To evaluate the efficacy and safety of pars plana vitrectomy for symptomatic floaters. Forty-eight vitreoretinal surgeons from 16 countries provided information on 581 eyes who underwent vitrectomy for floaters in this retrospective survey study conducted by European VitreoRetinal Society. Percentage symptomatic improvement, incidence of retinal tears/detachment and post-vitrectomy cataract surgery, and the factors associated with satisfaction and complications were investigated. Ninety-two percent were satisfied with the results, with 86.3% reporting complete resolution of daily-life symptoms. Overall satisfaction was lower in patients with smaller vitreous opacities at presentation (OR:0.4). Iatrogenic retinal breaks occurred in 29 eyes (5%). Core vitrectomy and cut rates of 1500–4000 or >4000 cuts/min were associated with lower risk of retinal breaks than complete vitrectomy (OR:0.05) and cut rates 1500 cuts/min are favoured. Proper patient selection and informed consent are the most important aspects of surgery.

Abstract

Purpose: To study and compare the efficacy of different therapeutic options for the treatment of central serous chorioretinopathy (CSCR). Methods: This is a nonrandomized, international multicentre study on 1719 patients (1861 eyes) diagnosed with CSCR, from 63 centres (24 countries). Reported data included different methods of treatment and both results of diagnostic examinations [fluorescein angiography and/or optical coherent tomography (OCT)] and best-corrected visual acuity (BCVA) before and after therapy. The duration of observation had a mean of 11 months but was extended in a minority of cases up to 7 years. The aim of this study is to evaluate the efficacy of the different therapeutic options of CSCR in terms of both visual (BCVA) and anatomic (OCT) improvement. Results: One thousand seven hundred nineteen patients (1861 eyes) diagnosed with CSCR were included. Treatments performed were nonsteroidal anti-inflammatory eye drops, laser photocoagulation, micropulse diode laser photocoagulation, photodynamic therapy (PDT; Standard PDT, Reduced-dose PDT, Reduced-fluence PDT), intravitreal (IVT) antivascular endothelial growth factor injection (VEGF), observation and other treatments. The list of the OTHERS included both combinations of the main proposed treatments or a variety of other treatments such as eplerenone, spironolactone, acetazolamide, beta-blockers, anti-anxiety drugs, aspirin, folic acid, methotrexate, statins, vitis vinifera extract medication and pars plana vitrectomy. The majority of the patients were men with a prevalence of 77%. The odds ratio (OR) showed a partial or complete resolution of fluid on OCT with any treatment as compared with observation. In univariate analysis, the anatomical result (improvement in subretinal fluid using OCT at 1 month) was favoured by age 500 μm (p = 0.03). The OR for obtaining partial or complete resolution showed that anti-VEGF and eyedrops were not statistically significant; whereas PDT (8.5), thermal laser (11.3) and micropulse laser (8.9) lead to better anatomical results with less variability. Abstract

Urinary tract infection is a common and frequently recurring condition in children. The susceptibility of the host, the presence of urinary tract abnormalities, and the virulence of the urinary pathogens are of primary importance in the development of the infection. Renal parenchymal scarring, hypertension, and renal insufficiency are well-established complications of the infection in children. To reduce the risk of renal damage, diagnosis and treatment must be prompt. The diagnosis demands radiologic evaluation of the urinary tract in all boys, all children younger than 5 years, all patients with voiding dysfunction, and school-aged girls with recurrent infection to identify those patients with vesicoureteral reflux, obstruction, or other urinary tract abnormalities. Both voiding cystourethrography and renal ultrasonography are the initial examinations to use to determine the next appropriate study. Children with vesicoureteral reflux or with recurrent urinary tract infections should receive prophylactic antibiotic therapy and should be observed closely to prevent renal scarring.

Abstract

Objective Colocation of mental health screening, assessment, and treatment in primary care reduces stigma, improves access, and increases coordination of care between mental health and primary care providers. However, little information exists regarding older adults' attitudes about screening for mental health problems in primary care. The objective of this study was to evaluate older primary care patients' acceptance of and satisfaction with screening for depression and anxiety. Methods The study was conducted at an urban, academically affiliated primary care practice serving older adults. Study patients ( N = 107) were screened for depression/anxiety and underwent a post-screening survey/interview to assess their reactions to the screening experience. Results Most patients (88.6%) found the length of the screening to be 'just right.' A majority found the screening questions somewhat or very acceptable (73.4%) and not at all difficult (81.9%). Most participants did not find the questions stressful (84.9%) or intrusive (91.5%); and a majority were not at all embarrassed (93.4%), upset (93.4%), or uncomfortable (88.8%) during the screening process. When asked about frequency of screening, most patients (72.4%) desired screening for depression/anxiety yearly or more. Of the 79 patients who had spoken with their physicians about mental health during the visit, 89.8% reported that it was easy or very easy to talk with their physicians about depression/anxiety. Multivariate results showed that patients with higher anxiety had a lower positive reaction to the screen when controlling for gender, age, and patient-physician communication. 22. Learning and teaching across generations

