Abstract Background The decision to allocate hospitals for the initial reception of hostages abducted on the October 7th Hamas attack introduced an array of unprecedented challenges. These challenges stemmed from a paucity of existing literature and protocols, lack of information regarding captivity conditions, and variability in hostage characteristics and circumstances. Objective To describe the rapid development, implementation and evaluation of the Hostage-ReSPOND protocol, a comprehensive trauma-informed procedure for the care of hostages, including young children, their caregivers and families, immediately following their release from prolonged captivity. Methods A multidisciplinary expert focus group conducted a comprehensive literature review to develop the ReSPOND protocol, consisting of: Readiness of teams via multifaceted trainings, utilizing live simulations and video debriefings; Specialized professional teams experienced in providing holistic trauma-informed care; Personalized care tailored to individualized and developmentally-informed needs; Optimal safety rooted in creating a secure environment and trauma-informed response to young children, adolescents, caregivers and families; and Navigating Discharge, through coordination with community-based care systems. Results A designated facility at the Children’s hospital was carefully prepared for receiving 29 hostages, aged 3.9–80 years, 28% under the age of 18. Implementation of the ReSPOND protocol, which prioritized holistic psychosocial interventions above urgent medical care, proved feasible and effective in managing the diverse and complex needs of returnees as per provider report. Finally, systemic assessment of returnee’s immediate and long-term mental health needs proved highly challenging. Conclusions There is currently no literature addressing the response to released hostages, especially those involving infants, young children and families within a children’s hospital facility. This study has the potential to fill a crucial gap in knowledge by introducing a novel protocol which could offer valuable insights for public health organizations tasked with providing acute care to diverse individuals and families experiencing extreme, multi-layered mass traumatization.

ST-segment elevation myocardial infarction (STEMI) is associated with a systemic and local inflammatory response with edema. However, their role at the tissue level is poorly characterized. This study aims to characterize T2 values of the noninfarcted myocardium (NIM) and surrounding tissue and to investigate prognostic relevance of higher NIM T2 values after STEMI. A total of 171 consecutive patients with STEMI without prior cardiovascular events who underwent cardiac magnetic resonance after primary percutaneous coronary intervention were analyzed in terms of standard infarct characteristics. Edema of the NIM, liver, spleen, and pectoralis muscle was assessed based on T2 mapping. Follow-up was available for 130 patients. The primary endpoint was major adverse cardiac events (MACE), defined as cardiovascular death, myocardial infarction, unplanned coronary revascularization or rehospitalization for heart failure. The median time from primary percutaneous coronary intervention to cardiac magnetic resonance was 3 days (IQR: 2-5 days). Higher (above the median value of 45 ms) T2 values in the NIM area were associated with larger infarct size, microvascular obstruction, and left ventricular dysfunction and did not correlate with C-reactive protein, white blood cells, or T2 values of the pectoralis muscle, liver, and spleen. At a median follow-up of 17 months, patients with higher (>45 ms) NIM T2 values had increased risk of MACE (P < 0.001) compared with subjects with NIM T2 values ≤45 ms, mainly caused by a higher rate of myocardial reinfarction (26.3% vs 1.4%; P < 0.001). At multivariable analysis, higher NIM T2 values independently predicted MACE (HR: 2.824 [95% CI: 1.254-6.361]; P = 0.012). CrossFit is characterized by being a standardized training program that improves physical performance through the provision of several stimuli regardless of the participant's strength level. This study aimed to compare the acute response in total repetitions as a measurement of performance, jump ability, physiological demand (heart rate and blood lactate), and perceived effort considering the participants' strength level with individualized intensity in CrossFit. Thirty-five participants were assessed and asked to participate on two separate days in a standardized and relative 'As Many Repetitions As Possible' (AMRAP) CrossFit circuit. Both AMRAPs comprised strength, gymnastic and aerobic exercises, although only strength was individualized according to the participant's level. Before the statistical analysis, participants were allocated to higher- or lower-strength groups following the one-repetition maximum-bodyweight ratio in the push press exercise. Results support the existence of a strong relationship between strength level and total repetitions in both AMRAPs. In addition, differences in total repetitions and rate of perceived exertion between strength groups are discarded when AMRAP intensity is individualized while physiological demand and jump ability are maintained. Thus, the higher-strength participants may benefit from similar responses with a lower number of repetitions. [Display omitted] Exposure of the esophageal mucosa to food allergens can cause acute mucosal responses in patients with eosinophilic esophagitis (EoE), but the underlying local immune mechanisms driving these acute responses are not well understood. We sought to gain insight into the early transcriptomic changes that occur during an acute mucosal response to food allergens in EoE. Bulk RNA sequencing was performed on esophageal biopsy specimens from adult patients with EoE (n = 5) collected before and 20 minutes after intramucosal injection of various food extracts in the esophagus. Baseline biopsy specimens from control subjects without EoE (n = 5) were also included. At baseline, the transcriptome of the patients with EoE showed increased expression of genes related to an EoE signature. After local food injection, we identified 40 genes with a potential role in the early immune response to food allergens (most notably CEBPB, IL1B, TNFSF18, PHLDA2, and SLC15A3). These 40 genes were enriched in processes related to immune activation, such as the acute-phase response, cellular responses to external stimuli, and cell population proliferation. TNFSF18 (also called GITRL), a member of the TNF superfamily that is best studied for its costimulatory effect on T cells, was the most dysregulated early EoE gene, showing a 12-fold increase compared with baseline and an 18-fold increase compared with a negative visual response. Further experiments showed that the esophageal