4. INDUSTRY 4.0 TECHNOLOGIES AND LAW IN ENHANCING HUMAN CAPACITY AMONG WOMEN IN THE NIGERIA CONSTRUCTION INDUSTRY: A SYSTEMATIC REVIEW.

Purpose: This study examines the impact of Industry 4.0 and the role of law in enhancing women's involvement in Nigeria's construction sector. Design/Methodology/Approach: The study adopted a systematic review of relevant literature from Scopus, IEEE Xplore, Science Direct, EBSCO, Emerald Insight, and Proquest Central between 2011 to 2023 on the various issues addressed in the research. A thematic analysis was adopted to discuss the findings from the reviewed articles in line with the study's objective. Findings: Industry 4.0 technologies enhance women's involvement in the construction industry by automating risky tasks, ensuring safety, and reducing working hours for family responsibilities. Limiting women's participation in construction results in unconstitutional gender-based discrimination and the law has not been fully explored in ensuring gender equality in the construction industry. Research Limitation/Implications: The study's reliance on a systematic literature review may limit capturing real-time industry dynamics. Practical Implication: Implementation of industry 4.0 technologies in the construction industry has the potential to create a safer, inclusive work environment by automating hazardous tasks and reducing working hours. Social Implication: Construction 4.0, if embraced, has the potential to challenge traditional gender norms, fostering a more equitable industry. Originality/Value: The study offers a novel approach that assesses the collective impact of technology and the law on enhancing women's participation and capacity-building initiatives, thereby contributing to gender equality and empowerment in the industry. The choice of caesarean section CS plays a significant role in maternal and neonatal health However suboptimal CS uptake suggests unmet obstetric care leading to adverse maternal and neonatal health Considering that maternal health problems in Nigeria remain a public health problem this present study aims to assess the prevalence and multilevel factors associated with caesarean section among women of reproductive age in Nigeria Data from the 2018 Nigeria Demographic and Health Survey were analysed Our analyses included 19 964 women of reproductive age with their last birth within five years preceding the survey Multilevel logistic regression analysis was carried out to examine the predictors of the caesarean section in Nigeria The prevalence of CS among women of reproductive age in Nigeria was 3 11 Women from the Yoruba ethnic group aOR 0 52 95 CI 0 32 0 84 with two children aOR 0 67 95 CI 0 52 0 88 three children aOR 0 49 95 CI 0 36 0 66 four children and above aOR 0 34 95 CI 0 26 0 46 those who practised Islam aOR 0 74 95 CI 0 56 0 99 and those that had a normal weighted baby aOR 0 73 95 CI 0 60 0 99 were less likely to report having a CS in Nigeria compared to those from Hausa Fulani ethnic group those who had one child those who practised Christianity and those who had a high weighted baby Also women residing in rural areas aOR 0 79 95 CI 0 63 0 99 and the South South aOR 0 65 95 CI 0 46 0 92 were less likely to have CS compared to those residing in urban areas and North Central The study concluded that several individual and community level factors such as religious belief number of children ethnicity place of residence and region of residence were associated with CS utilisation in Nigeria Our study highlights the need for different regional local and cultural contexts for evidence based policy and programmatic efforts to facilitate equitable access to a caesarean section in Nigeria

8. University Industry Collaboration and Graduates' Unemployment in Ondo State, Nigeria

- Abstract
Research evidence shows that university industry collaboration presents a wide range of benefits to industries, higher institutions, and to the development of a nation. This study investigated the existence of university industry collaboration, the factors militating against the collaboration, and the extent to which university industry collaboration can reduce unemployment in Nigeria. A descriptive survey was adopted in the study. The study revealed that a weak relationship exists between the universities and industries in Nigeria. It was also revealed that university industry relationships significantly reduce the level of unemployment among graduates in Nigeria. In addition, lack of interest on the part of industries and lack of awareness of the importance of university industry relationship were the major factors militating against university industry collaboration. Therefore, strengthening university industry will create more industries and entrepreneurial businesses, thereby minimizing the rate of unemployment in the country.

