors. In urban areas, living without a husband (odds ratio [OR], 3.82; 95% confidence interval [CI], 1.24 to 11.76), experiencing preterm birth (OR, 4.67; 95% CI, 1.50 to 14.50), having pregnancy complications (OR, 3.03; 95% CI, 1.20 to 7.66), and having postpartum complications (OR, 5.23; 95% CI, 1.98 to 13.80) were associated with a higher risk of postpartum depression. In rural areas, postpartum depression was significantly associated with a smaller household size (OR, 3.22; 95% CI, 1.00 to 10.38), unwanted pregnancy (OR, 4.40; 95% CI, 1.15 to 16.86), and pregnancy complications (OR, 3.41; 95% CI, 1.31 to 8.88). Conclusions In both urban and rural contexts, postpartum depression relates to the availability of others to accompany young mothers throughout the postpartum period and offer support with reproductive issues. Support from the family and the healthcare system is essential to young mothers’ mental health. The healthcare system needs to involve families to support young mothers’ mental health from pregnancy until the postpartum period.

Plain English Summary Pregnant and parenting adolescents are at a higher risk of poor mental health. If unaddressed or untreated, the consequences may be dire throughout an individual’s life. About one in four girls start childbearing before their 19th birthday in Africa. Most of these pregnancies are unintended or unplanned and occur outside wedlock, bringing untoward disruption to every facet of the girl’s life. The burden of early parenthood and the stigma of starting childbearing too early and outside wedlock can affect the mental wellness of girls. Yet only a few studies have focused on understanding the burden and drivers of poor mental health among pregnant and parenting girls. Our study contributes to the research on depression among pregnant and parenting girls, highlighting its burden and individual, family, and community drivers. We conducted this study in Ouagadougou, Burkina Faso, and Blantyre, Malawi, between March and September 2021. A total of 980 and 669 pregnant and parenting girls were recruited into this study in Burkina and Malawi. Our analysis shows that 18.8% and 14.5% of pregnant and parenting girls in Burkina Faso and Malawi, respectively, had probable depression. Depression symptoms was higher when girls were older than 17 years, suffered violence from their partners, their partners denied paternity, received no support from their partners and parents, lived in violent neighborhoods, and had no support system in the community. We conclude that depression is high among pregnant and parenting girls. It necessitates screening them for depression during antenatal and postnatal care visits. To effectively address depression among these girls, interventions must canvass support at the family and community levels.

Adolescent mothers experience poorer sleep than adult mothers, and Latina adolescent mothers are at greater risk of postpartum depression compared with other racial/ethnic groups. However, social support may be protective against the negative effects of poor sleep in this population. The current study examined (1) associations between the quality and quantity of Latina adolescent mothers' sleep and mental health (depressive symptoms and anxiety), and (2) whether social support buffered the effects of poor sleep on mental health. A sample of Latina adolescent mothers (N = 84) from an agricultural region in the United States reported on their sleep duration/quality, social support from family, friends, and significant others, and their depressive and anxiety symptoms. Results showed that adolescent mothers reported poorer sleep than pediatric recommendations, and poorer sleep quality was associated with greater depressive and anxiety symptoms. Interestingly, when adolescent mothers reported better sleep, they had fewer depressive symptoms in the context of high support from friends compared with low support from friends. Sleep is important for mental health in Latina adolescent mothers, and better sleep combined with strong social support has positive associations with mental health in this population. Findings hold implications for improving mental health in adolescent mothers.

