Accelerated idioventricular rhythm (AIVR) commonly follows coronary reperfusion and has been called a "reperfusion arrhythmia". Transient left bundle branch block (LBBB) is only rarely seen after interventional reperfusion and is usually considered a procedural complication. We report herein electrocardiograms (ECGs) in a case of acute lateral myocardial infarction which demonstrate both post-perfusion AIVR and a simultaneous transient LBBB with fusion complexes causing paradoxical QRS narrowing.

We describe the case of a 61 year-old female patient admitted to the hospital with acute lateral myocardial infarction, in pulmonary edema, who evolved to cardiogenic shock in the first hours. Transesophageal echodopplercardiogram and nuclear magnetic resonance imaging showed the diagnosis of a left ventricular pseudoaneurysm. Surgical repair was successfully undertaken.

Although vascular occlusion is a hallmark of sickle cell disease, documented myocardial infarction is an uncommon event in these patients. We report a case of an acute lateral myocardial infarction in a 25 year old male homozygous for HbS, who had previously had a relatively benign course of his sickle cell disease. Management of the case included an exchange transfusion and a 6 month period of chronic simple transfusions (2 units packed red cells every 3-4 weeks). We review here the available literature on cardiovascular manifestations of sickle cell disease.

Two cases of transient left posterior hemiblock associated with acute lateral myocardial infarction are reported. The main electrocardiographic features, diagnostic criteria and problems of differential diagnosis are analyzed. The association of left posterior hemiblock with the lateral infarction causes a difficult diagnostic problem. Left posterior hemiblock should be suspected when the ÂQRS direction is about +120 °, with an S1-Q3 pattern and a QRS interval within normal limits, provided right ventricular hypertrophy or a vertical heart can be excluded, and provided there is some form of left ventricular disease.