Background This study provides a global overview of the management of patients with acute cholecystitis during the initial phase of the COVID-19 pandemic. Methods CHOLECOVID is an international, multicentre, observational comparative study of patients admitted to hospital with acute cholecystitis during the COVID-19 pandemic. Data on management were collected for a 2-month study interval coincident with the WHO declaration of the SARS-CoV-2 pandemic and compared with an equivalent pre-pandemic time interval. Mediation analysis examined the influence of SARS-COV-2 infection on 30-day mortality. Results This study collected data on 9783 patients with acute cholecystitis admitted to 247 hospitals across the world. The pandemic was associated with reduced availability of surgical workforce and operating facilities globally, a significant shift to worse severity of disease, and increased use of conservative management. There was a reduction (both absolute and proportionate) in the number of patients undergoing cholecystectomy from 3095 patients (56.2 per cent) pre-pandemic to 1998 patients (46.2 per cent) during the pandemic but there was no difference in 30-day all-cause mortality after cholecystectomy comparing the pre-pandemic interval with the pandemic (13 patients (0.4 per cent) pre-pandemic to 13 patients (0.6 per cent) pandemic; P = 0.355). In mediation analysis, an admission with acute cholecystitis during the pandemic was associated with a non-significant increased risk of death (OR 1.29, 95 per cent c.i. 0.93 to 1.79, P = 0.121). Conclusion CHOLECOVID provides a unique overview of the treatment of patients with cholecystitis across the globe during the first months of the SARS-CoV-2 pandemic. The study highlights the need for system resilience in retention of elective surgical activity. Cholecystectomy was associated with a low risk of mortality and deferral of treatment results in an increase in avoidable morbidity that represents the non-COVID cost of this pandemic.

11. Role of a University of Technology in Human Capital Development in Nigeria

- Abstract
The greatness of nations appears to be based on the level of their human capital development as the world continues its march in the knowledge economy. It has become imperative for Nigeria to remain competitive in the comity of nations in the production, transfer and utilisation of knowledge. The realisation of the importance of human capital development, especially in the key areas of the economy, such as mining, telecommunications and agriculture has led to the establishment of the Universities of Technology in Nigeria. These universities promote technological advancement, through the teaching and training of manpower in various fields of Science and Technology. Science and Technology are recognized as the main catalysts for national development. This paper examined the role of the Federal University of Technology, Akure, (FUTA) in the development of human capital in Nigeria. The paper reviews the training programmes and the graduates produced at the University since its inception. It argues that FUTA, which was adjudged as the best University of Technology in 2004 and one of the best 100 Universities in Africa by 2010 Webometric ranking has qualified as a veritable institution in Human Capital Development. The study reveals that the graduates produced over the years have been in high demand in the various sectors of the economy; particularly oil and gas, construction, mining and agriculture. It posits that with necessary infrastructure, the institution will continue to produce competitive manpower that is needed to transform Nigeria to one of the best 20 world economies by the year 2020.

BackgroundBreastfeeding self-efficacy (BFSE) is a key variable that enhances exclusive breastfeeding (EBF) and promotes positive health outcomes for infants and their mothers. To increase BFSE and EBF of mothers, numerous interventions targeting prenatal and postnatal periods have been developed. However, there is paucity of studies utilizing drama interventions for improving BFSE and EBF.ObjectivesThis study assessed the effect of a drama intervention on BFSE, initiation, and EBF of pregnant women in rural communities in Lagelu and Egbeda Local Government Areas (LGAs), Ibadan, Oyo State, Nigeria.MethodologyA quasi-experimental study was conducted with pregnant women in their second trimester. Selected communities from Lagelu and Egbeda LGAs were randomized into experimental and control groups. A total of 200 pregnant women (100 experimental and 100 control groups) were enlisted and followed-up at one, three and six months postnatal periods. A six-session programme comprising four episodes of drama and two sessions on hygiene practices were presented to experimental groups prior to delivery, while the control group received health talk on hygiene practices. Computer-Assisted Personal Interview (ODK) was used to obtain information on socio-demographic, BFSE, initiation, and EBF at prenatal and postnatal periods. BSFE scores were categorized as low (14-32), average (33-51), and high (52-70). Descriptive and inferential statistics was used to analyze data at α0.05.ResultsMean ages of women were 28.4 ±6.5 and 27.0±6.2years in experimental and control groups respectively. Average and high BFSE pre-intervention (11.0%; 89.0% and 9.0%; 91.0%) and six months post-intervention (97.3%;100% and 95.2%; 95%) for experimental and control groups. Age, marital status, and occupation were predictors of BFSE, breastfeeding initiation and EBF at (R2 = 22.3; pConclusionThe experimental group had an increase in BFSE, initiation, and EBF practice compared to control group. The use of drama intervention is recommended for effective breastfeeding practices.

