7. Translation, adaptation and validation of an epilepsy screening instrument in two Ghanaian languages.

Abstract

Introduction: The prevalence of epilepsy in sub-Saharan Africa varies considerably, and the exact estimate for Ghana remains unclear, particularly in peri-urban areas where data are scarce. More community-based studies are required to understand better the actual burden of epilepsy in these areas and the difficulties in accessing healthcare. Objective: To adapt and validate a household survey epilepsy-screening instrument in Shai-Osudoku and Ningo-Prampram District of Greater Accra Region, Ghana. Methods: We developed a 17-item epilepsy screening instrument by modifying previously validated English language questionnaires. We included questions that could identify convulsive and non-convulsive seizures. Language experts forward- and back-translated the questionnaires into the two languages: Asante Twi and Dangme. Cases were people with confirmed epilepsy attending healthcare facilities where these languages are used. Controls were unaffected relatives of cases or people attending the same healthcare facilities for other medical conditions. We matched cases and controls for geographical location and ethnicity. An affirmative response to one of the seventeen questions by a participant was deemed a positive screen. The questionnaires were divided into two stages. The first stage consisted of broader, more general questions aimed at identifying potential cases of epilepsy. The second stage involved a more detailed and focused set of questions administered to those who screened positive in the first stage. Results: One hundred and forty Dangme speakers (70 cases and 70 controls) and 100 Asante Twi speakers (50 cases and 50 controls) were recruited. The sensitivity and specificity for Dangme were: Stage 1, 100% and 80%, and Stage 2, 98.6% and 85.7%. The Dangme version reliably identified epilepsy with positive predictive values of 83.3% and 87.3% at stages 1 and 2. The questionnaire excluded epilepsy with 100% and 98.4% negative predictive values. For the Asante Twi version, the sensitivity and specificity were 98% and 92% (95% at Stage 1, and for Stage 2, 96% and 94%. The Asante Twi questionnaire reliably specified epilepsy with positive predictive values of 92.5% and 94.1% at stages 1 and 2. It excluded epilepsy with negative predictive values of 97.9% and 95.9% for the two stages Conclusions: Our questionnaire is valid for the two languages and usable for community-based epilepsy surveys in Ghana. 9. Flexural performance of bamboo fiber‐reinforced concrete mixed with seawater and sea sand.

Abstract

In the face of the huge consumption of fresh water and river sand by the concrete industry and the poor flexural performance of plain concrete, it is theoretically feasible and environmentally friendly to use bamboo fiber as a replacement to reinforce concrete mixed with seawater and sea sand. In this research, taking the volume fraction (0.6%, 1.2%, and 2.4%), aspect ratio (10, 20, and 30) and diameter (1.0, 1.5, and 2.0 mm) of bamboo fibers as parameters, 15 groups of bamboo fiber‐reinforced concrete (BFRC) prisms and one control group of plain seawater sea sand concrete prisms were subjected to four‐point bending test, followed by analyzing the crack pattern, ultimate load, mid‐span deflection and strain. Under the condition of 1.2% volume fraction, 20 aspect ratio, 1.5 mm diameter, and 30 mm length, the maximum increase rate of flexural strength in this research was obtained, and then it was compared with that of flexural strength of concrete prisms reinforced by various natural fibers. In addition, relevant fitting equations and theoretical calculation formulas were derived, laying a foundation for the subsequent research and application of BFRC. [ABSTRACT FROM AUTHOR]

Abstract

Background: Amid concerns about severe COVID‐19 in patients with autoimmune rheumatic disease (AIRD) during the outbreak, it is crucial to explore behavioural changes, whether healthy or unhealthy, arising from this patient population in response to the changing healthcare environment. Aim: To investigate COVID‐19‐driven behavioural changes in patients with AIRD. Methods: This observational study invited patients who attended the rheumatology clinic of the Korle Bu Teaching Hospital from 1 August 2020 to 1 July 2021, to respond to a survey questionnaire distributed on the patient's WhatsApp platform. Variables observed were changes in patient behaviour and decision‐making related to medication, healthcare service utilisation and clinical advice. Results: Results for 233 patients were analysed in the study, the majority (89.7%) of whom were women. The most significant behavioural changes were a reduction in hydroxychloroquine (HCQ) dosage, adoption of telemedicine for clinical consultation and keen adherence to protective/preventive health measures. Patients also expressed anxiety regarding the risk of contracting COVID‐19 (52.5%), infecting their families (66.5%) and losing income (50.2%) due to the pandemic. Women and students were more likely to engage in self‐isolation/shielding behaviour. Employed participants practised social distancing more, reduced HCQ dosage and had more fear of losing income. Having mixed connective tissue disease is associated with being anxious about the risk of COVID‐19 infection. Conclusion: The COVID‐19 pandemic has resulted in behaviour changes among patients with AIRD. Despite the perceived risk, most of these patients continue to adhere to their prescribed medication regimens, especially maintaining the dosage of traditional immunosuppressive agents. Abstract