epithelium may be an important source of TNFSF18 in EoE, which was rapidly induced by costimulating esophageal epithelial cells with the EoE-relevant cytokines IL-13 and TNF-α. Our data provide unprecedented insight into the transcriptomic changes that mediate the acute mucosal immune response to food allergens in EoE and suggest that TNFSF18 may be an important effector molecule in this response. Abstract Background Although recent studies have investigated the effects of flywheel (FW) training on muscle function, the effects of transient FW exercise on jump performance in athletes are unknown. This study examined the effects of single and repeated bouts of FW squat exercises on jump performance and muscle damage in male collegiate basketball players. Methods The participants were 10 healthy college-age men (nonathletes) and 11 male basketball players (athletes). The intervention involved 100 squat exercises (10 repetitions × 10 sets) using an FW device. To examine the repeated-bout effects, the protocol was conducted again after a 2-week interval. Squat jumps, countermovement jumps, drop jumps, and rebound jumps were evaluated as jump performance, while isometric maximal voluntary contraction (MVC) torque in knee extension, muscle soreness, range of motion, thigh circumference, muscle thickness, and echo intensity were evaluated as markers of muscle damage. Objectives/hypothesisContrast agent (CA) aspiration is an established complication of upper gastrointestinal and videofluoroscopic swallow studies. The underlying molecular biological mechanisms of acute response to CA aspiration in the respiratory organs remain unclear. The aims of this study were to elucidate the histological and biological influences of three kinds of CAs on the lung and to clarify the differences in acute responses.Study designAnimal model.MethodsEight-week-old male Sprague Dawley rats were divided into five groups (n = 6 in each group). Three groups underwent tracheal instillation of one of three different CAs: barium (Ba) sulfate, nonionic contrast agents (NICAs), and ionic contrast agents (ICAs). A control group was instilled with saline and a sham group was instilled with air. All animals were euthanized on day 2 after treatment and histological and gene analysis was performed.ResultsNo animal died after CA or control/sham aspiration. Ba caused severe histopathologic changes and more prominent inflammatory cell infiltration in the lungs compared with the two other iodinated contrast agents. Increases in expressions of inflammatory cytokines (tumor necrosis factor [Tnf], interleukin-1β [Il1b], and interferon-γ [Ifng]) were observed in Ba aspiration rats, and upregulation of Il1b was seen in ICA aspiration rats. NICA did not cause obvious histologic changes or expressions of inflammatory cytokines and fibrosis-related genes in the lungs.ConclusionsBa caused significantly more acute lung inflammation in a rodent model than did ioinic and nonionic iodinated CAs. Microglial cells are recognized as very dynamic brain cells, screening the environment and sensitive to signals from all other cell types in health and disease. Apolipoprotein D (ApoD), a lipid-binding protein of the Lipocalin family, is required for nervous system optimal function and proper development and maintenance of key neural structures. ApoD has a cell and state-dependent expression in the healthy nervous system, and increases its expression upon aging, damage or neurodegeneration. An extensive overlap exists between processes where ApoD is involved and those where microglia have an active role. However, no study has analyzed the role of ApoD in microglial responses. In this work, we test the hypothesis that ApoD, as an extracellular signal, participates in the intercellular crosstalk sensed by microglia and impacts their responses upon physiological aging or damaging conditions. We find that a significant proportion of ApoD-dependent aging transcriptome are microglia-specific genes, and show that lack of ApoD in vivo dysregulates microglial density in mouse hippocampus in an age-dependent manner. Murine BV2 and primary microglia do not express ApoD, but it can be internalized and targeted to lysosomes, where unlike other cell types it is transiently present. Cytokine secretion profiles and myelin phagocytosis reveal that ApoD has both long-term pre-conditioning effects on microglia as well as acute effects on these microglial immune functions, without significant modification of cell survival. ApoD-triggered cytokine signatures are stimuli (paraquat vs. Ab oligomers) and sex-dependent. Acute exposure to ApoD induces microglia to switch from their resting state to a secretory and less phagocytic phenotype, while long-term absence of ApoD leads to attenuated cytokine induction and increased myelin uptake, supporting a role for ApoD as priming or immune training factor. This knowledge should help to advance our understanding of the complex responses of microglia during aging and neurodegeneration, where signals received along our lifespan are combined with damage-triggered acute signals, conditioning both beneficial roles and limitations of microglial functions. Background: Carvedilol has an established role in Chronic Liver disease (CLD) associated Portal Hypertension, however its role in specific subclasses of CLD has not been studied, neither the factors leading to non-response thereby predisposing such patients to variceal bleed with empiric therapy. Aims and Objectives: The purpose of this study was. 1. To evaluate the hemodynamic response to Carvedilol and optimum dosage in patients of CTP Class A Cirrhosis. 2. To determine any clinical variables that would help in differentiating responders from non-responders. Materials and Methods: In forty-three consecutive patients of chronic liver disease Child Turcotte Pugh (CTP) Class A with esophageal varices, Hepatic Venous Pressure Gradient (HVPG) was measured at baseline, 90 minutes after initial administration of 12.5 mg of Carvedilol and after 3 months of dose optimization. Results: Twenty-one (48.8%) patients demonstrated Acute Response and after dose optimization Chronic Response was seen in 29 (67.4%) patients with additional 8 (18.6%) patients responding to dose optimization. Fourteen (32.55%) patients failed to show any response. Low pre-drug Cardiac Output (CO), High pre-drug mean arterial pressure (MAP), more than 2.5mmHg drop in HVPG acutely, higher dose toleration of more than 18.5mg and lower change in HR predicted response. Conclusion: Patients with CTP Class A Cirrhosis with Portal Hypertension and Esophageal Varices tolerate Carvedilol well and nearly two-thirds show hemodynamic response when dose is optimized over a period of time, including patients who initially show no acute response. Nearly one-third of patients show no response even after dose optimization.