Considering the rural poor in developing countries like Nigeria, diets that are deficient in dietary fibre and micronutrients are common. Food diversification or dietary modifications such as increased consumption of vegetables may solve the problem. This study assessed the dietary fibre and micronutrient levels of selected starchy foods (jollof rice and yam porridge) using three underutilized green leafy vegetables, namely, Ebolo (Crassocephalum crepidiodes), Odu (Solanum nigrum L), and Yanrin (Launaea teracifolia L) as sprinkles. Fresh samples of Crassocephalum crepidioides, Solanum nigrum L, and Launaea teracifolia were purchased from Ota market. The leafy parts of the vegetables were washed, cut, oven-dried at 90°C for 6 h and pulverized. Jollof rice and porridge were cooked and 2g each of dried ebolo, odu and yanrin were sprinkled on 100g of each food and stirred thoroughly. Total dietary fibre, as well as iron (Fe) and Zinc (Zn) content of the resultant diets, were determined using standard methods. The highest total dietary fibre of 8.45% was obtained in jollof rice sprinkled with Yanrin. Zn (31.75mg/kg) was highest in jollof rice sprinkled with Yanrin, and Fe (87.75mg/kg) in porridge sprinkled with Ebolo. The underutilized vegetables added more nutritional values to jollof rice and yam porridge as Ebolo, Yanrin and Odu were sprinkled on them. [ABSTRACT FROM AUTHOR]

Adequate nutrition is important during infancy and childhood where a short period of malnutrition has long lasting effects on growth, development and health in the adult life. The period from six months to two years constitutes a critical window of opportunity for promoting optimal growth, health and development, while insufficient quantity and quality of complementary foods (CFs), poor feeding practice and high rate of infection have a detrimental impact on growth in these important years. This study was designed to evaluate the nutrient composition and suitability of four commonly used complementary foods in two Local Government Areas (LGAs) of Akwa Ibom State, Nigeria. A descriptive cross-sectional survey involving women with children aged 6-24 months old was carried out in two randomly selected LGAs to identify the commonly used CFs. A total of 300 consenting mothers (150/LGA) aged 18 to 60 years presenting their children for immunisation in the two LGAs were respondents. A pretested, semi-structured questionnaire was used to obtain information on sociodemographic characteristics, knowledge on breastfeeding, complementary feeding practice, and types of CFs used from the respondents. Standardised samples of the four most commonly used CFs were prepared and analysed for nutrient and anti-nutrient composition using standard AOAC methods of analyses. Data were analysed using descriptive statistics and Chi square test, with level of significance set at p = 0.05. Two types of unripe banana porridge, one mashed bean porridge, and mixed cereal pap with crayfish and ‘turn brown’ (soybean flour, groundnut and crayfish) were the four most commonly used CFs. One hundred grams portion of the CFs contained between 2.52 - 6.70 g crude protein, 1.26 - 7.23 g crude lipid, 8.16 - 13.97 g carbohydrates and yielded up to 415.57 kcal of energy. The mineral content ranged between 31.58 - 230.40 mg potassium, 46.78 - 184.68 mg calcium, 55.23 - 120.93 mg phosphorus, 10.37 - 23.26 mg iron, 7.53 - 18.53 mg of zinc / 100g portion. The four complementary foods were nutritionally adequate and were low in anti-nutrients (oxalates, phytates, trypsin inhibitors, saponins and tannins), hence there is little risk of malabsorption of available nutrients. Utilization of unripe banana and mixed cereals with turn brown for infants provides cheap CFs with adequate energy and nutrients, and promotes biodiversity.Key words: Nutrients, Complementary foods, Banana, Cereal, Beans.

