Tailoring the surface structure of the mesoporous metal catalysts is pivotal for influencing both the catalytic activity and selectivity. Through the dealloying of bimetallic alloys, surface modification is achieved by dissolving unstable components, thereby forming a multitude of catalytically active sites. In this study, highly enhanced electrocatalysts are fabricated via chemical etching of mesoporous AuCu alloy nanoparticles prepared through the self‐assembly of micelles. The presence of large mesoporosity (>10 nm) with rich defect sites on the surface, along with the synergistic effect arising from the AuCu bimetallic alloy, effectively boosts electrocatalytic performance for the ethanol oxidation reaction. This strategy demonstrates a promising strategy to further enhance the catalytic performance of Au‐based mesoporous nanoparticles by eliminating the unstable metal component and refining the mesoporous structure with an abundance of various active sites.

We analyzed the issue of a young woman who wanted our opinion regarding uncontrolled hypertension. Her hypertension was discovered to have a highly unusual origin, but it is fairly prevalent in nations like India.A 19-year-old woman who complained of blurred vision was presented to an ophthalmologist, who diagnosed her with grade IV hypertensive retinopathy. Her 2D ECHO was normal, hence a thorough screening for secondary hypertension was carried out. A left paravertebral tumor that may have squeezed the left renal artery and contributed to her hypertension was discovered during the workup by a CECT chest and abdomen scan. She also exhibited widespread lymphadenopathy; a condition known as granulomatous pathology. She was started on anti-TB medication, and after six months of treatment, her radiological and clinical conditions improved. This case highlights a rare instance of TB causing excessively elevated blood pressure.

Key message Tracheobronchopathia osteochondroplastica (TO) is a disorder caused by the accumulation of calcium phosphate in the submucosa of large airways. Benign proliferation of bone and cartilage lead to the narrowing of airways. Bronchoscopy is the diagnostic test for TO. It shows characteristic smooth nodules emerging from tracheal rings that never involves the posterior membranous wall.

Background: Social distancing, mask, and sanitizers are advised to cut the chain of transmission of the droplet and airborne infection since the first case of COVID-19 was reported from Wuhan. Universal mask usage is advised by the center for disease control. Despite this, it is assumed that there is no adequate mask practice. Aims and Objectives: The objectives of the study are as follows: (1) To assess the knowledge and practice regarding optimal mask usage in a different setting by the general community. (2) To identify the barriers for optimal mask usage as experienced by users. Materials and Methods: We did a cross-sectional study in Kerala using self-made validated questionnaire translated to Malayalam, the regional language distributed by web based Google form. Abstract Autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy is an inflammatory disease of the central nervous system (CNS), which affects various regions in the CNS, presenting by variable clinical manifestations. Meningoencephalitis is the most common clinical presentation and association with autoimmune disorders has been reported in about 20% of these patients. Diagnosis is confirmed by the presence of CSF or serum immunoglobulin‐G (IgG) against GFAP. The reported case is a 53‐year‐old woman with the history of long‐standing rheumatoid arthritis who first presented with acute‐onset dizziness and gait disturbance, periventricular linear and radial enhancement pattern on MRI, and normal CSF analysis, successfully treated with an increase in the dose of oral steroids. After a year she had a subacute‐onset, moderate to severe holocephalic headache, normal neurologic examination and CSF analysis, and bilateral diffuse, pachymeningeal, and leptomeningeal enhancement on MRI. According to her Brain MRI imaging with relapsing remitting course steroid responsive ataxia and aseptic meningitis, her serum was tested for GFAP IgG antibodies which was positive. The reported patient is the first in the literature reported pachymeningitis in GFAP astrocytopathy. This case highlights the co‐occurrence of rheumatoid arthritis with GFAP‐associated astrocytopathy, and expands on the previously reported cases with similar association. This might also suggest a common immune pathogenesis.

Background: The change in FEV1 after administration of a short-acting bronchodilator has been widely used for diagnosis of obstructive airway diseases. Many factors can influence the post bronchodilator reversibility. Aim: The aim of the present study was to estimate the presence of reversibility among the patients of obstructive airway disease and to identify the factors affecting it. Methods: Patients who presented to the department of respiratory medicine with symptoms of dyspnea were evaluated with spirometry. Spirometry and post bronchodilator reversibility (BDR) was defined as per international guidelines. SPSS 17 was used for statistical analysis and P < 0.05 was considered significant. Results: Out of 100 patients studied, 33 had BDR. Median age of the population was 58 ± 17 years. There were 72 non-smokers and 58 men. A total of 32 had chronic obstructive pulmonary disease (COPD), 56 had asthma, and 12 had normal spirometry. The median pre and post bronchodilator FEV1 was 1.34L/Sec and 1.46 L/sec respectively. Twenty-seven of asthma (41%) and 6 of COPD (19%) had BDR (P = 0.05). Other factors associated with BDR were smoking (P = 0.035). There was no statistically significant correlation found between eosinophilia, gender, severity of obstruction, BMI, height, weight and age. Conclusion: The prevalence of post BDR in the study population was 33%. The factors affecting BDR were smoking status, and asthma. The study did not show any significant correlation between BDR and eosinophilia, gender, height and age.

As the power grid continues to evolve with advanced wide-area monitoring, protection, and control (WAMPAC) algorithms, there is an increasing need for realistic testbed environments with industry-grade software and hardware-in-the-loop (HIL) to perform verification and validation studies. Such testbed environments serve as ideal platforms to perform WAMPAC prototyping, operator training, and also to study the impacts of different types of cyberattack scenarios on the operation of the grid. In this study, the authors introduce pacific northwest national laboratory(PNNL) cyber-physical systems testbed (PRIME): the testbed that integrates real-time transmission system simulator with commercial industry-grade energy management system software and remote HIL (RHIL). PRIME is an end-to-end, modular testbed that allows high-fidelity RHIL experimentation of a power system. We present two detailed case studies (fault location and clearing in the transmission system and operator training) to show the capabilities of their PRIME testbed. Finally, we briefly discuss some of the potential limitations of their testbed in terms of scalability and flexibility to set up larger test systems and identify directions for future work to address these limitations.