Abstract

This project examines how young and old people influence each other's learning in shared educational experiences. More specifically, it tests the hypothesis that attitudes about spirituality and sexuality transcend generational and age-related differences. Each of a dozen undergraduate students from the University of Michigan are paired with one or more of two dozen residents of a local assisted living facility, University Living. Partnerships meet regularly every week throughout an academic year in an effort to establish personal relationships. Interested students and residents also participate in weekly discussion groups on current events, spirituality, and analysis of select novels by John Updike. Partnership and group discussions emphasize age- and generation-related perceptions of behavior by young and old across broad domains of spirituality and sexuality e.g., individual recollections, media reports of current events, and Updike's literary image. Results generally support the hypothesis by demonstrating a similar diversity of attitudes and recollected behaviors among both generations of participants, and further suggest the superficiality of at least certain perceived generational differences.

Abstract

This study assessed the behavioural and psychological profiles of children conceived by in-vitro fertilization (IVF) who are now at school age. A total of 743 IVF children born at one institution and now of school age, over 4 years old, were surveyed with Achenbach questionnaires. Follow-up telephone interviews were conducted with non-responders. The results from the study group were compared to the questionnaire control group using one-tailed t-test with statistical significance set less than 0.05. There was an 84% overall response rate. Sixty-seven per cent returned questionnaires. An additional 17% completed a telephone interview. The study group had no statistically significant increase in the rate of behavioural or psychological problems compared with the control group. There were no statistically significant differences within the study group related to sex or to multiple gestation IVF births. This large group of school-age IVF children has normal psychological development with no identified adverse effect of their status as IVF children. Determining the role, if any, of IVF in the very small number of children with behavioural and psychological problems will require additional study. [ABSTRACT FROM AUTHOR]

38. Frusemide enhancement of netilmicin nephrotoxicity in dogs.

Abstract

Male mongrel dogs were given netilmicin (15 mg/kg) i.m. every 8 h for 10 days, or netilmicin (15 mg/kg)+ frusemide (2 mg/kg) i.m. every 8 h for 8 days. Dogs receiving netilmicin alone had minimal changes in renal function and histology. Dogs receiving netilmicin together with frusemide developed marked elevations in BUN (88·1 ± 24·1 v. 17·0 ± 1·4 mg/dl, <0·01), and serum creatinine (2·99 ± 0·88 v. 1·01 ± 0·06 mg/dl, <0·01), that were associated with severe diffuse acute tubular necrosis. Urinary -acetyl--glucosaminidase and glucuronidase activities were comparable to levels in dogs receiving netilmicin alone, but muramidase activity was significantly higher (223 ± 63 v. 58 ± 14 U, <0·05). 43. Gentamicin Nephrotoxicity in the Dog: Sequential Light and Electron Microscopy

Abstract

Sequential percutaneous renal biopsies in six dogs given gentamicin at 10 mg/kg every 8 hours (intramuscular) were examined before treatment and 5, 9 and 12 days after treatment. Renal function tests (blood urea nitrogen and serum creatinine) and urinary enzyme excretion (β glucuronidase, N acetyl-glucosaminidase, galactosidase and muramidase) were measured daily. Periodic acid Schiff (PAS)-stained, 3-μm sections of renal biopsies taken before drug treatment showed pale and swollen proximal tubular epithelium that occluded the tubular lumen. The apical surface of all proximal tubules (brush border) stained intensely. On day 5, brush border staining was decreased or absent and many proximal tubules had open lumina and hyaline globules in the cytoplasm. Ultrastructurally, the numbers of cytoplasmic lysosomes were increased and most contained large lamellar inclusions (myelin figures). Biopsies on day 9 were characterized by an increase in the size and number of cytoplasmic hyaline globules and an absence of brush border staining. Scattered proximal tubule cells were necrotic. Intact proximal tubules and other parts of the nephron frequently contained amorphous granular casts or necrotic debris. Ultrastructurally, proximal tubules had intact brush borders and showed a remarkable increase in the number and size of lysosomes of all types. On day 12 the severity of these changes progressed, and cytosegrosomes became numerous. Urinary enzyme levels (β glucuronidase and N acetyl-glucosaminidase) were elevated by day 2, galactosidase elevated by day 4 and muramidase elevated by day 5. Clinically significant elevations in serum creatinine (day 6) and blood urea nitrogen (day 12) were preceded by elevations in urinary enzyme excretion and by severe alterations in renal tubular morphology.