Protein malnutrition is widespread among the rural poor in developing countries and legumes serve as a major source of dietary protein where animal protein is very expensive. Pigeon pea is an important legume with high amount of protein, but its consumption is limited due to its hardness and time-consuming process of cooking. This study was carried out to determine effect of cooking methods on time and nutrient retention of pigeon pea as a means of promoting dietary diversity. Pigeon pea seeds were purchased from Oja Oba market in Ado-Ekiti, Ekiti State, Nigeria, cleaned, sorted, and divided into five portions. One portion was labelled as raw sample. The second portion was washed and cooked with distilled water on an electric cooker at 300°C for 2 hours and labelled as Sample 2. The third portion was washed and cooked at 3000C for two hours, with decanting and replenishing the water, and labelled as sample 3. The fourth portion was washed and pressure-cooked with distilled water at 300°C for 1hour and labelled as Sample 4; while the fifth portion was pressure-cooked at 300°C for 45 minutes, decanting and replenishing the water, cooked for 15 minutes, then labelled as sample 5. The five samples were analysed in triplicates for proximate, minerals and selected vitamins composition using standard methods of AOAC. Raw pigeon pea contained 11.9g moisture, 22.1g crude protein, 3.4g fat, 3.4g ash, 59.0g carbohydrates, 47.76mg sodium, 1025.63mg potassium, 100.25mg calcium, 377.87mg phosphorus, 13.01mg iron, 11.95mg zinc, and yielded 315.8kcal energy/100g sample. Boiling, and decanting the boiling water, and pressure cooking led to significant reduction in all macronutrients (p<0.05), the reduction being most pronounced in samples with cooking water decanted. Pressure-cooked samples retained more macronutrients with highest retention recorded in pressure-cooked sample without decanting the water (p<0.05). Boiling without decanting the water had highest retention of minerals, followed by pressure-cooked sample without decanting the water, while boiled sample with decanted water retained least minerals. Pressure cooking the pigeon pea significantly reduced cooking time (p<0.05), thereby reducing cost of electricity. Pigeon pea is a good source of protein, energy, potassium, calcium, phosphorus, iron and zinc, and can contribute significantly to meeting nutrient needs of consumers; hence, its consumption should be encouraged as a means of dietary diversity among the populace where it is available. A proper understanding of the freight flow in terms of their production and distribution is crucial to spatial development and planning of an area. This study therefore, attempts to develop a model for estimating the distribution of rural freight generated in rural markets of South Western Nigeria. A questionnaire survey of commodities loaded and transported as well as rural freight operators in some rural market locations using systematic random sampling of 1 out of every 10 freight vehicles was carried out to determine their types, volume, origin and destinations of rural goods. Based on the data collected, a rural freight distribution model is developed using regression model. A logarithm method used for the data analyzed shows that out of three variables used, only two of them that is; the Cost of Transportation (CP) and Population of Destination of Goods (POP) are significant at 1% and 5% level respectively in explaining the variation in the rural freight distribution in the study area, while the Proximity of the Attracting State (PS) is not found to be significant. The three variables however accounted for 65% of the explanation of variation in the rural freight distribution in the South Western Nigeria. Based on these findings, it was recommended that deliberate policy measures should be put in place by the government to reduce the cost of rural goods transportation by improving rural road condition, enhancing rural market accessibility and rural goods operation. This study assessed rural youth involvement in the use of Information and Communication Technologies (ICTs) in agriculture in Osun State, Nigeria. Specifically, it examined awareness and analysed attitude of rural youths in the use of ICTs to disseminate agricultural information. A multi-stage sampling procedure was used to select one hundred and twenty respondents for the study. The data were collected using structured interview schedule and analysed using descriptive and inferential statistics. The mean age of respondents was 18.0 years. Also, 71.7 percent of the respondents scored high in ICTs awareness in relation to agriculture. Telephone (56.0%) was commonly used as a mean of passing agricultural information among the youths and the level of computer/literacy was slightly above average (58.3). Result of the correlation analysis shows that there was a positive and significant relationship (r =0.481; p> 0.05) between the awareness of the usage of ICTs in agriculture and their attitude toward the use of ICTs. The study therefore concluded among others, that rigorous awareness on the inportance of ICTs in farming should be created among youths in Osun State in order enhance and bring about positive solutions to agricultural development stakeholders efforts in combating food insecurity in Nigeria. Adequate nutrition is important during infancy and childhood where a short period of malnutrition has long lasting effects on growth, development and health in the adult life. The period from six months to two years constitutes a critical window of opportunity for promoting optimal growth, health and development, while insufficient quantity and quality of complementary foods (CFs), poor feeding practice and high rate of infection have a detrimental impact on growth in these important years. This study was designed to evaluate the nutrient composition and suitability of four commonly used complementary foods in two Local Government Areas (LGAs) of Akwa Ibom State, Nigeria. A descriptive cross-sectional survey involving women with children aged 6-24 months old was carried out in two randomly selected LGAs to identify the commonly used CFs. A total of 300 consenting mothers (150/LGA) aged 18 to 60 years presenting their children for immunisation in the two LGAs were respondents. A pretested, semi-structured questionnaire was used to obtain information on sociodemographic characteristics, knowledge on breastfeeding, complementary feeding practice, and types of CFs used from the respondents. Standardised samples of the four most commonly used CFs were prepared and analysed for nutrient and anti-nutrient composition using standard AOAC methods of analyses. Data were analysed using descriptive statistics and Chi square test, with level of significance set at p = 0.05. Two types of unripe banana porridge, one mashed bean porridge, and mixed cereal pap with crayfish and 'turn brown' (soybean flour, groundnut and crayfish) were the four most commonly used CFs. One hundred grams portion of the CFs contained between 2.52 - 6.70 g crude protein, 1.26 - 7.23 g crude lipid, 8.16 - 13.97 g carbohydrates and yielded up to 415.57 kcal of energy. The mineral content ranged between 31.58 - 230.40 mg potassium, 46.78 - 184.68 mg calcium, 55.23 - 120.93 mg phosphorus, 10.37 - 23.26 mg iron, 7.53 - 18.53 mg of zinc / 100g portion. The four complementary foods were nutritionally adequate and were low in anti-nutrients (oxalates, phytates, trypsin inhibitors, saponins and tannins), hence there is little risk of malabsorption of available nutrients. Utilization of unripe banana and mixed cereals with turn brown for infants provides cheap CFs with adequate energy and nutrients, and promotes biodiversity. [ABSTRACT FROM AUTHOR]