Photo-sensitizers, usually organic dye molecules, are considered to be one of the most expensive components in dye sensitized solar cells (DSSCs). The present work demonstrates a cost effective and high throughput upcycling process on jackfruit rags to extract a natural photo-active dye and its application as a photo-sensitizing candidate on titanium dioxide (TiO2) in DSSCs. The jackfruit derived natural dye (JDND) exhibits a dominant photo-absorption in a spectral range of 350 nm–800 nm with an optical bandgap of ∼1.1 eV estimated from UV–visible absorption spectroscopic studies. The JDND in DSSCs as a major photo-absorbing candidate exhibits a photo-conversion efficiency of ∼1.1% with short circuit current density and open circuit voltage of 2.2 mA⋅cm−2 and 805 mV, respectively. Further, the results show that the concentration of JDND plays an influential role on the photovoltaic performance of the DSSCs due to the significant change in photo-absorption, exciton generation and electron injection into TiO2. The simple, high throughput method used to obtain JDND and the resulting DSSC performance can be considered as potential merits establishing a cost effective excitonic photovoltaic technology. Abstract Cadmium telluride (CdTe) thin films of different thicknesses deposited onto titanium dioxide (TiO2) nanoparticle layer by spray pyrolysis deposition (SPD) are demonstrated as major photo-active semiconductor in photo-electrochemical solar cell configuration using iodide/triiodide (I−/I3 −) redox couple as a hole transport layer. The CdTe–TiO2 heterogeneous films were characterized by X-ray photoelectron spectroscopy which identified doublet split of Cd 3d and Ti 2p which confirms CdTe and TiO2. Optical absorbance and transmittance of CdTe and TiO2 films which were examined by UV–Vis spectroscopy confirm that the optical bandgap of CdTe is 1.5 eV with a dominant photo-absorption in the spectral window of 350–800 nm, while TiO2 showed a bandgap of 3.1 eV and is optically transparent in the visible spectral window. The present work examined photo-anodes comprising 1, 3, 5, and 10 SPD cycles of CdTe coated on TiO2 nanoparticle layer. The solar cell with 5 SPD cycles of CdTe resulting in 0.4% efficiency. Results can be articulated to the CdTe deposited by 5 SPD cycles provided an optimum surface coverage in the bulk of TiO2, while the higher SPD cycles leads to agglomeration which blocks the porosity of the heterogeneous films.

Introduction: Lumbar disc herniation is one of the main causes of discogenic low back pain and reported to affect 60%–80% of people during their lifetime. The two main surgical modalities for intervertebral disc surgery are standard open discectomy and minimally invasive discectomy which include percutaneous endoscopic lumbar discectomy and microendoscopic discectomy (MED). We report our experience with the same technique of MED to evaluate the efficacy of MED for lumbar disc pathology. Aims and Objectives: The aims and objectives were to study the efficacy, advantages, and associated limitations and complications of MED in lumbar disc herniations. Materials and Methods: This study was carried out on 300 patients who had single-level herniated disc. The procedure was done by Microscopic Endoscopic Tubular Retraction System. Preoperative assessment of Visual Analog Scale (VAS) and modified Suezawa and Schreiber (MSS) clinical scoring system was documented 1 day prior to surgery. Postoperative results were determined to be excellent, good, fair, or poor according to MacNab criteria and also evaluated by MSS clinical scoring system on postoperative day 7 and after 6 months. Results: A total of 187 patients were males and 113 patients were females and a majority of patients were in the age group of 31–40 years. A total of 192 patients had disc herniations at L4–L5 level. The mean operative time was 82 min and the mean hospital stay was 5.3 days. Eighteen cases (6%) developed postoperative complications including discitis, dysesthesia, recurrent prolapsed intervertebral disc, residual disc, dural tear, and nerve root injury. Mean preoperative VAS score was 8.7 and the mean postoperative VAS scores at postoperative day 7 and at 6 months were 2.25 and 1.12, respectively. The mean preoperative MSS score was 3.27 and the MSS scores at postoperative day 7 and at 6 months were 7.42 and 8.2, respectively. The overall successful outcome of the endoscopic discectomy after 6-month follow-up on the basis of VAS improvement percentage was 87.6%, MSS scoring percentage was 91.6%, and MacNab scoring percentage was 92.67%. Conclusion: MED is a safe and effective technique. It offers decreased blood loss, shorter operative time, shorter in-hospital stay, decreased need for pain medication, decreased rate of infection, and a shorter return to work time. Limitations of this technique include a learning curve which is related to surgery time, complications, conversion to open procedures, and recurrent disc herniation.

Smart grid initiatives will produce a grid that is increasingly dependent on its cyber infrastructure in order to support the numerous power applications necessary to provide improved grid monitoring and control capabilities. However, recent findings documented in government reports and other literature, indicate the growing threat of cyber-based attacks in numbers and sophistication targeting the nation’s electric grid and other critical infrastructures. Specifically, this paper discusses cyber-physical security of Wide-Area Monitoring, Protection and Control (WAMPAC) from a coordinated cyber attack perspective and introduces a game-theoretic approach to address the issue. Finally, the paper briefly describes how cyber-physical testbeds can be used to evaluate the security research and perform realistic attack-defense studies for smart grid type environments.