Blood flow restriction may augment the skeletal response to whole-body vibration. This study used a randomised, crossover design to investigate the acute response of serum sclerostin and bone turnover biomarkers to whole-body vibration with blood flow restriction. Ten healthy males (mean±standard deviation; age: 27±8 years) completed two experimental conditions separated by 7 days: (i) whole-body vibration (10 1-minute bouts of whole-body vibration with 30 s recovery) or (ii) whole-body vibration with lower-body blood flow restriction (10 cycles of 110 mmHg inflation with 30 s deflation during recovery). Fasting blood samples were obtained immediately before and immediately after exercise, then 1 hour, and 24 hours after exercise. Serum samples were analysed for sclerostin, cross-linked C-terminal telopeptide of type I collagen, and bone-specific alkaline phosphatase. There was a significant time × condition interaction for bone-specific alkaline phosphatase (p=0.003); bone-specific alkaline phosphatase values at 24 hours post-exercise were significantly higher following whole-body vibration compared to combined whole-body vibration and blood flow restriction (p=0.028). No significant time × condition interaction occurred for any other outcome measure (p>0.05). These findings suggest that a single session of whole-body vibration combined with blood flow restriction does not significantly affect serum sclerostin or bone turnover biomarkers. Purpose: Enhanced external counterpulsation is a non-invasive treatment that increases coronary flow in patients with coronary artery disease (CAD). However, the acute responses of vascular and blood flow characteristics in the conduit arteries during and immediately after enhanced external counterpulsation (EECP) need to be verified.Methods: Forty-two patients with CAD and 21 healthy controls were recruited into this study to receive 45 min-EECP. Both common carotid arteries (CCAs), namely, the left carotid (LC) and right carotid (RC), the right brachial (RB), and right femoral (RF) artery were imaged using a Color Doppler ultrasound. The peak systolic velocity (PSV), end-diastolic velocity (EDV), mean inner diameter (ID), resistance index (RI), and mean flow rate (FR) were measured and calculated before, during, and after the 45 min-EECP treatment.Results: During EECP, in the CCAs, the EDV was significantly decreased, while the RI was markedly increased in the two groups (both P < 0.01). However, immediately after EECP, the RI in the RC was significantly lower than that at the baseline in the patients with CAD (P = 0.039). The FR of the LC was markedly increased during EECP only in the CAD patients (P = 0.004). The PSV of the patients with CAD was also significantly reduced during EECP (P = 0.015) and immediately after EECP (P = 0.005) compared with the baseline. Moreover, the ID of the LC, RB, and RF was significantly higher immediately after EECP than that at the baseline (all P < 0.05) in the patients with CAD. In addition, they were also higher than that in the control groups (all P < 0.05). Furthermore, by the subgroup analysis, there were significant differences in the FR, PSV, and RI between females and males during and immediately after EECP (all P < 0.05).Conclusions: Enhanced external counterpulsation creates different responses of vascular and blood flow characteristics in carotid and peripheral arteries, with more significant effects in both the carotid arteries. Additionally, the beneficial effects in ID, blood flow velocity, RI, and FR after 45 min-EECP were shown only in the patients with CAD. More importantly, acute improvement of EECP in the FR of the brachial artery was showed in males, while the FR and RI of the carotid arteries changed in females.

Exercise selection is one of crucial factors in designing a comprehensive training programme. The exposure of different exercise selection may stimulate the specific adaptation imposed demand. In the construction of any resistance training (RT) programme, it is important to choose whether to apply bilateral (BI) or unilateral (UNI) exercise. The present study aimed to look into the physiological responses of blood glucose (BG) between UNI and BI RT. Quantitative research method was used, RT (UNI versus BI training) as the independent variables whereas BG set as the dependent variable. In order to measure training effects following a single bout of different training intervention (UNI versus BI), a crossover experimental pre and post test design was implemented. A total of sixteen (n = 16) trained women with mean age of 23 (SD = 1.35) years old went through a single bout of RT involved a total body exercise using major muscles group with 80% of 1RM for each protocols (UNI and BI) for 10 repetitions to maximal effort (for 3 sets). Crossover design would be more accurate in exposing different training protocol to a similar characteristic of individuals as compared using different individuals. The results revealed that blood glucose (BG) were statistically changed (p < .001) across times (between PRE to IP, between PRE and 15P as well as between PRE and 30P), and finding shows there is no difference between training protocols (p = .39). Thus, similar responses of UNI and BI RT on BG concentration provides wide selection of exercise method to practitioners specifically to trained women. Future research on UNI versus BI RT could venture onto other types of hormones analysis including insulin, growth hormone and cortisol can be included. PurposeIt is commonly believed that central hemodynamics is closely associated with the presence of cardiovascular events. However, controversial data exist on the acute response of competitive sports on central hemodynamics. Moreover, the central hemodynamic response to exercise is too transient to be investigated. Therefore, this study aimed to investigate the central hemodynamic response in young basketball athletes and controls after 1 h recovery after exercise.MethodsFifteen young basketball athletes and fifteen aged-matched controls were recruited to perform the Bruce test. Central hemodynamics were measured and calculated, including heart rate (HR), aortic systolic, diastolic, and pulse pressure (ASP, ADP, and APP), ejection duration (ED), sub-endocardial viability ratio (SEVR), central augmentation index (AIx), and AIx@HR75. Intra-group and inter-group differences were analyzed by two-way repeated measures ANOVA.ResultsASP significantly decreased at 10 min after exercise in athletes, while it markedly declined at 15 min after exercise in controls (p < 0.01). Additionally, only in the athlete group, ADP significantly decreased at 50 min and at 1 h after exercise. AIx was also significantly reduced at 1–2, 20, 30, and 40 min after exercise (all p < 0.05). Moreover, there were significant differences in the changes of these parameters between the two groups at these measurement points (p < 0.05). SEVR significantly recovered to the baseline level after 30 min, while ED and HR returned to baseline levels at 40 min after exercise in both groups.ConclusionSustained decrease of aortic BPs was sooner after the cessation of exercise in athletes than in controls, and changes of aortic stiffness were more evident in athletes than those in controls during the 1 h recovery period. Additionally, SEVR returned to the baseline sooner than ED and HR in athletes.