There is a global drive for promotion of indigenous foods and feedstuffs as a means of dietary diversification in meeting dietary needs of the people living the traditional lifestyle. Cassava diets constitute a staple source of energy for most Nigerians. However, there is little or no documentation on the nutrient composition, effect of processing methods on nutrient retention and contribution of these diets to nutrient intake of consumers. Nutrition information on contribution of a particular food or diet to nutrient intake of consumers is of paramount importance in food labeling and consumer acceptability. This study, therefore, aimed at providing information on nutrient composition and effect of processing methods on nutrient retention and contribution of some diets prepared from cassava. Fresh cassava roots were obtained from a farm in Alegongo area, Akobo, Ibadan, Oyo State, Nigeria. Proximate and mineral composition of prepared samples was determined alongside the market samples using standard methods of Association of Official Analytical Chemists (AOAC) and atomic absorption spectrophotometry, respectively. The crude protein, lipid, fibre and ash contents of fresh cassava roots were low (0.9, 0.3, 0.5 and 0.4g/100g, respectively). Its mineral profile was: potassium 166.6, sodium 222.1, calcium 25.0, magnesium12.5, phosphorus 57.3, iron 1.7, and zinc 2.1 mg/100g sample. Processing cassava roots into various products improved availability of nutrients such as protein (1.3g in gari to 2.6g in fufu and amala), ash (0.5g in abacha to 2.6g in eba), potassium (234.5mg in three days fermented garri to 473.2mg in two days fermented lafun), calcium (22.7mg in eba to 67.3mg in two days fermented lafun), iron (1.0 - 4.3mg), zinc (2.5 - 6.7mg), as well as their calories (p<0.05). A 100g portion of raw and processed cassava into amala, eba, fufu and abacha yielded 140.5, 289, 284, 312, and 358 kilocalories of energy, respectively. Soaking fresh cassava for more than two days resulted in significant reduction in mineral content of prepared diets due to leaching. 100g portion of various diets can contribute between 12.3 to 16.1% energy, 6 to 14% iron, and up to 28% zinc to % RDAs of consumers. Background: The organoleptic evaluation of herbal drugs is as old as science, but the authentication of herbs on the basis of their fluorescence characteristics is difficult and sometimes impracticable because humans are limited by their sense of colour recognition. Aim: This article undertakes a computer-aided examination of some powdered herbal materials with a view to characterising them calourimetrically, thus providing a reliable organoleptic clue for their authentication, against possible misidentification. Setting: Research was conducted in Ogbomoso, Nigeria. Methods: Seventeen herbal materials (i.e. stem bark, root/root bark, vines, fruit calyx, leaf sheath and seed) used for two traditional powdered drugs were collected and pulverised into powders. Their colours were digitised by scanning the surface of the powders in petri dishes using a CamScanner installed on a Samsung Galaxy Tablet 10.1 Model 7500, and were qualitatively and quantitatively analysed by uploading the images into the online Cool Hypertext Preprocessor (Cool PHP) software tool, setting the number of colours at five, thus giving consideration to only five dominant colour shades in each image, all expressed as hexadecimal codes. The codes were uploaded into Chir.ag/art, another online tool, to read off the colour names. The relative mean percentage, frequency and relative colour intensity (RCI) of each colour shade were calculated, and the colour with the highest RCI was taken as the first or typical colour of each herb. Results: Nine of the 62 colours observed, namely Lucky, Sandrift, Cannon Pink, Potters Clay, Mandalay, Ferra, Domino, Russet and Roti, were highly restricted in distribution, each being the first or typical colour in only one species of the herbs (i.e. Enantia chlorantha, Garcinia kola, Hibiscus sabdariffa, Khaya senegalensis, Sarcocephalus latifolius, Sorghum bicolor, Theobroma cacao, Uvaria chamae and Zanthoxylum zanthoxyloides respectively). These colours were therefore substantially diagnostic of those herbs. Another nine colours among the most frequently observed colours and the number of species that had them were Pesto (9), Shadow (8), Driftwood (8), Barley Corn (5), Domino (4), Roman Coffee (4), Cape Palliser (4), Himalaya (4) and Husk (4); these were less diagnostic of the herbs in question. Based on the distribution of these colours, a diagnostic PHP colour chart was constructed for the authentication of the powdered medicinal herbs. Conclusion: Powders of the 17 medicinal herbs analysed have been characterised colourimetrically with each species being unambiguously diagnosed. The study has therefore circumvented the subjectivity of the human sense of colour recognition in medicinal herb authentication.