This study of multiple cases aimed to analyze the social support network of young mothers of children diagnosed with congenital syphilis in a municipality in Ceará with six participants. Information was collected through semi-structured interviews and analyzed using the cross-case synthesis analytical technique. The young women's social network comprised family members, people outside the family, and social facilities. We identified heterogeneous bonds and social support as essential in the gestational and postpartum periods after the diagnosis of syphilis. The vertical transmission of syphilis is permeated by subjectivities identified from understanding the context involving social networks, interpersonal relationships, and support for mothers and their children. Studies examining the relationship between young maternal age and maternal outcomes are often cross-sectional or short term. We review birth cohort studies that investigate life course outcomes of teen mothers past age 25. Strengths of birth cohort studies include a focus on a complete cohort, rather than a sample, and prospective data collection beginning before or at birth. Limitations are high cost, attrition and unmeasured background factors. Some 20 studies from six countries met study criteria. This narrative review describes how teenage mothers fare as adults, identifies factors that modify outcomes and examines whether outcomes reflect specific time periods or cohorts. Childhood disadvantage was a greater marker of teen mothering in more recent cohorts, even in countries with strong social welfare programmes. This study sought to examine the factors associated with timing of the first ANC visit and quality of ANC among adolescent mothers in Uganda.Methods: This study was based on quantitative data from 248 adolescent mothers aged 10– 19 years in Luuka district, Eastern Uganda. We selected adolescent mothers who were either pregnant or had infants aged 0– 3 months. We used logistic regression to identify factors associated with timing and receipt of good quality ANC.Results: The majority of the adolescents (82%) attended ANC for their most recent pregnancy. Of these, 47% made the first visit in the first trimester while 36% received good quality ANC. Having knowledge of danger signs in pregnancy was a determinant of both timing of first ANC visit (aOR = 2.89, 95% CI: 1.04– 8.06) and receipt of good quality ANC (aOR = 6.57, 95% CI: 1.75– 24.65). Other determinants for timing of first ANC visit were mother’s age, partner’s age, having ever given birth, decision maker on health care and daily earnings. Other determinants for quality of ANC included distance to health facility and knowledge of family planning methods.Conclusion: This study recommends expanding the network of public health facilities further into the rural communities in the district as a means of bringing health services closer to adolescent mothers, deliberate efforts to equip adolescent girls with reproductive health information on pregnancy danger signs, and family planning and empowering adolescent girls to start income-generating activities.Keywords: antenatal care, ANC timing, ANC quality, Luuka district, adolescent mothers, Uganda

Background: It is estimated that 38.8% of mothers develop postpartum depression (PPD) in South Africa. While empirical evidence documents an association between intimate partner violence (IPV) victimization in pregnancy and PPD among adult women, the association has been underexamined among adolescent mothers (< 19 years). The study's purpose is to examine whether IPV victimization during pregnancy is associated with PPD among adolescent mothers. Methods: Adolescent mothers (14–19 years) were recruited at a regional hospital's maternity ward in KwaZulu Natal, South Africa between July 2017-April 2018. Participants completed behavioral assessments at two visits (n = 90): baseline (up to 4 weeks postpartum) and follow-up (6–9 weeks postpartum, when PPD is typically assessed). The WHO modified conflict tactics scale was used to create a binary measure of any physical and/or psychological IPV victimization that occurred during pregnancy. Participants with scores ≥ 13 on the Edinburgh Postpartum Depression Scale (EPDS) were classified as having symptoms of PPD. We used a modified Poisson regression with robust standard errors to assess PPD in association with IPV victimization during pregnancy, controlling for relevant covariates. Results: Nearly one-half (47%) of adolescent mothers reported symptoms of PPD by 6–9 weeks post-delivery. Further, IPV victimization during pregnancy was highly prevalent (40%). Adolescent mothers who reported IPV victimization during pregnancy had marginally higher risk of PPD at follow-up (RR: 1.50, 95 CI: 0.97–2.31; p = 0.07). The association was strengthened and significant in covariate-adjusted analysis (RR: 1.62, 95 CI: 1.06–2.49; p = 0.03). Conclusions: Poor mental health was common among adolescent mothers, and IPV victimization during pregnancy was associated with PPD risk among adolescent mothers. Implementing IPV and PPD routine screenings during the perinatal period may aid in identifying adolescent mothers for IPV and PPD interventions and treatment. With the high prevalence of IPV and PPD in this vulnerable population and the potential negative impact on maternal and infant outcomes, interventions to reduce IPV and PPD are needed to improve adolescent mothers' well-being and their baby's health.