Many drugs act very rapidly — they can turn on or off their targets within minutes in a whole animal. What are the acute effects of drug treatment and how does an animal respond to these? We developed a simple assay to measure the acute effects of drugs on C. elegans movement and examined the effects of a range of compounds including neuroactive drugs, toxins, environmental stresses and novel compounds on worm movement over a time period of 3 hr. We found a wide variety of acute responses. Many compounds cause rapid paralysis which may be permanent or followed by one or more recovery phases. The recoveries are not the result of some generic stress response but are specific to the drug e.g., recovery from paralysis due to a neuroactive drug requires neurotransmitter pathways whereas recovery from a metabolic inhibitor requires metabolic changes. Finally, we also find that acute responses can vary greatly across development and that there is extensive natural variation in acute responses. In summary, acute responses are sensitive probes of the ability of biological networks to respond to drug treatment and these responses can reveal the action of unexplored pathways.

Each quarter of the bovine mammary gland is an anatomically and functionally distinct gland. However, mastitis in one quarter may affect function of adjacent, uninfected glands. To investigate the mechanisms and potential mediators of these effects, we quantified early responses of the mammary gland to intramammary lipopolysaccharide (LPS) challenge, distinguishing between local and systemic effects. Ten multiparous cows over 70 d in milk were blocked into pairs by breed, cow-level somatic cell count (SCC), and milk yield. Within block, one cow was assigned to LPS treatment (T) such that both the front and the rear quarter of a randomly selected udder half received an infusion of 50 µg of LPS in 10 mL of saline (T-L); the contralateral quarters received only 10 mL of saline (T-S). Similarly, each paired control cow (C) received either 10 mL of saline (C-S) or no infusion (C-N) into udder halves. Cows were quarter milked twice daily, with foremilk samples (∼30 mL, front quarters) taken at −24, 0, 3, 6, 12, and 24 h relative to infusions. At 24 h, average milk yield in T-L and T-S quarters fell to 23 and 32% of pre-infusion levels, respectively. For T cows, systemic effects were observed by 3 h post-infusion as rectal temperature was elevated and foremilk fat concentration was reduced in both T-L and T-S. However, SCC and concentrations of l -lactate and total protein in foremilk indicated a local response to LPS: protein was transiently higher at 3 h, whereas SCC and lactate were higher at 6 h in T-L compared with T-S. Lactose concentration showed a local effect at 6 h, being lower in T-L than in T-S, and then a systemic effect at 12 h, being lower in both T-L and T-S than C quarters. Concomitant with changes in milk, systemic effects were also observed in blood. Plasma antioxidant potential and glucose concentration were lower in T cows than in C cows at 6 or 12 h, respectively, although neither variable remained different at 24 h. In summary, unilateral LPS infusion induced distinct, time-dependent effects on each milk component. Depending on the component, effects were local, systemic, or both, suggesting involvement of multiple different mediators that collectively result in systemic inhibition of milk production. The purpose of this study was to investigate acute responses of endurance (E + SA), strength (S + SA), and combined endurance and strength exercise (C + SA) followed by a traditional sauna bath (70° C, 18% relative humidity) on neuromuscular performance and serum hormone concentrations. Twenty-seven recreationally physically active men who were experienced with taking a sauna participated in the study. All the subjects performed a sauna bath only (SA) first as a control measurement followed by S + SA and E + SA (paired matched randomization) and C + SA. Subjects were measured PRE (before exercise), MID (immediately after exercise and before sauna), POST (after sauna), POST30min (30 minutes after sauna), and POST24h (24 hours after PRE). Maximal isometric leg press (ILPFmax) and bench press (IBPFmax) forces, maximal rate of force development (RFD) and countermovement vertical jump (CMVJ), serum testosterone (TES), cortisol (COR), and 22-kD growth hormone (GH22kD) concentrations were measured. All exercise loadings followed by a sauna decreased ILPFmax (−9 to −15%) and RFD (−20 to −26%) in POST. ILPFmax, RFD, and CMVJ remained at significantly (p ⩽ 0.05) lowered levels after S + SA in POST24h. IBPFmax decreased in POST in S + SA and C + SA and remained lowered in POST24h. SA decreased ILPFmax and IBPFmax in POST and POST30min and remained lowered in ILPFmax (−4.1%) at POST24h. GH22kD, TES, and COR elevated significantly in all loadings measured in the afternoon in MID. SA only led to an elevation (15%) in TES in POST. The strength exercise followed by a sauna was the most fatiguing protocol for the neuromuscular performance. Traditional sauna bathing itself seems to be strenuous loading, and it may not be recommended 24 hours before the next training session. Introduction: ART is an integrated intermediate care service which provides clinical and personal care support for people who have health or social care needs, which previously would have been met by admission to hospital. The aims of the service are to provide time-specific care that is person-centred and co-ordinated, prevent hospital admissions or enable earlier, supported discharge. The team consists of general practitioners (GPs), senior nurses and therapists with integrated access to social care, and mental health. Ongoing support is provided by transitioning to mainstream services. Key drivers for the implementation of ART were fragmented services in primary and community care, social care, the ambulance service and secondary care; multiple transitions in care impacting on length of hospital stay and poor patient experiences; inconsistency of care; and poor communication at organisational level. Audit data demonstrated a positive impact on