Arising from the menace of city logistics problems in Lagos State with reference, in particular, to the Apapa and Tin Can Island seaport axis, the federal and state governments, in collaboration with private investors, seek to establish and/or develop some potential seaports to ease the burden of maritime logistics. Therefore, the objectives of this paper are to examine the prospects and challenges of the development of these proposed seaports and to analyze the efficiencies of the two selected seaports in order to determine the need for the required investment in seaport development. Descriptive analysis was used to examine the challenges of the selected seaports, while stochastic frontier analysis (SFA) was used to determine the efficiency of the selected seaports. The responses of the stakeholders and shipping companies to the various challenges were collected through a well-structured questionnaire, and the 2008–2017 cargo throughputs of the selected seaports were used as the secondary data for stochastic frontier analysis (SFA). It was discovered that the challenges associated with the Calabar Seaport were the draught level, cost of shipment, accessibility to industries, and condition of other modes of transport. From the stochastic frontier, the Lagos Apapa seaport is quite efficient, with an efficiency value of 0.9764921, while Calabar is slightly above average, with a mean efficiency value of 0.6086686. By implication, the congestion in the seaports in the Lagos seaport complex with the maximum level of efficiency creates the need for another seaport, which must be sited at a well-vetted location. In the case of the Calabar seaport, the efficiency level shows that the seaport is yet to be fully utilized. Hence, investment decisions regarding whether to build a new seaport or use dredging to upgrade the existing ones must be carefully analyzed, as the establishment of the proposed Ibom deep seaport may further affect the efficiency of the Calabar seaport(s). In conclusion, demand should be the driving force for port establishment: when a port cannot generate enough traffic, it may not yield returns on investment as expected.

