The elective consisted of six 2-hour sessions taught by a single faculty member over eight months. Sessions focused on development of EI skills and teaching a Resilience “PROGRAM” (Positive thinking, Reframing, Optimism, Gratitude, Reflection, Altruism, Meaning). Participants’ EI levels were assessed before and after the elective using the Bar-On Emotional Quotient Inventory 2.0 (EQ-i 2.0).Results: Over a period of 2 years, 70 students participated in the elective. The overall mean EI score significantly improved after the educational elective (100.05 ± 12.94 versus 108.14 ± 12.36, p < 0.001). Compared to the baseline scores, there was significant improvement in all EI components, including all five composite scales, all fifteen content subscales, and the well-being score (all p < 0.05). In a post-intervention survey assessing student perception of the elective, most students found the elective to be helpful (95%, 64/67), most students felt the elective should continue to be available for future students (95%, 64/67), and most would recommend the elective to other students (93%, 62/67).Conclusion: An EI-Resilience curriculum offered as an elective to second year medical students was well received by students. Our outcomes showed significant improvement in students’ EI scores and all sub-scores, including all components of the stress management composite and well-being score. Teaching EI skills and Resilience strategies in the preclinical setting might be an opportune time for this type of educational intervention.Keywords: wellness, medical education, curriculum, elective

Studies have begun analyzing the level of EI demonstrated by resident physicians in various specialties. Very few studies have assessed EI levels of residents in multiple specialties within one institution. The purpose of our study was to assess the EI level of all residents at our institution and to determine which variables were associated with higher EI scores.Methods: In 2018, residents voluntarily participated in the study by completing two online surveys. The first survey was a de-identified survey monkey asking demographic and other training information. The second survey was an online emotional intelligence survey assessing their EI skills, the Emotional Quotient Inventory 2.0 (EQ-i 2.0).Results: A total of 176 of 486 residents (36.2%) completed the study. There was no significant association between EI level and age, gender, type of medical degree, having an advanced degree, or being in a specific specialty. We found that residents who took time off before matriculating into medical school had higher overall EI scores than those who did not take time off (p = 0.02). Similarly, those who took time off before medical school scored higher in interpersonal skills (p = 0.04), empathy (p = 0.03), decision-making (p = 0.02), and impulse control (p = 0.03).Conclusion: Residents who had taken time off before matriculating in medical school had higher overall EI scores and higher scores in the EI components of interpersonal skill, empathy, decision-making, and impulse control.Keywords: emotional intelligence, gap year, time off

However, doctors with high levels of emotional intelligence (EI) may be immune to burnout, as they possess coping strategies which make them more resilient and better at managing stress. Educating physicians in EI may help prevent burnout and optimize their overall wellness. The purpose of our study was to determine if educational intervention increases the overall EI level of residents; specifically, their stress management and wellness scores.Participant and methods: Residents from pediatrics and med-ped residency programs at a university-based training program volunteered to complete an online self-report EI survey (EQ-i 2.0) before and after an educational intervention. The four-hour educational workshop focused on developing four EI skills: self-awareness; self-management; social awareness; and social skills. We compared de-identified median score reports for the residents as a cohort before and after the intervention.Results: Thirty-one residents (20 pediatric and 11 med-ped residents) completed the EI survey at both time intervals and were included in the analysis of results. We saw a significant increase in total EI median scores before and after educational intervention (110 vs 114, P=0.004). Accordingly, we conducted manual online searches of the PubMed and Google Scholar databases using pre-defined inclusion criteria. The initial manual screenings of 1192 article titles and abstracts, and subsequent reviews of full-length pdf versions identified 80 articles that were acceptable for inclusion. These articles indicated that event medical teams recognized hyperthermia and EHS in the majority of running and triathlon field studies (range, 58.8 to 85.7%), whereas few reports of hyperthermia and EHS appeared in cycling and open water swimming field studies (range, 0 to 20%). Sports medicine position statements and consensus reports also exhibited these event-specific differences. Thus, we proposed mechanisms that involved physiological effector responses (sweating, increased skin blood flow) and biophysical heat transfer to the environment (evaporation, convection, radiation, and conduction). We anticipate that the above information will help race directors to distribute pre-race safety advice to athletes and will assist medical directors to better allocate medical resources (eg, staff number and skill sets, medical equipment) and optimize the management of hyperthermia and EHS.Keywords: heat illness, epidemiology, pathophysiology, thermoregulation, athlete

A third of patients with biochemical recurrence (BCR) after radiation (RT) have intraprostatic radiorecurrence (IPR) on PSMA PET/CT. Patients with IPR have worse metastasis-free survival (MFS) - a surrogate for progression to lethal prostate cancer (pCa). There are limited prospective data on the best management of this growing population. We hypothesized that a dose-escalated focal salvage high dose rate (HDR) brachytherapy approach to treat IPR would be safe and effective. F-SHARP is a multi-institutional phase I/II trial of focal dose-escalated salvage HDR for IPR conducted at 3 centers. Eligibility criteria included a history of localized pCa treated with any form of definitive RT, and biopsy-proven IPR with no regional or distant metastasis. All patients had a PET/CT. Patients received up to 30 Gy in 1-2 fractions (Fx) to a focal target, prioritizing OAR constraints. The primary objective was to determine the acute RT-related grade ≥3 CTCAE v4.03 GU and GI toxicity rates. Secondary endpoints included all-grade acute and late toxicity, QoL (IPSS and EPIC-26), and biochemical/radiographic disease control and survival measures. Generalized estimating equations were used for toxicity and QoL analyses. The Kaplan-Meier method was used to estimate PSA-relapse-free survival (PSA-RFS), radiographic progression-free survival (rPFS), and MFS. Cox proportional hazards models were used for univariable and multivariable analyses. From 2017-2023, 62 patients were enrolled. Prior RT included conventional/hypofractionated photons (60%), LDR (24%), protons (11%), and SBRT (3%). Median BCR PSA was 4.7 ng/mL (Range: 2.2-20.3) and time from prior RT was 8 years (Range: 1.4-26.5). Median tumor D98 was 23 Gy (IQR: 20-27), for 1 Fx (n = 24) and 32 Gy (IQR: 30-34) for 2 Fx (n = 38). 5 patients had concurrent hormone therapy (HT). Median follow-up was 26.1 months (95% CI = 21-36). There were no grade ≥3 acute or late toxicities. Acute/late grade 2 GU toxicity occurred in 58%/63%. Acute/late grade 2 GI toxicity occurred in 2%/3%. Acute/late grade 2 sexual toxicity occurred in 16%/32%. The table shows the proportions of patients with a minimal clinically important decline in each QoL measure. There was no difference between 1 and 2 Fx for the risk of toxicity or worsening QoL (all p > 0.05). 3-year PSA-RFS was 59%, rPFS was 65%, and MFS was 91%. 5 patients required palliative HT. There was no difference between 1 and 2 Fx for any of these events (all P >.05). Patients with PSA ≥10 ng/mL were more likely to experience any event (HR = 3.9, 95% CI = 1.2 to 12.2; P =.02). Re-irradiation is a growing indication for RT in pCa. Dose-escalated focal salvage HDR is safe and has encouraging early efficacy. At this time, there is no difference in efficacy or toxicity between 1 vs. 2 Fx. Investigations into clinical/transcriptomic prognostic factors and patterns of failure are ongoing.