hospital admissions. The aims of this evaluation were to determine the extent to which ART delivered person-centred, co-ordinated care and to determine the experiences of professionals delivering the service and external stakeholders. Methods: Interviews were conducted with service users (n=16) and carers (n=13). The P3CEQ (Sugavanam et al, 2016)1, which measures the degree to which care is person-centred and co-ordinated was administered to users (n=21). Two focus group were conducted with the ART (n=10) and social care team (n=5). External stakeholders (n=5) were also interviewed. Descriptive statistics were used to analysis the survey data. Interviews and focus group data were analysed thematically. Results: The total score on the P3CEQ was 21.52/33 (SD±5.48) indicating a high degree of person-centred, co-ordinated care. Themes identified from user and carer interviews were experiences with receiving the service including timeliness; effects on health and wellbeing; support for self-management; care transitions and consistency of care. Amongst professionals, leadership, engagement, information sharing and the importance of personal relationships were key themes. Discussion: The ART service supported the delivery of complex care in users’ own homes and facilitated rapid access to different professionals and services. However, lack of capacity in mainstream services, particularly social care meant that discharging users from ART was difficult at times. Further work is needed in communicating information about the service in order to maximise uptake locally. Conclusions: A multiprofessional, integrated team can deliver person-centred care in an intermediate care setting. Having GPs with dedicated time to deliver the service was a key factor for success. Lessons learnt: Sustainability depends on adequate resources throughout the health and social care economy so that the team are able to stay focused on the main aims and objectives of the service. Limitations: The small sample size meant that further sub-group analysis of the P3CEQ was not possible. Suggestions for future research: The ART model of integrated intermediate care delivery could be compared to other rapid response services in terms of both process and patient-related outcomes. Abstract The aim of the present survey was to determine differences in the serum and urinary concentrations of several toxic trace elements (As, Be, Cd, Cs and Pb) between long distance runners and non-sportsmen living in the same area of Extremadura (Spain) in basal conditions as well as after the performance of a maximal effort test until exhaustion. 21 Spanish national long-distance runners were recruited before the start of their training season and 26 untrained students. All of them had similar ages. The effort test consisted in running on a treadmill incrementally in stages, until exhaustion. Serum and urine analysis of trace metals was performed by inductively coupled plasma mass spectrometry (ICP-MS). In the statistics the urine and blood basal concentrations was analyzed first using Student t test. Then the differences between the pre and post-test values were analyzed using Wilcoxon test. The serum concentrations of Be (p < 0.001), Cd (p < 0.01), Cs (p < 0.001) and Pb (p < 0.001) were higher in the athletes than the control groups in basal conditions. In relation to the exercise, the results showed that there were lower serum concentrations of Be, Cd and Pb after the test, but without statistically significance, in both groups. Cs (p < 0.001) concentrations were statistically lower after the exercise tests in both groups, but if the parameters were corrected in relation to hematocrit and hemoglobin concentrations the differences were significant only among the athletes. In urine, the concentrations of Be (p < 0.001) and Cd (p < 0.01) were higher among athletes than among controls, being the opposite case for urinary Pb values. In relation to the exercise-induced effect of the test, the results showed that among controls only Cd values were modified, but if creatinine corrections are applied this difference does not reach statistical significance. However, sportsmen suffered a diminution in Be values, that can be observed only if urine values are corrected. Also, an increase in Cd, both in corrected and normal values, was observed, fact that occurred also in the case of Cs, but only if urinary values are corrected. It can be concluded that an incremental exercise to exhaustion among elite athletes of long distance modalities can produce changes in serum and urinary concentrations of toxic trace elements which may reflect an adaptive mechanism to prevent toxicity, and which could be an interesting outcome for physicians in the treatment and prevention of toxic situations. Physical interventions are used to increase physical (sports) performance and considered as effective low-cost strategies in the fields of healthcare, disease or injury prevention, and medical treatment. In general, a considerable amount of evidence buttress the application of physical interventions in various fields as it has been demonstrated to contribute to the maintenance and recovery of physical performance, cognitive function, and overall state of health. To implement physical interventions effectively, it is essential to provide an appropriate exercise and training prescription. Exercise and training prescription are key for “dose” specification and for the individualization (personalizing) of physical exercise and training, precisely adjusted and controlled like medication. Since the physiological response to physical interventions is demonstrably individual and dependent on many influencing factors, individualization is an emerging approach aiming to maximize the efficiency of an intervention by accounting for the interindividual heterogeneity. The present brief viewpoint article aims to distinguish and to redefine between the terms dose and response in order to improve the understanding of practitioners, the methodology of study protocols, and to relate future findings to the actual biological (interindividual) variability of acute and chronic responses.