Male to female ratio was 1.2:1 in schizophrenia and 1:3 in mood disorder. Those with schizophrenia were younger and with an earlier age of onset of symptoms than those with mood disorders. "nConclusion:Catatonia associated with mood disorder was found to be increasing over the years when compared with schizophrenia. Differences were observed in socio-demographic characteristics and number of predominant catatonic symptoms. Having a separate category for catatonia due to the mood disorders in the current diagnostic guidelines (10th edition of the International Classification of Diseases and the 4th edition of the Diagnostic and Statistical Manual) will help in better diagnosis of catatonia.

Abstract Background Urinary tract infections (UTIs) are the most commonly treated infection among nursing home residents. Even in the absence of specific (e.g., dysuria) or non-specific (e.g., fever) signs or symptoms, residents frequently receive an antibiotic for a suspected infection. This research investigates factors associated with the use of antibiotics to treat asymptomatic bacteriuria (ASB) among nursing home residents. Methods This was a cross-sectional study involving multi-level multivariate analyses of antibiotic prescription data for residents in four nursing homes in central Texas. Participants included all nursing home residents in these homes who, over a six-month period, received an antibiotic for a suspected UTI. We investigated what factors affected the likelihood that a resident receiving an antibiotic for a suspected UTI was asymptomatic. Results The most powerful predictor of antibiotic treatment for ASB was the presence of an indwelling urinary catheter. Over 80 percent of antibiotic prescriptions written for catheterized individuals were written for individuals with ASB. For those without a catheter, record reviews identified 204 antibiotic prescriptions among 151 residents treated for a suspected UTI. Almost 50% of these prescriptions were for residents with no documented UTI symptoms. Almost three-quarters of these antibiotics were ordered after laboratory results were available to clinicians. Multivariate analyses indicated that resident characteristics did not affect the likelihood that an antibiotic was prescribed for ASB. The only statistically significant factor was the identity of the nursing home in which a resident resided. Conclusions We confirm the findings of earlier research indicating frequent use of antibiotics for ASB in nursing homes, especially for residents with urinary catheters. In this sample of nursing home residents, half of the antibiotic prescriptions for a suspected UTI in residents without catheters occurred with no documented signs or symptoms of a UTI. Urine studies were performed in almost all suspected UTI cases in which an antibiotic was prescribed. Efforts to improve antibiotic stewardship in nursing homes must address clinical decision-making solely on the basis of diagnostic testing in the absence of signs or symptoms of a UTI.