In the world of human–robot coexistence, ensuring safe interactions is crucial. Traditional logic-based methods often lack the intuition required for robots, particularly in complex environments where these methods fail to account for all possible scenarios. Reinforcement learning has shown promise in robotics due to its superior adaptability over traditional logic. However, the exploratory nature of reinforcement learning can jeopardize safety. This paper addresses the challenges in planning trajectories for robotic arm manipulators in dynamic environments. In addition, this paper highlights the pitfalls of multiple reward compositions that are susceptible to reward hacking. A novel method with a simplified reward and constraint formulation is proposed. This enables the robot arm to avoid a nonstationary obstacle that never resets, enhancing operational safety. The proposed approach combines scalarized expected returns with a constrained Markov decision process through a Lagrange multiplier, resulting in better performance. The scalarization component uses the indicator cost function value, directly sampled from the replay buffer, as an additional scaling factor. This method is particularly effective in dynamic environments where conditions change continually, as opposed to approaches relying solely on the expected cost scaled by a Lagrange multiplier. Abstract

The World Health Organization declared the Coronavirus disease of 2019 (COVID-19) a public health emergency of international concern on 30 January 2020, and a pandemic on 11 March 2020. Vaccines have proven to be vital in the effort to control and possibly eventually eradicate this viral infection. There have been reports of thromboembolic events associated with the use of vaccine but from available information, no reported case of atypical Hemolytic Uremic syndrome (HUS) in a black male has been described. We report a case of a 43-year-old black male Sub-Saharan African c5dwho presented with chills, fever, and generalized body aches of 3 days duration after receiving the second booster dose of the COVID-19 vaccine. He developed thrombocytopenia, hemolytic anaemia, and acute kidney injury on admission, and an initial diagnosis of malaria was made. He was managed with parenteral artesunate and then oral artemether/lumefantrine. His hemolytic anaemia was thought to be from malaria-associated hemolysis. This diagnosis was however later re-evaluated to hemolytic uremic syndrome and managed with 50mg daily oral prednisolone which resolved, and he resumed work a week later. Although mass vaccination is a key strategy to control the spread of COVID-19, critical observations should be made to confirm the risk of trigger for abnormal complement activation. Abstract

As the only renewable building material, wood has a strong carbon sequestration effect. Thus, timber structures have the natural advantages of saving energy and reducing emissions. Currently, the research objects of building energy consumption are rarely timber structures. To further control the operational energy consumption of timber structures, this paper takes six-story glued laminated timber beam-column frame and light woodframe shear wall structures as the research objects. Building energy consumption research is conducted through testing the heat transfer coefficient of the envelope structure, air circulation ratio, and Building Information Modeling (BIM) technology on-site. The results show that the energy consumption of the building is consistent with the current energy consumption of small- and medium-sized office buildings, and the heat gain and loss of the building are mainly due to solar radiation and heat conduction of the envelope, respectively. The airtightness of the building has the greatest influence on the energy consumption of the building, and the type of building structure and window-to-wall ratio have little influence on the energy consumption of the building. To meet objectives such as the sound insulation characteristics of timber structures and the comfort of residents, this study assessed an ordinary floor and three kinds of floating floors (Portuguese cork, graphite polystyrene sound insulation boards and expanded polypropylene plastic foam boards) in the same bedroom of a two-storey light wooden building. Adopting standard white noise and music noise excitation, structural design, theoretical checks and sound insulation tests of the floors were conducted. For the four floor structures, the sound insulation volume, sound pressure distribution characteristic spectrum of the building floor under different noise source environments, the frequency characteristics of the noise source, the multiple relationship of the sound insulation sound pressure level and the sound insulation mechanism were analysed. It was found that the sound pressure multiple of the space separating the upper and lower floors of the building was 8–18 times greater than that of the light wooden floor structure. The airborne sound insulation volume–frequency curves of the four floors, calculated by the transfer matrix method, were consistent with the actual measured values. Abstract

Bamboo curtain plywood is made of equal-thickness slotted bamboo slivers as its constituent units and is formed into boards with high pressure. In this paper, rectangular plates, beam specimens, and two sizes of square plates were tested to determine the elastic modulus and shear modulus of bamboo curtain plywood quickly, easily, and accurately. The free square board torsional vibration method was used to test the shear modulus of the square plates. The dynamic test results were also verified by means of the static four-point bending method. The main conclusions show that the dynamic elastic modulus, shear modulus and Poisson's ratio of this bamboo curtain plywood are 0.7 GPa, 1300 MPa and 0.12, respectively. The values of the elastic constants of these bamboo curtain plywood specimens measured by dynamic and static methods are consistent, and this dynamic test method has the advantages of fast, easy, good repeatability and high accuracy. Abstract

The shipping industry has set out to reduce CO2 emissions by 50% by 2050 compared to 2008, as op. cit. in the note by the International Maritime Organisation to the UNFCCC Talanoa Dialogue. In order to achieve this target, ship owners and operators are to consider replacing the conventional diesel generators and gas turbines with renewable energy and other cleaner power producing systems. In this paper, a techno-economic feasibility evaluation model for integrating hybrid renewable energy systems (HRES) into the power architecture of existing ships for greener and sustainable shipping is presented. For HRES to qualify as technically and economically feasible, a decision is made based on a 'techno-economic measure' which is estimated by combining space availability, load served and economic performance. The proposed model is applied to a patrol boat sailing along the Eastern Coast of Ghana with three different proposed HRES. The results from the applied case shows that the most technical and economically feasible HRES for the patrol boat that meets the key performance indicators and offers low GHG emission is a hybrid solar PV, wind turbine and diesel generator system.