Objectives: To evaluate the immediate responses to forearm compression of blood lactate concentration, heart rate, perceived exertion and local forearm muscle pain during severe climbing in elite climbers.Method: Seven elite climbers (18 ± 2 years; 164 ± 5 cm; 57.8 ± 5.3 kg) performed 3 × 3 climbing bouts with maximal intensity on a distinct 8 m boulder wall (lead grade: 7a–8b) in a single blinded, placebo-controlled cross-over design, wearing either forearm sleeves with compression (verum-compression) or placebo forearm sleeves with no compression (falsum-compression). Each climber’s heart rate was recorded during and capillary blood lactate concentration, perceived exertion and forearm muscle pain were assessed directly after climbing.Result: Heart rate (p = 0.45, ηp2 = 0.12), blood lactate concentrations (p = 0.44, ηp2 = 0.10), perceived exertion levels (p = 0.51, ηp2 = 0.08) and pain perception (p = 0.67, ηp2 = 0.03) were not affected by forearm compression. No condition × time interaction effect (compression × time) occurred for heart rate (p = 0.66, ηp2 = 0.04), blood lactate concentration (p = 0.70, ηp2 = 0.02), perceived exertion (p = 0.20, ηp2 = 0.26) and pain perception (p = 0.62, ηp2 = 0.04).Conclusion: In elite climbers performing severe climbing bouts, sleeves with forearm compression do not alter blood lactate concentration, heart rate, perceived exertion and local forearm muscle pain.

Many drugs act very rapidly -- they can turn on or off their targets within minutes in a whole animal. What are the acute effects of drug treatment and how does an animal respond to these? We developed a simple assay to measure the acute effects of drugs on C. elegans movement and examined the effects of a range of compounds including neuroactive drugs, toxins, environmental stresses and novel compounds on worm movement over a time period of 3 hr. We found a wide variety of acute responses. Many compounds cause rapid paralysis which may be permanent or followed by one or more recovery phases. The recoveries are not the result of some generic stress response but are specific to the drug e.g., recovery from paralysis due to a neuroactive drug requires neurotransmitter pathways whereas recovery from a metabolic inhibitor requires metabolic changes. Finally, we also find that acute responses can vary greatly across development and that there is extensive natural variation in acute responses. In summary, acute responses are sensitive probes of the ability of biological networks to respond to drug treatment and these responses can reveal the action of unexplored pathways. Objectives: This study aimed to investigate the effect of different rest interval durations between sets during full-body resistance exercise sessions on total load lifted and blood lactate concentrations in resistance-trained individuals. Methods: Ten healthy young men (age: 23 ± 6.5 years; total body mass: 82.8 ±1 0.6 kg, height: 177.3 ± 0.1 cm) randomly performed 3 resistance exercise sessions with different rest intervals between sets (30 seconds, 60 seconds and 120 seconds). The resistance exercise sessions consisted of 6 full-body exercises (bench press; seated low row; shoulder press; back squat; leg press 45° and seated calf raise) performed with 4 sets of 10 repetitionsmaximum(RM). Total load lifted (sets x repetitions x load (Kgf)) and lactate blood concentration were determined for each exercise session. One way, repeated measures analysis of variance (ANOVA) followed by Bonferroni's post-hoc test were used for multiple comparisons. Results: The total load lifted was significantly (P < 0.001) lower for 30 seconds (26382.0 ± 4100.3 kg) rest interval compared with 60 seconds (28864.5 ± 4139.7 kg) and 120 seconds (30064.4 ± 3966.4 kg). The blood lactate response was not different among the 3 rest interval conditions investigated, with mean peak values of 7.05 ± 1.69, 7.26 ± 2.77 and 6.90 ± 3.79 mM for 30 seconds, 60 seconds and 120 seconds, respectively. Conclusions: In conclusion, rest interval durations of 60 seconds and 120 seconds between sets favored a higher total load lifted in the exercise session when compared to the 30 seconds rest interval. The rest protocols implemented in the present study promoted a similar metabolic response (e.g. blood lactate concentration) to a full-body resistance exercise session in trained subjects. The Siamese fighting fish ( Betta splendens) is well known as an aggressive fish with unique spawning and parental care behavior. During reproduction, male fish construct a bubble nest, court females, protect the brood, and defend the territory through aggressive displays. Aggression in male Siamese fighting fish has long been the subject of investigation; however, the kinematics of aggression during contests have been largely overlooked. Here we investigated how nest-holding, male Siamese fighting fish use two different types of displays, gill flaring and fin spreading, towards intruders during various reproductive phases; before (BB) and after bubble nest building, and after spawning (AS), and hatching (AH). Males were more aggressive towards male than female intruders and the level of aggression changed significantly between reproductive phases. Gill flaring, the more energetically costly display, was the dominant initial display towards male and female intruders in BB, AS, AH phases. However, defending males switched to fin spreading after prolonged exposure to intruders. The results suggest that Siamese fighting fish use gill flaring as an acute response in order to defend their territory; this response may be replaced by fin spreading as a chronic response, probably to reduce the energetic costs during the contest. Background: Allergic rhinitis is a common disorder that leads to a negative impact on quality of life. Multiple options are