Background: COVID-19 pandemic widely disrupted health services provision, especially during the lockdown period, with females disproportionately affected. Very little is known about alternative healthcare sources used by women when access to conventional health services became challenging. This study examined the experiences of women and adolescent girls regarding access to sexual and reproductive health (SRH) services during the COVID-19 lockdown in Nigeria and their choices of alternative healthcare sources., Methods: The study sites were two northern states, two southern states, and the Federal Capital Territory. Qualitative data were obtained through 10 focus group discussion sessions held with married adolescents, unmarried adolescents, and older women of reproductive age. The data were transcribed verbatim and analysed using a thematic approach and with the aid of Atlas ti software., Results: Women reported that access to family planning services was the most affected SRH services during the COVID-19 lockdown. Several barriers to accessing SRH services during COVID-19 lockdown were reported, including restriction of vehicular movement, harassment by law enforcement officers, fear of contracting COVID-19 from health facilities, and fear of undergoing compulsory COVID-19 tests when seeking care in health facilities. In the face of constrained access to SRH services in public sector facilities during the COVID-19 lockdown, women sought care from several alternative sources, mostly locally available and informal services, including medicine vendors, traditional birth attendants, and neighbours with some health experience. Women also widely engaged in self-medication, using both orthodox drugs and non-orthodox preparations like herbs. The lockdown negatively impacted on women's SRH, with increased incidence of sexual- and gender-based violence, unplanned pregnancy resulting from lack of access to contraceptives, and early marriage involving adolescents with unplanned pregnancies., Conclusion: COVID-19 negatively impacted access to SRH services and forced women to utilise mostly informal service outlets and home remedies as alternatives to conventional health services. There is a need to ensure the continuity of essential SRH services during future lockdowns occasioned by disease outbreaks. Also, community systems strengthening that ensures effective community-based health services, empowered community resource persons, and health-literate populations are imperative for overcoming barriers to healthcare access during future lockdowns., (© 2024. Background: Surgical site infection (SSI) persists as a global challenge, accounting for 20%-25% of all healthcare-associated infections. The SSI rate has been reported to range from 2.5% to 41.9%. Skin preparation with acceptable antiseptic preparations has a high recommendation from the Centers for Disease Control as an SSI preventive measure., Aim: The aim was to compare the efficacy of 10% povidone-iodine in 70% isopropyl alcohol with 2% chlorhexidine in 70% isopropyl alcohol in preventing SSI., Method: This prospective randomized study included patients who were followed up for 30 days looking for SSI. Swabs were taken from wounds that developed SSI. A culture of all swabs was done., Result: One hundred and fifty-three patients were recruited into the study. Overall, eight (5.23%) of the 153 patients developed SSI. The SSI rate in clean wounds was 2.6%, while the SSI rate in clean-contaminated wounds was 7.9%. This study examined access to, and use of remote patient monitoring (RPM), medical applications, and wearables in a racially diverse, lower-income population. Data were obtained via a cross-sectional survey of adults from low-income communities in Houston, Los Angeles, and New York between April and August 2023. The survey examined access to, and use of RPM, medical applications, and wearables, among respondents. Binary responses to the following questions were examined using logistic regression models: In the past 12 months, have you (i) used RPM, (ii) used a medical app, and (iii) used an electronic wearable device to monitor or track health or activity? A total of 305 surveys were returned, of which 212 were complete (69.5% completion rate). Demographically, 22% self-identified as Hispanic, 41% as non-Hispanic Black individuals, and 33% as non-Hispanic White individuals. Overall, 69% of respondents reported a pre-tax annual household income of less than $35 000 and 96% indicated they own a smart phone. However, only 3 of 10 reported using RPM, 15% reported using a medical app, and 14% reported using wearables. Race was strongly associated with RPM usage, with Black respondents significantly less likely to have used RPM, compared to their white counterparts (OR: 0.31, P = .002). Education (bachelor's degree or more OR: 4.79, P = .03) and higher income ($35 001 + OR: 4.68, P = .008) were strongly associated with medical app usage. In the wearables model, the same trend was observed with education (bachelor's degree or more OR: 4.45, P = .04), and higher income ($35 001 + OR: 5.49, P = .01). Compared to earlier studies that have reported utilization rates of between 50% and 60%, our finding of much lower utilization in economically disadvantaged populations that are at greater risks for sub-optimal health outcomes gives cause for greater concern.