available for treatment of the symptoms, but preventing the reaction should be an effective strategy. Objective: We tested a nasal spray that contained microcrystalline cellulose designed to limit airborne allergens from penetrating the nasal mucosa (nasal blocker) and, therefore, prevent the initiation of the allergic reaction. Methods: We performed a randomized, double-blind, placebo-controlled, two-way crossover clinical trial with 20 subjects who had a history of seasonal grass and/or ragweed allergy symptoms. Each subject underwent two separate nasal challenges with antigen (provocations) after application of the investigational product or placebo to both nostrils. The allergen was delivered into one nostril at a time and was administered at 2-hour intervals (identified as challenge 1 and challenge 2). We assessed peak nasal inspiratory flow and total nasal symptoms as well as the number of sneezes recorded at both 15-minute and 1-hour intervals after challenge 1 and, later, challenge 2. After a washout period, the subjects returned to undergo the alternate therapy. Results: There was a significant overall decrease in peak nasal inspiratory flow after both treatments (investigational product, p = 0.005; placebo, p = 0.001), but, when the average of the change from baseline in peak nasal inspiratory flow was compared with the baseline, the results showed no significant differences between the groups (p = 0.31). Similar results were obtained for total nasal symptoms. Conclusion: The investigational product did not prove to be significantly better than placebo in treating seasonal allergic rhinitis symptoms. Because general information concerning the role of small ubiquitin-related modifier (SUMO) proteins in adult skeletal muscle is sparse, we investigated whether SUMO-1 proteins will be subjected to time-dependent changes in their subcellular localization in sarcoplasmic and nuclear compartments of human type I and II skeletal muscle fibers in response to acute stimulation by resistance exercise (RE). Skeletal muscle biopsies were taken at baseline (PRE), 15, 30, 60, 240 min and 24 h post RE from 6 male subjects subjected to a single bout of one-legged knee extensions. SUMO-1 localization was determined via immunohistochemistry and confocal laser microscopy. At baseline SUMO-1 was localized in perinuclear regions of myonuclei. Within 15 and up to 60 min post exercise, nuclear SUMO-1 localization was significantly increased (p < 0.01), declining towards baseline levels within 240 min post exercise. Sarcoplasmic SUMO-1 localization was increased at 15 min post exercise in type I and up to 30 min post RE in type II myofibres. The changing localization of SUMO-1 proteins acutely after intense muscle contractions points to a role for SUMO proteins in the acute regulation of the skeletal muscle proteome after exercise. Nanotechnology is a rapidly growing field with silver nanoparticles (AgNP) in particular utilized in a wide variety of consumer products. This has presented a number of concerns relating to exposure and the associated toxicity to humans and the environment. As inhalation is the most common exposure route, this study investigates the potential toxicity of AgNP to A549 alveolar epithelial carcinoma cells and the influence of a major component of lung surfactant dipalmitoylphosphatidylcholine (DPPC) on toxicity. It was illustrated that exposure to AgNP generated low levels of oxidative stress and a reduction in cell viability. While DPPC produced no significant effect on viability studies its presence resulted in increased reactive oxygen species formation. DPPC also significantly modified the inflammatory response generated by AgNP exposure. These findings suggest a possible interaction between AgNP and DPPC causing particles to become more reactive, thus increasing oxidative insult and inflammatory response within A549 cells. Sea bass were experimentally infected with Listonella anguillarum or Photobacterium damselae subsp. piscicida ( Phdp ). At 24 and 72 h post-infection, the expression analysis of immune-relevant genes (IL-1β, IL-6, IL-8, Hepcidin), the transcriptional level and detection of HSP70, and the quantification of serum iron were investigated in association with the histological analysis and the bacterial recognition in tissues by immunohistochemistry. At 15 days post-infection, the specific antibody response was detected in surviving fish, as well as the transcriptional levels of TcR and BcR sequences. Both experimental infections were characterized by a similar acute response, whereas different histological and immunohistochemistry evidences were observed. In particular, the early reaction appeared suitable for the clearance of L. anguillarum , thus limiting the histological lesions, the bacterial dissemination and the further development of acquired immunity in surviving fish. On the contrary, the innate response appeared not enough to resolve the Phdp infection, which was characterized by tissue damage, bacterial widespread and substantial detection of specific humoral immunity in surviving fish, also associated to lymphocytes clonal expansion. Besides the opportunistic conditions involved in fish vibriosis and pasteurellosis, the comparison between these experimental infection models seems to suggest that the rate of development of the acquired immunity is strictly linked to the activation of the host innate response combined to the degree of bacterial virulence. The article presents a study on neuromuscular fatigue from power actions, using a weight-stack device (WS) with and without a rubber band (RB). Topics include weight-based resistance training, medialis surface electromyography (EMG) parameters, and concentric contractions. Elastic resistance, deceleration phase, and acute response are also discussed.

Background Smoking and smoking cessation are considered to be associated with weight changes. We have recently shown that smoking acutely increases plasma levels of ghrelin, a known orexigenic hormone. Obestatin is a peptide encoded by the ghrelin gene, which opposes ghrelin effects on food intake. We conducted a study in adult volunteers measuring plasma levels of obestatin immediately after initiation of smoking. Methods 31 volunteers (mean age 32.2 ± 9.2 years and mean BMI 25.7 ± 4.1), 17 smokers and 14 non-smokers, were enrolled in our study. The 2 groups were matched in age and BMI. Plasma obestatin concentrations were determined at baseline (T0), 2 (T2), 5 (T5), 15 (T15), and 60 (T60) minutes after the initiation of smoking. Results In all 31 subjects, no significant difference in the mean values of plasma obestatin levels was observed from baseline at T2, T5, T15 and T60 after initiation of smoking (overall p = 0.15). However, a trend for higher obestatin levels was noted in smokers vs non-smokers (overall p = 0.069), which was not related to the pack-years. Conclusion On the contrary with ghrelin's response after smoking initiation, there is no such an acute response of plasma obestatin levels.

The present work compared metabolic and immune responses in genetically high-producing cows that produced a low amount of milk before expected involution and in cows with the same genetic potential that produced copious amounts of milk before their scheduled drying-off. Ten multiparous lactating Israeli Holstein cows producing approximately 10,500 L in the current lactation, without bacterial infection and scheduled for drying-off approximately 60 d before their expected parturition, were studied. Five of the cows that exhibited a sharp, spontaneous reduction in milk yield at the end of their lactation and produced less than ~14 L/d were defined as cows approaching natural involution (ANI), and 5 cows that produced between 25 and 35 L/d were defined as cows approaching forced involution (AFI). Three days before scheduled drying-off, milking was stopped and milk samples were collected from each quarter. After milking cessation, only modest swelling was observed in the udders of the ANI cows. In the ANI cows, lactose and fat concentrations decreased and the fat:lactose concentration ratio indicated that on d 1 and 2 fat concentrations decreased faster than lactose concentration, whereas on d 3, the rate of reduction was about the same for lactose and fat. In contrast, in AFI cows, fat concentrations increased on d 1 and the fat:lactose ratio indicated that changes in fat secretion were minor compared with those of lactose secretion. Rennet clotting time of milk after drying-off in the ANI cows increased, whereas curd firmness decreased rapidly, such that mammary secretions did not coagulate on d 3. In the AFI cows, such significant changes were observed only on d 3. The inflammatory response increased in both groups, but at each stage the increase was greater in ANI cows than in AFI cows. On d 1, the increase in leukocyte numbers in the ANI cows was made up of mononuclear cells (i.e., T lymphocytes and macrophages). In contrast, in the AFI cows, we observed a marked increase in leukocyte numbers, mainly in the form of polymorphonuclear cells. Our data indicate that the abrupt mammary involution induced in AFI cows provoked signs of distress, which were associated with neutrophilia in milk. In contrast, in the ANI cows, cessation of milking occurred without evidence of engorgement of the udder. The influence of adenosine mono phosphate ( AMP)-activated protein kinase ( AMPK) vs Akt-mammalian target of rapamycin C1 ( mTORC1) protein signaling mechanisms on converting differentiated exercise into training specific adaptations is not well-established. To investigate this, human subjects were divided into endurance, strength, and non-exercise control groups. Data were obtained before and during post-exercise recovery from single-bout exercise, conducted with an exercise mode to which the exercise subjects were accustomed through 10 weeks of prior training. Blood and muscle samples were analyzed for plasma substrates and hormones and for muscle markers of AMPK and Akt- mTORC1 protein signaling. Increases in plasma glucose, insulin, growth hormone ( GH), and insulin-like growth factor ( IGF)-1, and in phosphorylated muscle phospho- Akt substrate ( PAS) of 160 kDa, mTOR, 70 kDa ribosomal protein S6 kinase, eukaryotic initiation factor 4 E, and glycogen synthase kinase 3α were observed after strength exercise. Increased phosphorylation of AMPK, histone deacetylase5 ( HDAC5), cAMP response element-binding protein, and acetyl- CoA carboxylase ( ACC) was observed after endurance exercise, but not differently from after strength exercise. No changes in protein phosphorylation were observed in non-exercise controls. Endurance training produced an increase in maximal oxygen uptake and a decrease in submaximal exercise heart rate, while strength training produced increases in muscle cross-sectional area and strength. No changes in basal levels of signaling proteins were observed in response to training. The results support that in training-accustomed individuals, mTORC1 signaling is preferentially activated after hypertrophy-inducing exercise, while AMPK signaling is less specific for differentiated exercise. The objective of the present repeat-measures study was to determine whether plasma serum levels of testosterone, cortisol, osteocalcin or type I collagen C-telopeptide (CT) are acutely affected following an electro-myostimulation (EMS) bout, and their relation to bone mineral density and muscle mass. Ten men with recent (8 weeks) thoracic spinal cord injury (SCI) (ASIA A) and 10 age-matched able-bodied (AB) men performed one EMS bout on the quadriceps femoris muscle. Blood samples were drawn at basal condition, immediately after EMS, and 15 min, 30 min, 24 h and 48 h post-EMS. Muscle cross-sectional area was measured by magnetic resonance imaging. Bone mineral density (BMD) was determined by dual-energy X-ray absorptiometry. In the SCI group, a significant decrease in testosterone, cortisol and CT together with a significant increase in testosterone/cortisol ratio and osteocalcin/CT ratio was observed after EMS. For the AB subjects, only testosterone and CT decreased significantly following EMS. Muscle size was only related to testosterone/cortisol ratio in the SCI sample ( R = 0.659, p < 0.05), whereas BMD did not show any relation to any biomarker.
